Abstract:Background: Allergies affect children's health as well as their quality of life, stress levels, and family budget. The available literature suggests that family, social and psychological factors are affected by allergic pathologies such as rhinitis, asthma and atopic dermatitis. However, few studies have focused on quantifying such association in child food allergy. This study aims to enhance the understanding of the associations between caregiver variables and children's Food Allergy (FA). Methods: The study involved 206 participants: 103 mothers plus 103 children with IgE mediated FA. The analyses excluded two outliers comprising 101 subjects. For statistical analyses, each dyad -mother/child- was considered to be one subject unity. A between-subjects one-way ANOVA determined the association of children's cutaneous, gastric and respiratory symptoms with anxiety, depression, perceived social support and socioeconomic factors in the mothers. Results: There are significant associations between children's allergic symptoms (gastric and cutaneous) and mothers' psychological state (anxiety and depression); family budget; social interactions (with friends, family and partner); understanding of health care required by their child; and sleep disorders. Respiratory symptoms did not show any significant associations with the dependent variables. Conclusion: FA is a process in which children's symptoms are significantly associated with socioeconomic and psychological variables of the mothers. The presence or absence of some specific symptoms is directly associated with specific impacts on the mothers. An understanding of such dynamics supports the consideration of a comprehensive and multidisciplinary therapeutic approach to offer more ecological healthcare for "families living with FA."

Abstract:(1) Background: Food allergy (FA) is an immune-mediated hypersensitivity to foods, significantly contributing to childhood morbidity and mortality. This study aimed to assess the prevalence, characteristics, and influencing factors of parent-reported FAs among children in Saudi Arabia. (2) Methods: This cross-sectional study utilized a validated parental questionnaire distributed across all regions of Saudi Arabia. Data from 2130 participants were collected and analyzed using SPSS v. 26 and Prism software v. 10.3.0. (3) Results: Parent-reported FA prevalence was 15.2%. Egg was the most common allergen (6.2%), followed by tree nuts (4.1%), peanuts (4.0%), milk (3.8%), and sesame (3.2%). Significant geographical variations were observed, with the western region having the highest burden (p < 0.001). Older children had higher rates of shellfish and fish allergies. Parental allergies and co-existing asthma/drug allergies were positively associated with childhood FAs. (4) Conclusions: This study highlights a substantial burden of parent-reported FAs in Saudi Arabia, with regional variations in food allergen distribution. Parental allergies and co-existing allergic conditions may influence FA risk.
Food allergy educational needs of pediatric dietitians: a survey by the Consortium of Food Allergy Research

Abstract:Objective: To determine pediatric dietitians' self-reported proficiency, educational needs, and preferences regarding food allergy (FA) management. Design and setting: An Internet-based, anonymous survey was distributed to the Pediatric Nutrition Practice Group (PNPG) of the American Dietetic Association. Participants: Respondents (n = 311) were registered dietitians and members of the PNPG. Analysis: Results are presented using descriptive statistics. Chi-square tests were applied for subgroup analyses. Percentage responses were calculated per question based on the number of respondents answering the question. Results: On a 4-point scale of proficiency ("high," "moderate," "low," and "none"), respondents primarily rated themselves "moderate" for educating families, creating diets, and evaluating safe food items, and "low" for creating diagnostic food challenges. Education was primarily self-taught (75%). Preferences for future resources included handbooks (77%) and Web-based instructional programs (53%). On a 4-point scale ("very" to "not at all" needed) among practices that included >10% patients with FA, ratings of "very" were defined as need resources to update FA knowledge (87%) and need for a FA "tool kit" (84%). Conclusions and implications: Pediatric dietitians manage FA for a substantial patient base although their self-reported proficiency is overall only moderate. Dietitians would prefer and likely benefit from Internet-accessible management handbooks and patient handouts.

Abstract:Background: Food allergy (FA) is a common chronic condition among U.S. children. Children with FA and their families often report greater psychosocial burden, which is adversely impacted by the inability to participate in daily activities. Regularly attending school remains central to supporting the well-being of children with FAs and related academic success. Objective: The objective was to estimate the frequency of FA-related school absences, determine predictors, and understand how report of such absences is associated with FA-related psychosocial burden. Methods: A survey was administered to a nationally representative sample of U.S. households in 2015-2016, obtaining parent-proxy responses for 38,408 children. Prevalence estimates were based on responses from NORC's AmeriSpeak Panel (51% completion rate), which were augmented by nonprobability-based responses via calibration weighting to increase precision. Prevalence was estimated via weighted proportions. Multiple logistic regression models evaluated factors associated with FA-related missed school days. Results: Thirty-seven percent of children with FA who attended school in the past 12 months reportedly had one or more FA-related absence, with 13% missing 1-2 days (95% confidence interval [CI], 11.41-15.49 days), 17% missing 3-7 days (95% CI, 6.82-10.91 days), and 4% missing 8-14 days (95% CI, 3.13-6.20 days). Hispanic children were more likely to report missed school days in the past 12 months compared with white, non-Hispanic children with FA (odds ratio [OR] 1.62 [95% CI, 1.16-2.26]). Children with multiple FAs (OR 1.35 [95% CI, 1.03-1.76]), history of epinephrine use (OR 2.22 [95% CI, 1.70-2.90]), and anaphylaxis (OR 1.64 [95% CI, 1.26-2.14]) in the past 12 months, and those with a current epinephrine prescription (OR 1.05 [95% CI, 0.075-1.47]) have greater odds of reported FA-related school absence. Report of one or more FA-related absences was also associated with greater FA-related psychosocial burden (OR 1.72 [95% CI, 1.46-2.01]). Conclusion: Parent report of children missing school for reasons related to FA is remarkably common and associated with greater FA-related psychosocial burden.

Abstract:Background: Food allergy (FA) is associated with poor health-related quality of life and high levels of psychological distress. Psychological support is extremely important but not always available. As part of the Global Access to Psychological Services for Food Allergy (GAPS) study, we aimed to assess psychological distress and service use among adults, caregivers and children with FA in a global survey. Methods: Participants (n = 1329 adults with FA; n = 1907 caregivers of children with FA) from >20 countries were recruited through patient organisations, social media advertisements and online survey panels to complete an online survey. Surveys were available in six languages. Results: A total of 67.7% of adults and 77.2% of caregivers reported direct experience, and 51.6% of caregivers said their child had experienced FA-related psychological distress. The most commonly reported issue was anxiety about having an allergic reaction. Less than 20% had been assessed for FA-related psychological distress. There were significant differences across countries for levels of distress, screening for distress, seeing a mental health professional and being diagnosed with a FA-related mental health disorder (all p < .001). The United Kingdom, Australia and Brazil had the highest number of participants reporting distress. The most commonly reported barrier to seeing a mental health professional was cost. Conclusions: FA-related distress is common across countries, but with substantial country-to-country variability. Allergy providers are encouraged to routinely assess families for psychological distress and provide access to appropriate mental health resources. Development and implementation of evidence-based, patient-informed accessible, affordable FA interventions in multiple languages is urgently needed.

Abstract:Allergies have a significant impact on health and quality of life worldwide, yet there is limited research on the awareness and knowledge of allergies. This study aimed to explore the level of awareness and knowledge of allergies among visitors in Syrian hospitals. A cross-sectional study was conducted between May and September 2022, and a standardized international questionnaire was administered to 504 visitors in three hospitals in Syria. Data analysis was performed using the Statistical Package for Social Sciences (SPSS Inc., Chicago, IL, USA) version 23. The final sample comprised 504 questionnaires with 61.7% of participants achieving an average score. Statistical analysis revealed a significant difference in knowledge of allergy scores between the group with only elementary education (M = 3.76, SD = 1.67, p = 0.011), the group with no education (M = 3.65, SD = 1.65, p = 0.006), and the group with a university education (M = 4.44, SD = 1.25). Notably, no significant differences were found between the other educational groups. A one-way ANOVA was employed to assess the effect of place of living on knowledge of allergy, but no significant differences were observed between the groups (p = 0.462). Lastly, a significant negative correlation was detected between participant age and knowledge of allergy scores (r(502) = − 0.102, p = 0.022). Allergies represent a substantial global health concern that demands attention from healthcare providers, policymakers, and the public. This study emphasizes the importance of investing in health education and awareness campaigns to enhance knowledge and comprehension of allergies, particularly individuals with lower levels of education levels. By equipping individuals with the requisite information to effectively manage their allergies, their overall health and wellbeing can be improved.

Abstract:Background: Approximately 8% of elementary school-aged children in the United States have food allergies, a complicated health management situation that requires parents to use many types of health literacy, empowerment, and advocacy skills to work with school staff to protect their children. Objective: This cross-sectional study examined (a) whether the highest versus lowest levels of functional, communicative, and critical health literacy are associated with higher perceived effectiveness of parental advocacy behaviors for safe food allergy management in schools [parental advocacy]; and (b) whether communicative and critical health literacy are more strongly associated with parental advocacy than functional health literacy. Methods: A sample of parents of elementary school-aged children was recruited through 26 food allergy organizations and a research patient registry. Participants completed an anonymous online survey. Self-reported measurements of parental health literacy, empowerment, and advocacy were adapted and refined through pre-testing and pilot-testing. General linear model analyses were conducted to predict parental advocacy. Key Results: Participants ( N = 313) were predominantly White, college-educated mothers with moderately high levels of food allergy knowledge, health literacy, empowerment, and parental advocacy skills. Parents who scored at the highest levels in the three dimensions of health literacy reported they engaged in more effective advocacy behaviors than parents who scored at the lowest levels. Parental advocacy was predicted largely by parental empowerment and the quality of the relationship with the school ( B = .41 and B = .40, respectively). Functional health literacy and the child's diagnosis of asthma were smaller predictors. While accounting for covariates, functional health literacy was significantly associated with parental advocacy whereas communicative and critical health literacy were not. Conclusions: Interventions to impact parental empowerment and parent-school relationships, including a health-literate universal precautions approach of communicating food allergy school policies, may influence parental advocacy for food allergy safety in schools. Abstract:Abstract Objective Anxiety is among the major psychological concerns for children living with food allergy (FA). Yet research exploring the variables driving anxiety symptoms in FA remains sparse, and most studies still utilize homogeneous samples to assess anxiety symptoms. The current study seeks to evaluate the rates of clinically significant anxiety symptoms among a diverse sample of youth with FA and examine whether a heightened risk perception of FA outcomes and FA burden (vs. FA medical history) is associated with anxiety in youth. Methods 94 youth ages 10–14 and their parents were recruited from FA clinics at a mid-Atlantic children’s hospital. Both youth and parents completed demographic and FA medical history questionnaires, the Screen for Child Anxiety Related Emotional Disorders, and the Food Allergy Independent Measure as part of a longitudinal study about FA adjustment and adherence. Results Over a third (37%) of youth scored above clinical cut-offs for overall anxiety symptoms. At least 25% of youth reported clinically significant scores on panic disorder, generalized anxiety, social anxiety, separation anxiety, and school avoidance subscales. Perception of risk of adverse FA outcomes and burden—but not FA medical history–were associated with total anxiety, generalized anxiety, panic disorder, and school avoidance symptoms, but not social anxiety and separation anxiety. Having more FAs was associated with higher social anxiety scores but not with other anxiety subscales. Conclusions Youth with FA might benefit from psychosocial interventions that address FA risk perception management and promote appropriate FA vigilance to cope with anxiety symptoms.

Prevalence of food allergy increased 1.7 times in the past 10 years among Japanese patients below 6 years of age

Abstract:Background: While food allergy (FA) has been increasingly recognized as a growing public health burden worldwide, epidemiological studies on FA in Japan are limited. Methods: This was a noninterventional, observational study using the administrative claims data from 2010 to 2019 (10 years). Patients with physician-diagnosed FA in Japan (prevalent cohort) were divided into high-risk or low-risk cohorts using adrenaline prescription. The high-risk cohort was further divided into anaphylaxis or nonanaphylaxis cohort based on the occurrence of anaphylaxis or a serious allergic reaction (SAR) during 1 year after adrenaline prescription. The primary objective was to examine yearly prevalence of FA. The secondary objectives were to describe demographics/clinical characteristics and healthcare resource utilization (HCRU), to evaluate the number of occurrences of anaphylaxis/SAR in the high-risk cohort, and the impact of anaphylaxis/SAR on HCRU. Results: The overall standardized prevalence rate was 0.325% (95% confidence interval [CI], 0.311-0.339) in 2010 and 0.797% (95% CI, 0.790-0.804) in 2019 and predominant in patients age <6 years (preschool; 3.377% [95% CI, 3.229-3.525] in 2010 and 5.726% [95% CI, 5.663-5.789] in 2019). Majority of FA patients (>80%) were children/adolescent throughout the 10 years. While high-risk cohort was a relatively minor population (8.5% in the prevalent cohort in 2019), the occurrence of anaphylaxis/SAR in the high-risk cohort was 227,690/100,000 patient-years. Multivariate analysis showed a significant increase in HCRU variables in the anaphylaxis versus nonanaphylaxis cohort (e.g., 2.08 [95% CI, 2.05-2.11] times more FA-related outpatient visits). Conclusions: Prevalence of FA increased in a statistically significant way from 2010 to 2019; 1.7-fold increase was observed in patients <6 years old. Patients in the high-risk cohort appear to have suffered from frequent anaphylaxis/SAR, highlighting an unmet medical need for FA patients at "high-risk," considering the unavailability of approved medications to prevent anaphylaxis/SAR.

Abstract:Background: Infective endocarditis (IE) is a serious complication primarily affecting patients with cardiac conditions. It is widely recognized that oral microorganisms contribute to the development of IE. Objectives: The objective is to assess the knowledge, attitudes, and perception of parents of children at risk of IE by studying the preventive measures employed by them. Materials and methods: A self-constructed questionnaire composed of 14 questions was distributed among parents of children aged 0-12 years with known cardiac diseases. The parents/caregivers who were attending the pediatric outpatient clinics at King Fahad Medical City (KFMC) were selected randomly. Results: A total of 112 parents responded to this questionnaire. Almost 50% of the participants in this study reported that their children do not brush their teeth regularly, with only 12.5% of them brushing twice a day. Regarding the role of parents in supervising oral hygiene, 62.5% of them stated that they only provide advice but do not watch their child brush their teeth, and only 9.8% of parents are actively involved in advising and watching their children brush their teeth. In this study, 95.5% of parents agreed that maintaining good dental health was crucial for overall body health, and 58% of the participants showed interest in receiving more education about oral health and its impact on overall well-being. To assess the statistical significance, a nonparametric Pearson's chi-square test for fitness was employed. A p-value of ≤ 0.05 was used to report the statistical significance of the results. Conclusion: While the parents demonstrated adequate knowledge of oral health, their attitudes toward it were lacking. So, both parents and children require a modification in their attitudes toward dental care and oral health.

Abstract:Objective: This study aims to assess the level of awareness and knowledge regarding the first aid management of foreign body aspiration (FBA) in children among various demographic groups. Methods: A cross-sectional survey was conducted, involving 500 participants. The survey included questions about demographic information and knowledge of FBA management. The data were analyzed to identify significant differences in awareness levels based on gender, age, nationality, educational background, occupational status, and marital status. Results: The study found significant differences in awareness levels across various demographic groups. Females (80.2%) demonstrated higher awareness compared to males (71.5%), with a significant p-value of 0.035. Participants aged 30-39 years had the highest awareness (83.6%), though the difference was not statistically significant (p=0.057). Nationality did not significantly affect awareness levels. However, educational background showed variations, with postgraduate degree holders having somewhat lower awareness (65.9%) compared to bachelor's degree holders (78.1%), though this difference was not significant (p=0.271). Occupational status had a significant impact, with military personnel displaying the highest awareness (100%) and business professionals the lowest (67.7%), with a significant p-value of 0.009. Marital status also influenced awareness, with married individuals showing higher awareness (82.8%) compared to single (74.4%) and divorced (65.5%) individuals, with a significant p-value of 0.040. Conclusion: The study highlights the need for targeted educational programs to enhance first aid knowledge across different demographic groups. By addressing gaps in awareness through comprehensive training, especially among groups with lower awareness levels, we can improve the general population's ability to manage FBA incidents effectively, thereby reducing associated morbidity and mortality in children.
Parental Knowledge about Allergies and Problems with an Elimination Diet in Children Aged 3 to 6 Years

Abstract:Allergic diseases are highly prevalent, and they can exert a significant influence on the patients’ physical and mental well-being, thus affecting the quality of their lives and society as a whole. The aim of this study was to evaluate parental knowledge about allergens, allergy symptoms, and treatment of allergies, and to identify problems with adherence to an elimination diet and the underlying difficulties. Twelve kindergartens and the parents of 1350 preschoolers took part in the first stage of the study. In a screening trial, allergies were diagnosed in 197 children, and their parents participated in the second stage of the study. The child’s age at the onset of the first symptoms was significantly correlated with allergy type. Age was significantly correlated with selected symptoms of an allergic reaction, and skin allergies were more prevalent in younger children. Erythema, skin reddening, and urticaria occurred more frequently in children aged 3−4 years (OR 1.45; 95%CI 1.24−1.77, p < 0.05) and were diagnosed in skin tests (OR 1.36; 95%CI 1.22−1.59, p < 0.05). Allergies to numerous food items were associated with a long-term elimination diet (OR 1.89; 95%CI 1.33−2.19, p < 0.01), as well as problems with preparing safe meals, shopping for food, or dietary adherence when eating out. According to the respondents, lack of support from other family members and compliance with dietary restrictions in kindergartens and when eating out posed the greatest barriers to dietary adherence. Parents do not have sufficient knowledge about environmental allergens and effective strategies for coping with acute allergic reactions, including anaphylactic shock. Children with diagnosed food allergies should enjoy a similar quality of life to their healthy peers, which is why the parents should be educated about diet therapy, duration of treatment, and safe food substitutes.

Abstract:Parent-targeted oral health awareness campaigns should place a strong emphasis on issues like the value of deciduous teeth and how to treat them, frequent dental checkups, and dietary habits. The current study's objective is to evaluate parents' knowledge, behaviors, and attitudes regarding their children's oral health and how those factors affect the prevalence of dental caries in 5- to 10-year-old children in Riyadh, Saudi Arabia. Parents of school-aged children aged 5 to 10 who came to our hospital were the subjects of a cross-sectional questionnaire survey. The study included 209 children in all, together with their parents. Utilizing DMFT and the deft index, clinical examinations of children were conducted. Parents were given a self-designed questionnaire to evaluate their understanding, attitudes, and practices about their children's dental health. SPSS version 24 was used for the statistical analysis. A p-value of 0.05 or less was regarded as statistically significant when using the ANOVA test to examine the relationship between dental caries status and KAP. The mean deft and DMFT were 6.46 ± 1.6 and 1.56 ± 2.8, respectively. Most parents (72.2%) demonstrated adequate knowledge, behaviors, and attitudes. Parental KAP and mean DMFT/DEFT did not differ significantly. The study's participants had reasonable KAP for the oral health of their children. The significance of deciduous teeth and their treatment, frequent dental appointments, and dietary behaviors are among the knowledge gaps in oral health.
