Abstract:The objective of the study was to determine the short-term success rate of short and wide single-tooth implants, immediately placed into extraction sockets of the posterior area. A retrospective cohort study design was used. A total of 145 subjects received 162 short and wide-diameter single-tooth implants between 2006 and 2009. A minimal 7-mm residual height and 9-mm ridge width was available in all the implant sites, and the attached gingivae were at least 2 mm wide. All implants were placed and restored with the single crown by one experienced operator. The data were analyzed with descriptive statistics. All implants were placed in molar areas. There were 20 Ankylos implants with a diameter 5.5 or 7 mm and a length of 8 mm and 142 hydroxyapatite-coated implants with a diameter 5 or 6 mm and a length of 5.7 to 8 mm. One of the 162 implants failed before prosthetic restoration, resulting in a survival rate of 99.4% after loading. Patients were followed for up to 56 months (mean = 24 months) after loading of implants. The radiographic and clinical data revealed well-maintained hard and soft tissues with acceptable short-term results. For residual ridges with minimal height but adequate width, the immediate placement of short and wide-diameter implants in fresh extraction sockets may offer a simple and predictable treatment alternative if implants are positioned appropriately after a thorough preoperative analysis.
Retrospective Study of Short Versus Standard Posterior Implants and Analysis of Implant Failure Risk Factors

Abstract:Purpose: To compare the clinical and radiographic outcomes of short implants 8 mm) vs standard implants (> 8 mm, < 10 mm) and to uncover risk factors influencing implant failure in short implants. Materials and Methods: Short and standard implants were compared in the aspect of survival rates, biologic and mechanical complications, and marginal bone loss. To analyze risk factors of implant failure in short implants, several variables were taken into consideration, including sex, age, time interval, arch, implant brand, additional surgery, prosthesis material, restoration, smoking status, and crown-root ratio. Results: Three hundred twenty-one short implants and 136 standard implants were retrospectively followed up from 12 to 104 months with an average of 40 months (3.33 years) in short implants and 34 months (2.83 years) in standard implants. The survival rates of short implants were 95.6% at the implant-based analysis and 94.9% at the patient-based analysis, and rates of 96.3% and 94.0%, respectively, were calculated for standard implants. No statistically significant differences were observed between short and standard implants with respect to survival rate, complications, or marginal bone loss. The failure rates were 4.2% for implants and 5.4% for patients in total implants with an average of 38 months (3.17 years). In analyzing risk factors of short implants for survival rate, single short implants resulted in a higher failure rate compared with splinted short implants, while no significant variable was found in standard implants. Conclusion: Short implants tend to be a reliable alternative in atrophic posterior regions. Splinted prostheses were more ideal for short implant restorations.
Outcomes That May Affect Implant and Prosthesis Survival and Complications in Maxillary Fixed Prosthesis Supported by Four or Six Implants: A Systematic Review and Meta-Analysis

Abstract:Objective: To investigate whether the clinical and radiographical outcomes are affected when four or six implants support the maxillary fixed complete denture (FCD). Materials and methods: This study was registered on PROSPERO (CRD42021226432) and followed the PRISMA guidelines. The focused PICO question was, ''For an edentulous maxillary patient rehabilitated with an implant-supported fixed prosthesis, do the clinical and radiographical outcomes differ when four or six implants support the prosthesis ''. A thorough search of the relevant studies was designed and performed electronically. The survival rate of implant and prosthesis, marginal bone loss, and complications (mechanical and biological) were the primary outcomes, whereas implant distribution and using the surgical guide, follow-up, and framework material were evaluated as secondary outcomes. Results: Out of 1099 articles initially retrieved, 53 clearly stated the outcomes of interest and were included in this study. There were no significant differences in implant and prosthesis survival, technical/mechanical complications, and biological complications between the 4-implant group (4-IG) and the 6-implant group (6-IG). However, marginal bone loss (MBL) was significantly higher in the 4-IG (p < 0.01). The surgical guide and follow-up period did not significantly affect implant/prosthesis survival. Additionally, using the CAD/CAM milled framework and anteroposterior implant distribution were associated with significantly higher implant survival in the 6-IG (p < 0.01). Conclusion: The findings of this study indicated that having a greater number of implants, as seen in the 6-implant group, can lead to a decrease in technical and biological complications and reduce marginal bone loss. It is worth noting that factors such as using CAD/CAM frameworks and the anteroposterior distribution of implants were recognized as important in improving implant survival rates when more implants are present.
Clinical Evaluation of the Long-Term Survival and Success Rates of Different Types of Implant-Supported Prostheses

Abstract:Background: Dental implants have been essential to the therapy of patients who are partially edentulous. Aim: Clinical evaluation of the long-term survival and success rates of different types of implant-supported prostheses. Materials and methods: Patients who had a healthy edentulous ridge of three missing teeth in the posterior jaw in which two or three dental implants were placed having any of three prostheses, namely, three-unit fixed dental prostheses (FPD) on two implants, three splinted crowns on three implants, and three splinted crowns on three implants were included. Results: The survival rate in all patients with dental implants having different types of prostheses was 94.9%. The survival rate in FPD, three splinted crowns, and three nonsplinted crowns was 100, 88.5, and 93.6% respectively. Conclusion: FPD loaded on two dental implants was found to have a greater long-term survival rate and success rate with a lower frequency of peri-implantitis and prosthodontic complications.

Abstract:Background: Dental implant surgery has become a standard procedure for replacing missing teeth, boasting high success rates. However, complications can occur, affecting treatment outcomes. Understanding the nature and frequency of these complications is essential for optimizing patient care. Objective: To assess the prevalence and types of complications associated with dental implant surgery through retrospective research. Methods: Patient records were reviewed, focusing on demographics, medical history, surgical details, implant characteristics, and postoperative complications. Complications were categorized, and statistical analysis was performed to identify associations. Results: Among 150 cases, infection (15.3%), implant failure (8.0%), and peri-implantitis (12.0%) were the most common complications. Complication rates varied significantly across age groups (P < 0.05), with younger patients experiencing higher rates. Conclusion: Complications in dental implant surgery, including infection, implant failure, and peri-implantitis, are prevalent. Age appears to influence complication rates. Strategies to mitigate complications and optimize treatment outcomes are crucial.

Abstract:Purpose: The basic aim of the present study was to analyze the results of implant failures in two different implant populations, and how these results may vary depending on how the data are compiled and analyzed. Materials and methods: Two groups of consecutively treated patients were included who had been provided with either one single implant in a partially edentulous upper jaw (1881 patients) or four to eight implants in an edentulous upper jaw (2031 patients/12 454 implants). The risk of implant failure in the two groups separately and in combination was statistically compared by using uni- and multivariable analyses. Results: The two groups showed significant differences in inclusion, surgical treatment protocols, and the risk of implant failures (P < .05). Overall, 25-year patient-level cumulative survival rates (CSRs) were 75.8% and 96.3% for edentulous and single implant treatment, respectively. "Dental condition" was the variable associated with the greatest risk of implant failure (HR 6.00; edentulous). Only one variable was significantly associated with the risk of implant failure in all tested groups ("time after surgery"; a decreased risk was observed over time), and more variables were statistically associated with implant failures in the edentulous group than in the single implant group. Conclusions: Edentulous patients present a significantly and substantially higher risk of implant failures than patients provided with a single implant. When patients with different clinical conditions are pooled into the same group, patients with the most common condition in the total group have greatest impact on the result of the total group. Based on the present observations, risk patterns for a certain oral condition are not necessarily comparable with the implant treatment received by other patients, and the external validity may be limited in small, homogeneous groups of patients.

Abstract:BACKGROUND:Small-diameter implants (SDIs: diameter <3.5 mm) are often chosen as an alternative to bone augmentation in clinical practice, but the scientific evidence regarding SDI application in the posterior area remains deficient.PURPOSE:To evaluate the clinical and radiographic outcomes of SDIs supporting fixed prostheses without bone augmentation in the posterior region, and to analyze the potential influencing factors related to SDI failures.MATERIALS AND METHODS:Clinical and radiographic data of 243 SDIs in 156 patients were retrospectively assembled after 2 to 12 (mean 4.75) years of follow-up. Implant and prosthesis failures, mechanical and biological complications, and radiographic marginal bone loss (MBL) were evaluated. The influence of patient/implant characteristics and prosthetic design on SDI failures was investigated.RESULTS:Five implants in five patients failed, contributing to 10-year cumulative survival rates of 97.9% on an implant-based analysis and 96.8% on a patient-based analysis. Biological complications and mechanical complications were detected in 22 (9.1%) and 31 (12.8%) of implants, respectively. No implant fracture was detected. Peri-implant MBL during 10 years was 0.60 ± 0.90 mm on average. The implant type (bone-level or tissue-level) was the only factor that significantly influenced SDI failures.CONCLUSION:SDIs supporting fixed prostheses in the posterior region achieved predictable long-term clinical outcomes. However, tissue-level titanium SDIs should be avoided where possible.
Implant Survival and Complication Prevalence in Complete-Arch Implant-Supported Fixed Dental Prostheses: A Retrospective Study with a Mean Follow-up of 5 Years.

Abstract:Purpose: To evaluate the implant survival and the prevalence of biologic and mechanical complications in edentulous patients restored with complete-arch implant-supported fixed dental prostheses (IFDPs). Materials and Methods: Patients restored with complete-arch screw-retained IFDPs between January 2012 and December 2019 with a minimum 2-year follow-up were included. Outcome measures were cumulative survival rate (CSR) for implants and prostheses, biologic complications, and mechanical complications. A generalized estimating equation model was used to estimate potential risk factors for mechanical complications. Patient satisfaction was investigated using a standardized questionnaire. Results: A total of 44 prostheses supported by 268 implants in 30 patients were included for a mean duration of 4.8 years (range: 2 to 9 years). Eighteen of the prostheses were zirconia-ceramic (group ZC), and 26 were titanium-ceramic (group TC). The CSR for the implants and IFDPs was 99.3% (95% CI: 98.2% to 100.3%) and 92.5% (95% CI: 84.2% to 100.8%), respectively. The most common biologic complication was peri-implant mucositis (4.5%), followed by peri-implantitis (3.0%). The most common mechanical complication was ceramic chipping (45.5%), followed by crown debonding (13.6%) and framework fracture (4.5%). There was no significant difference in the prevalence of complications between groups TC and ZC (P > .050). The presence of cantilever (OR = 5.54, P = .048) and maxillary arch (OR = 5.94, P = .041) were significantly associated with mechanical complications. Patient satisfaction scores were generally high, but some continued to be bothered by speech problems (13.6%). Conclusion: Complete-arch IFDPs presented reliable clinical outcomes for edentulous patients with a high implant survival rate and a high level of patient satisfaction. However, a high incidence of mechanical complications occurred in the long term.

Abstract:Statement of problem Short implants are a therapeutic alternative for edentulous patients with severe bone resorption. Differences in peri-implant bone loss and complications of short implants depending on the type of connection are unclear. Purpose The main purpose of this clinical study was to evaluate the survival rate after 2 years of the short implants in the Oxtein system (Proclinic). Secondary objectives were to compare implant survival, peri-implant bone loss, peri-implant mucosal status, and associated complications in internal hexagonal connection versus external hexagonal connection implants. Material and methods A randomized clinical trial was carried out in 14 patients with a mean age of 62.7 ±8.5 years, with a total of 61 Oxtein L35 and L6 Proclinic implants being placed at the Faculty of Dentistry. A descriptive analysis, simple binary logistic regression model using generalized estimating equations. and Kaplan-Meier survival analysis were carried out (α=.05). Results Implant survival was 85.2% (52/61). Failure of all implants occurred before prosthetic loading; bleeding after probing occurred in 28 implants, being greater among external connection implants (57.6%) P =.025. The presence of plaque appeared in 36 of the implants, without statistically significant differences between external connection (72.8%) and internal connection (60.0%) ( P >.05). A total of 28 implants had at least 2 mm of keratinized mucosa, without statistically significant differences between external connection (63.6%) and internal connection (35.0%) P =.200. A total of 8 complications (13.1%) were recorded, including connection fractures, screw fracture, framework fracture, and buccal fenestrations. Conclusions Short implants are a therapeutic solution to more complex surgical techniques. However, as lower survival has been reported, the characteristics of the implants and operator experience are important factors for their success.

Use of narrow-diameter implants in the posterior segments of the jaws: A retrospective observational study of 2 to 11 years

Abstract:Statement of problem: The placement of narrow-diameter implants (NDIs) in the posterior region is still debated in view of the high biomechanical risks in these areas. Purpose: The purpose of this retrospective observational study was to evaluate the success and survival rates of NDIs restored with fixed prostheses in the posterior region (primary outcome) and analyze whether splinting multiple units (prosthesis design) affects the biological and mechanical complications (secondary outcome). Material and methods: Dental records from 2 private clinics were reviewed for NDIs in the posterior region installed between 2009 and 2018. Ninety study participants (58 women and 32 men) aged between 21 and 84 years (mean age 49.9 years) were recalled for the assessment of implant survival and success of 160 NDIs previously provided for partial posterior edentulism associated with moderate horizontal bone loss or reduced interradicular space (105 premolars and 55 molars). The implants were restored with metal-ceramic single crowns or splinted multiple-unit prostheses, either screw-retained or cemented on custom (n=100) or stock titanium abutments (n=60). Peri-implant probing depth (PPD), bleeding on probing (BOP), bone quality, type of edentulism, and patient satisfaction were scored. The chi-squared test for independence and 2-sample Welch t test were performed for statistical analysis (α=.05). Results: The overall success rate was 89.37%. One implant had been removed 4 years after loading, another after 9 years, yielding a cumulative survival rate of 98.75%. Fourteen implants exhibited PPD > 5 mm. One implant and 1 abutment screw fractured, and 16 restorations demonstrated porcelain chipping. The chi-squared test showed no significant relationship between prosthetic design and complications whether biological (P=.087) or mechanical (P=.805). Eighty-two percent of patients were satisfied with esthetics, 76% with function, 85% with total duration of treatment, and 90% with overall treatment cost. Conclusions: Within the limitations of this retrospective study, NDIs may be considered a reliable option to replace posterior teeth. The prosthetic design had no significant impact on biological or mechanical complications.

Abstract:The dental implants in modern dentistry brought out a new era in everyday dental practice. Increasing of dental implants usage, various failures and complications has occurred. Failures and complications of dental implant treatments that can happen in implant surgery, in mechanical or prosthetic problem and in biological aspect. The aim of this study was to assess implant failures for 6 years as well as find out how to overcome implant failure. In Jeju National University hospital, 26 dental implants in 17 patients were removed by implant failure through 2013.1.1.-2018.12.31. Each implant failure case was analyzed in report form with various informations about failure retrospectively. The present study showed 26 failed dental implants of 17 patients were removed. 13 implants(50%) of 8 patients(47.06%) were failed before functional loading and 13 implants(50%) of 9 patients(52.94%) were failed after functional loading. 11 implants(42.31%) of 7 patients(41.18%) with medical disease were extracted. 6 implants(23.08%) of 5 patients(29.41%) with additive bone graft were failed. We discuss with 26 failed dental implants of 17 patients about their causes, solutions and prevention retrospectively for 6 past years. Precise diagnosis and treatment planning are needed. Medically compromised patients and patients with implants and additive bone grafts should be cautiously treated with high failing possibilities.
Survival, complications, and patient-reported outcomes of endodontically treated teeth versus dental implant-supported prostheses: A systematic review

Abstract:Statement of problem Decision making for compromised teeth involving the choice between endodontic treatment and tooth extraction followed by an implant-supported prosthesis is challenging. However, systematic reviews examining studies using the same patients or clinical settings to provide conclusive evidence regarding the best approach are lacking. Purpose The purpose of this systematic review was to compare the survival rate, complications, failure, and patient-reported outcomes of endodontically treated teeth and implant-supported prostheses. Material and methods After the protocol had been registered at the International Prospective Register of Systematic Reviews (PROSPERO), PubMed, Scopus, and the Cochrane Database of Systematic Reviews were searched from database inception to July 2023 with no language restriction. A manual literature search was performed. The review protocol was based on the population, intervention, comparator, outcome, and study design (PICOS) criteria and included all observational and experimental human studies that directly compared the survival, complications, and patient-reported outcomes of teeth with pulpal and periapical disease after all types of endodontic treatment and subsequent restoration and tooth extraction followed by an implant-supported prosthesis. The risk of bias of the included studies was assessed by using the modified Newcastle-Ottawa scale. Results Eight observational studies were included in this systematic review: 3 retrospective cohort and 5 case-control studies. Three included studies revealed no difference in survival rate between endodontically treated teeth and implant-supported prostheses during the first 3 years, but the survival of endodontically treated teeth declined over time with a higher failure rate than implant-supported prostheses. In contrast, the other 3 included studies reported lower survival rate for implant-supported prostheses and more complications. In terms of patient-reported outcomes, patients were generally satisfied with both treatment modalities, with notable improvements in oral health-related quality of life in those receiving endodontic treatment. Conclusions Whether implant-supported prostheses or endodontically treated teeth are better in terms of survival outcome is unclear. Improved oral health-related quality of life was found after endodontic treatment.

Intricate Assessment and Evaluation of Effect of Bruxism on Long-term Survival and Failure of Dental Implants: A Comparative Study

Abstract:Introduction: Dental implants are one of the common lines of treatment used for the treatment of missing tooth. Various risk factors are responsible for the failure of the dental implants and occurrence of postoperative complications. Bruxism is one such factor responsible for the failure of the dental implants. The actual relation between bruxism and dental implants is a subject of long-term controversy. Hence, we carried out this retrospective analysis to assess the complications occurring in dental implants in patients with and without bruxism. Materials and methods: The present study included 1100 patients which were treated for rehabilitation by dental implant procedure at 21 dental offices of Ghaziabad (India) from 2004 to 2014. Analyzing the clinical records of the patients along with assessing the photographs of the patients was done for confirming the diagnosis of bruxism. Clinical re-evaluation of the patients, who came back for follow-up, was done to confirm the diagnosis of bruxism. Systemic questionnaires as used by previous workers were used to evaluate the patients about the self-conscience of the condition. Estimation of the mechanical complications was done only in those cases which occurred on the surfaces of the restoration of the dental implants. All the results were analyzed by Statistical Package for Social Sciences (SPSS) software. Student's t-test and Pearson's chi-square test were used to evaluate the level of significance. Results: In both bruxer and non-bruxers, maximum number of dental implants was placed in anterior maxillary region. Significant difference was obtained while comparing the two groups for dimensions of the dental implants used. On comparing the total implant failed cases between bruxers and non-bruxers group, statistically significant result was obtained. Statistically significant difference was obtained while comparing the two study groups based on the health parameters, namely hypertension, diabetes, and smoking habit. Conclusion: Success of dental implant is significantly affected by bruxism. Special attention is required in such patients while doing treatment planning. Clinical significance: For the long-term clinical success and survival of dental implants in patients, special emphasis should be given on the patient's deleterious oral habits, such as bruxism as in long run, they influence the stability of dental implants.
Long-term outcomes of short dental implants supporting single crowns in posterior region: a clinical retrospective study of 5-10 years.

Abstract:Objective Aimed to evaluate the long-term clinical and radiographic outcomes of short implants supporting single crowns in the posterior regions. Material and methods A retrospective study design was adopted. The clinical and radiographic data of 231 short implants (intra-bony length <= 8 mm) supporting single crowns in 168 patients, were collected after 510 (mean 7.22) years' follow-up. Implant and prosthesis failures, peri-implant marginal bone loss, biological and technical complications were evaluated. The influence factors on implant failure were studied. Results In total 4 implants and 11 prostheses failed. The 10-year (5-year) cumulative survival rate was 98.3% (98.7%) for implant-based analysis and 97.6% (98.2%) for patient-based analysis. The short implants placed in type IV bone yielded more failures than in type I-III and presented a survival rate of 94.0%. The 10-year survival rate of the prostheses was 95.2%. The mean marginal bone loss between implant installation and the 10 years' follow-up visit was 0.63 +/- 0.68 mm. The marginal bone loss between the first and 5th year was minimal (0.05 +/- 0.10mm and not statistically significant). 18 (7.8%) implants were exposed to biological complications, whereas 29 (12.6%) implants were involved in technical complications. Conclusions High survival rates for both the implants and the prostheses could be achieved after 510 years for short implants supporting single crowns, without severe marginal bone loss and complications. One may conclude that a single crown supported by a short implant is a predictable treatment modality. However, short implants in type IV bone sites should be applied with caution.

Methods: We performed a retrospective study on over 100 randomly selected patients per analysis to determine the effect of health status, smoking status, sex, implant location and operative conditions on first and second (re-implantation) implant survival. Analyses were conducted first using chi-squared test, followed by multiple logistic regression for significant variables. Results: In the cohort examining the effect of myriad risk factors on second implant survival, it was found that OPN and OP greatly impacted implant survival, wherein patients with osteoporosis or osteopenia had significantly more implant failures (p = 0.0353). Sex and operative conditions had no effect on implant survival, while implant location showed a notable effect wherein significantly more failures occurred in the maxilla vs mandible (p = 0.0299). Upon finding that OPN and OP have a significant effect on second implant survival, we conducted an additional study focusing on the impact of health status. Based on the multiple logistical regression analysis, we found that OPN and OP are the most significant factor in first implant survival (p = 0.0065), followed by diabetes (p = 0.0297). Importantly, it was observed that early implant failure is also significantly correlated with osteoporosis (p = 0.0044). Conclusion: We show here a marked relationship in which the risk of first and second implant failure are significantly higher in patients with osteoporosis and osteopenia.
Retrospective Radiographic Analysis of Peri-Implant Bone Loss in Mandibular Full-Arch Implant Rehabilitations

Abstract:Objectives: Can the type of implant rehabilitation influence peri-implant bone loss in case of full-arch mandibular prosthesis? The purpose of the study was to assess, using orthopantomograms (OPGs), the bone loss around implants in different types of implant-supported prosthetic rehabilitations and identify potential risk factors, associated with the number and location of implants, that may have an association with bone defects. Methods: A radiographic study was conducted on 22,317 OPGs from 2010 to 2024. All OPGs with implant-supported prosthetic mandibular rehabilitations were included in the study. Results: A total of 155 OPGs were evaluated, with peri-implant bone loss identified in 64 (41.3%). Distal implants (furthest from the center) across various positioning patterns were most susceptible to bone loss, with positions 3.6 and 4.6 demonstrating the most frequent occurrence (25 and 26 cases, respectively). The χ2 test revealed significant associations between both the implant positioning pattern (p < 0.001) and number of implants (p < 0.001) with peri-implant bone loss. Also, by updating the sample of OPGs, increased susceptibility to bone resorption was found for implants placed distal to the mental foramen compared to mesial ones in full-arch-implant-supported fixed prostheses. Conclusions: Prospective clinical studies will therefore be useful in investigating this finding further.

Abstract:OBJECTIVE To evaluate the long-term clinical and radiographic outcomes of dental implant placed using osteotome sinus floor elevation (OSFE) without bone grafts, and to analyze the potential influence factors of implant survival and endo-sinus bone remodeling. MATERIAL AND METHODS A retrospective study design was adopted. The clinical and radiographic data of 96 implants in 80 patients were collected after 4-9 (mean 5.40) years follow-up. Implant failures, peri-implant marginal bone loss (MBL), and endo-sinus bone remodeling on the radiographs were evaluated. A life-table analysis was used to assess the implant survival. Statistical models were established to investigate the potential influence factors of implant survival and endo-sinus bone gain (ESBG). RESULTS In total, nine implants in seven patients failed, giving the 9-year cumulative survival rates of 90.6% and 91.3% for implant-based analysis and patient-based analysis, respectively. The mean MBL between implant installation and the 4- and 9-year follow-up visit was 0.46 ± 0.88 and 0.50 ± 1.69 mm, respectively. The average ESBG on radiographs was 2.95 ± 1.25 and 2.16 ± 1.13 mm at the 4- and 9-year follow-up. The final ESBG was found to be positively correlated to implant protrusion length after surgery without any other factors related. The implant survival rate was significantly lower in severe atrophic site (residual bone height <5 mm). CONCLUSION Osteotome sinus floor elevation without bone grafts is a predictable treatment modality in the long run. But it should be used with caution when the initial bone height of the edentulous site is lower than 5 mm. The final endo-sinus bone height was found to be positively correlated to implant protrusion length measured on radiographs immediately after implant installation.

Abstract:Objective: This retrospective research was directed to investigate the longevity and maintenance requirements of implant-supported prostheses in subjects treated at a tertiary care center. Methods: Patient records of individuals who received implant-supported prostheses between January 2018 and December 2020 were retrospectively analyzed. Demographic information, implant type, prosthetic design, and follow-up data were collected. The primary outcomes assessed were prosthetic survival rates and the incidence of complications requiring maintenance interventions. Statistical analysis was performed to identify associations between demographic and clinical variables and the observed outcomes. Results: The research included 150 subjects with an average follow-up duration of 3 years. The overall survival rate of implant-supported prostheses was 90%, with 20% requiring maintenance interventions. Common complications included peri-implantitis (10%), prosthetic fracture (6%), and screw loosening (4%). Significant associations were found between smoking status and peri-implantitis (P < 0.05) and between prosthetic design and prosthetic fracture (P < 0.01). Conclusion: Implant-supported prostheses demonstrated a high overall longevity rate, with a subset of subjects requiring maintenance interventions due to complications. Peri-implantitis, prosthetic fracture, and screw loosening were the most prevalent complications, emphasizing the importance of tailored patient care and risk stratification.

Abstract:Dental implant therapy is considered a successful oral rehabilitation treatment. Advances in design, surface characteristics and surgical protocols make implant rehabilitation an effective treatment alternative for edentulous areas and solve functional, phonetic aesthetic and psychological problems; however, risk factors such as diabetes can compromise osseointegration or negatively affect the maintenance of peri-implant biological health. Marginal Bone loss and plaque index have been used to identify peri-implant diseases related to oral rehabilitation with implants. Objective: analyze the survival rate of dental implants in diabetic patients compared to non-diabetic patients. Methodology: The PRISMA statement parameters were used to develop the search strategy and answer the PICO question: does implant placement in healthy versus diabetic patients have a higher survival rate? Results: From a total of 175 articles, 13 were selected for bias analysis. The quantitative variables of the different studies were analyzed to answer the PICO question. Conclusions: It was found that there is no relationship between the survival rate un controlled diabetic and non-diabetic patients according to the analysis of marginal bone loss and plaque index, however, significant differences were found between uncontrolled diabetic patients and non-diabetic patients.

Abstract:Objective: To reevaluate the submucosally embedded in situ appendix as continence mechanism in a large single institutional series of ileocecal urinary reservoirs. Material and Methods: Between November 1990 and June 1999 an ileocecal reservoir with appendico–umbilical stoma was created in 118 patients (84 men, 34 women) aged 3.9–82.7 (mean 56.8) years as a primary urinary diversion or after failure of previous reconstruction. The most common indication for urinary diversion was bladder replacement after anterior exenteration for pelvic malignancies (n = 98), followed by functional or morphological bladder loss due to various benign conditions. The patients were followed prospectively according to a standard protocol. Results: There were no perioperative deaths. In 3 patients necrosis of the appendix resulted in total incontinence with subsequent replacement by an intussuscepted ileal nipple. Impaired catheterization due to stomal stenosis was observed in 19 patients with recurrence in 6 and a total of 25 minor revisions. With a mean follow–up of 60 months all patients are continent day and night. Conclusion: Over 10 years, the submucosally embedded in situ appendix has survived as a continence mechanism in the original technique reliably providing continence in ileocecal reservoirs.
