Abstract:Background: Orthodontics is a speciality of dentistry that uses a plethora of devices made from myriad materials to manage various malocclusions. Prolonged contact of orthodontic appliances with oral tissues can lead to cellular damage, highlighting the need for biocompatible materials to mitigate health risks. Objectives: To analyze the genotoxicity and cytotoxicity produced by metal brackets and coated metallic brackets with polymeric and nanoparticle coatings in oral mucosal cells. Materials & methods: The current study compares the toxicity of 3 different types of orthodontic brackets with control groups of oral mucosal cells. Each of the three treatment groups consisted of 10 samples of orthodontic brackets: stainless steel brackets(Group 1), nanoparticle-coated brackets(Group 2), and polymeric-coated brackets(Group 3) exposed to corrosion eluates employing an oral biomimicry model. Two types of oral mucosal cells- Human Gingival Fibroblasts and Buccal Epithelial Cells were used to study the cytotoxic and/or genotoxic effects of the elutes. Intergroup comparisons were conducted using one-way analysis of variance, while scanning electron microscopy evaluated surface characteristic. Results: The interaction between metal ions and oral mucosal cells showed no statistically significant difference for toxicity assays between the three groups(p > 0.005). However, polymeric and nanoparticle-coated groups showed reduced cellular differentiation when compared with conventional stainless-steel brackets. Conclusion: This in-vitro study shows that polymeric or nanoparticle coating of conventional metal brackets aids in enhancing corrosion-resistant characteristics of orthodontic appliances and reduces the toxic oral environment created by metal release in the oral cavity.
Metal Release and Cytotoxicity of Different Orthodontic Bracket-Wire Combinations: An In Vitro Study

Abstract:Aim: To quantify and compare the metal ions released from different bracket-wire combinations and to assess their cytotoxicity. Materials and methods: A total of 360 fabricated sectional fixed orthodontic appliances were divided into 6 groups. The first three groups consisted of stainless-steel brackets with stainless-steel, snickel-titanium (NiTi), and titanium-molybdenum alloy (TMA) archwires, and the other three groups were fabricated using ceramic brackets (polycrystalline alumina) with stainless-steel, NiTi, and TMA archwires. These appliances were immersed in artificial saliva (pH 6.5 ± 0.5, 37°C), for 1 week, 2 weeks, and 1 month. The nickel and chromium ions released in the artificial saliva were quantified using a flame atomic absorption spectrometer, and cytotoxicity assessment was performed using a 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide (MTT) assay on human cervical cancer cell line. Results: The stainless-steel bracket groups displayed a significantly greater release of nickel and chromium ions compared to the ceramic bracket groups (P < 0.05). However, no significant differences were identified when comparing the three archwire types within the stainless-steel/ceramic bracket groups. At the end of 1 month, the % cell viability demonstrated by the appliances was in the decreasing order of stainless-steel-TMA > ceramic-stainless steel > stainless-steel-NiTi > ceramic-NiTi > stainless-steel-stainless steel > ceramic-TMA. Conclusion: Considerably greater release of nickel and chromium ions was observed from the appliances utilizing stainless-steel brackets compared to those employing ceramic brackets. However, no remarkable difference in the levels of nickel and chromium ions was observed when comparing the three archwires: stainless steel, NiTi, and TMA. In the cytotoxicity assessment, the ceramic-TMA combination displayed the highest level of cytotoxicity, while the stainless-steel-TMA combination displayed the least cytotoxicity.

Abstract:Orthodontic treatment improves dental aesthetics. However, the prolonged duration of treatment and patient's oral hygiene results in more significant biofilm buildup and boosting concentrations of acidogenic bacteria, resulting in enamel demineralization. Plasma treatment technology has developed an interest in enhancing appliances' surface properties. The present study aimed to investigate cytotoxicity and antibacterial activity represented by anti-adhesion and anti-biofilm formation of Argon plasma treatment on orthodontic brackets against Streptococcus mutans and Lactobacillus acidophilus and evaluation the effect of change in plasma exposure duration on these properties in vitro. The study included three groups, group 1: untreated stainless steel brackets, group 2: brackets treated for 15 min, and group 3: brackets treated for 30 min. S.mutans and L.acidophilus were isolated from patients undergoing fixed orthodontic treatment. Microbiological tests included adhesion assay using plate counting method and biofilm formation assay using ELISA plate reader. S. mutans& L. acidophilus were cultivated in three groups, and their adherence and biofilm formation were measured and compared. The biocompatibility of treated stainless steel brackets was evaluated by MTT assay. Results showed that groups 2 & 3 had better anti-adhesion and anti-biofilm activity when compared with group 1, with a significant difference between group 1 &group 3, in which group 3 recorded the lowest biofilm formation and the highest anti- anti-A adhesion activity. MTT showed that groups 2 and 3 exhibited more than 80% cell viability after 24, 48, and 72 hours. Plasma surface treatment of metal brackets possesses antibacterial activity against S.mutans and L.acidophilus; alteration exposure time impacts the development of these properties. Keywords. Abstract:Abstract Background The purpose of this study was to assess the surface characterization and frictional resistance between stainless steel brackets and two types of orthodontic wires made of stainless steel and nickel-titanium alloys after immersion in a chlorhexidine-containing prophylactic agent. Methods Stainless steel orthodontic brackets with either stainless steel (SS) or heat-activated nickel-titanium (Ni-Ti) wires were immersed in a 0.2% chlorhexidine and an artificial saliva environment for 1.5 h. The frictional force was measured on a universal testing machine with a crosshead speed of 10 mm/min over a 5-mm of archwire. The surface morphology of bracket slots and surface roughness of archwires after immersion in chlorhexidine were also characterized using a scanning electron microscope (SEM) and an atomic force microscope (AFM), respectively. Results There was no significant difference in the frictional resistance values between SS and Ni-Ti wires immersed in either chlorhexidine or artificial saliva. The frictional resistance values for the SS and Ni-Ti wires immersed in 0.2% chlorhexidine solution were not significantly different from that inartificial saliva. No significant difference in the average surface roughness for both wires before (as-received) and after immersion in either chlorhexidine or artificial saliva was observed. Conclusions One-and-half-hour immersion in 0.2% chlorhexidine mouthrinse did not have significant influence on the archwires surface roughness or the frictional resistance between stainless steel orthodontic brackets and archwires made of SS and Ni-Ti. Based on these results, chlorhexidine-containing mouthrinses may be prescribed as non-destructive prophylactic agents on materials evaluated in the present study for orthodontic patients.

Evaluation of Mechanical and Elemental Properties of Bioceramic-Coated Orthodontic Brackets and Enamel Surface

Abstract:Objective: The aim is to coat orthodontic brackets with two different bioactive materials and to compare the mechanical and morphological properties of coated brackets and tooth surfaces. Materials and methods: A total of 120 stainless steel brackets were divided equally into three groups, that is, the uncoated brackets and nanohydroxyapatite (nHA)-coated, and nanobioactive glass (nBG)-coated brackets using a spin coater machine. The brackets were bonded on the enamel surface and underwent remineralization/demineralization cycles for days 1, 7, 14, and 30. At each time interval, the bond strength of the brackets was assessed using mechanical loading. An optical and scanning electron microscope (SEM) were used for surface evaluation, and the adhesive remanent index (ARI) values were obtained and quantified. Statistical analysis: One-way analysis of variance using Tukey's test was used to compare the differences among the groups. Results: A uniform distribution of nanoparticles occurred on the surfaces of brackets. The shear bond strength (SBS) showed no significant differences in any tested groups on days 1, 7, and 14. However, control and nBG showed a significant difference from nHA at day 30. On days 7, 14, and 30, the nHA group showed the highest SBS values among the groups. For ARI, most samples showed an adhesive nature of failure at the enamel-brackets interface. The images confirmed the presence of coated particles on brackets and remnants of adhesives after SBS. Conclusion: This study confirmed that the nHA- and nBG-coated brackets have a high potential for application in orthodontics regarding structural and mechanical properties.

Abstract:Introduction: The release of metallic ions from orthodontic brackets and wires typically depends on their quality (chemical composition) and the medium to which they are exposed, e.g., acidic, alkaline, substances with a high fluoride concentration, etc. This review examines corrosion and wear of orthodontic brackets, wires, and arches exposed to different media, including: beverages (juices), mouthwashes and artificial saliva among others, and the possible health effects resulting from the release of metallic ions under various conditions. Objective: This review aims to determine the exposure conditions that cause the most wear on orthodontic devices, as well as the possible health effects that can be caused by the release of metallic ions under various conditions. Sources: A search was carried out in the Scopus database, for articles related to oral media that can corrode brackets and wires. The initial research resulted in 8,127 documents, after applying inclusion and exclusion criteria, 76 articles remained. Conclusion: Stainless steel, which is commonly used in orthodontic devices, is the material that suffers the most wear. It was also found that acidic pH, alcohols, fluorides, and chlorides worsen orthodontic material corrosion. Further, nickel released from brackets and wires can cause allergic reactions and gingival overgrowth into patients.

Abstract:AIM: To evaluate the changes of static and kinetic frictional forces between the brackets and wires following exposure to a soft drink, acidic food ingredient, and acidulated fluoride prophylactic agents.MATERIALS AND METHODS: Two types of Roth prescription mandibular incisor brackets were used: 3M Unitek Victory stainless steel (SS) brackets (n = 40) and Transcend 6000 polycrystalline alumina (PCA) brackets (n = 40) as well as eighty 0.019 × 0.025" dimension ortho technology SS wires of 50 mm length each. Subsequently, brackets tied with SS wires divided into eight subgroups (n = 10) and were immersed in vinegar (pH = 3.5 ± 0.5), Pepsi ® (pH = 2.46), Colgate Phos-Flur mouth rinse (pH = 5.1), and artificial saliva (control group pH = 7) for 24 h. Changes in surface morphology under scanning electron microscope ×1000, surface roughness (Ra) with surface profilometer (single bracket and single wire from each subgroup), and frictional resistance using universal testing machine were evaluated.RESULTS: Highest mean (standard deviation) static frictional force of 2.65 (0.25) N was recorded in Pepsi ® followed by 2.57 (0.25) N, 2.40 (0.22) N, and 2.36 (0.17) N for Vinegar, Colgate Phos-Flur mouth rinse, and artificial saliva groups, respectively. In a similar order, lesser mean kinetic frictional forces obtained. PCA brackets revealed more surface deterioration and higher frictional force values than SS brackets. A significant positive correlation was observed between frictional forces and bracket slot roughness (r = 0.861 and 0.802, respectively, for static and kinetic frictional forces, p < 0.001 for both) and wire roughness (r = 0.243 and 0.242, respectively, for static and kinetic frictional forces, p < 0.05 for both).CONCLUSIONS: Findings may have long-term implications when acidic food substances are used during fixed orthodontic treatment. Further, in vivo studies are required to analyze the clinical effect of acidic mediums in the oral environment during orthodontic treatment.

Abstract:Materials used in orthodontics are constantly changing and improving, but biocompatibility remains one of their most important characteristics.The purpose of this study is to evaluate the in vitro cellular behavior of dental MSC cells when put in contact with metal and 3D printed resin orthodontic attachments.Cell monolayers were seeded in wells and left to adhere. Polymer and metal orthodontic attachments were placed into the wells. The cells were analyzed on an inverted light microscope. The cellular viability was determined using Alamar Blue assay. Results showed that the morphology and the density of cells are not affected by the presence of either orthodontic material compared to the controls.The metabolic activity of the cells was not altered. Similar cellular reactions,were observed in contact with both materials used in our short term in vitro study.
Factors Affecting the Shear Bond Strength of Orthodontic Brackets - a Review of In Vitro Studies

Abstract:The adhesive material used to bond orthodontic brackets to teeth should neither fail during the treatment period, resulting in treatment delays, untoward expenses or patient inconvenience nor should it damage the enamel on debonding at the end of the treatment. Although the effectiveness of a bonding system and any unfavorable effects on the enamel may be studied by conducting in-vivo studies, it is nearly impossible to independently analyze different variables that influence a specific bonding system in the oral environment. In-vitro studies, on the other hand, may utilize more standardized protocols for testing different bonding systems and materials available. Thus, the present review focused attention on in-vitro studies and made an attempt to discuss material-related, teeth-related (fluorotic vs non-fluorotic teeth) and other miscellaneous factors that influences the shear bond strength of orthodontic brackets. Within the limitations of this review, using conventional acid-etch technique, ceramic brackets and bonding to non-fluorotic teeth was reported to have a positive influence on the shear bond strength of orthodontic brackets, but higher shear bond strength found on using ceramic brackets can be dangerous for the enamel.
In Vitro Comparative Evaluation of Frictional Resistance of Connecticut New Arch Wires, Stainless Steel and Titanium Molybdenum Alloy Archwires Against Different Brackets

Abstract:Background and objectives Friction between the bracket and archwire during sliding mechanics is of great concern in orthodontics, as it reduces the effectiveness of the orthodontic appliance and slows down tooth movement. The aim of this study was to evaluate frictional resistance of stainless steel (SS), titanium molybdenum alloy (TMA), and Connecticut new arch (CNA) wires against SS and ceramic brackets. The surface textures of the brackets and wires were also evaluated by scanning electron microscopy (SEM) before and after testing. Method A total of 180 premolar brackets of SS (ORMCO Corp., Orange, CA) and 180 ceramic (3M Unitek, Maplewood, MN) with a 0.022-inch slot and 180 SS, TMA, and CNA wires of 0.017 x 0.025 inches and 0.019 x 0.025 inches were tested. The SS brackets and ceramic brackets were bonded onto the SS bar with cyanoacrylate adhesive with the help of a jig. The wire assembly was vertically mounted and clamped to the jaws of the universal testing machine with 10 N load cell, and friction was measured along with other readings. The surface roughness of brackets and wires were examined using SEM in 200 X magnification before and after testing. Results TMA wire showed the greatest frictional force compared to SS and CNA wire. The frictional force was greater in the 0.019 x 0.025-inch wire compared to the 0.017 x 0.025-inch wires. The highest frictional force was noted in the SS bracket and 0.019 x 0.02-inch TMA wire combination. A statistically significant difference was not seen between the SS bracket and 0.019 x 0.025-inch SS wire and the 0.019 x 0.025-inch CNA wire combinations. SEM showed that the TMA archwire had the roughest surface area compared to SS and CNA wires, and the ceramic bracket had more surface roughness than the SS bracket. Conclusion CNA wire demonstrated frictional resistance similar to the SS wire. CNA wire can be used instead of TMA wire because of its better range of action, high spring back, and less frictional resistance for space closure in sliding mechanics.

Annealing process influence and dopant-silicide interaction in self-aligned NiSi technology

Abstract:This paper reports the annealing process influence on interfacial electrical properties, dopant effects on silicide formation, and dopant redistribution during silicidation in self-aligned NiSi technology. The reverse leakage current results of NiSi/n-Si Schottky diodes show that two-step rapid thermal process (RTP) significantly improves the NiSi/Si area-contact characteristics in comparison with one-step RTP, and low temperature in RTPI is beneficial to the final Ni-silicide/Si contact properties. Both structural and electrical characterization shows substantially different Ni-silicidation behaviors on heavily-doped n and p Si substrates at low temperature (300/spl deg/C). The larger grain size of Ni/sub 2/Si formed on heavily As-doped Si is responsible for the lower resistivity, comparing with Ni:Si formed on heavily B-doped Si. Ni/Si reaction on highly doped Si substrates also results in significant dopant segregation at the silicide/Si interface and pile up in void-layer formed just underneath the silicide surface.

Evaluation of the Effects of Povidone Iodine and Hydrogen Peroxide Mouthwashes on Orthodontic Archwires: An In Vitro Study

Abstract:Aim: To evaluate the effects of two preprocedural mouthrinses, hydrogen peroxide (H2O2) and povidone iodine (PI) on the surface characteristics and mechanical properties of nickel-titanium (NiTi) and stainless steel (SS) orthodontic archwires. Materials and methods: Five wire specimens were used, each (0.016" NiTi, 0.016" SS wires, 0.016 × 0.022" NiTi and 0.016 × 0.022" SS wires) specimen was cut into 30 mm lengths and immersed in 9% of artificial saliva and 91% of two preprocedural mouthrinse solutions: 1.5% hydrogen peroxide mouthwash, 0.2% povidone-iodine mouthwash, and distilled water (control group) for 90 minutes and incubated at 37°C. The wire specimens were then subjected to a three-point bending test for mechanical testing and viewed under a scanning electron microscope (SEM) to evaluate their surface characteristics. The collected data were analyzed using one-way analysis of variance (ANOVA) and Bonferroni post hoc test. Results: The results showed a significant increase in the flexural modulus (E) of Nitinol wires in povidone-iodine gargle (p < 0.05) and a significant increase in the E of stainless steel wires in hydrogen peroxide mouthwash (p < 0.05). Analysis using SEM showed varying qualitative surface changes in the form of corrosion, voids, and ridges on the wires after exposure to both the mouthwashes. Conclusion: Though there were significant changes in the flexural modulus of archwires for both the mouthwashes, hydrogen peroxide did not show a significant difference in the E of wires at most of the deflection intervals when compared with the other two solutions, hence, could be used in orthodontic patients as an effective preprocedural mouthrinse. Clinical significance: Preprocedural mouthrinses can cause surface irregularities on the wires which in turn lead to an increase in friction at the bracket-wire interface, thereby disrupting effective tooth movement and extending the orthodontic treatment time.

Abstract:OBJECTIVE: The aim of this in vitro study was to evaluate the changes in friction between orthodontic brackets and archwires coated with aluminum oxide (Al2O3), titanium nitride (TiN), or chromium nitride (CrN). In addition, the resistance of the coatings to intraoral conditions was evaluated.METHODS: Stainless steel canine brackets, 0.016-inch round nickel-titanium archwires, and 0.019 × 0.025-inch stainless steel archwires were coated with Al2O3, TiN, and CrN using radio frequency magnetron sputtering. The coated materials were examined using scanning electron microscopy, an X-ray diffractometer, atomic force microscopy, and surface profilometry. In addition, the samples were subjected to thermal cycling and in vitro brushing tests, and the effects of the simulated intraoral conditions on the coating structure were evaluated.RESULTS: Coating of the metal bracket as well as nickel-titanium archwire with Al2O3 reduced the coefficients of friction (CoFs) for the bracket-archwire combination (p < 0.01). When the bracket and stainless steel archwire were coated with Al2O3 and TiN, the CoFs were significantly lower (0.207 and 0.372, respectively) than that recorded when this bracket-archwire combination was left uncoated (0.552; p < 0.01). The friction, thermal, and brushing tests did not deteriorate the overall quality of the Al2O3 coatings; however, some small areas of peeling were evident for the TiN coatings, whereas comparatively larger areas of peeling were observed for the CrN coatings.CONCLUSIONS: Our findings suggest that the CoFs for metal bracket-archwire combinations used in orthodontic treatment can be decreased by coating with Al2O3 and TiN thin films.

Abstract:This paper arises from research to develop a profile of the elderly living in the community. Data are presented from interviews with a stratified random sample of 1406 elderly people living in the Trafford area of Greater Manchester. The findings suggest that in reality community care for the elderly means care by lay carers. Assistance with all tasks ranging from intensely personal care, for example bathing and dressing, to the practical household tasks, such as cooking and cleaning, was more often provided to the elderly by spouses and daughters/daughters-in-law than by the statutory services.It is suggested that some form of eligibility criteria may be used either consciously or unconsciously by general practitioners and other health care workers when referring elderly people to the nursing and social services which may mean that the statutory services do not complement the care given by lay carers. Furthermore, the presence of a lay carer may prevent the situation coming to the attention of the primary health care team.
The Cytotoxicity of Orthodontic Metal Bracket Immersion Media.

Abstract:The purpose of the present study was to evaluate the cytotoxic effects of four different orthodontic metal bracket immersion media on primary human oral gingival fibroblasts (HGFs) and one permanent human osteogenic sarcoma cell line (U2OS). Four different metal brackets (Unitek, Tomy, Ormco, and Dentaurum) were immersed in buffer solutions of NaHNO(3) (1 mM) with a pH of 4 or 7, as well as artificial saliva. The concentrations for the experiments were 0.01, 0.1, and 1.0 mul/ml. At the end of the period of bracket immersion, morphological observations were conducted using light microscopy. The tetrazolium reduction assay was used to detect the survival rate of the target cells. Statistical analysis was conducted using one-way analysis of variance. The results showed microscopically no morphological changes of the HGF or U2OS cells exposed to the metal bracket immersion media. At pH 4, the survival rates of the U2OS cells and the HGFs differed statistically for the Unitek (P = 0.003) and Ormco (P = 0.000) groups. At pH 7, the survival rate for the HGFs and the U2OS cells differed statistically for the Dentaurum (P = 0.021) and Unitek (P = 0.03) groups. The results demonstrate that differing cells exhibit various cellular reactions on exposure to metal bracket immersion media, although the four types of brackets appear to be biocompatible with HGF and U2OS cells.

Abstract:The aim was to assess the composition of released metal ions from fixed orthodontic appliances both in an in vitro and in vivo setting and to compare their quantities to evaluate any possible health risks associated with them. For the in vitro setting, a set of 24 as-received sterile SS brackets, 2 SS sterile archwires, and 2 NiTi sterile archwires were aged for 90 days in artificial saliva, and released metal ion concentrations were determined by inductively coupled plasma mass spectrometry (ICP-MS), using matrix-matched standards for calibration. For the in vivo setting, 15 brackets, 15 SS, and 15 NiTi archwires were retrieved after 90 days of intraoral exposure in 15 subjects. Debris composition on each part of the orthodontic appliance was assessed using scanning electron microscopy with energy dispersive spectroscopy (SEM/EDS). The present study evidenced a difference in the composition of released metal ions from fixed orthodontic appliances detected in an in vitro and in vivo setting. Generally, the relative content of metal ions was higher in the retrieved debris than that released in artificial saliva. The concentration of released metal ions from all tested alloys was below the upper recommended limit of daily intake; however, possible local effects in terms of hypersensitivity due to their accumulation in the debris cannot be excluded even with such concentrations.
