Abstract:Introduction: The release of metallic ions from orthodontic brackets and wires typically depends on their quality (chemical composition) and the medium to which they are exposed, e.g., acidic, alkaline, substances with a high fluoride concentration, etc. This review examines corrosion and wear of orthodontic brackets, wires, and arches exposed to different media, including: beverages (juices), mouthwashes and artificial saliva among others, and the possible health effects resulting from the release of metallic ions under various conditions. Objective: This review aims to determine the exposure conditions that cause the most wear on orthodontic devices, as well as the possible health effects that can be caused by the release of metallic ions under various conditions. Sources: A search was carried out in the Scopus database, for articles related to oral media that can corrode brackets and wires. The initial research resulted in 8,127 documents, after applying inclusion and exclusion criteria, 76 articles remained. Conclusion: Stainless steel, which is commonly used in orthodontic devices, is the material that suffers the most wear. It was also found that acidic pH, alcohols, fluorides, and chlorides worsen orthodontic material corrosion. Further, nickel released from brackets and wires can cause allergic reactions and gingival overgrowth into patients.
Quantitative and qualitative analysis of metallic ion release of orthodontic brackets in three different pH conditions - An invitro study

Abstract:Background: Fluoridated mouth rinses improve anti-cariogenic environment but decrease oral pH below critical value, affecting orthodontic bracket surface topography and causing corrosive changes over prolonged use. This invitro study aimed to quantitatively and qualitatively assess the surface topography and metallic ion release of the stainless steel (SS) brackets at varying acidic and alkaline pH. Materials and methods: Forty unused SS brackets were divided into four groups (Group A, B, C, D) and immersed for 48- hours in solutions of artificial saliva and sodium fluoride (0.2 %) mouth rinse at varying pH of 5.5,6.7,7 and 8. The surface morphologic changes were analyzed under scanning electron microscope (SEM) at 50×, 150×, and 500× magnification. The changes in slot area were scored using the customized scale. The Energy Dispersive Xray Spectroscopy Analysis (EDAX) was used to estimate the probed elements' atomic and weight percentage. Results: The mean score of the scale was 3.4 for the brackets immersed in the acidic solution which was statistically significant (p = 0.00)and for alkaline and neutral solutions (p = 0.00). Chromium was found to be significantly higher in the alkaline solution (p = 0.016) followed by the neutral solution. Carbon was found excess in acidic solution than the neutral and alkaline solution. Conclusion: Quantitative and qualitative analysis of the ion release in stainless steel brackets using SEM and EDAX revealed the corrosive effect of fluoride ion causing maximum surface changes in acidic medium and chromium release in alkaline pH.

Abstract:Background: Orthodontics is a speciality of dentistry that uses a plethora of devices made from myriad materials to manage various malocclusions. Prolonged contact of orthodontic appliances with oral tissues can lead to cellular damage, highlighting the need for biocompatible materials to mitigate health risks. Objectives: To analyze the genotoxicity and cytotoxicity produced by metal brackets and coated metallic brackets with polymeric and nanoparticle coatings in oral mucosal cells. Materials & methods: The current study compares the toxicity of 3 different types of orthodontic brackets with control groups of oral mucosal cells. Each of the three treatment groups consisted of 10 samples of orthodontic brackets: stainless steel brackets(Group 1), nanoparticle-coated brackets(Group 2), and polymeric-coated brackets(Group 3) exposed to corrosion eluates employing an oral biomimicry model. Two types of oral mucosal cells- Human Gingival Fibroblasts and Buccal Epithelial Cells were used to study the cytotoxic and/or genotoxic effects of the elutes. Intergroup comparisons were conducted using one-way analysis of variance, while scanning electron microscopy evaluated surface characteristic. Results: The interaction between metal ions and oral mucosal cells showed no statistically significant difference for toxicity assays between the three groups(p > 0.005). However, polymeric and nanoparticle-coated groups showed reduced cellular differentiation when compared with conventional stainless-steel brackets. Conclusion: This in-vitro study shows that polymeric or nanoparticle coating of conventional metal brackets aids in enhancing corrosion-resistant characteristics of orthodontic appliances and reduces the toxic oral environment created by metal release in the oral cavity.
Differences in Metal Ions Released from Orthodontic Appliances in an In Vitro and In Vivo Setting

Abstract:The aim was to assess the composition of released metal ions from fixed orthodontic appliances both in an in vitro and in vivo setting and to compare their quantities to evaluate any possible health risks associated with them. For the in vitro setting, a set of 24 as-received sterile SS brackets, 2 SS sterile archwires, and 2 NiTi sterile archwires were aged for 90 days in artificial saliva, and released metal ion concentrations were determined by inductively coupled plasma mass spectrometry (ICP-MS), using matrix-matched standards for calibration. For the in vivo setting, 15 brackets, 15 SS, and 15 NiTi archwires were retrieved after 90 days of intraoral exposure in 15 subjects. Debris composition on each part of the orthodontic appliance was assessed using scanning electron microscopy with energy dispersive spectroscopy (SEM/EDS). The present study evidenced a difference in the composition of released metal ions from fixed orthodontic appliances detected in an in vitro and in vivo setting. Generally, the relative content of metal ions was higher in the retrieved debris than that released in artificial saliva. The concentration of released metal ions from all tested alloys was below the upper recommended limit of daily intake; however, possible local effects in terms of hypersensitivity due to their accumulation in the debris cannot be excluded even with such concentrations.

Metal Release and Cytotoxicity of Different Orthodontic Bracket-Wire Combinations: An In Vitro Study

Abstract:Aim: To quantify and compare the metal ions released from different bracket-wire combinations and to assess their cytotoxicity. Materials and methods: A total of 360 fabricated sectional fixed orthodontic appliances were divided into 6 groups. The first three groups consisted of stainless-steel brackets with stainless-steel, snickel-titanium (NiTi), and titanium-molybdenum alloy (TMA) archwires, and the other three groups were fabricated using ceramic brackets (polycrystalline alumina) with stainless-steel, NiTi, and TMA archwires. These appliances were immersed in artificial saliva (pH 6.5 ± 0.5, 37°C), for 1 week, 2 weeks, and 1 month. The nickel and chromium ions released in the artificial saliva were quantified using a flame atomic absorption spectrometer, and cytotoxicity assessment was performed using a 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide (MTT) assay on human cervical cancer cell line. Results: The stainless-steel bracket groups displayed a significantly greater release of nickel and chromium ions compared to the ceramic bracket groups (P < 0.05). However, no significant differences were identified when comparing the three archwire types within the stainless-steel/ceramic bracket groups. At the end of 1 month, the % cell viability demonstrated by the appliances was in the decreasing order of stainless-steel-TMA > ceramic-stainless steel > stainless-steel-NiTi > ceramic-NiTi > stainless-steel-stainless steel > ceramic-TMA. Conclusion: Considerably greater release of nickel and chromium ions was observed from the appliances utilizing stainless-steel brackets compared to those employing ceramic brackets. However, no remarkable difference in the levels of nickel and chromium ions was observed when comparing the three archwires: stainless steel, NiTi, and TMA. In the cytotoxicity assessment, the ceramic-TMA combination displayed the highest level of cytotoxicity, while the stainless-steel-TMA combination displayed the least cytotoxicity.

Abstract:On the basis of Fick's law of gas diffusion, it has been proposed that cells in conventional monolayer cultures may be severely hypoxic. Because knowledge of the cellular O2 availability is important for the interpretation of biochemical and toxicological cell culture work, microelectrode measurements of the pericellular PO2 were carried out using the erythropoietin (Epo)-producing human hepatoma cell lines Hep G2 and Hep 3B as an in vitro model. In confluent hepatoma cultures grown in polystyrene dishes and incubated in air with 5% CO2, the pericellular steady-state PO2 was < 1 mmHg. The rates of the production of immunoreactive Epo and lactate were high due to a misproportion between O2 supply and O2 requirements. Epo production decreased when shaken instead of static cultures were studied, or when the O2 concentration in the gas atmosphere was increased gradually up to 95%. In cultures grown on gas-permeable supports, pericellular and gas PO2 values were very similar, with increased Epo production at lowered PO2. In agreement with mathematical models, our experimental data make PO2 measurements desirable for studies of O2-dependent biological functions in cell cultures.
Metal Ion Release from New and Recycled Stainless Steel Brackets

Abstract:As orthodontic appliances can corrode with time in the oral environment, the aim of this study was to compare the release of metal ions from new and recycled brackets immersed in buffers of different pH values over a 48 week period. To simulate commercial recycling, the stainless steel brackets were divided into two groups: new and recycled. The bases of the latter were coated with adhesive and the brackets were heat treated before being immersed in the test solution for 48 weeks. The release of nickel, chromium, iron, copper, cobalt and manganese ions was analysed by atomic absorption spectrophotometry. Differences were compared using one-way analysis of variance. The results showed that recycled brackets released more ions than new brackets (P < 0.05). Brackets immersed in solutions of pH 4 released more ions than those immersed in solutions of pH 7, and the total amount of ions released increased with time over the 48 week period (P < 0.05). This study demonstrates that both new and recycled brackets will corrode in the oral environment. To avoid clinical side-effects, metal brackets should be made more resistant to corrosion, and recycled brackets should not be used.

Abstract:This paper reports the annealing process influence on interfacial electrical properties, dopant effects on silicide formation, and dopant redistribution during silicidation in self-aligned NiSi technology. The reverse leakage current results of NiSi/n-Si Schottky diodes show that two-step rapid thermal process (RTP) significantly improves the NiSi/Si area-contact characteristics in comparison with one-step RTP, and low temperature in RTPI is beneficial to the final Ni-silicide/Si contact properties. Both structural and electrical characterization shows substantially different Ni-silicidation behaviors on heavily-doped n and p Si substrates at low temperature (300/spl deg/C). The larger grain size of Ni/sub 2/Si formed on heavily As-doped Si is responsible for the lower resistivity, comparing with Ni:Si formed on heavily B-doped Si. Ni/Si reaction on highly doped Si substrates also results in significant dopant segregation at the silicide/Si interface and pile up in void-layer formed just underneath the silicide surface.

Abstract:Abstract Background The purpose of this study was to assess the surface characterization and frictional resistance between stainless steel brackets and two types of orthodontic wires made of stainless steel and nickel-titanium alloys after immersion in a chlorhexidine-containing prophylactic agent. Methods Stainless steel orthodontic brackets with either stainless steel (SS) or heat-activated nickel-titanium (Ni-Ti) wires were immersed in a 0.2% chlorhexidine and an artificial saliva environment for 1.5 h. The frictional force was measured on a universal testing machine with a crosshead speed of 10 mm/min over a 5-mm of archwire. The surface morphology of bracket slots and surface roughness of archwires after immersion in chlorhexidine were also characterized using a scanning electron microscope (SEM) and an atomic force microscope (AFM), respectively. Results There was no significant difference in the frictional resistance values between SS and Ni-Ti wires immersed in either chlorhexidine or artificial saliva. The frictional resistance values for the SS and Ni-Ti wires immersed in 0.2% chlorhexidine solution were not significantly different from that inartificial saliva. No significant difference in the average surface roughness for both wires before (as-received) and after immersion in either chlorhexidine or artificial saliva was observed. Conclusions One-and-half-hour immersion in 0.2% chlorhexidine mouthrinse did not have significant influence on the archwires surface roughness or the frictional resistance between stainless steel orthodontic brackets and archwires made of SS and Ni-Ti. Based on these results, chlorhexidine-containing mouthrinses may be prescribed as non-destructive prophylactic agents on materials evaluated in the present study for orthodontic patients.

Abstract:Plaque control is especially important during orthodontic treatment because areas of the teeth near brackets and wires are difficult to clean with a toothbrush, resulting in debris buildup of food or dental plaque, thus causing caries and periodontal disease. The objective of this study was to evaluate the antimicrobial properties of silver nanoparticles (AgNPs), titanium dioxide nanoparticles (TiO2NPs), and silver/titanium dioxide nanoparticles (Ag/TiO2NPs), synthesized on the surface of α-alumina ceramic brackets. The AgNPs and TiO2NPs were synthesized by a simple chemical method, and these were characterized by XRD, SEM, and XPS TEM; the antimicrobial activity was tested against Staphylococcus aureus and Escherichia coli by diffusion test. The results of this study demonstrated that by this simple chemical method, silver and titanium dioxide nanoparticles can be synthesized on the surface of α-alumina esthetic brackets, and these NPs possess good antimicrobial activity and the possibility of reducing dental caries, periodontal disease, and white spot generated during orthodontic treatment.

Abstract:In orthodontics, the surface roughness and corrosion behavior due to metal ion releases in orthodontic brackets and archwires affect the effectiveness of tooth movement. Atomic force microscopy (AFM) can provide three-dimensional information measurements which are used to evaluate the quantitative analysis of the surface changes of orthodontic brackets. Stainless steel orthodontic brackets and wires can release metal ions which lead to corrosion when exposed to various types of mouthwash.Aim: The main aim of this study is to compare and evaluate the surface changes from conventional metal brackets and self-ligating metal brackets after immersion in three different mouthwashes.Materials and Methods: Sixteen conventional metal brackets and sixteen metal self-ligating brackets were evenly grouped and immersed in deionized water, chlorhexidine mouthwash, herbal mouthwash, and betadine mouthwash for seven days and the surface changes were evaluated using nondestructive AFM.Results: The surface roughness was evaluated at 30 μm and the results showed that more surface roughness was observed P < 0.05 in betadine mouthwash groups and followed by herbal mouthwash and least in chlorhexidine mouthwash.Conclusion: Based on the AFM results, the surface changes were more pronounced in both groups which were immersed in betadine and herbal mouthwash than others and the choice of mouthwash should be considered before prescribing to the patients.

Abstract:Orthodontic treatment improves dental aesthetics. However, the prolonged duration of treatment and patient's oral hygiene results in more significant biofilm buildup and boosting concentrations of acidogenic bacteria, resulting in enamel demineralization. Plasma treatment technology has developed an interest in enhancing appliances' surface properties. The present study aimed to investigate cytotoxicity and antibacterial activity represented by anti-adhesion and anti-biofilm formation of Argon plasma treatment on orthodontic brackets against Streptococcus mutans and Lactobacillus acidophilus and evaluation the effect of change in plasma exposure duration on these properties in vitro. The study included three groups, group 1: untreated stainless steel brackets, group 2: brackets treated for 15 min, and group 3: brackets treated for 30 min. S.mutans and L.acidophilus were isolated from patients undergoing fixed orthodontic treatment. Microbiological tests included adhesion assay using plate counting method and biofilm formation assay using ELISA plate reader. S. mutans& L. acidophilus were cultivated in three groups, and their adherence and biofilm formation were measured and compared. The biocompatibility of treated stainless steel brackets was evaluated by MTT assay. Results showed that groups 2 & 3 had better anti-adhesion and anti-biofilm activity when compared with group 1, with a significant difference between group 1 &group 3, in which group 3 recorded the lowest biofilm formation and the highest anti- anti-A adhesion activity. MTT showed that groups 2 and 3 exhibited more than 80% cell viability after 24, 48, and 72 hours. Plasma surface treatment of metal brackets possesses antibacterial activity against S.mutans and L.acidophilus; alteration exposure time impacts the development of these properties. Keywords. Composition and in vitro corrosion of orthodontic appliances

Abstract:The high incidence of nickel allergy and the increasing use of nickel-containing dental biomaterials has been of growing concern. The purpose of this investigation was to analyze different types of alloys used in orthodontics, and to study whether nickel and chromium will be released from these alloys when stored in physiologic saline. Face-bows, brackets, molar bands, and arch wires were analyzed. Most of the different parts in the face-bows, brackets, and molar bands were similar to conventional 18/8 stainless steel. Except the wires, most appliances included a variable amount of silver solder, the greatest in face-bows. After 14 days in 0.9% sodium chloride (NaCl), the largest amount of nickel and chromium were leached out from the face-bows and the least amount from the arch wires. Soldered stainless steel face-bows seemed to be very susceptible to corrosion. The release of nickel seemed to be related to both the composition and the method of manufacture of the appliances, but the release was not proportional to the nickel content.

Abstract:Introduction This study aimed to investigate the surface free energy and surface roughness (SR) of metallic alloys under the influence of acid solutions. Methods The experiment involved the use of 270 rectangular wire samples measuring 0.019 × 0.025-in. These samples were sourced from 3 different commercial brands: Dentsply GAC, American Orthodontics, and Orthoclassic. This in vitro study categorized the samples into 3 groups based on the solutions employed: deionized water, citric acid, and phosphoric acid. Each group consisted of 90 samples, with 30 samples representing each type of alloy—stainless steel, nickel-titanium, and titanium molybdenum alloy (TMA). The wire segments were immersed in their respective solutions for 72 hours at a controlled temperature of 37°C, with continuous orbital agitation at 130 rpm. After the immersion period, the study analyzed both surface free energy and SR. The mean values obtained were subjected to an analysis of variance at a significance level of 5%. Results All alloys displayed hydrophobic behavior, as indicated by interaction free energy values <0. In acidic environments (phosphoric acid and citric acid), significant differences were observed among different brands and alloys, affecting surface energy and interaction free energy. Variations in SR among metallic alloys included steel with the lowest SR variations, followed by nickel-titanium and TMA. Notably, the TMA alloy stood out with significantly higher surface energy compared with that of the other alloys ( P <0.001). Conclusions In this study, all examined alloys demonstrated a hydrophobic nature, suggesting a limited attraction to water. Notably, TMA exhibited the least hydrophobic behavior among the alloys studied. However, when exposed to citric acid, TMA displayed the most substantial alterations in its surface properties. These results underscored the significance of accounting for the distinctive properties of each alloy and their responses to diverse challenges, such as exposure to acidic solutions, during the selection of orthodontic wires for orthodontics treatment.

Effect of Fluoride Content of Mouthwashes on the Metallic Ion Release in Different Orthodontics Archwires

Abstract:Metal ion release studies were carried out on three of the most commonly used orthodontic wires in the clinic: austenitic stainless steel, Ti-Mo, and superelastic NiTi, using three mouthwashes with different fluoride concentrations: 130, 200, and 380 ppm. Immersions were carried out in these mouthwashes at 37 °C for 1, 4, 7, and 14 days, and the ions released were determined by inductively coupled plasma-mass spectrometry (ICP-MS). All wires were observed by scanning electron microscopy (SEM). The results showed a moderate ion release in the stainless steel wires, with nickel and chromium values of 500 and 1000 ppb in the worst conditions for the wires: concentrations of 380 ppm fluoride and 14 days of immersion. However, in the Ti-Mo and NiTi alloys, an abrupt change in release was observed when the samples were immersed in 380 ppm fluoride concentrations. Titanium releases in Ti-Mo wires reached 200,000 ppb, creating numerous pits on the surface. Under the same conditions, the release of Ni and Ti ions from the superelastic wires also exceeded 220,000 ppb and 180,000 ppb, respectively. This release of ions causes variations in the chemical composition of the wires, causing the appearance of martensite plates in the austenitic matrix after 4 days of immersion. This fact causes it to lose its superelastic properties at a temperature of 37 °C. In the case of immersion in 380 ppm mouthwashes for more than 7 days, rich-nickel precipitates can be seen. These embrittle the wire and lose all tooth-correcting properties. It should be noted that the release of Ni ions can cause hypersensitivity in patients, particularly women. The results indicate that the use of mouthwashes with a high content of fluoride should not be recommended with orthodontic archwires.

Abstract:Aim: To evaluate the effects of two preprocedural mouthrinses, hydrogen peroxide (H2O2) and povidone iodine (PI) on the surface characteristics and mechanical properties of nickel-titanium (NiTi) and stainless steel (SS) orthodontic archwires. Materials and methods: Five wire specimens were used, each (0.016" NiTi, 0.016" SS wires, 0.016 × 0.022" NiTi and 0.016 × 0.022" SS wires) specimen was cut into 30 mm lengths and immersed in 9% of artificial saliva and 91% of two preprocedural mouthrinse solutions: 1.5% hydrogen peroxide mouthwash, 0.2% povidone-iodine mouthwash, and distilled water (control group) for 90 minutes and incubated at 37°C. The wire specimens were then subjected to a three-point bending test for mechanical testing and viewed under a scanning electron microscope (SEM) to evaluate their surface characteristics. The collected data were analyzed using one-way analysis of variance (ANOVA) and Bonferroni post hoc test. Results: The results showed a significant increase in the flexural modulus (E) of Nitinol wires in povidone-iodine gargle (p < 0.05) and a significant increase in the E of stainless steel wires in hydrogen peroxide mouthwash (p < 0.05). Analysis using SEM showed varying qualitative surface changes in the form of corrosion, voids, and ridges on the wires after exposure to both the mouthwashes. Conclusion: Though there were significant changes in the flexural modulus of archwires for both the mouthwashes, hydrogen peroxide did not show a significant difference in the E of wires at most of the deflection intervals when compared with the other two solutions, hence, could be used in orthodontic patients as an effective preprocedural mouthrinse. Clinical significance: Preprocedural mouthrinses can cause surface irregularities on the wires which in turn lead to an increase in friction at the bracket-wire interface, thereby disrupting effective tooth movement and extending the orthodontic treatment time.

Air-powder polishing on self-ligating brackets after clinical use: effects on debris levels

Abstract:Introduction:: Debris buildup on brackets and arch surfaces is one of the main factors that can influence the intensity of friction between bracket and orthodontic wire. Objective:: This study sought to evaluate the effect of air-powder polishing cleaning on debris levels of self-ligating ceramic brackets at the end of orthodontic treatment, compared to the behavior of conventional brackets. Methods:: Debris levels were evaluated in metal conventional orthodontic brackets (n = 42) and ceramic self-ligating brackets (n = 42) on canines and premolars, arranged in pairs. There were brackets with and without air-powder polishing. At the end of orthodontic treatment, a hemiarch served as control and the contralateral hemiarch underwent prophylaxis with air-powder polishing. Debris buildup in bracket slots was assessed through images, and Wilcoxon test was used to analyze the results. Results:: The median debris levels were statistically lower in the conventional metal brackets compared to self-ligating ones (p = 0.02), regarding brackets not submitted to air-powder polishing. Polishing significantly reduced debris buildup to zero in both systems, without differences between groups. Conclusions:: Ceramic self-ligating brackets have a higher debris buildup in comparison to conventional metal brackets in vivo, but prophylaxis with sodium bicarbonate jet was effective in reducing debris levels in self-ligating and also in conventional brackets.
