Exclusion criteria: Patients undergone TKR with comorbid; diabetes, congestive heart disease, hypertension, neuropathological diseases & renal disorders.& unwilling to participate. Results: The mean age of participants was 44.32 (S.D=3.5) Mean BMI was 29.4 (S.D=3.2). males were 16 while 42 females went under surgery. There was no association seen between BMI and post-op complications Average follow-up was 98 days (58-144). Co-relation between TKR and patellar resurfacing, there was no variation. Among fifty-eight patients patellar fracture, patellar matlracking, aseptic loosening,. Patellar osteonecrosis, polythene wear, Dislocation following total knee arthroplasty, and other hardware mal-function, Overstuffing of the PFJ in postoperative TKA was seen among three people 5.2% of individuals Conclusion: To reduce the danger of additional difficulties, patellar resurfacing must be performed with a high level of accuracy and precision since it is just as crucial as tibiofemoral replacement
Outcomes and Survivorships of Total Knee Arthroplasty Comparing Resurfaced and Unresurfaced Patellae

Abstract:Background: The decision to resurface the patella in total knee arthroplasty (TKA) is controversial. While there is some consensus that leaving the patella unresurfaced increases risk of reoperation, there is conflicting evidence about patient reported outcomes or indications for resurfacing. This study sought to determine recent rates of patellar resurfacing, examine factors affecting rates of resurfacing, and analyze the associations between patellar resurfacing and both revision rates and patient-reported outcome measures (PROMs). Methods: The American Joint Replacement Registry was used to identify primary TKAs performed for osteoarthritis between 2012 and 2021. Cases were classified as resurfaced patella and unresurfaced patella (URP). Outcomes analyzed included trends in patellar resurfacing, factors influencing rate of resurfacing, revision rates, operative time, and 2 PROMs. Results: Rates of patellar resurfacing decreased and rates of URP increased significantly faster in ambulatory surgery centers and among high volume surgeons. Operative time was significantly lower in URP versus resurfaced patella (88.15 versus 89.90 minutes). The URP were significantly more likely to require revision surgery (odds ratio = 1.206 (1.078, 1.35), P = .0011). There was no significant difference in the likelihood of achieving the minimal clinically important difference in PROMs between resurfaced and nonresurfaced TKAs at 1 year (odds ratio = 1.060 (0.710, 1.581), P = .7755). Conclusion: There was no difference in minimal clinically important difference between resurfaced and URPs and no clinically relevant difference in operative time. However, URPs were more likely to require revision surgery. Therefore, the decision not to resurface should be made carefully considering the known risk of revision and the uncertain benefit.
Early Improvement in Postoperative Clinical Outcomes without Patellar Resurfacing in Patella-Friendly Design of Medial Pivot TKA

Abstract:There is continued debate about the efficacy and indications for patellar resurfacing in total knee arthroplasty (TKA), especially with the emergence of patella-friendly designs. This study aimed to compare the postoperative outcomes in patients undergoing TKA with or without patellar resurfacing using the same implant design. This is a retrospective cohort study of patients who underwent TKA including those with patellar resurfacing (PR group) and those without (NPR group). Demographic data included age, gender, side of surgery, operative time, and body mass index (BMI). Outcomes included preoperative, 2-week, 6-week, and 1-year postoperative Knee Injury and Osteoarthritis Outcome Score and Joint Replacement (KOOS, JR) values along with knee range of motion (ROM). Postoperative complications were recorded. The power analysis with a large effect size indicated that a minimum sample size of 54 was required for Student's t-test and 34 for the paired t-test. A total of 90 medial pivot (MP) TKA were included in this study. There were 30 knees in the PR group and 60 in the NPR group. There was no significant difference between the groups for all demographic data, preoperative and postoperative ROM, and KOOS, JR values at all time points (p > 0.05 for all variables). The KOOS, JR significantly improved in the NPR groups at 2 weeks, 6 weeks, and 1 year postoperatively when compared with the preoperative score and at 6 weeks and 1 year postoperatively in the PR group (p < 0.01). No revisions related to the patellofemoral joint were observed in patients initially undergoing patellar resurfacing. One patient in the NPR group required secondary patellar resurfacing. The patella-friendly MP TKA yielded favorable postoperative outcomes, with or without patellar resurfacing. Improvements in KOOS, JR were observed earlier in the NPR group when compared with the PR group, suggesting that patellar resurfacing may not always be necessary for modern TKA designs. Abstract:The benefit of patellar resurfacing in total knee arthroplasty (TKA) remains uncertain, with conflicting evidence regarding associated revision rates and clinical outcomes. Although initial studies have reported higher revision rates associated with unresurfaced patellae, recent evidence questions the necessity of routine patellar resurfacing. This study aimed to evaluate the risk of revision following TKA performed with and without patellar resurfacing using data from the American Joint Replacement Registry (AJRR). The AJRR was queried for all patients aged 65 years and older undergoing elective TKA between January 2012 and March 2020 with a minimum 2-year follow-up. Cases were linked using supplemental Centers for Medicare and Medicaid data. Cases with hybrid fixation, highly constrained implants, and revision components were excluded. Patients were categorized into two groups: those with a resurfaced patella and those without. Cumulative incidence function (CIF) curves and cause-specific Cox models were utilized to assess all-cause revision risk, adjusting for sex, age, femoral design (cruciate retaining vs. posterior stabilized), fixation type (cemented vs. cementless), and Charlson Comorbidity Index. Of the 390,304 TKAs with minimum 2-year follow-up in our cohort, 22,829 had no patellar resurfacing performed. Adjusted hazard ratios (HRs) revealed no significant difference in all-cause revision (HR = 0.96, 95% confidence interval [CI]: 0.81–1.13, p = 0.656), revision for mechanical loosening (HR = 1.61 [0.88, 2.93], p = 0.122), or revision for infection (HR = 1.02 [0.79, 1.33], p = 0.860) associated with patellar resurfacing status. Our study found that patients with an unresurfaced patella do not face an increased short-term revision risk following TKA. These findings challenge the necessity of routine patellar resurfacing and underscore the importance of considering other factors, such as femoral design, patient comorbidities, and implant-related variables in revision risk stratification. Abstract:Background Leaving the patella unresurfaced in total knee arthroplasty (TKA) has increased significantly over the past decade in the United States, likely due to modern patella-friendly implants, complications with resurfacing, and the knowledge that historical studies were scientifically confounded. This study evaluated revision-free survivorship out to 8.5 years in a cohort of contemporary primary TKAs with patella-friendly femoral components and unresurfaced patellae. Methods A total of 1,053 consecutive primary TKAs with unresurfaced patellae were retrospectively reviewed. A selective patellar non-resurfacing protocol was used for all cases. Kaplan-Meier survivorship estimates were calculated based on patellar revision and the latest follow-up. An aggressive lateral patellar facetectomy was performed in 78% (823/1,053) of cases. The cohort was 62% women and 43% American Society of Anesthesiologists' physical status classification I or II with a mean age and body mass index of 65 years (range, 35 to 94) and 35 kg/m 2 (range, 18 to 65). Results A total of 4 (0.4%, 4/1,053) unresurfaced patellae were revised. Three were resurfaced as part of other procedures: two for global instability and one for aseptic loosening at a mean of 1.6 years; and one patella was resurfaced by an outside surgeon for unexplained pain. The all-cause revision-free survivorship estimate specifically related to the patella was 98.9% (95% confidence interval, 98 to 100) out to 8.5 years. No significant difference in survivorship was related to patellae with or without a lateral patellar facetectomy (99.5 versus 98.1%, P ≥0.191); however, 3 of 4 patellar revisions occurred in TKAs without a lateral patellar facetectomy ( P =0.035). Conclusion The results of this study demonstrate excellent revision-free survivorship related to unresurfaced patellae, particularly when a lateral facetectomy was performed. These early to mid-term results using modern patella-friendly femoral components are promising and comparable to resurfaced patellae in the literature.

Abstract:Total knee arthroplasty (TKA) remains a lifesaving procedure for advanced gonarthrosis. However, patella resurfacing (PR) in TKA remains a controversial procedure, leading to extensive discussions amongst orthopedic surgeons, regarding its indications and results. Based on these premises, we present a clinical case of a 70-year-old Caucasian woman admitted for pain, swelling and limitation of left knee joint mobility. Her medical history records an Ahlback stage IV gonarthrosis with simultaneous bilateral TKA surgery performed in different hospital, when two NexGen cemented total prostheses were implanted with patellar resurfacing being performed only on the right side. Our clinical (American Knee Society Score, Lonner and Feller scales) and radiological evaluations (CT scan and Xray) revealed left patellar arthrosis and a slight lateral subluxation of the patella. The chosen treatment plan was revision surgery for PR and patellar prosthesis with a cemented patellar component, cross-linked polyethylene, no 32 NexGen model with 8.5 mm thickness. The immediate and distant postoperative evolution was favorable. Extensive literature review shows that, at present, PR remains at surgeon`s discretion mainly based on his previous results. Therefore, we believe there is an imperative need to develop high quality studies based on accurate scientific evidence to universally establish valid guidelines for PR in TKA.

Comparison of Anterior Knee Pain after Total Knee Arthroplasty Using Genesis Ⅱ and PFC Knee Prostheses

Abstract:Objective To explore relationship between anterior knee pain after total knee arthroplasty and patella resurfacing and prosthesis design,and to evaluate which femoral prosthesis is more friendly with patella:Genesis Ⅱ or PFC knee prostheses.Methods Data of 145 patients (145 knees) who had undergone primary total knee arthroplasty for treating osteoarthritis were retrospectively analyzed.Posterior-cruciate-substituting total knee prostheses were used in all patients.Among 74 patients who had undergone patella resurfacing,Genesis Ⅱ knee prosthesis was used in 32 patients and PFC knee prosthesis in 42 patients.Among 71 patients who had not undergone patella resurfacing,Genesis Ⅱ knee prosthesis was used in 38 patients and PFC knee prosthesis in 33 patients.The Hospital for Special Surgery(HSS) score,patella score,patellar function score,range of motion of knee,anterior knee pain scale,and patient satisfaction were used to evaluate clinical outcomes.At the same time,the imaging results were evaluated by X-rays.Results 144 patients were successfully followed up for 21 to 43 months (average,33 months).There was no significant difference in incidence of anterior knee pain between the group with patella resurfacing and the group without patella resurfacing,while a significant difference was found between patients receiving Genesis Ⅱ knee prosthesis and those receiving PFC knee prosthesis.Four patients who had undergone total knee arthroplasty using PFC prosthesis underwent reoperation.There were no significant differences in postoperative HSS score,range of motion of knee and patient satisfaction between the Genesis Ⅱ group and the PFC group,while there were significant differences in patella score and patellar function score between them.Conclusion Postoperative anterior knee pain was related to the prosthesis design,rather than to the patella resurfacing.Genesis Ⅱ knee prosthesis was more friendly with patella than PFC knee prosthesis.
Colonic lipoma intussusception: a case report.

Abstract:Lipomas are the most common mesenchymal benign tumor of the colon. They are usually asymptomatic but may cause bleeding, obstruction, intussusception, or abdominal pain. The diagnosis can be made by colonoscopy, barium enema examination, and by computed tomography (CT). We have reported a case of large submucosal lipoma of the ascending colon with intestinal obstruction. At laparotomy there was an intussusception descending colon, and subtotal colectomy was performed.
Global Trends in Patellar Resurfacing From 2004 to 2022

Abstract:Background: International rates of patellar resurfacing in primary total knee arthroplasty (TKA) are highly variable. This study sought to determine how trends in patellar resurfacing rates have changed between 2004 and 2022. In addition, we investigated how modern rates of revision have varied between resurfaced and unresurfaced patellae in primary TKA among national joint registries. Methods: Data between 2004 and 2022 was obtained either from the publicly available joint registry annual reports, a literature review, or via direct correspondence with registry personnel in Sweden, New Zealand, Australia, the United States, Norway, the United Kingdom, the Netherlands, Switzerland, Canada, and India. Only English language national joint registries or data via direct correspondence with registry administrators were utilized. Additionally, the 10-year cumulative risk of revision TKA with and without patellar resurfacing was pulled from those registries that had this data available. Results: There were persistent differences in the rates of patellar resurfacing among countries. Australia documented a 40% increase in patellar resurfacing rates, while other countries demonstrated modest or little change in resurfacing rates. This may indicate that surgeons are making the decision to resurface based on national TKA revision rates. The average rates of patellar resurfacing in primary TKA ranged from 4% in Sweden to 94% in the United States. Canada, the United States, Australia, and Switzerland documented a lower risk of revision when the patella was resurfaced, while Sweden, conversely, showed a higher risk of revision with resurfacing. Conclusions: Rates of patellar resurfacing in primary TKA were highly variable among countries, as were rates of change over time. It appears that the optimal patellar resurfacing strategy may depend mostly on unique patient factors and surgeon expertise. Future studies should attempt to elucidate the individual patient characteristics that contribute to increased risks of revision or anterior knee pain to determine who will most benefit from patellar resurfacing in primary TKA.

Abstract:Introduction Randomized controlled trials (RCTs) evaluating patellar resurfacing in total knee arthroplasty (TKA) have conflicting findings, with some reporting its efficacy and others demonstrating no clinical significance. The fragility index (FI), reverse fragility index (rFI), and fragility quotient (FQ) evaluated statistical fragility of outcomes in RCTs evaluating patellar resurfacing in TKA. Methods The PubMed, Embase, and MEDLINE databases were systematically searched for RCTs (January 1, 2000, to August 1, 2023) assessing patellar resurfacing in TKA. Of 226 RCTs screened, 19 studies were included for analysis. We calculated FI and rFI, which represent the number of outcome event reversals required to alter statistical significance for significant and non-significant outcomes, respectively. The outcome categories of interest included anterior knee pain, complications/ adverse events, crepitus, reoperation, patient satisfaction, and clinical improvement. The FQ was determined by dividing the FI by the study sample size. Results Across 46 outcomes, the median FI was 5 (Interquartile Range (IQR) 3 to 8) with a median FQ of 0.041 (IQR 0.025 to 0.063). There were nine outcomes that were statistically significant, with a median FI of 3 (IQR 2 to 8) and a FQ of 0.011 (IQR 0.0044 to 0.039). There were 37 outcomes that were non-significant, with a median rFI of 5 (IQR 4 to 7) and FQ of 0.043 (IQR 0.031 to 0.062). Notably, in 47.8% of all outcomes, the number of patients lost-to-follow-up was greater than the outcome's respective FI or rFI. Outcomes regarding patient satisfaction (FI 4.5) and anterior knee pain (FI 5) were most fragile. Conclusion The outcomes of interest regarding patellar resurfacing from RCTs are statistically fragile, particularly significant outcomes and patient satisfaction outcomes. Data surrounding patellar resurfacing remains inconclusive, and combining P -values with FI/FQ metrics may aid in interpreting patellar resurfacing findings. Future studies may mitigate fragility by obtaining higher follow-up rates and sample sizes.

Patellar tilt does not affect patient reported outcomes after modern total knee arthroplasty

Abstract:Background: The focus of patella maltracking after total knee arthroplasty (TKA) has traditionally been on patella polyethylene damage and failure mechanisms rather than functional outcomes. The purpose of this study was to evaluate the effect of patellar tilt on patient reported outcomes (PROMS) after primary TKA performed with patellar resurfacing. Methods: A retrospective review using a single implant design was performed. Patella tilt was radiographically measured according to a standardized protocol. PROMS related to pain, function, and satisfaction were evaluated preoperatively and at minimum 1-year follow-up. Results: A total of 468 TKAs were included for analysis. Mean age and BMI were 64 years and 35 kg/m2; respectively while 63% of patients were female. The median follow-up period was 12.7 months. Overall, patellar tilt was corrected from a median of 5.0 degrees preoperatively to a median of 3.0 degrees postoperatively. Preoperative, postoperative, and the change in patellar tilt had no significant effects on PROMS at minimum 1-year follow-up (p ≥ 0.092). Satisfaction in knee function while getting out of bed was higher for patients with approximately the same patellar tilt before and after TKA compared to patients with an increase in lateral patellar tilt (95% vs 80%, p = 0.025). Conclusions: The range of patellar tilt studied in this cohort had little to no effect on PROMS. However, suboptimal patellar tracking may potentiate edge loading of the polyethylene and contribute to implant damage in the long-term. These results are helpful to focus efforts on the tibiofemoral articulation as the predominant determinant of patient outcomes.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11_suppl4, November 2024. ObjectivesResurfacing the patella in total knee arthroplasty(TKA) is a controversial procedure. Although there are some advantages to replace the patella, there is a conservative trend among surgeons who prefer to retain it, leading to a faster procedure, preserving bone stock, with fewer complications and similar outcomes. Traditional procedure consists in removing the osteophytes and performing a denervation around the patella. A different approach is to perform patelloplasty, defined as reshaping the patella for optimal tracking in the trochlea. Objective:to compare traditional preservation and patelloplasty in primary TKA.Methods25patients underwent bilateral non-simultaneous TKA with the preservation of the native patella, and the same implant in both knees. On one side, patellar denervation and osteophytes resection was performed, and on the other one, patelloplasty. All implants were patellar "friendly" throclea design. 19 patients were evaluated with the average age of 74 years(61 to 89), BMI of 29.7, and the follow up of 4.3 years(2.9 to 7.8) in the traditional patella preservation group and 1.7 years(0.6 to 2.8) in the patelloplasty. The patients were asked which side had the most satisfactory result, in their opinion. Range of motion(ROM), VAS, and KSS, Forgotten knee, KOOS and LEE scores were also evaluated.Results8(42%) of the patients didn't have a preference side, 7(37%) preferred traditional preservation knee, and 4(21%) the patelloplasty. Regarding pain, VAS was 1.2 in the traditional and 2 in the patellopalsty. The ROM was similar, showing 126.4 degrees in the traditional side and 126 in the patelloplasty. The differences in KSS(score and function), Forgotten, KOOS and LEE scores between both groups, were not statistically significant.ConclusionPatelloplasty is a safe procedure, with few complications, relieving pain, with functional outcomes similar to those of the traditional procedure.

Abstract:Purpose A cutting guide technique for patella resurfacing in total knee arthroplasty was expected to result in less patellofemoral syndromes. The aim of this study was to identify differences in the patellofemoral function, clinical outcomes, and radiographic parameters between the freehand and cutting guide patellar resection techniques in patients undergoing total knee arthroplasty. Methods A prospective randomized controlled trial was conducted. The study was registered in a public trials registry (International Standard Randomized Trial No. NCT02268097). One-hundred total knee arthroplasties in 100 patients were randomly allocated into one of the two groups, and their results were followed for a mean of 28 months (range, 18 to 38 months) in a double-blind (both patient and evaluator), prospective study. Evaluation was performed by an independent observer using patellofemoral functional capacity, the Knee Society clinical rating system, the Western Ontario and McMaster Universities Osteoarthritis Indices, and radiographic examination. Results In total, 14% of the patients were lost to follow-up. There was no difference in the incidence of anterior knee pain between the two groups. No patients received or required revisions. There was a significant difference in the outliers of lateral patellar tilt between the freehand and cutting guide groups (> 10°) ( p = 0.036); however, the mean value of lateral patellar tilt did not differ significantly. There were no differences between groups with respect to the 30 seconds stair climbing test, complications, the Knee Society clinical rating system, the Western Ontario and McMaster Universities Osteoarthritis Indices, patient satisfaction, physical examination, hip-knee-ankle angle, lateral patellar displacement, or the Insall-Salvati ratio. Meanwhile, gender, age, weight, height, body mass index, pre-operative Knee Society scores, and pre-operative range of motion were not found to be related to the development of anterior knee pain. Conclusions Cutting guide technique group did not yield lower incidence of anterior knee pain. More outliers of lateral patellar tilt were observed in the freehand technique group. Overall, all patients in both groups had identical results in terms of patellofemoral functional capacity, clinical outcomes, and other radiographic results.
Fragility analysis and systematic review of patellar resurfacing versus non-patellar resurfacing in total knee arthroplasty

Abstract:Introduction: Fragility analysis is a method of further characterising the robustness of statistical outcomes. This study evaluates the statistical fragility of randomised controlled trials (RCTs) comparing patellar resurfacing versus non-patellar surfacing in total knee arthroplasty (TKA). Methods: PubMed, MEDLINE and EMBASE were searched for RCTs comparing outcomes in TKA based on patellar resurfacing. Fragility index (FI) and reverse FI (collectively, "FI") were calculated for dichotomous outcomes as the number of outcome reversals needed to change statistical significance. Fragility quotient (FQ) was calculated by dividing the FI by the sample size for that outcome. Median FI and FQ were calculated for each individual outcome and for the overall study. Subanalyses were performed to assess FI and FQ based on outcome type, statistical significance and loss to follow-up. Results: Twenty-one RCTs were included in the analysis, capturing 3910 subjects. The overall median FI was 5.0 (interquartile range, [IQR] 4.0-6.0), and the overall median FQ was 0.048 (IQR 0.022-0.065). The outcome of anterior knee pain has a median FI of 6.0 (IQR 4.0-6.0) and a median FQ of 0.057 (IQR 0.025-0.065). Only five (7%) outcomes were significant. The loss to follow-up was greater than the FI in 12 of 19 studies (63%) with available data. Conclusion: RCTs comparing patellar resurfacing in TKAs show significant statistical fragility; a few outcome reversals can alter findings. The majority of outcomes were nonsignificant, indicating that the choice to resurface the patella may not affect most clinical outcomes; however, clinical conclusions are limited by the statistical fragility of the analysed outcomes. Larger RCTs for this comparison are necessary, and we suggest adding FI and FQ to RCT reports with p values to improve the interpretability of results. Level of evidence: Level II.
Long-term follow-up and survivorship of single-radius, posterior-stabilized total knee arthroplasty

Abstract:Background: We sought to determine the 10-year survivorship of single-radius, posterior-stabilized total knee arthroplasty (TKA) in Asian patients. We also aimed to determine whether the long-term clinical and radiographic results differed between patients with and without patellar resurfacing. Methods: This retrospective study included 148 (115 patients) consecutive single-radius, posterior-stabilized TKAs. Ten-year survivorship analysis was performed using the Kaplan-Meier method with additional surgery for any reason as the end-point. Furthermore, long-term clinical and radiographic results of 109 knees (74%; 84 patients) with more than 10-year follow-up were analyzed. Ten-year survivorship and long-term outcomes after surgery were determined, and outcomes were compared between patients with and without patellar resurfacing. Results: The cumulative survival rate of the single-radius posterior-stabilized TKA of 148 knees was 97.7% (95% confidence interval, 93.1%-99.3%) at 10 years after surgery. Three knees required additional surgery during the 10-year follow-up because of one case of instability and two cases of periprosthetic infections. Mean postoperative Knee Society knee score and function score were 97 points and 75 points, respectively. There were no cases of aseptic loosening of the prosthesis, even though a non-progressive radiolucent line was found in 10 (9%) knees. There were no differences in postoperative scores and degree of patellar tilt and displacement between patients with and without patellar resurfacing. Conclusions: Single-radius, posterior-stabilized TKA showed satisfactory long-term clinical and radiographic outcomes in Asian patients regardless of patellar resurfacing, with comparable survivorship to that reported in westerners.
A Preliminary Report of Patellofemoral Arthroplasty in Isolated Patellofemoral Arthritis

Abstract:BACKGROUND:Isolated patellofemoral osteoarthritis is not uncommon. Surgical treatment of isolated patellofemoral arthritis remains controversial and poses a challenging treatment dilemma. The present study aimed to evaluate the short-term results of patellofemoral arthroplasty for patients with isolated patellofemoral osteoarthritis.METHODS:We analyzed 11 patellofemoral arthroplasties performed from March 2006 to September 2009 in 11 patients with isolated patellofemoral arthritis. The patients comprised 2 males and 9 females with an average age of 53.7 years (range, 46 - 74 years). Standard weightbearing radiographs were taken in the anteroposterior, lateral, and 45° axial views. The knee pain and functional status were evaluated by the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scales and American Knee Society (AKS) scores. For comparison, 23 total knee arthroplasties in 23 patients with primary tibiofemoral osteoarthritis were matched according to age, gender, bilaterality and body mass index. The duration of follow-up was 23.7 months (range, 12 - 47 months).RESULTS:The majority of the 11 patients experienced improvement in their patellofemoral symptoms after patellofemoral arthroplasty. The WOMAC scores improved considerably by 7.4 points with respect to pain and by 5.2 points with respect to function. The AKS scores also improved considerably by 23.9 points with respect to pain and 44.3 points with respect to function. Although the clinical outcomes after patellofemoral arthroplasty were not better than those after total knee arthroplasty, patellofemoral arthroplasty exhibited advantages in the shorter operation time, lower blood loss and increased postoperative range of motion. At the latest follow-up, there was no clinical or radiographic evidence of patellofemoral maltracking, loosening or wear.CONCLUSIONS:On the basis of our experience in this relatively small series of patients with a short-term follow-up, patellofemoral arthroplasty is an effective treatment alternative to total knee arthroplasty in isolated patellofemoral arthritis. MRI and arthroscopy may contribute to define those patients with isolated patellofemoral degeneration.
Does Resurfacing the Patella Increase the Risk of Extensor Mechanism Injury within the First 2 Years after Total Knee Arthroplasty?

Abstract:Extensor mechanism injury (EMI) following total knee arthroplasty (TKA) is a potentially catastrophic complication and may lead to significant morbidity or need for revision reconstructive procedures. Patellar resurfacing (PR), while commonly performed during TKA, reduces overall patella bone stock and may increase the risk of EMI after TKA. The purpose of this study was to assess if PR in elderly patients raises the risk for subsequent EMI. The American Joint Replacement Registry (AJRR) was queried to identify Medicare patients ≥65 years old undergoing primary elective TKA for osteoarthritis between January 2012 and March 2020. Patient age, sex, and Charlson Comorbidity Index (CCI) were collected. Records were subsequently merged with Medicare claims records and evaluated for the occurrence of patella fracture, quadriceps tendon rupture, or patellar tendon rupture based on International Classification of Diseases 9 and 10 (ICD 9/10) diagnosis codes within 2 years of TKA. Patients were stratified based on whether PR occurred or not (NR). Logistic regression was used to determine the association between PR and EMI. A total of 453,828 TKA were eligible for inclusion and 428,644 (94.45%) underwent PR. The incidence of PR decreased from 96.06% in 2012 to 92.35% in 2022 ( p < 0.001). Patients undergoing PR were more often female (60.93 vs. 58.50%, p < 0.001) and had a lower mean CCI (3.09 [1.10] vs. 3.16 [1.20], p < 0.001). Odds for EMI did not differ based on whether PR was performed (odds ratio [OR]: 0.85, 95% confidence interval [CI]: 0.65–1.11, p = 0.2246). Increasing age (OR: 1.06, 95% CI: 1.05–1.07, p < 0.0001]) and CCI (OR: 1.06, 95% CI: 0.95–1.19, p = 0.0009) were associated with EMI. PR is commonly performed during TKA in the United States and was not found to increase odds for EMI within 2 years of TKA in patients ≥65 years old. Increased age and medical comorbidity were associated with higher odds for subsequent EMI. Abstract:Purpose: The aim of the present study was to report the approach of Greek surgeons regarding patellar management and provide the outcomes and the rate of the need for secondary patella resurfacing. Methods: Following the PRISMA guidelines, PubMed, Scopus, and The Cochrane Central Register of Controlled Trials (CENTRAL) databases were accessed in January 2024. Clinical studies evaluating the outcomes of patients undergoing primary TKA were considered eligible for this systematic review if the following predefined criteria were fulfilled: (1) published in English, (2) were conducted in Greece, and had a minimum of 2-year follow-up. The methodological quality and publication bias were assessed using the Modified Coleman Methodology Score (mCMS) and ROBIS tool, respectively. Data was presented in tables using absolute values from individual studies. Pooled data were presented as means, ranges, and percentages. Results: Six clinical studies with a total of 1084 TKAs were included. Four studies were retrospectives, and two studies were prospective. Patella was only resurfaced in 0.6% of total TKAs. TKA without patellar resurfaced, which resulted in an improvement in functional outcome. Among studies, the overall need for revision incidence was 7.6% (68 out of 892 TKAs). Additionally, Aseptic loosening (2.4%) was the most common reason for revision due to patellofemoral joint complications, followed by secondary patellar resurfacing (2.1%) and deep infection (1.3%). The mean mCMS demonstrated a fair methodological quality level, and the ROBIS toll a low risk of bias in all four domains. Conclusion: The available evidence supports that Greek surgeons mainly do not resurface the patella. However, patellar non-resurfacing yields good functional outcomes and presents relatively low revision rates for secondary patellar resurfacing. Level of evidence: IV.
