Abstract:With a novel Q-switching technique, 6-ns output pulses are produced from a single-mode Nd:YAG microchip laser pumped with 120 mW of cw optical power. Rate-equation analysis indicates that with a pump power of 500 mW, Q-switched Nd:YAG microchip lasers having subnanosecond pulses and peak powers of several kilowatts are feasible. Such miniature Q-switched lasers could find applications in range finding and nonlinear optics.
The influencing factors of infertility patients with pelvic adhesions and fallopian tube recanalization

Abstract:Objective To analyze the impact factors of infertility patients with pelvic adhesions,observed the effects of pelvic adhesions of fallopian tube recanalization.Methods Collected the clinical data of 386 cases of tubal obstruction infertility patients,divided into adhesions group and control group,adhesions group fallopian tube recanalization sepa ration of adhesions and laparoscopic surgery,the control group fallopian tube recanalization.Fallopian tube recanal ization was observed and pregnancy outcomes.Results Case of infertility >10 years were more,previous miscarriage,induced abortion,ectopic pregnancy,pelvic disease history and the uterine cavity operation history with higher propor tion(P<0.05).Miscarriage,ectopic pregnancy,pelvic disease history and the history of the uterine cavity operation infertility were main factors of pelvic adhesions(P<0.05).Percentage differences of different levels of adhesion ectopic pregnancy,pelvic disease,uterine operation were statistically significant,the frequency of ectopic pregnancy,the pelvic disease rate and uterine number of operations and pelvic adhesions were positively correlated(P<0.05),recanalization rate of adhesions group were lower than the control group(P<0.05),adhesion and control groups after 12 months of normal pregnancy,ectopic pregnancy,miscarriage rate,again blocking rate,the difference were statistically significant(P<0.05),differences of them among different degree of adhesion were statistically significant(P<0.05).Conclusion Miscarriage,ectopic pregnancy,pelvic disease history and the uterine cavity operation history are the main factors affecting of infertility patients with pelvic adhesions.Fallopian tube recanalization rate of pelvic adhesions and normal pregnancy rate after 12 months are low,risks of abortion,ectopic pregnancy,obstruction again are high.

Abstract:BACKGROUND:In patients undergoing peritoneal dialysis (PD), catheter dysfunction is a common complication. A misplaced catheter is one of the reasons contributing to its dysfunction. The present study aimed to describe a case of misplaced PD catheter with an unusual location of the catheter tip.CASE SUMMARY:A 61-year-old man undergoing PD for 4 years was investigated for progressive nausea and fatigue of 3 mo. Dialysis adequacy studies indicated inefficient dialysis. Imaging discovered that the PD catheter tip was mispositioned in the pelvic cavity with its tip outside the peritoneal cavity. Despite the dialysate accumulating outside the peritoneal cavity, the patient had not developed perineal or scrotal edema. The patient had experienced a sustainable prolonged dialysis efficacy in this case until the renal function deteriorated further in view of the poor dialysis outcome and worsening health condition. The patient was subsequently transitioned to hemodialysis.CONCLUSION:Proper placement of the catheter in the peritoneal cavity should always be confirmed and re-checked when necessary in patients undergoing PD to ensure dialytic adequacy.

Abstract:Abstract Background Peritoneal dialysis has been proven to be a safe and effective mode of renal replacement therapy for patients with end-stage renal disease. The usage of laparoscopic catheter placement technique was increased in recent years. But the advantages and disadvantages between the laparoscopic catheter placement technique and open laparotomy technique were still http://in controversy. The objective of this study is to access the operation-related data and complications of catheter placement for peritoneal dialysis (PD) patients, Then to determine the better method for catheter insertion. Methods We performed a systematic review and meta-analysis on published studies identified by the databases PubMed, EMBASE, Highwire, and the Cochrane Library. Analysis was performed using the statistical software Review Manager Version 5.0. Results We assessed the operation-related data and complications of four randomized controlled trials (RCTs) and ten observational studies. The available data showed that laparoscope prolonged the time for catheter insertion in PD patients, however, the two groups did not significantly differ in hospital stays, early and late complications, including infection, dialysate leaks, catheter migration, pericannular bleeding, blockage and hernia. Conclusions The data showed that Laparoscopic catheter placement had no superiority to open surgery. However, this treatment still needs to be confirmed in a large, multi-center, well-designed RCT.

Abstract:Recently, the sub-fertility of patients with adenomyosis, including infertility, abortion, premature delivery, stillbirth, fetal growth restriction, preeclampsia and the increase rate of cesarean section, has attracted much attention. Currently, adenomyomectomy is the priority option to treat adenomyosis patients with infertility following the failure of non-surgical conservative treatment of adenomyosis patients with infertility, such as medical therapy and high-intensity focused ultrasound. However, the clinical pregnancy success rate and complications after conservative surgical surgery for adenomyosis patients with infertility have always been controversial. Here, the difficulties, focal points and surgical indications of fertility-sparing surgery for patients with adenomyosis are put under the microscope, so as to provide reference for the treatment of adenomyosis patients with infertility.

Abstract:Background This study investigated the association of intra-abdominal adhesions with the risk of peritoneal dialysis (PD) catheter complications. Methods Individuals undergoing laparoscopic PD catheter insertion were prospectively enrolled from eight centers in Canada and the United States. Patients were grouped based on the presence of adhesions observed during catheter insertion. The primary outcome was the composite of PD never starting, termination of PD, or the need for an invasive procedure caused by flow restriction or abdominal pain. Results Seven hundred and fifty-eight individuals were enrolled, of whom 201 (27%) had adhesions during laparoscopic PD catheter insertion. The risk of the primary outcome occurred in 35 (17%) in the adhesion group compared with 58 (10%) in the no adhesion group (adjusted HR, 1.64; 95% confidence interval [CI], 1.05 to 2.55) within 6 months of insertion. Lower abdominal or pelvic adhesions had an adjusted HR of 1.80 (95% CI, 1.09 to 2.98) compared with the no adhesion group. Invasive procedures were required in 26 (13%) and 47 (8%) of the adhesion and no adhesion groups, respectively (unadjusted HR, 1.60: 95% CI, 1.04 to 2.47) within 6 months of insertion. The adjusted odds ratio for adhesions for women was 1.65 (95% CI, 1.12 to 2.41), for body mass index per 5 kg/m 2 was 1.16 (95% CI, 1.003 to 1.34), and for prior abdominal surgery was 8.34 (95% CI, 5.5 to 12.34). Common abnormalities found during invasive procedures included PD catheter tip migration, occlusion of the lumen with fibrin, omental wrapping, adherence to the bowel, and the development of new adhesions. Conclusions People with intra-abdominal adhesions undergoing PD catheter insertion were at higher risk for abdominal pain or flow restriction preventing PD from starting, PD termination, or requiring an invasive procedure. However, most patients, with or without adhesions, did not experience complications, and most complications did not lead to the termination of PD therapy.

Abstract:We aimed to introduce the puncture technique based on a novel device for preperitoneal tunneling in laparoscopic PD catheter placement and to evaluate the safety and efficacy of this technique. This novel device was used in our center from May 2016. We conducted a retrospective analysis of patients undergoing laparoscopic PD catheter placement. The traditional method was performed in 20 patients and the novel procedure in 18 patients. A straight Tenckhoff PD catheter was placed in all patients. No intraoperative complications were encountered in both groups. Compared the traditional technique group, the procedure based on the novel device had a shorter operative time (49.2 ± 11.8 vs 53.9 ± 12.5). One patient in the traditional tunneling group underwent catheter obstruction. There were no pericatheter leakage, exit site and subcutaneous infection, hernia and peritonitis in the early postoperative days. No mortality was observed in these patients. The 6-month survival rate of the catheter was 100%. Laparoscopic preperitoneal tunneling technique is an effective way to implant intra-abdominal catheter. Our method based on a novel puncture device for preperitoneal tunneling is safe and efficient.
Laparoscopic Vs Open Surgical Insertion of Peritoneal Dialysis Catheters: A Propensity Score-Matched Cohort Study

Abstract:The optimal peritoneal dialysis (PD) catheterization method remains under debated. To minimize catheter-related complications, we developed a novel modified low-position two-port laparoscopic technique, and evaluated the efficacy and safety of traditional open and advanced laparoscopic techniques. A total of 743 patients with uremia who underwent PD catheterization were retrospectively reviewed. For the placement of PD catheters, patients underwent low-position two-port laparoscopic surgery (Lap group) or traditional open surgery (open group). To minimize confounding bias, we performed a 1-to-2 propensity-score matching adjusted for the most clinically relevant potential confounders. The baseline characteristics, perioperative conditions, complications and outcomes were compared between the two groups. After propensity-score matching, a well-balanced cohort comprising 148 matched pairs were identified. In the matched analysis, patients in the Lap group showed significantly shorter operative time (43.0 ± 16.6 versus 81.6 ± 19.8 minutes, P < 0.001) and lower postoperative pain score within 24 hours (0.24 ± 0.62 versus 1.00 ± 1.41, P = 0.002). The incidence of catheter migration in the Lap group was significantly lower than those in the open group (0.7% versus 6.5%, P = 0.006), while other complications, such as catheter occlusion, omentum wrapping, dialysate leakage, bloody effluent, organ injury, hernia, hydrothorax, early tunnel infection and peritonitis, showed no difference (P > 0.05). The Lap group patients had a favorable catheter survival rate compared with group B (log rank, P = 0.032). Our low-position two-port laparoscopic technique is a safe, simple, and fast method that can minimize patient discomfort without additional cost and offers better catheter survival with minimal risk of migration.

Abstract:Peritoneal dialysis (PD), offering the benefits of stable hemodynamics, a low risk of bloodstream infection, preservation of residual renal function, and enhancement of quality of life, [ 1 ] has emerged as the preferred modality for renal replacement therapy in patients with end-stage renal disease. Percutaneous puncture catheterization, surgical catheterization, and laparoscopic catheterization are the most common methods for PD catheterization. Advancements in PD catheterization technology have led to a gradual decrease in the incidence of complications related to catheterization. However, these complications continue to be a primary cause of treatment failure in PD. The main complications include infection-related issues (exit site and tunnel infections) and mechanical problems (catheter displacement or occlusion, dialysate leakage, hernia, internal organ injury, and bleeding). [ 2 ] Hence, timely identification and correction of catheter-related complications are essential for enhancing the prognosis of PD patients.

A Case Report and Clinical Insights on Encapsulating Peritoneal Sclerosis: A Rare yet Critical Complication of Peritoneal Dialysis

Abstract:Peritoneal dialysis (PD) offers a valuable alternative to hemodialysis in the management of end-stage renal disease. While PD offers several advantages, such as improved patient autonomy and preservation of residual kidney functions. It has a wide spectrum of complications, which include mechanical ones such as catheter malfunction or migration, hernias and dialysate leak, or infectious complications, which can be limited to exit site and tunnel infections or extend interiorly to cause peritonitis. One detrimental long-term complication of PD is encapsulating peritoneal sclerosis (EPS), a rare condition characterized by formation of a fibrous cocoon around the bowel loops often initiated by chronic exposure to PD solutions. Other implicated factors include peritonitis, medications and systemic inflammatory conditions. Risk of EPS increases with the duration of PD, particularly after five years. Diagnosis of EPS is challenging and often delayed, given non-specific and wide spectrum of symptoms that may range from loss of appetite to frank signs of abdominal obstruction, which result in significant consequences that can lead to treatment failure and high mortality rate. Imaging in the form of a CT abdomen is the cornerstone in diagnosis, although many patients are diagnosed intraoperatively during exploratory laparotomy. Treatment is usually directed at eliminating provoking factors and directed therapy based on the disease phase. In this case, we are discussing a 69-year-old patient presenting with signs of abdominal obstruction and found to have a large cystic lesion compressing small bowels. Eventually, patient obstruction was relieved with draining though interventional radiology after a trial of conservative management failed. Our goal is to notify our colleagues that we have a high index of suspicion coupled with prompt imaging evaluation that can facilitate early diagnosis, offering hope for improved patient outcomes through timely management strategies.

Abstract:Adenomyosis is commonly treated by total hysterectomy. Adenomyomectomy is considered for women of reproductive age who wish to preserve their fertility. However, a high recurrence rate following adenomyomectomy has been reported because complete removal of the lesion is difficult, and uterine rupture during pregnancy remains a complication. We previously reported that laparoscopic adenomyomectomy using a cold knife prevented thermal damage to the myometrium and elastography to avoid residual lesions. Here, we report the case of a patient who underwent complete resection of a subtype II adenomyosis and resection of deep endometriosis (DE) with the closure of the pouch of Douglas. The patient was 31 years old, had severe dysmenorrhea, and had left ureteral stenosis and subtype II adenomyosis associated with the closure of the pouch of Douglas by the DE. After resection of the DE posterior wall adenomyosis, residual lesions were confirmed by laparoscopic real-time elastography. Eight weeks after surgery, postoperative transvaginal ultrasound showed that the myometrium had shrunk from 28 to 22.7 mm, and the hydronephrosis had disappeared, although a stent remained necessary. In this study, we report the complete resection of subtype II adenomyosis and DE, combined with elastography to visualize the lesions during resection.

Abstract:Peritoneal dialysis is an effective and inexpensive treatment compared with hemodialysis for end-stage renal failure. [ 1 ] Approximately 11% of dialysis patients receive peritoneal dialysis. [ 2 , 3 ] Northern Cyprus has a population of approximately 400,000. These are the available options for treating end-stage renal disease, including hemodialysis, peritoneal dialysis, and kidney transplantation. Each year, approximately 400 patients receive dialysis treatment at 4 state-operated centers. Patients requiring dialysis may opt for peritoneal dialysis, which involves inserting a catheter into the peritoneal cavity to facilitate efficient fluid exchange and improve outcomes. [ 4 , 5 ] A peritoneal dialysis catheter was placed during the open surgery. Currently, laparoscopic methods are widely preferred. [ 3 ]

Abstract:Background: In patients with obesity and end-stage kidney disease, implantation of the peritoneal dialysis (PD) catheter may be complicated by increased abdominal circumference or skin folds. Relocation of the implantation site to the upper abdomen could solve this problem. However, this would require an extended catheter. Methods: We developed an extended PD catheter based on a swan neck Missouri PD catheter with the help of two adaptors and a straight intraperitoneal extension segment. The extended catheter was assembled intraoperatively, and its length was adjusted individually to ensure correct positioning. After the operation, PD was commenced and handled as usual. Results: In the period from 2011 to 2021, we implanted 31 extended PD catheters in 29 patients (38% men) with end-stage renal failure and obesity. Median age was 53 (range 28-77) years and body mass index was 35.5 (range 26.4-46.9) kg/m2. The postoperative course was unremarkable except for seroma formation in one patient and dialysate leakage in another. Continuous ambulatory peritoneal dialysis (CAPD) was initiated in 20 and APD in 9 patients. The achieved median Kt/V was 2.10 (range 1.50-3.10). During the follow-up period lasting up to 51 months, there was one case of intraperitoneal catheter disconnection due to an avoidable handling error. The peritonitis rate was 1:40 months. The 1- and 2-year catheter survival was 92% and 67%, respectively, and paralleled patient survival. Conclusions: When using a PD catheter with an intraperitoneal extension, PD catheter implantation can be relocated to the upper abdomen in patients with obesity, thus providing optimal position and easy surgical access.

Abstract:Continuous ambulatory peritoneal dialysis is an important modality of renal replacement therapy in children. Catheter dysfunction (commonly obstruction) is a major cause of morbidity and is a significant concern that hampers renal replacement therapy. As omentum is a significant cause of obstruction, some recommend routine omentectomy during insertion of the peritoneal dialysis catheter. Omentopexy rather than omentectomy has been described in adults to spare the omentum as it may be needed as a spare part in many conditions. Laparoscopic approach is commonly preferred as it provides global evaluation of the peritoneal space, proper location of the catheteral end in the pelvis and lesser morbidity due to inherent minimally invasive nature.

Abstract:Objective To evaluate the effects of previous abdominal or pelvic surgery on peritoneal dialysis catheter’s placement via open technique in patients with end-stage renal diseases(ESRD). Methods A total of 111 ESRD patients were divided into two groups, 25 patients with previous abdominal or pelvic surgery(group A), and 86 patients without history of abdominal or pelvic operation(group B). The short- and long-term complications of the procedures of peritoneal dialysis catheter’s placement were observed. Results The rates of complications after catherter placement in group A and B were as following: wound haematoma 8.0% vs 3.5%, catheter malposition 0 vs 2.3%, pericatheter leakage 4.0% vs 3.5%, abdominal pain 12.0% vs 9.3%, exit site infection 0 vs 0, peritonitis 16.0% vs 12.8%, catheter blockage 0 vs 2.3% and scrotal swelling 0 vs 1.2%. However, no statistically difference was observed between these two groups. Conclusions Previous minor to moderate abdominal surgery does not have a major impact on the complication after peritoneal dialysis catheter’s placement via open technique. Preoperative evaluation of intra-abdominal adhesions and careful intraoperative manipulation are of great importance.

Abstract:Various methods of peritoneal dialysis (PD) catheter insertion are available. The purpose of this study was to evaluate a percutaneous insertion technique using ultrasound (US) and fluoroscopy performed under conscious sedation and as day case procedure. Data of 87 percutaneous inserted dialysis catheters were prospectively collected, including patients' age, gender, body mass index, history of previous abdominal surgery and cause of end stage renal failure. Length of hospital stay, early complications and time to first use were also recorded. Institutional review board approval was obtained. A 100% technical success rate was observed. Early complications included bleeding (n = 3), catheter dysfunction (n = 6), exit site infection (n = 1) and exit site leakage (n = 1). All cases of catheter dysfunction and one case of bleeding required surgical revision. Median time of follow-up was 18 months (range 3-35), and median time from insertion to first use was days 14 (1-47). Of the 82 patients who started dialysis, 20 (23%) ceased PD at some stage during follow-up. Most frequently encountered reasons include deteriorating patient cognitive or functional status (n = 5), successful transplant kidney (n = 4) and pleuro-peritoneal fistula (n = 4). Sixty-two (71%) PD catheter insertions were performed as day case. The remaining insertions were performed on patients already admitted to the hospital. Percutaneous insertion of dialysis catheter using US and fluoroscopy is not only safe but can be performed as day case procedure in most patients, even with a medical history of abdominal surgery and/or obesity.

Abstract:Background: Peritoneal dialysis (PD) is a more cost-effective therapy to treat kidney failure than in-center hemodialysis, but successful therapy requires a functioning PD catheter that causes minimal complications. In 2015, the North American Chapter of the International Society for Peritoneal Dialysis established the North American PD Catheter Registry to improve practices and patient outcomes following PD catheter insertion. Aims: The objective of this study is to propose a methodology for defining insertion-related complications that lead to significant adverse events and report the risk of these complications among patients undergoing laparoscopic PD catheter insertion. Methods: Patients undergoing laparoscopic PD catheter insertion were enrolled at 14 participating centers in Canada and the United States and followed using a Web-based registry. Insertion-related complications were defined as flow restriction, exit-site leak, or abdominal pain at any point during follow-up. We also included infections or bleeding within 30 days of insertion, and any immediate postoperative complications. Adverse events were categorized as PD never starting or termination of PD therapy, delay in the start of PD therapy or interruption of PD therapy, an emergency department visit or hospitalization, or need for invasive procedures. Cause-specific cumulative incidence functions were used to estimate risk. Results: Five hundred patients underwent laparoscopic PD catheter insertion between 10 November 2015 and 24 July 2018. The cumulative risk of insertion-related complications 6 months from the date of insertion that led to an adverse event was 24%. The risk of flow restriction, exit-site leak, and pain at 6 months was 10.2%, 5.7%, and 5.3%, respectively. PD was never started or terminated in 6.4% of patients due to an insertion-related complication. Leaks and flow restrictions were most likely to delay or interrupt PD therapy. Flow restrictions were the primary cause of invasive procedures. Fifty percent of the complications occurred before the start of PD therapy. Conclusions: Insertion-related complications leading to significant adverse events following laparoscopic placement of PD catheters are common. Many complications occur before the start of PD. Insertion-related complications are an important area of focus for future research and quality improvement efforts.
