Abstract:Abstract Background Available data proving the value of DWI for breast cancer diagnosis is mainly for enhancing masses; DWI may be less sensitive and specific in non-mass enhancement (NME) lesions. The objective of this study was to assess the diagnostic accuracy of DWI using different ROI measurement approaches and ADC metrics in breast lesions presenting as NME lesions on dynamic contrast-enhanced (DCE) MRI. Methods In this retrospective study, 95 patients who underwent multiparametric MRI with DCE and DWI from September 2007 to July 2013 and who were diagnosed with a suspicious NME (BI-RADS 4/5) were included. Twenty-nine patients were excluded for lesion non-visibility on DWI ( n = 24: 12 benign and 12 malignant) and poor DWI quality ( n = 5: 1 benign and 4 malignant). Two readers independently assessed DWI and DCE-MRI findings in two separate randomized readings using different ADC metrics and ROI approaches. NME lesions were classified as either benign (> 1.3 × 10 −3 mm 2 /s) or malignant (≤ 1.3 × 10 −3 mm 2 /s). Histopathology was the standard of reference. ROC curves were plotted, and AUCs were determined. Concordance correlation coefficient (CCC) was measured. Results There were 39 malignant (59%) and 27 benign (41%) lesions in 66 (65 women, 1 man) patients (mean age, 51.8 years). The mean ADC value of the darkest part of the tumor (Dptu) achieved the highest diagnostic accuracy, with AUCs of up to 0.71. Inter-reader agreement was highest with Dptu ADC max (CCC 0.42) and lowest with the point tumor (Ptu) ADC min (CCC = − 0.01). Intra-reader agreement was highest with Wtu ADC mean (CCC = 0.44 for reader 1, 0.41 for reader 2), but this was not associated with the highest diagnostic accuracy. Conclusions Diagnostic accuracy of DWI with ADC mapping is limited in NME lesions. Thirty-one percent of lesions presenting as NME on DCE-MRI could not be evaluated with DWI, and therefore, DCE-MRI remains indispensable. Best results were achieved using Dptu 2D ROI measurement and ADC mean.

Abstract:Objective: To compare diffusion-weighted images (DWI) acquired using single-shot echo-planar imaging (ss-EPI) and multiplexed sensitivity encoding (MUSE) in breast cancer. Methods 20 females with pathologically confirmed breast cancer (age 51 ± 12 years) were imaged with ss-EPI-DWI and MUSE-DWI. ADC, normalised ADC (nADC), blur and distortion metrics and qualitative image quality scores were compared. The Crété-Roffet and Mattes mutual information metrics were used to evaluate blurring and distortion, respectively. In a breast phantom, six permutations of MUSE-DWI with varying parallel acceleration factor and number of shots were compared. Differences in ADC and nADC were compared using the coefficient of variation in the phantom and a paired t-test in patients. Differences in blur, distortion and qualitative metrics were analysed using a Wilcoxon signed-rank test. Results: There was a low coefficient of variation (<2%) in ADC between ss-EPI-DWI and all MUSE-DWI permutations acquired using the phantom. 22 malignant and three benign lesions were identified in 20 patients. ADC values measured using MUSE were significantly lower compared to ss-EPI for malignant but not benign lesions (p < 0.001, p = 0.21). nADC values were not significantly different (p = 0.62, p = 0.28). Blurring and distortion improved with number of shots and acceleration factor, and significantly improved with MUSE in patients (p < 0.001, p = 0.002). Qualitatively, image quality improved using MUSE. Conclusion: MUSE improves the image quality of breast DWI compared to ss-EPI. Advances in knowledge: MUSE-DWI has superior image quality and reduced blurring and distortion compared to ss-EPI-DWI in breast cancer.

Abstract:Objective To evaluate the role of diffusion weighted imaging(DWI) and apparent diffusion coefficient(ADC) in the differential diagnosis of lymph node lesions.Methods Conventional MR imaging(spin echo T1WI,T2WI and STIR) was performed in 30 patients with histologically proven malignant tumors(16) and benign lesions(14) by using GE 1.5T scanner with cervical phased array coil.Six b values(30 cases) were acquired with diffusion gradients applied in all three orthogonal directions using body coil.Images were acquired independently in each of the gradient directions(X,Y and Z).Diffusion scan time was 40-52 seconds.ADC values were measured in the lymph node lesions.Results All benign and malignant lymph nodes were visible in DWI and showed significant hyperintensity signal.The differences in mean ADC of Group malignant lymph nodes([0.849±0.197]×10~(-3) mm~2/s) and Group benign lymph nodes([1.598±0.218]×10~(-3) mm~2/s) were statistically significant(P0.05).Conclusion Diffusion weighted imaging is a rapid and feasible method in detecting cervical lymph node lesions and shows a potential for distinguishing between benign and malignant lymph nodes.

Abstract:Purpose: Ongoing efforts are focusing on optimizing diffusion-weighted imaging (DWI) as an essential part of breast MRI protocol. Our study aimed to evaluate the effect of contrast media (CM) on the apparent diffusion coefficients (ADC) acquired following current recommendations. Patient and methods: Patients who underwent 3 T breast MRI with a histologically verified suspicious lesion were included in this IRB-approved, single-center, cross-sectional retrospective study. Breast MRI protocol included a DWI sequence with multiple b-values, which was acquired before and after CM administration. ADC maps were calculated by in-line monoexponential fitting with b-values 0 /800 and 50/800. Two independent readers (R1, R2) reviewed the images in separate sessions for b values 0/800 and 50/800, pre- and post-CM. Bland Altmann plots as well as intraclass correlation coefficients (ICCs) for inter-reader agreement, different b-values, and pre- and post-CM were calculated. Diagnostic accuracy was evaluated and compared by calculating the area under the receiver operating characteristics curve (AUC). Results: 91 lesions in 89 patients were examined (mean age 50.7 years, standard deviation 13.9). ADC values were significantly (P<0.05) lower post-CM (mean ranging from 1.28 x10-3 mm2/s to 1.30 x10-3 mm2/s) compared to pre-CM (mean ranging from 1.32 x10-3 mm2/s to 1.37 x10-3 mm2/s) for both b-values combinations (0/800 and 50/800 s/mm2). We found an almost perfect inter-reader agreement pre-/post-CM with b values 0/800 and 50/800 (ICC ranging from 0.853 to 0.939). Bland Altman plot demonstrated no systematic difference between readers. ROC analysis revealed good diagnostic performance without significant differences (P>0.05) between b values 0/800 and 50/800 s/mm2 as well as pre- and post-CM with areas under the ROC curve between 0.834 and 0.877. Conclusion: ADC values are slightly lower when acquiring b values 0/800 and post-CM. This effect does not reduce the diagnostic performance but may be relevant in case of definite cut-offs in medical decision making.

Abstract:ObjectiveWe aimed to evaluate the effectiveness of simultaneous multi-slice readout-segmented echo-planar diffusion-weighted imaging (SMS rs-EPI DWI), compared with readout-segmented echo-planar diffusion-weighted imaging (NOSMS rs-EPI DWI), in discriminating between benign and malignant breast lesions.Materials and methodsThis retrospective study evaluated breast lesions from 185 consecutive patients who had undergone preoperative breast MRI. The NOSMS rs-EPI DWI and the prototype SMS rs-EPI DWI sequences were performed on a 1.5-T MR scanner. Two independent radiologists evaluated the image quality of the two sequences using a 5-point scale (1 = poor, 5 = excellent) and then assessed the scores through visual grading characteristics analysis (VGC). The values of signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and whole-tumor-based histogram parameter (ADCmedian) were compared between the two sequences using the Wilcoxon test. Then ROC curves were used for statistical analysis.ResultsThe visual assessment showed that the SMS rs-EPI yielded superior overall image quality and lesion delineation than the NOSMS rs-EPI (AUCVGC = 0.980 and 0.984, respectively; all P < 0.001). There were no significant differences in relevant artifacts between the two sequences (AUCVGC = 0.531, P = 0.462). The SNR values for SMS rs-EPI DWI were significantly lower than for NOSMS rs-EPI DWI (P = 0.019) while there was no significant difference in CNR values between the two sequences (P = 0.955). In addition, evaluation of the diagnostic performance demonstrated that the difference in ADCmedian values for both DWI sequences between the malignant and benign lesions was statistically significant (P < 0.001). In contrast, the AUC for ADCmedian was higher with SMS rs-EPI than NOSMS rs-EPI (0.879 for SMS vs. 0.839 for NOSMS, P = 0.025).ConclusionThe SMS technique could further improve the image quality and the diagnostic performance of rs-EPI DWI in a comparable time.

Abstract:Objective To investigate the diagnostic value of ADC for breast lesions with different enhancement shape or mass size. Methods One hundred and thirty-six breast lesions confirmed by histopathology were included in this study. According to enhancement shape and size of the lesion, all lesions were divided into 3 groups: non-masslike enhancement ( G1 ), masslike enhancement with the largest diameter ＜ 2. 0 cm (G2a) and masslike enhancement with the largest diameter ＞ 2. 0 cm (G2b). Echo planar imaging DWI was performed and three b-values (0,500 and 1000 s/mm2) were applied. The t-test was used for testing the difference of ADC between malignant and non-malignant breast lesions in each group. ROC curve was deduced to test the diagnostic efficiency of ADC. The sensitivity, specificity, negative predictive value( NPV), positive predictive value(PPV) and accuracy of ADC for the diagnosis of breast lesions were calculated under the different threshold. Appropriate b value and threshold were determined with the combination of morphologic evaluation. Results There were no significant differences for the mean ADC values between malignant [b =800 mm2/s: ADC value = ( 1.13 ±0. 23) × 10-3 mm2/s,b=1000 mm2/s: ADC value = (1.05 ±0.20) × 10-3 mm2/s] and non-malignant breast lesions [b =800 mm2/s: ADC value = ( 1.28 ±0. 27) × 10-3 mm2/s, t = 1. 636, P =0. 112,b = 1000 mm2/s: ADC value=(1.20 ±0.23) × 10-3 mm2/s, t = 1.720, P =0. 109] in Group 1. Conclusion MR DWI is useful for the differential diagnosis of breast lesions with masslike enhancement rather than nonmasslike enhancement.
The Role of Diffusion-Weighted Imaging and the Apparent Diffusion Coefficient (ADC) Values for Breast Tumors

Abstract:OBJECTIVE: We wanted to evaluate the role of diffusion-weighted imaging (DWI) and the apparent diffusion coefficient (ADC) for detecting breast tumors, as compared with the T1- and T2-weighted images.MATERIALS AND METHODS: Forty-one female patients underwent breast MRI, and this included the T1-, T2-, DWI and dynamic contrast-enhanced images. Sixty-five enhancing lesions were detected on the dynamic contrast-enhanced images and we used this as a reference image for detecting tumor. Fifty-six breast lesions were detected on DWI and the histological diagnoses were as follows: 43 invasive ductal carcinomas, one mucinous carcinoma, one mixed infiltrative and mucinous carcinoma, seven ductal carcinomas in situ (DCIS), and four benign tumors. First, we compared the detectability of breast lesions on DWI with that of the T1- and T2-weighted images. We then compared the ADCs of the malignant and benign breast lesions to the ADCs of the normal fibroglandular tissue.RESULTS: Fifty-six lesions were detected via DWI (detectability of 86.2%). The detectabilities of breast lesions on the T1- and T2-weighted imaging were 61.5% (40/65) and 75.4% (49/65), respectively. The mean ADCs of the invasive ductal carcinoma (0.89+/-0.18 x 10(-3)mm(2)/second) and DCIS (1.17+/-0.18 x 10(-3)mm(2)/ second) are significantly lower than those of the benign lesions (1.41+/-0.56 x 10(-3)mm(2)/second) and the normal fibroglandular tissue (1.51+/-0.29 x 10(-3)mm(2)/ second).CONCLUSION: DWI has a high sensitivity for detecting breast tumors, and especially for detecting malignant breast tumors. DWI was an effective imaging technique for detecting breast lesions, as compared to using the T1- and T2-weighted images.

Abstract:Introduction With increasing spatial resolution, diffusion-weighted imaging (DWI) may be suitable for morphologic lesion characterization in breast MRI – an area that has traditionally been occupied by dynamic contrast-enhanced imaging (DCE). This investigation compared DWI with b values of 800 and 1600 s/mm 2 to DCE for lesion morphology assessment in high-resolution breast MRI at 3 Tesla. Material and methods Multiparametric breast MRI was performed in 91 patients with 93 histopathologically proven lesions (31 benign, 62 malignant). Two radiologists independently evaluated three datasets per patient (DWI b800 ; DWI b1600 ; DCE) and assessed lesion visibility and BIRADS morphology criteria. Diagnostic accuracy was compared among readers and datasets using Cochran's Q test and pairwise post-hoc McNemar tests. Bland-Altman analyses were conducted for lesion size comparisons. Results Discrimination of carcinomas was superior compared to benign findings in both DWI b800 and DWI b1600 ( p < 0.001) with no b value-dependent difference. Similarly, assessability of mass lesions was better than of non-mass lesions, irrespective of b value ( p < 0.001). Intra-reader reliability for the analysis of morphologic BIRADS criteria among DCE and DWI datasets was at least moderate (Fleiss κ ≥0.557), while at least substantial inter-reader agreement was ascertained over all assessed categories ( κ ≥0.776). In pairwise Bland-Altman analyses, the measurement bias between DCE and DWI b800 was 0.7 mm, whereas the difference between DCE and DWI b1600 was 2.8 mm. DWI b1600 allowed for higher specificity than DCE ( p = 0.007/0.062). Conclusions DWI can be employed for reliable morphologic lesion characterization in high-resolution breast MRI. High b values increase diagnostic specificity, while lesion size assessment is more precise with standard 800 s/mm 2 images.

Values of Apparent Diffusion Coefficient and Lesion-to-Spinal Cord Signal Intensity in Diagnosing Solitary Pulmonary Lesions: Turbo Spin-Echo versus Echo-Planar Imaging Diffusion-Weighted Imaging

Abstract:Objective. This study is aimed at comparing the image quality and diagnostic performance of mean apparent diffusion coefficient (ADC) and lesion-to-spinal cord signal intensity ratio (LSR) derived from turbo spin-echo diffusion-weighted imaging (TSE-DWI) and echo-planar imaging- (EPI-) DWI in patients with a solitary pulmonary lesion (SPL). Methods. 33 patients with SPL underwent chest imaging using EPI-DWI and TSE-DWI with b = 600 s/mm2 in free breathing. A comparison of the distortion ratio (DR), signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) was drawn between the two techniques using a Wilcoxon signed-rank test. The interprotocol reproducibility between quantitative parameters of EPI-DWI and TSE-DWI was evaluated using a Bland-Altman plot. ADCs and LSRs derived from EPI-DWI and TSE-DWI were calculated and compared between malignant and benign groups using the Mann–Whitney test. Results. TSE-DWI had similar SNR and CNR compared with EPI-DWI. DR was significantly lower on TSE-DWI than EPI-DWI. ADC and LSR showed slightly higher values with TSE-DWI, while the Bland-Altman analysis showed unacceptable limits of agreement between the two sequences. ADC and LSR of both DWI techniques differed significantly between lung cancer and benign lesions ( P < 0.05 ). The LSR(EPI-DWI) showed the highest area under the curve ( AUC = 0.818 ), followed by ADC(EPI-DWI) ( AUC = 0.789 ), ADC(TSE-DWI) ( AUC = 0.781 ), and LSR(TSE-DWI) ( AUC = 0.748 ), respectively. Among these parameters, the difference in diagnostic accuracy was not statistically significant. Conclusions. TSE-DWI provides significantly improved image quality in patients with SPL as compared with EPI-DWI. However, there was no difference in diagnostic efficacy between these two techniques, according to ADC and LSR.

Abstract:To determine whether limits to life-sustaining care are becoming more common, we attempted to quantify the incidence of recommendations to withhold or withdraw life support from critically ill patients, to describe how patients respond to these recommendations, and to examine how conflicts over these recommendations are resolved. In 1992 and 1993 we prospectively enrolled 179 consecutive patients from two intensive care units (ICUs) for whom a recommendation was made to withhold or withdraw life support. Where possible, we compared results with data collected in the same units over a similar time period in 1987 and 1988. Recommendations to withhold or withdraw life support preceded 179 of 200 deaths (90%) in 1992 and 1993, compared with 114 of 224 deaths (51%) in 1987 and 1988 (chi2 = 73.76, p < 0.001]. Cardiopulmonary resuscitation was initiated in 10% of deaths in 1992 and 1993 as compared with 49% in 1987 and 1988. Ninety percent of patients agreed within less than 5 d, and only eight patients (4%) refused physicians' recommendations to limit life support. In cases of conflict, physicians in 1992 and 1993 deferred to patients with one exception: physicians were willing to refuse surrogate requests for resuscitation of patients they considered hopelessly ill. We conclude that 90% of patients who die in these ICUs now do so following a decision to limit therapy, that this represents a major change in practice in these institutions over a period of 5 yr, that most patients and surrogates accept an appropriate recommendation to withhold or withdraw life support, and that physicians will refuse surrogate requests in certain circumstances.

Abstract:Purpose:To evaluate the value of diffusion-weighted imaging with different b values in the diagnosis of breast lesions at 3.0T MRI.Methods: Twenty normal controls and 34 patients with 20 benign breast lesions and 16 malignant ones underwent diffusion-weighted imaging at 3.0T MRI with different b values of 0,750,1000 and 1250s/mm2.The other conventional sequences(T2WI and T1WI dynamic contrast-enhanced MRI)were performed at 3.0T or 1.5T MRI.Image quality score,contrast-to-noise ratio(CNR) and apparent diffusion coefficient(ADC) of normal breast,benign lesions and malignant ones with the same b value and different b values were compared and analyzed.Sensitivity,specificity and accuracy for the diagnosis of breast cancer with different b values were calculated respectively,as well as the ROC curves.Results:(1) The difference of the image quality score among the different b values was not statistically significant(F=3.02,P=0.0516>0.05).(2) The difference of CNR of the breast lesion among the different b values was statistically significant(F=10.69,P=0.0001<0.01).Higher CNR was obtained when b value was 750s/mm2 than those when b value was 1000s/mm2 and 1250s/mm2,and the differences were statistically significant(P values were 0.004 and 0.000),but the difference of CNR between the b value of 1000s/mm2 and 1250s/mm2 was not statistically significant(P=0.800>0.05).(3) Average ADC value of normal breast was higher than benign lesion,while that of benign lesion was higher than that of malignant ones at the same b value.Among them the differences were statistically significant(P values were all 0.000).(4) The cutoff ADC value was 1.33×10-3mm2,1.25×10-3mm2/s,and 1.16×10-3mm2/s when b value was 0,750s/mm2,0,1000 and 0,1250s/mm2.Sensitivity,specificity and accuracy for diagnosis of breast cancer was 81.25%,87.5%,85.71%;81.25%,77.5%,78.57% and 81.25%,82.5%,82.14% respectively.And the area under ROC curve(AUC) was 0.8781,0.8672 and 0.8828 respectively.The difference among them was not statistically significant(P>0.05).Conclusion:(1) Better image quality and CNR score were obtained when b value was 750s/mm2 than those when b value was 1000s/mm2 and 1250s/mm2 at 3.0T.(2) Diagnosis of breast lesions with diffusion-weighted imaging must be made in combination with ADC value.(3) Accuracy in the detection of breast cancer at 3.0T was high with all b values.However we believe the best b value was 0,750s/mm2 because of its highest image quality and CNR.

Abstract:BACKGROUND: Quantitative diffusion-weighted imaging (DWI) MRI is a promising technique for cancer characterization and treatment monitoring. Knowledge of the reproducibility of DWI metrics in breast tumors is necessary to apply DWI as a clinical biomarker.PURPOSE: To evaluate the repeatability and reproducibility of breast tumor apparent diffusion coefficient (ADC) in a multi-institution clinical trial setting, using standardized DWI protocols and quality assurance (QA) procedures.STUDY TYPE: Prospective.SUBJECTS: In all, 89 women from nine institutions undergoing neoadjuvant chemotherapy for invasive breast cancer.FIELD STRENGTH/SEQUENCE: DWI was acquired before and after patient repositioning using a four b-value, single-shot echo-planar sequence at 1.5T or 3.0T.ASSESSMENT: A QA procedure by trained operators assessed artifacts, fat suppression, and signal-to-noise ratio, and determine study analyzability. Mean tumor ADC was measured via manual segmentation of the multislice tumor region referencing DWI and contrast-enhanced images. Twenty cases were evaluated multiple times to assess intra- and interoperator variability. Segmentation similarity was assessed via the Sørenson-Dice similarity coefficient.STATISTICAL TESTS: Repeatability and reproducibility were evaluated using within-subject coefficient of variation (wCV), intraclass correlation coefficient (ICC), agreement index (AI), and repeatability coefficient (RC). Correlations were measured by Pearson's correlation coefficients.RESULTS: In all, 71 cases (80%) passed QA evaluation: 44 at 1.5T, 27 at 3.0T; 60 pretreatment, 11 after 3 weeks of taxane-based treatment. ADC repeatability was excellent: wCV = 4.8% (95% confidence interval [CI] 4.0, 5.7%), ICC = 0.97 (95% CI 0.95, 0.98), AI = 0.83 (95% CI 0.76, 0.87), and RC = 0.16 * 10<sup>-3</sup> mm<sup>2</sup> /sec (95% CI 0.13, 0.19). The results were similar across field strengths and timepoint subgroups. Reproducibility was excellent: interreader ICC = 0.92 (95% CI 0.80, 0.97) and intrareader ICC = 0.91 (95% CI 0.78, 0.96).DATA CONCLUSION: Breast tumor ADC can be measured with excellent repeatability and reproducibility in a multi-institution setting using a standardized protocol and QA procedure. Abstract:ObjectivesTo investigate the influence of region-of-interest (ROI) placement and different apparent diffusion coefficient (ADC) parameters on ADC values, diagnostic performance, reproducibility and measurement time in breast tumours.MethodsIn this IRB-approved, retrospective study, 149 histopathologically proven breast tumours (109 malignant, 40 benign) in 147 women (mean age 53.2) were investigated. Three radiologists independently measured minimum, mean and maximum ADC, each using three ROI placement approaches:1 – small 2D-ROI, 2 – large 2D-ROI and 3 – 3D-ROI covering the whole lesion. One reader performed all measurements twice. Median ADC values, diagnostic performance, reproducibility, and measurement time were calculated and compared between all combinations of ROI placement approaches and ADC parameters.ResultsMedian ADC values differed significantly between the ROI placement approaches (p < .001). Minimum ADC showed the best diagnostic performance (AUC .928–.956), followed by mean ADC obtained from 2D ROIs (.926–.94). Minimum and mean ADC showed high intra- (ICC .85–.94) and inter-reader reproducibility (ICC .74–.94). Median measurement time was significantly shorter for the 2D ROIs (p < .001).ConclusionsROI placement significantly influences ADC values measured in breast tumours. Minimum and mean ADC acquired from 2D-ROIs are useful for the differentiation of benign and malignant breast lesions, and are highly reproducible, with rapid measurement.Key Points• Region of interest placement significantly influences apparent diffusion coefficient of breast tumours.• Minimum and mean apparent diffusion coefficient perform best and are reproducible.• 2D regions of interest perform best and provide rapid measurement times.

Abstract:PURPOSE: The purpose of this study was to investigate the utility of diffusion-weighted imaging (DWI) and the apparent diffusion coefficient (ADC) value in differentiating benign and malignant breast lesions and evaluating the detection accuracy of the cancer extension.MATERIALS AND METHODS: We used DWI to obtain images of 191 benign and malignant lesions (24 benign, 167 malignant) before surgical excision. The ADC values of the benign and malignant lesions were compared, as were the values of noninvasive ductal carcinoma (NIDC) and invasive ductal carcinoma (IDC). We also evaluated the ADC map, which represents the distribution of ADC values, and compared it with the cancer extension.RESULTS: The mean ADC value of each type of lesion was as follows: malignant lesions, 1.22+/-0.31 x 10(-3) mm2/s; benign lesions, 1.67+/-0.54 x 10(-3) mm2/s; normal tissues, 2.09+/-0.27 x 10(-3) mm2/s. The mean ADC value of the malignant lesions was statistically lower than that of the benign lesions and normal breast tissues. The ADC value of IDC was statistically lower than that of NIDC. The sensitivity of the ADC value for malignant lesions with a threshold of less than 1.6 x 10(-3) mm2/s was 95% and the specificity was 46%. A full 75% of all malignant cases exhibited a near precise distribution of low ADC values on ADC maps to describe malignant lesions. The main causes of false negative and underestimation of cancer spread were susceptibility artifact because of bleeding and tumor structure. Major histologic types of false-positive lesions were intraductal papilloma and fibrocystic diseases. Fibrocystic diseases also resulted in overestimation of cancer extension.CONCLUSIONS: DWI has the potential in clinical appreciation to detect malignant breast tumors and support the evaluation of tumor extension. However, the benign proliferative change remains to be studied as it mimics the malignant phenomenon on the ADC map.

Abstract:To investigate the feasibility and effectiveness of high-resolution readout-segmented echo planar imaging (rs-EPI), diffusion-weighted imaging (DWI) is used simultaneously with multi-slice (SMS) imaging (SMS rs-EPI) for the differentiation of breast malignant and benign lesions in comparison to conventional rs-EPI on a 3T MR scanner. A total of 102 patients with 113 breast lesions underwent bilateral breast MRI using a prototype SMS rs-EPI sequence and a conventional rs-EPI sequence. Subjective image quality was assessed using a 5-point Likert scale (1 = poor, 5 = excellent). Signal-to-noise ratio (SNR), lesion contrast-to-noise ratio (CNR) and apparent diffusion coefficients (ADC) value of the lesion were measured for comparison. Receiver operating characteristic curve analysis was performed to evaluate the diagnosis performance of ADC, and the corresponding area under curve (AUC) was calculated. The image quality scores in anatomic distortion, lesion conspicuity, sharpness of anatomical details and overall image quality of SMS rs-EPI were significantly higher than those of conventional rs-EPI. CNR was enhanced in the high-resolution SMS rs-EPI acquisition (6.48 ± 1.71 vs. 4.23 ± 1.49; p < 0.001). The mean ADC value was comparable in SMS rs-EPI and conventional rs-EPI (benign 1.45 × 10−3 vs. 1.43 × 10−3 mm2/s, p = 0.702; malignant 0.91 × 10−3 vs. 0.89 × 10−3 mm2/s, p = 0.076). The AUC was 0.957 in SMS rs-EPI and 0.983 in conventional rs-EPI. SMS rs-EPI technique allows for higher spatial resolution and slight reduction of scan time in comparison to conventional rs-EPI, which has potential for better differentiation between malignant and benign lesions of the breast.

Abstract:Objective To evaluate the b value of diffusion-weighted(DW)MRI in distinguishing between benign and malignant breast lesions.Methods Three diffusion-weighted sequences were implemented with 500,1000 and 2000 s/mm~2 b values respectively on 95 breast lesions in 83 patients.All lesions were confirmed by pathology.The apparent diffusion coefficient(ADC)values and signal intensity (SI)were recorded and compared in different lesions(breast cancer,benign lesion,cyst and normal beast tissue)with the same b value and the same lesions with the different b values.Results(1)The mean ADC value and SI of breast cancer were 1.375±0.378 and 839.713±360.493 respectively with b= 500 s/mm~2,1.176±0.311 and 459.314±229.609 with b=1000 s/mm~2,0.824±0.198 and 243.825± 110.616 with b=2000 s/mm~2.The differences in the mean ADC value were significant between two type lesions(cancer and benign lesion,cancer and cyst,cancer and normal breast tissue)with b values of 500 s/mm~2 and 1000 s/mm~2.But the significant differenee was only seen between cancer and benign lesions when b value was 2000 s/mm~2.(2)The one-side upper limits of 95% confidence interval of mean ADCs were adopted as the point to separate the malignant from the benign lesions,the sensitivity was 70.92%, 70.73% and 69.77%,the specificity was 77.19%,75.70% and 54.76%,the accuracy was 77.12%, 74.32% and 62.35% respectively with b values of 500 s/mm~2,1000 s/mm~2 and 2000 s/mm~2.The areas under ROC eurves were Az_(500)=0.775±0.046(P0.01),Az_(1000)=0.780±0.044(P0.01)and Az_(2000)=0.620±0.062(P0.05).Conclusion DWI MRI is useful for the differential diagnosis of breast lesions with b values of 500 s/mm~2 and 1000 s/mm~2.

Abstract:PURPOSE:To evaluate the value of diffusion-weighted imaging (DWI) in distinguishing between benign and malignant breast lesions.MATERIALS AND METHODS:Fifty-two female subjects (mean age = 58 years, age range = 25-75 years) with histopathologically proven breast lesions underwent DWI of the breasts with a single-shot echo-planar imaging (EPI) sequence using large b values. The computed mean apparent diffusion coefficients (ADCs) of the breast lesions and cell density were then correlated.RESULTS:The ADCs varied substantially between benign breast lesions ((1.57 +/- 0.23) x 10(-3) mm(2)/second) and malignant breast lesions ((0.97 +/- 0.20) x 10(-3) mm(2)/second). In addition, the mean ADCs of the breast lesions correlated well with tumor cellularity (P < 0.01, r = -0.542).CONCLUSION:The ADC would be an effective parameter in distinguishing between malignant and benign breast lesions. Further, tumor cellularity has a significant influence on the ADCs obtained in both benign and malignant breast tumors.
A multiparametric approach to diagnosing breast lesions using diffusion-weighted imaging and ultrafast dynamic contrast-enhanced MRI

Abstract:Purpose: To evaluate the diagnostic performance of a multiparametric approach to breast lesions including apparent diffusion coefficient (ADC) from diffusion-weighted images (DWI), maximum slope (MS) from ultrafast dynamic contrast enhanced (UF-DCE) MRI, lesion size, and patient's age. Materials and methods: In total, 96 lesions (73 malignant, 23 benign) were evaluated. UF-DCE MRI was acquired using a prototype 3D-gradient-echo volumetric interpolated breath-hold examination (VIBE) with compressed sensing. Images were obtained up to 1 min after gadolinium injection. MS was calculated as the percentage relative enhancement/s. An ADC map was automatically generated from DWI at b = 0 and b = 1000 s/mm2. MS and ADC values were measured by two radiologists independently. Interrater agreement was evaluated using intraclass correlation coefficients. Univariate and multivariate logistic regression analyses were performed using MS, ADC, lesion size, and the patient's age. The parameters of the prediction model were generated from the results of the multivariate logistic regression analysis. Area under the curve (AUC) was used to compare diagnostic performance of the prediction model and each parameter. Results: Interrater agreements on MS and ADC were excellent (ICC 0.99 and 0.88, respectively). MS, ADC, and patient's age remained as significant parameters after univariate and multivariate logistic regression analysis. The prediction model using these significant parameters yielded an AUC of 0.90, significantly higher than that of MS (AUC 0.74, p = 0.01). The AUCs of ADC, MS, patient's age were 0.87, 0.74 and 0.73, respectively. Conclusions: A multiparametric model using ADC from DWI, MS from UF-DCE MRI, and patient's age showed excellent diagnostic performance, with greater contribution of ADC. Combining DWI and UF-DCE MRI might reduce scanning time while preserving diagnostic performance.
Comparing DWI image quality of deep-learning-reconstructed EPI with RESOLVE in breast lesions at 3.0T: a pilot study

Abstract:The challenging spatial resolution of DWI could be addressed by deep learning based image reconstruction, by reducing noise without increasing acquisition time. To compare the image quality of the Echo Planar Imaging Deep Learning (EPI DL) DWI sequence with the clinically used simultaneous multi slice (SMS) RESOLVE in breast lesions. EPI DL and RESOLVE breast images from 20 participants were qualitatively evaluated. Quantitative image quality metrics of SNR and CNR on both high b-value (b800) images and ADC maps were calculated. SNR in RESOLVE vs. EP DL differed statistically significantly in manually delineations for b800 (p=0.006), ADC maps (p=0.001), and in ADC circularly delineations (0.001). DWI DL reconstruction may be clinically useful for addressing low-spatial resolution without compromising acquisition time and image quality. Such benefits coupled with the available methods of readout segmentation and SMS acquisitions may further enhance the value of DWI in breast imaging.

Abstract:Rationale and ObjectivesTo evaluate the value of adding Diffusion Weighted Imaging (DWI) with Apparent Diffusion Coefficient (ADC) mapping to dynamic contrast enhanced (DCE-MRI) to distinguish benign from malignant pathology subtypes and tumor recurrence.Method and MaterialsIn this retrospective IRB approved study, 956 consecutive patients underwent bilateral breast MRI between 1/2015 and 12/2015, with 156 BIRADS 4, 5, or 6 lesions detected in 111 patients. DWI imaging at B0, B100, B600, B1000 was performed with DCE-MRI. Values for diffusion and ADC images were recorded by two fellowship-trained breast radiologists. Mean ADC and signal intensity (SI) values were correlated with histology, tumor grade, hormone receptors (ER, PR, and HER-2)and Oncotype DX scores, when available. p ≤ 0.05 was considered significant.ResultsOf 156 lesions, there were 59 (38%) benign lesions, 24 (15%) Ductal Carcinoma In-Situ, 47 (30%) Invasive Ductal Carcinoma (IDC), 15 (10%) Invasive Lobular Carcinoma (ILC) and 2 (2%) Mucinous carcinoma (MC), five (5%) mixed IDC and ILC, and four (4%) other, including tubular and rare types of malignancy. Mean ADC values for malignancy were significantly lower than for benign lesions (1085 ± 343 × 10−6 vs 1481 ± 276 × 10−6 mm2/s), which is highly predictive (area under curve = 0.82). In addition, tumors with PR negativity and Oncotype score ≥18 (intermediate to high risk for recurrence) demonstrated significantly lower ADC values. SI at B100 and B600 was helpful in distinguishing benign versus IDC. There was no significant correlation between ADC values and tumor grade or ER/HER2 status.ConclusionADC value is important factor in distinguishing malignancy, differentiating tumors with higher Oncotype score, and PR negativity. Therefore, it can be used as an important tool to assist appropriate treatment selection.
