Abstract:Rationale: Re-expansion pulmonary edema (REPE) is a rare complication after chest tube insertion for the treatment of spontaneous pneumothorax. However, this complication can be life threatening when it occurs. Therefore, it is necessary to recognize REPE early and treat it appropriately. In the present study, we report a severe REPE case occurring after chest tube insertion in a patient with spontaneous pneumothorax. Patient concerns: A 27-year-old male patient visited out hospital with chest pain on the left, which had started a week ago. After diagnosed with pneumothorax and having chest tube insertion, the patient complained of sudden shortness of breath, persistent cough, foamy sputum, and vomiting. Diagnosis: Based on the symptoms and imaging findings, the patient was diagnosed as REPE. Interventions: After the condition of the patient deteriorated rapidly, he was transferred to intensive care unit and then mechanical ventilation and conservative treatment were performed after endotracheal intubation. Outcomes: After mechanical ventilation and conservative treatment in the intensive care unit, the symptoms and radiological findings improved, and then mechanical ventilation was weaned and the chest tube was removed from the patient. However, due to recurrent pneumothorax after removal of the chest tube, video assisted thoracoscopic surgery (VATS) wedge resection was performed. At 6 months post-operative follow up, he was well with normal radiological findings. Lessons: REPE occurs rarely, but once it does, it causes a serious condition that can be life-threatening. Therefore, patients with the risk factors related to it should receive a closed observation after chest tube insertion. Moreover, if REPE occurs, appropriate treatments should be carried out by recognizing it early.

Keywords: pulmonary edema, re-expansion, pneumothorax, pleural effusion, chest tube, Ethiopia Re-expansion Pulmonary Edema (RPE) was first reported in the medical literature in the 1860s. 1 Pediatric RPE cases are less common in the reports than adults. 1–3 RPE has also been reported following COVID-19 infection. 4 The diagnosis of RPE depends on high index of suspicion aided by chest imaging after a clinical deterioration following lung expansion. RPE has no specific treatment, and management is mainly supportive with positive pressure ventilation based on severity of illness. Additionally, proactive measures are recommended to modify the occurrence and severity of RPE in high-risk patients. Because of the failure of consideration of RPE in the differential diagnosis, especially in low-income settings such as ours, where TB and other pleuropulmonary infections are very prevalent, it is likely that RPE is easily missed in clinical practice. 1,2 Here we report a pediatric case from a low-income setting who succumbed to RPE after chest tube drainage of pneumothorax. We also present a brief review of the literature of this rare but potentially serious condition to highlight the need to anticipate, prevent, and consider RPE in the differential diagnosis of clinical deteriorations accompanying lung expansions. A 5-year-old female child was transferred from a local hospital for further management after 5 weeks of inpatient treatment with oxygen, parenteral broad-spectrum antibiotics, and anti-tuberculous (TB) drugs for non-resolving pneumonia and suspected pulmonary tuberculosis. Her symptoms started acutely with high fever, cough, and difficulty of breathing 8 weeks before admission. After 4 weeks of treatment, the fever subsided but the respiratory distress and hypoxemia persisted for which treatment for pulmonary tuberculosis was initiated empirically without strong clinical or laboratory/imaging evidence. Finally, the child was referred for better workup and management after she failed to respond to the anti-TB. Physical examination at presentation demonstrated an acutely sick and emaciated child who had significantly increased work of breathing (WOB) and was hypoxemic (SPO 2 = <70% at room air and 90–94% on 6–7 L/min facemask). She had crackles and decreased aeration over the left lower posterior chest with significantly decreased air entry over the right hemithorax and the remaining part of the systemic exam was unremarkable except for presenting as a grossly emaciated child. PA chest X-ray (CXR) revealed a significant right pneumothorax (Figure 1); other investigations including CBC, COVID-19 and HIV test were unremarkable except for a mildly elevated ESR (30 mm/hr) but negative C-reactive protein. Her antibiotics and anti-TB treatment were continued, and a chest tube was inserted for pneumothorax. Within the first hour of drainage, she started to have worsening cough and increased WOB with worsening hypoxemia. A repeat CXR 4 hours after her clinical deterioration showed an expanding but opacified right lung and worsening opacity of the left lung suggestive of bilateral pulmonary edema (Figure 2). When the Lung Refuses to Expand: A Deep Dive Into a Pneumothorax Ex Vacuo Case

Abstract:Pneumothorax ex vacuo and trapped lung represent challenging clinical entities, especially in the context of pre-existing comorbidities. This case report outlines the diagnostic and management pathway of a 38-year-old patient with cerebral palsy who initially presented with empyema. Following the evacuation of the empyema, the patient developed pneumothorax ex vacuo, a rare phenomenon occurring due to a vacuum-like negative intrapleural pressure initiated by lung collapse. Initially suspected to have an infectious etiology based on laboratory findings, the patient was later found to have a large hydropneumothorax through a combination of imaging, laboratory studies, and clinical evaluations without confirming infection or malignancy. Despite interventions including Tissue Plasminogen Activator (TPA) and Deoxyribonuclease (DNAse) administration to facilitate pleural drainage, the patient's condition persisted, necessitating a surgical intervention that evolved from a minimally invasive video-assisted thoracoscopic surgery (VATS) to a more invasive thoracotomy due to unforeseen pleural thickening. The patient's pre-existing condition of cerebral palsy increased his susceptibility to respiratory complications, including empyema, due to the risk of aspiration. This case highlights the importance of a multidisciplinary approach in managing such complex clinical scenarios. It also serves as a clinical reminder that pneumothorax ex vacuo is generally benign and does not typically require chest tube placement, as the primary issue is an unexpandable lung that is unresponsive to pleural drainage. The report emphasizes the need for flexible surgical planning and robust postoperative management to optimize patient outcomes. It also clarifies the distinct pathophysiology of pneumothorax ex vacuo compared to primary or secondary pneumothorax, advocating for a comprehensive diagnostic approach and the crucial role of a multidisciplinary team in the management of such intricate cases.

Abstract:Re-expansion pulmonary edema is a rare and potentially life-threatening complication that can occur after the rapid re-expansion of a collapsed lung due to pneumothorax or pleural effusion. It has a multifactorial pathogenesis, and risk factors for re-expansion pulmonary edema, such as chronic lung collapse, rapid re-expansion, and changes in pulmonary vascular permeability, have been identified. Clinical manifestations vary, ranging from almost asymptomatic to a rapidly fatal condition, and its incidence may be more common and less fatal than previously believed. The literature emphasizes the importance of early recognition and management to ensure favorable outcomes. However, there is ongoing debate regarding the indications for ventilatory support and the timing of non-invasive or invasive ventilation. Herein, we report a case series of three paradigmatic examples of massive re-expansion pulmonary edema occurring over a period of 10 years in our institution among a population of 815 patients with spontaneous pneumothorax. We also conducted a literature review on re-expansion pulmonary edema, with a particular focus on diagnosis and management. In each case, despite initially normal clinical parameters, severe respiratory distress developed following the insertion of a thoracic drainage tube for a massive spontaneous pneumothorax. Two patients required High-Flow Nasal Oxygen, and one was addressed to intensive management, including CPAP. In all cases, the patient's outcome was optimal.

Abstract:Objective:To investigate the principle of diagnosis,treatment and prevention of reexpansion pulmonary edema(RPE)following thoracotomy.The lung had been shrunk in these operations.Methods:Five patients got RPE,three of those were performed the pleurodesis with thoracoscope,and two patients were performed esophagectomy for treatment of esophageal carcinoma.Results:RPE related to the pulmonary shrink,for the shrinkable pulmonary tissues was hypoxemic and increased the permeability of the pulmonary capillary,hence caused RPE.The pulmonary shirnk caused a greater or lesser degree of RPE in conventional thoracotomy.Conclusion:To reduce the incidence of RPE,it is important to expand the lung intermittently and slowly during operation.

Abstract:The mechanism of re-expansion pulmonary oedema (Re-PE) is unclear. There are multiple variables in play when evaluating the response to evacuation of pleural fluid. We present an educational case of a critically ill patient admitted for respiratory failure who was fully dependent on ventricular pacing set at a constant rate throughout the episode of Re-PE. The transthoracic echocardiography (TTE) showed an ejection fraction of 38%, moderate mitral regurgitation (MR), mildly dilated right ventricle and moderate pulmonary hypertension. A pleural tap evacuated 850 mL of transudate, which was followed by tachypnoea and deteriorating oxygenation. Another repeat TTE revealed a Re-PE with elevated left ventricular end-diastolic pressure, severe MR, increased pulmonary hypertension and a decrease in stroke volume. There were no parallel changes in ventilation modality, heart rate, fluid therapy and vasopressor dosage. The treatment was initiated with dobutamine. The patient was extubated the next day and was later discharged to the cardiology department.

We present a case of RPE following atrial septal defect (ASD) closure using a thoracotomy approach and cardiopulmonary bypass (CPB). REP contributes to significant morbidity and extends the length of stays in the intensive care unit. Understanding the pathophysiology and risk factors of RPE allows us to prevent or minimize the incidence.

Abstract:Tracheostomy is a common procedure seen in critically ill patients that require long term ventilatory support. As with all airway access procedures, tracheotomy with prolonged tracheal tube placement comes with possible risks such as tracheal scarring, tracheal rupture, pneumothorax, tracheoesophageal fistula among others. Another possible complication, though rare, is escape of free air into the surrounding tissue, as well as pneumomediastinum (PM). This may occur due to various reasons, some of them being tracheal rupture, barotrauma or tracheal tube mispositioning. Pneumomediastinum may present with concurrent free air in other body cavities such as the peritoneum, thorax or subcutaneous tissue. Though often not life-threatening it may require treatment including high flow oxygen, ventilator management or occasionally, surgical intervention. Herein we describe a rare case of PM with communicating pneumoperitoneum and massive subcutaneous emphysema due to tracheal tube mispositioning along with a review of the literature.

Abstract:Mechanical ventilation and endotracheal intubation can cause airway damage and inflammation resulting in excessive mucus secretions, thereby increasing the risk of respiratory failure post extubation. An abundance of secretions may result in bronchial obstruction and lung collapse distant from the site of obstruction. If lung collapse is diagnosed, extra support, including oxygen and, rarely, reintubation, can be necessary. The combination of chest wall percussion and vibrations, patient positioning to facilitate mucus drainage, coughing, and breathing exercises was the chest physiotherapy method employed for airway clearance in this study. Since the late 20th century, pulmonary rehabilitation strategies have been a standard aspect of care to prevent lung collapse in postoperative cases. Bronchoscopic aspiration and lavage are the common techniques used to remove retained secretions or mucus plugs. Large-volume saline instillation in aliquots and repeated suctioning are required during the procedure. Thus, the current case series emphasizes the role of bronchoscopy and pulmonary rehabilitation in the management of acute lung atelectasis during the postoperative period.

Abstract:Pneumatoceles are a known complication of pneumonia or trauma, especially in young children. A 44-year-old male with pulmonary veno-occlusive disease and pulmonary hypertension underwent double lung transplantation with cardiopulmonary support. The patient had experienced severe primary graft dysfunction and bilateral lower lobe pneumonia. Posttransplant bronchoalveolar lavage fluid identified Staphylococcus aureus and Burkholderia cepacia . We started trimethoprim-sulfamethoxazole, meropenem, and minocycline. We also switched him to a prone position every 12 h for 3 days. The respiratory condition gradually improved with systemic therapy, prone position, oxygenation with ventilator, and venous-venous extracorporeal membrane oxygenation. Pneumatocele developed at the site of pneumonia. Although the pneumatocele was gradually increasing the size, we decided to continue conservative treatment. The pneumatocele spontaneously ruptured, and asymptomatic pneumothorax developed. We performed percutaneous drainage for pneumothorax, and the pneumatocele resolved. After he was discharged from the hospital, that pneumatocele shrank and disappeared. Pneumatocele can occur at the site of pneumonia after lung transplantation. It may be curable with conservative treatment, but the possibility of sudden rupture and pneumothorax should be considered.

Abstract:Pulmonary embolism (PE), often arising from deep vein thrombosis, can present with pleural effusion. Inserting a chest tube for massive effusions carries inherent risks, including chest tube malposition and methemoglobinemia. We report a case of a 32-year-old female with moderate-to-high risk PE and massive pleural effusion. Complications included chest tube malposition, secondary pneumothorax, and methemoglobinemia. Initial chest tube placement drained pleural fluid but resulted in pneumothorax and methemoglobinemia due to prilocaine use. Careful management and monitoring were crucial. Pleural effusion can complicate PE, warranting interventions, but complications like chest tube malposition and methemoglobinemia should be anticipated and managed promptly.

Abstract:Tube thoracostomy (TT) placement belongs among the most commonly performed procedures. Despite many benefits of TT drainage, potential for significant morbidity and mortality exists. Abdominal or thoracic injury, fistula formation and vascular trauma are among the most serious, but more common complications such as recurrent pneumothorax, insertion site infection and nonfunctioning or malpositioned TT also represent a significant source of morbidity and treatment cost. Awareness of potential complications and familiarity with associated preventive, diagnostic and treatment strategies are fundamental to satisfactory patient outcomes. This review focuses on chest tube complications and related topics, with emphasis on prevention and problem-oriented approaches to diagnosis and treatment. The authors hope that this manuscript will serve as a valuable foundation for those who wish to become adept at the management of chest tubes.

Abstract:OBJECTIVES:Early removal of chest tube is an important step in enhanced recovery after surgery protocols. However, after pulmonary resection with a wide dissection plane, such as pulmonary segmentectomy, prolonged air leak, a large volume of pleural drainage and the risk of developing empyema in patients can delay chest tube removal and result in a low rate of completion of the enhanced recovery after surgery protocol. In this study, we aimed to assess the safety of discharging patients with a chest tube after pulmonary segmentectomy.METHODS:We retrospectively reviewed a single surgeon's experience of pulmonary segmentectomy from May 2019 to September 2019. Patients who fulfilled the criteria for discharging with a chest tube were discharged and provided written instructions. They returned for chest tube removal after satisfactory resolution of air leak or fluid drainage.RESULTS:In total, 126 patients underwent pulmonary segmentectomy. Ninety-five (75%) patients were discharged with a chest tube postoperatively. The mean time to chest tube removal after discharge was 5.6 (range 2-32) days, potentially saving 532 inpatient hospital days. Overall, 90 (95%) patients experienced uneventful and successful outpatient chest tube management. No life-threatening complications were observed. No patient experienced complications resulting from chest tube malfunction. Five (5%) patients experienced minor complications. Overall, all patients reported good-to-excellent mobility with a chest tube.CONCLUSIONS:Successful postoperative outpatient chest tube management after pulmonary segmentectomy can be accomplished in selected patients without a major increase in morbidity or mortality.
Foreign Body Misdiagnosed as Mucus Plugging After Percutaneous Tracheostomy

Abstract:We report a case of a 59-year-old male who presented with a persistent cough for a year after being discharged from critical care following a subarachnoid haemorrhage. As part of his initial critical care management and in order to allow full neurological assessment, the patient required a period of prolonged mechanical ventilation, which necessitated a percutaneous tracheostomy. Following recovery and subsequent discharge, the patient presented on multiple occasions with cough, undergoing serial computed tomography (CT) scans which reported mucus plugging as a possible cause of the cough. As his symptoms continued to worsen, a flexible bronchoscopy was carried out, which identified a foreign body in the trachea. This object was later recognised as a retained part of the guiding catheter, part of the percutaneous tracheostomy tube dilator. After the object was retrieved, the patient reported a complete resolution of symptoms. Percutaneous tracheostomy is a common procedure within critical care units, and early complications such as bleeding or airway obstruction are typically recognised immediately after insertion. This report documents a late complication caused by the retention of a foreign object from insertion, which was misdiagnosed on serial CT scans, leading to persistent cough over a period of months.

Abstract:The Society for Translational Medicine and The Chinese Society for Thoracic and Cardiovascular Surgery conducted a systematic review of the literature in an attempt to improve our understanding in the postoperative management of chest tubes of patients undergoing pulmonary lobectomy. Recommendations were produced and classified based on an internationally accepted GRADE system. The following recommendations were extracted in the present review: (I) chest tubes can be removed safely with daily pleural fluid of up to 450 mL (non-chylous and non-sanguinous), which may reduce chest tube duration and hospital length of stay (2B); (II) in rare instances, e.g., persistent abundant fluid production, the use of PrRP/B <0.5 when evaluating fluid output to determine chest tube removal might be beneficial (2B); (III) it is recommended that one chest tube is adequate following pulmonary lobectomy, except for hemorrhage and space problems (2A); (IV) chest tube clearance by milking and stripping is not recommended after lung resection (2B); (V) chest tube suction is not necessary for patients undergoing lobectomy after first postoperative day (2A); (VI) regulated chest tube suction [−11 (−1.08 kPa) to −20 (1.96 kPa) cmH2O depending upon the type of lobectomy] is not superior to regulated seal [−2 (0.196 kPa) cmH2O] when electronic drainage systems are used after lobectomy by thoracotomy (2B); (VII) chest tube removal recommended at the end of expiration and may be slightly superior to removal at the end of inspiration (2A); (VIII) electronic drainage systems are recommended in the management of chest tube in patients undergoing lobectomy (2B).
Study on Postoperative Ventilation-Associated Pneumonia and Its Risk Factors in Children with Congenital Heart Disease

Abstract:Objective: To identify the postoperative ventilation associated pneumonia (VAP) and its risk factors in children with congenital heart diseases (CHD). Methods: Of 950 CHD patients undergoing cardiac surgery during a 3 year period in our hospital, 62 cases required prolonged ventilation (≥3 days). We studied the development of VAP among them and identified its risk factor. Results: All 62 cases were discharged, 39 of 62 cases( 62 9%) developped VAP. VAP occurred (4 79±2 81)day after operation. Culture of tracheal aspirate was positive in 11 cases (28 21%) on (5 67±2 90) postoperative day. Gram negative bacilli were the most frequent causes of VAP. Using logistic regression analysis, three factors were associated with the risk of VAP: chronic preoperative pneumonia, low plasma immunologic protein and use of histamine 2 receptor blockade postoperatively. Using nonparametric tests, prolonging of ventilation frequently caused VAP (P0 001). Conclusion: Chronic preoperative pneumonia, low plasma immunologic protein level, prolonged mechanical ventilation, with use of histamine 2 receptor blockade are closely related with development of VAP.
Predictors of surgical intervention in first episode primary spontaneous pneumothorax requiring chest drain insertion

Abstract:Background: Primary spontaneous pneumothorax occurs in patients with no underlying lung disease and guidelines recommend chest tube drainage for the first episode, with surgical intervention reserved for recurrent episodes, persistent air leak or failure of lung re-expansion. Modern surgical management is associated with reduced length of hospital stay and superior freedom from recurrence compared with chest tube drainage alone. The objective of this study was to identify risk factors for failed chest tube drainage in patients who present with first episode primary spontaneous pneumothorax. Methods: A retrospective analysis of patients who presented to Royal Prince Alfred Hospital, Australia with first episode PSP and underwent chest tube insertion was performed. Patient demographics and size of pneumothorax were examined in relation to the primary outcome, a composite of failed chest tube drainage and recurrent ipsilateral pneumothorax. Results: Fifty-five patients underwent chest tube drainage for first episode primary spontaneous pneumothorax between 1st January 2017 and 31st December 2020. Complete lung collapse on admission chest x-ray was associated with an increased risk of the primary outcome (63% versus 19%, OR 7.3 [96% CI 2.0-27.4), P = 0.004). Conclusion: This small retrospective study found that patients that undergo chest drain insertion for first episode primary spontaneous pneumothorax who present with complete lung collapse on admission are at high risk of requiring pleurodesis and therefore may benefit from early surgical referral.

Abstract:Respiratory complications occurring in the immediate postoperative period are well known to the seasoned postanesthesia care unit nurse. The most common adverse respiratory events originating in this setting are airway obstruction, hypoventilation, hypoxemia, and pulmonary aspiration of gastric contents. The focus of this article details airway compromise secondary to edema of the larynx and adjacent structures as a consequence of translaryngeal intubation. Postextubation laryngeal edema is a relatively rare problem; however, severe episodes may have life-threatening ramifications. A review of pertinent airway anatomy and airflow dynamics as they relate to this compromised airway condition is presented. Risk factors for the development of postextubation laryngeal edema plus contemporary patient treatment strategies will be reinforced. Patient management issues are addressed, with emphasis placed on the ambulatory patient in which discharge to a remote location is anticipated.

Secondary Spontaneous Pneumothorax in a Patient With Interstitial Lung Disease Undergoing Routine Outpatient Pulmonary Function Tests

Abstract:We present the case of a 64-year-old female with a past medical history significant for unclassified interstitial lung disease (ILD) from suspected hypersensitivity pneumonitis secondary to chronic mold exposure with steroid responsiveness and prior pneumothorax. The patient developed shortness of breath and pleuritic chest pain after undergoing routine outpatient pulmonary function tests (PFTs). She was immediately transferred to the emergency department and found to have a moderate left basilar pneumothorax. She underwent emergent surgical chest tube placement followed by doxycycline pleurodesis. Repeat chest imaging showed inadvertent retraction of the chest tube and extensive subcutaneous emphysema. The surgical chest tube was replaced by a pigtail catheter with an improvement of subcutaneous emphysema. This case demonstrates the development of a rare but serious complication of pneumothorax that could occur in patients who have ILD undergoing routine PFTs. Clinicians should be aware of this risk when patients who have ILD present for PFTs and counsel them to seek immediate medical attention if they develop signs of acute onset dyspnea after performing PFTs.
