Abstract:The development of pulmonary atelectasis is common in the surgical patient. Pulmonary atelectasis can cause various degrees of gas exchange and respiratory mechanics impairment during and after surgery. In its most serious presentations, lung collapse could contribute to postoperative respiratory insufficiency, pneumonia, and worse overall clinical outcomes. A specific risk assessment is critical to allow clinicians to optimally choose the anesthetic technique, prepare appropriate monitoring, adapt the perioperative plan, and ensure the patient's safety. Bedside diagnosis and management have benefited from recent imaging advancements such as lung ultrasound and electrical impedance tomography, and monitoring such as esophageal manometry. Therapeutic management includes a broad range of interventions aimed at promoting lung recruitment. During general anesthesia, these strategies have consistently demonstrated their effectiveness in improving intraoperative oxygenation and respiratory compliance. Yet these same intraoperative strategies may fail to affect additional postoperative pulmonary outcomes. Specific attention to the postoperative period may be key for such outcome impact of lung expansion. Interventions such as noninvasive positive pressure ventilatory support may be beneficial in specific patients at high risk for pulmonary atelectasis (e.g., obese) or those with clinical presentations consistent with lung collapse (e.g., postoperative hypoxemia after abdominal and cardiothoracic surgeries). Preoperative interventions may open new opportunities to minimize perioperative lung collapse and prevent pulmonary complications. Knowledge of pathophysiologic mechanisms of atelectasis and their consequences in the healthy and diseased lung should provide the basis for current practice and help to stratify and match the intensity of selected interventions to clinical conditions.
Lung Expansion Therapy for the Potential Populations: A Literature Review

Abstract:Secretion retention, atelectasis, and reduced lung compliance occur when endotracheal or tracheostomised patients cough ineffectively. There is a possibility of infection if the collapsed regions of the lungs are not reinflated. Therefore, to improve clinical outcomes, such as diaphragm mobility and thickness, lung volume, and thickness of the abdominal muscles, and decrease the length of hospitalizations, mechanical and manual techniques, such as balloon blowing exercises and incentive spirometer, are required. PubMed, Google Scholar, Pedro, Clinical Keys, Helinet, ProQuest, and Science Direct databases were used for the literature search considering the inclusion and exclusion criteria. The several manual and mechanical methods that were employed for lung expansion treatment for the potential populations are highlighted in this overview of the literature. Ten studies were considered in this review: five on balloon-blowing exercises, four on tracheostomy with incentive spirometry exercises, and one on incentive spirometry with balloon-blowing exercises. The effects were examined on individual outcomes that included rate of perceived exertion (RPE), diaphragm mobility, pulmonary function, volume of breath, length of hospitalization, and postoperative day complications. The structured protocols proved to be effective in improving lung expansion and pulmonary function for the potential population that involved healthy adults, noncritical COVID-19 adults, smokers, thoracotomy patients, and tracheostomised patients. The road to recovery is yet unexplored and underachieved because of the lack of evidence.
[Effect of Bronchial Intubation for Constant-Pressure Expanding Ipsilateral Lung on Postoperatively Intractable Atelectasis in Hypo-Analgesia and Muscle Relaxation].

Abstract:OBJECTIVE:To investigate the effect of bronchial intubation for constant-pressure expanding ipsilateral lung on postoperative intractable atelectasis.METHODS:For this prospective study, we recruited 18 patients with pulmonary atelectasis who could not been relieved by bronchoscopic suctioning, closed thoracic drainage, backslap, blowing hall and other routine treatments for over a week. After bronchial intubation, ipsilateral lung was expanded with a constant pressure. And the therapeutic effect was evaluated by chest radiographic examination and auscultation at the following day.RESULTS:Collapsed lung tissue were examined in 15 patients (83.3%) after the first treatment and in 2 patients (11.1%) after twice inflation. And another case failed even after three times treatment. During the procedure, the vital signs of all patients were stable and no complication occurred.CONCLUSION:Constant-pressure expanding of ipsilateral lung during bronchial intubation is a safe and effective treatment for postoperative intractable atelectasis.

Abstract:An intensive treatment of patients undergoing thoracic surgery is important, foremost because of the extensity of the surgical procedures and the generally poor condition of the patients. As a first stage of preoperative preparation an evaluation of the functional capacity of the vital organs (heart, lungs and kidneys) is performed, and the most important infection's focci of the oro-pharynx, tracheobronchial tree, urinary tract and skin have to be detected and treated. Respiratory physiotherapy before the surgery improves the ventilatory function, enabling the patient to breath regularly and effectively cough, wherewith a bronchial spasm is prevented and bronchopulmonary infection limited. Before surgery any hypovolaemia, anaemia, hypoproteinemia and dysproteinaemia should also be corrected; in such patients the parenteral alimentation (hyperalimentation) through the central venous catheter, is also important. Immediately following the operation a continuous supervision of vital functions (usually managed by well-experienced surgical nurses) is very essential. Isothermia, isovolemia, a correct oxygenation and analgesia should be maintained permanently. To loose sight of hypoventilation and hypoxia can likely induce respiratory insufficiency. Symptoms indicating tracheal intubation and mechanical ventilation should be watched for and treated at the right moment. Following the surgery, prevention of pulmonary atelectasis and pneumonia, providing an effective thoracic drainage, and respiratory physiotherapy is of utmost importance. The prophylaxis of postoperative pulmonary embolism in particularly jeopardized patients consists in the administration of heparin. Antibiotics in accordance with antibiogram (material: samples taken by a catheter or by bronchoscope from the lung directly).
Patient in Mayfield Head Pins Requiring Prone Bronchoscopy for Mucus Plug Obstruction: A Case Report

Abstract:Mucus plug obstruction is a common complication in prone patients associated with loss of ventilation and hemodynamic instability. This case presents a 62-year-old female with chronic bronchitis who underwent posterior cervical fusion for a type III dens fracture with extension into the pars articularis and pedicles. Glycopyrrolate was administered to assist with fiberoptic intubation. After successful intubation, bronchoscopy revealed copious endotracheal secretions requiring preoperative therapeutic removal. Despite extensive removal of thick endotracheal secretions preoperatively, obstructive mucus plugging developed intraoperatively with complete loss of end-tidal carbon dioxide (ETCO2) while the patient was in Mayfield head pins. With limited airway access, suctioning and prone flexible bronchoscopy were performed, successfully restoring ETCO2. This experience underscores the need for heightened awareness and preparedness for mucus plug obstruction in chronic bronchitis patients undergoing prone cervical spine surgeries.

Abstract:Postoperative pulmonary edema is a well-known postoperative complication caused as a result of numerous etiological factors which can be easily detected by a careful surveillance during postoperative period. However, there are no preoperative and intraoperative criteria which can successfully establish the possibilities for development of postoperative pulmonary edema. The aims were to review the possible etiologic and diagnostic challenges in timely detection of postoperative pulmonary edema and to discuss the various management strategies for prevention of this postoperative complication so as to decrease morbidity and mortality. The various search engines for preparation of this manuscript were used which included Entrez (including Pubmed and Pubmed Central), NIH.gov, Medknow.com, Medscape.com, WebMD.com, Scopus, Science Direct, MedHelp.org, yahoo.com and google.com. Manual search was carried out and various text books and journals of anesthesia and critical care medicine were also searched. From the information gathered, it was observed that postoperative cardiogenic pulmonary edema in patients with serious cardiovascular diseases is most common followed by noncardiogenic pulmonary edema which can be due to fluid overload in the postoperative period or it can be negative pressure pulmonary edema (NPPE). NPPE is an important clinical entity in immediate post-extubation period and occurs due to acute upper airway obstruction and creation of acute negative intrathoracic pressure. NPPE carries a good prognosis if promptly diagnosed and appropriately treated with or without mechanical ventilation.

Noninvasive clearance of airway secretions

Abstract:Airway clearance techniques are indicated for specific diseases that have known clearance abnormalities (Table 2). Murray and others have commented that such techniques are required only for patients with a daily sputum production of greater than 30 mL. The authors have observed that patients with diseases known to cause clearance abnormalities can have sputum clearance with some techniques, such as positive expiratory pressure, autogenic drainage, and active cycle of breathing techniques, when PDPV has not been effective. Hasani et al has shown that use of the forced exhalatory technique in patients with nonproductive cough still resulted in movement of secretions proximally from all regions of the lung in patients with airway obstruction. It is therefore reasonable to consider airway clearance techniques for any patient who has a disease known to alter mucous clearance, including CF, dyskinetic cilia syndromes, and bronchiectasis from any cause. Patients with atelectasis from mucous plugs and hypersecretory states, such as asthma and chronic bronchitis, patients with pain secondary to surgical procedures, and patients with neuromuscular disease, weak cough, and abnormal patency of the airway may also benefit from the application of airway clearance techniques. Infants and children up to 3 years of age with airway clearance problems need to be treated with PDPV. Manual percussion with hands alone or a flexible face mask or cup and small mechanical vibrator/percussors, such as the ultrasonic devices, can be used. The intrapulmonary percussive ventilator shows growing promise in this area. The high-frequency oscillator is not supplied with vests of appropriate sizes for tiny babies and has not been studied in this group. Young patients with neuromuscular disease may require assisted ventilation and airway oscillations can be applied. CPAP alone has been shown to improve achievable flow rates that will increase air-liquid interactions for patients with these diseases or airway malacia. Use of positive pressure to maintain airway patency in these children allows cephalad clearance of secretions. Patients with segmental atelectasis, particularly related to asthma, may benefit from intrapulmonary percussive ventilator, positive expiratory pressure, or PDPV. Prevention of postoperative atelectasis is particularly well suited to positive expiratory pressure, which is not as painful as techniques using oscillations. Neurologically abnormal patients who are unable to cooperate with any active method are also treated using intrapulmonary percussive ventilator, PDPV, and suctioning, if necessary. Musculoskeletal abnormalities, muscular dystrophies, myasthenia gravis, poliomyelitis, or other similar diseases require stabilization of bellows function. Optimizing ventilation in patients with such abnormalities may require positive pressure ventilation either during sleep or continuously. Externally applied pressure, such as with the In-Exsufflator or the cyclically inflated pneumatic belt, can augment the patient's own efforts and is sometimes helpful. Normalizing the vital capacity and functional residual capacity typically helps to improve the ability to cough and clear secretions. Assisted cough devices or maneuvers are described in other papers by Bach and Hill. Not all patients who have weak muscles require nocturnal or continuous support, and may benefit from positive expiratory pressure mask treatments. Further studies are sorely needed for this population. Long-term controlled trials are urgently needed to help establish the best types of treatment for patients with CF and bronchiectasis. Such studies will become more complicated by the introduction of new treatments, such as DNase and other therapies that alter secretions, and may begin to change mucociliary or cough clearance. The selection of appropriate outcome measures is central to studying these questions, and it is unclear which are the most important. (ABSTRACT TRUNCATED)

Strange Saucer-Shaped Lung Collapse Successfully Salvaged in Spite of Delayed Intervention

Abstract:A middle-aged woman presented in November 2023 with exertional dyspnea and a chronic cough for three months. She had undergone a repeat mitral valve replacement (MVR) surgery five months prior. She had a tissue MVR in 2016, which degenerated, making her symptomatic, and hence had to be replaced with a metallic valve. As the respiratory symptoms recurred two months post-op, she was evaluated for the integrity of the newly placed mitral valve, which was found to be functioning well. The left ventricular function was well preserved, and she had no vegetation or clots. She was therefore investigated further for other possible causes. Chest X-ray showed a strange saucer-shaped (or disc-shaped) opacity above the middle portion of the right hemidiaphragm, which itself was found to be elevated. An ultrasound of the chest ruled out subpulmonic pleural effusion and confirmed right hemidiaphragm palsy. A computed tomography (CT) scan of the thorax was suggestive of a strange-shaped collapse of the right lower lobe with tortuous air bronchograms and a small intraluminal soft tissue shadow in the right lower lobe bronchus. A bronchoscopy confirmed collapsing segments of the right lower lobe due to external compression. It also ruled out any intrabronchial pathology causing obstruction, effectively confirming that the tissue shadow was probably just a mucus plug. A possible phrenic nerve injury during thoracotomy at the time of MVR was thus concluded. A diaphragmatic plication was advised considering that she had significant orthopnea and low peripheral oxygen saturation and that the collapsed lung would not possibly expand beyond six months or so and could in itself act as a focus for further mucus stagnation, leading to infection and further bronchiectasis, which had probably already started developing. It was difficult to persuade the patient for a third thoracotomy and she took much time to decide. Good counselling, rapport building, and assurance that, although the lung may not fully expand beyond six months, at least the orthopnea would significantly improve, she finally consented to diaphragmatic plication, which was done after 10 months of the MVR surgery. Aggressive post-operative chest physiotherapy and rehabilitation were promptly initiated. The lung completely expanded one month post-op and was thus successfully salvaged.

Abstract:We have investigated the movement of green fluorescent protein-tagged neurofilaments at the distal ends of growing axons by using time-lapse fluorescence imaging. The filaments moved in a rapid, infrequent, and asynchronous manner in either an anterograde or retrograde direction (60% anterograde, 40% retrograde). Most of the anterograde filaments entered the growth cone and most of the retrograde filaments originated in the growth cone. In a small number of cases we were able to observe neurofilaments reverse direction, and all of these reversals occurred in or close to the growth cone. We conclude that neurofilament polymers are delivered rapidly and infrequently to the tips of growing axons and that some of these polymers reverse direction in the growth cone and move back into the axon. We propose that 1) growth cones are a preferential site of neurofilament reversal in distal axons, 2) most retrograde neurofilaments in distal axons originate by reversal of anterograde filaments in the growth cone, 3) those anterograde filaments that do not reverse direction are recruited to form the neurofilament cytoskeleton of the newly forming axon, and 4) the net delivery of neurofilament polymers to growth cones may be controlled by regulating the reversal frequency.

Abstract:Extract For people with bronchiectasis, achieving long-term adherence to physiotherapy strategies is a major challenge. The regular use of airway clearance techniques (ACTs) reduces the burden of sputum-related symptoms ( e.g. increased expectoration during ACT session and reduced need to expectorate/cough throughout the rest of the day), improves quality of life and may reduce exacerbations, according to a recent European Respiratory Society statement [1]; however, there are barriers to long-term implementation (figure 1). There are traditional ( e.g. positioning, manual percussions and vibrations) and contemporary strategies ( e.g. breathing techniques and devices providing vibrations and/or positive or negative pressures) for airway clearance [1]. The selection of the most appropriate ACT for an individual patient is usually determined by respiratory physiotherapists, with selection based on clinical judgement and the patient's characteristics, symptoms, understanding and preferences [2].

Abstract:Atelectasis occurs due to impaired airway clearance or complete airway obstruction caused by inflammation, leading to the collapse of tiny airways either partially or completely. This results in impaired exchange of carbon dioxide and oxygen. A chest X-ray, Computed Tomography (CT), and/or thoracic ultrasound are helpful in identifying atelectasis. In the present case, a 5-month-old female infant was reported to the Paediatric Department with the chief complaint of seizure and loss of consciousness. An X-ray was performed, revealing complete left lung atelectasis with no air entry. The infant was referred for physiotherapy in addition to medical management. Through seven intensive and regular sessions of respiratory physiotherapy, which included techniques such as percussion, vibration, and postural drainage for more than 30 minutes, there was successful resolution of the consolidation and improvement in presenting symptoms. After the final sessions of physiotherapy, the infant was discharged with physiotherapy home programs. This suggests that physiotherapy is an integral part of paediatric critical care, ensuring a good prognosis and faster recovery.

Abstract:The management of bronchial secretions is one of the main problems encountered in a wide spectrum of medical conditions ranging from respiratory disorders, neuromuscular disorders and patients undergoing either thoracic or abdominal surgery. The purpose of this review is illustrate to the reader the different ACTs currently available and the related evidence present in literature. Alongside methods with a strong background behind as postural drainage, manual techniques or PEP systems, the current orientation is increasingly aimed at devices that can mobilize and / or remove secretions. Cough Assist, Vacuum Techniques, systems that modulate airflow have more and more scientific evidence. Different principles combination is a new field of investigation that goes toward an increasing of clinical complexity that will facing us.

Cholecystectomy in a patient with Broncho pleural fistula - An Anesthesiologist's tight rope walk

Abstract:Patients presenting for surgery postlobectomy with Broncho pleural fistula are prone for perioperative respiratory complications and pose a significant challenge to anesthetist. Published data to guide perioperative management of such cases especially for abdominal surgeries are scarce. We describe a 51-year-old gentleman status postlobectomy with Broncho pleural fistula posted for cholecystectomy in view of symptomatic gallstones. Laparoscopic surgery was later converted to open surgery in view of surgical difficulties. Perioperative period was uneventful and proper modes of analgesia helped in faster recovery. Proper understanding of physiological and anatomical changes and proper planning of anesthesia facilitated safe and uneventful anesthesia.

Abstract:Background: Respiratory diseases such as bronchopneumonia contribute to the highest cause of death in children aged less than two years. Airway clearance becomes a problem due to the accumulation of secretions, resulting in airway obstruction. Independent nursing interventions such as chest physiotherapy can be carried out to free the airway and reduce the burden of breathing. Objective: This study aimed to analyze the application of chest physiotherapy intervention in bronchopneumonia children with airway clearance problems. Method: The research design uses a case study with a nursing process approach. The sample for this study was one child patient aged nine months with bronchopneumonia who was taken using a convenience sampling technique. Data was collected using interviews, observation, and documentation studies using pediatric nursing care format instruments. Chest physiotherapy intervention was carried out three times with a duration of 10-15 minutes with a combination of postural drainage, which was then evaluated before and after the procedure. Results: Chest physiotherapy intervention in patients can improve coughing ability, increase sputum production and respiratory rate 40 times per minute, and decrease rhonchi and oxygen saturation by 96%. Conclusion: Chest physiotherapy can be an effective independent nursing intervention to overcome the problem of ineffective airway clearance nursing in children with bronchopneumonia.

Abstract:Surgical resection is frequently the intervention required for post-tuberculous empyema or other sequels. However, pneumonectomy may not be feasible in some situations, and video-assisted thoracoscopic surgery (VATS) plays a role in such a scenario. Whether a patient undergoes open resection of VATS, isolation of infected lung is integral to one-lung ventilation and better access to the surgical field, and a double-lumen tube (DLT) remains the preferred choice. Difficulties in DLT placement after pneumonectomy are reported; however, failure to isolate a lung by appropriately placed DLT is scarce or absent. A 28-year cachectic gentleman with poor preoperative lung function was suffering from endobronchial tuberculosis. He also had one episode of tuberculosis twelve-year back. At presentation, he had a massive pneumothorax and stage-III empyema as a sequel, including a rare finding of plastered mediastinum mimicking vanishing lung syndrome. He underwent uniportal-VATS under general anesthesia using one-lung ventilation. Complete lung destruction from active tuberculosis and its sequel leading to the plastered mediastinum and deformed airway pose a significant lung isolation challenge. U-VATS can be considered for therapeutic purpose where standard thoracotomy and pneumonectomy is contra-indicated. However, lung isolation in such patients is tricky and poses a risk. The present case highlights the challenges faced with lung isolation using a DLT and discusses the probable remedy to these problems.

Abstract:BACKGROUND AND PURPOSE: This case report describes the physiotherapy assessments and interventions provided to a 56-year-old male in an Australian intensive care unit. The non-intubated patient suffered from respiratory deterioration, secretion retention, complete opacification of the left hemithorax on chest X-ray (CXR) and widespread coarse crackles on lung auscultation. The addition of lung ultrasound (LUS) facilitated more accurate diagnosis than what was formed on the basis of CXR and lung auscultation resulting in more effective treatment.METHODS: The method used is a case report. Written consent was obtained from the patient for these data to be published.SUMMARY: Secretion retention was initially suspected (based on clinical examination and CXR) and treated with airway clearance techniques, patient positioning and nasopharyngeal suction. Even though this assisted with secretion removal, the patient continued to have a high perceived work of breathing and increasing oxygen requirements. Physiotherapist performed LUS, under supervision, and revealed a large pleural effusion (1500 mL), which was then drained by the medical team.DISCUSSION: The addition of LUS to the critical care physiotherapist's assessment skills may provide clinical benefit. The increased diagnostic accuracy of LUS compared with other routine assessments warrants further investigation.

Abstract:CONTEXT AND OBJECTIVE: Abdominal surgical procedures increase pulmonary complication risks. The aim of this study was to evaluate the effectiveness of chest physiotherapy during the immediate postoperative period among patients undergoing elective upper abdominal surgery. DESIGN AND SETTING: This randomized clinical trial was performed in the post-anesthesia care unit of a public university hospital. METHODS: Thirty-one adults were randomly assigned to control (n = 16) and chest physiotherapy (n = 15) groups. Spirometry, pulse oximetry and anamneses were performed preoperatively and on the second postoperative day. A visual pain scale was applied on the second postoperative day, before and after chest physiotherapy. The chest physiotherapy group received treatment at the post-anesthesia care unit, while the controls did not. Surgery duration, length of hospital stay and postoperative pulmonary complications were gathered from patients' medical records. RESULTS: The control and chest physiotherapy groups presented decreased spirometry values after surgery but without any difference between them (forced vital capacity from 83.5 ± 17.1% to 62.7 ± 16.9% and from 95.7 ± 18.9% to 79.0 ± 26.9%, respectively). In contrast, the chest physiotherapy group presented improved oxygen-hemoglobin saturation after chest physiotherapy during the immediate postoperative period (p < 0.03) that did not last until the second postoperative day. The medical record data were similar between groups. CONCLUSIONS: Chest physiotherapy during the immediate postoperative period following upper abdominal surgery was effective for improving oxygen-hemoglobin saturation without increased abdominal pain. Breathing exercises could be adopted at post-anesthesia care units with benefits for patients.

EFFICIENT RESULTS OF PHYSIOTHERAPY REHABILITATION IN HYDRO PNEUMOTHORAX

Abstract:Hydro pneumothorax is a rare variant type of pneumothorax. It occurs when both free fluid and air are within the pleural space. The goal of treating a hydropneumothorax is to relieve the pressure on your lung, allowing it to re-expand. Treatment options may include observation, needle aspiration, chest tube insertion, non-surgical repair or surgery along with a very important role of physiotherapy. A 54-year male patient came to the hospital with complaints of breathlessness (MMRC grade II), dry cough, and high-grade fever for one month. The patient had a history of alcohol consumption and tobacco chewing for 10 years. Laboratory investigations showed a WBC count 18,600/ml. Complete blood count gave normal results. The arterial blood gases revealed PO2 as 69. Chest X-ray which showed fairly large opacities with the fluid level in right side. The costophrenic angle on the right side is obliterated. Multiple, small patchy opacities are also seen in the left lower zones. Along with these findings, there was a contralateral mediastinal shift. Physiotherapy intervention included patient education, breathing retraining, airway clearance techniques, positioning, walking program with supplemented oxygen and psychological support. Outcome measures have shown improvement in functional independence and performance of activities of daily living. Physiotherapy management showed efficient results in improving lung function, quality of life, inducing relaxation, and early return to pre-disease life in patient with hydro pneumothorax.
Clinical review: post-extubation laryngeal edema and extubation failure in critically ill adult patients

Abstract:Laryngeal edema is a frequent complication of intubation. It often presents shortly after extubation as post-extubation stridor and results from damage to the mucosa of the larynx. Mucosal damage is caused by pressure and ischemia resulting in an inflammatory response. Laryngeal edema may compromise the airway necessitating reintubation. Several studies show that a positive cuff leak test combined with the presence of risk factors can identify patients with increased risk for laryngeal edema. Meta-analyses show that pre-emptive administration of a multiple-dose regimen of glucocorticosteroids can reduce the incidence of laryngeal edema and subsequent reintubation. If post-extubation edema occurs this may necessitate medical intervention. Parenteral administration of corticosteroids, epinephrine nebulization and inhalation of a helium/oxygen mixture are potentially effective, although this has not been confirmed by randomized controlled trials. The use of non-invasive positive pressure ventilation is not indicated since this will delay reintubation. Reintubation should be considered early after onset of laryngeal edema to adequately secure an airway. Reintubation leads to increased cost, morbidity and mortality.
