Abstract:Carinal resection of tumour involving trachea and carina remains as a challenge for thoracic surgeons and anaesthesiologists. Resection is technically demanding and can be associated with significant morbidity and mortality. In this case report, we describe the successful management of carinal tumour with carinal resection in a 45-year-old female. The tumour was involving lowermost trachea, carina and bilateral primary bronchi causing 60% narrowing of the lower trachea just before carina, more than 90% narrowing of right main bronchus and 50% luminal narrowing of left main bronchus. Carinal resection and reconstruction were successfully performed under general anaesthesia. Patient was managed with conventional orotracheal intubation with Micro laryngeal endotracheal tube and positioned in left principal bronchus railroaded over a paediatric bronchoscope for lung isolation. After thoracotomy, the left main bronchus was intubated directly across the operative field with a sterile flexometallic endotracheal tube. With intermittent ventilation, anastomosis was completed. During anastomosis Micro laryngeal endotracheal tube cuff was damaged twice and we had to reintubate the patient twice in lateral position itself. At the end of anastomoses, flexometallic tube was removed and wound repaired. After confirming no leakage at anastomotic site, Micro laryngeal endotracheal tube was removed and Laryngeal Mask Airway was inserted and bronchial toileting done with adult bronchoscope. Meticulous planning and communication between the anaesthesia and surgical teams are mandatory for the safe and successful anaesthetic management of carinal resection surgeries.

Abstract:BACKGROUND: Supraglottic airway devices (SADs) may have advantages over endotracheal intubation for tracheal resection and reconstruction in cases of severe and proximally located subglottic stenosis. This retrospective case series examines the feasibility of using SADs as a novel approach to airway management in tracheal resections. METHODS: All patients who were managed with SADs for cervical tracheal resection and reconstruction during the study period (2010–2015) in our university hospital were included. To examine the feasibility of airway management with SADs for tracheal resection, medical records were obtained from our institution’s electronic database and reviewed. RESULTS: SADs were used in 10 patients who had extensive tracheal stenosis and a high prevalence of severe comorbidities. SAD insertion and subsequent positive pressure ventilation were successful in all patients, although 1 patient with preoperative respiratory failure had persistent hypercarbia. During the phase of resection and reconstruction, high-frequency jet ventilation was used to ensure adequate oxygenation. There were no intraoperative complications related to anesthetic management, apart from transient hypercarbia during and after jet ventilation. Most patients (n = 6; 60%) had an uneventful postoperative course. In this high-risk cohort, postoperative complications (ie, vocal cord edema, postoperative hemorrhage, pneumonia) occurred in 4 patients (40%). CONCLUSIONS: This retrospective case series demonstrates the feasibility of using supraglottic airways alongside high-frequency jet ventilation for airway management in at least some cases of cervical tracheal resection and reconstruction. However, the small number of cases examined limits conclusions regarding indications, contraindications, and periprocedural safety.

In this review, emphasis is placed on anesthesia challenges for patients undergoing major tracheal or carinal surgery with specific considerations related to perioperative management.

Abstract:A retrospective review of 69 tracheal and cricotracheal resections at a single institution was performed; nature and timing of complications—including hematoma, anastomotic dehiscence, reintubation, revision tracheostomy, and death—were reviewed. Most complications occur within 8 days of surgery, and restenosis usually occurs within 1–3 months post‐operatively. Objective Tracheal resection (TR) and cricotracheal resection (CTR) are performed for patients with airway stenosis, tracheal tumor, and tracheoesophageal fistula. Post‐operative complications include airway edema requiring reintubation, hematoma, anastomotic dehiscence, restenosis, and death. Although these complications and associated risk factors have been well described, the time where clinical suspicion should be highest post operatively has not been characterized. Methods Patients who underwent TR or CTR at a single center between 2015 and 2022 were reviewed. Variables including demographics and comorbidities were recorded. Rate, nature, and time in days of post‐operative complications were evaluated. Results Sixty‐nine cases were reviewed. Average patient age was 46.8 years old and 63.8% were male. The average follow‐up period was 625 ± 724 days. 19 (27.5%) patients experienced one or more major complications including four (5.8%) who died. Eight (11.6%) patients required reintubation and 4 (5.8%) patients underwent revision tracheostomy. Most complications occurred within 8 days of surgery. Restenosis was noted an average of 42.6 days after surgery, with no new restenosis occurring after 3 months. Conclusions In this single‐center study, most post‐operative complications after TR or CTR, including hematoma and anastomotic dehiscence, occurred within 8 days post‐operatively. Restenosis was noted approximately 1–3 months after surgery. This may inform clinical decision‐making regarding patient monitoring and surveillance after open airway surgery. Abstract:Purpose: Tracheostomy is an extremely common procedure performed by a variety of surgical specialties. The purpose of the present study was to review the intraoperative and perioperative management and complications, present our surgical technique, and discuss the role of our service in providing this care within a large community hospital setting. Patients and methods: The 112 patients in our retrospective study were divided into 3 subsets: those referred by medical specialties, tumor/reconstructive surgery patients, and trauma victims. Cases of percutaneous dilational and intensive care unit bedside tracheostomy were excluded. Intraoperative and immediately postoperative complications were included. Bleeding complications were defined as those necessitating a return to the operating room. The patients were followed up for a 24-hour period postoperatively. Results: The medical referral, tumor/reconstructive, and trauma patients made up 55%, 29%, and 16% of the included patients, respectively. The overall complication rate was 2.7%. Conclusions: Conventional open tracheostomy in an operating room is associated with a low complication rate. The low incidence of perioperative bleeding can be attributed to the use of electrocautery in the division of the thyroid isthmus. This service provided an exceedingly safe and efficient surgical treatment by focusing on precise surgical protocols in an operating room setting. Intense coordination of consultation response, operating room scheduling, and communication with other services involved in these patients' care is critical to develop and maintain the privilege to provide this treatment. Our report can be used to educate the medical community regarding the role of an oral and maxillofacial surgery service in providing tracheostomy.
Predictors of Anastomotic Complications After Resection and Anastomosis for Tracheal Stenosis

Abstract:Resection and anastomosis is an effective option for the management of complex tracheal stenosis, however, it’s not without the complications. This study aims at evaluating various factors predicting anastomotic complications after trachea resection and anastomosis. This is a retrospective analysis of database from a dedicated thoracic surgical unit in New Delhi, India over 7 years. An analysis of demographic details, perioperative variables including complications were carried out. Analysis of various factors predicting anastomotic complications was performed. Out of 65 patients in the study, 49 (75.3%) were males and 16 (24.7%) were females. Median age of the patients was 31 years. Stenosis was cervical in 80%, cervico-thoracic in 15.4% and thoracic in 4.6% of patients. Median length of stenosis was 2.9 cm (1–4.2). 53 (81.6%) patients had some kind of preoperative intervention, where as rest 12 (18.4%) patients had no intervention at all. Out of 65 patients, 26 (40%) had crico-tracheal anastomosis while 39 (60%) had tracheo-tracheal anastomosis. Median length of resected tracheal segment was 3.3 cm (1–5). Overall complication rate (anastomotic + non-anastomotic) was 18.4% in which anastomosis related were in 4 (6.1%) patients. Resection of tracheal segment ≥$$\ge $$ 3.5 cm, presence of diabetes mellitus and pre-operative use of corticosteroids were statistically significant factors for the onset of complications. Perioperative mortality rate was 1.5% (n = 1). Length of resection > 3.5 cm, presence of diabetes mellitus and pre-operative prolonged use of corticosteroids were significant predictors for the anastomotic complications.

Abstract:Background A tracheostomy is routinely performed following free tissue transfer (FTT) for oral cavity reconstruction; however, its avoidance whenever possible is advocated to enhance patient recovery and reduce hospital length of stay (LOS). Purpose This study aims to measure and compare clinically relevant outcomes for patients who have endotracheal intubation versus tracheostomy for FTT for oral cavity reconstruction. Study Design, Setting, Sample A retrospective cohort study was conducted to evaluate subjects undergoing FTT of the oral cavity for benign and malignant pathology at the University of Alabama at Birmingham from 2014 to 2021. Subjects with unresectable tumors or defects that were not primarily located in the oral cavity were excluded. Independent Variable The independent variable was perioperative airway management and was divided into 2 groups: 1) endotracheal intubation or 2) tracheostomy. Main Outcome Variable(s) The main outcome measure was defined as a postoperative airway-related complication and required escalation of care to an intensive care unit. LOS and surgical complications were also analyzed. Covariates The covariates were classified as demographic, medical, pathologic, and operative. Analyses Bivariate and multivariate statistical analyses were conducted to compare the outcomes between subjects who were immediately extubated and tracheotomized. Subject demographics and operative parameters were also analyzed. Results A total of 560 subjects met the inclusion criteria, with 122 subjects in the immediate extubation group and 438 subjects in the tracheostomy group. The mean age was 59.7 ± 16.3 years in the immediate extubation group and 59.3 ± 13.8 years in the tracheostomy group ( P = .8). The proportion of males was 57.4% in the immediate extubation group and 60% in the tracheostomy group ( P = .6). No postoperative airway-related complications occurred in the endotracheal intubation group. After controlling for confounding factors, tobacco use was associated with airway-related complications (odds ratio [OR]: 2.66; 95% confidence interval: 1.1-6.3; P = .03). LOS was shorter in the endotracheal intubation versus tracheostomy group (6.8 vs 9 days, P < .01). Conclusion and Relevance In subjects who underwent FTT for oral cavity reconstruction, postoperative airway-related complications were associated with a tracheostomy and tobacco use status.

High Tracheal Resection With Intralaryngeal Extension as an Alternative to Cricotracheal Resection for Treatment of Subglottic Stenosis

Abstract:Objective: Some patients with subglottic stenosis (SGS) require open airway reconstruction, which traditionally involves resection of the anterior cricoid cartilage. As an alternative, we present a novel technique: cricoid-sparing high tracheal resection with excision of subglottic stenotic tissue from below. A novel set of posterior circumferential cricoid sutures is used to reline the exposed cricoid plate. Study design: Retrospective chart review. Setting: Single tertiary care center. Methods: The surgical technique is described and illustrated. A chart review was performed for all patients who underwent surgery between January 1, 2016, and June 30, 2021. Results: Fourteen patients (100% female) underwent this airway resection and reconstruction surgery during the study time period. After a mean follow-up of 12 months, no patients required tracheostomy. Twelve of 14 patients (86%) had durable airways. Two patients (14%) have required repeated endoscopic procedures for recurrent stenosis and are considered treatment failures. None exhibited postoperative dysfunction to suggest posterior cricoarytenoid muscle injury. Thirteen patients (93%) had a postoperative normal voice or only transient dysphonia. One patient had permanent unilateral vocal fold paralysis. Conclusion: Cricoid-sparing high tracheal resection is a safe and effective alternative to cricotracheal resection, especially for predominantly posterior SGS. The cricoid cartilage and cricothyroid muscles are left undisturbed, potentially decreasing the risk of postoperative dysphonia, namely lowered fundamental frequency. This is especially meaningful in the setting of a predominantly female patient population.

Abstract:Acquired supracarinal tracheal stenosis can be defined as postintubation tracheal stenosis of the lower third of the trachea that cannot be bypassed by standard or adjustable flange tracheostomy tubes. Owing to difficulty of access in the deep trachea, the transoral laser-assisted endoscopic surgical management of such stenoses is a challenge both for the surgeon and the anaesthetist alike. It requires good rapport and efficient cooperation between the teams to ensure appropriate oxygenation and carbon-dioxide washout while providing intermittent apnoeic phases to the surgeon for clear visualisation of the target area and use of Diode laser which is otherwise hazardous in an oxygen/nitrous oxide rich environment.From our tertiary care referral centre with strong work load for such pathologies, we report one such case of acquired tracheal stenosis successfully managed with transoral Diode laser-assisted surgery.
Tracheobronchoplasty for severe tracheobronchomalacia: a case-series of patients with acute and chronic critical comorbidities

Abstract:Objectives: There are little data within the literature regarding tracheobronchoplasty in the setting of the acute and chronically ill, morbidly obese or ventilator-dependent patients with tracheobronchomalacia. Short- and long-term outcomes are studied. Methods: The series represents 12 tracheobronchomalacia patients with American Society of Anesthesiologists (ASA) physical status scores of 3-5. Candidacy was based on bronchoscopic findings during spontaneous respirations with >90% collapse of the trachea and both mainstem bronchi. We used dynamic computed tomography scan as an adjunct in those not mechanically ventilated. Our operative approach was a complete portal robotic approach for those outpatients (wheelchair dependent) and right thoracotomy for those who were already mechanically ventilated with 100% fraction of inspired oxygen with high pressure. Extracorporeal support was used in 2 patients. Results: Patients who underwent robotic repair were discharged without complications. Two patients who were critically ill and required extracorporeal support for their surgeries were separated from extracorporeal membrane oxygenation on postoperative day 2. Three patients died at the follow-up. In 1 patient, the prolene mesh migrated into trachea and caused obstruction of the trachea and required removal with endobronchial techniques. Conclusions: The repair of tracheobronchomalacia in patients with multiple comorbidities and with severe life-threatening problems in or outside the intensive care unit may have improvement due to the ability to wean from positive pressure ventilation. Surgical technique and the utilization of mesh support in tracheobronchoplasty operations may need to be debated due to duration of the surgery in patients with severe comorbidities.
Management of a case of left tracheal sleeve pneumonectomy under cardiopulmonary bypass: anesthesia perspectives

Abstract:The lung tumors with carinal involvement are frequently managed with tracheal sleeve pneumonectomy and tracheobronchial anastomosis without use of cardiopulmonary bypass (CPB). Various modes of ventilation have been described during tracheal resection and anastomosis. Use of CPB during this period allows the procedure to be conducted in a more controlled way. We performed tracheal sleeve pneumonectomy for adenoid cystic carcinoma of left lung involving carina. The surgery was performed in two stages. In the first stage, left pneumonectomy was performed and in the second stage after 48 h, tracheobronchial resection and anastomosis was performed under CPB. Second stage was delayed to avoid excessive bleeding (due to heparinization) from the extensive vascular raw area left after pneumonectomy. Meticulous peri-operative planning and optimal post-operative care helped in successful management of a complex case, which is associated with high morbidity and mortality.

Abstract:Postintubation tracheal stenosis is the most common cause of benign tracheal stenosis. Surgical treatment is more challenging in long-segment stenosis. Suprahyoid release can increase tracheal length resected without anastomotic tension in patients with post-intubation tracheal stenosis. Its effect on swallowing has not been objectively studied and this article aims to explore its virtues and potential complications in a tertiary center for airway surgery. A prospective cohort study was conducted on forty consecutive patients from June 2020 till December 2023. Patients of both genders had tracheal resection anastomosis surgery with routine suprahyoid muscle release for resected tracheal segment of more than 2.5 cm in length aiming to decrease the anastomotic tension. Within two weeks postoperatively, a video naso-laryngoscope was done on all the patients to detect any vocal cord disorders, then they were examined by video fluoroscopy swallowing study VFSS to detect swallowing problems. Follow up was done for 6 months postoperatively. 40 patients were studied. Twenty-five patients (62.5%) were males. 21 patients (52.5%) had a cricotracheal resection. VFSS was performed on 38 patients (two patients excluded for serious morbidity). Six (15.7%) and four (10.5%) patients had residual semisolid and solid food in the vallecula and pyriform fossa respectively during swallowing. Five patients (13%) out of eight patients with abnormal VFSS had aspiration and dysphagia. Video nasolaryngoscopy was done pre- and post-operatively and showed that 7 patients (17.5%) had unilateral vocal cord paralysis, two of them had the same lesion preoperatively. Two patients developed postoperative anastomotic complications. All symptoms of dysphagia improved within 3 weeks of the procedure and improvement persisted for 6 months. Suprahyoid muscle release had a considerable reversible drawback on the process of swallowing. Its routine use in high-risk patients requiring long segment tracheal resections could be considered.

Abstract:Objectives: This prospective observational study aimed to assess the clinical outcomes of perioperative airway and ventilatory management in patients undergoing surgery for oral cavity cancer. The study described the frequencies and types of procedures for securing the airway and the duration and types of postoperative ventilatory support. We compared the findings with those of the TRACHY study. Patients and methods: One hundred patients undergoing oral cavity oncological surgeries were included. Airway assessment included inter-incisor gap, Mallampati class, neck movements, and radiological features. Surgical parameters, postoperative ventilatory support, and complications were documented. Results: The buccal mucosa was the most common cancer site (48.0%), and direct laryngoscopy was deemed difficult in 58.0% of patients. Awake fibreoptic intubation or elective tracheostomy was required in 43.0% of cases. Thirty-three patients were extubated on the table, and 34 patients were successfully managed with a delayed extubation strategy. In comparison with the TRACHY study, variations were observed in demographic parameters, tumour characteristics, and surgical interventions. Our mean TRACHY score was 1.38, and only five patients had a score ≥4. Prophylactic tracheostomy was performed in 2.0% of cases, in contrast to the TRACHY study in which 42.0% of patients underwent the procedure. Conclusion: The study emphasizes the challenges in airway management for oral cavity cancer surgery. While prophylactic tracheostomy may be necessary in specific cases, individualized approaches, including delayed extubation, are preferrable to maximize safety. Our findings contribute to better understanding and managing perioperative challenges in oral cancer patients and highlight the need for personalized strategies. Scoring systems like TRACHY should not be accepted as universally applicable.

Abstract:To the Editor Research on perinatal mental disorders other than postnatal depression is long overdue; therefore, we were pleased to see the study by Yonkers et al, 1 which reported a 4-fold elevated risk for preterm birth in women who had a likely diagnosis of posttraumatic stress disorder or a major depressive episode. However, we were concerned by the omission of any measure of domestic violence in the study, as domestic violence is a correlate of posttraumatic stress disorder and major depression in pregnancy 2 and of adverse pregnancy outcomes including preterm birth. 3 The prevalence of antenatal domestic violence is about 4% to 9% in high-income settings. 4 In a systematic review of domestic violence and perinatal mental disorders, we reported that women with antenatal depression and posttraumatic stress disorder had high prevalence (15%-30%) and raised odds (3fold to 6-fold) of domestic violence experiences within the past year or during pregnancy. There were a few longitudinal studies, confined to postpartum depression, providing evidence for a bidirectional relationship between domestic violence and postpartum depression. Therefore, domestic violence is potentially important both as a trigger and a consequence of antenatal and postnatal mental disorders. The mechanisms linking domestic violence with adverse pregnancy outcomes 3 include poor antenatal care; chronic gynecological problems; behavioral risk factors, such as substance misuse; direct effects of physical trauma; and poor mental health. 3 Domestic violence has broader implications for mother and child beyond poor pregnancy outcomes including increased risks for maternal death and child abuse. 3,4 The high prevalence of domestic violence among preg

METHODS Data sources included MEDLINE, Scopus, Ovid, and the Cochrane databases. Inclusion criteria consisted of studies in adult patients with post-intubation tracheal stenosis that reported characteristics, surgical management and outcomes. RESULTS 125 articles were identified, of which 10 were included in the final analysis. All studies were case reports or case series (level 4 evidence) grouping 110 patients, 75 males and 35 females. The age ranged from 15 to 71 years. Cotton-Myer stenosis grade was 1 (1 [0.9%]), 2 (25 [22.7%]), 3 (70 [63.6%]), and 4 (14 [12.7%]). Stenosis location was in the tracheal upper-third in 108 (98.2%), the middle-third in 1 (0.9%), and the lower-third in 1 (0.9%). Stenosis length ranged from 1 to 5.6 cm. Follow-up ranged from 1 to 60 months (two years for the most). Most frequent complications were transitory dysphagia in 13 (11.3%), granuloma formation in 8 (7.3%), dehiscence or air leak in 5 (4.5%), and wound infection in 4 (3.6%). Restenosis rate ranged from 2% to 25%. There was no perioperative mortality. CONCLUSIONS Tracheal resection and primary anastomosis in post-intubation tracheal stenosis appear to be safe and effective in the short and mid-terms; however, the very low level of evidence found prevents definitive conclusions.

Unexpected Airway Collapse: A Rare Case of Spontaneous Postoperative Tracheobronchomalacia in the Absence of Identifiable Risk Factors

Abstract:We report the case of a 53-year-old female who developed tracheobronchomalacia immediately following an uncomplicated robotic hysterectomy with bilateral salpingo-oophorectomy to treat postmenopausal bleeding. Induction of anesthesia was notable for moderately difficult intubation, managed with applied cricothyroid pressure and a small 6.5 endotracheal tube placement via GlideScope. The surgical course was uneventful. The patient remained intubated in the post-anesthesia care unit but was not providing end-tidal volumes. Attempts to replace the endotracheal tube with a larger tube were unsuccessful and the patient was temporarily unable to ventilate. Rapid troubleshooting discovered that a laryngeal mask airway (LMA) could sufficiently ventilate the patient. An otolaryngologist was able to perform direct bronchoscopy, which revealed more than 50% dynamic anterior-posterior collapse of the trachea and bronchi. The patient was subsequently awakened from anesthesia and monitored in the intensive care unit, ventilating with an LMA. After a couple of hours, it was determined that the patient's airway was protected, and the LMA was removed.

Abstract:Sleep polygraph and questionnaire data of 18 chronic primary insomniacs were compared with those of 18 age- and sex-matched controls. The insomniacs had significantly longer sleep latencies, less total sleep, less sleep efficiency, more terminal wake time, and less delta sleep. There were significant discrepancies between the insomniacs' and controls' subjective assessments of their sleep and the sleep-polygraph data, but in opposite directions. The insomniacs' recorded sleep also showed more night-to-night variability than that of the controls. However, the controls, in contrast to the insomniacs, reported sleeping worse in the laboratory than at home. Significant differences between insomnia subtypes validly reflected the insomniacs' subjective complaints and were generally in accord with expectations based on them.

We would like to congratulate the authors on their publication as it highlights a number of important issues regarding tracheostomy management in anaesthesia practice. We would like to make some comments regarding the case report. We agree with the authors recommendation that an anaesthesia specific guideline should be used for every patient with a tracheostomy undergoing surgery. Crises can challenge even the most experienced clinicians and cognitive aids can provide valuable real time decision support (2,3). In the case report Villasenor and colleagues identified a number of actions that could have been undertaken if a well-designed cognitive aid had been available and they presented a new checklist and guideline (1). Key points they emphasized were: having awareness of the age of the tracheostomy and the relative difficulty of previous upper airway management, confirmation of a patent airway with capnography before commencing anaesthesia, the importance of passing a suction catheter to confirm tube patency and manage obstruction, the value of deflating the cuff and the importance of prompt tube removal (1). The Austin Hospital, Melbourne, Australia has developed a double sided, A3 sized Emergency Tracheostomy Management cognitive aid which travels with the patient wherever they go in the hospital including the operating room (4). The major content source for our cognitive aid was the National Tracheostomy Safety guidelines and algorithm (5). We suggest that the use of our cognitive aid (or equivalent) would have prevented and/or enabled more rapid management of this challenging situation. The cognitive aid is navigated by a cognitive aid reader who is separate to the team leader (3). The front side of the cognitive aid is for primary responders who are the bedside/ward staff. This page also contains tracheostomy and upper airway information. The second side of the cognitive aid is the advanced responders' side, which is to be used by the code blue team and in critical care environments including intensive care, emergency department and the operating room. The advanced responders' side of the cognitive aid could have been used in this case as follows: We have observed in simulation on multiple occasions the phenomenon of an experienced clinician passing a suction catheter into a completely blocked tracheostomy and incorrectly reaching the conclusion that the tube is patent. We suggest to learners to gauge depth of the suction catheter on the spare tracheostomy tube at the bedside before inserting it into the patient. In this case the clinician may not have appreciated that the suction catheter had not passed the entire length of the tracheostomy tube. If the team had been using the Austin cognitive aid, failure of passage of a suction catheter to stabilise the patient would still have led them on to the next step (step 3) cuff deflation and if the patient remained unstable to continue until the emergency was resolved. Cognitive aids should be developed in a comprehensive manner by diverse teams (6,7). The emergency tracheostomy management cognitive aid in our hospital is part of hospital-wide, system level approach to tracheostomy care led by the Tracheostomy Review and Management Service (TRAMS).
