Abstract:Objectives: To evaluate the lipolysis effect of air cooling assisted long-pulsed 1064 laser for improving local adiposity. Materials and methods: The second-level (pulse duration of 0.3-60 s) long-pulsed Nd:YAG 1064 nm laser (LP1064 nm) with or without forced-air cooling was used to irradiate ex-vivo subcutaneous adipose tissue (SAT) of pig or human and in-vivo inguinal fat tissue of Sprague Dawley rats. The temperature of skin surface as well as 5 mm deep SAT was monitored by a plug-in probe thermal couple, and the former was confined to 39°C or 42°C during the treatment. Histological analysis of SAT response was evaluated by SAT sections stained with hematoxylin-eosin and oil red O. Ultra-microstructure changes were examined by transmission electron microscopy. A pilot study on human subject utilizing LP1064 nm laser with air cooling was conducted. The changes in gross abdomen circumference and ultrasonic imaging were studied. Results: Histological examination showed that LP1064 nm laser treatment induced adipocyte injury and hyperthermic lipolysis both in- and ex-vivo. It was also confirmed by clinical practice on patients. By real-time temperature monitoring, we found that in comparison with LP1064 nm laser alone, additional air cooling could increase the temperature difference between epidermis and SAT, promoting heat accumulation deep in fat tissue, as well as providing better protection for epidermis. Conclusion: LP1064 nm laser provided reliable adipose tissue thermolysis when the temperature of skin surface was sustained at 39°C or 42°C for 10 min. Application of air-cooling during the laser treatment achieved better effect and safety of photothermal lipolysis. LP1064 nm laser, as a noninvasive device, has comparable thermal lipolysis effect as other common heat-generating devices.

Abstract:We have developed a lipolysis laser system that can be commercialized using wavelengths of 1980 nm and 2300 nm with excellent absorption in fat and water. An 808 nm laser diode and Nd:YVO4 were used to generate a 1064 nm wavelength light source, which is used as pumping light for nonlinear crystals. The oven was designed and fabricated to precisely control the temperature of the nonlinear crystal and applied to the mid-infrared lipolysis laser system. The characteristics of the developed laser were validated by measuring the change of the wavelength depending on the temperature and the output according to the wavelength. We analyzed fat reduction efficacy appears for two selected wavelengths.

Abstract:Objectives Submental fullness has been associated with being perceived as unattractive. Technology combining radiofrequency and muscle stimulation offers submental contouring through fat reduction, muscle stimulation, and skin tightening. This study aims to demonstrate the effectiveness and safety of fat reduction aspect with a novel submentum applicator delivering HIFES and synchronized radiofrequency+ (RF+) energies. Materials and Methods Six white pigs (sus scrofa domesticus, n = 6, 60–80 kg) were recruited for this study, five in the active group (n = 5) received four treatments on the abdominal area, one sow served as a control (n = 1). Ultrasound, histological, and RT‐qPCR methods were used as evaluation methods. Results Fat thickness decreased at 1 month by −17.35% and at 2 month by 31.40%. Proapoptotic caspase‐9 gene expression increased (at 1 h, 6 h, 24 h to +43.45%, +21.22%, −8.36%), as well as caspase‐3 (+15.28%, +21.77%, −6.71%), while bcl2l1 activity decreased (−11.46% at 1 h, −17.02% at 6 h, −3.9% at 24 h). While the AI in the control animal had minimal change (at 1 h −0.08%, at 6 h −0.09%, and at 24 h −0.025%), the active group's AI increased from the baseline of 9.14 to 44.85 at 1 h (+391%), peaked at 6 h to 53.50 (+485%), and at 24 h to 38.17 (+318%). Conclusion The study results indicate the efficacy and safety of subcutaneous fat reduction following the novel technology combining HIFES and RF+ energies, designed to target small localized areas.

Abstract:Objective To observe the therapeutic effect and safety of 1 064 nm Nd: YAG second laser combined with long-pulse Er:YAG laser for lumboabdominal fat reduction and Body sculpture. Methods Thirty Patients were treated with 1064nm Nd:YAG second laser and long-pulse Er:YAG laser (SP Dynamis device, Fotona), to scan lumboabdominal areas sequentially. A total of eight times treatments were required for each patient, at10-15 days intervals. Abdominal Circumference and the thickness of subcutaneous fat with ultrasound were observed before and after therapy as well as evaluation of pain degree, subjective score and safety. Results Twenty nine patients completed the study. The means of upper, middle, lower abdominal Circumference were (83.550±9.212)cm,(90.047±8.009)cm and (94.384±7.326)cm at the baseline,(80.209±8.219) cm,(87.606±7.695)cm and(92.128±6.478)cm after treatment respectively,which showed significant differences before and after treatment (P<0.05). The same results were found on the subcutaneous fat thickness (P<0.01). The mean pain score were 4.48±0.91 (1 064nm Nd:YAG second laser) and 3.28±0.75 (long-pulse Er:YAG laser) on Visual Analogue Scale. Lumboabdominal skin tightness, abdominal flatness and waist reduction showed statistically significant improvement (P<0.01) meanwhile no adverse event occurrence. Conclusion Combined 1 064nm Nd:YAG second laser and long-pulse Er:YAG laser treatment is one of safe and effective method for lumboabdominal fat reduction and body sculpture.

Abstract:Background Localized deposits of fat (adiposis) located in the face, chin, neck, and arms usually are more compact and difficult to treat using conventional liposuction. It is necessary to find an effective approach for this type of localized adiposis. This study aimed to investigate the clinical application of laser lipolysis for the treatment of localized adiposis in Asian patients. Methods In this study, 35 patients with localized adiposis were treated by laser lipolysis. Neodymium:yttrium-aluminum-garnet (Nd:YAG) laser energy was transferred through an optic fiber into the adipose tissue. The optimal energy and frequency of the laser were controlled precisely to melt and destroy the adipose tissue without damage to the skin and deeper tissues. The histologic study of treated adiposis tissues, the postoperation reaction, and the clinical results were observed and studied. Results After laser lipolysis, the localized adipose tissues were liquefied effectively, and the compact fibrous tissues were broken down into a looser structure. Bleeding was noted to be less than with conventional liposuction due to the coagulation of capillary vessels and the thermal effect of the laser. Laser lipolysis caused less edema, petechiae, and other postoperative complications, producing rapid recovery and satisfactory results. Conclusion The Nd:YAG laser has a definite lipolysis effect on compact adipose tissue and is a more effective approach for the treatment of localized adiposis than conventional liposuction.
Electrochemical Lipolysis Induces Adipocyte Death and Fat Necrosis: In Vivo Pilot Study in Pigs

Abstract:Background: Current minimally invasive fat reduction modalities use equipment that can cost thousands of U.S. dollars. Electrochemical lipolysis (ECLL), using low-cost battery and electrodes (approximately $10), creates acid/base within fat (width, approximately 3 mm), damaging adipocytes. Longitudinal effects of ECLL have not been studied. In this pilot study, the authors hypothesize that in vivo ECLL induces fat necrosis, decreases adipocyte number/viability, and forms lipid droplets. Methods: Two female Yorkshire pigs (50 to 60 kg) received ECLL. In pig 1, 10 sites received ECLL, and 10 sites were untreated. In pig 2, 12 sites received ECLL and 12 sites were untreated. For ECLL, two electrodes were inserted into dorsal subcutaneous fat and direct current was applied for 5 minutes. Adverse effects of excessive pain, bleeding, infection, and agitation were monitored. Histology, live-dead (calcein, Hoechst, ethidium homodimer-1), and morphology (Bodipy and Hoechst) assays were performed on day 0 and postprocedure days 1, 2, 7, 14 (pig 1 and pig 2), and 28 (pig 2). Average particle area, fluorescence signal areas, and adipocytes and lipid droplet numbers were compared. Results: No adverse effects occurred. Live-dead assays showed adipocyte death on the anode on days 0 to 7 and the cathode on days 1 to 2 (not significant). Bodipy showed significant adipocyte loss at all sites ( P < 0.001) and lipid droplet formation at the cathode site on day 2 ( P = 0.0046). Histology revealed fat necrosis with significant increases in average particle area at the anode and cathode sites by day 14 (+277.3% change compared with untreated, P < 0.0001; +143.4%, P < 0.0001) and day 28 (+498.6%, P < 0.0001; +354.5%, P < 0.0001). Conclusions: In vivo ECLL induces fat necrosis in pigs. Further studies are needed to evaluate volumetric fat reduction. Clinical relevance statement: In vivo ECLL induces adipocyte death and fat necrosis. ECLL has the potential to be utilized in body fat contouring.

Abstract:Introduction Performing laser hair removal treatments on dark skin is limited by the quantity of melanin within the skin. To minimize side effects, lower values of fluence are selected when using standard 755 or 810 nm diode lasers. However, this approach may limit the effectiveness of the procedure, particularly when treating areas with thin and less pigmented hair, which is often the case in facial regions. To improve results, high‐power triple wavelength diode lasers can be used. This study aims to assess the efficacy, safety, and comfort of treatments that remove facial hair with a high‐power triple wavelength diode laser (810, 940, and 1060 nm) in static mode on Asian patients with thin and less pigmented hair. Materials and Methods A single‐center retrospective cohort study was carried out using a high‐power triple wavelength diode laser (810, 940, and 1060 nm), with a 2.7 cm2 spot size, on faces with thin and less pigmented hair. The study comprised 23 subjects with Fitzpatrick skin types IV and V. Effectiveness was measured by counting the hairs that appeared in high‐resolution photos taken prior to and following the procedure, in addition to the Global Aesthetic Improvement Scale (GAIS). Furthermore, mathematical 3D simulations were created on the COMSOL Multiphysics® software to allow for comparisons to be made with regard to thermal damage sustained by the hair follicles and epidermal heating. Assessments were also made in relation to side effects. Results An average of 66% hair reduction was observed. Patient satisfaction was between 4 and 5 points on the GAIS scale, indicating that the treatment was very well received and tolerated. Adverse side effects were not observed. Conclusion It can be concluded that the use of a high‐power triple wavelength diode laser (810, 940, and 1060 nm) is safe and effective for the treatment of very fine and less pigmented facial hair on Asian skin. Furthermore, a triple wavelength (810, 940, and 1060 nm) laser is absorbed less by the melanin in the skin, enabling the use of higher fluences in stamping mode, with greater efficacy and safety for darker skin.

Abstract:A GENERAL HYPOTHESIS OF CARCINOGENESIS IS PROPOSED CONSISTING OF THE FOLLOWING FEATURES: (1) It is suggested that all cells possess multiple structural genes (Tr) capable of coding for transforming factors which can release the cell from its normal constraints on growth. (2) In adult cells they are suppressed by diploid pairs of regulatory genes and some of the transforming genes are tissue specific. (3) The Tr loci are temporarily activated at some stage of embryogenesis and possibly during some stage of the cell cycle in adult cells. (4) Spontaneous tumors, or tumors induced by chemicals or radiation, arise as the result of a double mutation of any set of regulatory genes releasing the suppression of the corresponding Tr genes and leading to transformation of the cell. (5) Autosomal dominant hereditary tumors, such as retinoblastoma, are the result of germ-line inheritance of one inactive regulatory gene. Subsequent somatic mutation of the other regulatory gene leads to tumor formation. (6) The Philadelphia chromosome produces inactivation of one regulatory gene by position effect. A somatic mutation of the other leads to chronic myelogenous leukemia. (7) Oncogenic viruses evolved by the extraction of host Tr genes with their conversion to viral transforming genes. As a result, in addition to the above mechanisms, tumors may also be produced by the reintroduction of these genes into susceptible host cells.
Laser-induced optical breakdown effects of micro-lens arrays and diffractive optical elements on ex vivo porcine skin after 1064 nm picosecond laser irradiation

Abstract:The current study aims to investigate the effects of micro-lens arrays (MLA) and diffractive optical elements (DOE) on skin tissue via intra-dermal laser-induced optical breakdown (LIOB) after irradiation of 1064-nm picosecond laser light at high energy settings. Irradiation with MLA and DOE was tested on dimming paper, tissue-mimicking phantom, and dark pigmented porcine skin to quantitatively compare distributions of micro-beams, micro-bubbles, and laser-induced vacuoles in the skin. DOE yielded more uniform distributions of the micro-beams on the paper and laser-induced micro-bubbles in the phantom, compared to MLA. The ex vivo skin test confirmed that the DOE-assisted irradiation accompanied more homogeneous generation of the micro-beams on the tissue surface (deviation of ≤ 3%) and a high density of small laser-induced vacuoles (∼78 µm) in the dermis than the MLA-assisted irradiation (deviation of ∼26% and ∼163 µm). The DOE-assisted picosecond laser irradiation may help to achieve deep and uniformly-generated vacuolization under the basal membrane after intra-dermal LIOB for effective fractional skin treatment.
Histological examination of skin tissue in the porcine animal model after application of a new monopolar radiofrequency

Abstract:Objectives This study aims to evaluate the safety of the radiofrequency device and its efficacy in various treatment and refrigeration modes. Methods Four 4‐week Bama miniature pigs were used in this study, and four repeated treatment sites were selected on the pig's abdomen, each site consisting of 6 different treatment and cooling modes, with radiofrequency device (YouMagic; WE Medical Technology Co., Ltd.) administered every 3–5 s for a total of five treatments. The handheld infrared thermometer (HIKMICRO; Hangzhou Hikmicro Sensing Technology Co., Ltd.) was used to monitor the surface temperature of skin. Twenty minutes after the completion of treatment, a biopsy of the treatment and control area was performed on the pigs using a 4‐mm biopsy punch. One‐month after the treatment, samples were obtained using surgical scalpels. After that we used proper staining to estimate the therapeutic efficacy. At last, SPSS and Image J were used to proceed to the next step of analysis. Results During the therapy, no side effects were observed apart from mild transient erythema caused by the heating of skin temperature, staining of biopsy samples taken 20 min after treatment showed no serious damage of dermis. After 1 month of treatment, it can increase collagen I and elastin production. In addition, increases in energy setting at a standard pass number also increased the expression of collagen I. Meanwhile, we also found an increase in the thickness of the dermal layer among all treatment groups. Conclusions The new monopolar radiofrequency instrument possesses excellent therapeutic safety. After 1 month of treatment, it can increase collagen I and elastin production in 2‐month‐old Bama miniature pigs.

In Vivo Experiment Study of Non-Ablative Photo-Rejuvenation by Using A 595-Nm Pulsed Dye Laser

Abstract:The pulsed dye laser (PDL) has a history of producing safe and effective clearance of dermal vascular lesions; however, non-ablative treatments of rhytids with 595nm PDL are seldom studied. The purpose of our research is to evaluate the changes of skin elasticity, histology and the amount of hydroxyproline after 595nm PDL non-ablative rejuvenation and to offer references for effective clinical treatments. Forty KM mice were used for this experiment. Laser parameters were as follows: an energy fluence of 8 to 12J/cm(2), a pulse duration of 10ms, and a spot size of 7mm with 10% overlap. Skin elasticity was measured using Reviscometer(R) RVM 600. Specimens were sectioned for hematoxylin-eosin and Van-Gieson staining, and dermal thickness was recorded in an ocular micrometer. The amount of hydroxyproline in the dermis was quantified by the biochemical method. No marked side effects such as blister and purpura were noted during laser treatments. New collagen synthesized with an improvement in the organization of collagen fibrils. The 12 J/cm(2) group improved skin elasticity by 31.7%, dermal thickness by 25.3% and the amount of hydroxyproline by 55.9%. There were the good correlations between dermal thickness and the amount of hydroxyproline. Therefore 595nm PDL non-ablative photo-rejuvenation is a safe and effective method for wrinkle reduction. And the energy level of 12J/cm(2) has the greatest effect in improving skin mechanical properties and accelerating new collagen formation.
Comparison of two porcine (Sus scrofa domestica) skin models for in vivo near-infrared laser exposure

Abstract:Background and purpose: The current safety standards for lasers operating in the 1,400- to 2,000-nanometer (nm) wavelength region are based on only a few observations at specific wavelengths. On the basis of experimental results conducted with Yorkshire pigs (Sus scrofa domestica), these standards may not accurately reflect the potential for laser injury when humans are exposed to these wavelengths. It is our belief that one of the damage mechanisms involved in these laser injuries results from energy absorption by skin pigmentation (melanin), and a more highly pigmented animal model, the Yucatan hairless minipig, may be a more suitable subject for laser exposure studies. Methods: Skin specimens were collected from Yorkshire pigs and Yucatan minipigs for histologic examination, and the thickness of the epidermis was measured. Epidermal thickness of human skin also was determined, and a qualitative assessment of the melanin content in the epidermal layers was conducted. Results: Mean +/- SD thicknesses of the Yucatan minipig flank and dorsal neck epidermis were 68 +/- 34 and 68 +/- 25 microm, respectively. Thicknesses of the Yucatan minipig skin were closely comparable to the thicknesses of human epidermis from the face (68 +/- 26 microm), neck (65 +/- 24 microm) and arms (68 +/- 21 microm). The Yorkshire pig lacks substantial melanin in the epidermis, whereas the skin of the Yucatan minipig is more similar to that of humans. Conclusion: On the basis of epidermal skin thickness measurements and melanin assessment, the flank and dorsal neck of the Yucatan minipig are better suited to laser injury studies than are the Yorkshire pig models of human skin.
Tattoo Removal In Micropigs With Low-Energy Pulses From A Q-Switched Nd : Yag Laser At 1064nm

Abstract:Treatment of pigmented lesions in skin with visible or near-infrared nanosecond (ns) laser pulses often causes significant collateral tissue damage because the current approach uses pulses with energy of 300mJ or larger. Additionally, this requires large Q-switched laser systems. To overcome these disadvantages, we have investigated a different approach in delivering ns laser pulses for cutaneous lesion treatment. Tattoo removal in an animal model with a focused laser beam from a Q-switched Nd:YAG laser has been investigated in two Yucatan micropigs tattooed with blue, black, green and red pigments. The tattoos were treated with a focused beam of 12ns pulses at 1064nm, with different depth under the skin surface, while the micropig was translated to achieve an effect of single pulse per ablation site in the skin. With the pulse energy reduced to a range from 38 to 63mJ, we found that nearly complete clearance was achieved for blue and black tattoos while clearance of red and green tattoos was incomplete. Analysis of the skin appearance suggested that the pulse energy can be decreased to below 20mJ which may lead to further reduction of the collateral tissue damage and improve the clearance of red and green tattoos.

Abstract:The graphic abstract illustrates a thorough evaluation of 100 ps Nd:YAG laser tattoo removal using a minipig model, combining macroscopic and histological approaches to assess the response of tattooed skin to different laser settings, providing critical data for refining tattoo removal protocols. Using a minipig model, we evaluated the efficacy of the 100 ps Nd:YAG laser in the removal of tattoo pigments, specifically blue, green, red, and yellow. We observed distinct pigment responses to 532/1064 nm wavelengths at various energy settings. Through a combination of clinical, spectroscopic, and histological methods, we found the 532 nm wavelength to be most effective in disrupting all colors, with notable results for green and yellow at 0.4 J/cm2 and red at 0.72 J/cm2. The 1064 nm wavelength reduced pigment in yellow (1.51 J/cm2), green (1.35 J/cm2), and blue (1.11 J/cm2) tattoos, but was surpassed by the 532 nm in efficiency. Our data underscores the crucial interplay between pigment traits and laser settings in tattoo removal. We advocate for tailored treatment strategies, integrating pigment hue and laser wavelength, to enhance removal outcomes.

Porcine skin damage thresholds and histological damage characteristics from 1319-nm laser radiation

Abstract:There is an increasing use of near-infrared lasers in biomedical applications operating in the spectrum between 1300 and 1400 nm. To corroborate and expand the existing safety data for skin exposure to lasers in this wavelength region, the in-vivo ED50 damage thresholds were determined in Guizhou miniature pigs for 1319-nm laser radiation. Exposure durations of 0.4, 1.0, and 3.0 s and 1 / e2 beam diameters of 0.98 and 1.96 cm were employed. Damage lesion determinations were performed at 1- and 24 h post exposure. The Bliss probit analysis was employed to establish the ED50 damage thresholds. Histopathological studies of skin damage were performed at 48 h after irradiation to reveal the damage characteristics. The skin damage thresholds at 1 h post exposure, given in peak radiant exposure, were 35.5, 36.1, and 37.1 J / cm2 at exposure durations of 0.4, 1.0, and 3.0 s with the spot diameter of 0.98 cm, and 28.6 J / cm2 at exposure duration of 3.0 s with the spot diameter of 1.96 cm. At 24 h post exposure, the ED50s increased slightly. Histologically, the thermal damage characteristics at the near-threshold level included gathering of the nuclear chromatin and cell vacuolation in the epidermis and deposition of blood cells in the capillary vessels. However, at the apparently above-threshold level, the damage characteristics included obvious stretching of the nuclear chromatin in the epidermis, closing of the capillary lumen, structural change of collagen fibers, and coagulative necrosis of the hair follicle cells. The damage induced by this laser could go deep into the fatty tissue. The obtained results may contribute to the knowledge base for the damage mechanisms and expand the database for the refinement of laser safety standards in the wavelength range of 1300 to 1400 nm.
In Vivo Histological Study Evaluating Non‐Ablative Fractional 1940‐nm Laser

Abstract:Objectives Non‐ablative fractional lasers (NAFL) have increased in demand compared to ablative laser treatments as they provide lesser down time, fewer side‐effects, and are safer to use. Non‐ablative fractional treatment with lasers ranging from 1320 to 1927‐nm have been shown to be safe and effective for skin resurfacing procedures. The objective of this study is to investigate healing of the 1940‐nm NAFL‐induced microthermal treatment zones (MTZs) in human skin from a histologic perspective. Methods Three subjects received 1940‐nm NAFL treatment to test areas on the abdomen at various timepoints during the study. The minimum 5 mJ/MTZ and maximum 20 mJ/MTZ energy settings were used at 20% coverage. Biopsies were taken coinciding with immediately posttreatment, 1, 3, 7 days, and 6 weeks posttreatment. Blinded analysis of hematoxylin and eosin stained slides was performed to measure the width and depth of the MTZs and evaluate the inflammatory and healing response of the skin over time (immediately to 6 weeks posttreatment). Safety was evaluated by assessing local skin responses and adverse events immediately after treatment and at all study visits. Results Histological analysis of tissue following NAFL 1940‐nm treatments showed mild early inflammatory response (presence of lymphotic infiltrate) in some test areas and zones of necrosis and coagulation having widths and depths (immediately–3 days posttreatment) that scaled with the 1940‐nm pulse energy. Signs of healing such as presence of dermal mucin, evidence of fibrosis, and absence of necrosis were observed long‐term (7 days to 6 weeks posttreatment). Evidence of the MTZ persisted beyond the 6‐week study and was predicted to last for 100 days. All local clinical skin responses healed within 6 weeks and were limited to mild, transient erythema and edema which resolved in less than 12–24 h following treatment. No serious adverse events occurred during the study. Conclusions NAFL 1940‐nm treatments are safe for inducing small fractional coagulation and necrosis zones in abdominal skin. NAFL 1940‐nm laser creates fractional columns of injury with sufficient depth and coverage that suggest effective skin resurfacing, like other non‐ablative fractional lasers.

Abstract:Low-level laser therapy is a new subspecialty for the medical application of lasers that provides therapeutic rather than surgical outcomes for many medical indications. Recently, low-level laser therapy was reported to "liquefy" or release stored fat in adipocytes by the opening of specialized yet not identified cell membrane-associated pores after a brief treatment. Currently, low-level laser therapy is a U.S. Food and Drug Administration-approved technology for improving pain alleviation. To explore these data further, a series of in vitro studies on human preadipocytes and institutional animal care and use committee-approved protocols in a porcine Yucatan model and an institutional review board-approved clinical study were performed. Using a 635-nm low-level laser of 1.0 J/cm supplied to the authors by the vendor, these studies were designed to determine whether alteration in adipocyte structure or function was modulated after low-level laser therapy. Cultured human preadipocytes after 60 minutes of laser therapy did not change appearance compared with nonirradiated control cells. In the porcine model, low-level laser therapy (30 minutes) was compared with traditional lipoplasty (suction-assisted lipoplasty) and ultrasound-assisted lipoplasty. From histologic and scanning electron microscopic evaluations of the lipoaspirates, no differences were observed between low-level laser therapy-derived and suction-assisted lipoplasty-derived specimens. Using exposure times of 0, 15, 30, and 60 minutes in the presence or absence of superwet wetting solution and in the absence of lipoplasty, total energy values of 0.9 mW were delivered to tissue samples at three increasing depths from each experimental site. No histologic tissue changes or specifically in adipocyte structure were observed at any depth with the longest low-level laser therapy (60 minutes with superwet fluid). Three subjects undergoing large-volume lipoplasty were exposed to superwet wetting fluid infiltration 14 minutes before and 12 minutes after, according to vendor instructions. Tissue samples from infiltrated areas were collected before suction-assisted lipoplasty and lipoaspirates from suction-assisted lipoplasty. No consistent observations of adipocyte disruptions were observed in the histologic or scanning electron microscopy photographs. These data do not support the belief that low-level laser therapy treatment before lipoplasty procedures disrupts tissue adipocyte structure.
Prospective Evaluation of the Safety and Efficacy of a 1060-nm Large Spot Size, Vacuum-Assisted Hair Removal Diode Laser System in Asian/Pacific Fitzpatrick's Skin Types IV-V Patients

Abstract:Laser-based photoepilation of dark skin types demands a delicate combination of appropriate light wavelengths and spot size to achieve optimal epidermal-to-follicular energy absorption ratios. This prospective study assessed the axillary, arm, thigh, and back hair clearing ef cacy of the LightSheer In nity 1060 nm diode laser in 10 Fitzpatrick skin type IV-V patients. Each area was treated up to ve times, at 4-6-week intervals, after which immediate skin responses and adverse events were recorded. Hair count, color and coarseness were assessed before each treatment session, as well as 1, 3, and 6 months following the last session. Both patients and the treating physician rated the degree of improvement with time, and patients also ranked their satisfaction with the treatment outcome. Percent hair reduction from baseline gradually increased with treatment and peaked at 74.6%, 68.4%, and 65.7% for axillary, arm and thigh regions, respectively, 6 months following the last treatment session. Baseline hair growth patterns precluded effective selection of a representative area for hair counting. Patients satisfaction was consistently higher for axillary hair clearance rates, followed by thigh and arm responses. Throughout the follow-up period, the investigator rated 50-67% of the treated axillae as presenting "good" or "very good" hair clearance, and provided similar ratings for 67% of the treated thigh regions at both the 1 and 6 month follow-up sessions. Immediate responses to treatment were mild to moderate and short-lived and no incidents of brosis or scarring were reported. Taken together, the LightSheer In nity 1060 HS Handpiece provided for an ideal ef cacy-safety balance in treating dark-skinned patients, providing for long-term hair clearance with minimal downtime. Abstract:BACKGROUND: Treatment for most cases of keratosis pilaris requires simple reassurance and general skin care recommendations. Many Asian patients find lesions due to pigmented keratosis pilaris to be cosmetically unappealing. Treatment of post-inflammatory hyperpigmentation using a 1064-nm Q-switched Nd:YAG laser with low fluence is reported.OBJECTIVE: To investigate the efficacy of a novel Q-switched Nd:YAG laser for the treatment of pigmented keratosis pilaris in Asian patients.METHODS: Ten patients with pigmented keratosis pilaris underwent five weekly treatments using a Q-switched Nd:YAG laser (RevLite(®); HOYA ConBio(®), Freemont, CA, USA) at 1064 nm with a 6-mm spot size and a fluence of 5.9 J/cm(2). Photographic documentation was obtained at baseline and 2 months after the final treatment.RESULTS: Clinical improvement was achieved in all 10 patients with minimal adverse effects.CONCLUSION: For the treatment of keratosis pilaris, the use of a Q-switched Nd:YAG laser can be helpful for improving cosmetic appearance as it can improve pigmentation.
