Abstract:OBJECTIVE To explore the Mechanism of nonablative skin rejuvenation. METHOD The Kunming mice be used as subjects and divided into three groups (A, B, C). A, B, C groups were irradiated with 1 320 nm cooltouch laser (20 J/cm2) in the skin of left back; B and C groups were irradiated two and three times respectively; the skin of right back of A, B, C groups was adopted as control. The expression of bFGF and TGF-beta1 in the mouse skin was examined by the immunohistochemistry . The fibroblasts were isolated from the foreskin and cultured. One group is a control and other three ones are low, intermediate and high energetic groups respectively. The fibroblasts were irradiated by laser with 15 J/cm2 ,20 J/cm2 and 24 J/cm2 energy for three times. We examined the levels of bFGF and TGF-beta1 by ELISA in 0, 24, 48 and 72 hours. RESULTS According to this research on immunohistochemistry result, there are significant differences in the expression of bFGF and TGF-beta1 between the group irradiated by three times and others (P < 0.01). The number of fibroblasts get increased after being irradiated by laser. The ELISA result indicates that the secretion of bFGF increased in the group of intermediate and high energetic level after laser irradiating and may reach the peak at 24 hours (P < 0.01). The amount of TGF-beta1 secretion, however, seems to get decreased in each group at all energetic levels, and at 24 hours it can reach the top level as well. CONCLUSION The direct influence of laser on the fibroblasts is to promote secretion of bFGF and to inhibit secretion of TGF-beta1, while its influence on the tissue is to promote the secretions of the both. Nonablative skin rejuvenation not only can induce fibroblasts to secrete more bFGF but also induce the blood vessels to release cytokines which stimulate endothelial cell to express more of bFGF and TGF-beta1. Furthermore, fibroblastic proliferation can accelerate by laser's irradiating.
Evaluation of angiogenesis, inflammation, and healing on irradiated skin graft with low-level laser therapy in rats (Rattus norvegicus albinus wistar)

Abstract:The reconstructive techniques have been widely used in Veterinary Medicine. The post-operative adjuvants therapies like the low-level laser therapy (LLLT) are used to decrease inherent complications to reconstructive surgeries. This article purposed to define the LLLT effects on the healing, inflammation, and vascularization of the skin grafts in applicable time intervals to veterinary surgical routine. Forty rats (Rattus norvegicus albinus wistar) were used and each one was submitted to autogenous cutaneous mesh grafting in the interescapular region. The rats were randomly distributed in five groups (G1, G2, G3, G4, and G5) in accordance with the 6 J/cm2 or 10 J/cm2 dose every 3 or 5 days. These treatments were applied on the skin graft for 15 days. The histochemical evaluation with Picrosirius showed greater expression of collagen type 1 - red in grafts of G5 (p < 0.05), while in G1 did not; the expression of collagen type III - green was not induced by LLLT. The histochemical evaluation with hematoxylin-eosin showed greater numbers of fibroblasts in grafts of G4 (p < 0.05) and less hemorrhage in grafts of G5 (p < 0.05). There was modulation of the inflammatory response in irradiated skin grafts. It is concluded the exhibition of the skin grafts to 6 J/cm2 or 10 J/cm2 dose every 5 days improved the healing and the modulation of the local inflammation.
Photobiomodulation on Full‐Thickness Skin Graft Survival in Rats

Abstract:This study presents the first experimental protocol of laser photobiomodulation to treat the skin graft receptor area. The red and infrared wavelengths were compared using the same parameters, and red wavelength showed more promising results for further studies on this topic. Objective To evaluate the photobiomodulation effects on the receptor area for full‐thickness skin graft integration. Methods Thirty‐Six Wistar rats were divided: red laser (660 nm), infrared laser (808 nm), and control. A skin segment with 5 × 3 cm was removed. In the control, the skin was reallocated after a 180° rotation. For the 660 nm and 808 nm, the receptor area was first irradiated, and then the skin was reallocated the same as the control. Euthanasia occurred on the third and seventh days after the procedure, and macroscopical of necrosis and histological analysis were realized. Results The 660 nm reached the lowest necrosis percentage on Day 7. In the 808 nm, necrosis increased between the two periods. Similar morphological findings were observed for the control and 660 nm; however, the 808 nm showed significant alterations in fibrosis and inflammatory infiltrate. Conclusion The infrared wavelength showed inferior performance on skin graft integration compared to the control and the red wavelength.

Healing Process of Rat Skin Wounds Treated With Vitamin C and Low-Intensity Laser Therapy

Abstract:Introduction: This study evaluated, from a histological point of view, the process of repairing skin wounds caused in the dorsal region of rats when subjected to treatment with vitamin C, low-intensity laser, and association of both. Methods: Forty-eight adult male rats (Rattus norvegicus, albinus, Wistar), weighing between 250 and 300 g were used in this study. The rats were anesthetized with sodium pentobarbital (10 mg/kg) intraperitoneally (IP) and a circular area of skin of approximately 8 mm in diameter was removed from the dorsal region of their back by a punch. The animals were randomly divided into four groups of 12 individuals: Group I, control group, was treated with saline solution; Group II was treated with topical application of vitamin C; Group III was treated with low-intensity laser; and Group IV was treated with both low-intensity laser and topical application of vitamin C. Samples were histologically analyzed through optical microscopy with hematoxylin and eosin staining and collagen I and III concentrations were quantified using the picrosirius-hematoxylin histochemical method and further submitted to statistical analysis. Results: Whilst the control and vitamin C groups admittedly showed slight epithelial proliferation at the wound edges, the group irradiated with low-intensity laser and the group treated with both laser and vitamin C had already partially formed epidermis, with a more organized underlying connective tissue and less evident inflammatory process. The group treated with laser alone obtained a higher concentration of type I collagen fibers and the group with the highest amount of type III collagen fibers was the one treated with the association of vitamin C and laser. Conclusion: The present findings suggest that in spite of all treatments being effective in the repair of skin wounds compared to the control group, the isolated use of low-intensity therapy laser and its combined use with topical vitamin C showed the most favorable results, indicating that those could be further used for the treatment of skin wounds.

Abstract:Gallium-arsenide (GaAs) and helium-neon (HeNe) lasers are the most commonly used low-energy lasers in physiotherapy for promoting wound healing and pain modulation. The aim of this study was investigate the effect of low-power laser irradiation (LPLI) at different wavelengths and doses on oxidative stress and fibrogenesis parameters in an animal model of wound healing. The animals were randomly divided into five groups (n=6): Controls (skin injured animals without local or systemic treatment), skin injury treated with HeNe 1 J/cm(2) (two seg); skin injury treated with HeNe 3 J/cm(2) (six seg); skin injury treated with GaAs 1 J/cm(2) (three seg); skin injury treated with GaAs 3 J/cm(2) (nine seg). A single circular wound measuring 8 mm in diameter was surgically created on the back of the animal. The rats were irradiated at 2, 12, 24, 48, 72, 96, and 120 h after skin injury. The parameters, namely hydroxyproline content, activities of the antioxidant enzymes superoxide dismutase (SOD) and catalase (CAT), and lipid (TBARS) and protein oxidation (carbonyl groups) measurements were assessed. In addition, wound size regression was also analyzed. The results showed an improvement in the wound healing reflected by the reduction in wound size and increased collagen synthesis. Moreover, a significant reduction in TBARS levels, carbonyl content, and SOD and CAT activities were observed after laser irradiation, particularly with the treatments HeNe laser 1 and 3 J/cm(2) dose and GaAs 3 J/cm(2) dose. The data strongly indicate that LPLI therapy is efficient in accelerating the skin wound healing process after wounding, probably by reducing the inflammatory phase and inducing collagen synthesis.
In Vivo Experiment Study of Non-Ablative Photo-Rejuvenation by Using A 595-Nm Pulsed Dye Laser

Abstract:The pulsed dye laser (PDL) has a history of producing safe and effective clearance of dermal vascular lesions; however, non-ablative treatments of rhytids with 595nm PDL are seldom studied. The purpose of our research is to evaluate the changes of skin elasticity, histology and the amount of hydroxyproline after 595nm PDL non-ablative rejuvenation and to offer references for effective clinical treatments. Forty KM mice were used for this experiment. Laser parameters were as follows: an energy fluence of 8 to 12J/cm(2), a pulse duration of 10ms, and a spot size of 7mm with 10% overlap. Skin elasticity was measured using Reviscometer(R) RVM 600. Specimens were sectioned for hematoxylin-eosin and Van-Gieson staining, and dermal thickness was recorded in an ocular micrometer. The amount of hydroxyproline in the dermis was quantified by the biochemical method. No marked side effects such as blister and purpura were noted during laser treatments. New collagen synthesized with an improvement in the organization of collagen fibrils. The 12 J/cm(2) group improved skin elasticity by 31.7%, dermal thickness by 25.3% and the amount of hydroxyproline by 55.9%. There were the good correlations between dermal thickness and the amount of hydroxyproline. Therefore 595nm PDL non-ablative photo-rejuvenation is a safe and effective method for wrinkle reduction. And the energy level of 12J/cm(2) has the greatest effect in improving skin mechanical properties and accelerating new collagen formation.
The energy density of laser light differentially modulates the skin morphological reorganization in a murine model of healing by secondary intention

Abstract:This study investigates the influence of gallium-arsenide (GaAs) laser photobiostimulation applied with different energy densities on skin wound healing by secondary intention in rats. Three circular wounds, 10 mm in diameter, were made on the dorsolateral region of 21 Wistar rats weighting 282.12 ± 36.08 g. The animals were equally randomized into three groups: Group SAL, saline solution 0.9%; Group L3, laser GaAs 3 J/cm(2); Group L30, laser GaAs 30 J/cm(2). Analyses of cells, blood vessels, collagen and elastic fibres, glycosaminoglycans and wound contraction were performed on the scar tissue from different wounds every 7 days for 21 days. On day 7, 14 and 21, L3 and L30 showed higher collagen and glycosaminoglycan levels compared to SAL (P < 0.05). At day 21, elastic fibres were predominant in L3 and L30 compared to SAL (P < 0.05). Type-III collagen fibres were predominant at day 7 in both groups. There was gradual reduction in these fibres and accumulation of type-I collagen over time, especially in L3 and L30 compared with SAL. Elevated density of blood vessels was seen in L30 on days 7 and 14 compared to the other groups (P < 0.05). On these same days, there was higher tissue cellularity in L3 compared with SAL (P < 0.05). The progression of wound closure during all time points investigated was higher in the L30 group (P < 0.05). Both energy densities investigated increased the tissue cellularity, vascular density, collagen and elastic fibres, and glycosaminoglycan synthesis, with the greater benefits for wound closure being found at the density of 30 J/cm(2).

Abstract:Background: In recent decades, low-level laser therapy (LLLT) has occupied a prominent position and has been studied in various fields of knowledge, and your effects have been widely observed in studies about numerous tissues, such as tendons, peripheral nerves, cutaneous tissue, bone, and muscle, in different fields of knowledge. Purpose: To analyze the power transmitted by low-level laser therapy (LLLT) to different tissue samples by using distinct wavelengths. Methods: Skin samples of rat (n = 7, 1.17-1.63 mm) and pig (n = 10; 1.20-2.30 mm); pig fat (n = 10; 2.71-14.01 mm) and pig muscle (n = 10; 1.91-8.91 mm) were analyzed and interposed between the emitter and the power analyzer sensor. All the samples were irradiated sequentially three times, at five equidistant points and average power levels of 35.34(±1.03), 32.40(±0.70), and 42.32(±0.82) mW, for the wavelengths 660, 830, and 904 nm, respectively. Transmitted radiation was measured with a power analyzer connected to a laser emitter. Statistical analysis was performed with a Shapiro-Wilk test followed by ANOVA with Tukey's post hoc test, with a significance level of 5%. Results: The transmitted power of LLLT on skin, fat, and muscle of tissues decreases with the increase of thicknesses, presenting minor attenuation on rat skin, pig fat, and pig muscle for 904 nm. The pig skin has the slight attenuation for 830 nm. Conclusion: The LLLT should be applied after considering the transmission loss taking place in different anatomical structures, following the Beer-Lambert law and attenuation coefficient presented for more practical application in many fields.
Non-ablative collagen remodeling initiated by different laser effects: A comparative study on mouse model

Abstract:Many lasers have claimed the clinical efficacy on skin rejuvenation. Systematic and comparative studies are needed to compare different laser effects and probe into the mechanism of laser skin rejuvenation. We performed this study to compare collagen remodeling with different laser effects on mice model in vivo. After depilation, the back skin of KM mice was used for the study. The 595nm pulsed dye laser (10ms), 1320nm Nd:YAG laser(350 mu s), 1064nm Q-switched (5ns) and long-pulsed Nd:YAG(0.3ms) lasers were applied based on optimal tissue reaction fluence test to irradiate one side of the mice back and leave the other side as the control. Then the collagen remodelling was evaluated at 0, 1, 7, 21, 30 and 60 days, with biophysical parameters' measurements, histological and biochemical examination. All lasers applied showed a statistical improvement in skin elasticity, dermal thickness and synthesis of hydroxyproline compared with their own controls. The Q-switched 1064-mn laser resulted in greater improvement of skin elasticity, dermal thickness, and higher synthesis of hydroxyproline than the other lasers after two months of treatments, while there was no significant difference among the 595nm, 1320nm and long-pulsed 1064nm lasers. Collagen type III increased markedly after the Q-switched 1064-nm laser treatment whereas more collagen type 1 was elicited by the 1320-nm laser.
High‐powered 675‐nm laser: Safety and efficacy in clinical evaluation and in vitro evidence for different skin disorders

Abstract:Background Laser technology is a viable therapeutic option for treating a number of skin pathologic conditions, including pigmented lesions, vascular lesions and acne scars. Aim In this work, through in vitro and clinical investigations we test the efficacy, the safety and the speed of treatment of high‐powered laser system emitting a 675‐nm in the management of various skin condition. Materials and methods In vitro experiments were performed irradiating adult human dermal fibroblasts cells (HDFa) with 675‐nm laser for 24, 48 and 72 h with different fluences and Ki‐67+ cells were counted. The confocal microscopy images of control and treated samples were acquired. Clinical skin rejuvenation/diseases treatments with 675 nm laser device were performed with different laser parameters in 11 patients with pigmented lesions, 5 patients with acne scars and 23 patients for skin rejuvenation. Data were evaluated with the validated global score using 5‐point scales (GAIS) and patient's satisfaction scale. Results The application of the high‐power 675 nm laser has proven effective in stimulating cell proliferation in in vitro experiments and it led to good results for all skin pathologies. GAIS showed values between 3 and 4 points for all treated pathologies, all scores between '75%‐good improvements' and '100%‐excellent improvements'. The treatment time was reduced by 50% compared to the old parameters setting, resulting in a faster and good patient's satisfying technique. No serious adverse effects were recorded. Conclusion: the preclinical and clinical data confirm the efficacy and safety of this high‐powered 675 nm laser for several skin condition.

Abstract:Objective:To compare the non-ablative skin rejuvenation effect of Q-switched 1064 nm and 532 nm Nd:YAG lasers on rat skin.Methods:Eighteen rats were randomly divided into two groups.One group received irradiation of Q-switched 532 nm Nd:YAG laser and the other group received irradiation of Q-switched 1064 nm Nd:YAG laser.The dorsal area of each rat was shaved and denuded,then was marked in four grids:three for treatments with time interval of one day and one for control.Results:The biophysical parameter measurements of skin elasticity and water content were carried out with MPA 9 at 1,3 and 4 weeks after the last laser intervention.Specimens were taken for histological analysis and hydroxyproline contents assay at four weeks after last laser interventions.Conclusions:All lasers treatments led to improvements in elasticity,water content,dermal layer thickness and hydroxyproline content compared with their controls,among which the Q-switched 1064 nm Nd:YAG laser was better compared with 532 nm Nd:YAG laser.
Photobiomodulation invigorating collagen deposition, proliferating cell nuclear antigen and Ki67 expression during dermal wound repair in mice

Abstract:Abstract The present investigation focuses on understanding the role of photobiomodulation in enhancing tissue proliferation. Circular excision wounds of diameter 1.5 cm were created on Swiss albino mice and treated immediately with 2 J/cm 2 and 10 J/cm 2 single exposures of the Helium-Neon laser along with sham-irradiated controls. During different days of healing progression (day 5, day 10, and day 15), the tissue samples upon euthanization of the animals were taken for assessing collagen deposition by Picrosirius red staining and cell proliferation (day 10) by proliferating cell nuclear antigen (PCNA) and Ki67. The positive influence of red light on collagen synthesis was found to be statistically significant on day 10 ( P < 0.01) and day 15 ( P < 0.05) post-wounding when compared to sham irradiation, as evident from the image analysis of collagen birefringence. Furthermore, a significant rise in PCNA ( P < 0.01) and Ki67 ( P < 0.05) expression was also recorded in animals exposed to 2 J/cm 2 when compared to sham irradiation and ( P < 0.01) compared to the 10 J/cm 2 treated group as evidenced by the microscopy study. The findings of the current investigation have distinctly exhibited the assenting influence of red laser light on excisional wound healing in Swiss albino mice by augmenting cell proliferation and collagen deposition.

The 800-nm diode laser irradiation induces skin collagen synthesis by stimulating TGF-β/Smad signaling pathway

Abstract:The 800-nm diode laser is used clinically for hair removal and leg vein clearance. However, the effects of the laser on skin collagen synthesis have not been established. This study aims to research whether the 800-nm laser can be used for non-ablative rejuvenation and its possible mechanism by using an animal model. Eight 2-month-old rats were irradiated with the 800-nm diode laser at 20, 40, and 60 J/cm2, respectively. Skin samples were taken for histological study and dermal thickness measurement at day 30 after laser irradiation. The expression of procollagen type I, III, IV, transforming growth factor-β (TGF-β), Smad2, 3, 4, and phosphorylated-Smad2, 3 in the rat skin was analyzed 24 h after completing all laser treatments by using RT-PCR and Western blot. Immunohistochemistry was performed to evaluate the content of type I collagen in the skin at day 30 after laser irradiation. The 800-nm diode laser treatments markedly improved the histological structure and increased dermal thickness compared to the non-irradiated controls. Laser irradiation at 40 J/cm2 significantly up-regulated the expression of procollagen type I and IV, TGF-β and Smad2, 3, 4. The p-Smad2 and p-Smad3 levels were also enhanced in the laser-irradiated skin. The 800-nm laser is effective in improving skin structure and inducing skin new collagen expression. New collagen synthesis induced by the 800-nm laser was mediated by TGF-β/Smad signaling pathway. Thus, it seemed that the 800-nm laser could be used for non-ablative rejuvenation in the future.

Abstract:Laser therapy has been widely used to treat port-wine stains (PWS) and other cutaneous vascular lesions via selective photothermolysis. Animal models are a valuable tool for investigating thermal responses beneath the skin. However, in previous animal experiments, such as the dorsal skin chamber model, one side of the skin was removed, resulting in the loss of mechanical support for the target blood vessel. In this study, the optical clearing technique was applied to the dorsal skin, allowing direct observation of real thermal responses within the tissue without removing the covering skin. The target blood vessels were irradiated with a pulsed 1064 nm Nd: YAG laser. The corresponding thermal responses were recorded using a CCD camera. Additionally, variations in skin reflectance spectra were measured before and after laser irradiation. Due to the optical clearing and reflectance spectra measurement, vessel responses such as contraction, reperfusion, and full occlusion were correlated with specific variation patterns in reflectance spectral signals.

Four laser treatment modalities were used: the 595-nm pulsed dye laser (PDL) (10 ms), 1,320-nm neodymium-yttrium-aluminum garnet (Nd:YAG) laser (0.35 ms), 1,064-nm Nd:YAG laser with Q-switched (5 ns), and long-pulsed (0.3 ms) mode. Each modality exposed one side of the mouse dorsal skin leaving the other side as the contralateral. control. Then skin histology, fibroblast number, and the genesis of collagen type I and III were studied by comparing the treatment site and control site at 1 hour, I day, 1 week, 3 weeks, 4 weeks, and 8 weeks after laser treatment. Hydroxyproline content of the skin tissue was measured 4 weeks and 8 weeks after laser exposure.Results: All laser treatments led to marked improvements in dermal layer thickness and collagen fiber density, and the increase in fibroblast number and hydroxyproline content compared with their own controls. Collagen synthesis and remodeling induced by the Q-switched 1, 064-nm laser was most effective 4 weeks after treatment, while there was no significant difference among the other three modalities. Among the new collagen genesis after the, different laser treatments, collagen type Ill increased sharply after the Q-switched 1,064-nm laser treatment whereas more collagen type I was elicited by the other laser treatment modalities.Conclusions: The efficacy of photo-mechanical effects in promoting more effectively the synthesis of collagen type III, whereas the photo-thermal effect favored more the formation of collagen type I. Abstract:Researchers have utilized infrared (IR) lasers as energy sources in laser therapy for curing skin diseases and skin injuries with remarkable effects. Preliminary experiments have also shown that high-intensity IR laser pulses could penetrate thick body tissues, resulting in remarkable effects for recovery from injuries in deep muscles and cartilage tissues. However, for deep-level IR laser therapy, it is unclear how much of the laser power density penetrates the body tissues at certain depths and which of the three major effects of laser irradiation, namely, laser-induced photo-chemical effect, photo-thermal effect and mechanical dragging effect, play a key role in the curing process. Thus, in this study, we developed micro-sized thin-film thermocouple (TFTC) arrays on freestanding Si3N4 thin-film windows as sensors for laser power density and local temperature. These devices showed excellent linear responses in output voltage to laser power density with wavelengths in the range of 325-1064 nm, and also indicated the local temperature at the laser spot. We systematically measured the penetrating effect and thermal effect through thick porcine tissues for high-intensity IR pulses with a laser system used in clinical treatment and subtracted the attenuation parameters for the porcine skin, fat and muscle tissue from the experimental data. The results offered reliable quantitative references for safe irradiation doses of high-intensity IR laser pulses in practical laser therapy.
Non-ablative collagen remodeling initiated by two different laser effects: comparative study on mouse model

Abstract:Many lasers have claimed the clinical efficacy on skin rejuvenation. Generally, there are two main laser effects consistent with clinical applications. Systematic and comparative studies are needed to compare different laser effects and probe into the mechanism of laser skin rejuvenation. To conduct a comparative study of collagen remodeling with two different laser effects on mice model in vivo. After depilation, the back skin of KM mice was used for the study. The 1064 nm Q-switched (5 ns) and long-pulsed (0.3 ms) Nd:YAG lasers were applied based on optimal tissue reaction fluence test to irradiate one side of the mice back and leave the other side as the control. Then the collagen remodelling were evaluated at 0, 1, 7, 21, 30 and 60 days, with biophysical parameters' measurements, histological and biochemical examination. The two lasers applied showed statistical improvements in skin elasticity, dermal thickness and synthesis of hydroxyproline compared with their own controls. Between the two lasers, the Q-switched 1064-nm laser resulted in greater improvements than that of the long-pulsed 1064-nm laser after two months of treatments. Collagen type III increased markedly after the Q-switched 1064-nm laser treatment whereas more collagen type I was elicited by the long-pulsed 1064-nm laser. Q-switched 1064-nm laser was more effective in collagen remodeling compared with long-pulsed 1064-nm laser. Photo-mechanic reaction can cause more synthesis of collagen type III whereas the photo-thermal effect is in favor of formation of collagen type I.
Polydeoxyribonucleotide improves wound healing of fractional laser resurfacing in rat model

Abstract:Background: Polydeoxyribonucleotide (PDRN) is an active compound that can promote wound healing. PDRN stimulates wound healing by enhancing angiogenesis and increasing fibroblast growth rates. Laser skin resurfacing is a popular cosmetic procedure for skin rejuvenation. Despite excellent improvement of photo-damaged skin and acne scarring, it is accompanied with drawbacks, such as prolonged erythema and crusting. Objective: This study was designed to assess the effect of PDRN on wounds induced by fractional laser resurfacing. Methods: Twelve male rats aged 8 weeks were randomly assigned to the PDRN treatment group and the control group. Wounds were induced using a fractional ablative CO2 laser. The treatment group received daily injections of PDRN and the control group received injections of the vehicle. Wound healing assessed by clinical features and histopathologic findings. Results: The process of wound healing was faster in the treatment group than in the control group. In the histopathological examination, the granulation tissue thickness score of the treatment group was significantly higher than that of the control group. Results of immunohistochemical staining showed a marked increase of VEGF-positive cells and PECAM-1/CD31-positive microvessels in the treatment group. Conclusion: PDRN may be a beneficial option to promote wound healing after laser treatment.
