Abstract:Background: The aim was to investigate the effect of inferior oblique (IO) operation (IO myectomy or graded recession and anteriorization) for unilateral and bilateral superior oblique muscle palsy (SOP); Methods: A total of 167 eyes undergoing IO surgery by a single surgeon between 2008 and 2015 were retrospectively reviewed. The method for treating symmetric bilateral SOP was bilateral IO myectomy (n = 102) and the method for treating unilateral SOP or non-symmetric bilateral SOP was IO-graded recession and anteriorization (n = 65). Associated clinical results and other factors were analyzed; Results: Head tilt, vertical deviation, IO overaction, SO underaction degree and ocular torsion angle were all clearly changed, but there was no statistically significance between these two procedures. Mean preoperative torsional angle was 15.3 ± 6.4 degree, which decreased to 5.3 ± 2.7 degree after surgery. Preoperative torsional angle, IOOA and SOUA degree were all significantly affected in postoperative torsional angle (p = 0.025, 0.003 and 0.038). Horizontal rectus muscle and IO muscle operation did not interfere with each other’s results (p = 0.98); Conclusions: Symmetric bilateral SOP could be treated with bilateral IO myectomy and IO-graded recession and anteriorization should be reserved for unilateral SOP or non-symmetric bilateral SOP.

Abstract:To enhance the utility of radiochromic films for high-resolution dosimetry of small and modulated radiotherapy fields, we propose a means to negate the effects of heterogeneities in EBT2 (and other) films. The results of using our simple procedure for evaluating radiation dose in EBT2 film are compared with the results of using the manufacturer's recommended procedure as well as a procedure previously established for evaluating dose in older EBT film. It is shown that Newton's ring-like scanning artefacts can be avoided through the use of a plastic frame, to elevate the film above the scanner's surface. The effects of film heterogeneity can be minimized by evaluating net optical density, pixelwise, as the logarithm of the ratio of the red-channel pixel value in each pixel of each irradiated film to the red-channel pixel value in the same pixel in the same film prior to irradiation. The application of a blue-channel correction was found to result in increased noise. It is recommended that, when using EBT2 film for radiotherapy quality assurance, the films should be scanned before and after irradiation and analysed using the method proposed herein, without the use of the blue-channel correction, in order to produce dose images with minimal film heterogeneity effects.

Abstract:Increased serum IgA in patients with direct toxic alcoholic peripheral neuropathy was studied prospectively in 33 patients with an obvious clinical polyneuropathy confirmed by electrophysiological investigations. The average concentration of IgA in this group (450 mg/100 ml) was significantly higher than in a group of 24 matched alcoholic control patients without peripheral neuropathy (306 mg/100 ml), p less than 0.0001. The authors discuss possible direct and indirect modifications of the humoral immune response due to the peripheral neuropathy.

Abstract:Asymmetric superior oblique palsy (SOP) is a complicated, non-comitant type of strabismus. Patients usually present with small-angle hypertropia in the affected eyes, accompanied by inferior oblique muscle overaction (IOOA).<a class="ejp-citation-link js-ejp-citation-link">[1]</a> Compensatory head posture and asymmetric development of the face are typically observed in congenital SOP, while diplopia is more common in an acquired SOP.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> Prism glasses can improve diplopia and vertical deviation in the primary position, but they are not so successful at correcting non-comitant paralytic strabismus. In addition, vertical strabismus and compensatory head posture will return after the prism glasses are removed, wearing prism glasses will lower the vision of children prominently. Surgery like an inferior oblique muscle (IO) weakening procedure is often required eventually in those patients. However, conventional procedures for IO weakening may cause overcorrection and/or secondary A pattern in patients who have mild to moderate IOOA together with small-angle asymmetric SOP, posing a risk to its safety.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> In the study by Caldeira,<a class="ejp-citation-link js-ejp-citation-link">[4]</a> 21% of the patients with mild to moderate IOOA developed A pattern following the surgery. Therefore, it remains a clinical challenge to select the most appropriate type of surgery for the treatment of small-angle asymmetric SOP. Inferior oblique muscle belly transposition (IOMBT),<a class="ejp-citation-link js-ejp-citation-link">[5]</a> a new IO weakening procedure introduced to our practice lately, has shown positive results in treating small-angle asymmetric SOP as well as a lower risk of complications such as overcorrection and secondary A pattern. Abstract:Objectives: To evaluate the effect of inferior oblique (IO) myectomy on ocular torsion according to the absence of the trochlear nerve in unilateral congenital superior oblique palsy (UCSOP). Methods: We retrospectively reviewed the clinical data of patients who had been diagnosed with UCSOP and underwent ipsilateral IO myectomy (n = 43). Patients were classified into the present and absent groups according to the absence of the trochlear nerve and superior oblique hypoplasia on magnetic resonance imaging (MRI). For quantitative analysis of ocular torsion, disc-fovea angles (DFA) were collected in both eyes using fundus photographs taken within three months before surgery and one month after surgery. Results: DFA of the paretic eye did not differ according to the absence of the trochlear nerve (9.4±5.6° in the present group vs. 11.0±5.4° in the absent group, p = 0.508). However, the present group had a larger DFA in the non-paretic eye than the absent group (14.1±6.7° in the present group vs. 8.0±5.0° in the absent group, p = 0.003). The change of ocular torsion after IO myectomy in the paretic eye was -5.3±3.7° in the present group and -4.8±3.5° in the absent group, respectively (p = 0.801). In the non-paretic eye, the change in DFA was -1.5±3.0° in the present group, which was larger than that in the absent group (0.7±2.6°, p = 0.047). In the multivariate analysis, the change in DFA was correlated with only the preoperative DFA (standardized β = -0.617, p<0.001 in the paretic eye, and standardized β = -0.517, p<0.001 in the non-paretic eye). Conclusions: In the paretic eye, there was no significant difference in the change of ocular torsion between both groups, whereas in the non-paretic eye, the present group had a larger change in DFA after IO myectomy than the absent group. However, in the multivariable analysis, the change in ocular torsion was significantly correlated with preoperative excyclotorsion but not with the presence of the trochlear nerve itself.

Abstract:Purpose: To evaluate the outcomes of inferior oblique (IO) weakening surgery, whether recession or myectomy, and to assess the dose-response relationship and correlation with angle of preoperative hypertropia. Methods: The medical records of all patients with vertical deviation in primary gaze who underwent unilateral IO-weakening surgery, either recession or myectomy, at Boston Children's Hospital over an 8-year period with a minimum postoperative follow-up of 1 month were reviewed retrospectively. Outcome measures were effect of IO weakening surgery on vertical deviation in primary gaze and its correlation with the preoperative angle of hyperdeviation. Secondary outcomes included resolution of abnormal head posture, reduction of ocular torsion, and postoperative under- and overcorrection. Results: A total of 94 patients were identified (mean age at surgery, 29.3 ± 19.8 years; range, 1-69). The mean postoperative follow-up period was 17.2 ± 15 months. IO recession was performed in 30 patients; IO myectomy, in 64. Surgical success in primary position was achieved in 72 patients (77%), with resolution of anomalous preoperative head posture in 93%. The mean effect on alignment in primary position was 11.3Δ ± 6.8Δ. The response to IO-weakening surgery was strongly correlated with the preoperative hyperdeviation for both recession (R2 = 0.53) and myectomy (R2 = 0.87). Conclusions: As with other types of strabismus surgery, IO weakening has a "self-grading" contribution, in which the surgical effect strongly correlates with the magnitude of preoperative deviation. A large range of vertical misalignment can be corrected with the same surgical approach.
The Outcome of Inferior Oblique Myectomy for Apparent Inferior Oblique Overaction Associated with Craniosynostosis

Abstract:Purpose: Strabismus in patients with craniosynostosis is common, but surgical correction of strabismus in these patients remains challenging. We report our findings in six patients (four of whom were Korean) with craniosynostosis who underwent strabismus surgery to specifically address V-pattern horizontal strabismus with moderate-to-severe inferior oblique (IO) overaction, using IO myectomy at a single tertiary hospital between 2005 and 2016. Methods: We recorded preoperative characteristics including sex, age, type of strabismus, versions grading, refractive error, and visual acuity. The grading of cyclorotation of horizontal rectus muscles by V-pattern categorized using coronal computed tomography imaging. Results: Of the six patients, exodeviation was found in four patients and vertical deviation in two patients in primary position. One patient had both horizontal and vertical strabismus. Available computed tomography imaging showed that V-patterns were category 1 (mild) in two patients, category 2 (moderate) in one patient, and category 3 (severe) in two patients. Complete success was defined as absence of IO overaction any more. Overall complete success rate of IO myectomy was 83.3%. Conclusions: IO myectomy appeared to have some benefits in V-pattern horizontal strabismus with moderate-to-severe IO overaction in patients with craniosynostosis.

Abstract:Background: Inferior oblique muscle overaction (IOOA) is a prevalent ocular muscle motility disorder. This study aimed to investigate the outcomes and impact of IO muscle myectomy with and without tucking the proximal end of the muscle into Tenon's capsule in two surgical groups as a treatment for IOOA. Methods: Randomized cohort study that involves two surgical groups. A total of 33 patients (55 eyes). Patients were randomly divided into two groups: first group underwent myectomy without tucking the proximal end of the muscle, whereas the second group underwent myectomy with tucking of the proximal end of the muscle. Each patient had four visits. During each visit, each patient had a comprehensive ophthalmic examination. Surgical success was defined as IOOA grade range of 0 to + 1 and correction of hypertropia in primary gaze. Results: The median age was 6 years. Sixteen (48.5%) patients underwent IO muscle myectomy with tucking, while 17 (51.5%) had an IO myectomy without tucking. Both the tucking and without tucking groups showed reductions in the angles of deviations, but there was no significant difference between the groups at baseline or postoperatively at 6 months. The success rate was 92.85% for the tucking group and 92.59% for the without tucking group. Conclusion: The study suggests that both surgical techniques are effective in correcting IOOA and associated HT, without any clear benefits of the addition of tucking. Ultimately, the two surgical techniques are equally effective, and the choice of surgical technique remains a personal preference of the surgeon.

Abstract:by four articles on factors regulating mature neural cell function. The importance of neurotrophin and TAM receptor signalling in regulating myelin development and myelin breakdown, respectively, are highlighted in comprehensive reviews from the Murray and Kilpatrick laboratories. The next two reviews are focussed on neurons. The first, by Cole, examines what little is currently known about the PCTAIRE kinases and their probably important roles in neuronal function, highlighting areas that need to be addressed and proposing ways in which this might be done. This is followed by a review from Scarr, providing an overview of what is known about the role of muscarinic receptors, their potential to be targeted in psychiatric disorders and the gaps in knowledge that need to be filled. The final review, by Munro and Perreau, highlights the complexities of performing microarrays for transcriptional profiling of the nervous system and describes current and emerging applications for this technology, with specific reference to gene regulation following CNS disease and injury. It also provides an experimental design flow chart to help researchers contemplating using transcriptomics to navigate and design their own projects. What is clear from this series is that we still have a long way to go before we will understand the many complex and interacting pathways that regulate nervous system function, not only under normal conditions, but also under the even more complex and challenging conditions that arise following CNS injury or disease. It is hoped that this snapshot of some of the research being undertaken in these areas within the Centre for Neuroscience will provide a useful and thought-provoking resource for those with an interest in signalling pathways modulating neural cell function in the healthy and the damaged CNS. It was created to provide a strategic focus for neuroscience research within the University of Melbourne and to build on a strong and established research base. The Centre acts as a hub to bring together the breadth of neuroscience excellence in the Parkville precinct, to address a variety of problems within the broad field of neuroscience. One of the main underlying themes of research within the core group of researchers at the Centre is based on gaining an understanding of the signal transduction processes regulating neural development and neural cell function in health and disease, with the ultimate aim of translating this knowledge into therapeutic strategies. Some of these research areas are highlighted in this series of reviews, which are largely focussed on understanding of signalling pathways involved in promoting neural regeneration. However, it is clear that mechanisms that result in neural degeneration, via disease or injury, also need to be understood if regenerative efforts are to succeed and this is addressed in several of the articles. The first, by Denham and Dottori, describes the current state of knowledge regarding the general specification of neural differentiation and specific neural cell fates of human ES cells. This is followed by an examination of the role of lysophospholipids in regulating neural differentiation by Pitson and Pébay, which compares and contrasts the different effects of these molecules in mouse and human stem and neural progenitor cells. The third is from the Cate laboratory, on the role of BMP signalling in the regulation of adult neural precursor cells and the influence that BMPs may have on neural precursor function following CNS injury. Superior oblique tuck-effectiveness in reducing vertical deviations when performed as a primary versus secondary procedure in superior oblique paresis

Abstract:Purpose: To quantify the effectiveness of superior oblique tuck (SOT) surgery in patients with a hyperdeviation secondary to superior oblique paresis (SOP). Surgical outcomes were compared in patients undergoing SOT surgery as a primary procedure with those who had previously undergone ipsilateral inferior oblique weakening surgery. Methods: This retrospective study assessed surgical outcomes from all patients undergoing SOT surgery for SOP between 2012 and 2021 across 2 hospitals. The effectiveness of SOT surgery in reducing the hyperdeviation was assessed in the primary position (PP) and in contralateral elevation and depression. Results were compared between those undergoing primary SOT surgery with those who had previously undergone ipsilateral inferior oblique weakening surgery. Results: A total of 60 SOT procedures were performed between 2012 and 2021. 7 were removed due to incomplete data. The remaining 53 cases experienced a mean reduction in hyperdeviation of 6.5 prism dioptres (PD), 6.7PD and 12.0PD in the PP, contralateral elevation and contralateral depression respectively. In eyes with previous IO weakening, the reduction of hyperdeviation was larger than in those eyes with no previous IO weakening surgery, with mean reductions of 8.0PD vs 5.2PD, 7.4PD vs 6.2PD and 12.4PD vs 11.6PD in the PP, contralateral elevation and contralateral depression respectively. Conclusion: SOT surgery is a safe and effective procedure with high patient satisfaction and resolution of symptoms in those patients with troublesome diplopia in downgaze secondary to SOP. This is true in both unoperated eyes and in those who have previously undergone inferior oblique weakening surgery.

Abstract:Purpose: This study aimed to evaluate the effect of inferior oblique myectomy (IOM) surgery on the cornea and anterior segment. Methods: The sheimpflug corneal topographies of 56 eyes of 33 patients who underwent IOM were analyzed at preoperative, postoperative 1 week, 1 month, 3 months, and 6 months. Astigmatism degree (Cyl), astigmatism axis (Ax), K1, central corneal thickness (CCT), anterior chamber depth (ACD), anterior chamber volume (ACV), iridocorneal angle (ICA), and horizontal visible iris diameter (HVID) were evaluated. Results: The mean corneal Cyl, K1, ICA, ACD, and HVID did not change after IOM (p=0.671, p=0.377, p=0.431, p=0.588, p=0.795, respectively). There was a statistically significant change in Ax (p=0.025). The right Ax decreased, and the left Ax increased the most at 1 month (p=0.025, p=0.882, respectively). Ax increase was also detected in the left eye that was not operated on. The mean CCT increased and mean ACV decreased at 1 month (p=0.588, p=0.270, respectively), but these changes returned to preoperative values at 3 months. Conclusion: The manifestation of anterior segment alterations and intorsion in both eyes following inferior oblique myectomy may contribute to diminished visual acuity and the potential onset of amblyopia. These alterations merit consideration during the assessment, and if deemed necessary, the provision of new spectacles should be considered as a preventive measure against anisometropic amblyopia.
A standardized recession of the inferior oblique extraocular muscle - a safe and self-grading surgical procedure for trochlear nerve palsy: a 10-year material

Abstract:Purpose: To evaluate the results of surgical treatment in a large group of patients with trochlear nerve palsy, with emphasis on the self-grading effect of a standardized recession of the ipsilateral inferior oblique muscle. Methods: All patients who underwent first-time surgery for trochlear nerve palsy in the period 2005-2014 in our department (n = 114) were retrospectively evaluated regarding pre- and postoperative data, surgical procedure, and the need for reoperations. Mean follow-up time was 8.5 ± 13.8 months. Results: Among the 114 patients, 73 (64.0%) had a congenital palsy, 31 (27.2%) an acquired palsy, while in 10 cases (8.8%) the type of palsy was uncertain. A standardized recession of the ipsilateral inferior oblique muscle with reattachment at the lateral border of the inferior rectus muscle was carried out in 97.3% of the congenital palsies and in 80.5% of the acquired/uncertain group. In the total patient material, further surgery was needed in 15.8%. Among the patients who underwent a single standardized recession of the inferior oblique muscle, a significant correlation between preoperative vertical angle of deviation and the postoperative change in deviation was demonstrated (correlation coefficient -0.70, p < 0.001), confirming the self-grading nature of this procedure. Subjectively, 97.1% in the congenital and 91.4% in the acquired/uncertain group reported complete/near complete resolution or significant improvement of their symptoms at the final control examination. Conclusion: In the majority of patients with trochlear nerve palsy, a favourable outcome may be achieved after a single, standardized recession of the ipsilateral inferior oblique muscle.

Abstract:Background: Overelevation in adduction is common in patients with primary esotropia. This study evaluates the variation in ocular motility pattern in patients with primary inferior oblique (IO) muscle overaction after esotropia surgery. Methods: The medical records of consecutive patients who underwent surgery for infantile, partially accommodative, and basic esotropia over eleven years and had at least one year of follow-up were reviewed. Patients with primary inferior oblique muscle overaction (IOOA) presented at baseline or during follow-up were selected and divided according to the first surgery performed concurrently with horizontal rectus surgery: without IO recession (NO-recess), with unilateral IO recession (UNIL-recess), and with bilateral IO recession (BIL-recess). The success (version normalisation or at least 2 points upgrade in severity scale [0-4] in the operated eye), recurrence rates, and the evolution of the non-operated IO muscles were evaluated. Results: One hundred and ten patients were included - 53 NO-recess, 26 UNIL-recess, and 31 BIL-recess. Medial rectus muscle posterior fixation sutures surgery (PFS) was performed in 88.2% of patients for esotropia. A recession with graded anterior transposition was the weakening IO procedure. In the NO-recess group, 28 (52.8%) patients normalised their mild IOOA after PFS surgery alone. In the UNI-recess group, the success rate was 88.5%, with 16 (61.5%) patients showing worsened IO muscle of the fellow eye, which prompted additional surgery in 10 patients. In the BIL-recess group, all 31 patients improved the adduction pattern of the operated eye for an 80.6% success rate (6 improved marginally). Conclusion: Graded anterior transposition of the inferior oblique muscle effectively normalises versions. However, it's frequent for a contralateral overaction to become manifest after unilateral IO surgery.

Abstract:Purpose: Biomechanical modeling consistently indicates that superior oblique (SO) muscle weakness alone is insufficient to explain the large hypertropia often observed in SO muscle palsy. Magnetic resonance imaging (MRI) was used to investigate if any size or contractility changes in the inferior rectus (IR) muscle may contribute. Design: Prospective, case-control study. Participants: Seventeen patients with unilateral SO muscle palsy and 18 orthotropic subjects. Methods: Surface coils were used to obtain sets of contiguous, 2-mm-thick, high-resolution, coronal MRI views in different gazes. Cross-sectional areas of the IR and SO muscles were determined in supraduction and infraduction for evaluation of size and contractility. Diagnosis of SO muscle palsy was based on clinical presentations, subnormal contractility, and SO muscle size less than the normal 95% confidence limit. Main outcome measures: Cross-sectional areas of the IR and SO muscles. Results: Patients had 15.9+/-7.2 prism diopters (Delta; mean+/-standard deviation) of central gaze hypertropia and exhibited ipsilesional SO muscle atrophy and subnormal contractility. Mean ipsilesional, contralesional, and normal IR muscle cross-sections were 28.5+/-3.5 mm(2), 31.9+/-3.8 mm(2), and 31.8+/-5.8 mm(2), whereas mean contractility was 16.5+/-3.8 mm(2), 20.5+/-4.1 mm(2), and 16.6+/-4.8 mm(2), respectively. Ipsilesional IR muscle cross-section and contractility was significantly less than contralesional cross-section and contractility (P<0.01). Conclusions: In SO muscle palsy, the contralesional IR muscle is larger and more contractile than the ipsilesional IR muscle, reflecting likely neurally mediated changes that augment the relatively small hypertropia resulting from SO muscle weakness alone. Recession of the hyperfunctioning contralesional IR muscle recession in SO muscle palsy is a physiologic therapy.
