Abstract:To enhance the utility of radiochromic films for high-resolution dosimetry of small and modulated radiotherapy fields, we propose a means to negate the effects of heterogeneities in EBT2 (and other) films. The results of using our simple procedure for evaluating radiation dose in EBT2 film are compared with the results of using the manufacturer's recommended procedure as well as a procedure previously established for evaluating dose in older EBT film. It is shown that Newton's ring-like scanning artefacts can be avoided through the use of a plastic frame, to elevate the film above the scanner's surface. The effects of film heterogeneity can be minimized by evaluating net optical density, pixelwise, as the logarithm of the ratio of the red-channel pixel value in each pixel of each irradiated film to the red-channel pixel value in the same pixel in the same film prior to irradiation. The application of a blue-channel correction was found to result in increased noise. It is recommended that, when using EBT2 film for radiotherapy quality assurance, the films should be scanned before and after irradiation and analysed using the method proposed herein, without the use of the blue-channel correction, in order to produce dose images with minimal film heterogeneity effects.
Strabismus surgery outcomes for internuclear ophthalmoplegia with exotropia

Abstract:Exotropia (XT) in internuclear ophthalmoplegia (INO) is a difficult problem to treat. The purpose of this study is to describe surgical approaches in treating INO and glean insight into various pre-operative factors that may affect outcomes for XT in INO. We conducted a retrospective review from 1 January 1995 - 5 January 2021 and identified seven patients with INO who underwent strabismus surgery for XT. Patient age, sex, etiology of INO, pre-operative alignment and sensorimotor exam, presence of diplopia, surgery performed, subsequent surgeries, use of adjustable sutures, post-operative alignment, presence of post-operative diplopia, presence of post-operative diplopia with use of prism correction, and length of follow-up were all collected. Initial surgeries undertaken included unilateral medial rectus (MR) plication and lateral rectus (LR) recession, bilateral medial rectus (MR) plications or resections, or bilateral MR plications combined with either unilateral or bilateral LR recessions. Chart review yielded ten charts, however two were excluded due to manifest esotropia (ET), and one was excluded due to incomplete records. Seven total patients were used in final analysis. The cohort age range was from 29 to 79 years. Pre-operative horizontal distance alignment ranged from 35 to 95 XT with an average exodeviation of 67.8 ± 22.6 prism diopters (PD). Horizontal adduction deficit ranged from -1 to -4 and was present bilaterally in all patients. A variety of initial surgical approaches were undertaken. After two muscle surgeries, distance deviation had an average change of 57.3 PD. After three muscle surgeries, distance deviation had an average change of 75 PD. After four muscle surgeries, distance deviation had an average change of 60 PD. Three patients required additional surgery for XT. Time to follow-up ranged from 1 to 58 months. Horizontal distance alignment in primary gaze at latest follow-up ranged from 30 ET to 30 XT with an average of 0 (orthotropia) ± 16.0 PD. One patient had a consecutive esotropia of 30 PD, one had a persistent exotropia of 30 PD, and five patients were orthotropic at distance. All patients reported relief of diplopia in primary gaze at near and distance either with or without use of prism. Horizontal ductions improved to some degree in all patients. Horizontal rectus surgery can treat many cases of XT in INO. Surgeons should consider INO etiology and concomitant vertical deviations when considering surgery. The degree of pre-operative adduction limitation is another important factor, though did not always dictate final motor and sensory outcomes.

Abstract:OBJECTIVE To construct a RU486 inducible recombinant adenovirus of murine IL-12 protein and study its effect and safety on colonic cancer. METHODS The replication-defective recombinant adenovirus were produced after cotransfection of shutter vector pDC-RUmIL-12 and adenovirus DNA helper plasmid pBHGloxDeltaE1, 3Cre into HEK293 cells. The recombined adenovirus was purified by CsCl density gradient centrifugation and its titer was determined by end point dilution assay. Expression of this regulatable recombinant adenovirus vector in infected C26 colonic carcinoma cells was tested by ELISA kit in vitro. The tumor model was established by hypodermic inoculation of C26 cells. Sixty tumor-bearing mice were randomly divided into 4 groups: Ad-buffer group; Ad-RUmIL-12 group; Ad-RUmIL-12 + RU486 group and Ad-mIL-12 group, and the treatment effects and side effects were evaluated. RESULTS The adenoviral vector containing murine IL-12 gene was identify by PCR. The viral titer of Ad-RUmIL-12 was 4.62 x 10(10) pfu/ml. The expression of IL-12 protein was induced by the RU486 and the highest expression (516 +/- 43) pg/ml whereas no significant IL-12 protein was detected without inducer or getting rid of the inducer [(38 +/- 3) pg/ml and (42 +/- 5) pg/ml respectively]. The tumor size increased rapidly in group Ad-buffer and group Ad-RUmIL-12 (P > 0.05). Administration of Ad-RUmIL-12 + RU486 and Ad-mIL-12 were showed to delay markedly the growth of transplanted C26 tumor (P > 0.05). Significantly necrosis was observed in both Ad-mIL-12 and Ad-RUmIL-12 + RU486 experimental groups, but the level of the serum alanine transaminase and the rate of side effect was higher in Ad-mIL-12 group (4/15 and 10/15 respectively, P < 0.05). CONCLUSION A RU486 regulatable recombinant adenoviral vector containing IL-12 gene was successfully constructed. The expression of vector Ad-RUmIL-12, regulated by inducer RU486 in vivo, can obviously improve safety in tumor treatment and provide a good primer for further researches on in vivo gene therapy.
Unilateral four muscle surgery for extra-large monocular exotropia

Abstract:Introduction: Recurrent exotropia is common following surgery for monocular large angle constant sensory exotropia. Surgery is usually limited to operations on the affected eye. Simultaneous oblique weakening surgery may enhance the effect of the horizontal rectus muscles surgery by decreasing the abducting forces. We report the results of simultaneous oblique muscle weakening procedures combined with ipsilateral horizontal rectus muscle surgery constant monocular exotropia greater than 35 prism diopters (PD). Methods: Retrospective case series of patients who underwent unilateral lateral rectus recession combined with medial rectus muscle resection and simultaneous weakening of both ipsilateral oblique muscles. Primary outcome measure was ocular alignment in primary position. Results: Twelve eyes of 12 patients were included. The mean preoperative exotropia improved from 57.9 ± 15.1 (range 35-80; median 60 PD) to 3.3 ± 5.5 (range 0-16; median 0 PD) postoperatively (p < 0.005). Two of 3 patients with a pre-existing vertical deviation had resolution of their vertical misalignment postoperatively. At the last postoperative follow up 92% of the patients had an exodeviation of 10 PD or less (range 0-16 PD median 0 PD), and 7 (58%) measured near and distance orthotropia. Postoperative abduction measured -0.6 ± 1 (0 to -3) and adduction -0.4 ± 0.7 (0 to -2). Conclusion: Weakening the ipsilateral oblique muscles may enhance the effect of the horizontal rectus muscles surgery by decreasing the abducting vectorial forces when operating for a large angle monocular exotropia. As an additional potential advantage, oblique muscle surgery may be used simultaneously to address associated vertical deviations.
The surgical treatment of consecutive strabismus following concomitant horizontal strabismus surgery

Abstract:Objective To investigate the results and techniques of surgical treatment of consecutive strabismus following concomitant horizontal strabismus surgery. Methods Fifty-five patients operated on for consecutive strabismus following their first surgical treatment of concomitant horizontal deviation were analyzed retrospectively. Preoperative visual acuity, deviation, mobility evaluation, stereoacuity with Titmus test and traction test were performed as well as post-operative residual deviation and binocular function. The mean postoperative follow-up time was 2years. Results Thirteen patients with consecutive exotropia due to overcorrection for primary concomitant esotropia were managed with exploratory operation (3 patients) or treated with unilateral lateral rectus muscle recession combined with medial rectus muscle advancement (10 patients),in which 10 patients (76.9%) achieved satisfactory alignment at final follow-up. Eleven patients with consecutive esotropia due to overcorrection for primary concomitant exotropia underwent medial rectus muscle resection and lateral rectus muscle complete of partial advancement to the original insertion and 9 patients in which achieved long-term satisfactory alignment. Fifteen patients with undercorrection for concomitant esotropia were treated with medial rectus marginal myotomy combined with lateral rectus muscle recession (8 patients), unilateral lateral rectus muscle recession (2 patients) or medial rectus recession and lateral rectus resection combined with inferior oblique muscle recession in fellow eye (5 patients with Oblique muscle abnormal), and postoperative orthophoria rate was 86.7%. Sixteen patients with undercorrection for concomitant exotropia were treated with lateral rectus muscle recession and medial rectus muscle resection (6 patients) or combined with inferior oblique muscle recession (2 patients with V-exotropia), cicatricotomy for lateral rectus muscle (4 patients) or adjusted of releasable sutures (4 patients), in which postoperative orthophoria rate was 87.5%. Conclusions The reversal of unilateral medial rectus and lateral rectus recession or resection is an effective alternative for treating postoperative consecutive exotropia due to overcorrection of concomitant horizontal strabismus. Adjusting of releasable sutures provide more stable expections of the results achieved by the re-operation.

Surgical outcome of single-staged three horizontal muscles squint surgery for extra-large angle exotropia

Abstract:Purpose: To report the surgical outcomes of 24 patients undergoing single-staged three horizontal muscles squint surgery for extra-large angle exotropia. Patients and methods: Prospective case series of 24 consecutive patients with primary exotropia>60 prism diopters (PD) at distant and underwent single-staged three horizontal muscles squint surgery from 2003 to 2006. Surgery consisted of bilateral lateral rectus recession of 9.0 mm for 50 PD exotropic correction. For every 5 PD remaining angle exceeding 50 PD, we additionally performed 1.0 mm of unilateral medial rectus resection. The mean follow-up period was 15.8 months (range 6.0-38.0 months; SD 9.5 months). Results: The mean age at surgery was 31.2 years old (range 7-78 years old, SD 18.2 years old). The mean distant preoperative deviation was 71.3 PD (range 60-85 PD, SD 7.7 PD). No limitation of eye movement or diplopia was found. The success rate was higher in the intermittent group (88.2%) than the constant group (42.9%) (P=0.02) and in cases with preoperative deviation of <80 PD (84.2%) compared with those with deviation>or=80 PD (40.0%) (P=0.042). Conclusion: Measurement of preoperative deviation and the surgery for extra-large angle exotropia made management of this condition difficult. Single-staged three horizontal muscles squint surgery can be one of the options. Further researches on management of exotropia of >80 PD are warranted.
Surgical success and lateral incomitance following three-muscle surgery for large-angle horizontal strabismus

Abstract:Purpose: To evaluate motor alignment, motility, and sensorial outcomes of simultaneous three-muscle surgery for large-angle horizontal strabismus, with special attention to lateral incomitance and long-term success. Methods: The medical records of consecutive patients with large-angle deviations (≥30Δ) who underwent simultaneous surgery on three horizontal muscles to correct esotropia or exotropia were reviewed retrospectively. Successful motor alignment was defined as residual deviation of ≤10Δ and consecutive deviation in primary position of ≤4Δ, with no induced lateral incomitance >5Δ between lateral gazes. Sensory success was defined as an improvement in stereopsis of ≥2 octaves. Surgical procedures included a combination of recessions and resections/plications depending on surgeon preference. The majority of cases were two-muscle recessions combined with one-muscle resection or plication. Results: A total of 19 patients with exotropia and 9 patients with esotropia were included. In the esotropic group, the mean age at surgery was 48 ± 15 years and the mean preoperative deviation improved from 45.6Δ ± 11.9Δ to 1.5Δ ± 1.6Δ (P < 0.001); there was no undercorrection, recurrence or overcorrection. In the exotropic group, the mean age at surgery was 44 ± 25 years, and the mean preoperative deviation improved from 44.1Δ ± 8.7Δ to 5.8Δ ± 9.6Δ (P < 0.001), recurrence occurred in 2 (22%). Overall motor success at distance was 85%, with 1 (3.8%) unsuccessful patient due to induced incomitance. Sensory success was 44% for esotropia and 31% for exotropia. No patient lost stereopsis. Conclusions: Patients undergoing three-muscle surgery for horizontal strabismus had good motor outcomes with low incidence of induced lateral incomitance. Motor and sensorial outcomes for esotropia were very stable.
Some clinical characteristics of V-pattern exotropia and surgical outcome after bilateral recession of the inferior oblique muscle: a retrospective study of 22 consecutive patients and a comparison with V-pattern esotropia

Abstract:Background and purpose: A V-pattern exotropia (XT) with bilateral overaction of the inferior obliques (IO) and/or underaction of the superior obliques (SO) is a common finding. The clinical characteristics of this condition in a large series are not available in the scientific literature. Also, data is lacking about the surgical outcome of graded bilateral IO recession. Lastly, a comparison of the pre- and postoperative findings of a V-pattern exotropia population with those of a V-pattern esotropia population is also lacking in the literature. Subjects and methods: Twenty-two consecutive patients without complicating factors were fully evaluated and submitted to bilateral graded recessions of the IO; 19 were also operated for a horizontal binocular alignment imbalance. Results: Preoperative findings: The distribution of V patterns showed 77.2% in the range of 15 to 25 prism diopters (PD) and 22.8% in the range 26 to 35 PD. An overaction of the IO was present bilaterally in 86.4% of the patients and unilaterally in 4.5%. Bilateral underaction of the SO was observed in 4.5% and unilateral in 13.6%. Bilateral overaction of the SO was seen in 18.2% and unilateral in 31.8%. Elevation in adduction was observed bilaterally in 13.6% and unilaterally in 31.8%. A vertical deviation was seen in 50.0% and a marked tendency of it to vanish or diminish in the up- and down positions, as well as at near, was observed. After surgery, 72.7% had less than 15 PD of V pattern or less than 10 PD of A pattern; 36.4% had the V pattern fully corrected. Surgery eliminated a vertical imbalance in 72.7% of the patients, reduced in 9.1%, left unchanged in 9.1% and increased in 9.1% Of the 11 patients devoid of vertical deviation, 72.7% remained so and in 27.3% a vertical deviation was created. Binocularity: There was an improvement of the fusional status with surgery, (ascertained with the Worth Four Dot Test and the major amblyoscope measurement). Conclusion: All cases were in the range of 15-35 PD preop'. Overaction of the IO was a prominent finding. Underaction of the SO was less prevalent as compared with a V-ET population. Overaction of the SO was observed bilaterally in 18.2% and unilaterally in 31.8%, a finding never detected in a V-ET population. A vertical imbalance was observed in one-half of the cases. A good outcome (collapse of the V pattern) was obtained with bilateral graded recession of the IO, This procedure is relatively safe concerning the vertical alignment.
Outcome of unilateral lateral rectus recession and medial rectus resection in primary exotropia

Abstract:Background: The purpose of this study was to measure the success rate of unilateral lateral rectus recession and medial rectus resection in primary exotropia. Methods: This is an interventional case series of 55 patients with primary exotropia (degree of deviation 15-85 PD), above the age of 5 years. Patients were treated in the Department of Ophthalmology, Jinnah Postgraduate Medical Center, Karachi, Pakistan, during the period of July 2009 to March 2010. All the patients underwent surgical procedure i.e., lateral rectus muscle recession (maximum up to 10 mm) and medial rectus muscle resection (up to 6 mm) of one eye, according to the Park's method. Surgery was done based on prism cover test measurements obtained at 6 m with appropriate optical correction in place. Patients were re evaluated at one day, one month, two months and six months post operatively. Final outcome was considered at the end of six months at which achievement of ≤10 PD of exotropia was the success. Data was analyzed on SPSS version 17.0. Results: We obtained success (≤10 PD) in 42 out of 55 patients (76.4%) and 13 out of 55 patients (23.6%) did not meet our criteria for surgical success (>10 PD). Analysis of success with the type of primary exotropia showed that success was achieved in 22 out of 24 cases of intermittent type (91.6%) and 20 out of 31 cases of constant type (64.5%)(P Value 0.019). The highest percentage of success was achieved in patients with the pre-operative deviation of ≤70 PD i.e., 93.3% (42 out of 45 cases), while none of the patients with the pre-operative deviation of >70 PD (10 out of 10 cases) achieved the criteria for success. Conclusion: We conclude that pre-operative deviation is one of the strongest predictor for favorable surgical outcome. Therefore, eliminating the factors causing error in the correct determination of pre-operative deviation should improve the success and predictability of the surgical outcome. Despite the obstacles in the surgical management of strabismus, our results are encouraging.

Botulinum toxin augmented bilateral lateral rectus recession versus three muscles surgery in large-angle intermittent exotropia

Abstract:Aim: To report the surgical outcomes of correcting large angle intermittent exotropia in adult patients by bilateral lateral rectus muscle recession with intraoperative use of botulinum toxin and to compare the results with those of bilateral lateral rectus muscle recession and unilateral medial rectus resection. Methods: The medical records of patients who underwent surgical correction of large angle intermittent exotropia [exotropia >50 prism dioptre (PD)] were retrospectively reviewed. Two groups of patients were identified; Group I (21 patients) had bilateral lateral recti recession augmented with intraoperative botulinum toxin A (BTA) injection into the recessed muscles and group II (30 patients) were treated by bilateral lateral recti recession with unilateral medial rectus muscle resection. Preoperative data were extracted for age, gender, refraction, type of exotropia, angle of stereopsis and angle of deviation. The main outcome measures were the postoperative angle of deviation and stereoacuity angle by Titmus test measured at the end of one year of postoperative follow up. Results: By the end of the first postoperative year, 10 patients in group I (47.6%) and 20 patients in group II (66.7%) achieved esotropia/esophoria <5 PD or exotropia/exophoria <10 PD. The difference in surgical success rate was not statistically significant (P=0.1) but there was a statistically significant higher rate of undercorrection in group I (P=0.03). On the other hand, 3 patients in group I (14.3%) and 5 patients in group II (16.7%) had improved stereopsis; this difference in the sensory outcome was not statistically significant (P=0.8). In the BTA augmented surgery group, good stereoacuity and smaller preoperative angle of deviation were associated with significantly higher surgical success rate (P=0.004, 0.01 respectively). Conclusion: BTA augmented bilateral lateral recti recession is associated with higher rate of undercorrection as compared to bilateral lateral recti recession with unilateral medial rectus resection in the correction of large angle intermittent exotropia. The surgical success rate in BTA augmented surgery group is observed to be higher in patients with preoperative smaller angle of deviation and in patients with good stereoacuity.
Surgical outcomes in sensory exotropia

Abstract:Purpose: To determine success rates over time for strabismus surgery for sensory exotropia and to determine factors associated with successful outcomes. Methods: We retrospectively reviewed medical records of patients with sensory exotropia (best-corrected visual acuity ≤20/200 in the affected eye) who underwent strabismus surgery between May 2009 and December 2019. Patients with paralytic/restrictive exotropia and patients who did not follow up postoperatively were excluded. Surgical success was defined as exotropia of ≤10Δ or esotropia of ≤6Δ. Cox-proportional hazard models were used to evaluate covariate relationships with surgical outcome (α = 0.05). Results: A total of 94 patients (64% female) were included. Mean patient age was 27.2 years (range, 3-69). Mean follow-up was 2.35 ± 2.77 years. The mean preoperative near deviation was 39Δ ± 14.8Δ of manifest or intermittent exotropia. Successful alignment was achieved in 51 of 83 patients (61%) at 1 month, 19 of 32 (59%) at 1 year, and 8 of 16 (50%) at 5 years. We found a significant correlation (P value = 0.0476) between success and smaller surgical doses in patients that underwent one- and two-muscle surgeries. Conclusions: In our study cohort of 94 patients, 50% of patients still had satisfactory ocular alignment at 5 years.

Abstract:Background: The combined superior oblique tendon tuck and inferior oblique recession is the method of choice for the treatment of congenital and acquired superior oblique muscle palsies with large vertical and cyclodeviations. An additional transposition of the insertions is indicated if the vertical deviation differs greatly from the excyclodeviation. Patients and methods: 38 patients who underwent combined oblique muscle surgery for congenital superior oblique muscle palsy were divided into 3 groups. Group A: 14 patients with small excyclodeviations in spite of large vertical deviations, even after diagnostic occlusion of 3 days. A superior oblique tendon tuck of 5 or 6 mm with transposition of the new insertion to the posterior margin of the original insertion was performed combined with a recession of the inferior oblique muscle of 5 or 6 mm with anterior displacement of 4-6 mm. Group B: 15 patients without a discrepancy between the vertical deviation and the cyclodeviation. They were treated with a conventional superior oblique tendon tuck of 5 or 6 mm combined with a recession of the inferior oblique muscle. Group C: 9 patients without a discrepancy between the vertical deviation and the cyclodeviation, but deviations smaller than in group B. A conventional superior oblique tendon tuck of 6 mm combined with a posterior tenotomy of the inferior oblique muscle was performed. The surgical effect was calculated as the difference between the preoperative deviation after diagnostic occlusion of 3 days and the postoperative deviations (1 week and 3 to 6 months). All angles of deviation were measured subjectively after dissociation of the binocular vision with a dark red filter in front of the fixating, not affected eye at 2.5 m distance using the Harms tangent screen. Results: 3 months postoperatively, the vertical deviation was diminished in primary position by 12.1 degrees/6.8 degrees/6.8 degrees in groups A/B/C, in 25 degrees adduction of the affected eye by 18.4 degrees/13.8 degrees/9.5 degrees. The reduction of the cyclodeviation in primary position was 6.4 degrees/8.0 degrees/5.6 degrees. As a side effect a clinically insignificant, consecutive Brown syndrome occurred in each group. In each group a significant positive correlation between the preoperative deviation and the surgical effect was found. Conclusions: The combined superior oblique tendon tuck and inferior oblique recession with transposition of the insertions is a suitable method for a selective treatment of the vertical deviation and the cyclodeviation in congenital superior oblique palsy.
Unilateral lateral rectus recession is an effective surgery for intermittent exotropia in young children

Abstract:Background: Different surgical methods have been suggested for the correction of intermittent exotropia. Unilateral lateral rectus recession has been described as a surgical alternative for small and moderate-angle exotropia. In general, previous studies did not focus on the outcomes of unilateral lateral rectus recession in young children with intermittent exotropia. The purpose of this study is to evaluate the surgical outcomes of unilateral lateral rectus recession in the treatment of moderate-angle exotropia (≤ 25 PD (prism diopters)) in children. Methods: The charts of all patients younger than 12 years of age with moderate-angle exotropia (up to 25 PD) who were operated during the years 2006-2018 were retrospectively reviewed. Fifty-eight patients underwent unilateral lateral rectus recession and had a minimum follow up of 6 months. The angle of exotropia (PD) before and after surgery and the success rate were documented. Results: Mean age at surgery was 6.4 ± 1.9 (range 3.5-11.0) years. Exotropia improved from a preoperative angle of 21.4 ± 4.0 PD to 3.5 ± 5.9 PD postoperatively (p < 0.001). Success rate, defined as deviation of ≤ 10 PD, was achieved in 86.2%. There were 2 (3.4%) cases of overcorrection (consecutive esotropia). There were no intra- or postoperative complications. The mean follow-up duration after surgery was 2.3 ± 1.7 years. Conclusions: In children with moderate angle exotropia, good postoperative success rate was achieved by performing unilateral lateral rectus recession.
Single Stage Surgical Outcomes for Large Angle Intermittent Exotropia

Abstract:Although there were many prior studies about exotropia, few focused on large-angle intermittent exotropia. The goal of this study was to evaluate single-stage surgical outcomes for large-angle intermittent exotropia and analyze risk factors that may affect the success of surgery. Records from intermittent exotropia patients with exodeviations >60 prism diopters(PD) who were surgically treated at the Zhongshan Ophthalmic Center, of Sun Yat-Sen University were reviewed. Included within this review were data on, pre- and post-operative ocular motility, primary alignment, binocular vision and complications. Patients with exodeviations ≤70PD received two-muscle surgery, while those with exodeviations >70PD were subjected to a three-muscle procedure. A total of 40 records were reviewed. The mean exodeviation was 73±9PD at distance and 75±26PD at near. There were 25 patients received two-muscle surgery and 15 the three-muscle procedure. Orthophoria (deviation within 8PD) was obtained in 77.5% of these patients and the ratios of surgical under-correction and over-correction were 15% and 7.5% respectively. However, when combining ocular alignment with binocular vision as the success criteria, success rates decreased to 30%. No statistically significant differences in success rates were obtained between the two- and three-muscle surgery groups. Seven subjects experienced an abduction deficit during the initial postoperative stages, but eventually showed a full recovery. One patient required a second surgery for overcorrection. No statistically significant risk factors for poor outcome were revealed. Our data showed that single-stage two- and three-muscle surgeries for large-angle intermittent exotropia are effective in achieving a favorable outcome.
Long-term Surgical Outcomes of Initial Postoperative Overcorrection in Adults with Intermittent Exotropia

Abstract:Purpose: To evaluate the relationship between initial postoperative overcorrection and long-term surgical success in exotropia patients. Methods: The medical records of 46 patients who underwent surgery for intermittent exotropia after the age of 18 were enrolled. Enrolled patients also had at least 2 years of postoperative follow-up. Based on the initial postoperative deviation at distance measured by prism and the alternating cover test at 1 week, patients were assigned to one of the following groups: group A included patients who demonstrated any esodeviation, while group B included patients who showed orthophoria to exodeviation of 10 prism diopters. The records were analyzed to determine the preoperative deviation with stereoacuity and postoperative deviations with stereoacuity at the follow-up examinations at the following intervals: 1 week; 1, 3, and 6 months; and 1 and 2 years. A comparison between groups for demographic data and preoperative and postoperative angles of deviation was performed using analysis of variance. Results: Of the 46 patients with intermittent exotropia included in this study, 18 (39%) belonged to group A, while 28 (73%) belonged to group B. The postoperative angle of deviation for distant fixation until 2 years of follow-up showed statistically significant differences in each group (p < 0.003 in all comparisons). The amount of exodrift until 2 years in group A (from ?9.7 ± 6.1 to 1.6 ± 3.7) was greater than that in group B (from 2.0 ± 2.7 to 6.8 ± 5.6). The long-term surgical success rate within 2 years of surgery was significantly better in group A than in group B (p = 0.027). The number of patients with intermittent diplopia and the duration of diplopia were greater in group A (n = 8) than in group B (n = 2). Conclusions: Long-term surgical success was achieved in 89% of patients who were initially overcorrected. Overcorrection of an average of 10 prism diopters at the first postoperative week was found to be associated with a more favorable long-term surgical outcome.

Abstract:Purpose: To describe the evolution of a surgical technique for the correction of large-angle incomitant exodeviations. Methods: Retrospective review of an interventional case series from 2005 to 2019 in a single centre, with analysis of surgical procedure, prism diopter (PD) deviations and complications. Results: Thirty-one patients underwent surgery at an average age of 42 years (range 4-75 years) for minimal medial rectus function, mostly from oculomotor nerve palsy (23/31; 74%). The mean pre-operative exodeviation was 75 PD (range 30-200PD). Sixteen patients (52%) had undergone previous strabismus surgery. Thirty-eight operations were performed in which the medial rectus insertion was anchored to the periosteum of the posterior lacrimal crest via a retrocaruncular transconjunctival approach. The ipsilateral lateral rectus (LR) was disinserted and fixed to lateral orbital tissue in 29/38 (76%) operations, injected with botulinum toxin in 5, recessed in 2 and had already undergone maximal LR recession in 2. In all but the first 8 operations, temporary limbal sutures were passed through the eyelids to maximally adduct the globe post-operatively. At last follow-up (mean 24 months; range 2-130), the mean reduction in exodeviation was 49PD (range 10-80) and overall residual deviation was 26PD (range 80PD base-in to 14PD base-out). The 5 LR toxin procedures had a mean reduction of 22PD (range 10-40). Seven patients had persistent diplopia, one a transient corneal erosion and one caruncle suture exposure 4 years after surgery. Conclusion: Large-angle exodeviations can be markedly improved by bi-rectus fixation. This approach is both safe and effective and can be performed in complex patients with multiple previous procedures.
