Abstract:Purpose We sought to determine if online continuing medical education (CME) could improve the knowledge, competence and confidence of cardiologists and primary care physicians (PCPs) related to the diagnosis and treatment of ATTR-CM in underserved patient populations. Methods This CME intervention comprised of a collection of 4 online video-based CME chapters, presented by 3 respected expert faculty. A repeated pairs pre-/post-assessment study design was used. Three multiple choice questions assessed knowledge/competence, and 1 question assessed confidence. Statistical tests to assess significance included the following: paired samples t-test for overall average number of correct responses and for confidence rating; McNemar's test for individual questions (5% significance level, P <.05). Data were collected April 7 through June 14, 2022. Results In total, 134 cardiologists and 112 PCPs were included in the analysis. Overall, 38% cardiologists and 36% of PCPs improved their knowledge and competence, respectively. For the cardiologists, the correct responses significantly increased (P<.001) from 39% (pre) to 49% (post), while twice more respondents answered all questions correctly after education. For the PCPs, the correct responses significantly increased (P<.001) from 30% (pre) to 40% (post), while 150% more respondents answered all questions correctly after education. Further results showed: • Knowledge regarding the unmet needs in the care of patients with ATTR-CM: 16% of cardiologists improved and 54% reinforced; the correct response significantly increased (p<.01) from59% (pre) to 69% (post). For PCPs, 17% improved and 41% reinforced; the correct responses increased (P=.058) from 49% (pre) to 58% (post). • Competence related to the application of strategies to facilitate appropriate diagnosis of patients earlier in the disease course: 16% of cardiologists improved and 31% reinforced; the correct response increased (P=.117) from 40% (pre) to 46% (post); 18% of PCPs improved and 12% reinforced; the correct response significantly increased (P<.01) from 17% (pre) to 29% (post). • Knowledge regarding the treatment options for patients with confirmed ATTR-CM: 19% of cardiologists improved and 13% reinforced; the correct response significantly increased (P<.001) form 17% (pre) to 32% (post); 17% of PCPs improved and 17% reinforced; the correct response increased (P=.058) from 25% (pre) to 345 (post). • Confidence in ability to identify patients at risk of ATTR-CM: 45% of cardiologists and 47% PCPs had a measurable increase in confidence after education, resulting in 23% of cardiologists and 13% of PCPs who were mostly or very confidence after education (P<.001). Conclusion This study demonstrates the success of online CME on improving knowledge, competence and confidence of cardiologists and PCPs related to the diagnosis and treatment of ATTR-CM in underserved patient populations. Continued gaps were identified for future educational targets.
