Abstract:Importance:Microvascular abnormalities in amblyopia are becoming evident with high-resolution imaging, such as optical coherence tomography angiography (OCT-A); however, to our knowledge, the clinical significance and use of these findings are unknown.Objective:To assess changes in quantitative OCT-A metrics in amblyopic eyes and explore their association with visual acuity in children.Design, Setting, and Participants:This population-based nested case-control study included children aged 6 to 8 years who were consecutively recruited between January 2016 and July 2017 from the population-based Hong Kong Children Eye Study (HKCES) at the Chinese University of Hong Kong Eye Centre. All participants underwent OCT-A with a swept-source OCT and detailed ophthalmic investigations. Macular microvasculature of the superficial capillary plexus was quantified by a customized automated image analysis program. A multivariable linear regression was conducted to evaluate the differences in OCT-A metrics between amblyopic and nonamblyopic eyes after adjustment for all known confounders. Data analysis was conducted from September to November 2018.Main Outcomes and Measures:Differences in OCT-A metric (foveal avascular zone [FAZ]) area, FAZ circularity, vessel density, vessel diameter index, and fractal dimension between amblyopic and nonamblyopic eyes.Results:There were 30 participants with amblyopia (mean [SD] age, 7.57 [1.2] years; 16 girls [53.3%]) and 1045 controls (mean [SD] age, 7.65 [1.0] years; 580 girls [55.5%]) in this cohort. Compared with control eyes, amblyopic eyes had decreased FAZ circularity (-0.058; 95% CI, -0.096 to -0.021, P = .002), decreased fractal dimension (-0.014; 95% CI, -0.024 to -0.003; P = .01), and increased vessel diameter index (0.002; 95% CI, 0.002 to 0.003; P < .001). A difference was not identified between FAZ area and vessel density. LogMAR visual acuity was associated with FAZ circularity (sβ, -0.133; P < .001) and vessel diameter index (sβ, 0.097; P = .001) but not with vessel density nor FAZ area.Conclusions and Relevance:The results of this population-based study in children supports the presence of macular microvascular abnormalities in amblyopic eyes. Such changes as measured by OCT-A metrics are associated with visual acuity, inferring retinal involvement in the development of amblyopia and suggesting a potential role of quantitative OCT-A metrics in the diagnosis and recognition of amblyopia.

Abstract:This study aims to quantify fundus tessellated (FT) density and optic disc (OD) morphology using deep learning (DL) techniques and to investigate the correlations between these fundus characteristics and refractive function in young patients with myopia. We constructed two DL-based segmentation models to delineate the FT, OD, peripapillary atrophy (PPA), and macula at a pixel-level resolution. The study sought to identify differences in fundus characteristics between eyes categorized as having high myopia versus mild or moderate myopia. Furthermore, the correlation between fundus measurements and various ocular parameters was statistically analyzed. Correlation analysis indicated that the spherical equivalent and axial length were significantly associated with all fundus measurements ( p < 0.001). Additionally, corneal curvature (K1, K2), lens thickness, and foveal thickness exhibited significant correlations with some of the fundus measurements at a 0.01 significance level. Using DL algorithms, it is feasible to automatically quantify FT and OD characteristics in young myopic patients. The study findings suggest that both FT and OD characteristics are highly correlated with the severity of myopia, particularly as it progresses from mild or moderate to high levels. Moreover, a significant relationship exists between most of these fundus characteristics and a spectrum of refractive function parameters.

Optic disk ovality as an index of tilt and its relationship to myopia and perimetry

Abstract:Purpose: To examine the association of optic disk ovality with myopic refractive error, axial length, and the visual field. Design: Prospective observational case series. Methods: The study included 150 randomly recruited male subjects with myopia. In all cases, one eye was randomly selected, and subjective refraction, slit-lamp examination, applanation tonometry, gonioscopy, A-scan ultrasonography, funduscopy, and color optic disk stereo photography were performed. Disk ovality was assessed using the ratio of minimum to maximum disk diameter (index of tilt). A ratio of < or = 0.8 was considered as significant disk tilt. Visual fields were tested using static automated threshold perimetry with two methods of optical correction: trial lenses and contact lenses. Results: Of the total sample, 137 subjects completed the study. Mean age was 21.2 +/- 1.1 year, and mean spherical equivalent was -6.36 +/- 3.56 diopters. Mean ovality ratio was 0.83 +/- 0.09; 55 subjects (40.2%) had significant tilted optic disks. Greater optic disk ovality (tilt) correlated with greater myopia (P = .009) and longer axial length (P = .009); 95.6% of subjects had normal visual fields with both methods of optical correction. Using multiple linear regression analysis, greater tilt was independently related to a higher mean defect on testing with trial lenses (P < .01). Conclusions: Increased optic disk tilt was associated with higher myopia and reduced sensitivity on field testing. These factors are important in the assessment of glaucoma in patients with myopia.
Imaging Features by Machine Learning for Quantification of Optic Disc Changes and Impact on Choroidal Thickness in Young Myopic Patients

Abstract:Purpose: To construct quantifiable models of imaging features by machine learning describing early changes of optic disc and peripapillary region, and to explore their performance as early indicators for choroidal thickness (ChT) in young myopic patients. Methods: Eight hundred and ninety six subjects were enrolled. Imaging features were extracted from fundus photographs. Macular ChT (mChT) and peripapillary ChT (pChT) were measured on swept-source optical coherence tomography scans. All participants were divided randomly into training (70%) and test (30%) sets. Imaging features correlated with ChT were selected by LASSO regression and combined into new indicators of optic disc (IODs) for mChT (IOD_mChT) and for pChT (IOD_pChT) by multivariate regression models in the training set. The performance of IODs was evaluated in the test set. Results: A significant correlation between IOD_mChT and mChT (r = 0.650, R 2 = 0.423, P < 0.001) was found in the test set. IOD_mChT was negatively associated with axial length (AL) (r = -0.562, P < 0.001) and peripapillary atrophy (PPA) area (r = -0.738, P < 0.001) and positively associated with ovality index (r = 0.503, P < 0.001) and torsion angle (r = 0.242, P < 0.001) in the test set. Every 1 × 10 μm decrease in IOD_mChT was associated with an 8.87 μm decrease in mChT. A significant correlation between IOD_pChT and pChT (r = 0.576, R 2 = 0.331, P < 0.001) was found in the test set. IOD_pChT was negatively associated with AL (r = -0.478, P < 0.001) and PPA area (r = -0.651, P < 0.001) and positively associated with ovality index (r = 0.285, P < 0.001) and torsion angle (r = 0.180, P < 0.001) in the test set. Every 1 × 10 μm decrease in IOD_pChT was associated with a 9.64 μm decrease in pChT. Conclusions: The study introduced a machine learning approach to acquire imaging information of early changes of optic disc and peripapillary region and constructed quantitative models significantly correlated with choroidal thickness. The objective models from fundus photographs represented a new approach that offset limitations of human annotation and could be applied in other areas of fundus diseases.

Abstract:Aims/Purpose: To analyse the parameters of the optic nerve head (ONH) as markers of the progression of myopia with the usage of optical coherence tomography (OCT). Methods: 72 patients (144 eyes) aged 9–57 years with myopia were investigated. The standard OCT parameters of the ONH, the ovality index (OI), the zone of parapapillary atrophy (PPA), location of the lamina cribrosa (LC) of the sclera and the trunk of the central retinal vessels (CRV) were evaluated. Data was statistically approved. Results: A positive correlation between the axial length (AL) and the optic disc cup area, its horizontal and vertical diameters was found. Whilst the volume and the depth of the optic disc cup increased till the AL reached a certain critical value (26.3 mm) with subsequent reduction. An inverse correlation between the AL and the neuroretinal rim area, volume and rim/disc ratio was revealed. These changes in the ONH can be explained by scleral remodelling during elongation of the eye. Formation of γ‐zone PPA and an increase in the β‐zone PPA was accompanied by the tendency of thinning of the retinal nerve fibre layer (RNFL) in the inferior, nasal‐superior and superior sectors of the ONH. In 27.63% cases tilted optic discs were noted, of which 61.90% were vertically and 38.1% ‐ horizontally tilted. The direction of the tilt was determined by the location of the PPA zone with a tendency to thinning of the RNFL in the same sectors. A positive correlation between the AL of eye and the location of the LC of sclera was found. In 29.17% the superficial location of the LC was determined. In 54.17% elongation was accompanied by displacement of the trunk of the CRV on ONH in the nasal ‐ 71.79%, or in the superior‐nasal direction ‐ 28.21%. Conclusions: The revealed changes of optic disc cup, neuroretinal rim and the parapapillary zones; a superficial displacement of LC of sclera and ‘nasalization’ of the trunk of CRV; a decrease in the OI with growth in the AL; relationships of the PPA zone with a tendency to thinning of the RNFL and direction of the tilt can be useful as OCT markers in interpreting normative data in patients with myopia, predicting its progression, and also in differential diagnosis with glaucomatous optic neuropathy.

Compartmental analysis of three-dimensional choroidal vascularity and thickness of myopic eyes in young adults using SS-OCTA

Abstract:Purpose: We aimed to investigate the change of three-dimensional (3D) choroidal thickness (ChT), choroidal vessel volume (CVV), and choroidal vessel index (CVI) in young myopic adults using swept-source optical coherence tomography angiography (SS-OCTA) and compare the difference of these indicators in different quadrants of the macula and optic disc. Methods: A total of 248 eye samples from 135 participants were used in this cross-sectional study. Each participant underwent detailed history taking and ocular examinations. Based on axial length (AL), patients were divided into the emmetropia (EM) group, mild-myopia (MIM) group, moderate-myopia (MOM) group, and high-myopia (HM) group. 6 mm × 6 mm (1,024 × 1024 B-scans) SS-OCTA scans were performed centered on the fovea and optic disc. 3D ChT, CVV, and CVI were measured based on a built-in deep learning algorithm. Differences in ChT, CVV, and CVI were analyzed in different regions and different myopic groups. Results: Significant reduction in the global CVV were found in the HM group (1.930 ± 0.865) in comparison with the EM (3.486 ± 0.992), MIM (3.238 ± 1.033), and MOM (2.589 ± 1.083) groups ( p < 0.001). The global CVI was also lower in the HM group (0.258 ± 0.061) than in the EM (0.320 ± 0.055), MIM (0.320 ± 0.051), and MOM (0.286 ± 0.066) groups ( p < 0.001). The ChT was thinner in eyes with HM (242.753 ± 65.641) than in eyes with EM (377.532 ± 80.593), MIM (348.367 ± 78.191), or MOM (300.197 ± 87.175) ( p < 0.001). Compartmental analysis revealed that ChT, CVV, and CVI in the nasal quadrant of the macula and temporal and inferior quadrants of the optic disc were much lower than those in other quadrants ( p < 0.05). Correlation analyses found that ChT, CVV, and CVI were negatively correlated with AL and spherical equivalence. Conclusion: 3D ChT, CVV, and CVI gradually decreased as the degree of myopia increased. The changes were more dramatic on the nasal side of the macula and the temporal and inferior sides of the optic disc. These findings demonstrated the 3D choroidal change and highlighted the papillo-macular bundle as a sensitive region in myopic development.

Abstract:PURPOSE: To measure the optic disc rotation around the vertical and horizontal disc axis and to evaluate associations with general and ocular parameters.DESIGN: Population-based study.METHODS: In the Beijing Eye Study, 3468 participants (mean age:64.6±9.8 years; range:50-93 years) underwent an ophthalmological examination which included spectral-domain optical coherence tomography (OCT) with enhanced depth imaging. Using the OCT images, we determined the amount of the rotation of the optic disc (defined as Bruch´s membrane opening (BMO)) around the vertical axis and horizontal axis.RESULTS: Optic disc rotation measurements were available for 3037 (87.6%) individuals. In multivariate analysis, larger optic disc rotation around the vertical axis (range:-4.90° to 41.0°) was associated (regression coefficient r:0.27) with high axial myopia (axial length ≥26.5 mm) (P<0.001;standardized regression coefficient beta beta:0.09), longer disc-fovea distance (P = 0.001;beta:0.09) and wider parapapillary beta/gamma zone (P<0.001;beta0.12). Larger optic disc rotation around the horizontal axis (range:-7.10° to 26.4°) was associated (r:0.32) with high axial myopia (P = 0.001;beta:0.08), larger optic disc-fovea angle (P<0.001;beta:0.13), thinner superior nasal retinal nerve fiber layer (RNFL) thickness (P<0.001;beta:-0.19) and thicker inferior nasal RNFL thickness (P<0.001;beta:0.17).CONCLUSIONS: Vertical optic disc rotation was associated with highly myopic axial elongation, increased disc-fovea distance and development or enlargement of parapapillary, Bruch´s membrane free, gamma zone, while macular Bruch´s membrane length is not affected. Horizontal optic disc rotation was associated with inferior dislocation of the fovea, in addition to a thinner superior nasal RNFL and thicker inferior nasal RNFL. The latter association may be taken into account in the interpretation of RNFL thickness profiles.

Abstract:IntroductionThe aim of this work is to investigate the differences in the measurement of foveal retinal thickness in myopic patients between two display modes (1:1 pixel and 1:1 micron) on optical coherence tomography (OCT).MethodsHorizontal OCT line scan through the central fovea was used for manual measurement of foveal retinal thickness under the two display modes, and the values were compared using Wilcoxon signed-rank test. Correlations between the OCT image tilting angle (OCT ITA) and differences in OCT measurement were analyzed by Spearman’s test.Results127 participants with a median age of 28 years, a median spherical equivalent (SE) of − 8.5 D, and a median axial length (AL) of 27.04 mm. There were significant differences between the two display modes, with a median absolute difference (median relative difference) of 13.33 μm (2.75%) for the central foveal thickness (CFT), 5.33 μm (1.28%) for the Henle fiber and outer nuclear layer thickness (HFL + ONL), 3 μm (6.47%) for the external limiting membrane to ellipsoid zone distance (ELM-EZ), and 4 μm (8.77%) for the ellipsoid zone to retinal pigment epithelium distance (EZ-RPE) (all p < 0.05). The differences in foveal retinal thickness between the two display modes were significantly correlated with the OCT ITA (r = 0.732 for CFT, 0.561 for HFL + ONL, 0.642 for ELM-EZ, and 0.471 for EZ-RPE, all p < 0.05).ConclusionsDisparities between the two display modes were found in the manual measurement of foveal retinal thickness and correlated to the OCT ITA.

Abstract:Purpose To investigate the relationship between optic disc morphology, axial length, and regional distribution of retinal vessels in healthy eyes of young adults. Methods Nine hundred and two healthy eyes were enrolled in this university-based, cross-sectional study. Spectral-domain optical coherence tomography angiography was used to measure the parapapillary retinal vessel density. We automated the process of calculating optic disc tilt and rotation by using a program written in Python. Relationships between optic disc rotation, optic disc tilt, parapapillary vessel density, and other ocular parameters were analyzed using regression models. Results As axial length increased, optic disc morphology became more tilted and rotated inferiorly. The superficial vessel density (SVD) and radial peripapillary capillary density (RPCD) gradually decreased in all regions except for the temporal quadrant. Increased temporal SVD (OR [95% CI] = 1.081 [1.039, 1.124], p < 0.001), reduced nasal SVD (OR [95% CI] = 0.898 [0.861, 0.937], p < 0.01), and short relative lens position (OR [95% CI] = 0.126 [0.032, 0.495], p = 0.003) were significantly associated with the presence of a tilted optic disc. Inferior disc rotation was associated with decreased superior deep vessel density (DVD) and increased inferior DVD and temporal DVD after adjusting for sex and axial length. Conclusion The tilted and rotated optic discs were associated with the distribution of SVD and DVD, respectively. We should fully consider the influence of optic disc morphology on parapapillary vessel density in eyes with myopia.

Abstract:Purpose The aim of this study was to investigate changes in the retinal and choroidal thickness between high myopic amblyopia (HMA), low myopia (LM), moderate myopia (MM), high myopia (HM), and normal group (NG) using a spectral-domain optical coherence tomography (SD-OCT). Materials and Methods A total of 75 Chinese children (128 eyes; mean age 10.5 years) were recruited. Retinal thickness (RT) and choroidal thickness (CT) were measured at different locations including subfoveal (SF), and at 0.5 mm/1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to the fovea in superior, nasal, inferior, and temporal sectors using enhanced depth imaging (EDI) system of SD-OCT. Axial length (AL), best-corrected visual acuity (BCVA), and refraction errors were also collected. Results No significant differences were found in subfoveal retinal thickness (SFRT). Moreover, a significantly thinner subfoveal choroidal thickness (SFCT) was found in HMA compared to NG, LM, and MM, but not compared to HM. RT at 0.5 mm to fovea, HMA was significantly thinner compared to LM and MM in the three sectors (superior, inferior, and temporal). Nevertheless, no significant differences were found compared to NG and HM. CT at 0.5 mm to fovea, HMA was the significantly thinnest in all four sectors compared to NG, LM, and MM. RT at 1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to fovea, HMA was thinner compared to NG, LM, and MM. CT at 1.0 mm/1.5 mm/2.0 mm/2.5 mm/3.0 mm to fovea, HMA was thinner compared to NG, LM, and MM. At the superior and inferior sectors, HMA showed to be statistically thinner compared with HM. Moreover, SFCT in the HMA, HM, and NG were negatively correlated with AL. Conclusions Thinner retina and choroidal tissue appear to be related to HMA, and thus can be used as useful parameters for discovering the underlying mechanisms of the disease.

Abstract:AimsTo investigate the association between the myopic severity and retinal microvascular density, choroidal vascularity and retrobulbar blood flow in adult anisomyopes.MethodsThis study comprised 90 eyes of 45 myopic anisomyopes who were recruited for Colour Doppler imaging (CDI) and optical coherence tomography angiography (OCTA). The superficial vessel density (SVD), deep vessel density (DVD), choroidal thickness (ChT) and choroidal vascularity, including total choroidal area (TCA), luminal area (LA), stromal area (SA) and Choroidal Vascularity Index (CVI), were measured using OCTA. Moreover, the Pulsatile Index, peak systolic velocity (PSV) and end diastolic velocity (EDV) of posterior ciliary artery (PCA), central retinal artery (CRA) and ophthalmic artery (OA) were quantified by CDI, and all parameters were compared between two eyes and the correlations among parameters were analysed.ResultsThe mean difference of spherical equivalent (SE) and axial lengths (AL) between eyes were −6.00±2.94 D and 2.48±1.31 mm, respectively. The SVD, DVD, ChT, TCA, LA, SA and CVI were significantly lower in more myopic eyes compared with the contralateral eyes. In more myopic eyes, CDI parameters of CRA and PSV and EDV of PCA were also significantly lower. After adjusting for age and sex, the binocular asymmetry in LA and ChT was independent risk factor affecting interocular difference in both AL and SE.ConclusionRetinal microvascular density, choroidal vascularity and retrobulbar blood flow were simultaneously lower in adult myopic anisomyopes with more myopic eyes and disturbed choroid circulation was related to the severity of myopia. Further longitudinal study was helped to identify the effect of choroidal parameters for myopic progression.

Abstract:PURPOSE. To investigate the difference between two-dimensionally measured disc diameters (DDs) based on fundus images and three-dimensionally measured DDs based on enhanced depth imaging optical coherence tomography (EDI-OCT).METHODS. The DDs were three-dimensionally measured on EDI-OCT images of optic nerve heads and two-dimensionally measured on near-infrared reflectance fundus images in 90 normal eyes of 90 subjects. Disc rotation around sagittal axis was defined as the angle between maximal DD and vertical axis. Disc rotation around horizontal and vertical axes was defined as cosine of DD on fundus images divided by DD on EDI-OCT images.RESULTS. Mean optic disc rotation around vertical axis was 14.4 +/- 9.38, rotation around sagittal axis was 23.0 +/- 21.38, and rotation around horizontal axis was 4.7 +/- 6.68. Horizontal, vertical, minimal, and maximal DDs as measured three-dimensionally were significantly (P < 0.001) higher than those measured two-dimensionally. Difference between three-dimensional and two-dimensional measurement of horizontal DD and vertical DD was associated with axial length (r = 0.38; P < 0.001, and r = 0.23; P = 0.03, respectively). Difference between three-dimensional and two-dimensional measurements was larger (P < 0.001) for horizontal DD (69.6 +/- 68.1 mu m) than for vertical DD (14.8 +/- 25.6 mu m). Correspondingly, the ratio of maximal-to-minimal DD (ovality index) was significantly larger (P < 0.001) for two-dimensional (1.33 +/- 0.21) than for three-dimensional (1.27 +/- 0.16) measurements.CONCLUSIONS. Two-dimensional measurement of DDs leads to falsely low results, more for horizontal DD than for vertical DD. The discrepancy between low two-dimensional compared to three-dimensional disc measurements increases with longer axial length. The angle of disc rotation around vertical and horizontal axes can be measured with EDI-OCT imaging.
Correlative Comparison of Three Ocular Axes to Tilt and Decentration of Intraocular Lens and Their Effects on Visual Acuity

Abstract:Purpose: To evaluate which ocular axis, the corneal topographic axis (CTA), pupillary axis (PA) or line of sight (LOS), for measuring the tilt and decentration of intraocular lens (IOL) is most relevant to correct distance visual acuity (CDVA). Methods: A Scheimpflug device (Pentacam HR) was prospectively used to determine the tilt and decentration of IOLs in vivo 3 months after cataract surgery. A new method was developed to reliably measure PA and LOS. We further evaluated CTA and then used Spearman correlation coefficient and linear regression to assess the correlation between CDVA and IOL displacement based on the data of three different ocular axes. Results: Forty-six eyes from 46 patients were evaluated. The majority of decentration and tilt of IOL with reference to CTA, PA and LOS were towards the subtemporal direction. We found that the horizontal meridian data measured using CTA and PA were statistically significantly different (p = 0.011 for tilt; p = 0.005 for decentration). The correlation between CDVA and the distance of decentration temporally (r = –0.344, p = 0.035) and inferiorly (r = –0.336, p = 0.042) of the IOL with regard to CTA was significant. PA and LOS measurements had no correlation with any indices. Conclusion: Assessment of tilt and decentration of the IOL with reference to different ocular axes was markedly different. Abstract:BackgroundTo determine the influences of myopia and optic disc size on ganglion cell-inner plexiform layer (GCIPL) and peripapillary retinal nerve fiber layer (RNFL) thickness profiles obtained by spectral domain optical coherence tomography (OCT).MethodsOne hundred and sixty-eight eyes of 168 young myopic subjects were recruited and assigned to one of three groups according to their spherical equivalent (SE) values and optic disc area. All underwent Cirrus HD-OCT imaging. The influences of myopia and optic disc size on the GCIPL and RNFL thickness profiles were evaluated by multiple comparisons and linear regression analysis. Three-dimensional surface plots of GCIPL and RNFL thickness corresponding to different combinations of myopia and optic disc size were constructed.ResultsEach of the quadrant RNFL thicknesses and their overall average were significantly thinner in high myopia compared to low myopia, except for the temporal quadrant (all Ps ≤0.003). The average and all-sectors GCIPL were significantly thinner in high myopia than in moderate- and/or low-myopia (all Ps ≤0.002). The average OCT RNFL thickness was correlated significantly with SE (0.81 μm/diopter, P < 0.001), axial length (-1.44 μm/mm, P < 0.001), and optic disc area (5.35 μm/mm2, P < 0.001) by linear regression analysis. As for the OCT GCIPL parameters, average GCIPL thickness showed a significant correlation with SE (0.84 μm/diopter, P < 0.001) and axial length (-1.65 μm/mm, P < 0.001). There was no significant correlation of average GCIPL thickness with optic disc area. Three-dimensional curves showed that larger optic discs were associated with increased average RNFL thickness and that more-myopic eyes were associated with decreased average GCIPL and RNFL thickness.ConclusionMyopia can significantly affect GCIPL and RNFL thickness profiles, and optic disc size has a significant influence on RNFL thickness. The current OCT maps employed in the evaluation of glaucoma should be analyzed in consideration of refractive status and optic disc size.

Abstract:BACKGROUND: The basic method of glaucoma diagnosis is visual field examination, however, in patients with high myopia, the diagnosis of glaucoma is difficult.AIM: To explore the value of optical coherence tomography (OCT) for measuring optic disc parameters and macular thickness as a screening tool for glaucoma in patients with high myopia.METHODS: Visual values (contrast sensitivity, color vision, and best-corrected visual acuity) in three groups, patients with high myopia in Group A, patients with high myopia and glaucoma in Group B, and patients with high myopia suspicious for glaucoma in Group C, were compared. Optic disc parameters, retinal nerve fiber layer (RNFL) thickness, and ganglion cell layer (GCC) thickness were measured using OCT technology and used to compare the peri-optic disc vascular density of the patients and generate receiver operator characteristic (ROC) test performance curves of the RNFL and GCC for high myopia and glaucoma.RESULTS: Of a total of 98 patients admitted to our hospital from May 2018 to March 2022, totaling 196 eyes in the study, 30 patients with 60 eyes were included in Group A, 33 patients with 66 eyes were included in Group B, and 35 patients with 70 eyes were included in Group C. Data were processed for Groups A and B to analyze the efficacy of RNFL and GCC measures in distinguishing high myopia from high myopia with glaucoma. The area under the ROC curve was greater than 0.7, indicating an acceptable diagnostic value.CONCLUSION: The value of OCT measurement of RNFL and GCC thickness in diagnosing glaucoma in patients with high myopia and suspected glaucoma is worthy of development for clinical use.

Abstract:Objective To study the characteristics of retinal nerve fiber layer(RNFL)thicknes and its associated factors. Methods According to the myopia degree 213 subjects were divided into 3 groups:51 subjects were in slight myopia group,91 subjects were in medium myopia group and 71 subjects were in high myopia group. The different quadrants RNFL thickness, diskarea,rimarea cup/disk area ratio ,intraocular pressure (IOP)and pupil diameters in each groups were measured by optical conhenrence tomography(OCT) , noncontact tonometer and Corneal topograph respectively.The data were analyzed by SPSS statistics software system. Results Compared with each group, the difference between the different quadrants RNFL thickness in myopia groups was significant (P<0.01), except the superior quandrant RNFL thickness in slight and medium myopia groups(P>0.05). The RNFL thicknesses in inferior, superior , nasal and temporal quadrant are associated with spherical equivalent diopter(r=-0.259、-0.316、-0.183、0.318, P <0.01) . The mean RNFL thickness is associated with diskarea,rimarea and cup/disk area ratio (r =0.165, P <0.05;r =0.262, P <0.01;r =-0.140, P <0.05). The mean RNFL thickness is not related with I0P and pupil diameters(r =-0.088、0.050, P >0.05). Conclusion The RNFL thicknesses in inferior, superior and nasal quadrant are thinner and thinner with an increase of myopia degree.But the RNFL thickness in temporal quadrant was thicker. The RNFL thickness become thicker with an increase of diskarea,rimarea and become thinner with an increase of cup/disk area ratio.

Abstract:Purpose: To evaluate the effects of age and race on optic disc, retinal nerve fiber layer (RNFL), and macular measurements with spectral-domain optical coherence tomography (SD OCT). Design: Cross-sectional observational study. Participants: Three hundred fifty adult subjects without ocular disease. Methods: Data from SD OCT imaging of the optic nerve head, peripapillary RNFL, and macula of 632 eyes from 350 subjects without ocular disease were imaged with SD OCT. Multivariate models were used to determine the effect of age and race on quantitative measurements of optic disc, RNFL, and macula. Main outcome measures: Optic nerve, RNFL, and macular measurements with SD OCT across racial strata and age. Results: For optic nerve parameters, participants of European descent had significantly smaller optic disc area than other groups (P<0.0001), and Indian participants had significantly smaller rim area than other groups (P<0.0001). Indian and Hispanic participants had thicker global RNFL measurements than other groups (P<0.0001). Participants of African descent were associated with thinner inner retinal thickness in the macula (P<0.0001). Age was associated with several parameters, with rim area reducing by 0.005 mm(2)/year, RNFL thickness reducing by 0.18 μm/year, and inner retinal thickness reducing by 0.1 μm/year (P<0.0001 for all age associations). Conclusions: Optic nerve, RNFL, and macular measurements with SD OCT all varied across racial groups and with age. These differences are important in defining the range of normal variation in differing populations and should be considered in the use of these instruments in the detection of optic nerve and macular disease across these population groups. Macular Measurements Using Spectral-Domain Optical Coherence Tomography in Chinese Myopic Children.

Abstract:PURPOSE:To evaluate the macular thickness/volume in Chinese myopic children using spectral-domain optical coherence tomography (SD-OCT) and assess its correlation with spherical equivalent refraction (SER), axial length (AL), sex, and age.METHODS:A total of 194 eyes from 194 children (aged 6-17 years old) with emmetropia (-0.5 diopters [D] < SER ≤ 0.5 D), low myopia (-3.0 D < SER ≤ -0.5 D), and moderate to high myopia (SER ≤ -3.0 D) were recruited in the study. Each child underwent standardized ophthalmic examinations including visual acuity (VA), cycloplegic refraction, and AL measurement. The macular thickness for the nine Early Treatment Diabetic Retinopathy Study (ETDRS) regions and the average macular thickness/volume were measured and calculated.RESULTS:Analyses of macular thickness for the ETDRS regions showed that the fovea was the thinnest of the nine regions, followed by the outer ring; the inner ring was the thickest. When compared to children with emmetropia or low myopia, children with moderate to high myopia tended to have greater foveal thickness, thinner quadrant-specific thickness in the outer ring, and smaller average macular thickness/volume. Also, there were significant differences in foveal, superior outer, inferior outer, and temporal outer quadrants among lowest, middle, and highest AL groups. Boys were found to have greater macular thickness than girls in fovea and inner ring regions.CONCLUSIONS:Our study highlights the variations and sex differences of macular thickness/volume in Chinese myopic children using SD-OCT.
