Abstract:The aim of this study was to determine the factors influencing adherence to self-care behaviors among low health literacy hypertensive patients based on health belief model. A cross-sectional study was conducted among 152 hypertensive patients with low health literacy. Patients with limited health literacy were identified by S-TOFHLA. The data were collected using H-scale for assessing self-care behaviors and, HK-LS for assessing knowledge of hypertension. A researcher-made questionnaire was applied for collecting data of health belief model constructs. Data were analyzed by SPSS version 22 with using multiple logistic regression analyses. Perceived self-efficacy was associated with all self-care behaviors except medication regimens. There was a significant association between perceived susceptibility and adherence to both low-salt diet (OR = 3.47) and nonsmoking behavior (OR = 1.10). Individuals who had more perceived severity (OR = 1.82) had significantly greater adherence to their medication regimens. Perceived benefits and barriers were not significantly associated with either type of hypertension self-care behaviors. It seems that designing and implementation of educational programs to increase self-efficacy of patients and promote their beliefs about perceived susceptibility and severity of complications may improve self-care behaviors among low health literacy hypertensive patients.

Disease awareness, self-efficacy status and influencing factors of hospitalized coronary heart disease patients

Abstract:Objective To investigate coronary heart disease patients hospitalized for disease awareness and self-efficacy status and influencing factors. Methods From February 2012 to December 2012, 230 patients with coronary artery dis-ease as an object of study in Hangzhou Normal University Affiliated Hospital were collected general information and disease-related information, the use of coronary heart disease knowledge and cognitive scale and patients with coronary heart disease awareness and self-efficacy survey by self-efficacy scale. Results The group of patients with coronary heart disease knowledge and cognitive scale score points was (12.31 ±2.22), score index was (0.61± 0.02), low cognitive level. Of different ages, education levels, economic levels, and on the job and retirement, urban and rural, with or without a family history of patients score differences were statistically significant( P < 0.05), the multi-factor analysis showed that level of education, occupational status, living, the economic level and family history of coronary heart dis-ease risk awareness of the disease were statistically significant( P < 0.05). The group of patients with coronary heart disease self-efficacy score was (84.19 ±6.54), score index was (0.70±0.03), self-efficacy was not high. Different ages,education level, economic level and high salt and high-salt diet, adhere to exercise, urban and rural, multi-factor analysis showed that patients with or without a family history of score differences were statistically significant( P < 0.05), age,level of education, exercise, place of residence and economic level of self-efficacy in patients with coronary heart dis-ease were statistically significant( P < 0.05). Conclusion The level of knowledge of the disease and self-efficacy of coronary heart disease hospitalized patients is low. Developed nursing management measures for the influencing factors will be beneficial to patients with coronary heart disease awareness and self-efficacy.
Predictors of self-care behaviors in patients with hypertension: The integrated model of theories of "planned behavior" and "protection motivation"

Abstract:Background: Assessment of self-care behaviors in patients with hypertension may provide clinicians and practitioners with important information about how to better control hypertension. Therefore, the present study aimed to explore the predictors of self-care behaviors in patients with hypertension based on an integrated model of theories of planned behavior and protection motivation. Materials and methods: The present cross-sectional study was part of a larger study, conducted in 2022 on 344 hypertensive patients from Khamir County in Hormozgan Province in Iran. The sample was selected through a systematic random sampling. The patients completed a researcher-made questionnaire based on constructs of the theory of planned behavior and protection motivation theory. They also completed, Hypertension Self-Care Activity Level Effects (H-SCALE), Hypertension Knowledge-Level Scale (HK-LS), and a demographic questionnaire. The data were analyzed in SPSS 21 using the Pearson correlation coefficient, independent-samples T test, one-way ANOVA, and linear regression analysis. Results: The participants' mean age was 47.34 ± 13.68 years. Adherence to medication, physical activity, weight management, diet, and nonsmoking scores were 42.7, 27.3, 66.3, 14.2, and 81.1, respectively. The linear regression model showed that behavioral beliefs (β = 0.366, P < 0.001), perceived rewards (β = -0.248, P < 0.001), control beliefs (β = 0.133, P = 0.013), and normative beliefs (β = 0.143, P = 0.025) were the major predictors of self-care behaviors in patients with hypertension. Conclusion: In light of the present findings, it can be concluded that it is crucial to change patients' attitudes toward self-care behaviors and improve the social acceptance of behavior and its development in society, and improve patients' control beliefs through goal-oriented education that develops resistance against the perceived rewards of the unfavorable behavior. Considering these constructs can act as strategies to promote self-care behaviors among patients.

Abstract:Abstract Introduction: Self-care programs can raise health in patients with Multiple Sclerosis (MS). This study aimed to identify the self-care behaviors and determinants in patients with MS according to the Health Belief Model (HBM). Methods: In this cross-sectional study, we included 280 MS patients through convenience sampling method. The collection tool was a self-administered questionnaire based on HBM. The participants were the members of MS society in Kerman, Iran. The data were analyzed using descriptive statistics, path analysis, and multivariable linear regression in SPSS software Version 22. Results: The mean (SD) score for self-care practices was 2.86 (0.64), and medication adherence was the most conducted practice. The perceived benefits and cues to action exerted positive influence on self-care practices. The most frequent symptoms experienced by the participants were fatigue (82.5%), visual impairment (76.4%), headaches (72.1%), and muscle weakness (71.4%). The most important cues to action for self-care behavior were the physician (77%), media (52%), and other MS patients (32%). Conclusion: The quality of life (QOL) of MS patients is heavily influenced by self-care behaviors. In this study, only about half of the patients accomplished self-care behaviors, which seems to be insufficient. Since the perceived benefits and cues to action are the main predictors of self-care practices, intervention based on these two constructs can be utilized to promote self-care programs and QOL in MS patients. Health-care providers should pay more attention to these factors for promoting self-care behaviors.

Abstract:Introduction: Chronic kidney disease is a serious problem and patients need active self-care. This study focuses on the relationship between self-care and its predictive factors in hemodialysis (HD) patients. Methods: This multicenter correlational study was conducted on 201 HD patients referring to HD centers in the west of Tehran, Iran. A multi-stage sampling method was used to select the samples. Data were collected using self-care Scale, Paloutzian-Ellison Spiritual Well-Being Scale, and Connor Davidson Resilience Questionnaires. Data were analyzed using SPSS software version 13 and descriptive statistics and linear regression with stepwise method. Results: The mean (SD) scores of self-care, resilience, and spiritual well-being were 35.5 (5.69), 53.4 (12.94), and 97.4 (17.9), respectively. Univariate analysis showed that self-care had a statistically significant relationship with gender, occupation, education, being diabetic, and hyperlipidemia. Self-care positively correlated with the resilience and negatively correlated with age. Multivariate regression indicated that the resilience (β=0.78, P=0.001), being diabetic (β=-0.09, P=0.01), and age (β=-0.11, P=0.005) could be predictors of self-care. These variables accounted for 78% of variance in self-care in HD patients. Conclusion: Our results showed that resilience was positively correlated with self–care, but being diabetic and age were negatively correlated with self-care in HD patients. Therefore, the health care providers should pay more attention to HD patients who are diabetic and older, as they are at a higher risk of having impaired self-care.
Association between perception of caring behaviors and self-efficacy in patients with cardiovascular disease at coronary care units: a cross-sectional study

Abstract:BACKGROUND: Assessing and improving patient self-efficacy are among the major roles of nurses. Nurses are also responsible for providing direct patient care, and they play a major role in improving patient care quality. Therefore, it is essential to evaluate nurses' care-giving behaviors. This study aimed to determine the association between caring behaviors and self-efficacy in patients with cardiovascular disease.METHODS: In this cross-sectional study, 400 patients with cardiovascular disease who were admitted to hospitals in Jahrom, southern Iran, were selected through a stratified sampling. The Caring Behaviors Inventory and the Strategies Used by People to Promote Health questionnaires were used to collect data. Data were analyzed using descriptive statistics, Kolmogorov-Smirnov test and Spearman correlation coefficient in SPSS ver. 22.RESULTS: Results showed significant relationships between perception of caring behaviors and self-efficacy (r=0.16, P=0.001) as well as subscales of respectful deference to others (r=0.12, P=0.01), assurance of human presence (r=0.12, P=0.02), and positive connectedness (r=0.18, P=0.001). Additionally, among the subscales of caring behaviors, "attentive to others' experience," with a mean of 5.17±1.10, was the highest priority and "positive connectedness," with a mean of 4.81±1.31, was the lowest priority for patients. The mean self-efficacy score was 73.94±29.78, and 169 patients (43.2%) had low self-efficacy.CONCLUSIONS: Given the positive relationship between perception of caring behaviors and self-efficacy in patients with cardiovascular disease, self-efficacy could be improved by paying more attention to patient care priorities and improving patient perception of caring behaviors.

Abstract:BACKGROUND: An assessment of an individual's hypertension self-care behavior may provide clinicians and practitioners with important information regarding how to better control hypertension.OBJECTIVES: The objective of this study was to investigate the self-care behaviors of hypertensive patients.PATIENTS AND METHODS: This cross-sectional study was conducted in 2014 in a sample of 1836 patients of both genders who had been diagnosed with hypertension in urban and rural health centers in the Kohgiluyeh Boyerahmad Province in southern Iran. They were randomly selected and were invited to participate in the study. Self-care activities were measured using the H-hypertension self-care activity level effects.RESULTS: The mean age of the respondents was 63 (range: 30 - 92), and 36.1% reported adherence to the recommended levels of medication; 24.5% followed the physical activity level guidelines. Less than half (39.2%) met the criteria for practices related to weight management, and adherence to low-salt diet recommendations was also low (12.3%). Overall, 86.7% were nonsmokers, and 100% abstained from alcohol. The results of a logistic regression indicated that gender was significantly associated with adherence to physical activity (OR = 0.716) and non-smoking (OR = 1.503) recommendations; that is, women were more likely to take part in physical activity than men. There was also a significant association between age and adherence to both a low-salt diet (OR = 1.497) and medication (OR = 1.435).CONCLUSIONS: Based on our findings, it is crucial to implement well-designed educational programs to improve hypertension self-care behaviors.

Abstract:Software switching has been largely dismissed as an inefficient option compared to hardware switching, both in terms of cost and performance. Recent increases in processing power for commodity hardware has given reason to revisit software switching. The flexibility of a commodity hardware, software based platform allows us to rapidly create prototypes, while the increased performance of the hardware is starting to enable software based network devices to be used in production environments. Combining these two aspects of commodity hardware based switching has led us to create our own prototype implementation of a next generation frame forwarding protocol. In this paper, we detail the architecture of our implementation and compare the performance of Ethernet switching platforms to our implementation with a synthetic benchmark, designed to measure the maximum frame forwarding rate of the platforms. We explore the throughput characteristics of all platforms in single- and multi core configurations, while keeping the rest of the platform configuration as identical as possible.
Predictive Role of Resilience and Hope on Adherence to Treatment in Hemodialysis Patients

Abstract:Objective: To determine the predictive role of resilience and hope on adherence to treatment in hemodialysis patients hospitalized in two hospitals affiliated to Shiraz University of Medical Sciences (Shiraz, Iran). Methods: This is a descriptive-analytical study that was conducted in 2021-2022 on 120 patients treated in hemodialysis sections in Namazi and Shahid Faqihi teaching hospitals. Sampling was conducted using a stratified random method. Demographic information questionnaires, Connor and Davidson's resilience, Snyder's hope and adherence to kidney patients' treatment questionnaires were used to collect the data. Results: The finds showed that the levels of resilience, hope, and adherence to treatment had hight level. More specifically, it was indicated that the mean and standard deviation for the total resilience score, the hope variable, and adherence to total treatment was 75.45±14.34, 40.43±3.66, and 80.12±18.20, respectively; which have maximum possible scores of 100, 48 and 100. Thus, it can be said that no correlation was observed between resilience and adherence to treatment variables (p>0.05); hope variable and adherence to treatment (p>0.05), and adherence to treatment with hope and resilience variables (p>0.05). However, hope and resilience variables showed a direct and weak correlation with each other (r=0.36, p<0.05); that is, patients who had more hope indicated better resilience as well. Conclusion: Although in this study we found that the resilience and hope variables were not able to predict the treatment adherence, hope and resilience indicated a direct and weak correlation. It is recommended that nurses should pay more attention to hope and resilience of hemodialysis patients in order to promote their health.
Predictors of Adherence to Self-Care in Rural Patients With Heart Failure

Abstract:Background: The common reality of not following a recommended course of treatment is a major cause of poor health outcomes in patients with heart failure (HF). The purpose of this study was to identify predictors of adherence to HF self-care recommendations in rural HF patients who received an intervention to promote symptom management and self-care. Methods: Data from 349 rural HF patients (42% female, 90% Caucasian) randomized to the intervention arms of the study were used. Adherence was measured using the European Heart Failure Self-Care Scale questionnaire, a brief measure that asks patients to report their adherence to a variety of recommended HF symptom management behaviors (ie, daily weight monitoring, when to call the physician, medications, diet, and exercise). The following predictors were tested: age, gender, marital status, education level, depression score (measured using PHQ-9), anxiety score (measured with the Brief Symptom Inventory), and level of perceived control (measured using Control Attitudes Scale-R). Multivariate linear regression was used to test the model. Results: The model to predict adherence was significant (P < .0001). Of the covariates tested in the regression model, being a male (P = .009), having less anxiety (P = .018), not being depressed (P = .017), and having higher perceived control (P = .003) were predictors of improved self-care score at 3 months. Conclusion: Adherence is a multifaceted and a challenging behavior based on the assumption that the patient agrees with self-care recommendations. These data suggest interventions designed to promote adherence behaviors should include an assessment of gender, anxiety, depression, and perceived control for optimal outcomes.
The relationship between perceived social support and self-care behaviors among patients with heart failure in Jordan

Abstract:Perceived social support is known to affect self-care. No previous studies have been conducted to determine the association between perceived social support and self-care among patients with heart failure in Jordan. This study aimed to identify the predictors of self-care behaviors among patients with heart in Jordan. A cross-sectional correlational design was used. A convenience sample of 254 patients with heart failure was invited to participate. The Personal Resource Questionnaire (PRQ)-2000 and the Self-Care Heart Failure Index (SCHFI) were used to measure perceived social support and self-care among patients with heart failure in Jordan. The mean perceived social support score was low at 59 ( SD = 15). The mean social support score was also low at 59 ( SD = 25). The maintenance subscale score ranged from 10–86.66 and achieved the highest mean (57.77), followed by the management subscale (mean = 56.29, range10-90), while the confidence subscale mean was the lowest (mean = 50.80, range 5.56–88.96). All self-care subscales mean scores were below the target level (≥ 70) for self-care. Lower perceived social support, lower income, older age, smaller duration of disease, less persons living at home, worse cardiac functional status, and the presence of hypertension or diabetes were the significant predictors of low self-care. The culture and the health care system may clarify some results. Providing patients with sufficient social support enhances self-care. More consideration should be provided to the elderly and patients with low income, worse functional status, short duration of disease, hypertension, and diabetes to enhance self-care.

Abstract:Abstract Background: Kidney failure is one of the most important late complication in type 2 patients with diabetes, so more of half-people are wait to graft kidney are people that caused to diabetes. The purpose of this study, was predict to care of kidney in type 2 diabetes patients based on Health Belief Model (HBM). Methods: In this analytical cross-sectional study, 242 patients with type 2 diabetes select with simple sampling of health centers in Arak ( a city in Iran) in 2020 and health behaviors of kidneys collected based on HBM with reliable and validity of questionaries’ and analyses by linear regression logistic. Results: The mean age of patients was 47.51 ±7.86 years. FBS average was 159.65 ± 55.01 mi/dc and average of performance of patients about care of kidney was 30/31± 6/42. Among HBM construct predicted about %38 perceived severity, %67.6 knowledge, %31.8 self-efficacy and %16.4 internal cues to action, these cases have the most power for prediction and total of construct predict health behavior of kidneys that are about %49. Conclusion: This study showed that increasing patients information in field of recognizing kidney failure, emphasis on role of cues to action and finally increasing self-efficacy in patients with type 2 diabetes about care of kidney should be emphasis in education programs for preventing of kidney failure.
A Mixed-Methods Study of Social Determinants and Self-care in Adults With Heart Failure

Abstract:Background: The burden of heart failure (HF) is unequally distributed among population groups. Few study authors have described social determinants of health (SDoH) enabling/impeding self-care. Aim: The aim of this study was to explore the relationship between SDoH and self-care in patients with HF. Methods: Using a convergent mixed-methods design, we assessed SDoH and self-care in 104 patients with HF using the Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) and the Self-Care of HF Index v7.2 with self-care maintenance, symptom perception, and self-care management scales. Multiple regression was used to assess the relationship between SDoH and self-care. One-on-one in-depth interviews were conducted in patients with poor (standardized score ≤ 60, n = 17) or excellent (standardized score ≥ 80, n = 20) self-care maintenance. Quantitative and qualitative results were integrated. Results: Participants were predominantly male (57.7%), with a mean age of 62.4 ± 11.6 years, with health insurance (91.4%) and some college education (62%). Half were White (50%), many were married (43%), and most reported adequate income (53%). The money and resources core domain of PRAPARE significantly predicted self-care maintenance ( P = .019), and symptom perception ( P = .049) trended significantly after adjusting for other PRAPARE core domains (personal characteristics, family and home, and social and emotional health) and comorbidity. Participants discussed social connectedness, health insurance coverage, individual upbringing, and personal experiences as facilitators of self-care behavior. Conclusion: Several SDoH influence HF self-care. Patient-specific interventions that address the broader effects of these factors may promote self-care in patients with HF.
Efficacy of a home-based educational strategy involving community health volunteers in improving self-care in patients with chronic heart failure in western Iran: A randomized controlled trial

Abstract:BACKGROUND: This study aimed to evaluate the effectiveness of a home-based educational strategy using community health volunteers (CHVs) in improving self-care of patients with chronic heart failure (CHF) in comparison with an educational strategy using formal health professionals (FHPs) at hospital, and also with a control group receiving usual care in western Iran.METHODS AND RESULTS: A three-arm controlled trial randomly allocated 231 patients with CHF into a control group and two intervention groups undertaking two different educational approaches - a face-to-face education program by CHVs at the patients' homes and a formal education program using paid FHPs at hospital. Data obtained through interviewing patients before and two months after interventions were analyzed. Self-care components significantly increased after the intervention in both interventional groups compared to the control group (p<0.001). Differences between the two strategies were not significant, except for self-care confidence being greater in the groups exposed to the health professionals (p=0.004). The mean and standard deviation (SD) of the changes in self-maintenance, self-management and self- confidence score (each with a maximum score of 100) for the CHVs group were 26.2±12.7, 29.4±11 9.5±17; for the health professional group were 29.5±12, 31.3±12, 18.1±17; and for the control group were 2.7±9, 10.2±10, -0.30±11 respectively.CONCLUSION: The home-based face-to-face education by CHVs improved self-care maintenance and self-care management in patients with CHF as effectively as the education provided by health professionals in a formal health education program, and much better than the usual care.

Abstract:Heart failure (HF), a worldwide epidemic with significant morbidity and mortality risks, is frequently secondary to cardiovascular disorders and probably is the common final way to survive patients. Almost 25% of hospitalized patients with acute HF are expected to be readmitted within 30 days post-discharge, and the rates of rehospitalization increase to almost one-third at 60 days and 60 percent within one year of discharge. Although care planning for patients with heart failure is complex, multidisciplinary, and resource-dependent, optimal self-care management along with appropriate educational intervention and follow-up strategy could be able to reduce readmissions, decline the duration of hospitalization, increase life expectancy, decrease the rates of mortality, and reduce costs of healthcare services for patients with HF. However, there are contradictions in previous reports about the efficacy of self-care, mainly due to patients' non-adherence to self-care behaviors. Therefore, the current study aimed to review the investigations on the effectiveness of self-care of HF patients in reducing hospital readmissions and increasing quality of life, and discuss novel approaches for predischarge educational interventions and postdischarge follow-up strategies.
Effects of health belief model in promoting self-care behaviors among hypertensive older adults

Abstract:BACKGROUND: Aging has the fastest growth rate in age groups. With age, the prevalence of a variety of chronic diseases also increases. Among chronic diseases, hypertension is one of the most important diseases among older adults and self-care is one of the effective approaches to control it. The aim of the present study was to apply the health belief model (HBM) in promoting self-care in hypertensive older adults to help improve self-care behaviors in this age group.SETTINGS AND DESIGN: This was a quasi-experimental study. This study was conducted at comprehensive health centers in the east of Guilan province, Iran.METHODS AND MATERIAL: This intervention study was conducted on 106 elderly people with hypertension who were selected with a multi-stage random sampling method from those referred to east health centers of Gilan, Iran, in 2019. The data gathering tool was a questionnaire including demographic characteristics, HBM structure, hypertension knowledge, and hypertension self-care questionnaires. The intervention group received an educational program based on the HBM model. The questionnaire was completed by the elderly before and 2 months after the intervention. The data were analyzed by descriptive statistics, Mann-Whitney, Chi-square, Wilcoxon, Fisher, and ANCOVA tests usingSPSS ver.16.RESULTS: There was a significant difference in knowledge and all HBM constructs before and after the intervention in the intervention group (P < 0.001). In the control group, knowledge (P-value = 0.023), self-efficacy (P-value = 0.001), perceived severity (P-value = 0.005), and cues to action (P-value = 0.008) were significantly different (P < 0.05) after the intervention.CONCLUSION: The present study showed that HBM-based education can be considered an effective educational framework for improving self-care of the hypertensive older adults.
Prediction of self-care behaviors among diabetic patients based on social cognitive theory

Abstract:Background: Diabetes is a global health issue and the most prevalent chronic metabolic disease, which requires lifelong self-care behaviors. Thus, the present study was conducted with the aim of predicting the factors of self-care behaviors based on social cognitive theory in diabetic patients referred to comprehensive health service centers in Fasa. Methods: This cross-sectional research comprised 106 type 2 diabetes patients who were referred to Fasa service providers to determine the optimal solution. Outcomes were assessed using an existing demographic questionnaire, the diabetic self-care questionnaire, and a unique questionnaire to assess social cognitive theory, the validity and reliability of which were estimated. SPSS21 software with a Chi license was used for data analysis, and Spearman correlations, independent t-tests, and one-way ANOVA were performed. Results: The age range of patients was between 33 and 67 years. There was a significant relationship between gender, education, and type of treatment with self-care. According to the findings, the variables of self-efficacy ( r = 0.200), self-efficacy of overcoming obstacles ( r = 0.285), environmental factors ( r = 0.334), observational learning ability ( r = 0.148), situational perception ( r = 0.297), emotional adaptation ( r = 0.051), outcome assessment ( r = 0.114) and outcome expectation ( r = 0.082) had a positive and significant correlation with self-care behaviors. Also, the self-efficacy variable (Beta coefficient = 0.340) had the highest predictive value. Conclusion: In designing and implementing educational interventions for self-care of diabetic patients, the theory of social cognition can be used as a framework to promote and maintain public health in patients.
