Abstract:Due to the complexity of heart failure (HF) and its treatment process, a high level of patient and informal caregiver engagement is required for management results. We aimed to explore the views of HF patients, informal caregivers, and healthcare professionals about personal experiences, perceived needs, and barriers to optimal HF management. A qualitative study using semi-structured interviews with HF patients (n = 32), their informal caregivers (n = 21), and healthcare professionals (n = 5) was conducted in the outpatient HF clinic in Slovenia in 2018. A content analysis method was used to analyze the data. Negative emotional response to disease and its limitations (especially the inability to continue with work) and changes in family roles were the most prevalent topics regarding the impact of HF on livelihood. Among the most common barriers to HF self-care, were the difficulties in changing lifestyle, financial difficulties due to the disease, traditional cuisine/lack of knowledge regarding a healthy diet and lack of self-confidence regarding physical activity. Despite psychological and social difficulties due to HF being highlighted by patients and informal caregivers, only healthcare professionals emphasized the need to address psychosocial aspects of care in HF management. Established differences could inform the implementation of necessary support mechanisms in HF management.

A Mixed-Methods Study of Social Determinants and Self-care in Adults With Heart Failure

Abstract:Background: The burden of heart failure (HF) is unequally distributed among population groups. Few study authors have described social determinants of health (SDoH) enabling/impeding self-care. Aim: The aim of this study was to explore the relationship between SDoH and self-care in patients with HF. Methods: Using a convergent mixed-methods design, we assessed SDoH and self-care in 104 patients with HF using the Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) and the Self-Care of HF Index v7.2 with self-care maintenance, symptom perception, and self-care management scales. Multiple regression was used to assess the relationship between SDoH and self-care. One-on-one in-depth interviews were conducted in patients with poor (standardized score ≤ 60, n = 17) or excellent (standardized score ≥ 80, n = 20) self-care maintenance. Quantitative and qualitative results were integrated. Results: Participants were predominantly male (57.7%), with a mean age of 62.4 ± 11.6 years, with health insurance (91.4%) and some college education (62%). Half were White (50%), many were married (43%), and most reported adequate income (53%). The money and resources core domain of PRAPARE significantly predicted self-care maintenance ( P = .019), and symptom perception ( P = .049) trended significantly after adjusting for other PRAPARE core domains (personal characteristics, family and home, and social and emotional health) and comorbidity. Participants discussed social connectedness, health insurance coverage, individual upbringing, and personal experiences as facilitators of self-care behavior. Conclusion: Several SDoH influence HF self-care. Patient-specific interventions that address the broader effects of these factors may promote self-care in patients with HF.

Background Older adults affected by chronic conditions (i.e., heart failure or any other chronic condition) need to perform self-care. It includes behaviors to maintain a healthy lifestyle (self-care maintenance), monitor signs and symptoms of chronic diseases (self-care monitoring), and manage them (self-care management). Performing self-care, allows patients to improve their quality of life, reduce hospital admission rates, and mortality. In patients affected by Heart Failure (HF) and other chronic conditions, also specific self-care behaviors are needed. To date, no prior studies have investigated general and specific self-care behaviors in patients with HF and other chronic conditions. Purpose Investigate how patients affected by HF and other chronic conditions perform general and specific self-care. Methods Cross-sectional study. We collected data from April 2017 to October 2021 in Italy. Patients with HF and other chronic conditions with more than 65 y/o were enrolled in community and outpatient settings. The Self-Care of Chronic Illness Inventory (SC-CII) and The Self-Care of Heart Failure Index (SCHFI) were used to measure generic and specific self-care behaviors, respectively. Values ≥70 indicate adequate self-care behaviors. Paired t-test was performed to identify differences between SC-CII and SCHFI scores. Results A sample of 369 patients with HF was enrolled. 53.3% was female with a mean age of 78.91(±7.41) years, and a median of 3 chronic conditions. The two chronic conditions most frequently associated with HF were Hypertension (75%) and Diabetes Mellitus (70%). Results showed inadequate general self-care maintenance (66.11±14.7) and management (57.77±20.1). Instead, general self-care monitoring was adequate (76.26±20.4). Specific HF self-care maintenance, monitoring and management were inadequate and equal to 57.54 (±17.24), 53.41 (±25.32) and 54.48 (±20.02), respectively. Specific self-care in HF is significantly lower than general self-care behaviors (p<0.001). Conclusion Patients showed more difficulties performing general self-care maintenance and management respect self-care monitoring. Regarding specific HF self-care, patients perform inadequate self-care behaviors in all dimensions. Knowing the lower self-care behaviors is important for clinicians to develop tailored interventions. In particular, patients with HF and other chronic conditions need to be supported in specific self-care behaviors.

Predictors of Adherence to Self-Care in Rural Patients With Heart Failure

Abstract:Background: The common reality of not following a recommended course of treatment is a major cause of poor health outcomes in patients with heart failure (HF). The purpose of this study was to identify predictors of adherence to HF self-care recommendations in rural HF patients who received an intervention to promote symptom management and self-care. Methods: Data from 349 rural HF patients (42% female, 90% Caucasian) randomized to the intervention arms of the study were used. Adherence was measured using the European Heart Failure Self-Care Scale questionnaire, a brief measure that asks patients to report their adherence to a variety of recommended HF symptom management behaviors (ie, daily weight monitoring, when to call the physician, medications, diet, and exercise). The following predictors were tested: age, gender, marital status, education level, depression score (measured using PHQ-9), anxiety score (measured with the Brief Symptom Inventory), and level of perceived control (measured using Control Attitudes Scale-R). Multivariate linear regression was used to test the model. Results: The model to predict adherence was significant (P < .0001). Of the covariates tested in the regression model, being a male (P = .009), having less anxiety (P = .018), not being depressed (P = .017), and having higher perceived control (P = .003) were predictors of improved self-care score at 3 months. Conclusion: Adherence is a multifaceted and a challenging behavior based on the assumption that the patient agrees with self-care recommendations. These data suggest interventions designed to promote adherence behaviors should include an assessment of gender, anxiety, depression, and perceived control for optimal outcomes.

Abstract:Background: Self-care as a disease management strategy in heart failure (HF) is important for the prevention of rehospitalisation, but it is difficult to assess and does not always reflect actual self-care practices. This study aimed to compare self-reported self-care assessments and actual self-care practices in older patients with HF and examine factors influencing self-care performance. Methods: We included 64 patients with HF aged ≥65 years who had been hospitalised and prescribed rehabilitation with stable symptoms between April 2019 and September 2020. We assessed the B-type natriuretic peptide levels and New York Heart Association (NYHA) classification, Short Physical Performance Battery (SPPB), grip strength, and the Japanese version of the Montreal Cognitive Assessment (MoCA-J). We used the European Heart Failure Self-care Behaviour Scale (EHFScBS) to assess self-care behaviour in patients with HF. The patients filled in a self-assessment chart during their stay, which was used to work out the self-care implementation rate. Results: No significant correlation between the self-care implementation rate and the EHFScBS was identified (r= −0.160, p=0.207). Multiple regression analysis of factors associated with the rate of self-care and EHFScBS revealed that age, NYHA classification, SPPB and MoCA-J scores were significantly associated with the rate of self-care; however, no factors were associated with EHFScBS. The results were similar irrespective of the presence or absence of cognitive decline. Conclusion: In older patients with HF, the self-reported assessment of self-care did not reflect actual self-care performance. A reliable self-care behaviour scale needs to be developed.

Abstract:Background: Self-care plays a crucial role in the management of heart failure (HF) and is especially important for older patients who are frail. However, there is limited knowledge about how frail, older patients with HF perceive and experience self-care. Therefore, the aim of this study was to describe the experiences of self-care among frail, older patients with HF. Methods: A qualitative descriptive design with semi-structured interviews with frail, older patients diagnosed with HF (n = 19; median age 82 years). Thematic analysis, guided by Braun and Clarke, was used to analyse the data. Results: Two main themes emerged from the analysis: 1) "To maintain my health," encompassing various aspects such as hygiene practices, engaging in physical activity, medication adherence, following a healthy diet, and ensuring adequate rest; and 2) "To maintain my well-being and happiness," highlighting the importance of hobbies, maintaining independence, participating in social activities, and creating a supportive environment. Conclusion: This study provides valuable insights into the perspectives of frail, older patients with HF regarding self-care. It was observed that older patients often associate self-care with general well-being, hygiene, and happiness. Clear communication between healthcare providers and patients is essential to align different perspectives on self-care and ensure that self-care plans are tailored to individual needs. Moreover, addressing the emotional well-being and happiness of patients should be prioritized, as these factors play a significant role in promoting self-care adherence.

Abstract:BACKGROUND: Social support can influence treatment adherence of patients with chronic illnesses, which may explain the positive effects of social support on heart failure (HF) outcomes.PURPOSE: To investigate the effects of social support among patients with HF, we examined whether aspects of social support were associated with self-care, including medication adherence, dietary adherence, and HF symptom monitoring functions.METHODS: We recruited 74 patients with HF from cardiology clinics of a Veterans Affairs Medical Center and a university-affiliated hospital, and tested the relationships between social support and the patients' self-care.RESULTS: Consistent with previous research in older adults, family members, especially spouses, were often involved in the medical care of patients with chronic HF and provided a range of levels of support to patients. Self-care was generally poor, as measured across several self-care domains. Perceived social support was moderately associated with relatively better self-reported medication and dietary adherence, and other aspects of self-care such as daily weighing.CONCLUSIONS: These findings suggest that a relatively higher level of self-care is an important correlate of social support and may explain how social support influences HF outcomes. This study also suggests that family members should play a greater part in clinical care focused on improving self-care.

Abstract:A width-modulated air-slot photonic crystal (PhC) nanocavity is designed and analyzed. A tapered air-slot, instead of a regularly arranged rectangular slot, is used at the center of a W1 PhC waveguide. This is to reduce the vertical radiation loss which occurs at the abrupt termination of a rectangular slot, and also to increase the inline coupling from a waveguide to the cavity. A three dimensional finite difference time domain method is used to calculate the resonant wavelength, quality factor and modal volume of the cavity which are found as 1.559 μm, 3.4×106 and 0.154 μm3 respectively.

Abstract:Background Prior reports have documented extremely poor adherence to evidence-based medications among South Asian patients with established chronic cardiovascular diseases. Treatment adherence is now considered a part of the ‘self-care’ process, the determinants of which have not been adequately explored or explained among South Asian patients with chronic heart failure (CHF). Our objective was to qualitatively ascertain the determinants of the self-care process among Indian patients with a lived experience of heart failure. Methods We conducted in-depth interviews (audio-recorded) among 22 purposively sampled patients living with chronic heart failure, diagnosed at least 4 weeks prior to the interview and 17 caregivers (n = 39) in a tertiary care teaching hospital in Southern India. We employed an inductive analytical approach using Charmaz’s constructivist grounded theory. Initial line-by-line coding and categorization was followed by memo writing, reflexive analysis after interviewing and analyzing four, eight and twelve patients, and at each stage further theoretical sampling was carried out until we reached thematic saturation. We used NVivo ver. 12 to analyze and organize data. Results The mean age of our patients was 61 years and they represented 5 Indian states and spoke seven languages, distributed across socio-economic strata and literacy levels. We classified self-care determinants into 3 broad, simple categories and defined underlying themes namely, negative determinants (passivity, entrenched beliefs, negative affect, lack of knowledge, financial difficulties, and fatalism), intermediate factors (patient expectations, provider/hospital hopping) and facilitators or positive self-care determinants (intrinsic and extrinsic facilitators). Gender and the cultural background of patients’ upbringing appear to shape these determinants, thereby affecting self-care decision making in chronic heart failure. Conclusion We have empirically described a unique set of self-care determinants among Indian chronic heart failure patients, which in turn are shaped by economic and socio-cultural factors. Assessing for and addressing these determinants during clinical interactions through multi-factorial approaches may help improve self-care among Indian CHF patients, thus improving treatment adherence and clinical outcomes.

Abstract:Background. Symptoms of heart failure (HF) and coping resources, such as social support and social problem-solving, may influence self-care behaviors. Research regarding the influence of HF symptomatology characteristics and components of social support and social problem-solving on self-care is limited. Objective. To identify predictors of HF self-care behaviors using characteristics of HF symptomatology, components of social support and social problem-solving, and demographic and clinical factors. Methods. Using a cross-sectional, correlational predictive design, a convenience sample ( N = 201 ) of outpatients with HF answered self-report surveys. Multiple linear regression with stepwise variable selection was conducted. Results. Six predictors of HF self-care were identified: race, symptom frequency, symptom-related interference with enjoyment of life, New York Heart Association Class HF, rational problem-solving style, and social network ( β = 34.265 , R 2 = 0.19 , P = 0.001 ). Conclusions. Assessing the influence of race on self-care behaviors in middle to older age patients with HF is important. Clinical assessment that focuses on symptom frequency, symptom-related interference with enjoyment of life, and HF Class might also impact self-care behaviors in this population. Rational problem-solving skills used and evaluation of the size of and satisfaction with one’s social network may be appropriate when assessing self-care.
Prevalence and predictors of poor self-care behaviors in patients with chronic heart failure

Abstract:Despite the indispensable role of self-care behavior in managing heart failure, the practice of self-care behavior remains poor, especially in developing countries. There is a scarcity of research focusing on poor self-care behavior and its determinants within our specific context. Therefore, the objective of this study was to investigate the prevalence and predictors of poor self-care behavior among ambulatory heart failure patients. A facility-based cross-sectional study was conducted at a tertiary care hospital in Ethiopia, involving patients with heart failure. We utilized the European Heart Failure Self-Care Behavior Scale (EHFScBS-9) to evaluate adherence to self-care behaviors. Data were gathered through patient interviews and a review of medical records. A binary logistic regression analysis was performed to identify predictors of poor self-care behavior in heart failure patients. We included a total of 343 participants in the final analysis of this study. The findings revealed that a majority of the patients (73.8%) demonstrated poor overall self-care behavior. Specifically, the majority of patients did not engage in regular exercise (76.1%), failed to consult doctors in case of rapid weight gain (75.6%), did not monitor weight daily (71.5%), did not restrict fluid intake (69.9%), and did not contact doctors in case of experiencing fatigue (68.6%). Additionally, 32.4% of patients did not reach out to doctors when experiencing shortness of breath, 30% did not restrict salt intake, 29% did not adhere to prescribed medication, and only 7% did not consult doctors if edema occurred. Our findings indicated that rural residence (AOR: 5.76, 95% CI: 2.47–13.43), illiteracy (AOR: 2.64, 95% CI: 1.52–6.31), prior hospitalization (AOR: 2.09, 95% CI: 1.21–3.61), and taking five or more medications (AOR: 1.83, 1.01–3.33) were significant predictors of poor self-care behavior. In conclusion, a majority of the participants in our study demonstrated poor self-care behavior. Risk factors for this behavior included rural residence, illiteracy, prior hospitalization, and taking five or more medications. Therefore, it is crucial to prioritize these high-risk patients and implement interventional programs aimed at improving self-care behaviors and overall treatment outcomes in heart failure patients.

Abstract:Background: Self-care behavior is crucial for patients with heart failure to have positive outcomes, and it also serves as a predictor of wellbeing. The self-care tasks that these patients are required to carry out include managing their symptoms, sticking to numerous pharmaceutical regimens, following dietary salt restrictions, and self-monitoring weight. In many cases, a crucial aspect of heart failure treatment is still challenging. Objective: To determine the self-care practices of patients with heart failure at Hasan Sadikin General Hospital in Bandung, Indonesia. Methods: The study used a descriptive study with a consecutive sampling of 196 patients with heart failure in the clinic. Results: the demographic characteristics were mean of age (in year) 56.26 ± 12.49, majority were male 144 respondents (73.57%), above the senior high school 128 respondents (65.3%), New York Heart Association NYHA II category 76 respondents (38.8%), NYHA III category at 92(46.9%), NYHA IV category at 28(14.3%), EF 33.65±9.311, length of HF 2.02± 1.08, BMI 25.48±1.98, total score of self-care behaviour 88.35±7.736. The score of the subvariable of self-care behavior is as follows: consulting behaviors 19.68±2.405, dietary behaviors 8.16 ± 1.207, symptom perception 21.93 ±2.188, symptom recognition 9.65 ±1.393, self-care management 8.64±0.853, problem-solving behaviors 4.64±1.278, self-efficacy 20.20 ±1.979. Conclusion: Patients with heart failure exhibit poor self-care behavior; hence, nurse practitioners' involvement is required to promote self-care behavior among patients with heart failure.

Abstract:<p>Self‐care is essential in the long‐term management of chronic heart failure. Heart failure guidelines stress the importance of patient education on treatment adherence, lifestyle changes, symptom monitoring and adequate response to possible deterioration. Self‐care is related to medical and person‐centered outcomes in patients with heart failure such as better quality of life as well as lower mortality and readmission rates. Although guidelines give general direction for self‐care advice, health care professionals working with patients with heart failure need more specific recommendations. The aim of the management recommendations in this paper is to provide practical advice for health professionals delivering care to patients with heart failure. Recommendations for nutrition, physical activity, medication adherence, psychological status, sleep, leisure and travel, smoking, immunization and preventing infections, symptom monitoring, and symptom management are consistent with information from guidelines, expert consensus documents, recent evidence and expert opinion.</p>

Abstract:Perceived social support is known to affect self-care. No previous studies have been conducted to determine the association between perceived social support and self-care among patients with heart failure in Jordan. This study aimed to identify the predictors of self-care behaviors among patients with heart in Jordan. A cross-sectional correlational design was used. A convenience sample of 254 patients with heart failure was invited to participate. The Personal Resource Questionnaire (PRQ)-2000 and the Self-Care Heart Failure Index (SCHFI) were used to measure perceived social support and self-care among patients with heart failure in Jordan. The mean perceived social support score was low at 59 ( SD = 15). The mean social support score was also low at 59 ( SD = 25). The maintenance subscale score ranged from 10–86.66 and achieved the highest mean (57.77), followed by the management subscale (mean = 56.29, range10-90), while the confidence subscale mean was the lowest (mean = 50.80, range 5.56–88.96). All self-care subscales mean scores were below the target level (≥ 70) for self-care. Lower perceived social support, lower income, older age, smaller duration of disease, less persons living at home, worse cardiac functional status, and the presence of hypertension or diabetes were the significant predictors of low self-care. The culture and the health care system may clarify some results. Providing patients with sufficient social support enhances self-care. More consideration should be provided to the elderly and patients with low income, worse functional status, short duration of disease, hypertension, and diabetes to enhance self-care.

Abstract:Background: Self-care is vital for successful heart failure (HF) management. Mastering self-care is challenging; few patients develop sufficient expertise to avoid repeated hospitalization. Objective: To describe and understand how expertise in HF self-care develops. Methods: Extreme case sampling was used to identify 29 chronic HF patients predominately poor or particularly good in self-care. Using a mixed-methods (qualitative and quantitative) design, participants were interviewed about HF self-care, surveyed to measure factors anticipated to influence self-care, and tested for cognitive functioning. Audiotaped interviews were analyzed using content analysis. Qualitative and quantitative data were combined to produce a multidimensional typology of patients poor, good, or expert in HF self-care. Results: Only 10.3% of the sample was expert in HF self-care. Patients poor in HF self-care had worse cognition, more sleepiness, higher depression, and poorer family functioning. The primary factors distinguishing those good versus expert in self-care were sleepiness and family engagement. Experts had less daytime sleepiness and more support from engaged loved ones who fostered self-care skill development. Conclusion: Engaged supporters can help persons with chronic HF to overcome seemingly insurmountable barriers to self-care. Research is needed to understand the effects of excessive daytime sleepiness on HF self-care.
Social support and adherence to self-care behavior among patients with coronary heart disease and heart failure: A systematic review

Abstract:Cardiovascular diseases stand out as the foremost cause of mortality on a global scale and encompass conditions that require long term self-care. Coronary heart disease and heart failure are two cardiovascular conditions that require significant lifestyle modifications. Adherence to self-care is a multifaceted phenomenon, and is influenced by various factors that include social, economic, disease-related and healthcare system-related factors. A key factor in adherence to self-care in chronic illnesses is social support. To explore this relationship between social support and adherence to self-care, a systematic review was carried out across Scopus, EBSCO host and ProQuest from October 2022 to February 2023 using predefined search criteria. Studies from inception to February 2023 were considered for the review, ultimately incorporating a total of 11 studies. Six studies had an adult population with coronary heart disease while the remaining five had adults with heart failure. All the studies reported a significant positive correlation between social support and adherence to self-care. Our findings revealed that social support plays a significant role in promoting self-care, emphasizing the need for a holistic understanding of self-care to develop effective interventions. Along with self-report measures, objective measures should be used to assess adherence accurately. There is a need for scales that assess all aspects of self-care, as well as the development of new interventions and teaching strategies to facilitate the individual’s self-care journey. In addition, family members and trusted resources should be involved in encouraging self-care, and interventions should target both patients and their family members.
Testing a model of patient characteristics, psychologic status, and cognitive function as predictors of self-care in persons with chronic heart failure

Abstract:Objective: Self-care is a key component in the management of chronic heart failure (CHF). Yet there are many barriers that interfere with a patient's ability to undertake self-care. The primary aim of the study was to test a conceptual model of determinants of CHF self-care. Specifically, we hypothesized that cognitive function and depressive symptoms would predict CHF self-care. Methods: Fifty consecutive patients hospitalized with CHF were assessed for self-care (Self-Care of Heart Failure Index), cognitive function (Mini Mental State Exam), and depressive symptoms (Cardiac Depression Scale) during their index hospital admission. Other factors thought to influence self-care were tested in the model: age, gender, social isolation, self-care confidence, and comorbid illnesses. Multiple regression was used to test the model and to identify significant individual determinants of self-care maintenance and management. Results: The model of 7 variables explained 39% (F [7, 42] 3.80; P = .003) of the variance in self-care maintenance and 38% (F [7, 42] 3.73; P = .003) of the variance in self-care management. Only 2 variables contributed significantly to the variance in self-care maintenance: age (P < .01) and moderate-to-severe comorbidity (P < .05). Four variables contributed significantly to the variance in self-care management: gender (P < .05), moderate-to-severe comorbidity (P < .05), depression (P < .05), and self-care confidence (P < .01). When cognitive function was removed from the models, the model explained less of the variance in self-care maintenance (35%) (F [6, 43] 3.91; P = .003) and management (34%) (F [6, 43] 3.71; P = .005). Conclusion: Although cognitive function added to the model in predicting both self-care maintenance and management, it was not a significant predictor of CHF self-care compared with other modifiable and nonmodifiable factors. Depression explained only self-care management.
