A morphological study of lumbar vertebral endplates: radiographic, visual and digital measurements

Abstract:Purpose Clinical observations suggest that endplate shape and size are related to complications of disc arthroplasty surgery. Yet, the morphology of the vertebral endplate has not been well defined. This study was conducted to characterize the morphology of lumbar vertebral endplates and to quantify their morphometrics using radiographic, visual and digital measures. Methods A total of 591 vertebral endplates from 76 lumbosacral spines of men were studied (mean age 51.3 years). The shape of the vertebral endplates was classified as concave, flat and irregular, and was evaluated from both radiographs and cadaveric samples. Each endplate was further digitized using a laser scanner to quantify diameters, surface area and concavity for the whole endplate and its components (central endplate and epiphyseal rim). The morphological characteristics and morphometrics of the vertebral endplates were depicted. Results In both radiographic and visual assessments, more cranial endplates (relative to the disc) were concave and more caudal endplates were flat at all disc levels ( p < 0.001). On average, the mean concavity depth was 1.5 mm for the cranial endplate and 0.7 mm for the caudal endplate. From L1/2 down to L5/S1 discs, the vertebral endplate gradually changed into a more oval shape. The central endplate was approximately 70 % of the diameter of the whole endplate and the width of the epiphyseal rim varied from 3 to 7 mm. Conclusions There is marked morphological asymmetry between the two adjacent endplates of a lumbar intervertebral disc: the cranial endplate is more concave than the caudal endplate. The size and shape of the vertebral endplate also vary considerably between the upper and lower lumbar regions.
Double foramen transversarium in dried cervical vertebra: An osteological study with its clinical implications

Abstract:Objectives: The aim of the present study was to know the incidence of double foramen transversarium (FT) in the dried cervical vertebrae and its clinical importance. Materials and Methods: A total number of 150 dried cervical vertebrae were collected from the department of anatomy to investigate the presence of double FT. The occurrence of double foramina was observed on both sides of the same vertebrae. Results: In the present study out of 150 cervical vertebrae, double FT was observed in 33 (22.00%) vertebrae among them double foramen seen unilaterally in 16 (10.67%) vertebrae and bilaterally in 17 (11.33%) vertebra.Conclusion: In the present study, bilateral double FT was found to be more common than unilateral, contrary to the findings as mentioned in most of the previous similar studies. This anatomical knowledge is clinically important since the course of the vertebral artery may be distorted in such situations and may result in certain clinical conditions like posterior headache, migraine, fainting attack, etc. This study may be useful to neurosurgeons and radiologists.
Morphometric Variations of the Foramen Lacerum in Dry Skulls and Its Clinical Significance: A Cross-sectional Study

Abstract:Introduction: The foramen lacerum is located in the middle cranial fossa and transmits a number of nerves and blood vessels. Occlusion can result in the compression of the neurovascular bundle, and the foramen has been reported to be involved in the spread of tumours into the cranium. Aim: The aims of the present study was to describe the morphology of the foramen lacerum and to compare the anatomical parameters on the left and right sides of the skull. Also, the significance of the anatomical parameters in male versus female skulls and right side versus left side of the skull where studied. Materials and Methods: This cross-sectional study was conducted in May and June 2023 using 62 dry skulls (124 foramen lacerum) available in the Department of Anatomy at Gautam Buddha Chikitsa Mahavidyalay, a tertiary care teaching institute in Dehradun, Uttarakhand, India. The anatomical parameters, including the Anteroposterior (AP) and transverse diameters, were recorded using a calibrated digital Vernier caliper. The shape of the foramen was also observed. The mean and standard deviation of the recorded parameters were obtained. Results: Out of the 62 skulls, 41 were males and 21 were females, resulting in a male-to-female ratio of 1.8:1. The mean AP diameter on the Right Side (RT) and Left Side (LT) in males was 6.26 mm and 6.24 mm, respectively, while in females, the mean diameter was 4.97 mm (RT) and 5.4 mm (LT). The mean transverse diameter recorded in the present study was 6.14 mm (RT) and 6.2 mm (LT) in males, while in females, this measurement was 5.88 mm on both sides. The most common shape of the foramen lacerum observed in the study was bilaterally round, in both males and females. Conclusion: There are morphological differences in the AP diameter between male and female skulls, with males having a larger AP diameter on their right side and females having larger AP diameter on their right side. The overall size of the foramen lacerum was smaller in the present study, indicating a higher likelihood of neurovascular bundle compression with age. The recent advances in skull base surgery warrant further and larger studies on this neglected yet important foramen to gather more data on its morphometric variations in different geographical regions.
Lumbar Pedicle Morphometry of Dry Vertebral Columns in Relation to Transpedicular Fixation: A Cross-Sectional Study From Central India

Abstract:Introduction: The lumbar vertebrae are the largest vertebrae of the vertebral column, which support the maximum body weight. There has been an increased focus on transpedicular spinal fixation for addressing various lumbar spine pathology. However, its safety and efficacy require precise knowledge of the lumbar pedicle anatomy. Mismatched size of screw and pedicle may lead to failure of instrumentation. It may result in cortex perforation or pedicle fracture and loosening of the pedicle screw. The oversizing of the pedicle screw can result in dural tears, leakage of the cerebrospinal fluid, and injuries to the nerve root. As the racial variations in the anatomy of a pedicle are well known, this study was performed to assess the morphological parameters of the lumbar vertebrae pedicles in the Central Indian population so that the appropriate sizes of pedicular implants can be selected. Material and methods: The present study was conducted at a tertiary-level hospital and medical college on dry lumbar vertebrae specimens available in the department of anatomy. The measurement of morphometric parameters of the lumbar vertebrae pedicles was performed in 20 dry lumbar specimens using vernier calipers and a standard goniometer. The morphometric parameters included in the study are pedicle transverse external diameter (pedicle width), pedicle sagittal external diameter (pedicle height), transverse angle of the pedicle, and sagittal angle of the pedicle. Statistical analysis was performed using Statistical Package for the Social Sciences (SPSS) system version 25 (Chicago, IL: SPSS Inc.). Results: The broadest external transverse diameter was at the L5 level, with a mean of (17.54±1.6 mm) in the lumbar vertebrae. The broadest external sagittal pedicle diameter was at the L1 level (13.7±0.88 mm). The maximum transverse angle of the pedicle was at L5 with a mean of 25.39±3.10°. The maximum sagittal angle was at L1 with a mean of 5.44±0.71°. Conclusion: The increased concern regarding the internal fixation of the spine with pedicle screw systems created the need to have almost accurate anatomical knowledge of lumbar pedicles. Due to the dynamic nature of the lumbar spine and the body's load, maximum degeneration occurs at this spine segment, making it the most commonly operated region of the vertebral column. In our study, pedicle dimensions are comparable to populations of other Asian countries. However, the pedicle dimension of our population is lower than the White American population. This morphological variation of pedicle anatomy will help surgeons choose appropriate size screws and optimum angulations to insert the implant, decreasing complications.
Morphological Features of the Basivertebral Foramen of T(12)-L3 in CT and Its Clinical Significance

Abstract:Objective To observe the morphological features of the basivertebral foramen (BF) in thoracolumbar (T12,L1,L2 and L3).Methods A total of 36 health adults were underwent multi-slice CT thin slice scans and three-dimensional reconstruction.In the horizontal and sagittal CT reconstruction images,BF weight (BFW),BF depth (BFD),BF high (BFH),BF relative to the body weight (BFWr),BF relative to the body depth (BFDr),and BF relative to the body high (BFHr),were measured.The distance between the BF and each side in the horizontal and sagittal CT images were also measured.The correlation between the measured parameters of BF and the gender,age,body mass index (BMI) were analyzed statistically.Results 81％ BF was triangular or trapezoid.There was a bone interval within 6％ BF.In the same level in 36 cases,the maximum of BFW,BFD and BFH were approximately 1/3 vertebral body in the same plane.The average BFW,BFD and BFH were approximately 25％ vertebral body in the same plane.In all four levels,the distance from the BF to the cranial endplate was shorter than to the caudal endplate (P＜O.05).The mean BFW,BFH,BFWr,BFHr in different levels were no significant difference(P＞0.05).The mean BFD and BFDr in L3,however,were greater than other levels.There were no significant correlations among gender and BFW,BFWr,BFD,BFDr,BFH,BFHr.There was a correlation between the age and L1BFHr and L2BFHr,and a correlation between BMI and L1BFH,L2BFW,L3BFH and L3BFHr.Conclusion There was a BF in all specimens.The most common appearance of BF was triangular or trapezoid.BF was closer to the cranial endplate in all four levels in the sagittal CT images.Variations of BF were significantly different in individuals.
Experimental Study on Morphological Characteristics of Sacrum

Abstract:Objective To study the morphological characteristics of foramina of sacrum in mongolias and the anatomic relationship between foramina and nerve roots of sacrum, and then to investigate the surgical indication of sacral fracture with nerve injury. Methods Fifteen fresh cadaver sacrum were dissected from the healthy adults, including 11 males and 4 females, ranged from 27 to 48 years old with an average of 32. 6. The dimensions of foramina and nerve roots were measured with a caliper. The foramina of sacrum were filled up with Heraplast BC after dissecting in order to obtain the cross section area(CSA) of entrance and exit of sacral foramina. Then, the CSA of entrance and exit of sacral foramina and nerve roots were measured with applied software. Statistics analysis was made with SPSS10.0 FOR WINDOWS software. Results From S1 to S4, the rate of the sacral nerve roots’ aera share the foramina in entrance were about 40%, 30%, 20%, 15% respectively. Likewise, the rate of the sacral nerve roots’ aera share the foramina in exit were 30%, 20%, 15%, 15% respectively. The exports of the anterior sacral foramina were round shape and entrances of the sacral foramina were oval-shape. The CSA of S2,S3,S4 nerve roots in entrance were about 70%, 25%, 10% of the S1 cross section respectively, meanwhile about 75%, 30 %, 15% of the S1 cross section in exit. The dural sac ended at inferior of S1 vertebra in two cases(13.3%),at S2 vertebra in thirteen cases(86.7%). Conclusion The sacral roots would be injured when the sacral foramina are deformed. That foraminal entrapment of S1 and S2 was more than that of S3 and S4. The rate of the nerve roots'transverse diameters shared the foramina's in the entrance was high. This reveal that foraminal entrapment of entrance was more likely to happen than that of exit. The dural sac most ends at S2 level.

Abstract:A BSTRACT Background: Surgical procedures in the anterior mandibular region require a profound understanding of the complex anatomical structures to ensure patient safety. The lingual foramen (LF) and its intra-osseous canal are pivotal structures, supplying vascular and neural support to the mental region. The prevalence of LF is widely recognized, with variations among populations. Methods: The study involved 650 adult participants. Data were collected using high-resolution cone-beam computed tomography (CBCT), and statistical analysis was conducted, establishing inter-rater agreement. Results: The population had a varied age distribution, with males comprising 53.23% and females comprising 46.77%. The median lingual canal was found in the Supra and Infra spinosum (65.38%). Most participants had lateral lingual canals (60.92%). The most prevalent lingual foramina was 4 (30.62%). The study found no significant associations between gender and LF diameter, distribution in different travel directions, or LF locations. However, males had a slightly larger diameter. Conclusion: The findings provide valuable insights into LF variations based on anatomical locations, and there were no statistically significant differences in measurements across different age groups. This knowledge contributes to the evolving field of oral and maxillofacial surgery, ensuring better patient outcomes.
Nutritive quality and palatability of switchgrass hays for sheep: effects of cultivar, nitrogen fertilization, and time of adaptation.

Abstract:Feeding and palatability trials were conducted with four cultivars (cv) of switchgrass (Panicum virgatum L.), fertilized at three levels of N (0, 75, or 150 kg of N/ha) in 2 yr. Wether sheep had ad libitum access to chopped hays, and intake, apparent digestibility, particle passage rates, and concentrations of blood metabolites were determined. Palatability was measured with mature sheep in cafeteria trials. Nitrogen fertilization did not affect (P > .05) DM digestibility (DMD) or DMI, but there was a year x N interaction (P < .01) for NDF digestibility. Dry matter digestibility values for combined years and N levels were 56.9, 54.4, 56.6, and 57.9% (P < .01) for Pathfinder, New Jersey 50, Kentucky 1625, and Trailblazer cv, respectively; mean DMI values were 60.4, 60.8, 57.7, and 64.0 g/kg BW.75 (year x cv, P < .01). An apparently greater quality of Trailblazer was masked by weed invasion of N-fertilized stands of this cv in yr 2, with changes in hay composition. Lambs adapted to hay diets with time; mean DMI for cv and N levels combined increased (P < .001) from 50.5 to 71.4 g/kg BW.75 between wk 2 and 3 and wk 10 and 11, with no change in DMD. Intakes of NDF increased from 37.6 to 55.6 g/kg BW.75, an increase of 48%. Marker (Yb) measurements indicated little change in particle passage rates with treatment. There were no major differences in blood composition, apart from increases in blood urea N, as a result of N fertilization. Cafeteria trials showed preference by sheep for Trailblazer and KY 1625 compared with NJ 50 and Pathfinder, with a N x cv interaction (P < .01). Trailblazer was preferred to KY 1625 in a two-choice situation (P < .01). Results show relatively small effects of cv and N fertilization on quality of switchgrass and indicate the need for a lengthy period of adaptation by sheep to the feeding of a warm-season grass.

Abstract:BACKGROUND A novel inhalation-driven multidose dry powder inhaler (MDPI) that eliminates the need for the patient to coordinate device actuation with inhalation has been developed for delivery of inhaled asthma medications. OBJECTIVE To characterize the pharmacokinetics of single-dose fluticasone propionate (Fp) MDPI compared with single doses of Fp dry powder inhaler (DPI) and a metered-dose inhaler (MDI) in healthy subjects. METHODS This was a single-center, open-label, randomized, three-period crossover, single-dose pilot study in healthy adults ages 18 to 45 years. Eligible subjects (N = 18) were randomized to one of six treatment sequences that contained three treatment arms: Fp MDPI 400 μg/inhalation × two inhalations (800 μg total dose); Fp DPI 250 μg/inhalation × four (1000 μg total dose); and Fp MDI 220 μg/inhalation × four (880 μg total dose). Pharmacokinetics (area under concentration-versus-time curve [AUC], maximum plasma concentration [Cmax], time to Cmax [tmax], and elimination half-life [t½]), safety, and tolerability were assessed for each treatment. RESULTS Plasma Fp concentration-versus-time curves were comparable across treatments. Geometric mean AUC0-t and Cmax for Fp MDPI 800 μg were 19% and 18% higher, respectively, compared with Fp DPI 1000 μg, and 47% and 82% higher, respectively, compared with Fp MDI 880 μg. Median tmax (60.0-60.6 minutes) and median t1/2 (9.1-9.8 hours) were comparable across the three treatments. Single-dose Fp was well tolerated, with no new safety issues noted. CONCLUSION Single-dose administration of Fp MDPI 800 μg produced systemic exposure comparable with those for Fp DPI 1000 μg and Fp MDI 880 μg.
Morphometric Analysis of Cervical Neuroforaminal Dimensions from C2-T1 Using Computed Tomography of 1,000 Patients

Abstract:Background Race and sex differences are not consistently reported in the literature. Fundamentally, anatomical differences of cervical neuroforaminal dimensions (CNFD) amongst these groups would be important to know. Purpose To establish normative radiographic morphometric measurements of CNFD and uncover the influence of patient sex, race, and ethnicity while also considering anthropometric characteristics. Study Design Retrospective radiographic morphometric study. Patient Sample 1,000 patients between 18 and 35 years of age who were free of spinal pathology. Outcome Measures Foraminal height, axial width, and area of cervical neural foramen. Methods Cervical CTs were reviewed to measure CNFD, defined as follows: foraminal height, axial width, and area. Statistical analyses were performed to assess associations between CNFD, and patient height, weight, sex, race, and ethnicity. Results CNFD measurements followed a bimodal distribution pattern moving caudally from C2-T1. Irrespective of disc level, cervical CNFD were as follows: left and right widths of 6.6 ± 1.5 and 6.6 ± 1.5 mm, heights of 9.4 ± 2.4 and 9.4 ± 3.2 mm, and areas of 60.0 ± 19.5 and 60.6 ± 20.7 mm 2 . Left and right foraminal width were highest at C2-C3 and lowest at C3-C4. Left and right foraminal height were highest at C7-T1 and C6-C7 respectively and lowest at C3-C4. Left and right foraminal areas were highest at C2-C3 and lowest at C3-C4. Significant differences were observed for all CNFD measurements across disc levels. CNFD did not vary based on laterality. Significant CNFD differences were observed with respect to patient sex, race, and ethnicity. Male height and area were larger compared to females. In contrast, female foraminal width was larger compared to males. The Asian cohort demonstrated the largest foraminal widths. White and Hispanic patients demonstrated the largest foraminal heights and areas. Black patients demonstrated the smallest foraminal widths, heights, and areas. Patient height and weight were only weakly correlated with CNFD measurements across all levels from C2-T1. Conclusions This study describes 36,000 normative measurements of 12,000 foramina from C2-T1. CNFD measurements vary based on disc level, but not laterality. Contrasting left- versus right-sided neuroforamina of the same level may aid in determining the presence of unilateral stenosis. Patient sex, race, and ethnicity are associated with CNFD, while patient anthropometric factors are weakly correlated with CNFD.

Exploring the atlantic part of the vertebral artery in the South Indian population and its implications in spine surgery

Abstract:Study design: A descriptive, quantitative cross-sectional study of the atlantic part (V3) of the vertebral artery (VA). Purpose: This study aimed to bridge the research gap in the morphometry of the V3 segment of the VA in the South Indian population. Overview of literature: The microsurgical anatomy of this segment of the VA has been explored in various populations, and a thorough understanding of the anatomy and course of the VA, particularly the V3 segment, is essential to prevent iatrogenic complications. Several computed tomography studies but a few cadaveric studies have explored the V3 segment of the VA in the South Indian population. Methods: This study examined 40 VAs from 20 embalmed cadavers that were obtained from the voluntary donation program, and Institutional Ethical Clearance was obtained before the study. The length, diameter, and angle of the vertical, horizontal, and exit parts of the V3 segment of the VA were documented after its exposure. Results: The mean lengths of the right and left VAs on each part were nearly similar, except for the mean length of the horizontal part (right: 38.937 mm, left: 40.237 mm) and total length of the V3 segment (right: 66.870 mm, left: 70.350 mm). Conclusions: These morphometric parameters are essential to spine surgeons who intend to operate on a small part (vertical, horizontal, or exit part) of the V3 segment of the VA. The mean values of the parameters obtained in this study give average measurements or safe limits to surgeons for safe surgical procedures such as the occipital condyle screw technique and C1 laminectomy.
Experimental study of correlation between morphological characteristics of the intervertebral foramina and disc heights in low lumbar spine

Abstract:Objective To investigate the morphological characteristics of the intervertebral foramina and their alterations in the process of disc height loss in low lumbar spine in order to provide basic data for the diagnosis and treatment of foraminal stenosis. Methods Eight fresh cadaveric lumbar spines of L3 to S1 were obtained for study, which were cleaned off their surrounding muscles and kept the ligaments, annulus fibrous and nerve roots attached. The morphological characteristics of foramina of L4,5 and L5S1 and their anatomic relationships with the nerve roots were observed with and without nucleus pulposus in situ under different loading conditions (0, 300, 500 N). The foraminal dimensions including heights, maximal widths and minimal widths were measured, and the disc heights were measured on lateral radiograph under above loading conditions as well. Results Without any loading, the foramina were inverted teardrop shaped, the foramina of L4,5 were similar to that of L5S1. The nerve roots were located in the upper part of the foramina, and passed across the foramina obliquely. With 500 N loading, the shapes of the foramina and the relationships with the nerve roots were found to be changed little. After removal of nucleus pulposus and more loading was applied, the foramina became stenotic gradually and the nerve roots were pushed against the superior pedicle. The foraminal heights, maximal widths and the anterior, posterior disc heights became narrower, and were significantly different from those under no loading condition (P 0.05). The foraminal heights had multiple linear correlations with the posterior and the anterior disc heights. Conclusion The shape and dimension of foramina changes as the disc height alters. The foraminal height is significantly correlated with the anterior and the posterior disc height. The alteration of the foraminal morphological characteristics may jeopardize the nerve roots of excessive compression.

Abstract:The greatest challenge to human health of recent times is the pandemic caused by the retroviruses known as HIV1 and HIV2. These have unleashed a plethora of opportunistic infections with organisms ranging from other viruses to helminths, some of which are newly recognised taxa or organisms never before identified as serious pathogens of man. Animal and bird migration as well as human travel and international trade have extended, to an alarming degree, the dissemination of other viral infections. Malaria is an increasing global challenge to human health, a reversal of the downward trend that was achieved in the latter half of the 20th century. Moreover, evidence is currently accumulating for the existence of a fifth species of Plasmodium in man. In the bacterial and protozoal fields, other novel organisms are being described. Powerful new tools of molecular biology are being deployed to probe parasite genetics, for example to reveal the genome of Plasmodium falciparum. These and molecular immunological techniques are also helping to resolve puzzling taxonomic questions, as well as aiding epidemiological studies that are revealing the unforeseen scale of human infection with a number of parasitic helminths in both tropic and temperate climatic areas. This paper presents a brief review of the major, as well as some of the relatively minor changes and advances of recent decades.
Mastoid Emissary Foramina in Dry Skulls: Its Morphometry, Topography, and Associated Clinical Implications

Abstract:Mastoid emissary foramen transmitting mastoid emissary vein connects the posterior auricular vein with the sigmoid sinus. This foramen and so the mastoid emissary vein varies in prevalence, number, size and location, knowledge of which is essential for carrying out uneventful surgeries, especially retrosigmoid, mastoidectomy, and skull base surgeries. There is a paucity of literature on this foramen in the Indian context, so the study was done. The purpose of the study is to elaborate on the prevalence, number, size, and location of mastoid foramen in dry adult skulls. The study was conducted in the Department of Anatomy using 90 dry skulls of unknown age and sex, and prevalence, number, size, and location in these skulls were noted. The mastoid foramen was detected in 27.8% of skulls, with an incidence of 31.1% and 12.2% on right and left sides of skulls, respectively. The number of foramina ranged between 1 and 4. The mean diameter of this foramen was 0.9 mm, and the most frequent location was mastoid process. The detailed morphology and morphometry of mastoid foramen are of utmost use to neurosurgeons, ENT surgeons, radiologists, and vascular surgeons as it transmits mastoid emissary vein and meningeal branch of the occipital artery, which may be injured during various surgical procedures involving mastoid region and skull base causing catastrophic hemorrhage. In addition to this, mastoid emissary vein may be the source of thrombus, causing thrombus of sigmoid sinus creating helm of neurological complications.

Presence of antilingula and its relationship to mandibular foramen-An anatomical study

Abstract:Aim: Aim of this study was to evaluate the presence of antilingula and its relationship with mandibular foramen. Materials and methods: In this study, a total of 50 specimens of dry human hemi-mandibles were studied to analyze the presence and relationship of antilingula to mandibular foramen. A 1-mm fissure bur was used to drill a hole perpendicular to bone from the deepest aspect of the concavity at the center of the mandibular foramen from the medial to lateral side, the drill perforated both the medial and lateral cortex of the mandible. The distance from the antilingula to hole was measured and recorded in both antero-posterior and supero-inferior planes. The data were collected and put to statistical analysis. Results: From this study, it was concluded that the antilingula was present in 90% (n = 45) of mandibles and was absent in 10% (n = 5) of mandibles. Antero-posteriorly, the antilingula was present anterior to mandibular foramen in 22.2% (n = 10) of mandibles, posteriorly in 57.7% (n = 26) of mandibles, and there was complete concordance in 20% (n = 9) of mandibles. The supero-inferior relation shows that antilingula was present superior to mandibular foramen in 97.8% (n = 44) of mandibles, and it was present inferiorly in 2.20 (n = 1) of mandibles. Conclusion: Although vertical ramus osteotomy is not a commonly used procedure in these days. From our study, it was concluded that antilingula was present as an elevation in 90% of cases on the lateral ramus border and the mandibular foramen is present inferior and anterior to the antilingula.
Exploring the micromorphological characteristics of adult lower cervical vertebrae based on micro-computed tomography

Abstract:We will use micro-computed tomography to scan 31 sets of the adult lower cervical vertebrae (155 vertebrae) to observe the morphological characteristics and direction of trabeculae in the lower cervical vertebrae by outlining and reconstructing the regions of interest and to calculate the variation laws of the microstructure in the regions of interest to reveal their structural characteristics and weak areas. As a result, the images showed that the trabeculae in the lower cervical pedicle near the medial and lateral cortices were relatively dense, and their bone plates were lamellar. There were cavities between the superior and inferior articular processes where the ossification centers had not been absorbed after ossified. The lamellar trabeculae in the vertebral plates near the cortical bones were only 1–2 layers, extended and transformed into rod-shaped trabeculae in a radial shape toward the medullary space. The lamellar trabeculae of the vertebral plate extend over the spinous process near the cortical bone. The statistical results of the trabeculae's morphological parameters showed significant differences in bone volume fraction values among the four parts (P < 0.05). There were substantial differences in BS/BV, except for no differences between the pedicle and the vertebral plate (P < 0.05). There was a significant difference in trabecular pattern factor values between the articular process, the spinous process, and the vertebral plate (P < 0.05) and a significant difference between the pedicle, the spinous process, and the vertebral plate (P < 0.05). There were no significant differences in trabecular bone thickness and trabecular space values among the four parts (P < 0.05). The anatomical microstructural perspective confirms that the optimal choice is internal fixation via the pedicle. If using pedicle screws, the nail tract needs to be placed into the spinous process to increase its holding power and resistance to extraction.

Abstract:BACKGROUND:Thoracic intervertebral foramen puncture is the key step for interventional therapy on the thoracic nerve roots or dorsal root ganglia. The anatomical features of the thoracic spine are complex, and puncture injury to the pleura, blood vessels, spinal cord, and other tissues may cause serious complications. The spatial anatomical characteristics and related parameters for thoracic intervertebral foramen puncture remain poorly understood. AIM:To observe and summarize the spatially applied anatomical characteristics for intervertebral foramen puncture on different vertebral segments. METHODS:A total of 88 patients (41 males and 47 females) who underwent thoracic minimally invasive interventional treatment at Nanjing Drum Tower Hospital from January 2019 to June 2020 were included. Computed tomography images of 167 thoracic vertebral segments scanned in the prone position were collected. The width of the intertransverse space (DP), the height of the rib neck/head above the lower transverse process (DR), the width of the lateral border of the articular process/lamina (WP), and the width of the posterior border of the vertebral body (WV) were measured. At the upper 1/3 of the intervertebral foramina, the horizontal inclination angle (α) from the lateral border of the articular process/lamina to the posterolateral border of the vertebral body was measured. The ratios DR/DP and WP/WV were calculated. The intervertebral foramen parameters were compared between segments. RESULTS:No rib head/neck occlusion (DR/DP > 0) was found in the intertransverse spaces of T1-2 and T12-L1. The incidence of occlusion for the upper thoracic segments (T1-5, n = 138), middle thoracic segments (T5-9, n = 116), and lower thoracic segments (T9-L1, n = 80) were 76.81%, 100%, and 82.50%, respectively. The incidence of occlusion for the middle thoracic segments was significantly higher than that for the upper and lower thoracic segments (P < 0.05). The incidence of > 1/2 occlusion (DR/DP > 1/2) for the upper, middle, and lower thoracic segments was 7.97%, 74.14%, and 32.50%, respectively. The incidence of > 1/2 occlusion for the middle thoracic segments was significantly higher than that for the upper and lower thoracic segments (P < 0.05). WP was longer than WV on T1-2 to T9-10 and shorter than WV on T10-11 to T12-L1. The horizontal puncture angle (α) into the external opening of the intervertebral foramina was positively correlated with the segments of the thoracic vertebrae from the cephalic to caudal portion (left: r = 0.772, P < 0.01; right: r = 0.771, P < 0.01), and the horizontal inclination angle for T11-12 and T12-L1 was 90°. CONCLUSION:It is necessary to identify the spatial impact of the rib head/neck on the puncture path of the intervertebral foramina and design appropriate puncture angles for different segments.
