Abstract:Granulocyte colony-stimulating factor (G-CSF) is increasingly being used to prevent febrile neutropenia associated with chemotherapy. Large-vessel vasculitis (LVV) has been recognized as a rare side effect of G-CSF treatment. We report a case of G-CSF associated LVV in a patient with breast cancer. While clear pathogenic mechanisms remain unknown, G-CSF may cause vasculitis due to inflammatory cytokines production. This adverse reaction should be recognized in patients with suggestive symptoms following the administration of pegfilgrastim. A 56-year-old woman with luminal B breast cancer who had undergone surgery and adjuvant chemotherapy, initially with paclitaxel, was started on a doxorubicin plus cyclophosphamide protocol, followed by supportive use of long-acting G-CSF pegfilgrastim. Following the administration of pegfilgrastim, the patient developed intermittent fever and was given empiric antibiotics in the outpatient setting with no improvement. There were no signs of cancer progression, and the contrast-enhanced CT scan highlighted wall thickening of the aortic arch and the proximal segment of the subclavian artery, which was not present in previous imaging studies. The patient was diagnosed with LVV, and a differential diagnosis was performed to rule out paraneoplastic setting, immune-mediated diseases, infection or other drug-induced vasculitis. Treatment with steroids was initiated and tapered with significant improvement and resolution of the radiological signs of aortitis.

Abstract:A 3-year-old girl presented with fever, abdominal distension and anorexia for 3 days. Physical examination showed pallor, abdominal distension and hepatomegaly (4 cm below the costal margin). Laboratory tests showed granulocytosis (17.0–31.3 × 10/L), anaemia (Hb 60–85 g/L), eosinophilia (0.58–1.89 × 10/L), raised immunoglobulin E (IgE) (>400 IU/mL), C-reactive protein (CRP) (>1600 mg/L) and erythrocyte sedimentation rate (ESR) (135 mm/h). Chest computed tomography showed multiple small nodules with sharp borders and irregular size scattered in both lung fields (Fig. 1a). Magnetic resonance imaging showed multiple nodular lesions in the liver (Fig. 1b), and enlarged abdominal aortic and superior mesenteric lymph nodes. Abstract:We report a case of myeloid neoplasm with the abnormality of PDGFRB gene secondary interstitial pneumonia. A 66-year-old male complained dry cough and shortness of breath after regular activities. Two years ago, high-resolution computerized tomography (HRCT) showed interstitial lung diseases, and no clear etiology was found. At that time, the diagnosis was idiopathic pulmonary fibrosis. In the period of hospitalization, the interstitial lung disease was significantly advanced, and the peripheral blood eosinophil was significantly increased. ETV6-PDGFRB fusion genes was positive detected by FISH. Myeloid neoplasm with the abnormality of PDGFRB gene was diagnosed. The patient was treated with the tyrosine kinase inhibitor-imatinib mesylate which is the first choice for this disease currently.Myeloid neoplasm with the abnormality of PDGFRB gene is a rare hematologic tumor characterized by myeloid dysplasia, eosinophilia, and PDGFR gene rearrangement. Most of this tumor occurs in male, often with splenomegaly, and a few have damages in liver, skin, heart, and other organ tissues.
Granulocyte-Colony Stimulating Factor (G-CSF)-Induced Aortitis: A Case Report

Abstract:Pegylated granulocyte colony-stimulating factor (G-CSF), commonly used in chemotherapy-induced neutropenia, has been associated with rare instances of aortitis. This study describes a 67-year-old female patient with estrogen receptor (ER)-positive, human epidermal growth factor receptor-2-positive breast cancer, undergoing chemotherapy with an epirubicin/cyclophosphamide (EC) regimen (epirubicin, cyclophosphamide) and pegylated G-CSF for neutropenia prophylaxis. Post-treatment, she developed symptoms including intermittent fever and severe arthralgia. Laboratory tests revealed an elevated white blood cell count, C-reactive protein levels, and erythrocyte sedimentation rate, while a computed tomography scan showed thickening in the aortic arch and descending aorta. Given the clinical presentation and exclusion of other potential causes, pegylated G-CSF-induced aortitis was suspected. The patient's symptoms improved significantly following the cessation of pegylated G-CSF, aiding in the differentiation from other types of aortitis. This study highlights the importance of considering pegylated G-CSF as a potential cause of aortitis in patients presenting with unexplained symptoms of fever and inflammation after chemotherapy. The rapid improvement upon discontinuation of the drug is a key feature distinguishing it from other aortitis causes. In conclusion, while rare, aortitis should be considered in the differential diagnosis of patients treated with pegylated G-CSF who exhibit relevant clinical symptoms. Early detection and management, including the discontinuation of the causative agent, are crucial for patient recovery and prognosis.
[A Case of Granulocyte Colony-Stimulating Factor-Related Aortitis that Developed during the Treatment of Advanced Prostate Cancer with Neuroendocrine Differentiation]

Abstract:An 81-year-old man with prostate cancer (cT3aN0M0), who had been undergoing hormonal therapy for 4 years and had maintained low prostate specific antigen levels, developed metastasized pelvic lymph nodes. A tissue biopsy revealed neuroendocrine differentiation of prostate cancer in the metastatic lymph nodes. Consequently, chemotherapy with carboplatin＋etoposide was initiated. During the first course, filgrastim was administered for 2 days due to a drop in his neutrophil count to 230/μl. During the second course, pegfilgrastim was administered as prophylaxis on day 4. However, on day 10 of the second course, he started to develop a fever and fatigue. Suspecting infection, antibiotics were administered, but failed to ameliorate his symptoms. On day 14, plain computed tomography revealed signs of aortic inflammation. Given the lack of improvement even after one week of antibiotic therapy, steroid treatment was initiated on the suspicion of granulocyte colony-stimulating factor (G-CSF) -induced aortitis, which rapidly improved his symptoms. Therefore, when encountering a case in which a fever remains unresponsive to antibiotics during chemotherapy with G-CSF agents, a differential diagnosis of aortic inflammation caused by G-CSF agents needs to be considered.
Granulocyte-colony stimulating factor-associated aortitis in a woman with advanced breast cancer: a case report and review of the literature

Abstract:Background: Granulocyte-colony stimulating factor (G-CSF) is increasingly been used to prevent febrile neutropenia (FN) associated with the administration of chemotherapy for various cancers. The most common adverse effects of G-CSF are bone pain and injection-site reactions and aortitis has rarely been reported. We report herein a rare case of G-CSF associated with aortitis in a woman with advanced breast cancer. Case presentation: A 72-year-old woman with estrogen receptor-negative human epidermal growth factor 2-positive breast cancer with distant metastases in the lung was admitted. Her treatment was initiated with docetaxel in combination with trastuzumab and pertuzumab followed by the supportive use of a long-acting G-CSF, pegfilgrastim. After administration of pegfilgrastim on day 5, the patient had an intermittent fever (body temperature up to 39.6 °C) on day 9 which continued irrespective of taking levofloxacin. She visited our outpatient clinic on day 13 with no objective symptoms other than fever. Laboratory tests revealed a high neutrophil count (15,000/μl) and a high C-reactive protein (CRP) level (46.35 mg/dl) without any other abnormalities. There was no response upon administration of antimicrobial agents. An 18F-fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) revealed thickening of the wall of the descending thoracic aorta and left pleural effusion. Therefore, thoracic aortitis induced by pegfilgrastim was suspected. On day 19, the fever resolved spontaneously followed by a gradual reduction in the neutrophil count and CRP level. In the follow-up CT, the aortic wall thickness and pleural effusion had disappeared. Conclusions: G-CSF may cause aortitis due to stimulation of the production of inflammatory cytokines. In case of high continuous fever after administration of pegfilgrastim, aortitis should be suspected unless there are other infectious findings.

Abstract:BACKGROUND:Angiosarcoma is a rare, highly malignant tumor prone to recurrence and metastasis. Angiosarcoma is insidious in the initial stage, and its clinical manifestation lacks specificity. The diagnosis is based on histopathology and immunohistochemistry findings.CASE PRESENTATION:A 73-year-old man was hospitalized following complaints of persistent cough 6 months and hemoptysis for 2 months. Anti-infective treatment was ineffective. A CT-guided percutaneous core needle biopsy of pulmonary lesions revealed organized pneumonia, and the removed skin of purpuric rash area on the left calf revealed vasculitis. Chest CT was used during the patient follow-up. Hormonal therapy combined with immunoglobulins did not lead to improvement, and there was rapid progression of the lung lesions. Subsequently, the patient underwent a surgery, the diseased tissue was separated and removed completely beside the left submandibular gland under local anaesthesia. The immunohistochemical staining indicated CD31 (+) and CD34 (+) confirming a diagnosis of metastatic angiosarcoma. The expression of PD-L1 was 70%, therefore, anlotinib and pembrolizumab treatments were initiated. The patient eventually died.CONCLUSION:Angiosarcoma is a malignant tumor in the clinic that lacks typical and specific signs and symptoms. The diagnosis depends on immunohistochemistry, which requires repeated biopsies of multiple sites in highly suspected cases.

Abstract:A 72-year-old man with diffuse large B-cell lymphoma underwent fluorine-18 fluorodeoxyglucose (FDG) PET/CT, revealing lymphoma lesions and no evidence of aortitis. The patient received chemotherapy and was treated with granulocyte colony-stimulating factor (G-CSF) for neutropenia. During chemotherapy, the patient underwent PET/CT again, revealing FDG accumulation and wall thickening at the aortic arch, which suggested aortitis. The patient was only experiencing fatigue. G-CSF-associated aortitis was suspected, and the original G-CSF was switched to another G-CSF while continuing chemotherapy. Three months later, the third round of PET/CT showed that FDG accumulation and wall thickening of the aortic arch vanished. PET/CT may be useful for not only the diagnosis but follow-up of G-CSF-associated aortitis. Radiologists should recognize incidental aortitis on PET/CT in patients receiving G-CSF administration.

Abstract:Gastric adenosquamous carcinoma (ASC) is a rare type of gastric cancer. It is a mixed neoplasm, consisting of glandular cells and squamous cells. It is often diagnosed at an advanced stage, thus carrying a poor prognosis. We describe a case of a 73-year-old male, who presented with refractory fever and an intra-abdominal mass on imaging. He underwent a laparoscopic exploration followed by a successful totally laparoscopic total gastrectomy with D2 lymphadenectomy for gastric cancer. Postoperative pathology revealed primary gastric ASC (T4aN0M0). The patient received adjuvant radiotherapy and chemotherapy with S1 and is alive 20 mo after surgery without recurrence. This is the first case of advanced gastric ASC with fever as the initial presentation treated with totally laparoscopic total gastrectomy reported in the English literature.

Abstract:Abstract Granulocyte-colony stimulating factor (G-CSF) producing tumor is one type of growth factor producing tumor that induces leukocytosis. On the other hand, adenocarcinoma with enteroblastic differentiation disease is known as α-fetprotein (AFP), glypican3, and Sal-like protein 4 (SALL4) producing tumor. We report a first case of G-CSF producing esophageal cancer with enteroblastic differentiation which has never been reported in the world. Methods [Case presentation]: A fifty-six year-old man was admitted to our hospital to treat esophageal carcinoma. His body temperature was elevated. Blood biochemistry tests showed leukocytosis (WBC 14000/μl). Gastroscopy revealed a 10-cm diameter, superficial elevated lesion in the lower thoracic esophagus. Biopsy findings indicated a diagnosis of adenocarcinoma. No distant metastasis, but right subclavian and para esophageal lymph nodes swelling were identified via computed tomography. Thus, we planned neo adjuvant chemotherapy (NAC) (5-FU 800 mg/m2 plus Cisplatin 80 mg/m2) before surgical resection. The patient’s hyperthermia and leukocytosis improved by not antibiotics but NAC. Therefore, we suspected G-CSF producing tumor. Results The serum level of G-CSF was markedly elevated at 133 pg/ml (normal range, <39 pg/ml). Additionally, PET/CT showed abnormally high uptake of (18) F-FDG not only by the tumor itself, but also diffusely throughout the bone marrow. The patient underwent minimally invasive esophagectomy in the prone position with two-field lymph node dissection. Immunohistochemical studies showed G-CSF producing adenocarcinoma with enteroblastic differentiation (AFP negative, glypican3 and SALL4 positive). Pathological stage was pT2N0M0, stageII. After resection, improvement of hyperthermia, leukocytosis, and serum level of G-CSF can be seen. Findings of (18) F-FDG throughout the bone marrow were also disappeared. Conclusion We experienced an extremely rare case of G-CSF producing, esophageal adenocarcinoma with enteroblastic differentiation.

Abstract:A 16-year-old adolescent girl with CD19 chimeric antigen receptor (CAR) T-cell therapy for acute lymphoblastic leukemia experienced new onset of the fever. 18 F-FDG PET/CT studies acquired at 1 and 2 months, respectively, after CAR-T, showed foci of abnormal activity in the mediastinal lymph nodes not seen on the study before therapy. However, these foci of abnormal activity were later proven due to newly developed tuberculosis after CAR T-cell therapy.
[A Case of Aortitis Caused by Pegfilgrastim Use during Neoadjuvant Chemotherapy for Treating Breast Cancer]

Abstract:Granulocyte colony-stimulating factor(G-CSF)is useful for preventing febrile neutropenia induced by chemotherapy. Recently, some cases of aortitis have been reported following administration of G-CSF. Here, we present a case of aortitis induced by pegfilgrastim(peg-G)use during neoadjuvant chemotherapy for treating breast cancer. A 61-year-old woman with breast cancer(cT2N1M0, stage ⅡB, triple negative)started neoadjuvant chemotherapy FEC(100). Eleven days after the third course of peg-G administration, the patient developed a fever and general malaise. Blood test results showed an increase in inflammatory markers and severe anemia. The symptoms were not controlled with antibiotics. Blood and urine culture test results were negative. Computed tomography revealed remarkable wall thickening of the aorta. Therefore, we suspected aortitis induced by peg-G. The symptoms rapidly improved with prednisolone therapy. The possibility of aortitis should be considered for those with fever or raised inflammatory markers following the use of G-CSF. Steroids can be used for the treatment of G-CSF-induced aortitis.
A case of aortitis during cisplatin-based chemotherapy for cervical cancer

Abstract:A case of aortitis in a patient undergoing adjuvant cisplatin and topotecan chemotherapy for cervical cancer following presentation with pyrexia of unknown origin and raised inflammatory markers is presented. Although many chemotherapy agents are known to cause small vessel vasculitis and there are several reported cases of large vessel vasculitis following gemcitabine chemotherapy, there is only one previously described case of aortitis following cisplatin administration. This case is presented with corresponding CT and 18F-FDG PET-CT imaging with discussion of the literature regarding vasculitis and chemotherapy.

Abstract:Background: Arrhythmias in patients with cancer are predominantly attributed to heart disease, anti-tumor drugs, and primary cardiac tumors. Arrhythmias directly induced by noncardiac primary tumors are rare. To date, there have been no reported cases of paroxysmal supraventricular tachycardia or sinus bradycardia caused by a posterior mediastinal tumor originating from the lung tissue. Case description: We present the case of a 65-year-old male diagnosed with a giant posterior mediastinal tumor originating from the lung tissue, A mediastinal lymph node biopsy was performed, and histopathological examination revealed a poorly differentiated adenocarcinoma of pulmonary origin. Based on the PET-CT and cranial enhancend MRI results, the posterior mediastinal tumor has a diameter exceeding 7 cm and has metastasized to the right hilar and mediastinal lymph nodes, with no distant metastasis observed. On the basis of these findings, the patient was diagnosed with stage IIIB mediastinal lung adenocarcinoma (cT4N3M0). The tumor measured 12.8 cm × 8.1 cm and could not be completely resected. Prior to immunochemotherapy, the patient experienced paroxysmal supraventricular tachycardia, followed by sinus bradycardia. After the first cycle of immunochemotherapy, the arrhythmia was corrected, and the body temperature returned to normal. However, the massive tumor led to malnutrition and various complications prior to the second cycle of treatment. Despite undergoing chemotherapy combined with immunotherapy, the patient's condition worsened, ultimately resulting in death. The overall survival period was 3 months. Conclusion: This case report highlights the fact that a giant mediastinal lung adenocarcinoma can induce arrhythmias, which may manifest as supraventricular tachycardia or sinus bradycardia. Additionally, tumor fever also can induce elevated temperatures of 39 °C. Notably, giant posterior mediastinal tumors that are unsuitable for resection have a significantly poorer prognosis.

Abstract:A 48-year-old male was admitted to Peking Union Medical College Hospital presented with intermittent fever for two years. The maximum body temperature was 39 ℃, and could spontaneously relieve. The efficacy of antibacterial treatment was poor. He had no other symptoms and positive signs. He had a significant weight loss, and the serum lactate dehydrogenase increased significantly. It was highly alert to be lymphoma, but bone marrow smear and pathology, and PET-CT had not shown obvious abnormalities. Considering high inflammatory indicators, increased ferritin and large spleen, the patient had high inflammatory status, and was treated with methylprednisolone. Then the patient's body temperature was normal, but the platelet decreased to 33×109/L. During hospitalization, he had suddenly hemoperitoneum and hemorrhagic shock. He was found spontaneous spleen rupture without obvious triggers, and underwent emergency splenectomy. The pathological diagnosis of spleen was diffuse large B-cell lymphoma.

Abstract:Background: Disseminated carcinomatosis of the bone marrow (DCBM) is a widespread metastasis with a hematologic disorder that is mainly caused by gastric cancer. Although it commonly occurs as a manifestation of recurrence long after curative treatment, the precise mechanism of relapse from dormant status remains unclear. Granulocyte colony-stimulating factor (G-CSF) can promote cancer progression and invasion in various cancers. However, the potential of G-CSF to trigger recurrence from a cured malignancy has not been reported. Case summary: A 55-year-old Japanese woman was diagnosed with Ewing sarcoma localized on the fifth lumbar vertebrae 6 years after curative gastrectomy for T1 gastric cancer. After palliative surgery to release nerve compression, pathological diagnosis of the resected specimen was followed by curative radiation and chemotherapy. During treatment, G-CSF was administered 32 times for severe neutropenia prophylaxis. Eight months after completing definitive treatment, she complained of severe back pain and was diagnosed as multiple bone metastases with DCBM from gastric cancer. Despite palliative chemotherapy, she died of disseminated intravascular coagulation 13 d after the diagnosis. Immunohistochemical examination of the autopsied bone marrow confirmed a diffuse positive staining for the G-CSF receptor (G-CSFR) in the relapsed gastric cancer cell cytoplasm, whereas the primary lesion cancer cells showed negative staining for G-CSFR. In this case, G-CSF administration may have been the key trigger for the disseminated relapse of a dormant gastric cancer. Conclusion: When administering G-CSF to cancer survivors, recurrence of a preceding cancer should be monitored even after curative treatment.
18F-FDG PET/CT Imaging in Detection of Intravascular Large B-Cell Lymphoma

Abstract:Intravascular large B-cell lymphoma (IVLBCL) is a rare type of extranodal large B-cell lymphoma. Now, it remains a diagnostic challenge, because of non-specific findings on clinical, laboratory, and imaging studies. Here we present a case of an IVLBCL patient, who presented with fever of unknown origin and had skin involvement, that 18F-FDG PET/CT showed increased metabolism on systemic subcutaneous fat layer with a SUVmax of 1.29. After five courses of R-CHOP, 18F-FDG PET/CT showed disappearance of the diffuse FDG accumulation on systemic subcutaneous fat layer with a SUVmax of 0.55. These features make this case unique.
Diffused Pulmonary Infiltrates with T-Cell Large Granular Lymphocytic Leukemia

Abstract:SESSION TITLE: Pulmonary Manifestations of Systemic Disease SESSION TYPE: Case Report Slide PRESENTED ON: Saturday, April 16, 2016 at 09:45 AM - 11:15 AM INTRODUCTION: Here we presented a case with bilateral diffused pulmonary infiltration accompanied with a rare T-cell large granular lymphocytic leukemia (T-cell LGL leukemia). CASE PRESENTATION: A 50-year-old Chinese male presented with fever, pancytopenia and splenomegaly 4 years ago. Splenectomy was performed, but lymphocytosis was revealed in follow-up. He developed recurrent non-productive cough with exertional dyspnea in recent one and a half years, which temporarily improved after antibiotic treatment. However, serial chest CT scans showed progressive bilateral diffused pulmonary infiltrates (fig 1). Bone marrow examination prior to admission revealed a prominent lymphocytosis with a proportion of 96%, predominated by mature lymphocytes. Flow cytometry showed monoclonal CD3+CD8+TCRVβ cells, among which TCRVβ20 cells accounted for 84.93%. T-cell LGL leukemia was confirmed by transmission electron microscopy. He was admitted to our hospital for an unrelieved deteriorating episode twenty days ago. The peripheral leukocyte count was 51.89×109/L, with 90.3% lymphocytes. Bronchoscopy showed no bronchial mucosal congestion or purulent secretion. The cytometry of bronchoalveolar lavage fluid showed 22% macrophages and 78% lymphocytes. Video-assisted thoracoscopic lung wedge resection was performed, and the pathology showed perivascular infiltration of lymphocyte-like cells with large and irregular nuclear, consistently in size (figure 2). Immunohistochemical staining revealed CD3 (major+), CD5 (major+), CD8 (major+), CD20 (minor+), CD23 (-), CD38 (focal+), CD56 (-), cyclinD1 (-), CD4 (minor+), CD7 (minor+), κ (+), λ (+), TIA (+), GramB (+), Ki-67 (scattered +). Finally, the pulmonary infiltration was confirmed as a local involvement of T-cell LGL leukemia. Fifteen milligram (mg) of methotrexate per week and 40mg methylprednisolone per day was administered. His symptoms, count of lymphocytes and lung infiltrates were improved after treatment for a month. DISCUSSION: T-cell LGL leukemia is characterized by clonal proliferation of large granular lymphocytes (LGLs) and cytopenia. Approximately 40% of patients have an associated autoimmune disease, such as rheumatoid arthritis, and one-third of patients are asymptomatic at diagnosis1. There is no previous reports about diffused pulmonary infiltrates caused by T-cell LGL leukemia so far. Infection should be considered in differential diagnosis, since it may occur due to neutropenia. Currently, there is no standard treatment, while immunosuppressive therapy with low doses of methotrexate or cyclophosphamide is commonly used2. Abstract:A 60-year-old Chinese female patient was admitted to the hospital with complaint of intermittent fever for more than seven months. The main clinical manifestations were acute kidney injury and nephrotic syndrome which developed into a hemophagocytic syndrome. The symptoms did not improve with antibiotics. Moreover, prednisone could only reduce the fever. Finally, a kidney biopsy showed many CD20-positive cells in the glomerulus and some in the peritubular capillaries. This led to a diagnosis of renal intravascular large B-cell lymphoma.
