Abstract:Objective The management of fluid infusion is crucial in severe sepsis/septic shock patients.The correlation of extravascular lung water index(EVLWI) versus oxygenation index ( PaO2/FiO2 ) and EVLWI versus intrathoracic blood volume index(ITBVI) were analysed in this present study. Method Totally 24 patients,admitted to the Intensive Care Unit of Second Affiliated Hospital of Zhejiang University, College of Medicine and diagnosed as severe sepsis/septic shock with acute lung injury and/or acute respiratory distress syndrome,were enrolled. ITBVI and EVLWI were detected with PiCCO technique. Correlation of EVLWI and PaO2/FiO2, ITBVI and EVLWI were analysed,respectively. Simple correlation and simple linear regression were used for statistical analysis. Results Significant negative correlation was found of EVLWI and PaO2/FiO2 ( r = - 0. 45, P ＜ 0.01).EVLWT = 14 mL/kg was defined as the cutoff value for the subgroup analysis. No correlation was found between EVLWI and PaO2/FiO2 in the subgroup with EVLWI≤ 14 mL/kg ( r = 0. 12, P = 0.243), but in the subgroup with EVLWI ＞ 14 mL/kg, significant negative correlation was found ( r = - 0. 47, P ＜ 0. 01 ). When EVLWI was higher than 14 mL/kg,EVLW should be decreased to improve oxygenation and other aspects should be taken into account. No significant correlation was found between ITBVI and EVLWI. A ITBVI value 1000 mL/m2 was also defined as the cutoff value for the subgroup analysis. No significant correlation was found in the subgroup with ITBVI≤ 1000 mL/m2( r = 0.13, P = 0.17), while significant positive correlation was found in the subgroup with ITBVI ＞ 1000 mL/m2. This result suggested that in patients of severe sepsis/septic shock with ALI/ARDS, when the blood volume is high, ITBV should be decreased to improve the oxygenation,however,it is not useful in the situation of high pulmonary vascular permeability. Conclusions Extravascular lung water has a important role in the fluid management in patients of severe sepsis/septic shock with ALI/ARDS.
Effects of lung recruitment maneuver on extravascular lung water index in patients with hydrostatic pressure pulmonary edema

Abstract:Objective To investigate the effects of lung recruitment maneuver (RM) on extravascular lung water index in patients with hydrostatic pressure pulmonary edema. Methods Thirty nine patients with hydrostatic pressure pulmonary edema and normal pulmonary vascular permeability index (PVPI) and extravascular lung water index (EVLWI)≥10ml/kg were enrol ed in the study. Among them, 21 patients received RM (RM group) and RM was not executed in other 18 patients (control group). In the RM group, the EVLWI, global end- diastolic volume index(GEDVI), cardiac index(CI), oxygenation index(PO2/FiO2), dynamic lung compliance(Cdyn), oxygen delivery(DO2), oxygen consumption(VO2), alveolo- arterial oxygen partial pressure difference (PA-aO2), liquid intake(T0) and output volume at 0(T1),1(T2), 2(T3), 3(T4), 6(T5),12(T6) and 24h(T7) after RM, and length of intensive care unit stay and mechanical ventilation were documented and compared with control group. Results EVLWI and GEDVI in RM group were lower than those in control group at T1~T4. PO2/FiO2 was higher than that in control group at T1~T5. Cdyn was higher than that in control group at T1~T4, PA-aO2 higher than that in control group at T1~T2 (P<0.05). There was no significant difference in EVLWI in RM group at T1~T3 and there was significant difference at T1~T5. There were no significant differences in DO2, VO2, length of intensive care unit stay and mechanical ventilation between RM and control groups(P>0.05). GEDVI, PO2/FiO2, PA-aO2 and Cdyn levels were correlated with EVLWI (al P<0.05). Conclusion RM can reduce the level of ELVWI transiently in hydro-static pressure pulmonary edema and improve PO2/FiO2 and Cdyn, but can not affect DO2 and VO2, and shorten the length of intensive care unit stay and mechanical ventilation.

Abstract:The prognostic value of extravascular lung water (EVLW) has been widely investigated; however, a wide range of its predictive accuracy has been reported.A meta-analysis of diagnostic test studies was conducted.Various patient populations in the intensive care unit were included, such as burned patients and patients with acute lung injury/acute respiratory distress syndrome and sepsis.A computerized search of PubMed, Current Contents, CINAHL, and EMBASE from inception until March 1, 2011, was performed to identify potentially relevant articles. The inclusion criteria were studies investigating the prognostic value of EVLW in critically ill patients. There was no language restriction in the searching.The EVLW index (EVLWI) was used.The outcome was mortality (including in-hospital mortality, intensive care unit mortality, and 28-day mortality).We analyzed data from 11 studies and 9 countries involving 670 patients. Overall, the EVLWI was significantly higher in nonsurvivors than in survivors, with a mean difference of 5.06 mL/kg (95% confidence interval, -7.53 to -2.58). The heterogeneity was significant with I(2) = 90%. The pooled statistics of diagnostic accuracy together with relevant 95% confidence interval were as follows: sensitivity, 0.81 (0.72-0.88); specificity, 0.66 (0.55-0.76); diagnostic odds ratio, 8.84 (3.83-20.4), positive likelihood ratio, 2.44 (1.69-3.52); negative likelihood ratio, 0.28 (0.16-0.46).The sample sizes of included studies were small.The EVLWI appears to be a good predictor of mortality in critically ill patients.
[Effect of Fluid Resuscitation on Extravascular Lung Water in Early Stage of Septic Shock].

Abstract:OBJECTIVE To determine the effect of fluid resuscitation on extravascular lung water (EVLW) in early stage of septic shock. METHODS Twenty septic shock patients with hypovolemia [intrathoracic blood volume index (ITBVI)<750 ml/m(2)] were randomly divided into two groups: EVLW index (EVLWI)< or =7 ml/kg group (n=8) and EVLWI >7 ml/kg group (n=12) according to the EVLWI value determined on admission day. Fluid resuscitation was given at 250 ml in bolus every 15 minutes until the end point of ITBVI>850 ml/m(2) was reached. Repeated haemodynamic measurements were done at baseline (Tb), at the end point (T0) then at 1 hour (T1), 2 hours (T2) and 4 hours (T4) after the end point of resuscitation was reached. EVLWI, pulmonary vascular permeability index (PVPI), ITBVI, global end-diastolic volume index (GEDVI), cardiac index (CI), stroke index (SI), central venous pressure (CVP) and oxygenation index (PaO(2)/FiO(2)) were determined at each time point, and the outcomes of these patients after 28 days were recorded. RESULTS (1)ITBVI, GEDVI, CI, SI, CVP were increased significantly at T0 and remained elevated at T1 and T2 in two groups (P<0.05 or P<0.01), but declined at T4 with no significant difference compared with those at Tb (all P>0.05). There were also no significant differences at the same period of time between the two groups (all P>0.05). (2)No changes in PaO(2)/FiO(2) and EVLWI were found over time in two groups compared with those at Tb (all P>0.05), but the PaO(2)/FiO(2) in EVLWI>7 ml/kg group were all lower than that of EVLWI< or =7 ml/kg group at any time point (all P<0.05). (3)EVLWI was not significantly correlated with ITBVI, GEDVI, CI, SI, CVP but negatively correlated with PaO2/FiO2 (r=-0.765, P<0.01), and positively correlated with PVPI (r=0.678, P<0.01). (4)Six patients died within 28 days. EVLWI of the dead patients were prominently higher at Tb and T4 than those of survived patients (both P<0.01), but the EVLWI gap (DeltaEVLWI) had no difference before and after resuscitation (P>0.05). CONCLUSION Fluid resuscitation in early stage of septic shock can improve ITBVI, GEDVI, CI, SI, CVP, with neither increased EVLWI nor worsened oxygenation. EVLWI has significantly negative correlation with PaO(2)/FiO(2) but not with haemodynamics. The increase in PVPI is responsible for the elevation of EVLWI. The patient's outcome is significantly correlated with the value of EVLWI.
Effect of Early Protective Lung Ventilation on Mortality and Hemodynamics in Patients With Acute Myocardial Infarction Complicated With Cardiogenic Shock and Pulmonary Edema Undergoing Emergency Interventional Therapy

Abstract:Abstract Background: To investigate the effect of early protective lung ventilation (EPLV) on mortality and hemodynamic parameters in patients with acute myocardial infarction complicated with cardiogenic shock (CS) and pulmonary edema undergoing emergency percutaneous coronary intervention (PCI). Methods: From January 2015 to June 2017, patients with acute myocardial infarction complicated with CS and pulmonary edema were admitted to the Tianjin chest Hospital. Based on the use of a mechanical ventilator, patients were divided into the EPLV and Non-invasive ventilation (NIV) groups. Hemodynamic indexes and in-hospital mortality of patients between the two groups was analyzed. Results: The EPLV group consisted of 51 patients and the NIV group consisted of 38 patients. The difference in mortality rates was statistically significant between the EPLV and NIV groups ( P= 0.01). Central venous pressure, pulmonary artery pressure, and pulmonary capillary wedge pressure after emergency PCI in the EPLV group were lower compared to patients in the NIV group (( P< 0.05). Man arterial pressure in patients in the EPLV group was higher compared to NIV patients ( P< 0.05). Logistic regression analysis showed that EPLV did not increase the risk of mortality ( P= 0.37, OR=2.16, 95% CI (0.31, 9.52)). Conclusion: EPLV resulted in lower mortality and improved hemodynamic function in patients with acute myocardial infarction complicated with CS and pulmonary edema undergoing emergency PCI.
Clinical significance of extravascular lung water index in patients with acute cerebral disease in ICU

Abstract:Objective:To investigate the clinical significance of extravascular lung water index (EVLWI) in patients with acute cerebral disease in ICU.Methods:This study retrospectively investigated the medical records of 71 patients with acute cerebral disease in ICU of Peking University Shougang Hospital from January 2009 to December 2014. All the patients underwent PICCO catheter placement to monitor hemodynamic parameters within six hours. The general information including age, sex, APACHE II score, Glasgow score, and outcome was collected. The clinical diagnostic data such as the type of cerebral disease, hypertension, coronary heart disease, and heart failure were also collected. EVLWI was measured with a PULSION PICCO plus monitor. All patients were divided into either a death group or a survival group according to 28-day outcome and EVLWI was compared between the two groups.Results:There was a significant difference in Glasgow score between the two groups (P=0.02). There was a higher EVLWI in the death group on the 3rd and 5th day after the onset of the disease (P=0.024 and 0.010, respectively), and the death group also had a higher percentage of patients with EVLWI elevation on the 5th and 7th day (P=0.047 and 0.028, respectively). Correlation analysis showed that EVLWI and the percentage of patients with EVLWI elevation were positively correlated with the number of days of onset (r=0.14, P=0.005; r=0.991, P=0.000), and EVLWI was also positively correlated with the number of days of onset in the death group (r=0.337, P=0.000), but not in the survival group (r=0.039, P=0.587). In both groups, the proportion of patients with increased EVLWI was positively correlated with the number of days of onset (r=0.862, P=0.013; r=0.938, P=0.001). Further survival analysis showed that on the 5th and 7th day, EVLWI had an effect on the survival rate of the patients (P=0.038 and 0.011), and the survival rate of the patients with increased EVLWI was decreased significantly.Conclusion:After the onset of acute craniocerebral disease, EVLWI is related to the course of disease and the survival of patients.
Effect of the Application of Continuous Cenous Venous Hemofiltration on Pulmonary Vascular Permeability in the Early Stage of Septic Shock

Abstract:Objective To investigate the effect of continuous veno-venous hemofiltration (CVVH) on pulmonary vascular permeability in the early stage of septic shock.Methods Fifty-one patients of septic shock admitted in intensive care unit of the First Affiliated Hopsital of Medical College,Zhejiang University between June 2010 and December 2011 were randomized into two groups by simple random method.Routine treatment was carried out to patients in group A,and routine treatment plus CVVH to patients in group B in the first 72 hours.Repeated respiratory mechanic hemodynamic measurements were done at baseline (Tb) before treatment,at 48 hours (T48) and 72 hours (T72) after the treatment.Pulmonary compliance (Cst),platform pressure (Pplat),oxygenation index (PaO2/FiO2),extravascular lung water index (EVLWI) and E-selectin determined at each time point were recorded in two groups.Results (1) Pplat were decreased significantly after treatment in two groups,and the Pplat in group B were all lower than those in group A at T48 and T72 (t =2.215,P ＜ 0.05 ; t =4.266,P ＜ 0.01).Cst were elevated after treatment in two groups,and Cst in group B were all higher than those in group A at T48 and T72 (t =2.516,P ＜0.05 ; t =3.052,P ＜ 0.01).(2) Compared to before treatment,PaO2/FiO2 increased significantly after treatment in two groups,and PaO2/FiO2 in group B were all higher than those in group A at T48 and T72 (t =2.732,P ＜0.01 ; t =3.511,P ＜0.01).(3) EVLWI were decreased significantly after treatment in two groups,and the EVLWI in group B were all lower than those in group A at T48 and T72 (t =2.597,P ＜0.05; t =2.125,P ＜ 0.05).ITBVI,CI,SVRI and MAP did not change over time in two groups compared with those at Tb (all P ＞ 0.05).(4) E-selectin were decreased significantly after treatment in two groups,and the E-selectin in group B were all lower than those in group A at T48 and T72 (t =2.154,P ＜0.05 ; t =3.581,P ＜ 0.01).Conclusion CVVH in early stage of septic shock can improve pulmonary vascular permeability and oxygenation,increase Cst,and decline EVLWI,with neither increased hemodynamics.
The Influence of High Volume Hemofiltration on Extra Vascular Lung Water and Alveolar-Arterial Oxygen Pressure Difference in Patients with Severe Sepsis.

Abstract:OBJECTIVE: To explore the effects of high-volume hemofiltration (HVHF) on the plasma interleukin-6 (IL-6), pro-calcitonin (PCT), extra vascular lung water index (EVLWI) and alveolar-arterial oxygen exchange in patients with septic shock.PATIENTS AND METHODS: 97 cases intensive patients with septic shock were enrolled from Department of Intensive Care Unit (ICU) of the Provincial Hospital affiliated to Shandong University between January 2011 and December 2014. According to the puting into practice of high-volume hemofiltration (HVHF) or not, all the patients were divided in two groups (NHVHF group, group A, n = 46 cases) and (HVHF group, group B, n = 51 cases). The plasma IL-6, PCT, intrathoracic blood volume index (ITBVI), extravascular lung water index (EVLWI) and pulmonary vascular permeability index(PVPI) was detected before treatment and after treatment 24h, 72h. The Alveolar-arterial oxygen pressure difference P(A-a) DO2 was checked by arterial blood gas analysis (ABGA) at first and after treatment 24 hour, 72 hour, 7 day in two groups. The mortality at 28 day was compared between two groups.RESULTS: After 72h treatment, the plasma IL-6, PCT in group B has a significant decrease. After 72h treatment, the level ITBVI, EVLWI and PVPI in group B had a significant improvement. The levels of P(A-a) DO2 in HVHF group were reduced more significantly than N-HVHF group after 7 day. The EVLWI and P(A-a) DO2 had a significant positive correlation (correlation ratio = 0.712, 95% confident interval [0.617, 0.773], p = 0.001). The mortality at 28 day had a significant decrease between groups (15.22% vs. 34.15%,.2 = 4.242, p = 0.038).CONCLUSIONS: HVHF could decrease plasma inflammatory factors and EVLWI so that it could improve the levels of alveolar-arterial-oxygen exchange in patients with septic shock, so it could improve the survival rate of patients.
Prediction of Pulmonary Edema by Plasma Protein Levels in Patients with Sepsis.

Abstract:Purpose: The difficulties of fluid therapy in patients with septic shock are to maintain sufficient vascular volume while preventing pulmonary edema formation. Thus, it is important to find a biomarker that can reliably predict pulmonary edema formation after fluid loading. We evaluated the association of plasma protein levels with the increase in extravascular lung water index (Delta EVLWI) after fluid loading.Methods: This was an observational study in which we retrospectively reviewed medical records of septic patients in whom hemodynamic variables were measured by transpulmonary thermodilution technique before and after fluid loading. Plasma protein levels were measured before fluid loading. Patients were divided into 2 groups according to the changes in EVLWI (Delta EVLWI >= 10%) after fluid loading. Diagnostic performance of plasma proteins in predicting pulmonary edema formation was assessed.Results: A total of 62 patients were included, and 27 of them showed a Delta EVLWI 10% or higher after fluid loading. Plasma albumin and transferrin were significantly lower in this group than in the group with Delta EVLWI less than 10% (21.7 +/- 5.8 vs 25.3 +/- 5.0 g/L for albumin, P < .05; 107.9 +/- 50.1 vs 136.8 +/- 44.2 mg/dL for transferrin, P < .05). Areas under the curve of albumin and transferrin were 0.68 (cardiac index, 0.54-0.83) and 0.72 (cardiac index, 0.59-0.86), respectively. At a cutoff value of 87.9 mg/dL, transferrin had a sensitivity of 0.91 in predicting Delta EVLWI 10% or higher.Conclusions: Plasma transferrin and albumin levels were associated with Delta EVLWI 10% or higher after fluid loading. The high sensitivity of both biomarkers indicated that patients with normal values were less likely to develop pulmonary edema after fluid loading. (C) 2012 Elsevier Inc. All rights reserved.

Abstract:Severe acute respiratory distress syndrome (ARDS) is commonly seen in intensive care units (ICUs). It is characterized by capillary endothelium and alveolar epithelium damage that results in significant increases in capillary permeability and extravascular lung water. It is difficult to quantify pulmonary edema accurately using conventional imaging. Pulse index continuous cardiac output (PiCCO) is a parameter based on transpulmonary thermodilution that overcomes these shortcomings. Several measurements derived from PiCCO can be determined continuously and accurately, including extravascular lung water index (ELWI), and pulmonary vascular permeability index (PVPI). Several prospective studies have demonstrated that early goal-directed treatment (EGDT) according to these measurements can help with volume management and ultimately reduce the in-hospital mortality rate in critically-ill patients.<sup><a class="ejp-citation-link js-ejp-citation-link">[1]</a></sup>

The Influence of Positive End-Expiratory Pressure on Stroke Volume Variation for the Accuracy of Evaluating Volume

Abstract:OBJECTIVE:To investigate the influence of positive end-expiratory pressure (PEEP) on hemodynamic and the ability of stroke volume variation (SVV) to predict cardiac preload.METHODS:Thirty healthy anesthetized pigs were given tracheal intubation and ventilated. With the envelope method, they were all randomly divided into control group(n = 10), hypovolemia group (n = 10) and hypervolemia group (n = 10). Hypovolemia group were exsanguinated 20% blood volume within 5 minutes, hypervolemia group: additional infusion of hydroxyethyl starch equal to 20% blood volume,and control group: no intervention. In each group, ventilator settings were changed in a randomized order by changing PEEP [0, 5, 10 and 15 cm H(2)O, PEEP0, PEEP5, PEEP10, PEEP15, 1 cm H(2)O = 0.098 kPa]. The changes in hemodynamic parameters, including heart rate (HR), mean arterial pressure (MAP), central venous pressure (CVP), cardiac index (CI), stroke volume index (SVI), systemic vascular resistance index (SVRI), intrathoracic blood volume index (ITBVI) and SVV, were monitored with a pulse-indicated continuous cardiac output (PiCCO).RESULTS:In the control group, HR, CVP, SVRI and SVV were evaluated accompanying with the increasing of PEEP, but CI, SVI and ITBVI submitted decreasing tendency. The value reached peaking or valley on the level of PEEP15, and there was significance compared with PEEP0 [HR (bpm): 124 ± 18 vs. 88 ± 12, CVP (mm Hg, 1 mm Hg = 0.133 kPa): 11 ± 1 vs. 8 ± 3, SVRI [kPa×s×L(-1)×m(-2)]: 289.6 ± 81.5 vs. 215.0 ± 79.1, SVV: (23 ± 6)% vs. (11 ± 2)%, CI [L×min(-1)×m(-2)]: 3.1 ± 0.8 vs. 4.3 ± 1.4, SVI [ml×min(-1)×m(-2)]: 26 ± 7 vs. 41 ± 4, ITBVI [ml/m(2)]: 440 ± 43 vs. 491 ± 47, all P < 0.05]. There was no change in MAP. In the hypovolemia group, HR, CVP and SVV were evaluated accompanying with the increasing of PEEP, but MAP, CI, SVI and ITBVI submitted decreasing tendency. The value reached peaking or valley on the level of PEEP15, and there was significance compared with PEEP0 [HR (bpm): 146 ± 31 vs. 115 ± 27, CVP (mm Hg): 11 ± 2 vs. 5 ± 1, SVV: (28 ± 4)% vs. (20 ± 5)%, MAP(mm Hg): 90 ± 26 vs. 115 ± 19, CI [L×min(-1)×m(-2)]: 2.3 ± 0.6 vs. 3.4 ± 1.1, SVI [ml×min(-1)×m(-2)]: 20 ± 6 vs. 31 ± 9, ITBVI [ml/m(2)]: 355 ± 34 vs. 396 ± 53, all P < 0.05]. There was no change in SVRI. In the hypervolemia group, SVV submitted increasing tendency with the increasing of PEEP, but CI, SVI and ITBVI were in the tendency of decreasing, the value reached peaking or valley on the level of PEEP15, and there was significance compared with PEEP0 [SVV: (18 ± 4)% vs. (6 ± 2)%, CI [L×min(-1)×m(-2)]: 4.5 ± 0.9 vs. 5.0 ± 1.2, SVI [ml×min(-1)×m(-2)]: 37 ± 9 vs. 49 ± 7, ITBVI [ml/m(2)]: 473 ± 71 vs. 565 ± 94, all P < 0.05]. There was no change in HR, MAP, CVP and SVRI. SVV was increased in the hypovolemia group compared with control group, and decreased in the hypervolemia group. In the control group, SVV was negatively related to CI on different level of PEEP [r(PEEP0) = -0.831, r(PEEP5) = -0.790, r(PEEP10) = -0.875, r(PEEP15) = -0.560, P < 0.05 or P < 0.01].CONCLUSION:SVV was a precise indicator of cardiac preload, however high PEEP may influence hemodynamic and the accuracy of SVV.

Abstract:Objective: To investigate the effect of inferior vena cava variability (IVCV) combined with difference of central venous-to-arterial partial pressure of carbon dioxide (Pcv-aCO2) on guiding fluid resuscitation in septic shock. Methods: Patients with septic shock admitted to the department of critical care medicine of Jiangxi Provincial People's Hospital from January 1, 2018 to December 31, 2020 were enrolled, and they were divided into control group and observation group according to random number table method. Patients in both groups were given fluid resuscitation according to septic shock fluid resuscitation guidelines. The patients in the control group received fluid resuscitation strictly according to the early goal-directed therapy (EGDT) strategy. Resuscitation target: central venous pressure (CVP) 12-15 cmH2O (1 cmH2O≈0.098 kPa), mean arterial pressure (MAP) > 65 mmHg (1 mmHg≈0.133 kPa), mean urine volume (UO) > 0.5 mL×kg-1×h-1, central venous oxygen saturation (ScvO2) > 0.70. In the observation group, the endpoint of resuscitation was evaluated by IVCV dynamically monitored by bedside ultrasound and Pcv-aCO2. Resuscitation target: fixed filling of inferior vena cava with diameter > 2 cm, IVCV < 18%, and Pcv-aCO2 < 6 mmHg. The changes in recovery indexes before and 6 hours and 24 hours of resuscitation of the two groups were recorded, and the 6-hour efficiency of fluid resuscitation, 6-hour lactate clearance rate (LCR) and 6-hour and 24-hour total volume of resuscitation were also recorded; at the same time, the duration of mechanical ventilation, length of intensive care unit (ICU) stay, 28-day mortality and the incidence of acute renal failure and acute pulmonary edema between the two groups were compared. Results: A total of 80 patients were enrolled in the analysis, with 40 in the control group and 40 in the observation group. The MAP, CVP and ScvO2 at 6 hours and 24 hours of resuscitation in the two groups were significantly higher than those before resuscitation, while Pcv-aCO2 and blood lactic acid (Lac) were significantly decreased, and UO was increased gradually with the extension of resuscitation time, indicating that both resuscitation endpoint evaluation schemes could alleviate the shock state of patients. Compared with before resuscitation, IVCV at 6 hours and 24 hours of resuscitation in the observation group were decreased significantly [(17.54±4.52)%, (18.32±3.64)% vs. (27.49±10.56)%, both P < 0.05]. Compared with the control group, MAP and ScvO2 at 6 hours of resuscitation in the observation group were significantly increased [MAP (mmHg): 69.09±4.64 vs. 66.37±4.32, ScvO2: 0.666±0.033 vs. 0.645±0.035, both P < 0.05], 24-hour MAP was increased significantly (mmHg: 75.16±3.28 vs. 70.12±2.18, P < 0.05), but CVP was relatively lowered (cmH2O: 9.25±1.49 vs. 10.25±1.05, P < 0.05), indicating that the fluid resuscitation efficiency was higher in the observation group. Compared with the control group, 6-hour LCR in the observation group was significantly increased [(55.64±6.23)% vs. (52.45±4.52)%, P < 0.05], 6-hour and 24-hour total volume of resuscitation was significantly decreased (mL: 2 860.73±658.32 vs. 3 568.54±856.43, 4 768.65±1 085.65 vs. 5 385.34±1 354.83, both P < 0.05), the duration of mechanical ventilation was significantly shortened (days: 6.78±3.45 vs. 8.45±2.85, P < 0.05), while the incidence of acute pulmonary edema was significantly decreased [2.5% (1/40) vs. 20.0% (8/40), P < 0.05]. There was no significant difference in the length of ICU stay, 28-day mortality or incidence of acute renal failure between the two groups. Conclusions: Dynamic monitoring of IVCV and Pcv-aCO2 can effectively guide the early fluid resuscitation of patients with septic shock, and compared with EGDT, it can significantly shorten the duration of mechanical ventilation, reduce the amount of fluid resuscitation, and reduce the incidence of acute pulmonary edema. Combined with its non-invasive characteristics, it has certain clinical advantages.

Abstract:Objective To investigate effects of Levosimendan on hemodynamics in heart failure patients with low cardiac output. Methods A total of 27 heart failure patients with low cardiac output were recruited in the surgical intensive care unit (SICU)of Beijing Hospital from July 2009 to May 2016. All patients were treated with continuous intravenous infusion of Levosimendan 12.5 mg within 24 h at a rate of 0.1~0.2 μg· kg -1·min-1. Hemodynamic variables were monitored by pulse-indicated continuous cardiac output(PICCO)at baseline ,24 h ,4 d and 7 d after infusion. Results There were no significant differences in systolic blood pressure(SBP) ,diastolic blood pressure(DBP) ,heart rate(HR)and global end diastolic volume index (GEDI)as comparing baseline with 24 h ,4 d and 7 d after infusion of levosimendan. The significant differences were found in central venous pressure (CVP) ,systemic vessel resistance index (SVRI) , extravascular lung water index (EVLWI ) among baseline versus 24 h ,4 d and 7 d after infusion of levosimendan[(8.34± 2.87)mmHg vs.(6.35± 2.31),(6.81± 2.03),(5.92± 2.23)mmHgforCVP ,(2 682.0 ± 388.9)dyn·s-1·cm -5·m-2vs. (2 170.0 ± 410.9) ,(2 062.0 ± 340.6 ) ,(1 960.0 ± 380.2 )dyn ·s-1·cm-5·m-2for SVRI ,(8.90 ± 2.45)ml/kg vs. (6.60 ± 2.66) ,(5.92 ± 2.88) ,(5.46 ± 2.52) ml/kg for EVLWI ,P=0.001 ,0.003 or＜0.01 ,respectively].As compared with hemodynamic variables at baseline ,brain natriuretic peptide levels were decreased ,and cardiac index ,cardiac function index and stroke volume index were increased at 24 h ,4 d and 7 d after infusion of Levosimendan (P＜0.01 ,0.05 or 0.01 , respectively ). Conclusions A single-dose intravenous infusion of Levosimendan can increase cardiac output and decrease systemic vascular resistance in heart failure patients with low cardiac output ,which maintain quite a long time.

Abstract:Background: Cardiogenic shock is a clinical syndrome caused by primary heart disease that results in decreased cardiac output and insufficient systemic perfusion. A study was conducted to determine what factors affect survival in patients with cardiogenic shock treated with extracorporeal membrane oxygenation (ECMO). Methods: A systematic search was conducted across various databases, including CKNI, VIP, Wan Fang, CBM, Embase, PubMed, Cochrane Library, and Web of Science databases, to gather factors linked to the prognosis of patients with cardiogenic shock who underwent ECMO treatment. The search period for each database was set to conclude on April 30, 2024. Results: The findings suggest that, in comparison to the death group, the lactic acid levels of the survival group after treatment were significantly lower (95% confidence interval [CI]: −0.79, −0.58). In addition, the creatinine levels of the survival group after treatment were also significantly lower than those of the death group (95% CI: −0.39, −0.14). Furthermore, the troponin levels in the survival group after treatment were lower than those in the death group (95% CI: −0.32, 0.04), and the total bilirubin levels in the survival group after treatment were also lower than those in the death group (95% CI: −0.62, −0.23). Conclusions: According to the study, total bilirubin, creatinine, and lactic acid levels were lower in the survival group than in the death group when ECMO was used to treat cardiogenic patients, suggesting a better prognosis for patients with cardiogenic shock. Therefore, total bilirubin, creatinine, and lactic acid could be influential factors in the prognosis of survival in patients with cardiogenic shock.

Abstract:Objective: To investigate the volume management of intermittent veno-venous hemofiltration (IVVH) guided by critical care ultrasound in the treatment of acute kidney injury (AKI) in patients with heart failure (HF). Methods: A total of 216 patients with HF and AKI treated with IVVH in the coronary care unit (CCU) of the Third Central Hospital of Tianjin from April 2019 to June 2022 were selected as the study subjects, the patients were randomly divided into conventional guidance group (107 cases) and ultrasound guidance group (109 cases). According to the recovery of renal function, IVVH was performed 12 hours every day or 12 hours every other day. The conventional guidance group selected the conventional method to formulate IVVH prescription, and the ultrasound guidance group used critical care ultrasound to adjust the treatment parameters of IVVH on the basis of the conventional guidance group. Respiratory variation index (RVI) of inferior vena cava (IVC), right left ventricular end-diastolic transverse area ratio, early diastolic peak mitral flow velocity/mitral annulus velocity peak (E/E'), aortic flow velocity time integral (VTI), cardiac output (CO), bilateral lung ultrasound B-line range, bilateral renal interlobar arteries resistance index (RI) were recorded before and 3, 6, 9 hours after each treatment. The net dehydration rate was adjusted in real time according to the comprehensive results. Urine volume, serum creatinine (SCr), estimated glomerular filtration rate (eGFR), blood B-type brain natriuretic peptide (BNP), β2-microglobulin (β2-MG) and cystatin C (Cys C) levels of patients in both groups were monitored before and 3, 7 and 10 days after initial treatment, and renal function recovery and clinical prognostic indexes of patients in both groups were recorded. Results: The dehydration rate of the ultrasound guidance group was slow at the beginning of IVVH, and gradually increased after 6 hours, and the overall dehydration rate was significantly slower than that of the conventional guidance group. In the ultrasound guidance group using critical care ultrasound, the RVI gradually increased, the right left ventricular end-diastolic area ratio gradually decreased, the E/E' ratio gradually decreased, and the range of B-line of bilateral lungs gradually decreased, RI of bilateral renal interlobar arteries decreased. At 3, 7 and 10 days after the first IVVH, renal function related indexes in both groups were significantly improved compared with before treatment, and the decline rate of β2-MG and Cys C in the ultrasound guidance group was faster than that in the conventional guidance group at early (3 days) [β2-MG (mg/L): 3.69±1.31 vs. 3.99±1.45, Cys C (mg/L): 2.91±0.95 vs. 3.14±0.96, both P < 0.05], urine volume, SCr and eGFR at 7 days were also significantly improved compared with the conventional guidance group [24-hour urine volume (mL): 1 128.23±153.92 vs. 1 015.01±114.18, SCr (μmol/L): 145.86±32.25 vs. 155.64±28.42, eGFR (mL/min): 50.26±11.24 vs. 46.51±10.61, all P < 0.05]. The time of SCr recovery, the time of reaching polyuria, the total time of IVVH treatment, the time of non-invasive mechanical ventilation and the time of living in CCU in the ultrasound guidance group were shorter than those in the conventional guidance group. The incidences of hypotension, long-term RRT, incidence of major cardiovascular adverse event (MACE) and at 28-day mortality were all lower than those in the conventional guidance group. Kaplan-Meier survival curve showed that the 28-day cumulative survival rate in the ultrasound guidance group was significantly lower than that in the conventional guidance group (Log-Rank test: χ2 = 3.903, P = 0.048). Conclusions: The strategy of IVVH guided by critical care ultrasound in the treatment of HF with AKI has unique advantages.
Left ventricular-arterial coupling as a predictor of stroke volume response to norepinephrine in septic shock - a prospective cohort study

Abstract:Background: Left ventricular-arterial coupling (VAC), defined as the ratio of arterial elastance (Ea) to left ventricular end-systolic elastance (Ees), is a key determinant of cardiovascular performance. This study aims to evaluate whether left VAC can predict stroke volume (SV) response to norepinephrine (NE) in septic shock patients. Methods: This was a prospective cohort study conducted in an intensive care unit of a tertiary teaching hospital in China. We recruited septic shock patients who had persistent hypotension despite fluid resuscitation and required NE to maintain mean arterial pressure (MAP) > 65 mmHg. Those patients in whom the target MAP was not reached after NE infusion were ineligible. Echocardiographic variables were measured before (baseline) and after NE infusion. SV responder was defined by a ≥ 15% increase in SV after NE infusion. Results: Of 34 septic shock patients included, 19 (56%) were SV responders. Before NE infusion, SV responders had a lower Ees (1.13 ± 0.24 mmHg/mL versus 1.50 ± 0.46 mmHg/mL, P = 0.005) and a higher Ea/Ees ratio (1.47 ± 0.40 versus 1.02 ± 0.30, P = 0.001) than non-responders, and Ea in SV responders was comparable to that in non-responders (1.62 ± 0.36 mmHg/mL versus 1.43 ± 0.28 mmHg/mL, P = 0.092). NE significantly increased Ea and Ees in both groups. The Ea/Ees ratio was normalized by NE administration in SV responders but unchanged in non-responders. The baseline Ea/Ees ratio was positively correlated with NE-induced SV increases (r = 0.688, P < 0.001). Logistic regression analysis indicated that the baseline Ea/Ees ratio was a predictor of SV increases induced by NE (odd ratio 0.008, 95% confidence interval (CI): 0.000 to 0.293), with an area under the receiver operating characteristic curve of 0.816 (95% CI: 0.646 to 0.927). Conclusions: The left VAC has the ability to predict SV response to NE infusion in septic shock patients. Clinical value of CVP+VIVC in predicting fluid resuscitation in patients with septic shock

Abstract:Objective: To explore the clinical value of central venous pressure (CVP) + inferior vena cava respiratory variability (VIVC) in fluid resuscitation in spontaneously breathing patients with septic shock. Methods: In retrospective observational study, during October 2019 to December 2021, 145 patients with septic shock treated in our hospital were enrolled by the method of observational study. According to the change rate of cardiac output (△ CO) ≥15% or △ CO<15% after 30 minutes, they were assigned into volume-responsive and volume-unresponsive group depending early fluid resuscitation in sepsis. The clinical value of combination of CVP and VIVC in predicting fluid resuscitation in patients with septic shock was compared. Results: The CVP of the study group was higher at 12h and 24h after fluid resuscitation, and the VIVC level of the study group at 6h, 12h and 24h after fluid resuscitation was higher (P<0.05). Pearson correlation analysis indicated that CVP, and VIVC levels were noticeably correlated with fluid resuscitation in patients with septic shock (P<0.05). The area under curve (AUC) of receiver operating characteristic curve (ROC) of CVP for predicting fluid resuscitation in septic shock patients was 0.694 and the cut-off value was 0.932, the sensitivity was 46.9%, and the specificity was 87.5%. VIVC predicted fluid resuscitation in septic shock patients with an AUC of 0.776, which was a cut-off value of 0.688, a sensitivity of 50.0%, and a specificity of 90.0%. Combination of CVP and VIVC predicted fluid resuscitation in septic shock patients with an AUC of 0.948, which was a cut-off value of 1.420, a sensitivity of 90.6%, and a specificity of 87.5%. Conclusion: Combination of CVP and VIVC may have a good effect on the evaluation of volume responsiveness in patients with septic shock, which is better than single CVP and VIVC. Combination of CVP and VIVC can be adopted to predict fluid responsiveness volume responsiveness in septic shock patients, which is of great significance for guiding clinical fluid responsiveness therapy. Abstract:Journal of Intensive Care Medicine, Ahead of Print. BackgroundSepsis is a serious complication that occurs after trauma, burns, and infections, and it is an important cause of death in intensive care unit (ICU) patients. Despite many new measures being proposed for sepsis treatment, its mortality rate remains high; sepsis has become a serious threat to human health, and there is an urgent need to carry out in-depth clinical research related to sepsis. In recent years, it has been found that septic shock-induced vasoplegia is a result of vascular hyporesponsiveness to vasopressors. Therefore, this study intended to establish an objective formula related to vasoplegia that can be used to assess the prognosis of patients and guide clinical treatment.Materials and MethodsA retrospective cohort study was conducted using data from 106 septic shock patients admitted to the ICU of Jining No. 1 People's Hospital from January 2020 to December 2022. The patients were divided into mortality and survival groups based on 28-day survival, and hemodynamics were monitored by the pulse index continuous cardiac output system. The dose and duration of vasopressors, major hemodynamic parameters, lactic acid (Lac) levels, and Sequential Organ Failure Assessment scores were recorded within 48 h of hospital admission. Multifactorial logistic regression was used to analyze the independent risk factors affecting the prognosis of patients, and the predictive value of the vascular response index (VRI) was analyzed by the receiver operating characteristic (ROC) curve.ResultsThe differences between the survival and mortality groups in terms of age, sex ratio, body weight, ICU length of stay, distribution of infection sites, underlying disease conditions, baseline Lac levels, and some hemodynamic parameters were not statistically significant (P > .05). The results of multifactorial logistic regression showed that the admission Acute Physiology and Chronic Health Evaluation II score, Lac level at 24 h of treatment, maximal vasoactive inotropic score at 24 h (VISmax24), maximal vasoactive inotropic score at 48 h (VISmax48), and VRI were independent risk factors affecting 28-day mortality. Within 48 h of receiving vasopressor therapy, the VRI was lower in the mortality group than in the survival group. The area under the ROC curve for the VRI was 0.86, and the best cutoff value of the VRI for predicting 28-day mortality was 32.50 (YI = 0.80), with a sensitivity of 0.90, a specificity of 0.90, and a better prediction of mortality than the other indicators.ConclusionsThe VRI is a good predictor of mortality in patients with septic shock, and a lower VRI indicates more severe vasoplegia, poorer prognosis, and higher mortality in patients with septic shock.

Assessment of Extravascular Lung Water Using Lung Ultrasound in Critically Ill Patients Admitted to Intensive Care Unit

Abstract:Background: Lung ultrasound (LUS) is a simple bedside tool to assess overhydration. Our study aimed to assess extravascular lung water (EVLW) using B-lines and correlate it with weaning, duration of mechanical ventilation, and mortality in critically ill patients admitted to the intensive care unit (ICU). Patients and methods: 150 mechanically ventilated ICU patients prospectively observed over 18 months, with their demographic and clinical data noted. Extravascular lung water was monitored using LUS in four intercostal spaces (ICS) from day 1 to day 5, day 7, day 10, and weekly thereafter. Pulmonary fluid burden was graded as low (1-10), moderate (11-20), and high (21-32). Weaning outcome, duration of weaning, mechanical ventilation, ICU stay, and mortality were compared in patients with and without EVLW. Results: Out of 150, 54 patients (36.0%) had EVLW. The mean lung score amongst our patients was 8.57 ± 6.0. The mean time for detection of EVLW was 1.43 ± 2.24 days. Lung score was low in 40 (26.67%) patients, moderate in 9 (6.00%) patients, and high in 5 (3.33%) patients. Incidence of weaning failure (p-value = 0.006), duration of weaning, mechanical ventilation, ICU stay (p-value < 0.0001 each), and overall mortality were significantly higher in patients with EVLW (p-value = 0.006). Conclusion: We conclude that a good proportion of critically ill patients have EVLW. Extravascular lung water significantly increases the duration of weaning, mechanical ventilation days, ICU stay, and overall mortality in critically ill patients.
