Abstract:OBJECTIVE: In adulthood, excess BMI is associated with cardiovascular disease (CVD); it is unknown whether risk differs by BMI trajectories from adolescence to adulthood.DESIGN AND METHODS: The National Longitudinal Study of Adolescent Health, a nationally representative, longitudinal adolescent cohort (mean age: 16.9 years) followed into adulthood (mean age: 28.8 years) [n = 13,984 individuals (41,982 observations)] was examined. Separate logistic regression models for diabetes, hypertension, and inflammation were used to examine odds of risk factors at given adult BMI according to varying BMI trajectories from adolescence to adulthood.RESULTS: CVD risk factor prevalence at follow-up ranged from 5.5% (diabetes) to 26.4% (hypertension) and 31.3% (inflammation); risk differed across BMI trajectories. For example, relative to men aged 27 years (BMI = 23 kg/m(2) maintained over full study period), odds for diabetes were comparatively higher for men of the same age and BMI ≈ 30 kg/m(2) with ≈8 BMI unit gain between 15 and 20 years (OR = 2.35; 95% CI, 1.51, 3.66) or in those who maintained BMI ≈ 30 kg/m(2) across the study period (OR = 2.33; 1.92, 2.83) relative to the same ≈8 BMI unit gain, but between 20 and 27 years (OR = 1.44; 1.10, 1.87).CONCLUSIONS: Specific periods and patterns of weight gain in the transition from adolescence to adulthood might be critical for CVD preventive efforts.

Longitudinal Changes of Cognition and Frailty with All-Cause and Cause-Specific Mortality in Chinese Older Adults: an 11-Year Cohort Study.

Abstract:Abstract Background and Objectives Physical function deterioration is always accompanied by a cognitive decline in older adults. However, evidence is lacking for the long-term simultaneous changing patterns of cognition and physical frailty and their associations with mortality among older adults. Research Design and Methods This study included 8,231 adults aged ≥65 with a baseline and at least one follow-up assessment of both cognition and physical frailty from the 2007–2018 Chinese Longitudinal Healthy Longevity Survey. Physical frailty (FRAIL phenotype) and cognition (Mini-Mental State Examination) were applied. Group-based joint trajectory modeling was used to fit the joint trajectories of cognition and physical frailty. Cox proportional hazards model was used to evaluate the trajectory-mortality associations. Results Three distinct joint trajectories were identified: no joint progression (34.4%), moderate joint progression (47.0%), and rapid joint progression (18.6%). During a median follow-up of 8.3 years, the rapid joint progression group, compared to the no joint progression, had the highest risk for all-cause mortality (hazard ratio (HR), 3.37 [95% CI: 2.99–3.81]), cardiovascular (CVD) mortality (3.21 [2.08–4.96]) and non-CVD mortality (2.99 [2.28–3.92]), respectively. Joint trajectory was found to be more predictive of mortality as compared to baseline measures of cognition and/or frailty (C-statistic ranged from 0.774 to 0.798). Higher changing rates of cognition and frailty were observed among all-cause decedents compared to CVD and non-CVD decedents over a 45-year span (aged 65–110) before death. Discussion and Implications Our study suggested that subjects with the worst cognitive decline and severest physical frailty progression were at the highest risk for all-cause and cause-specific mortality. Our findings expand the limited prior knowledge on the dynamic course of cognition and frailty.
Higher Cardiorespiratory Fitness During A Given Period and Lower Incidence of Type 2 Diabetes

Abstract:Increased cardiorespiratory fitness (CRF) is associated with decreased risk of type 2 diabetes, and time-dependent change in CRF is also associated with the incidence of type 2 diabetes. However, whether higher level of CRF during a prolonged period has a protective influence on the subsequent incidence of type 2 diabetes has not been reported. PURPOSE: To investigate the association between level of CRF during a given period and the incidence of type 2 diabetes among Japanese men. METHODS: We studied 7,169 Japanese men (age: 20-60 y) who were free from diabetes in 1986. Participants were given a submaximal exercise test at least four times between 1979 and 1986. The area under the curve (AUC) of CRF plotted against time (1979 to 1986) and standardized by individual measurement period because of varying measurement times. Men were then divided into quartiles based on the standardized AUC (AQ1, AQ2, AQ3, and AQ4) of CRF from 1979 to 1986. Participants were considered to develop diabetes during follow-up, 1986-2009, if they met one of the following criteria at least twice: 1) fasting blood glucose ≥ 126 mg/dL or HbA1c ≥ 6.1% (the Japan Diabetes Society criteria) or 2) self-reported diagnosis of diabetes by a physician and treatment with hypoglycemic medication, with the earliest year defined as the year of diabetes development. Hazard ratios (HRs) and 95% confidence intervals (CIs) for the incidence of diabetes were obtained using Cox proportional hazards models. RESULTS: During the follow-up period, 1,011 men developed diabetes. The crude HRs (CIs) for AQ2 through AQ4, using AQ1 as a reference, were 0.62 (0.53-0.72), 0.47 (0.40-0.55), and 0.29 (0.24-0.35), respectively (P for trend < 0.001). After the adjustment for potential confounding factors, the HRs (CIs) for AQ2 through AQ4 were 0.79 (0.67-0.93), 0.71 (0.58-0.86), and 0.51 (0.39-0.66), respectively (P for trend < 0.001). CONCLUSIONS: Higher levels of an integrated measure of CRF over the long-term period has a protective influence on the subsequent incidence of type 2 diabetes among Japanese men.

Abstract:There has only been one study examining the effect of cardiorespiratory fitness (CRF) on blood glucose with aging among Americans. Whether CRF also has a similar effect on aging-associated change in blood glucose among Japanese with have lower prevalence of obesity as compared with Americans. PURPOSE: To investigate whether the aging-related increase of glucose in Japanese men with higher CRF is smaller than that of those with lower CRF. METHODS: We studied 6,153 Japanese men (age: 20-60 y) free of diabetes, cardiovascular disease and stroke in 1986. The participants completed annual health examinations including fasting blood glucose until 2009. CRF was measured by a submaximal exercise test between 1979 and 1986. Participants were divided into quartiles (Q1, Q2, Q3, and Q4) for CRF based on the cumulative mean of CRF during the period. Time-invariant covariates included BMI, smoking status, drinking habit, desk work, family history of diabetes, and frequency of measurements of blood glucose, and time-variant covariates included systolic blood pressure and fasting blood triglyceride. The trajectories of glucose with aging were analyzed using linear mixed models. RESULTS: Fasting glucose increased at a linear rate with aging. Glucose increased at a yearly rate of 0.49 mg/dL (95% confidence interval: 0.48-0.50). After adjustment for covariates, the aging-related glucose increase in men with Q4 of CRF (0.42 [0.40-0.44] mg/dL per year) was smaller than that in men with Q1 (0.55 mg/dL [0.50-0.60] per year), Q2 (0.51 mg/dL [0.46-0.56] per year), Q3 (0.48 mg/dL [0.42-0.53] per year), respectively. CONCLUSIONS: Higher levels of CRF is inversely associated with the increase in blood glucose with aging among Japanese healthy men.

Abstract:Background: Obesity- and hypertension-related cardiovascular (CV) risk has been shown to originate in childhood. Higher body mass index (BMI) and blood pressure (BP) have been associated with increased large artery stiffness and a lower microvascular arteriolar-to-venular diameter ratio (AVR) in children. This study aimed to investigate the association of cardiorespiratory fitness (CRF) with development of BMI, BP and vascular health during childhood. Methods: In our prospective cohort study, 1,171 children aged 6–8 years were screened for CRF, BMI, BP, retinal vessel diameters and pulse wave velocity using standardized protocols. Endurance capacity was assessed by 20 m shuttle run test. After 4 years, all parameters were assessed in 664 children using the same protocols. Results: Children with a higher CRF at baseline developed a significantly lower BMI (β [95% CI] −0.09 [−0.11 to −0.06] kg/m 2 , p < 0.001), a lower systolic BP (β [95% CI] −0.09 [−0.15 to −0.03] mmHg, p = 0.004) and a higher AVR (β [95% CI] 0.0004 [0.00004 to 0.0007] units, p = 0.027) after 4 years. The indirect association of CRF with development of retinal arteriolar diameters was mediated by changes in BMI. Conclusion: Our results identify CRF as a key modulator for the risk trajectories of BMI, BP and microvascular health in children. Obesity-related CV risk has been shown to track into adulthood, and achieving higher CRF levels in children may help counteract the development of CV risk and disease not only in pediatric populations, but may also help reduce the burden of CVD in adulthood. Abstract:Parents sometimes eat their young to reduce the consequences of brood overcrowding, for nutritional gain, and/or to redirect investment toward future reproduction. It has been predicted that filial cannibalism should be more prevalent when mate availability is high as parents can more easily replace consumed young. Reviewing the available evidence—which comes almost exclusively from studies of paternal caring fish—we find support in some species, but not others. To explain this, we hypothesize that sexual selection against filial cannibalism and/or the tendency to acquire larger broods under conditions of high mate availability discourages filial cannibalism. Additionally, filial cannibalism might occur when mate availability is low to facilitate survival until access to mates improves. Since attractiveness can also influence remating opportunities, we review its effect on filial cannibalism, finding that attractive parents engage in less filial cannibalism. More research is needed to determine if this relationship is a result of individuals showing adaptive plasticity in filial cannibalism based on self-perceived attractiveness, or if the attractiveness of individuals is reduced by their propensity to commit filial cannibalism. More generally, to advance our understanding of how mate availability influences filial cannibalism, future studies should also focus on a wider range of taxa.

Abstract:Abstract Background Studies on the association between estimated cardiorespiratory fitness (CRF) and changes in metabolic risk in the Chinese population are limited. This study aims to examine the associations between CRF and changes in metabolic risk. Subjects and methods We included 4,862 and 2,700 participants recruited from 28 provinces in the China Health and Retirement Longitudinal Study (CHARLS) in the baseline (Wave 1) and follow-up (Wave 4) analyses, respectively. CRF was calculated using sex-specific longitudinal non-exercise equations. Metabolic indicators included systolic blood pressure (SBP), diastolic blood pressure (DBP), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and fasting plasma glucose (FPG) levels. The metabolic score was calculated as the number of changes in the above metabolic indicators above the 75th percentile of the distribution of changes (equal to or below the 25th percentile for HDL-C). Results In the baseline analysis, CRF was negatively associated with SBP, DBP, TG, and FPG, and positively correlated with HDL-C after adjusting for age, smoking status, and drinking status (all P < 0.0001) in both males and females. In the follow-up analysis, higher baseline CRF was significantly related to a decrease in SBP, DBP, TG, FPG, and metabolic score (all P < 0.0005), and increased HDL-C ( P < 0.0001) after further adjustment for corresponding baseline metabolic indicators. The associations remained significant after stratification by sex, except for the changes in HDL-C levels in females. Furthermore, improved CRF was associated with favorable changes in DBP, TG, HDL-C, FPG, and metabolic scores in all populations and males. Significant associations between changes in CRF and DBP, TG, and FPG levels were found in females. Conclusion Higher baseline CRF and improved CRF were associated with favorable changes in metabolic indicators.

Abstract:Purpose: Although the benefit of high cardiorespiratory fitness (CRF) for the prevention of type 2 diabetes mellitus (T2DM) is widely accepted, whether consistently high CRF is necessary or transiently high CRF is sufficient is unclear. The present study was conducted to examine the hypothesis that consistently high level of CRF is more beneficial than transiently high CRF for the prevention of T2DM. Methods: This cohort study was conducted in nondiabetic 7158 men age 20 to 60 yr, enrolled from 1986 to 1987. The area under the curve with respect to ground (AUC(G)) for CRF measurements during an 8-yr measurement period (1979-1987) was calculated as an index of integrated CRF level during the period. The differences (Delta AUC(P)) between AUC(G) and peak AUC (peak CRF-measurement period) was also calculated as an index of the presence and the size of a "spike'' in CRF. T2DM was defined by fasting blood glucose and a self-reported diagnosis of diabetes for participants with blood tests. For participants without blood tests, T2DM was defined by the result of oral glucose test after a nonfasting urinary test and a self-reported diagnosis of diabetes. T2DM was determined on health checkups until 2009. Results: During the follow-up period, 1495 men developed T2DM. After adjustment for confounders, as compared with the first quartile of AUC(G) for CRF, the hazard ratio (95% confidence interval) for the second, third, and fourth quartiles were 0.87 (0.76 to 1.00), 0.80 (0.68 to 0.95), and 0.72 (0.58 to 0.89), respectively. For CRF spike, there was no association between Delta AUC(P) in CRF and the incidence of T2DM. Conclusions: Consistently higher level of CRF over time was associated with lower risk of T2DM.

Abstract:Introduction The aim of this prospective cohort study was to investigate the associations between changes in cardiorespiratory fitness (CRF) and the risk of colorectal and prostate cancer in men. Methods Data from men who completed a health assessment both in military conscription in youth and an occupational health profile assessment (HPA) later in life were used. CRF was assessed as estimated V̇O2max, using a cycle ergometer fitness test at both time points. We linked the assessment data to national register data on colorectal and prostate cancer incidence, and hazard ratios and confidence intervals were estimated using Cox proportional hazard regression. Results 139,764 men with a mean age of 18 (SD 0.6) at conscription and 43 (SD 8.9) at HPA were included. The average time between the two assessments was 25.9 (SD 9.0) years and mean follow‐up time following HPA test was 10.0 (SD 5.6) years for prostate and colorectal cancer. Annual percentage change in relative and absolute V̇O2max from conscription to HPA was inversely associated with the risk of colorectal cancer incidence, hazard ratio of 0.83 (95% CI [0.72–0.94]) and 0.88 (95% CI [0.79–0.99]), respectively. These associations were driven by data from individuals in the lowest and moderate level fitness tertials at conscription. Change in CRF was not associated with prostate cancer incidence risk. Conclusions Changes in, not only level of, CRF from youth to adulthood are related to colorectal cancer incidence risk and therefore, improving CRF should be considered as an important colorectal cancer risk reduction strategy.

Abstract:Background: The "Physical Activity Reference for Health Promotion 2013" provides "fit" reference values for cardiorespiratory fitness (CRF) for good health. The importance of achieving a fit CRF level for several years on the subsequent prevention of type 2 diabetes mellitus (T2DM) remains to be clarified. Methods: This cohort study was conducted in 2,235 nondiabetic males aged 21 to 59 years, enrolled in April 1986 through March 1987. We calculated the ratio of the area under the curve (AUC(ratio)) for actual measured values and the AUC for the reference values of CRF in each individual during an 8-year measurement period before the baseline. According to whether they met a fit CRF level or not, participants were categorized into groups based on the AUCratio (Fit(AUC) or Unfit(AUC)) and initial CRF (Fit(initial) or Unfit(initial)), respectively. T2DM was evaluated on health checkups until March 2010. Results: During the follow-up period, 400 men developed T2DM. After adjustment for confounders, as compared with those in the Fit(AUC) group, the hazard ratio (HR) for those in the Unfit(AUC) group was 1.33 (95% confidence interval [CI], 1.06-1.65). A combined analysis with the categories of initial value and AUC(ratio) showed that, compared with the Fit(initial) and Fit(AUC) group, the HRs of Fit(initial) and Unfit(AUC), Unfit(initial) and Fit(AUC), and Unfit(initial) and Unfit(AUC) groups were 1.41 (95% CI, 0.99-2.00), 1.18 (95% CI, 0.81-1.70), and 1.40 (95% CI, 1.08-1.83), respectively. Conclusion: Achievement of a fit CRF level established in the Japan physical activity guideline for several years was associated with lower subsequent risk of T2DM.
Association between change in cardiorespiratory fitness and prostate cancer incidence and mortality in 57 652 Swedish men

Abstract:Objectives To examine the associations between changes in cardiorespiratory fitness (CRF) in adulthood and prostate cancer incidence and mortality. Methods In this prospective study, men who completed an occupational health profile assessment including at least two valid submaximal CRF tests, performed on a cycle ergometer, were included in the study. Data on prostate cancer incidence and mortality were derived from national registers. HRs and CIs were calculated using Cox proportional hazard regression with inverse probability treatment weights of time-varying covariates. Results During a mean follow-up time of 6.7 years (SD 4.9), 592 (1%) of the 57 652 men were diagnosed with prostate cancer, and 46 (0.08%) died with prostate cancer as the primary cause of death. An increase in absolute CRF (as % of L/min) was associated with a reduced risk of prostate cancer incidence (HR 0.98, 95% CI 0.96 to 0.99) but not mortality, in the fully adjusted model. When participants were grouped as having increased (+3%), stable (±3%) or decreased (−3%) CRF, those with increased fitness also had a reduced risk of prostate cancer incidence compared with those with decreased fitness (HR 0.65, 95% CI 0.49 to 0.86), in the fully adjusted model. Conclusion In this study of employed Swedish men, change in CRF was inversely associated with risk of prostate cancer incidence, but not mortality. Change in CRF appears to be important for reducing the risk of prostate cancer.

Abstract:CLINICAL CASE We report the case of an 81-day-old female infant who was brought to the Emergency Department because of a seizure. At 20 weeks of gestational age she was diagnosed to have agenesis of the corpus callosum, with this being confirmed later by magnetic resonance imaging. Ophthalmological examination of the fundus showed peripapillar chorioretinal lesions (lacunar chorioretinopathy) in both eyes. DISCUSSION Chorioretinal lacunar and retinal pigment epithelial abnormalities are the basis for the diagnosis of this syndrome.
Submaximal fitness and mortality risk reduction in coronary heart disease: a retrospective cohort study of community-based exercise rehabilitation

Abstract:OBJECTIVES: To examine the association between submaximal cardiorespiratory fitness (sCRF) and all-cause mortality in a cardiac rehabilitation (CR) cohort.DESIGN: Retrospective cohort study of participants entering CR between 26 May 1993 and 16 October 2006, followed up to 1 November 2013 (median 14 years, range 1.2-19.4 years).SETTING: A community-based CR exercise programme in Leeds, West Yorkshire, UK.PARTICIPANTS: A cohort of 534 men (76%) and 136 women with a clinical diagnosis of coronary heart disease (CHD), aged 22-82 years, attending CR were evaluated for the association between baseline sCRF and all-cause mortality. 416 participants with an exercise test following CR (median 14 weeks) were examined for changes in sCRF and all-cause mortality.MAIN OUTCOME MEASURES: All-cause mortality and change in sCRF expressed in estimated metabolic equivalents (METs).RESULTS: Baseline sCRF was a strong predictor of all-cause mortality; compared to the lowest sCRF group (<5 METs for women and <6 METs for men), mortality risk was 41% lower in those with moderate sCRF (HR 0.59; 95% CI 0.42 to 0.83) and 60% lower (HR 0.40; 95% CI 0.25 to 0.64) in those with higher sCRF levels (≥7 METs women and ≥8 METs for men). Although improvement in sCRF at 14 weeks was not associated with a significant mortality risk reduction (HR 0.91; 95% CI 0.79 to 1.06) for the whole cohort, in those with the lowest sCRF (and highest all-cause mortality) at baseline, each 1-MET improvement was associated with a 27% age-adjusted reduction in mortality risk (HR 0.73; 95% CI 0.57 to 0.94).CONCLUSIONS: Higher baseline sCRF is associated with a reduced risk of all-cause mortality over 14 years in adults with CHD. Improving fitness through exercise-based CR is associated with significant risk reduction for the least fit.

Association of estimated cardiorespiratory fitness in middle-aged and elderly people with cardiovascular disease: Evidence from the China health and retirement longitudinal study

Abstract:Background and aim: Cardiovascular diseases (CVD) is a major threat to public health, while cardiorespiratory fitness (CRF) is a key predictor of chronic disease. Given this, the purpose of this study was to investigate the relationship between estimated CRF (eCRF) and CVD in middle-aged and elderly Chinese people. Methods and results: The China Health and Retirement Longitudinal Study (CHARLS) with 4761 individuals were included in analysis. Participants were divided into three groups according to eCRF quantile in sex subgroups. Cox proportional hazards regression models were used to explore the correlation of eCRF with CVD (stroke or cardiac events). In total, 4761 participants were included in this cohort study (2500 [52.51%] women). During a 7-year follow-up from 2011 to 2018, 796 CVDs (268 Strokes and 588 cardiac events) were recorded. In multivariable-adjusted analyses, for per 1 SD increase of eCRF, the age-adjusted risk of CVD was reduced by about 18% (HR = 0.82; 95% CI, 0.72-0.93) in men, and was reduced by about 29% (HR = 0.71; 95% CI, 0.62-0.81) in women. Similar associations were also found between eCRF and stroke and cardiac events. Both subgroup and interaction analyses showed that the interaction of age had a statistically significant effect on CVD risk. Conclusion: ECRF was inversely associated with CVD risk (stroke or cardiac events) in both men and women. Remarkable sex and age differences exist in the effectiveness of increasing eCRF to reduce the risk of CVD. As a potential, efficient and cost-effective risk prediction tool, eCRF deserves further attention and wide application.

Abstract:Background Cardiorespiratory fitness (CRF) could reduce the risk of metabolic syndrome (MetS) while the association between muscular endurance capacity (MEC) and incident MetS has rarely been investigated in young adults. Methods A total of 2890 military men and women, aged 18–39 years, free of baseline MetS in Taiwan, were followed for incident MetS from baseline (2014) until the end of 2020. All subjects received annual health examinations for assessment of MetS. Physical fitness was assessed by CRF (estimated maximal oxygen uptake, VO2 max [mL/kg/min], in a 3000‐m run) and MEC (numbers of 2‐min push‐ups). MetS was defined according to the International Diabetes Federation (IDF) criteria. Multiple Cox regression analysis was conducted with adjustments for baseline age, sex, substance use status and physical activity to determine the associations of CRF and MEC with incidences of new‐onset MetS and related features, for example, central obesity, hypertension, dyslipidaemia and prediabetes or diabetes. To examine the combined effects of CRF and MEC status on incidence of MetS, high and low levels of CRF and MEC were separately defined by over and under the sex‐specific median in each exercise test. Results During a median follow‐up of 5.8 years, there were 673 (23.3%) new‐onset MetS. Higher CRF was associated with a lower incidence of MetS (hazard ratio [HR] and 95% confidence interval: 0.905 [0.877–0.933]), and its components separately, except hypertension. No association was observed between MEC and incident MetS, and its components separately, except hypertension. When evaluating the combined effects of MEC and CRF status on the incidence of MetS, it was observed that compared with the low CRF/low MEC, the high CRF/high MEC (HR: 0.553 [0.439–0.697]) and the high CRF/low MEC (HR: 0.730 [0.580–0.918]) had a lower incidence of new‐onset MetS (P value for the intergroup difference = 0.04). There was no significant result for the low CRF/high MEC. Conclusions This study highlights that although the protective effects of MEC to reduce the incidence of MetS and most of its related features were mainly driven by CRF in young adults, there was an addictive effect of greater MEC on CRF to prevent the development of new‐onset MetS before midlife.

Cardiorespiratory Fitness and Pericardial Adipose Tissue: Coronary Artery Risk Development in Young Adults (cardia) Study

Abstract:PURPOSE: Pericardial adipose tissue (PAT), an ectopic adiposity surrounding the coronary arteries, is a pathogenic risk factor for cardiometabolic disease; however, the association of cardiorespiratory fitness (CRF) with PAT is poorly understood. We examined the longitudinal association of 20-year trajectories of CRF with prospectively measured PAT. METHODS: Young adults (N = 1,877, mean age = 25.1 years, 57.3% female, 43.0% blacks) from the CARDIA Study were included. Treadmill exercise test duration (sec) was measured at baseline (1985-1986, ages 18-30), exam years 7 (1992-1993, ages 25-37), and 20 (2005-2006, ages 38-50) using a symptom-limited maximal treadmill exercise test. PAT volume (mL) was quantified at year 25 (2010-2011, ages 43-55) using computed tomography. Covariates measured at baseline, year 7, or year 20 included sociodemographic factors, cardiovascular disease risk factors, including diet quality score and self-reported moderate-to-vigorous intensity physical activity, inflammatory markers, and waist circumference. Latent trajectory analysis was used to characterize different CRF patterns over exam years. Analysis of covariance was used to examine the least squares means of PAT across trajectory groups. RESULTS: Latent trajectory analysis identified three different CRF trajectories (Figure 1), each declining with age at different starting CRF levels: low fit (N = 652, 34.7%); moderate fit (N = 811, 42.2%); and high fit (N = 426, 23.1%). In the fully adjusted model, PAT decreased across increasing trajectory groups (low fit → high fit) from 59.3 mL to 53.4 mL (p for trend = 0.015; Figure 1). CONCLUSIONS: Although all three trajectory groups reflected age-related declines in CRF, high CRF was associated with lower levels of PAT measured prospectively. Maintaining high levels of CRF may be associated with less PAT accumulation with age in early to middle adulthood.
Individual and interpersonal correlates of cardiorespiratory fitness in adults – Findings from the German Health Interview and Examination Survey

Abstract:Abstract Cardiorespiratory fitness (CRF) is an established predictor of adverse health outcomes. The aim of this study is to investigate potential behavioral, interpersonal and socioeconomic correlates of CRF among men and women living in Germany using data from a population-based nationwide cross-sectional study. 1,439 men and 1,486 women aged 18–64 participated in the German Health Interview and Examination Survey (2008–2011) and completed a standardized sub-maximal cycle ergometer test. Maximal oxygen consumption ( $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x ) in ml·min −1 ·kg −1 was estimated. Mean values of VO 2 max for various anthropometric, behavioral, interpersonal, and sociodemographic variables were estimated. Linear regression analyses using multiple imputations technique for missing values was performed to analyze the influence of potential correlates on CRF. Women with high alcohol consumption had higher $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x , ( β = 2.20; 95% CI 0.98 to 3.42) than women with low alcohol consumption and women with high occupational status had higher $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x ( β = 1.83; 95% CI 0.21 to 3.44) in comparison to women with low occupational status. Among men, high fruit intake ( β = 1.52; 95% CI 0.63 to 2.40), compared to low or medium fruit intake and performing at least 2.5 hours of total PA per week (β = 2.19; 95% CI 1.11 to 3.28), compared to less than 2.5 hours was associated with higher $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x . Among both men and women, lower body mass index, lower waist circumference and higher levels of physical exercise were considerably associated with higher $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x . Among women, those in higher age groups showed a considerably lower level of $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x compared with those aged 18–24. Furthermore, mean estimated $$\dot{V}{O}_{2}max$$ V ̇ O 2 m a x was higher among men (36.5; 95% CI 36.0 to 37.0) than among women (30.3; 95% CI 29.8 to 30.7). Despite the cross-sectional nature of the current study, we conclude that several behavioral, anthropometric, and sociodemographic factors are associated with CRF in the general adult population in Germany. These results can provide evidence to tailor prevention measures according to the needs of specific subgroups.

Abstract:Background: Lifestyles generally change across the life course yet no prospective study has examined direct associations between healthy lifestyle trajectories and subsequent cardiovascular disease (CVD) or all-cause mortality risk. Methods: Healthy lifestyle score trajectories during 2006–2007, 2008–2009, and 2010–2011 were collated through latent mixture modeling. An age-scale based Cox proportional hazard regression model was implemented to calculate hazard ratios (HR) with corresponding 95% confidence intervals (CI) for developing CVD or all-cause mortality across healthy lifestyle trajectories. Results: 52,248 participants were included with four distinct trajectories identified according to healthy lifestyle scores over 6 years i.e., low-stable ( n = 11,248), high-decreasing ( n = 7,374), low-increasing ( n = 7,828), and high-stable ( n = 25,799). Compared with the low-stable trajectory, the high-stable trajectory negatively correlated with lower subsequent risk of developing CVD (HR, 0.73; 95% CI, 0.65–0.81), especially stroke (HR, 0.70; 95% CI, 0.62–0.79), and all-cause mortality (HR, 0.89; 95% CI, 0.80–0.99) under a multivariable-adjusted model. A protective effect for CVD events was observed only in men and in those without diabetes, while a reduced risk of all-cause mortality was observed only in those older than 60 years, though interactions were not statistically significant. Marginally significant interactions were observed between the changing body mass index (BMI) group, healthy lifestyle score trajectories and stratified analysis. This highlighted an inverse correlation between the high-stable trajectory and CVD in BMI decreased and stable participants as well as all-cause mortality in the stable BMI group. The low-increasing trajectory also had reduced risk of CVD only when BMI decreased and in all-cause mortality only when BMI was stable. Conclusions: Maintaining a healthy lifestyle over 6 years corresponds with a 27% lower risk of CVD and an 11% lower risk in all-cause mortality, compared with those engaging in a consistently unhealthy lifestyle. The benefit of improving lifestyle could be gained only after BMI change is considered further. This study provides further evidence from China around maintaining/improving healthy lifestyles to prevent CVD and early death.
