Abstract:Purpose: To develop a 3D multitasking multi-echo (MT-ME) technique for the comprehensive characterization of liver tissues with 5-min free-breathing acquisition; whole-liver coverage; a spatial resolution of 1.5 x 1.5 x 6 mm(3); and simultaneous quantification of T-1, water-specific T-1 (T-1w), proton density fat fraction (PDFF), and R-2*. Methods: Six-echo bipolar spoiled gradient echo readouts following inversion recovery preparation was performed to generate T-1, water/fat, and R-2* contrast. MR multitasking was used to reconstruct the MT-ME images with 3 spatial dimensions: 1 T-1 recovery dimension, 1 multi-echo dimension, and 1 respiratory dimension. A basis function-based approach was developed for T-1w quantification, followed by the estimation of R-2* and T-1-corrected PDFF. The intrasession repeatability and agreement against references of MT-ME measurements were tested on a phantom and 15 clinically healthy subjects. In addition, 4 patients with confirmed liver diseases were recruited, and the agreement between MT-ME measurements and references was assessed. Results: MT-ME produced high-quality, coregistered T-1, T-1w, PDFF, and R-2* maps with good intrasession repeatability and substantial agreement with references on phantom and human studies. The intra-class coefficients of T-1, T-1w, PDFF, and R-2* from the repeat MT-ME measurements on clinically healthy subjects were 0.989, 0.990, 0.999, and 0.988, respectively. The intra-class coefficients of T-1, PDFF, and in healthy subjects and 0.980, 0.999, and 0.998 in patients. The T-1w was independent to PDFF (R = -0.029, P = .904). Conclusion: The proposed MT-ME technique quantifies T-1, T-1w, PDFF, and R-2* simultaneously and is clinically promising for the comprehensive characterization of liver tissue properties.
3D Isotropic Resolution Diffusion-Prepared Magnitude-Stabilized Bssfp Imaging with High Geometric Fidelity at 1.5 Tesla

Abstract:IntroductionMRI has been increasingly used in radiation therapy to facilitate tumor and organ delineation and assess treatment response. Diffusion MRI can provide cellularity information and may enable functional‐based treatment planning and adaptation. However, strong distortion associated with the conventional diffusion‐weighted single‐shot echo‐planar imaging (DW‐ssEPI) sequence is problematic for accurate target delineation. The goal of this work is to propose a 3D diffusion sequence with minimal distortion for radiation therapy applications.MethodsA 3D diffusion‐prepared magnitude‐stabilized balanced steady‐state free precession sequence (DP‐MS‐bSSFP) was developed. A 2D navigator was acquired during the linear catalyzation stage of the bSSFP readout to estimate the phase, which was then used in a plane‐by‐plane low‐rank constrained reconstruction to correct the shot‐to‐shot k‐space inconsistency. A diffusion phantom was scanned to evaluate and compare the geometric reliability and apparent diffusion coefficient (ADC) accuracy with the conventional DW‐ssEPI. Eight landmarks were selected on each slice of the images to calculate the target registration error (TRE), which was used as a surrogate for geometric fidelity. The phantom was scanned under both 0℃ and room temperature. Brain scans were performed on five healthy volunteers. In the first volunteer, protocols of 1, 2, and 4 shots per Kz plane were compared. In vivo geometric fidelity and ADC accuracy were evaluated on the remaining four volunteers using the protocol of four shots per Kz plane. In the geometric fidelity study, 8–10 landmarks were picked on each slice to calculate the TRE. Regions of interest were placed on the white matter, the cerebellum, and the cerebrospinal fluid region to evaluate the ADC agreement between DW‐ssEPI and DP‐MS using the Bland‐Altman plot. All scans were performed at 1.5 mm isotropic resolution to meet the high‐resolution requirement of many radiotherapy applications.ResultsThe DP‐MS had drastically improved geometric accuracy compared with DW‐ssEPI on the phantom. The mean TRE decreased from 2.09 mm to 0.70 mm. The percentage difference of the ADC values between the two diffusion sequences were less than 5.5% and 7% for the 0℃ and room temperature study, respectively. The DW‐ssEPI had strong distortion and susceptibility‐related artifacts at tissue air boundary, whereas distortion was minimal in DP‐MS images. Overall, the mean/max TRE was over 2 mm/7 mm in the volunteers for DW‐ssEPI, whereas less than 0.8 mm/2 mm for DP‐MS. Good ADC agreement was observed for the white matter, the cerebellum, and the CSF based on the Bland‐Altman plots.ConclusionA 3D diffusion sequence was developed and validated. It provided high‐resolution diffusion imaging with mean distortion less than 1 mm at 1.5 T, and is a promising imaging technique for treatment planning and adaptive radiotherapy.

Abstract:PurposeDiffusion time (Delta) effect in diffusion measurements has been validated as a sensitive biomarker in liver fibrosis by rat models. To extend this finding to clinical study, a reliable imaging technique is highly desirable. This study aimed to develop an optimal stimulated echo acquisition mode (STEAM) diffusion-weighted imaging (DWI) method dedicated to human liver imaging on 3 Tesla (T) and preliminarily investigate the dependence effect in healthy volunteers.MethodsSTEAM DWI with single-shot echo planar imaging readout was used as it provided better signal-to-noise ratio (SNR) than spin echo DWI methods when a long was needed for liver imaging. Additionally, a slice-selection gradient reversal method was used for fat suppression. Motion compensation and SNR improvement strategies were used to further improve the image quality. Five b-values with three s were tested in 10 volunteers.ResultsEffective fat suppression and motion compensation were reproducibly achieved in the optimized sequence. The signal decay generally became slower when the s increased. Obvious reduction of diffusion coefficients was observed with increasing s in the liver.ConclusionThe results verified the dependence in diffusion measurements, indicating restricted diffusion in healthy human livers for the first time at 3T. Diffusion-weighted Imaging and Variable Flip Angle T1 Mapping: a Supplement for Image-Guided Biopsy in Follow-Up Analysis of Liver Fibrosis

Abstract:Purpose To evaluate the performance of diffusion-weighted imaging (DWI) and variable flip angle (VFA) T1 mapping as a supplement to image-guided biopsy in follow-up analysis of liver fibrosis.Materials and Methods This prospective study was approved by the institution's committee on human research,and written informed consent was provided from the enrolled patients.We investigated five MRI parameters of DWI and VFA T1 mapping,collected from 11 patients who underwent serial ultrasound image-guided biopsy with follow-up MRI within 1.5 years after treatment for liver fibrosis/cirrhosis.For each patient,four consecutive MRI examinations were conducted,including baseline MRI before treatment and three follow-up MRI examinations after treatment at each 0.5-year interval.ADC values at four b values and T1 relaxation times were correlated to pathology-confirmed liver fibrosis stages,which were subsequently divided into two groups,stages F2-3 and F4.The receiver operating characteristic (ROC) analysis and repeated measurement analysis of variance were used for statistical analysis.Results Among these ADC parameters,ADC value (b =500 s/mm2) was the most consistent in differentiating between stage F2-3 and F4 liver fibrosis.Repeated measurement analysis showed that the intra-group and inter-group differences were 0.447 and 0.024,respectively.T1 relaxation time could not consistently differentiate between the F2-3 and F4 groups;however,it was repeatable,and the intra-group and inter-group differences were 0.410 and 0.042,respectively.Conclusion MRI-ADC value at a b value of 500 s/mm2 can be a promising biomarker for differentiating stages F2-3 and F4 liver fibrosis.A combination of this biomarker with repeatable T1 relaxation time may function as a non-invasive tool for follow-up liver fibrosis in patients who reject repeated image-guided biopsy.

Abstract:We propose liver-BUDA-SAGE (liver Blip Up-Down Acquisition for Spin And Gradient Echo imaging), an efficient echo-planar imaging (EPI) sequence to rapidly obtain high-resolution T 2 and $T_2^{\ast}$ quantitative mapping of the human liver at 3.0 Tesla (T) with high-coverage and high geometric fidelity. This multi-shot navigator-free acquisition includes multiple $T_{{2^ - }}^{\ast},T_{{2^ - }}^\prime$ and T 2 -weighted contrasts with alternating phase-encoding polarities across the interleaved shots. A field map estimated from interim reconstructions was incorporated into the joint multi-shot EPI reconstruction with a structured low-rank constraint to eliminate geometric distortion. An adapted quantitative multi-parametric mapping method was established using a combined least-square solution and Bloch dictionary matching to obtain maps based on the re-constructed echoes. In-vivo results demonstrate the ability of liver-BUDA-SAGE to provide high-coverage, distortion-free, multi-contrast images and quantitative ${T_2}/T_2^{\ast}$ maps at 1.5×1.5 mm 2 in-plane resolution in ~20s for 16 slices. Estimated quantitative maps showed good agreement in phantom compared to conventional mapping methods with slight biases of 2.6 ms and -0.2 ms for T 2 and $T_2^{\ast}$. Measured in-vivo liver values (mean±SD) for T 2 and $T_2^{\ast}$ were 21.26±2.37 ms and 30.59±3.70 ms which are consistent with those reported in the literature.
Comparative Study of Diffusion-weighted Imaging of the Normal Liver Parenchyma at 3.0 Tesla and 1.5 Tesla MRI

Abstract:Purpose:To compare breath-hold diffusion-weighted imaging(DWI) of normal liver parenchyma between 1.5T and 3.0T in terms of image quality and the apparent diffusion coefficient(ADC) measurement in the same patients.Methods:DW images at both 1.5T and 3.0T in the same 16 patients were analyzed.The b factors of 0,500s/mm~2 were used.Image quality was assessed on a 3-point scale.The signal-to -noise ratios(SNRs) of left lobe and right lobe,contrast-to-noise ratios(CNRs) of left lobe-muscle and right lobe-muscle,and the ADC values were calculated and compared statistically.Results:DWI at 1.5T showed better image quality when compared with that at 3.0T(2.60 vs.2.20,P=0.0312).The SNRs of left lobe and right lobe were significantly higher at 1.5T than that at 3.0T(47.9 vs.32.8,P=0.0018 for left lobe,46.2 vs. 27.4,P=0.0004 for right lobe).The CNRs of left lobe- muscle and right lobe-muscle were slightly higher at 1.5T than that at 3.0T,and the ADC values of left lobe and right lobe were slightly lower at 1.5T than that at 3.0T.No statistical significant differences were noted in CNRs and ADC values of normal liver(P0.05). Conclusion:Compared with 1.5T,3.0T MR-DWI provided the similar ADC values of the normal liver parenchyma,however,was with worse image quality.

Abstract:We compared the clinical utility of single-shot echo-planar imaging (SS-EPI) using different breathing schemes, readout-segmented EPI and zoomit EPI in the repeatability of apparent diffusion coefficient (ADC) measurements, cortico-medullary contrast to noise ratio (c-mCNR) and image quality. In this institutional review board-approved prospective study, some common clinically applicable diffusion-weighted imaging (b = 50, 400, 800 s/mm 2 ) of kidney on 3.0 T MRI were performed on 22 volunteers using SS-EPI with breath-hold diffusion-weighted imaging (BH-DWI), free-breathing (FB-DWI), navigator-triggered (NT-DWI) and respiratory-triggered (RT-DWI), readout-segmented DWI (RS-DWI), and Zoomit DWI (Z-DWI). ADC and c-mCNR were measured in 12 anatomic locations (the upper, middle, and lower pole of the renal cortex and medulla), and image quality was assessed on these DWI sequences. A DWI with the optimal clinical utility was decided by systematically assessing the ADC repeatability, c-mCNR and image quality among the DWIs. For ADC measurements, Z-DWI had an excellent intra-observer agreement (intra-class correlation coefficients (ICCs): 0.876–0.944) and good inter-observer agreement (inter-class ICCs: 0.798–0.856) in six DWI sequences. Z-DWI had the highest ADC repeatability in most of the 12 anatomic locations of the kidneys (mean ADC absolute difference: 0.070–0.111 × 10 −3 mm 2 /s, limit of agreement: 0.031–0.056 × 10 −3 mm 2 /s). In all DWIs, Z-DWI yielded a slightly higher c-mCNR than other DWIs in most representative locations ( P > 0.05), which was significantly higher than BH-DWI and FB-DWI in the middle pole of both kidneys and the upper pole of the left kidney ( P < 0.05). In addition, Z-DWI yielded image quality that was similar to RT-DWI and NT-DWI ( P > 0.05) and superior to BH-DWI, FB-DWI and RS-DWI ( P < 0.05). Our results suggest that Z-DWI provides the highest ADC reproducibility, better c-mCNR and good image quality on 3.0 T MRI, making it the recommended sequence for clinical DWI of the kidney.

Abstract:Objectives: To investigate and optimize diffusion-weighted imaging (DWI) acquisitions for pancreatic cancer at 3.0 T.Methods: Forty-five patients with pancreatic cancer were examined by four DWI acquisitions with b values = 0 and 600 s/mm(2) at 3.0 T, including breath-holding DWI (BH-DWI), respiratory-triggered DWI (TRIG-DWI), respiratory-triggered DWI with inversion-recovery technique (TRIGIR-DWI), and free-breathing DWI with inversion-recovery technique (FBIR-DWI). Artifacts, contrast ratio (CR), contrast-to-noise ratio (CNR) and apparent diffusion coefficient (ADC) of pancreatic cancer were statistically compared among DWI acquisitions.Results: TRIGIR-DWI displayed the lowest artifacts and highest CR compared to other DWI acquisitions. CNRs of pancreatic cancer in TRIG-DWI and TRIGIR-DWI were statistically higher than that in FBIR-DWI and BH-DWI. Different ADCs between pancreatic cancer and noncancerous pancreatic tissues were noticed by a paired-samples T test in TRIG-DWI (p = 0.017), TRIGIR-DWI (p = 0.00001) and FBIR-DWI (p = 0.000041).Conclusions: TRIGIR-DWI may be the optimal acquisition of DWI for pancreatic cancer at 3.0 T. Non-contrast based approach for liver function quantification using Bayesian-based intravoxel incoherent motion diffusion weighted imaging: A pilot study

Abstract:Purpose: Liver cirrhosis disrupts liver function and tissue perfusion, detectable by magnetic resonance imaging (MRI). Assessing liver function at the voxel level with 13-b value intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) could aid in radiation therapy liver-sparing treatment for patients with early impairment. This study aimed to evaluate the feasibility of IVIM-DWI for liver function assessment and correlate it with other multiparametric (mp) MRI methods at the voxel level. Method: This study investigates the variability of apparent diffusion coefficient (ADC) derived from 13-b value IVIM-DWI and B1-corrected dual flip angle (DFA) T1 mapping. Experiments were conducted in-vitro with QIBA and NIST phantoms and in 10 healthy volunteers for IVIM-DWI. Additionally, 12 patients underwent an mp-MRI examination. The imaging protocol included a 13-b value IVIM-DWI sequence for generating IVIM parametric maps. B1-corrected DFA T1 pulse sequence was used for generating T1 maps, and Gadoxatate low temporal resolution dynamic contrast-enhanced (LTR-DCE) MRI was used for generating the Hepatic extraction fraction (HEF) map. The Mann-Whitney U test was employed to compare IVIM-DWI parameters (Pure Diffusion, Dslow ; Pseudo diffusion, Dfast ; and Perfusion Fraction, Fp ) between the healthy volunteer and patient groups. Furthermore, in the patient group, statistical correlations were assessed at a voxel level between LTR-DCE MRI-derived HEF, T1 post-Gadoxetate administration, ΔT1%, and various IVIM parameters using Pearson correlation. Results: For-vitro measurements, the maximum coefficient of variation of the ADC and T1 parameters was 12.4% and 16.1%, respectively. The results also showed that Fp and Dfast were able to distinguish between healthy liver function and mild liver function impairment at the global level, with p = 0.002 for Fp and p < 0.001 for Dfast . Within the patient group, these parameters also exhibited a moderate correlation with HEF at the voxel level. Conclusion: Overall, the study highlighted the potential of Dfast and Fp for detecting liver function impairment at both global and pixel levels.
Free-Breathing Liver Magnetic Resonance Imaging With Respiratory Frequency-Modulated Continuous-Wave Radar-Trigger Technique: A Preliminary Study

Abstract:Purpose: The aim of this study is to evaluate the performance of free-breathing liver MRI with a novel respiratory frequency-modulated continuous-wave radar-trigger (FT) technique on T2-weighted imaging (T2WI) and diffusion-weighted imaging (DWI) for both healthy volunteers and patients in comparison to navigator-trigger (NT) and belt-trigger (BT) techniques. Methods: In this prospective study, 17 healthy volunteers and 23 patients with known or suspected liver diseases were enrolled. Six sequences (T2WI and DWI with FT, NT, and BT techniques) were performed in each subject. Quantitative evaluation and qualitative assessment were analyzed by two radiologists. Overall image quality, blurring, motion artifacts, and liver edge delineations were rated on a 4-point Likert scale. The liver and lesion signal-to-noise ratio (SNR), the lesion-to-liver contrast-to-noise ratio (CNR), as well as the apparent diffusion coefficient (ADC) value were quantitatively calculated. Results: For volunteers, there were no significant differences in the image quality Likert scores and quantitative parameters on T2WI and DWI with three respiratory-trigger techniques. For patients, NT was superior to other techniques for image quality on T2WI; conversely, little difference was found on DWI in qualitative assessment. The mean SNR of the liver on T2WI and DWI with BT, NT, and FT techniques was similar in patients, which is in line with volunteers. FT performed better in terms of higher SNR (705.13 ± 434.80) and higher CNR (504.41 ± 400.69) on DWI at b50 compared with BT (SNR: 651.83 ± 401.16; CNR:429.24 ± 404.11) and NT (SNR: 639.41 ± 407.98; CNR: 420.64 ± 416.61) ( p < 0.05). The mean ADC values of the liver and lesion with different techniques in both volunteers and patients showed non-significant difference. Conclusion: For volunteers, the performance of T2WI as well as DWI with three respiratory-trigger techniques was similarly good. As for patients, FT-DWI is superior to BT and NT techniques in terms of higher lesion SNR and CNR at b50.

Abstract:Background: Multi-echo chemical-shift-encoded magnetic resonance imaging (MRI) has been widely used for fat quantification and fat suppression in clinical liver examinations. Clinical liver water-fat imaging typically requires breath-hold acquisitions, with the free-breathing acquisition method being more desirable for patient comfort. However, the acquisition for free-breathing imaging could take up to several minutes. The purpose of this study is to accelerate four-dimensional free-breathing whole-liver water-fat MRI by jointly using high-dimensional deep dictionary learning and model-guided (MG) reconstruction. Methods: A high-dimensional model-guided deep dictionary learning (HMDDL) algorithm is proposed for the acceleration. The HMDDL combines the powers of the high-dimensional dictionary learning neural network (hdDLNN) and the chemical shift model. The neural network utilizes the prior information of the dynamic multi-echo data in spatial respiratory motion, and echo dimensions to exploit the features of images. The chemical shift model is used to guide the reconstruction of field maps, R2∗ maps, water images, and fat images. Data acquired from ten healthy subjects and ten subjects with clinically diagnosed nonalcoholic fatty liver disease (NAFLD) were selected for training. Data acquired from one healthy subject and two NAFLD subjects were selected for validation. Data acquired from five healthy subjects and five NAFLD subjects were selected for testing. A three-dimensional (3D) blipped golden-angle stack-of-stars multi-gradient-echo pulse sequence was designed to accelerate the data acquisition. The retrospectively undersampled data were used for training, and the prospectively undersampled data were used for testing. The performance of the HMDDL was evaluated in comparison with the compressed sensing-based water-fat separation (CS-WF) algorithm and a parallel non-Cartesian recurrent neural network (PNCRNN) algorithm. Results: Four-dimensional water-fat images with ten motion states for whole-liver are demonstrated at several R values. In comparison with the CS-WF and PNCRNN, the HMDDL improved the mean peak signal-to-noise ratio (PSNR) of images by 9.93 and 2.20 dB, respectively, and improved the mean structure similarity (SSIM) of images by 0.058 and 0.009, respectively, at R=10. The paired t-test shows that there was no significant difference between HMDDL and ground truth for proton-density fat fraction (PDFF) and R2∗ values at R up to 10. Conclusions: The proposed HMDDL enables features of water images and fat images from the highly undersampled multi-echo data along spatial, respiratory motion, and echo dimensions, to improve the performance of accelerated four-dimensional (4D) free-breathing water-fat imaging.

Abstract:Background In diffusion-weighted magnetic resonance imaging (DWI) using single-shot echo planar imaging (ss-EPI), both reduced field-of-view (FOV) excitation and sensitivity encoding (SENSE) alone can increase in-plane resolution to some degree. However, when the two techniques are combined to further increase resolution without pronounced geometric distortion, the resulted images are often corrupted by high level of noise and artifact due to the numerical restriction in SENSE. Hence, this study is aimed to provide a reconstruction method to deal with this problem. Methods The proposed reconstruction method was developed and implemented to deal with the high level of noise and artifact in the combination of reduced FOV imaging and traditional SENSE, in which all the imaging data were considered jointly by incorporating the motion induced phase variations among excitations. The in vivo human spine diffusion images from ten subjects were acquired at 1.5 T and reconstructed using the proposed method, and compared with SENSE magnitude average results for a range of reduction factors in reduced FOV. These images were evaluated by two radiologists using visual scores (considering distortion, noise and artifact levels) from 1 to 10. Results The proposed method was able to reconstruct images with greatly reduced noise and artifact compared to SENSE magnitude average. The mean g-factors were maintained close to 1 along with enhanced signal-to-noise ratio efficiency. The image quality scores of the proposed method were significantly higher (P < 0.01) than SENSE magnitude average for all the evaluated reduction factors. Conclusion The proposed method can improve the combination of SENSE and reduced FOV for high-resolution ss-EPI DWI with reduced noise and artifact.
Simultaneous multislice echo-planar diffusion-weighted imaging (DWI) in patients with focal liver lesions: a comparative study with conventional DWI

Abstract:Background: Simultaneous multislice (SMS) technology improves acquisition efficiency of diffusion-weighted imaging (DWI). This study aimed to evaluate the performance of SMS-DWI in image quality and apparent diffusion coefficient (ADC) measurements for focal liver lesions (FLLs) as compared with that of conventional DWI (CON-DWI). Methods: The institutional ethics committee of West China Hospital, Sichuan University approved this single-center, prospective study conducted from February 2021 to March 2022. Free-breathing SMS-DWI and CON-DWI examinations were acquired on a 3-T scanner with b-values of 50, 400, and 800 s/mm2. Qualitative image quality and quantitative measurements of signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and ADC were compared between SMS-DWI and CON-DWI. The ADC values for FLLs were further compared between SMS-DWI and CON-DWI in different patient subgroups. The intra- and interreader agreements were assessed. Significance was set at P<0.05. Results: This study included 116 patients (96 males, 20 females; mean age 52.0±10.7 years) with 119 FLLs. No significant differences were observed between SMS-DWI and CON-DWI regarding overall image quality in any b-value DWIs, and there were also no differences observed between SMS-DWI and CON-DWI (b=800 s/mm2) for either SNR or CNR (both P values >0.05). ADC values obtained from CON-DWI were higher than those from SMS-DWI in all FLLs [(1.31±0.47)×10-3 vs. (1.26±0.46)×10-3 mm2/s; P=0.004], and similar findings were observed across the different patient subgroups. The consistency analysis showed intrareader intraclass correlation coefficient (ICC) values of 0.792-0.944 and interreader ICC values of 0.758-0.861 for quantitative measurements (SNR, CNR, and ADC) and kappa values of 0.609-0.878 for qualitative image quality. Conclusions: SMS-DWI achieved a 37% reduction in scan time compared to CON-DWI while maintaining comparable overall image quality. Notably, the ADC values for FLLs were observed to be quantitatively lower with SMS-DWI.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>To investigate the clinical feasibility of single-breath-hold (SBH) T2-weighted (T2WI) liver MRI with deep learning-based reconstruction in the evaluation of image quality and lesion delineation, compared with conventional multi-breath-hold (MBH) T2WI.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>One hundred and fifty-two adult patients with suspected liver disease were prospectively enrolled. Two independent readers reviewed images acquired with conventional MBH-T2WI and SBH-T2WI at 3.0 T MR scanner. For image quality analyses, motion artifacts scores and boundary sharpness scores were compared using nonparametric Wilcoxon matched pairs tests between MBH-T2WI and SBH-T2WI. With the reference standard, 89 patients with 376 index lesions were included for lesion analyses. The lesion detection rates were compared by chi-square test, the lesion conspicuity scores and lesion-liver contrast ratio (CR) were compared using nonparametric Wilcoxon matched pairs tests between the two sequences.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>For both readers, motion artifacts scores of SBH-T2WI were significantly lower than MBH-T2WI (<em>P</em> < 0.001). Boundary sharpness scores of SBH-T2WI were significantly higher than MBH-T2WI (<em>P</em> < 0.001). The lesion detection rates for SBH-T2WI were significantly higher than MBH-T2WI (<em>P</em> < 0.001); the differences of lesion detection rates between the two sequences were statistically significant for small (≤ 10 mm) liver lesions (<em>P</em> < 0.001), while not significant for larger (> 10 mm) lesions (<em>P</em> > 0.05). Lesion conspicuity scores were significantly higher on SBH-T2WI than MBH-T2WI in the entire cohort as well as in both stratified subgroups of lesions ≤10 mm and > 10 mm (<em>P</em> < 0.001 for all). CRs for focal liver lesions were also significantly higher with SBH-T2WI (<em>P</em> < 0.001).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The SBH-T2WI sequence with deep-learning based reconstruction showed promising performance as it provided significantly better image quality, lesion detectability, lesion conspicuity and contrast within a single breath-hold, compared with the conventional MBH-T2WI.</p>

Abstract:Purpose DWI is an important contrast for prostate MRI to enable early and accurate detection of cancer. This study introduces a Dixon 3‐shot‐EPI protocol with structured low‐rank reconstruction for navigator‐free DWI. The aim is to overcome the limitations of single‐shot EPI (ssh‐EPI), such as geometric distortions and fat signal interference, while addressing the motion‐induced phase variations of multishot EPI and simultaneously allowing water/fat separation. Methods DWI data were acquired from 7 healthy volunteers using both Dixon 3‐shot EPI and standard fat‐suppressed ssh‐EPI with similar scan times for comparison. Two readers evaluated image quality using a 5‐point Likert scale regarding different aspects. The ADC values were quantitatively compared between protocols. Abstract:Background:Conventional liver T1 mapping techniques are typically performed under breath-holding conditions; they have limited slice coverage and often rely on multiple acquisitions. Furthermore, liver fat affects the accuracy of T1 quantification. Therefore, we aim to propose a free-breathing technique for simultaneous water-fat separation and T1 mapping of the whole liver (SWALI) in a single scan.Methods:The proposed SWALI sequence included an inversion recovery (IR) preparation pulse followed by a series of multiecho three-dimensional (3D) golden-angle radial acquisitions. For each echo time (TE), a series of images containing a mix of water and fat were reconstructed using a sliding window method. For each inversion time (TI), water and fat were separated, and then water and fat T1 estimation was conducted. The fat fraction (FF) was calculated based on the last TI image. The FF and water T1 quantification accuracy were compared with the gold standard sequences in the phantom. The in vivo feasibility was tested in 9 healthy volunteers, 2 patients with fatty liver, and 3 patients with hepatocellular carcinoma (HCC). The reproducibility was evaluated in the patients with fatty liver and in the healthy volunteers.Results:The mean FF and the mean water T1 values obtained by the SWALI sequence showed good agreements with chemical shift-encoded magnetic resonance imaging (CSE-MRI; r=0.998; P<0.001) and fat-suppressed (FS) IR-spin echo (SE; r=0.997; P<0.001) in the phantom. For the patients with fatty liver and the healthy volunteers, the SWALI sequence showed no significant difference with CSE-MRI in FF quantification (P=0.53). In T1 quantification, comparable T1 values were obtained with the SWALI sequence and modified Look-Locker inversion recovery (MOLLI; P=0.10) in healthy volunteers, while the water T1 estimated by the SWALI sequence was significantly lower than the water-fat compound T1 estimated by MOLLI (P<0.001) in patients with fatty liver. In the reproducibility study, the intraclass correlation coefficients (ICCs) for the estimated FF and water T1 were 0.997 and 0.943, respectively. Water T1 of the patients with HCC calculated using the SWALI sequence showed a significant reduction after the contrast administration (P<0.001).Conclusions:Free-breathing water-fat separation and T1 mapping of the whole liver with 2.5 mm isotropic spatial resolution were achieved simultaneously using the SWALI sequence in a 5-min scan.

Abstract:OBJECTIVE: The evaluate the feasibility of a novel deep learning-reconstructed ultra-fast respiratory-triggered T2WI sequence (DL-RT-T2WI) In liver imaging, compared with respiratory-triggered Arms-T2WI (Arms-RT-T2WI) and respiratory-triggered FSE-T2WI (FSE-RT-T2WI) sequences.METHODS: 71 patients with liver lesions underwent 3-T MRI and were prospectively enrolled. Two readers independently analyzed images acquired with DL-RT-T2WI, Arms-RT-T2WI, and FSE-RT-T2WI. The qualitative evaluation indicators, including overall image quality (OIQ), sharpness, noise, artifacts, lesion detectability (LC), lesion characterization (LD), cardiacmotion-related signal loss (CSL), and diagnostic confidence (DC), were evaluated in two readers, and further statistically compared using paired Wilcoxon rank-sum test among three sequences.RESULTS: 176 lesions were detected in DL-RT-T2W and Arms-RT-T2WI, and 175 were detected in FSE-RT-T2WI. The acquisition time of DL-RT-T2WI was improved by 4.8-7.9 folds compared to the other two sequences. The OIQ was scored highest for DL-RT-T2WI (R1, 4.61 ± 0.52 and R2, 4.62 ± 0.49), was significantly superior to Arms-RT-T2WI (R1, 4.30 ± 0.66 and R2, 4.34 ± 0.69) and FSE-RT-T2WI (R1, 3.65 ± 1.08 and R2, 3.75 ± 1.01). Artifacts and sharpness scored highest for DL-RT-T2WI, followed by Arms-RT-T2WI, and were lowest for FSE-RT-T2WI in both two readers. Noise and CSL for DL-RT-T2WI scored similar to Arms-RT-T2WI (P > 0.05) and were significantly superior to FSE-RT-T2WI (P < 0.001). Both LD and LC for DL-RT-T2WI were significantly superior to Arms-RT-T2WI and FSE-RT-T2WI in two readers (P < 0.001). DC for DL-RT-T2WI scored best, significantly superior to Arms-RT-T2WI (P < 0.010) and FSE-RT-T2WI (P < 0.001).CONCLUSIONS: The novel ultra-fast DL-RT-T2WI is feasible for liver imaging and lesion characterization and diagnosis, not only offers a significant improvement in acquisition time but also outperforms Arms-RT-T2WI and FSE-RT-T2WI concerning image quality and DC.

Abstract:Objectives This study evaluates the impact of liver steatosis on the discriminative ability for liver fibrosis and inflammation using a novel Dixon water-only fat-corrected Look-Locker T1 mapping sequence, compared with a standard shortened Modified Look-Locker Inversion Recovery (shMOLLI) sequence, with the aim of overcoming the limitation of steatosis-related confounding in liver T1 mapping. Materials and Methods 3 T magnetic resonance imaging of the liver including the 2 T1 mapping sequences and proton density fat fraction (PDFF) was prospectively performed in 24 healthy volunteers and 38 patients with histologically proven liver fibrosis evaluated within 90 days of liver biopsy. Paired Mann-Whitney test compared sequences between participants with and without significant liver steatosis (PDFF cutoff 10%), and unpaired Kruskal-Wallis test compared healthy volunteers to patients with early (F0–2) and advanced (F3–4) liver fibrosis, as well as low (A0–1) and marked (A2–3) inflammatory activity. Univariate and multivariate logistic regression models assessed the impact of liver steatosis on both sequences. Results Dixon_W T1 was higher than shMOLLI T1 in participants without steatosis (median 896 ms vs 890 ms, P = 0.04), but lower in participants with liver steatosis (median 891 ms vs 973 ms, P < 0.001). Both methods accurately differentiated between volunteers and patients with early and advanced fibrosis (Dixon_W 849 ms, 910 ms, 947 ms, P = 0.011; shMOLLI 836 ms, 918 ms, 978 ms, P < 0.001), and those with mild and marked inflammation (Dixon_W 849 ms, 896 ms, 941 ms, P < 0.01; shMOLLI 836 ms, 885 ms, 978 ms, P < 0.001). Univariate logistic regression showed slightly lower performance of the Dixon_W sequence in differentiating fibrosis (0.69 vs 0.73, P < 0.01), compensated by adding liver PDFF in the multivariate model (0.77 vs 0.75, P < 0.01). Conclusions Dixon water-only fat-corrected Look-Locker T1 mapping accurately identifies liver fibrosis and inflammation, with less dependency on liver steatosis than the widely adopted shMOLLI T1 mapping technique, which may improve its predictive value for these conditions.

Abstract:Purpose Breath‐held fat‐suppressed volumetric T1‐weighted MRI is an important and widely‐used technique for evaluating the abdomen. Both fat‐saturation and Dixon‐based fat‐suppression methods are used at conventional field strengths; however, both have challenges at lower field strengths (<1.5T) due to insufficient fat suppression and/or inadequate resolution. Specifically, at lower field strengths, fat saturation often fails due to the short T1 of lipid; and Cartesian Dixon imaging provides poor spatial resolution due to the need for a long ∆TE, due to the smaller ∆f between water and lipid. The purpose of this work is to demonstrate a new approach capable of simultaneously achieving excellent fat suppression and high spatial resolution on a 0.55T whole‐body system. Methods We applied 3D stack‐of‐spirals Dixon imaging at 0.55T, with compensation of concomitant field phase during reconstruction. The spiral readouts make efficient use of the requisite ∆TE. We compared this with 3D Cartesian Dixon imaging. Experiments were performed in 2 healthy and 10 elevated liver fat volunteers. Results Stack‐of‐spirals Dixon imaging at 0.55T makes excellent use of the required ∆TE, provided high SNR efficiency and finer spatial resolution (1.7 × 1.7 × 5 mm3) compared Cartesian Dixon (3.5 × 3.5 × 5 mm3), within a 17‐s breath‐hold. We observed successful fat suppression, and improved definition of structures such as the liver, kidneys, and bowel. Conclusion We demonstrate that high‐resolution single breath‐hold volumetric abdominal T1‐weighted imaging is feasible at 0.55T using spiral sampling and concomitant field correction. This is an attractive alternative to existing Cartesian‐based methods, as it simultaneously provides high‐resolution and excellent fat‐suppression.
