Abstract:Early disease detection in veterinary care relies on identifying subclinical abnormalities in asymptomatic animals during wellness visits. This study introduces an algorithm designed to distinguish between wellness and other veterinary visits.The purpose of this study is to validate the use of a visit classification algorithm compared to manual classification of veterinary visits by three board-certified veterinarians. Using a dataset of 11,105 clinical visits from 2012 to 2017 involving 655 animals (85.3% canines and 14.7% felines) across 544 U.S. veterinary establishments, the model was trained using a Gradient Boosting Machine model. Three validators were tasked with classifying 400 visits, including both wellness and other types of visits, selected randomly from the same database used for initial algorithm training, aiming to maintain consistency and relevance between the training and application phases; visit classifications were subsequently categorized into "wellness" or "other" based on majority consensus among validators to assess the algorithm's performance in identifying wellness visits. The algorithm demonstrated a specificity of 0.94 (95% CI: 0.91 to 0.96), implying its accuracy in distinguishing non-wellness visits. The algorithm had a sensitivity of 0.86 (95% CI: 0.80 to 0.92), indicating its ability to correctly identify wellness visits as compared to the annotations provided by veterinary experts. The balanced accuracy, calculated as 0.90 (95% CI: 0.87 to 0.93), further confirms the algorithm's overall effectiveness. The algorithm exhibits strong specificity and sensitivity, ensuring accurate identification of a high proportion of wellness visits. Overall, this algorithm holds promise for advancing research on preventive care's role in subclinical disease identification, but prospective studies are needed for validation.

Using a gradient boosted model for case ascertainment from free-text veterinary records

Abstract:Case ascertainment for prevalence and incidence studies from veterinary clinical data poses a major challenge because medical notes are not consistently structured or complete. Using natural language processing (NLP) and machine learning, this study aimed to obtain accurate case recognition for feline upper respiratory tract infections (primarily caused by viruses such as feline herpes virus (FHV-1) and feline calici virus (FCV), and bacteria such as Chlamydophila felis, Mycoplasma felis and Bordetella bronchiseptica using retrospective electronic veterinary records from the Royal Society for Prevention of Cruelty to Animals, Queensland (RSPCA Qld). Data cleaning and NLP on eight years of free-text veterinary records from RSPCA Queensland was carried out to derive text-based predictors. The NLP steps included sorting records by length of stay, vectorising, tokenising and spell checking against a bespoke veterinary database. A gradient boosted model (GBM) was trained to predict the probability of each animal having a diagnosis of upper respiratory infection. A manually annotated dataset was used for training the algorithm to learn dominant patterns between predictors (frequencies of n-grams) and responses (manual binary case classification). The GBM's performance was tested against an out of sample validation dataset, and model agnostics were used to interrogate the model's learning process. The GBM used patient-level frequencies of 1250 unique n-grams as predictor variables and was able to predict the probability of cases in the validation dataset with an accuracy of 0.95 (95% CI 0.92, 0.97) and F1 score of 0.96. Predictors that exerted the highest influence on the model included frequencies of "doxycycline", "flu", "sneezing", "doxybrom" and "ocular". The trained GBM was deployed on the full dataset spanning eight years, comprising 60,258 clinical entries. The prevalence in the full dataset was predicted to be 23.59%, which is in line with domain expertise from practicing veterinarians at the shelter. Case ascertainment is a crucial step for further epidemiological study of cat flu. Ultimately, this tool can be extended to other clinical procedures, conditions, and diseases such as intensive care treatment due to snake bites and tick paralysis, physical injuries such as orthopaedic fractures or chest injuries and labour-intensive infectious diseases like parvovirus, canine cough, and ringworm, all of which require prolonged quarantine and care.
The Use of Triaxial Accelerometers and Machine Learning Algorithms for Behavioural Identification in Domestic Cats (Felis catus): A Validation Study

Abstract:Animal behaviour can be an indicator of health and welfare. Monitoring behaviour through visual observation is labour-intensive and there is a risk of missing infrequent behaviours. Twelve healthy domestic shorthair cats were fitted with triaxial accelerometers mounted on a collar and harness. Over seven days, accelerometer and video footage were collected simultaneously. Identifier variables (n = 32) were calculated from the accelerometer data and summarized into 1 s epochs. Twenty-four behaviours were annotated from the video recordings and aligned with the summarised accelerometer data. Models were created using random forest (RF) and supervised self-organizing map (SOM) machine learning techniques for each mounting location. Multiple modelling rounds were run to select and merge behaviours based on performance values. All models were then tested on a validation accelerometer dataset from the same twelve cats to identify behaviours. The frequency of behaviours was calculated and compared using Dirichlet regression. Despite the SOM models having higher Kappa (>95%) and overall accuracy (>95%) compared with the RF models (64–76% and 70–86%, respectively), the RF models predicted behaviours more consistently between mounting locations. These results indicate that triaxial accelerometers can identify cat specific behaviours.

Fine-tuning foundational models to code diagnoses from veterinary health records

Abstract:Veterinary medical records represent a large data resource for application to veterinary and One Health clinical research efforts. Use of the data is limited by interoperability challenges including inconsistent data formats and data siloing. Clinical coding using standardized medical terminologies enhances the quality of medical records and facilitates their interoperability with veterinary and human health records from other sites. Previous studies, such as DeepTag and VetTag, evaluated the application of Natural Language Processing (NLP) to automate veterinary diagnosis coding, employing long short-term memory (LSTM) and transformer models to infer a subset of Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT) diagnosis codes from free-text clinical notes. This study expands on these efforts by incorporating all 7,739 distinct SNOMED-CT diagnosis codes recognized by the Colorado State University (CSU) Veterinary Teaching Hospital (VTH) and by leveraging the increasing availability of pre-trained large language models (LLMs). Ten freely-available pre-trained LLMs were fine-tuned on the free-text notes from 246,473 manually-coded veterinary patient visits included in the CSU VTH's electronic health records (EHRs), which resulted in superior performance relative to previous efforts. The most accurate results were obtained when expansive labeled data were used to fine-tune relatively large clinical LLMs, but the study also showed that comparable results can be obtained using more limited resources and non-clinical LLMs. The results of this study contribute to the improvement of the quality of veterinary EHRs by investigating accessible methods for automated coding and support both animal and human health research by paving the way for more integrated and comprehensive health databases that span species and institutions.

Abstract:Parents who abuse their children may not accept traditional therapy but may be influenced by the child's primary care physician. A comparative study of abusive and nonabusive mothers showed abusers to have lower self-concept and higher self-concept incongruence and inconsistency than nonabusers. They were also found to value authority over others more, and conformity and benevolence less, than nonabusers. Practically applied, the data lead the pediatrician to an educative and supportive role in which he or she may enhance self-esteem and lower unrealistic expectations in the course of treating the child. In addition, there seems to be a need to develop access to support groups, day care, and other avenues for the mother's personal growth. This may be done either within a pediatric practice or through liaison with community resources.

Abstract:The present study has examined the molecular events leading to formation of alpha beta dimers in normal murine thymocytes and mature T cells. We demonstrate that TCR assembly proceeds by initial association of TCR alpha with CD3 delta epsilon proteins and by association of TCR beta with CD3 gamma epsilon proteins to form alpha delta epsilon and beta gamma epsilon trimers; these trimers then associate to form alpha delta epsilon-beta gamma epsilon complexes, within which alpha-beta disulfide bond formation occurs. We also show that TCR-associated protein (TRAP) associates uniquely with CD3 gamma epsilon pairs and that formation of beta gamma epsilon trimers occurs subsequent to TRAP dissociation. Importantly, we document that the assembly step that is quantitatively limiting in CD4+ CD8+ thymocytes is the initial association of TCR alpha with CD3 delta epsilon chains, which appears necessary to protect nascent TCR alpha proteins from accelerated degradation within the ER of immature thymocytes.
Development and internal validation of diagnostic prediction models using machine-learning algorithms in dogs with hypothyroidism

Abstract:Introduction: Hypothyroidism can be easily misdiagnosed in dogs, and prediction models can support clinical decision-making, avoiding unnecessary testing and treatment. The aim of this study is to develop and internally validate diagnostic prediction models for hypothyroidism in dogs by applying machine-learning algorithms. Methods: A single-institutional cross-sectional study was designed searching the electronic database of a Veterinary Teaching Hospital for dogs tested for hypothyroidism. Hypothyroidism was diagnosed based on suggestive clinical signs and thyroid function tests. Dogs were excluded if medical records were incomplete or a definitive diagnosis was lacking. Predictors identified after data processing were dermatological signs, alopecia, lethargy, hematocrit, serum concentrations of cholesterol, creatinine, total thyroxine (tT4), and thyrotropin (cTSH). Four models were created by combining clinical signs and clinicopathological variables expressed as quantitative (models 1 and 2) and qualitative variables (models 3 and 4). Models 2 and 4 included tT4 and cTSH, models 1 and 3 did not. Six different algorithms were applied to each model. Internal validation was performed using a 10-fold cross-validation. Apparent performance was evaluated by calculating the area under the receiver operating characteristic curve (AUROC). Results: Eighty-two hypothyroid and 233 euthyroid client-owned dogs were included. The best performing algorithms were naive Bayes in model 1 (AUROC = 0.85; 95% confidence interval [CI] = 0.83-0.86) and in model 2 (AUROC = 0.98; 95% CI = 0.97-0.99), logistic regression in model 3 (AUROC = 0.88; 95% CI = 0.86-0.89), and random forest in model 4 (AUROC = 0.99; 95% CI = 0.98-0.99). Positive predictive value was 0.76, 0.84, 0.93, and 0.97 in model 1, 2, 3, and 4, respectively. Negative predictive value was 0.89, 0.89, 0.99, and 0.99 in model 1, 2, 3, and 4, respectively. Discussion: Machine learning-based prediction models were accurate in predicting and quantifying the likelihood of hypothyroidism in dogs based on internal validation performed in a single-institution, but external validation is required to support the clinical applicability of these models.
Computational diagnosis and risk evaluation for canine lymphoma

Abstract:The canine lymphoma blood test detects the levels of two biomarkers, the acute phase proteins (C-Reactive Protein and Haptoglobin). This test can be used for diagnostics, for screening, and for remission monitoring as well. We analyze clinical data, test various machine learning methods and select the best approach to these oblems. Three families of methods, decision trees, kNN (including advanced and adaptive kNN) and probability density evaluation with radial basis functions, are used for classification and risk estimation. Several pre-processing approaches were implemented and compared. The best of them are used to create the diagnostic system. For the differential diagnosis the best solution gives the sensitivity and specificity of 83.5% and 77%, respectively (using three input features, CRP, Haptoglobin and standard clinical symptom). For the screening task, the decision tree method provides the best result, with sensitivity and specificity of 81.4% and >99%, respectively (using the same input features). If the clinical symptoms (Lymphadenopathy) are considered as unknown then a decision tree with CRP and Hapt only provides sensitivity 69% and specificity 83.5%. The lymphoma risk evaluation problem is formulated and solved. The best models are selected as the system for computational lymphoma diagnosis and evaluation of the risk of lymphoma as well. These methods are implemented into a special web-accessed software and are applied to the problem of monitoring dogs with lymphoma after treatment. It detects recurrence of lymphoma up to two months prior to the appearance of clinical signs. The risk map visualization provides a friendly tool for exploratory data analysis.

Deep learning-based ultrasonographic classification of canine chronic kidney disease

Abstract:Objectives: In veterinary medicine, attempts to apply artificial intelligence (AI) to ultrasonography have rarely been reported, and few studies have investigated the value of AI in ultrasonographic diagnosis. This study aimed to develop a deep learning-based model for classifying the status of canine chronic kidney disease (CKD) using renal ultrasonographic images and assess its diagnostic performance in comparison with that of veterinary imaging specialists, thereby verifying its clinical utility. Materials and methods: In this study, 883 ultrasonograms were obtained from 198 dogs, including those diagnosed with CKD according to the International Renal Interest Society (IRIS) guidelines and healthy dogs. After preprocessing and labeling each image with its corresponding IRIS stage, the renal regions were extracted and classified based on the IRIS stage using the convolutional neural network-based object detection algorithm You Only Look Once. The training scenarios consisted of multi-class classification, categorization of images into IRIS stages, and four binary classifications based on specific IRIS stages. To prevent model overfitting, we balanced the dataset, implemented early stopping, used lightweight models, and applied dropout techniques. Model performance was assessed using accuracy, recall, precision, F1 score, and receiver operating characteristic curve and compared with the diagnostic accuracy of four specialists. Inter- and intra-observer variabilities among specialists were also evaluated. Results: The developed model exhibited a low accuracy of 0.46 in multi-class classification. However, a significant performance improvement was observed in binary classifications, with the model designed to distinguish stage 3 or higher showing the highest accuracy of 0.85. In this classification, recall, precision, and F1 score values were all 0.85, and the area under the curve was 0.89. Compared with radiologists, whose accuracy ranged from 0.48 to 0.62 in this experimental scenario, the AI model exhibited superiority. Intra-observer reliability among radiologists was substantial, whereas inter-observer variability showed a moderate level of agreement. Conclusions: This study developed a deep-learning framework capable of reliably classifying CKD IRIS stages 3 and 4 in dogs using ultrasonograms. The developed framework demonstrated higher accuracy than veterinary imaging specialists and provided more objective and consistent interpretations. Therefore, deep-learning-based ultrasound diagnostics are potentially valuable tools for diagnosing CKD in dogs.

Abstract:Dogs are beloved pets and loyal companions to millions of people worldwide. Unfortunately, dogs can also suffer from a variety of skin diseases that can cause discomfort, pain, and even life-threatening complications. Some dog skin diseases can be transmitted to humans through direct contact. Early detection and treatment of these skin diseases are crucial for the health and well-being of dogs and humans. Our project aims to provide a quick and precise approach to identifying various types of skin diseases in dogs. To expedite the process of identifying and diagnosing infections related to canine interaction, we plan to utilize a machine-learning model. This approach aims to reduce the time and expertise required for accurate and consistent diagnosis, which can otherwise be challenging and time-consuming. Two models, InceptionV3 and MobileNetV2, were utilized and compared in our implementation. In the case of InceptionV3, a training accuracy of 0.99 and a validation accuracy of 0.98 were achieved. For MobileNetV2, we attained a validation accuracy of 96 and a categorical accuracy of 97. Abstract:Assessing the behaviour and physical attributes of domesticated dogs is critical for predicting the suitability of animals for companionship or specific roles such as hunting, military or service. Common methods of behavioural assessment can be time consuming, labour-intensive, and subject to bias, making large-scale and rapid implementation challenging. Objective, practical and time effective behaviour measures may be facilitated by remote and automated devices such as accelerometers. This study, therefore, aimed to validate the ActiGraph® accelerometer as a tool for behavioural classification. This study used a machine learning method that identified nine dog behaviours with an overall accuracy of 74% (range for each behaviour was 54 to 93%). In addition, overall body dynamic acceleration was found to be correlated with the amount of time spent exhibiting active behaviours (barking, locomotion, scratching, sniffing, and standing; R2 = 0.91, p < 0.001). Machine learning was an effective method to build a model to classify behaviours such as barking, defecating, drinking, eating, locomotion, resting-asleep, resting-alert, sniffing, and standing with high overall accuracy whilst maintaining a large behavioural repertoire.

Further evaluation and validation of the VETSCAN IMAGYST: in-clinic feline and canine fecal parasite detection system integrated with a deep learning algorithm

Abstract:Abstract Background Fecal examinations in pet cats and dogs are key components of routine veterinary practice; however, their accuracy is influenced by diagnostic methodologies and the experience level of personnel performing the tests. The VETSCAN IMAGYST system was developed to provide simpler and easier fecal examinations which are less influenced by examiners’ skills. This system consists of three components: a sample preparation device, an automated microscope scanner, and analysis software. The objectives of this study were to qualitatively evaluate the performance of the VETSCAN IMAGYST system on feline parasites ( Ancylostoma and Toxocara cati ) and protozoan parasites ( Cystoisospora and Giardia ) and to assess and compare the performance of the VETSCAN IMAGYST centrifugal flotation method to reference centrifugal and passive flotation methods. Methods To evaluate the diagnostic performance of the scanning and algorithmic components of the VETSCAN IMAGYST system, fecal slides were prepared by the VETSCAN IMAGYST centrifugal flotation technique with pre-screened fecal samples collected from dogs and cats and examined by both an algorithm and parasitologists. To assess the performance of the VETSCAN IMAGYST centrifugal flotation technique, diagnostic sensitivity and specificity were calculated and compared to those of conventional flotation techniques. Results The performance of the VETSCAN IMAGYST algorithm closely correlated with evaluations by parasitologists, with sensitivity of 75.8–100% and specificity of 93.1-100% across the targeted parasites. For samples with 50 eggs or less per slide, Lin’s concordance correlation coefficients ranged from 0.70 to 0.95 across the targeted parasites. The results of the VETSCAN IMAGYST centrifugal flotation method correlated well with those of the conventional centrifugal flotation method across the targeted parasites: sensitivity of 65.7–100% and specificity of 97.6–100%. Similar results were observed for the conventional passive flotation method compared to the conventional centrifugal flotation method: sensitivity of 56.4–91.7% and specificity of 99.4–100%. Conclusions The VETSCAN IMAGYST scanning and algorithmic systems with the VETSCAN IMAGYST fecal preparation technique demonstrated a similar qualitative performance to the parasitologists’ examinations with conventional fecal flotation techniques. Given the deep learning nature of the VETSCAN IMAGYST system, its performance is expected to improve over time, enabling it to be utilized in veterinary clinics to perform fecal examinations accurately and efficiently. Natural Language Processing-Assisted Classification Models to Confirm Monoclonal Gammopathy of Undetermined Significance and Progression in Veterans' Electronic Health Records

Abstract:Purpose: To develop and validate natural language processing (NLP)-assisted machine learning (ML)-based classification models to confirm diagnoses of monoclonal gammopathy of undetermined significance (MGUS) and multiple myeloma (MM) from electronic health records (EHRs) in the Veterans Health Administration (VHA). Materials and methods: We developed precompiled lexicons and classification rules as features for the following ML classifiers: logistic regression, random forest, and support vector machines (SVMs). These features were trained on 36,044 EHR documents from a random sample of 400 patients with at least one International Classification of Disease code for MGUS diagnosis from 1999 to 2021. The best-performing feature combination was calibrated in the validation set (17,826 documents/200 patients) and evaluated in the testing set (9,250 documents/100 patients). Model performance in diagnosis confirmation was compared with manual chart review results (gold standard) using recall, precision, accuracy, and F1 score. For patients correctly labeled as disease-positive, the difference between model-identified diagnosis dates and the gold standard was also computed. Results: In the testing set, the NLP-assisted classification model using SVMs achieved best performance in both MGUS and MM confirmation with recall/precision/accuracy/F1 of 98.8%/93.3%/93.0%/96.0% for MGUS and 100.0%/92.3%/99.0%/96.0% for MM. Dates of diagnoses matched (±45 days) with those of gold standard in 73.0% of model-confirmed MGUS and 84.6% of model-confirmed MM. Conclusion: An NLP-assisted classification model can reliably confirm MGUS and MM diagnoses and dates and extract laboratory results using automated interpretation of EHR data. This algorithm has the potential to be adapted to other disease areas in VHA EHR system.

Abstract:Background: Colorectal Polyps are the main source of precancerous lesions in colorectal cancer. To increase the early diagnosis of tumors and improve their screening, we aimed to develop a simple and non-invasive diagnostic prediction model for colorectal polyps based on machine learning (ML) and using accessible health examination records. Methods: We conducted a single-center observational retrospective study in China. The derivation cohort, consisting of 5426 individuals who underwent colonoscopy screening from January 2021 to January 2024, was separated for training (cohort 1) and validation (cohort 2). The variables considered in this study included demographic data, vital signs, and laboratory results recorded by health examination records. With features selected by univariate analysis and Lasso regression analysis, nine machine learning methods were utilized to develop a colorectal polyp diagnostic model. Several evaluation indexes, including the area under the receiver-operating-characteristic curve (AUC), were used to compare the predictive performance. The SHapley additive explanation method (SHAP) was used to rank the feature importance and explain the final model. Results: 14 independent predictors were identified as the most valuable features to establish the models. The adaptive boosting machine (AdaBoost) model exhibited the best performance among the 9 ML models in cohort 1, with accuracy, sensitivity, specificity, positive predictive value, negative predictive value, F1 score, and AUC (95% CI) of 0.632 (0.618–0.646), 0.635 (0.550–0.721), 0.674 (0.591–0.758), 0.593 (0.576–0.611), 0.673 (0.654–0.691), 0.608 (0.560–0.655) and 0.687 (0.626–0.749), respectively. The final model gave an AUC of 0.675 in cohort 2. Additionally, the precision recall (PR) curve for the AdaBoost model reached the highest AUPR of 0.648, positioning it nearest to the upper right corner. SHAP analysis provided visualized explanations, reaffirming the critical factors associated with the risk of colorectal polyps in the asymptomatic population. Conclusions: This study integrated the clinical and laboratory indicators with machine learning techniques to establish the predictive model for colorectal polyps, providing non-invasive, cost-effective screening strategies for asymptomatic individuals and guiding decisions for further examination and treatment.

Abstract:Background and objective: To develop the COVid Veteran (COVet) score for clinical deterioration in Veterans hospitalized with COVID-19 and further validate this model in both Veteran and non-Veteran samples. No such score has been derived and validated while incorporating a Veteran sample. Derivation cohort: Adults (age ≥ 18 yr) hospitalized outside the ICU with a diagnosis of COVID-19 for model development to the Veterans Health Administration (VHA) (n = 80 hospitals). Validation cohort: External validation occurred in a VHA cohort of 34 hospitals, as well as six non-Veteran health systems for further external validation (n = 21 hospitals) between 2020 and 2023. Prediction model: eXtreme Gradient Boosting machine learning methods were used, and performance was assessed using the area under the receiver operating characteristic curve and compared with the National Early Warning Score (NEWS). The primary outcome was transfer to the ICU or death within 24 hours of each new variable observation. Model predictor variables included demographics, vital signs, structured flowsheet data, and laboratory values. Results: A total of 96,908 admissions occurred during the study period, of which 59,897 were in the Veteran sample and 37,011 were in the non-Veteran sample. During external validation in the Veteran sample, the model demonstrated excellent discrimination, with an area under the receiver operating characteristic curve of 0.88. This was significantly higher than NEWS (0.79; p < 0.01). In the non-Veteran sample, the model also demonstrated excellent discrimination (0.86 vs. 0.79 for NEWS; p < 0.01). The top three variables of importance were eosinophil percentage, mean oxygen saturation in the prior 24-hour period, and worst mental status in the prior 24-hour period. Conclusions: We used machine learning methods to develop and validate a highly accurate early warning score in both Veterans and non-Veterans hospitalized with COVID-19. The model could lead to earlier identification and therapy, which may improve outcomes.

Abstract:Introduction: Myxomatous mitral valve disease (MMVD) is the most common cause of heart failure in dogs, and assessing the risk of heart failure in dogs with MMVD is often challenging. Machine learning applied to electronic health records (EHRs) is an effective tool for predicting prognosis in the medical field. This study aimed to develop machine learning-based heart failure risk prediction models for dogs with MMVD using a dataset of EHRs. Methods: A total of 143 dogs with MMVD between May 2018 and May 2022. Complete medical records were reviewed for all patients. Demographic data, radiographic measurements, echocardiographic values, and laboratory results were obtained from the clinical database. Four machine-learning algorithms (random forest, K-nearest neighbors, naïve Bayes, support vector machine) were used to develop risk prediction models. Model performance was represented by plotting the receiver operating characteristic (ROC) curve and calculating the area under the curve (AUC). The best-performing model was chosen for the feature-ranking process. Results: The random forest model showed superior performance to the other models (AUC = 0.88), while the performance of the K-nearest neighbors model showed the lowest performance (AUC = 0.69). The top three models showed excellent performance (AUC ≥ 0.8). According to the random forest algorithm's feature ranking, echocardiographic and radiographic variables had the highest predictive values for heart failure, followed by packed cell volume (PCV) and respiratory rates. Among the electrolyte variables, chloride had the highest predictive value for heart failure. Discussion: These machine-learning models will enable clinicians to support decision-making in estimating the prognosis of patients with MMVD.

Abstract:Abstract Background Fecal examination is an important component of routine companion animal wellness exams. Sensitivity and specificity of fecal examinations, however, are influenced by sample preparation methodologies and the level of training and experience of personnel who read fecal slides. The VETSCAN IMAGYST system consists of three components: a sample preparation device, a commercially available scanner, and an analysis software. The VETSCAN IMAGYST automated scanner and cloud-based, deep learning algorithm, locates, classifies, and identifies parasite eggs found on fecal microscopic slides. The main study objectives were (i) to qualitatively evaluate the capabilities of the VETSCAN IMAGYST screening system and (ii) to assess and compare the performance of the VETSCAN IMAGYST fecal preparation methods to conventional fecal flotation techniques. Methods To assess the capabilities of VETSCAN IMAGYST screening components, fecal slides were prepared by the VETSCAN IMAGYST centrifugal and passive flotation techniques with 100 pre-screened fecal samples collected from dogs and cats and examined by both the algorithm and parasitologists. To determine the diagnostic sensitivity and specificity of the VETSCAN IMAGYST sample preparation techniques, fecal flotation slides were prepared by four different techniques (VETSCAN IMAGYST centrifugal and passive flotations, conventional centrifugal flotation, and passive flotation using OVASSAY® Plus) and examined by parasitologists. Additionally, required sample preparation and scanning times were estimated on a subset of samples to evaluate VETSCAN IMAGYST ease-of-use. Results The algorithm performance of the VETSCAN IMAGYST closely matched that of the parasitologists, with Pearsonʼs correlation coefficient ( r ) ranging from 0.83–0.99 across four taxa of parasites, Ancylostoma , Toxocara , Trichuris and Taeniidae. Both VETSCAN IMAGYST centrifugal and passive flotation methods correlated well with conventional preparation methods on all targeted parasites (diagnostic sensitivity of 75.8–100%, specificity of 91.8–100%, qualitative agreement between methods of 93.8–94.5%). Sample preparation, slide scan and image analysis were completed within 10–14 min by VETSCAN IMAGYST centrifugal and passive flotations, respectively. Conclusions The VETSCAN IMAGYST scanning system with the VETSCAN IMAGYST sample preparation methods demonstrated a qualitative match in comparison to the results of parasitologists’ examinations with conventional fecal flotation techniques. The VETSCAN IMAGYST is an easy-to-use, next generation qualitative and possibly quantitative diagnostic platform that brings expert clinical results into the hands of veterinary clinics.

Abstract:Background: Temporal phenotyping of patient journeys, which capture the common sequence patterns of interventions in the treatment of a specific condition, is useful to support understanding of antimicrobial usage in veterinary patients. Identifying and describing these phenotypes can inform antimicrobial stewardship programs designed to fight antimicrobial resistance, a major health crisis affecting both humans and animals, in which veterinarians have an important role to play. Objective: This research proposes a framework for extracting temporal phenotypes of patient journeys from clinical practice data through the application of natural language processing (NLP) and unsupervised machine learning (ML) techniques, using cat bite abscesses as a model condition. By constructing temporal phenotypes from key events, the relationship between antimicrobial administration and surgical interventions can be described, and similar treatment patterns can be grouped together to describe outcomes associated with specific antimicrobial selection. Methods: Cases identified as having a cat bite abscess as a diagnosis were extracted from VetCompass Australia, a database of veterinary clinical records. A classifier was trained and used to label the most clinically relevant event features in each record as chosen by a group of veterinarians. The labeled records were processed into coded character strings, where each letter represents a summary of specific types of treatments performed at a given visit. The sequences of letters representing the cases were clustered based on weighted Levenshtein edit distances with KMeans+ + to identify the main variations of the patient treatment journeys, including the antimicrobials used and their duration of administration. Results: A total of 13,744 records that met the selection criteria was extracted and grouped into 8436 cases. There were 9 clinically distinct event sequence patterns (temporal phenotypes) of patient journeys identified, representing the main sequences in which surgery and antimicrobial interventions are performed. Patients receiving amoxicillin and surgery had the shortest duration of antimicrobial administration (median of 3.4 days) and patients receiving cefovecin with no surgical intervention had the longest antimicrobial treatment duration (median of 27 days). Conclusion: Our study demonstrates methods to extract and provide an overview of temporal phenotypes of patient journeys, which can be applied to text-based clinical records for multiple species or clinical conditions. We demonstrate the effectiveness of this approach to derive real-world evidence of treatment impacts using cat bite abscesses as a model condition to describe patterns of antimicrobial therapy prescriptions and their outcomes.
