Abstract:Abstract Funding Acknowledgements Type of funding sources: None. Introduction Instruments that allow us to measure the users' knowledge about their health can be useful in clinical practice to allow adjusting interventions, quantifying health gains and developing cost-effective interventions. Purpose To determine knowledge about heart disease among the population with diagnosed ischemic heart disease. Methods Cross-sectional, descriptive study using a sequential sample of patients with ischemic heart disease admitted between May 2nd and July 30th, 2022 at the cardiology service of the Hospital Centre of our city. A self-administered questionnaire was developed and applied to 165 patients, encompassing several dimensions: sociodemographic characteristics, clinical history, lifestyle, number of hospitalizations due to heart disease and characterization of the current admission. An instrument was used to determine knowledge about heart disease (Questionário de Avaliação de Conhecimentos em Doença Cardíaca: QACDC - Questionnaire for the Assessment of Knowledge in Heart Disease), the Medical Term Recognition Test (METER) to assess literacy and the Hospital Anxiety and Depression Scale (HADS). Patients were assessed during their stay at the hospital unit where they had been admitted due to an acute illness or for elective procedures. Using, preferably, parametric tests, we studied the association between the average score obtained in the QAC-DC and the variables studied. Results Women, the less educated and those with inadequate literacy levels were the groups with the lowest levels of knowledge. Regarding the areas of knowledge, the least recognized cardiovascular risk factors were the family history of ischemic heart disease (recognized by 86%), diabetes mellitus (recognized by 73%) and gender (marked by 19% of respondents). The most frequently identified symptom of heart disease was chest discomfort (92%), followed by difficulty breathing or shortness of breath (84%), while jaw, neck or interscapular discomfort was only recognized by 54% of respondents. Symptoms of stroke and acute myocardial infarction were confused by 88% of the sample. Conclusions There are important gaps in some areas of knowledge about heart disease among patients with ischemic heart disease. These gaps are particularly important in certain patient subgroups. Health intervention strategies must be sensitive to these data. The QAC-DC knowledge assessment instrument proved to be reliable, valid and applicable in clinical practice.

Abstract:Background: Significant advances in managing congenital heart disease (CHD) have occurred over the past few decades, resulting in a fast-growing adult patient population with distinct needs requiring urgent attention. Research has recently highlighted the prevalence of neurocognitive differences among adults living with CHD. Yet, there is a lack of knowledge about the perspectives of people living with CHD and family members/caregivers on brain health. We sought to explore their perspectives to guide future research and clinical endeavours. Methods: Using the principles of integrated knowledge translation and qualitative interpretive description, we conducted 2 focus groups with 7 individuals with CHD and their family members as part of a virtual forum on brain health in CHD. Data analysis followed the principles of interpretive description. Results: A lack of understanding about overall brain health and neurocognitive differences in adult CHD was identified. To increase overall knowledge about brain health, initiatives should (1) focus on the individual living with CHD, involving family members and peers; (2) use social media and health care encounters for knowledge exchange; and (3) ensure a "balancing act" in the information provided to avoid feelings of worry and uncertainty about the future while simultaneously empowering people living with CHD. Conclusions: There is a pressing need for better education about brain health among individuals living with CHD. Our findings can guide clinicians in developing programmes of care and (re)design health services that address the brain-heart axis and neurocognitive differences in CHD.

Is Knowledge Level of Coronary Heart Disease and Risk Factors Among Post–Percutaneous Coronary Intervention Patients Adequate?

Abstract:BACKGROUND: Percutaneous coronary intervention (PCI) is now commonly used in the treatment of coronary heart disease. However, shorter hospital stays after intervention may affect patients' knowledge and subsequent required lifestyle changes.OBJECTIVE: The aim of this study is to investigate participants' risk factor profile, knowledge of coronary heart disease, and the influence of demographic and risk factors on this knowledge.METHODS: This prospective, cross-sectional 1-site study recruited both elective and emergency PCI patients postdischarge. The questionnaire collected data on demographics, risk factor profile, and coronary heart disease knowledge as measured on the Bergman Heart Disease Knowledge Questionnaire. Bivariate and multivariate analyses were used to analyze the influence of 11 risk and sociodemographic factors on knowledge.RESULTS: The response rate was 67% (n = 84). The sample was mostly male and aged 65.79 ± 9.9 years, and 59% had an elective PCI. Risk factor burden was high; 2 or more risk factors were seen in 66% of participants. Mean knowledge score overall was 51%, with the highest score achieved in the risk factor domain (61%). Lowest scores were in the medical and symptoms domains (both 46%). Neither the bivariate nor the multivariate analyses were significant. A large proportion of patients believed that coronary heart disease was no longer a concern for them after PCI.CONCLUSIONS: As expected, the risk factor profile of post-PCI patients was high. However, their knowledge levels and awareness were unrelated to risk factor profile and poor in comparison with studies in other cardiac patients. This, in addition to the short stay in hospital and the low attendance of this cohort at cardiac rehabilitation, identifies this group of patients as a priority for further targeted education. Innovations are needed to increase knowledge and begin behavioral change predischarge after PCI. This should include target and goal setting for lifestyle change to avail of this critical education opportunity.

Abstract:Background and aim Congenital heart disease (CHD) is the most common birth defect with prevalence of 0.8%. Thanks to tremendous progress in medical and surgical practice, nowadays, >90% of children survive into adulthood. Recently European Society of Cardiology (ESC), American College of Cardiology (ACC)/ American Heart Association (AHA) issued guidelines which offer diagnostic and therapeutic recommendations for the different defect categories. However, the type of technical exams and their frequency of follow-up may vary largely between clinicians and centres. We aimed to present an overview of available diagnostic modalities and describe current surveillance practices by cardiologists taking care of adults with CHD (ACHD). Methods and results A questionnaire was used to assess the frequency cardiologists treating ACHD for at least one year administrated the most common diagnostic tests for ACHD. The most frequently employed diagnostic modalities were ECG and echocardiography for both mild and moderate/complex CHD. Sixty-seven percent of respondents reported that they routinely address psychosocial well-being. Conclusion Differences exist between reported current clinical practice and published guidelines. This is particularly true for the care of patients with mild lesions. In addition, some differences exist between ESC and American guidelines, with more frequent surveillance suggested by the Americans.

Abstract:• Volume 119 • Number 2 www.anesthesia-analgesia.org 251 A 70-year-old man underwent coronary artery bypass grafting and double valve replacement (CarpentierEdwards Magna: 33-mm prosthesis for rheumatic severe mitral regurgitation and 23 mm for moderate aortic stenosis). After cardiopulmonary bypass (CPB), his pulmonary artery wedge pressure (PAWP) was elevated to 31 mm Hg and central venous pressure to 18 mm Hg compared to pre-CPB values of 24 and 12 mm Hg, respectively. Transesophageal echocardiography (TEE) in the midesophageal 4-chamber and long-axis view revealed a left atrial (LA) linear structure of homogeneous intensity and random motion, dividing the LA into a small anterior (adjacent to the aortic valve, along the anterior mitral annulus) and a large posterior (along the posterior mitral annulus) compartments (Video Clips 1 and 2, see Supplemental Digital Content 1, http://links.lww.com/AA/A912, Supplemental Digital Content 2, http://links.lww.com/ AA/A913; Fig. 1, A and B). Neurocognition in adults with congenital heart disease post-cardiac surgery: A systematic review

Abstract:Background: Congenital heart disease (CHD) is the most common birth defect worldwide. Neurocognitive deficits and psychiatric disorders, which can impact daily life, have been reported in over 50% of adolescents and young adults with moderate to complex CHD. Objective: Conduct a systematic review of sex, clinical, psychological and social determinants of health (SDoH) factors affecting neurocognition in adults with CHD post-cardiac surgery. Methods: PubMed, Cumulated Index to Nursing and Allied Health Literature, and Embase were searched for relevant studies over the past 5 years. Thirteen articles met inclusion criteria of: 1) CHD post-cardiac surgery, 2) age ≥ 18 years, and 3) used a validated measure of neurocognition. Results: A total of 507 articles were identified. After screening, 30 articles underwent full text review yielding 13 eligible articles. Twelve articles reported deficits in multiple domains including executive function, intellectual functioning, visuospatial ability, and verbal fluency in more complex CHD. Only three studies examined cognition based on sex, with female and lower parental SES associated with worse cognitive outcomes. Most studies were from Europe, predominantly sampled Caucasian participants, had heterogeneous samples of CHD complexity, and lacked standardized cognitive measures which limited generalizability of findings. Conclusions: Adults with CHD present with a wide variety of cognitive deficits, with some associations with sex, clinical history, and SDoH factors. It remains unclear to what degree these factors affect cognition in adults with moderate to complex CHD. Future longitudinal studies should focus on age-related effects on cognition and potential health care disparities in diverse CHD samples.

Abstract:Objective: The objective of this study was to identify socioeconomic and clinical factors associated with disease-related knowledge of cardiac rehabilitation (CR) patients. Methods: Adults with coronary artery disease (CAD) were recruited during CR Phase 1 and completed questionnaires on the 1st day of Phase 2. Disease-related knowledge was assessed by the short version of the CAD Education Questionnaire. Socioeconomic status was defined by educational level, family income, and employment status. MannWhitney U and Spearman correlation were calculated to determine the association of knowledge with socioeconomic factors, number of risk factors, and wait time between hospital discharge and start of outpatient CR. Results: A convenience sample of 39 patients were recruited. Overall, the mean knowledge was 12.00 ± 3.3, which corresponds to 60% of possible scores. Monthly family income and number of risk factors influenced medical condition knowledge (P < 0.05), and employment status influenced total knowledge (P = 0.005) and risk factor knowledge (P = 0.002). Participants with three or more risk factors presented significantly higher knowledge (P = 0.02). Those that waited more than 17 weeks to start the CR presented significantly lower knowledge (P = 0.04). Conclusion: Participants with low income and unemployed were more likely to have inadequate disease-related knowledge; however, the entire sample presented low understanding of their condition. Public health strategies and educational interventions must continue to focus on these vulnerable groups.
Caring for patients with congenital heart disease: a nationwide survey of nurses' educational needs

Abstract:Background/Aims Around one in 130 babies are born with some form of congenital heart defect. Advances in treatment have resulted in increasing numbers of these individuals surviving with complex disease; around 90% survive into adult life. In order for these patients to receive high-quality care throughout their lives, it is imperative that nurses are educated appropriately. This study explored the educational needs of nurses who encounter this group of patients. Methods A survey designed by the British Adult Congenital Cardiac Association was distributed to all congenital heart disease networks in the UK, as well as on social media and via networks and forums for nurses and allied health professionals. The survey took place between 4 June 2020 and 6 July 2020. Responses were analysed using summary statistics. Results A total of 513 survey responses were received from nurses and allied health professionals. Band 5 nurses reported a higher level of anxiety about caring for patients with congenital heart defect (17.9%) than average for the study population (12.5%). In the previous 2 years, nearly one-third of the band 5 staff had received no training. Respondents who worked in non-congenital heart defect centres reported that learning to manage care for patients with congenital heart defects, manage emergencies and have a simple understanding of the condition should be an educational priority. Local taught study days were identified as the most desirable means of learning. Conclusions While education within specialist centres appears to be developed and well-embedded, junior nurses (band 5), and particularly those working in non-cardiac units, are being left behind. Easy access information about emergency care and simple descriptions of care requirements would benefit nurses who rarely encounter congenital heart defect patients. Including the fundamentals of care within the nursing curriculum could provide a base from which nurses can develop. Preferences for accredited study sessions support the need for further development of postgraduate education.

Abstract:BACKGROUND: A comprehensive understanding of adult congenital heart disease outcomes must include psychological functioning. Our multisite study offered the opportunity to explore depression and anxiety symptoms within a global sample.OBJECTIVES: In this substudy of the APPROACH-IS (Assessment of Patterns of Patient-Reported Outcomes in Adults With Congenital Heart Disease-International Study), the authors we investigated the prevalence of elevated depression and anxiety symptoms, explored associated sociodemographic and medical factors, and examined how quality of life (QOL) and health status (HS) differ according to the degree of psychological symptoms.METHODS: Participants completed the Hospital Anxiety and Depression Scale, which includes subscales for symptoms of anxiety (HADS-A) and depression (HADS-D). Subscale scores of 8 or higher indicate clinically elevated symptoms and can be further categorized as mild, moderate, or severe. Participants also completed analogue scales on a scale of 0 to 100 for QOL and HS. Analysis of variance was performed to investigate whether QOL and HS differed by symptom category.RESULTS: Of 3,815 participants from 15 countries (age 34.8 ± 12.9 years; 52.7% female), 1,148 (30.1%) had elevated symptoms in one or both subscales: elevated HADS-A only (18.3%), elevated HADS-D only (2.9%), or elevations on both subscales (8.9%). Percentages varied among countries. Both QOL and HS decreased in accordance with increasing HADS-A and HADS-D symptom categories (P < 0.001).CONCLUSIONS: In this global sample of adults with congenital heart disease, almost one-third reported elevated symptoms of depression and/or anxiety, which in turn were associated with lower QOL and HS. We strongly advocate for the implementation of strategies to recognize and manage psychological distress in clinical settings. Abstract:Purpose The aim of this study was to gain a deeper insight into patients’ perception of chronic heart failure (CHF) symptoms by analyzing their compliance with nonpharmacological recommendations. Patients and methods This was a prospective, single-center survey-based registry. Patients included in this study were hospitalized between December 2014 and January 2016 at the 1st Department of Cardiology, University Hospital of Lord’s Transfiguration, Poznań University of Medical Sciences, and had been diagnosed with CHF at least 3 months prior to inclusion. Participants were divided according to New York Heart Association (NYHA) functional class into mild CHF (NYHA I and II) and severe CHF (NYHA III and IV), and according to age into young (<50 years), middle-aged (50–70 years) and old (>70 years). The survey included information about the patients’ sex, age, education, length of the illness and 12 questions about their perception of CHF. This study included 201 patients. The mean age was 58±15 years. Results The younger the patients, the more often they thought that CHF is curable. The patients presenting with severe CHF tended to think that CHF is incurable significantly more often than those with mild CHF. Most of the patients considered loss of appetite, cough and vomiting the least alarming symptoms. Significantly more patients with severe CHF exercised less and reported reduced sexual activity more often in comparison to the mild CHF patients. Most of the young patients reported no changes to their sexual activity, body mass index (BMI) or exercise after diagnosis of CHF. Most of the old patients exercised less than before diagnosis of CHF. Significantly more middle-aged patients reduced their BMI, quit smoking and reported lower sexual activity after diagnosis of CHF in comparison to the other groups. Conclusion Patients need to be better educated about the nature of CHF and the importance of lifestyle changes.

Abstract:Background: Motivation for the study. In Peru, the knowledge level of patients about their cardiovascular health is unknown at the first level of care, which receives 85% of the population, and where primary disease prevention activities are carried out. Background: Main findings. More than 70% of people had an inadequate level of knowledge about their own cardiovascular health. Background: Implications. Our results highlight the need to improve the education of people on cardiovascular health issues at the first level of care, taking into account that these diseases are increasingly frequent in the population. Background: This study aimed to determine the level of knowledge about cardiovascular diseases in people in a primary healthcare center (PHCC). A descriptive and cross-sectional study was carried out by surveying people who attended a PHCC in Lima, Peru. A score less than 6 was considered inadequate knowledge. A total of 400 people were surveyed, 66.3% were women and the mean age was 46.8 ± 16.2 years. The average score was 4.52 +/- 1.85. We found that 71% of those surveyed had an inadequate level of knowledge, regardless of age, gender or education level. Our findings show that the level of knowledge about risk factors and cardiovascular disease was inadequate in the primary care population. It is necessary to achieve proper specific education in cardiovascular risk factors in order to reduce the impact of these diseases.

Abstract:Abstract Background/introduction Patient education is a crucial component in the care of patients with coronary syndrome. Nursing has an important role in disease prevention and health promotion. More than half of patients with coronary artery disease have limited knowledge which is associated with low commitment to lifestyle modification, increased hospitalizations and higher risk of mortality. The core competencies of the nurses is to promote, maintain, and restore the health of individuals in preventive care. Knowledge assessment can support nurses to plan educational strategies to address the information needs of their patients, providing effective support in secondary prevention. Purpose (1) To assess secondary prevention knowledge in patients with coronary syndrome and (2) to compare knowledge levels among groups of patients aged <60 years and aged >60 years. Methods This was a cross-sectional study of hospitalised patients with coronary syndrome. The Coronary Artery Diseases Education Questionnaire (CADE-Q II) was used to evaluate knowledge. CADE-Q II has 31 items with 4 possible answers. Questions are related to health status, risk factors, exercise, nutrition, and psychosocial risk. The maximum score is 93, with higher scores indicating greater knowledge. Descriptive analysis of the CADE-Q II was performed. SPSS Version 22 was used for analysis, and the level of significance was set at 0.05 for all tests. No items were missing. Results We included 155 patients; mean age 58,26±8,37; 22 % female; 78 % male; 56 % < 60 years old; 44 % > 60 years old; 71 % hypertension; 22 % diabetes; 32 % smokers. Mean knowledge score of participants was 61,4/93. Patients obtained values of completely correct answers in the following areas: health status 55 %; risk factors 47 %; exercise 64 %; nutrition 50 %; psychosocial risk 64 %. The level of knowledge based on the scores obtained: great 0 %; good 37 %; acceptable 55 %; poor 7 %; inadequate 1 %. Within the age groups, we obtained statistically significant results, patients aged <60 years got score 63,2/93; patients aged >60 years got score 59,0/93 (p=0.012); in the exercise domain, patients aged <60 years got score 16,5/21; patients aged >60 years got score 13,9/21 (p=0.001); in the psychosocial domain, patients aged <60 years got score 10,9/15; patients aged >60 years got score 9,7/15 (p=0.034). Conclusion The results of study point to a lower of knowledge of patients with coronary syndrome regarding their disease and factors affecting cardiovascular risk in all studied domains. The use of an assessment tool in patients with coronary syndrome allows nurses to plan educational process, providing effective support in prevention. Adequate information, promotion of health literacy of patients are an essential part of prevention and minimization of risk factors. Nurses during the nursing process by using CADE-Q II ought to promote knowledge and attitudes towards cardioprotective lifestyle.

Cognitive Functioning and Psychosocial Outcomes in Adults with Complex Congenital Heart Disease: A Cross-sectional Pilot Study

Abstract:Adults with complex congenital heart disease (CHD) are at risk for cognitive dysfunction. However, associations between cognitive dysfunction and psychosocial outcomes are poorly defined. Between June and November 2022, we prospectively recruited 39 adults with complex CHD who completed a computerized cognitive assessment (Cogstate) and validated psychosocial scales measuring psychological distress, health-related quality of life (HRQOL), and resilience. Participants had a mean age of 36.4 ± 11.2 years. Over half (62%) were women, most (79%) had complex biventricular CHD, and 21% had Fontan physiology. Prevalence of cognitive dysfunction was greatest in the domains of attention (29%), working memory (25%), and psychomotor speed (21%). Adjusting for age and sex, Pearson partial correlations between Cogstate z -scores and self-reported cognitive problems were small. Participants who lived in the most disadvantaged areas and those with a below-average annual household income had lower global cognitive z -scores ( p = 0.02 and p = 0.03, respectively). Two-thirds (64%) reported elevated symptoms of depression, anxiety, and/or stress. Small correlations were observed between psychological distress and cognitive performance. Greater resilience was associated with lower psychological distress ( r ≥ −0.5, p < 0.001) and higher HRQOL ( r = 0.33, p = 0.02). Our findings demonstrate that adults with complex CHD have a high risk of cognitive dysfunction, though may not recognize or report their cognitive challenges. Lower socioeconomic status may be an indicator for those at risk of poorer cognitive functioning. Psychological distress is common though may not be a strong correlate of performance-based cognitive functioning. Formal cognitive evaluation in this patient population is essential. Optimizing resilience may be a protective strategy to minimize psychological distress and bolster HRQOL.

Abstract:Abstract Aims Less than one-third of adolescents with congenital heart disease (CHD) successfully transition to adult care, missing out on education of their cardiac condition, and risking loss to follow-up. We assessed the efficacy of our transition clinic on patient education and empowerment and identified correlates of successful transition. Methods and results Overall, 592 patients were seen at least once in our transition service between 2015 and 2022 (age 15.2 ± 1.8 years, 47.5% female). Most adolescents (53%) had moderate CHD, followed by simple (27.9%) and severe (19.1%) CHD. Learning disability (LD) was present in 18.9% and physical disability (PD) in 4.7%. In patients without LD, knowledge of their cardiac condition improved significantly from the first to the second visit (naming their condition: from 20 to 52.3%, P < 0.0001; describing: 14.4–42.7%, P < 0.0001; understanding: 26.1–60.7%, P < 0.0001), and from the second to the third (naming: 67.4%, P = 0.004, describing: 61.4%, P < 0.001, understanding: 71.1%, P = 0.02;). Patients with LD did not improve their disease knowledge over time (all P > 0.05). Treatment adherence and management involvement, self-reported anxiety, and dental care awareness did not change over time. Successful transition (attendance of ≥ 2 clinics) was achieved in 49.3%. Younger age at the first visit, simpler CHD, and absence of PD were associated with successful transition. Conclusion A transition service positively impacts on patient education and empowerment in most CHD adolescents transitioning to adult care. Strategies to promote a tailored support for patients with LD should be sought, and earlier engagement should be encouraged to minimize follow-up losses.

Abstract:Background: Congenital heart disease places survivors at increased risk for cardiovascular complications as they age and requires long-term medical management. Perceived health competence, or how capable one feels in managing one’s health, is linked to emotional adjustment in various disease populations, but has not been investigated among congenital heart disease survivors. Aim: The purpose of this study was to examine the relationship of perceived health competence at baseline (T1) as a predictor of anxiety and depressive symptoms three years later (T2). Methods: Congenital heart disease survivors ( n=125; M T1age =27; 58.6% female; severity of cardiac lesion: 25% simple, 44% moderate, 31% complex) were recruited from a pediatric and an adult hospital as part of a larger study. Participants completed the Perceived Health Competence Scale and the Youth or Adult Self-Report at T1 for anxiety and affective/depressive symptoms. At T2, participants completed the Hospital Anxiety and Depression Scale. New York Heart Association functional class, a measure of functional impairment, was abstracted from medical charts at T2. Results: Lower T1 perceived health competence was significantly associated with greater emotional distress at T1 (depression r=−0.47; anxiety: r=−0.45), as well as greater T2 functional impairment ( r=−0.41). T1 perceived health competence also predicted T2 anxiety and depressive symptoms, which remained significant when including T1 anxiety and depressive symptoms and T2 functional impairment. Conclusion: Congenital heart disease survivors who feel more competent in managing their health may be less likely to experience future anxiety and depressive symptoms. Perceived health competence may be a worthwhile target for psychosocial intervention to promote emotional wellbeing among congenital heart disease survivors and ensure the best outcomes.

Abstract:Purpose: To explore parental needs related to their experiences of living with a child with congenital heart defect (CHD) since the diagnosis. Design and methods: An interpretative qualitative study developed with nine parents of children between the ages of five months and 11 years diagnosed with CHD. Interviews were conducted at an ambulatory pediatric cardiology centre. Data were analyzed using inductive thematic analysis. The consolidated criteria for reporting qualitative research (COREQ) was followed for quality reporting. This research was approved by a research committee. Results: One central theme emerged, namely 'A desire to feel safe in dealing with the demands of CHD,' along with two main themes. The first is 'Looking for effective relations with healthcare professionals and health care systems' which encompasses three types of need: (1) need for continuous, clear and accurate information; (2) need for resolution and the support of services such as the public health care system and social services; (3) need for trust in health care professionals. The second theme is 'Looking for balance in daily life' with two main needs: (1) maintain family functioning and (2) learn to deal with the child and CHD. Conclusion: The main parental needs are related to their interactions with healthcare professionals and healthcare systems, highlighted by a need for information and trust relationships to feel safe in their daily lives. Practice implications: Our results imply rethinking the nurse presence in ambulatory care, implementation of a family-centered care approach and addressing the diverse and multifaceted experiences and needs of parents and children with CHD in different health care contexts.
