Abstract:Background: Hyperuricemia refers to a metabolic disorder associated with high serum uric acid levels and is regarded as a major risk factor not only for the development of gout but also for various chronic diseases. However, current clinical hypouricemic medications still have some undesired side effects and are contra-indicated by multiple comorbidities. Thus, the development of natural therapeutic products as novel, safe, and effective alternatives to pharmacological agents is urgently needed.Scope and approach: This review mainly focuses on the research progress of phytochemicals in the treatment of hyperuricemia and gout. Additionally, the chemical structure, specific mechanism, and effective dose of natural phytochemicals are comprehensively highlighted.Key findings and conclusions: Many in vivo and/or in vitro studies have indicated that phytochemicals derived from natural sources, including alkaloids, phenolic acids, and flavonoids, possess the beneficial properties of down-regulating uric acid levels and dissolving monosodium urate crystals. Therefore, they have great potential to be novel and safe agents that can prevent and combat hyperuricemia and gout.
Protective effect of urolithin a on cisplatin-induced nephrotoxicity in mice via modulation of inflammation and oxidative stress

Abstract:Limitation of widely used anti-cancer agent cisplatin for a patient is nephrotoxicity. Nephrotoxicity is presentable in mice by injecting cisplatin at 25 mg/kg with 3 days endpoint. We used the same model to understand the protective role of urolithin A. Cisplatin-induced renal damages measured by histological damage in proximal tubular cells and by the increase in serum neutrophil gelatinase-associated lipocalin (NGAL), blood urea nitrogen (BUN), creatinine and urinary Kidney Injury Molecule-1 (KIM-1). Urolithin A pretreatment reduced all the above renal damage parameters in a significant way. Urolithin A attenuated cisplatin-induced pro-inflammatory cytokine/chemokine tumor necrosis factor α (TNFα), interleukin 23 (IL-23), interleukin 18 (IL-18) and macrophage inflammatory protein 2 (MIP2). Cisplatin-induced CD11b positive macrophages in kidneys reduced by urolithin A. Urolithin A also attenuated cisplatin-induced renal oxidative/nitrative stress, which was measured by lipid peroxidation(4-hydroxy-2-nonenal or 4-HNE protein adducts) and protein nitration. Urolithin A cisplatin-induced kidney injury in mice through the down regulation of inflammatory cytokines/chemokine, immune cells, and oxidative/nitrative stress thus improving cisplatin-induced proximal tubular cell death.

Abstract:In the phytochemical screening, it was found that Dolichos Biflorus seed extract contained the presence of tannins steroids, protein, flavonoids, terpenoids, mucilage, saponin, and carbohydrate, but no alkaloids fixed oil. So, this plant has highly diuretic activity. Which can use for the treatment of urolithiasis. Objective: The Present study aims to investigate the anti-Urolithiatic activity of Dolichos Biflorus methanolic seed extract in a rat model. Methods: Dolichos Biflorus Seeds Extract were evaluated for anti-Urolithiatic activity. Urolithiasis has been induced in Wistar rats through Ethylene glycol (0.75%v/v, p.o). Upon completion of the experiment, animals in all groups have been sacrificed and biochemical parameters such as; calcium, creatinine, Phosphorus, Uric acid, Alkaline Phosphatase BUN (Blood Urea Nitrogen), and histology of Kidney have been observed. Furthermore, Potassium, Oxalate levels, and Alanine Amino Transferases were measured. Results: The extract of Dolichos Biflorus was proven to be safe in the toxicity findings. It has been shown that in-vivo significant effect of plant extract was able to manage the urolithiatic markers such as calcium, creatinine, phosphorus, uric acid, alkaline phosphatase, BUN (Blood Urea Nitrogen), and potassium. In the urolithiatic rats it has been observed that abundant crystal depositions, renal epithelial cells had more tubular dilatation and damage shown by large spaces in the tissue, renal stone deposition damages the renal tissue and detoriate the renal function. Urolithiatic markers such as calcium, creatinine, phosphorus, uric acid, alkaline phosphatase, BUN (Blood Urea Nitrogen), and potassium were normal in animals receiving plant extract and prevent renal cell injury. Conclusion: All its findings and phytoconstituents existing inside the extract must stay the viable chemical materials involved in the prevention of Urolithiatic.

Abstract:The aim of our presented study is to assess the effect of Rhizome extract of Iris pseudacorus L. and seed extract of Dolichos biflorus L. as preventive agent in experimentally induced urolithiasis model in rats. Rats were administered Sodium Oxalate (70 mg/kg, i. p.) in drinking water for 28 days in drinking water. In addition to this, Saponin extract of Iris pseudacorus and Dolichos biflorus of low dose and high dose were administered along with Sodium Oxalate on 14-28th day. After the experimental period, blood samples were collected by cardiac puncture to analyse for Creatinine, Calcium, Blood Urea Nitrogen (BUN), Phosphorus, Uric acid, Alkaline Phosphate, Potassium, and Alanine Amino Transferases followed by various antioxidants and kidney histopathology. The ethylene glycol feeding resulted in an increased level of all parameters evaluated compared to normal rats. All these conditions were reversed with plant extract treatment. Histopathological analysis also showed that rats treated with Sodium Oxalate had large deposits of calcium oxalate crystals, and that deposits were reduced in rats treated with plant extract. Results were also compared with the marketed product cystone as a standard. These data suggest that Iris pseudacorus and Dolichos biflorus Saponin extracts has a protective activity against urolithiasis.

Abstract:Background The recurrent nature and socioeconomic burden of nephrolithiasis demand effective treatments. Delineating the crosstalk between inflammatory processes and the endogenous oxalate metabolism pathway, which underpins nephrolithiasis pathogenesis, is essential for advancing treatment strategies. Purpose We aim to screen therapeutic Chinese herbal remedies and their bioactive constituents for kidney stone treatment using a fruit fly model, followed by efficacy and mechanism validation in a rodent nephrolithiasis model as well as in vitro human cell culture model. Study design and methods We developed a fruit fly model to screen for efficient traditional Chinese medicine herbs and their active compounds for kidney stone treatment. Candidate active compounds from efficient herbs were separated and identified by solid-phase chromatography coupled with LC-MS analysis. Fruit fly genetic tools were employed to manipulate the expression of related genes to explore the therapeutic mechanisms of the Lycii Cortex and kukoamine A (KuA). To confirm the therapeutic effects and mechanisms of KuA for mammalian nephrolithiasis, mouse model of glyoxylate-induced kidney stone and human renal tubular cells were used. The therapeutic role of kukoamine A in nephrolithiasis was evaluated by assessing tubular injury, crystal deposition, and adhesion. The level of expression and phosphorylation in cells and mice was assessed using RT-qPCR and western blot. Results Our findings indicate that Lycii Cortex potently inhibits calcium oxalate stone formation via activation of the JNK/Upd3/JAK/STAT signaling cascade, resulting in diminished endogenous oxalate synthesis by downregulating D-amino acid oxidase (DAO) gene expression, predominantly in fruit fly Malpighian tube stellate cells. KuA was identified as the principal bioactive constituent mediating these effects. Both mouse models and human cell assays confirmed KuA's efficacy in preventing calcium oxalate nephrolithiasis in mammals, through hepatic JAK/STAT3 pathway activation and upregulation of IL-6, culminating in reduced urinary crystal deposition. Conclusion Our research underscores the potential of kukoamine A as a lead compound in treating nephrolithiasis and reveals the interplay between the IL-6/JAK/STAT3 inflammatory pathway and endogenous oxalate metabolism in nephrolithiasis pathogenesis. Additionally, it highlights the utility of the fruit fly model as a powerful tool for deciphering the therapeutic mechanisms of traditional Chinese herbs.

Abstract:Abstract Background Despite rising public awareness and improvements in diagnostic and treatment methods, there are adverse effects brought on by drug resistance, an increase in overall treatment costs, and unanticipated side effects from pharmaceuticals. Main body Therefore, efforts for development strategy that is more efficient, more affordable, and more secure are underway. Such a strategy involves employing naturally occurring phytochemicals to delay the beginning, prevent it from happening, or treat it, and it sparks an increase in interest in studies looking for an effective agent in herbs and other plant materials used in traditional medicines. Urolithins are polyphenol chemicals generated by the gut microbiota studied for potential health benefits and have a high bioavailability. After being ingested, urolithins can move throughout the body and mediate in different locations. Urolithins are studied for over 40 years, but their mechanistic role has been explored recently in understanding their potential health benefits. Short conclusion This review gives an overview of the current Urolithin A research on human health. The findings highlight the importance of exploring the potential of urolithins as a natural compound for therapeutic applications. Elucidating the mechanisms behind the disease process and pinpointing candidate molecules and pathways to target preventive and therapeutic intervention are the need of the hour.

Abstract:Nephrolithiasis is a pathological condition characterized by the formation of solid crystals in the kidneys or other parts of urinary tract. Kidney stones are a serious public health issue and financial burden for health care system, as well as a painful and uncomfortable condition for patients, resulting in renal tissue injury in severe cases. Dietary habits, low fluid and high salt intake predominantly, contribute to the development of kidney stones. Current research suggests that polyphenols have a protective effect in the pathogenesis of kidney stones. Polyphenols are a group of naturally occurring compounds found in plant-based foods such as fruits, vegetables, tea, and coffee. In this review, we explore mechanisms underlying the beneficial effects of polyphenols, such as oxidative stress reduction and modulation of inflammatory pathways, in various in vitro models of nephrolithiasis. Additionally, certain polyphenols, such as catechins found in green tea, have been shown to inhibit the formation and growth of kidney stones in animal studies. This review highlights the antioxidant and anti-inflammatory effects, as well as the inhibition of crystal formation, as results of polyphenol treatment in vitro. Further research is required to determine the specific effects of polyphenols on kidney stone formation in humans; however, current knowledge implicates that incorporating a variety of polyphenol-rich foods into the diet may be a beneficial strategy for individuals at risk of developing nephrolithiasis.

Plant-based therapies for urolithiasis: a systematic review of clinical and preclinical studies

Abstract:Purpose: Urolithiasis, the formation of kidney stones, is a common and severe condition. Despite advances in understanding its pathophysiology, affordable treatment options are needed worldwide. Hence, the interest is in herbal medicines as alternative or supplementary therapy for urinary stone disease. This review explores the use of plant extracts and phytochemicals in preventing and treating urolithiasis. Methods: Following PRISMA standards, we systematically reviewed the literature on PubMed/Medline, focusing on herbal items evaluated in in vivo models, in vitro studies, and clinical trials related to nephrolithiasis/urolithiasis. We searched English language publications from January 2021 to December 2023. Studies assessing plant extracts and phytochemicals' therapeutic potential in urolithiasis were included. Data extracted included study design, stone type, plant type, part of plant used, solvent type, main findings, and study references. Results: A total of 64 studies were included. Most studies used ethylene glycol to induce hyperoxaluria and nephrolithiasis in rat models. Various extraction methods were used to extract bioactive compounds from different plant parts. Several plants and phytochemicals, including Alhagi maurorum, Aerva lanata, Dolichos biflorus, Cucumis melo, and quercetin, demonstrated potential effectiveness in reducing stone formation, size, and number. Conclusions: Natural substances offer an alternative or supplementary approach to current treatments, potentially reducing pain and improving the quality of life for urolithiasis patients. However, further research is needed to clarify their mechanisms of action and optimize their therapeutic use. The potential of plant-based therapies in treating urolithiasis is promising, and ongoing research is expected to lead to treatment advancements benefiting patients globally.

Abstract:Objective: This study aimed to evaluate the potential antiurolithiatic activity of the aqueous extract of *Catharanthus roseus* in a rat model of ethylene-glycol-induced urolithiasis. Methods: Thirty male Wistar rats were divided into five groups: Group I served as the control, Group II received ethylene glycol (0.75% v/v) in drinking water for the induction of urolithiasis, Group III received standard treatment (cystone), and Group IV was treated with the aqueous extract of Catharanthus roseus (200 mg/kg). Throughout the experimental period, various parameters such as urinary calcium, phosphate, oxalate levels, and kidney histopathology were assessed to evaluate the antiurolithiatic potential of Catharanthus roseus. Results: The results demonstrated a significant reduction in urinary calculi formation, lowered calcium and oxalate excretion, and ameliorated histological changes in the kidneys of rats treated with the aqueous extract of Catharanthus roseus. Conclusion: Our findings suggest that the aqueous extract of *Catharanthus roseus* possesses promising antiurolithiatic activity, potentially attributed to its ability to modulate urinary stone-forming parameters. Further studies are warranted to elucidate the underlying mechanisms and explore the therapeutic potential of *Catharanthus roseus* in managing urolithiasis.

Abstract:Oxidative stress is a state of excess of prooxidative species relative to the antioxidant defenses (enzymatic and nonenzymatic) in a living organism. The consequence of this imbalance is damage of the major cellular macromolecules (carbohydrates, lipids, proteins, and DNA), which further leads to a gradual loss of tissue and organ function. It has been shown that oxidative stress plays an important role in the pathogenesis of many chronic diseases (cardiovascular, metabolic, and neurodegenerative diseases and cancer) and in the process of aging. Thus, many strategies to combat oxidative stress have been proposed and tested. In this context, food rich in antioxidants has received great attention. Pomegranate, berries, and walnuts have been recognized as "superfood" particularly for their cardioprotective effects. The common characteristic of these foods is the high content of ellagitannins. Since tannins are not bioavailable, they have been neglected in nutrition science and even considered antinutrients for a long time. However, this view has changed dramatically once it was recognized that ellagic acid, released from ellagitannins in the gastrointestinal system, is further metabolized by colonic microbiota to bioavailable compounds-known as urolithins. Thus, urolithins (3,4-benzocoumarin derivatives) have emerged as novel natural bioactive compounds and are now the focus of extensive investigations. So far, urolithins were shown to be powerful modulators of oxidative stress and agents with potential anti-inflammatory, antiproliferative, and antiaging properties. Furthermore, a few synthetic derivatives of urolithins were recognized as lead compounds for new drug development. Available data on urolithin synthesis, physicochemical and pharmacokinetic characteristics, biological activity, and safety will be presented in this review.

Abstract:Ethnopharmacological relevance: Calcium oxalate crystals play a key role in the development and recurrence of kidney stones (also known as urolithiasis); thus, inhibiting the formation of these crystals is a central focus of urolithiasis prevention and treatment. Previously, we reported the noteworthy in vitro inhibitory effects of Aspidopterys obcordata fructo oligosaccharide (AOFOS), an active polysaccharide of the traditional Dai medicine Aspidopterys obcordata Hemsl. (commonly known as Hei Gai Guan), on the growth of calcium oxalate crystals. Aim of the study: To investigated the effectiveness and mechanism of AOFOS in treating kidney stones. Materials and methods: A kidney stones rats model was developed, followed by examining AOFOS transport dynamics and effectiveness in live rats. Additionally, a correlation between the polysaccharide and calcium oxalate crystals was studied by combining crystallization experiments with density functional theory calculations. Results: The results showed that the polysaccharide was transported to the urinary system. Furthermore, their accumulation was inhibited by controlling their crystallization and modulating calcium ion and oxalate properties in the urine. Consequently, this approach helped effectively prevent kidney stone formation in the rats. Conclusions: The present study emphasized the role of the polysaccharide AOFOS in modulating crystal properties and controlling crystal growth, providing valuable insights into their potential therapeutic use in managing kidney stone formation.

Abstract:This research demonstrates the diuretic effect of naringenin, a flavanone aglycone found in citrus, on spontaneously hypertensive female and male rats (SHR). The data reinforces existing literature findings that male SHR exhibits higher systolic blood pressure than age‐matched females. Urine volume assessed over 8 hours was lower when obtained from SHR males than females. When these animals were orally treated with different doses of naringenin (0.1 – 1 mg/kg), this increased urinary volume in both genders at the highest dose tested. In contrast, the lowest dose promoted a significant natriuretic effect. The other electrolytes analyzed in urine were not significantly altered, except potassium excretion, which was shown to be increased in the urine of SHR males. Furthermore, naringenin showed promise in reducing calcium oxalate (CaOx) crystal formation in an in vitro model, presenting potential advantages in lithiasis prevention.

Abstract:Urolithiasis, commonly known as kidney stones, is a prevalent urological disorder affecting millions of people worldwide. The present study aims to investigate the potential Antiurolithiatic activity of the aqueous leaves extract of Bauhinia variegata against ethylene glycol-induced urolithiasis in Wistar rats. This research involves a comprehensive pharmacological evaluation, including various biochemical, histological, and molecular analyses, to elucidate the mechanism of action and therapeutic potential of the plant extract. When ethylene glycol (EG) is given to rats in their drinking water at a concentration of 0.75 percent (v/v), oxalate is immediately and considerably excreted in the urine. Thirty inbred Wistar rats (weighing between 180 and 200g each) were utilized. There were five groups created for this 28-day research. The first group (the controls) received merely water. Group 2 was the uroolithiatic-induced group and received Ethylene glycol (EG) in drinking water at a concentration of 0.75 percent (v/v). Ethylene glycol (EG) drinking water (0.75% v/v) and 50 mg/kg body weight (i.p.) were administered to Group 3. Test-1 consisted of an aqueous Bauhinia variegate leaf extract. Group 4 received 100 mg/kg of body weight i.p. and ethylene glycol (EG) in water at a concentration of 0.75 v/v. Test-2 consisted of an aqueous Bauhinia variegate leaf extract. Group 5 received the gold-standard treatment, 5 ml/kg body weight p.o. of Cystone (Himalaya health care Pvt. Ltd) functioned as a benchmark sample for comparison. Biochemical and histological analysis indicated that Bauhinia variegate leaf extract prevented oxidative stress and harm to renal cells brought on by calcium oxalate crystals.

Abstract:Desmosium styracifolium (D. styracifolium), which is considered as a Chinese herbal medicine, has been reported to treat the kidney stone diseases. However, the potential phytochemically active components and the underlying mechanisms associated with its efficacy in targeting urolithiasis remain to be elucidated. This study aims to investigate the anti-urolithiatic effect of total flavonoids of D. styracifolium (TFDS) on calcium oxalate (CaOx) renal stones in Sprague–Dawley rats. Animal models of CaOx urolithiasis were established in male Sprague–Dawley rats by adding 5% w/w hydroxy-l-proline (HLP) in regular rat chow. The TFDS orally at 100, 400 mg/kg, respectively, were administered along with HLP for 28 days. At the end of 28 days of treatment, urine and serum samples were collected for crystalluria determination and various biochemical analysis. Kidney tissues were isolated and processed for antioxidant parameters measurement and histopathological examinations. HLP-induced hyperoxaluria alone reliably caused CaOx nephrolithiasis in rats. We showed that TFDS significantly reduced crystalluria and CaOx crystal deposits in the kidney sections as compared to untreated HLP group. Also, TFDS was observed to decrease urinary oxalate excretion, alleviate the pro-acidosis condition, improve the impaired renal functions and renal epithelial cell injury. Moreover, TFDS protected against the oxidative stress changes via reducing MDA content, increasing CAT and GSH-Px activities in renal homogenate, as well as attenuating the expression of MCP-1, OPN and TGF-β proteins. These results indicated that TFDS had beneficial effect on inhibition of CaOx formation in the rat kidney probably through a combination of antioxidant, anti-inflammatory, urine alkalinizing activities, and lowering the concentration of urinary stone-forming constituents. Thus, TFDS might have clinical implications in preventing oxidative renal cell injury and, ultimately, kidney stone formation. The data provide a rationale for the medicinal use of TFDS in nephrolithiasis and identify this agent as a potential source of new antiurolithic drugs.

SGLT2 Inhibitors and Their Effect on Urolithiasis: Current Evidence and Future Directions

Abstract:Urolithiasis (UL) is increasingly prevalent due to rising cardiorenometabolic diseases, posing significant management challenges despite advances in urological techniques. Sodium-glucose cotransporter-2 (SGLT2) inhibitors, primarily used for type 2 diabetes mellitus, chronic kidney disease, and heart failure, have emerged as a potential novel approach for UL treatment. These inhibitors may help reduce the risk of urolithiasis, particularly in patients with diabetes, by improving glycemic control and altering urinary chemistry, which are crucial factors in stone formation. However, the changes in urinary composition induced by SGLT2 inhibitors might also increase the risk of uric acid stone formation. This review evaluates the potential of SGLT2 inhibitors in managing UL, highlighting both the benefits and the risks. While these inhibitors show promise in reducing new and recurrent urinary stones in patients with diabetes, data on their effects in patients without diabetes who form stones are limited. Current human evidence largely comes from post hoc analyses of randomized controlled trials (RCTs) and large-scale database studies, with only one study providing detailed stone composition data. Experimental studies in animal models and cell lines have focused on calcium oxalate (CaOx) stones, showing that SGLT2 inhibitors specifically target CaOx stone formation and related renal inflammation. Although primarily studied for CaOx stones, their potential impact on other calcium-containing stones, such as calcium phosphate, remains promising. Further research is needed to explore their therapeutic potential and optimize treatment strategies.
Anti-urolithiatic effect of Cucumis melo L. var inodorous in male rats with kidney stones

Abstract:Melon seed extracts have high antioxidant activities and are effective against a variety of diseases, including kidney stones. In kidney stone model rats, the anti-urolithiatic effects of the hydro-ethanolic extract of melon seed and potassium citrate were studied and compared. After urolithiasis induction by ethylene glycol, the extract and potassium citrate were treated orally for 38 days concurrent with ethylene glycol. Then, urine and kidney sampling were done, and the urinary parameter levels were measured. The melon and potassium citrate treatments reduced the kidney index, the levels of urinary calcium and oxalate, calcium oxalate deposit numbers, the score of crystal deposits, histo-pathological damages, and the score of inflammation in the kidney sections, while elevating the urinary pH, magnesium, and citrate levels, and also the expression of the UMOD, spp1, and reg1 genes in the kidney of treated animals. The effect of potassium citrate is the same as the effect of melon in treated animals. So, their effects could be by normalizing urinary parameters, reducing crystal deposits, excreting small deposits from the kidney, reducing the chance of them being retained in the urinary tract, and elevating the expression of the UMOD, spp1, and reg1 genes, which are involved in kidney stone formation.

Abstract:Nephrolithiasis is a complex disease that results from a combination of factors related to both urine composition and kidney morphoanatomy. Development of calcium oxalate monohydrate papillary calculi is linked to initial subepithelial calcification of renal papilla. Progressive tissue calcification depends on preexisting injury and involves reactive oxygen species. Many plant extracts that protect against oxidative stress manifest antilithiasic activity. Our study focused on determining the effects of polyphenols on a lithiasis rat model. Rats were pretreated with polyphenols and grape seed extracts, followed by posterior induction of hyperoxalosis via treatment with ethylene glycol plus NH 4 Cl. The concentrations of calcium and other elements in kidney were determined, along with histological examination of kidney and 24 h urine analysis. Significant differences were observed in the renal calcium content between the control plus ethylene glycol-treated group and the epicatechin plus ethylene glycol-treated, red grape seed extract plus ethylene glycol-treated, and white grape seed extract plus ethylene glycol-treated groups, with reductions of about 50%. The antioxidant activity of polyphenols extracted from red and white grape seeds may be critical in the prevention of calcium oxalate monohydrate papillary calculus formation, particularly if calculi are induced by lesions caused by cytotoxic compounds with oxidative capacity.
