Abstract:Ethnopharmacological relevance: Helichrysum (Asteraceae) flowers, known as "altın otu, yayla çiçeği, kudama çiçeği" , are widely used to remove kidney stones and for their diuretic properties in Turkey. Aim of the study: To determine the curative effect of infusions prepared from capitulums of Helichrysum graveolens (M. Bieb.) Sweet (HG) and H. stoechas ssp. barellieri (Ten.) Nyman (HS) on sodium oxalate induced kidney stones. Materials and methods: Infusions prepared from the capitulums of HG and HS were tested for their curative effect on calcium oxalate deposition induced by sodium oxalate (70mg/kg i.p.). Following the injection of sodium oxalate for 5 days, plant extracts were administered to rats at two different doses. Potassium citrate was used as positive control. Water intake, urine volume, body, liver and kidney weights were measured; biochemical and hematological analyses were conducted on urine and blood samples. Additionally, histopathological examinations were done on kidney samples. Results: H. stoechas extract showed prominent effect at 156mg/kg dose (stone formation score: 0.33), whereas number of kidney stones was maximum in sodium oxalate group (stone formation score: 2.33). The reduction in the uric acid and oxalate levels of urine samples and the elevation in the urine citrate levels are significant and promising in extract groups. Some hematological, biochemical and enzymatic markers are also ameliorated by the extracts. Conclusions: This is the first report on the curative effect of immortal flowers. Our preliminary study indicated that Helichrysum extracts may be used for treatment of urolithiasis and Helichrysum extracts are an alternative therapy to potassium citrate for patients suffering from kidney stones.
Prophylactic effects of quercetin and hyperoside in a calcium oxalate stone forming rat model

Abstract:Quercetin and hyperoside (QH) are the two main constituents of the total flavone glycosides of Flos Abelmoschus manihot, which has been prescribed for treating chronic kidney disease for decades. This study aimed to investigate the effect of QH on calcium oxalate (CaOx) formation in ethylene glycol (EG)-fed rats. Rats were divided into three groups: an untreated stone-forming group, a QH-treated stone-forming group (20 mg/kg/day) and a potassium citrate-treated stone-forming group (potassium citrate was a worldwide-recognized calculi-prophylactic medicine). Ethylene glycol (0.5 %) was administered to the rats during the last week, and vitamin D3 was force-fed to induce hyperoxaluria and kidney calcium oxalate crystal deposition. 24 h urine samples were collected before and after inducing crystal deposits. Rats were killed and both kidneys were harvested after 3 weeks. Bisected kidneys were examined under a polarized light microscope for semi-quantification of the crystal-formation. The renal tissue superoxide dismutase and catalase levels were measured by Western blot. QH and potassium citrate have the ability to alkalinize urine. The number of crystal deposits decreased significantly in the QH-treated stone-forming group as compared to the other groups. Superoxide dismutase and catalase levels also increased significantly in the QH-treated stone-forming group, as compared with the untreated stone-forming group. QH administration has an inhibitory effect on the deposition of CaOx crystal in EG-fed rats and may be effective for preventing stone-forming disease.
Orthosiphon grandiflorum has a protective effect in a calcium oxalate stone forming rat model

Abstract:This study amied to investigate the effects of Orthosiphon grandiflorum on the renal tubular cell injury induced by oxalate and the inhibitory effects of O. grandiflorum on urinary deposit formation in an animal model and compared the results with those from a potassium citrate treatment. Rats were divided into three groups: an untreated stone-forming group, an O. grandiflorum-treated stone-forming group and a potassium citrate-treated stone forming group. Ethylene glycol (0.5%) was administered to the rats during the last week, and vitamin D3 (0.5 mum) was force fed to induce hyperoxaluria and kidney calcium oxalate crystal deposition. Twenty-four hour urine samples were collected before and after inducing crystal deposits. Rats were killed and both kidneys were harvested after 3 weeks. Oxidative stress was examined by 8-OHdG immunohistofluorescence. O. grandiflorum and potassium citrate have the ability to alkalinize urine. Among all groups, the number of crystal deposits and the level of 8-OHdG staining decreased significantly in the O. grandiflorum-treated stone forming group, as compared to the other groups. Superoxide dismutase and catalase levels also increased significantly in the O. grandiflorum-treated stone-forming group, as compared with the untreated stone-forming group. The results indicate that O. grandiflorum has a significant inhibitory effect on crystal deposition in the calcium oxalate-stone-forming rat model.

Abstract:When plasma osmolality exceeds 350 milliosmoles per kilogram of plasma water during cardiopulmonary resuscitation, a disproportionate increase in mortality is observed. The increase of plasma osmolality is in part related to the administration of sodium bicarbonate. If iatrogenic risks of alkali therapy are to be minimized, repetitive measurements of plasma osmolality are advised. Therefore, the authors investigated a rapid method for determining osmolality, in which whole blood rather than plasma was utilized, thereby obviating the time required for erythrocyte separation. This study demonstrated that the difference between osmolality measured on whole blood and that measured on plasma were comparable to duplicate measurements of osmolality on plasma. The authors conclude that osmolality measured on whole blood serves as a reliable and accurate estimate of plasma osmolality.

Abstract:The aim of our presented study is to assess the effect of Rhizome extract of Iris pseudacorus L. and seed extract of Dolichos biflorus L. as preventive agent in experimentally induced urolithiasis model in rats. Rats were administered Sodium Oxalate (70 mg/kg, i. p.) in drinking water for 28 days in drinking water. In addition to this, Saponin extract of Iris pseudacorus and Dolichos biflorus of low dose and high dose were administered along with Sodium Oxalate on 14-28th day. After the experimental period, blood samples were collected by cardiac puncture to analyse for Creatinine, Calcium, Blood Urea Nitrogen (BUN), Phosphorus, Uric acid, Alkaline Phosphate, Potassium, and Alanine Amino Transferases followed by various antioxidants and kidney histopathology. The ethylene glycol feeding resulted in an increased level of all parameters evaluated compared to normal rats. All these conditions were reversed with plant extract treatment. Histopathological analysis also showed that rats treated with Sodium Oxalate had large deposits of calcium oxalate crystals, and that deposits were reduced in rats treated with plant extract. Results were also compared with the marketed product cystone as a standard. These data suggest that Iris pseudacorus and Dolichos biflorus Saponin extracts has a protective activity against urolithiasis.

Abstract:Introduction: Both watermelon and Persian melon extracts have various pharmacological properties like anti-diabetic, anti-viral, anti-cancer, and anti-urolithiasis effects. Objectives: The present study was conducted to investigate the effects of hydroalcoholic extracts of watermelon and Persian melon rind on kidney stone prevention in male Wistar rats. Materials and Methods: Fifty-six Wister rats were randomly divided into seven groups and treated for 28 days. The first group (healthy control) and the second group (negative control) received drinking water and water containing 1% ethylene glycol, respectively. The third and fourth groups, received 100 mg/kg/d hydroalcoholic extract of watermelon rind and Persian melon rind, respectively in addition to 1% ethylene glycol. The fifth and sixth groups, received 400 mg/kg/d hydroalcoholic extract of watermelon rind and Persian melon rind, respectively in addition to 1% ethylene glycol. The seventh group received 0.5 mEq/kg/d potassium citrate in addition to 1% ethylene glycol for prevention and treatment of kidney stone. A 24-hour urine collection was conducted to determine the levels of sodium, calcium, uric acid, oxalate and citrate concentration. Histological study of calcium oxalate crystals was also performed. The serum levels of urea, creatinine, uric acid, calcium, phosphorus, magnesium, SGPT (serum glutamic-pyruvic transaminase), SGOT (serum glutamic-oxaloacetic transaminase), total antioxidant capacity, and malondialdehyde (MDA) of blood were determined accordingly. Results: In the present study, administration of high-dose extract of watermelon and Persian melon rind (400 mg/kg/d) and potassium citrate showed significant changes in variables of sodium, calcium, uric acid, citrate, urine volume ( P <0.01), blood creatinine, blood uric acid, blood calcium, and serum SGPT ( P <0.05). The histological study of calcium oxalate crystals showed a significant reduction in oxalate levels in all prevention groups. Conclusion: The extracts of watermelon and Persian melon rind are effective in preventing calcium oxalate stones by decreasing the levels of oxalate, sodium, and calcium and increasing citrate levels and urine volume and affecting the total antioxidant capacity. Persian melon rind extract was more effective than potassium citrate and watermelon rind extract in reducing urine sodium. High-dose watermelon rind extract showed similar effects as potassium citrate.
An In vitro Study of Orthosiphon stamineus (Misai Kucing) Standardized Water Extract as a Chemolytic Agent in Urolithiasis

Abstract:Background: Orthosiphon stamineus was reported to have diuretic effects in experimental rats, and this leads to inhibition of kidney stones through the abundant levels of minerals and flavonoids in it. This study aimed to determine the in vitro effects of O. stamineus water extract as a potential chemolytic agent in urolithiasis. Materials and methods: In this prospective experiment, a total of 15 stone samples collected from patients who underwent stone extraction were used in each concentration (4 mg/ml, 2 mg/ml, and 1 mg/ml) of the O. stamineus extract and control solution. The effects of pH change in the chemolysis of the stones were assessed using the O. stamineus extract 4 mg/ml under pH 7 and 8. Results: The percentage weight reduction of calcium oxalate stone was highest in the 4 mg/ml concentration. O. stamineus extract 4 mg/ml showed a better effect in terms of chemolytic action on calcium oxalate stone than the potassium citrate solution (70% vs. 41%). Regarding the calcium oxalate stone, the percentage weight reduction has shown about 70% in the pH 5, 48% in pH 7, and <10% in pH 8. The percentage weight reduction of uric acid stone was determined as 47%, 11%, and 14% for pH 5, 7, and 8, respectively. The percentage weight reduction of combination stone was 40%, 60%, and 80% in the pH 5, pH 7, and pH 8, respectively. Data analysis showed that the percentage weight reduction of combination stone was significantly different between acidic, neutral, and alkaline conditions (P = 0.027). Conclusions: In this in vitro study, we are able to show that O. stamineus water extract do have some dissolving capability of urinary stones.
In vivo investigation of the inhibitory effect of Peganum harmala L. and its major alkaloids on ethylene glycol-induced urolithiasis in rats

Abstract:Ethnopharmacological relevance: Peganum harmala L. is a traditional medicinal plant used for centuries in folk medicine. It has a wide array of therapeutic attributes, which include hypoglycemic, sedative, anti-inflammatory, and antioxidant properties. The fruit decoction of this plant was claimed by Avicenna as traditional therapy for urolithiasis. Also, P. harmala seed showed a clinical reduction in kidney stone number and size in patients with urolithiasis. Aim of the study: In light of the above-mentioned data, the anti-urolithiatic activities of the seed extracts and the major β-carboline alkaloids of P. harmala were investigated. Materials and methods: Extraction, isolation, and characterization of the major alkaloids were performed using different chromatographic and spectral techniques. The in vivo anti-urolithiatic action was evaluated using ethylene glycol (EG)-induced urolithiasis in rats by studying their mitigating effects on the antioxidant machinery, serum toxicity markers (i.e. nitrogenous waste, such as blood urea nitrogen, uric acid, urea, and creatinine), minerals (such as Ca, Mg, P, and oxalate), kidney injury marker 1 (KIM-1), and urinary markers (i.e. urine pH and urine output). Results: Two major alkaloids, harmine (P1) and harmalacidine HCl (P2), were isolated and in vivo evaluated alongside the different extracts. The results showed that P. harmala and its constituents/fractions significantly reduced oxidative stress at 50 mg/kg body weight, p.o., as demonstrated by increased levels of glutathione (GSH), glutathione reductase (GR), glutathione peroxidase (GPx), and catalase (CAT) in kidney homogenate as compared to the EG-treated group. Likewise, the total extract, pet. ether fraction, n-butanol fraction, and P1, P2 alleviated malondialdehyde (MDA) as compared to the EG-treated group. Serum toxicity markers like blood urea nitrogen (BUN), creatinine, uric acid, urea, kidney injury molecule-1 (Kim-1), calcium, magnesium, phosphate, and oxalate levels were decreased by total extract, pet. ether fraction, n-butanol fraction, P1, and P2 as compared to the EG-treated group. Inflammatory markers like NFκ-B and TNF-α were also downregulated in the kidney homogenate of treatment groups as compared to the EG-treated group. Moreover, urine output and urine pH were significantly increased in treatment groups as compared to the EG-treated group deciphering anti-urolithiatic property of P. harmala. Histopathological assessment by different staining patterns also supported the previous findings and indicated that treatment with P. harmala caused a gradual recovery in damaged glomeruli, medulla, interstitial spaces and tubules, and brown calculi materials as compared to the EG-treated group. Conclusion: The current research represents scientific evidence on the use of P. harmala and its major alkaloids as an effective therapy in the prevention and management of urolithiasis.
Rat pineal alpha 1-adrenoceptors: identification and characterization using [125I]iodo-2-[beta-(4-hydroxyphenyl)-ethylaminomethyl]tetralone.

Abstract:[125I]Iodo-2-[beta-(4-hydroxyphenyl)-ethylaminomethyl]tetralone ([125I]HEAT), a selective, high affinity, high specific activity alpha 1-adrenoceptor ligand, is used to characterize alpha-adrenoceptors in the rat pineal gland. Binding of [125I]HEAT to membranes is rapid [association rate constant (Kon) = 3.1 nM-1 min-1] and readily reversible either by 100-fold dilution or by addition of excess unlabeled HEAT [apparent dissociation rate constant (Koff) = 0.153 min-1). Saturation experiments indicate a single class of noncooperative binding sites with an equilibrium binding constant (KD) of 41 +/- 9 pM and a Bmax of 399 +/- 63 fmol/mg protein. The relative potency of a number of adrenoceptor agonists and antagonists in competing with [125I] HEAT indicates the receptor is an alpha 1-subtype. In addition, inhibition of binding is stereospecific; (-)epinephrine and norepinephrine are more than 100-fold more potent than their (+)isomers. The identification of alpha 1-adrenoceptors in the pineal gland is consistent with evidence indicating a role for these receptors in the regulation of melatonin synthesis and phosphatidylinositol turnover.
Dietary Plants for the Prevention and Management of Kidney Stones: Preclinical and Clinical Evidence and Molecular Mechanisms

Abstract:Kidney stones are one of the oldest known and common diseases in the urinary tract system. Various human studies have suggested that diets with a higher intake of vegetables and fruits play a role in the prevention of kidney stones. In this review, we have provided an overview of these dietary plants, their main chemical constituents, and their possible mechanisms of action. Camellia sinensis (green tea), Rubus idaeus (raspberry), Rubia cordifolia (common madder), Petroselinum crispum (parsley), Punica granatum (pomegranate), Pistacia lentiscus (mastic), Solanum xanthocarpum (yellow-fruit nightshade), Urtica dioica (stinging nettle), Dolichos biflorus (horse gram), Ammi visnaga (khella), Nigella sativa (black-cumin), Hibiscus sabdariffa (roselle), and Origanum vulgare (oregano) have received considerable interest based on scientific evidence. Beside these dietary plants, phytochemicals-such as catechin, epicatechin, epigallocatechin-3-gallate, diosmin, rutin, quercetin, hyperoside, and curcumin-as antioxidant dietary phyto-phenols were found to be effective for the prevention of urolithiasis (the process of stone formation in the urinary tract). The main underlying mechanisms of these dietary plants and their isolated phytonutrients in the management of urolithiasis include diuretic, antispasmodic, and antioxidant activity, as well as an inhibitory effect on crystallization, nucleation, and aggregation of crystals. The results as presented in this review demonstrate the promising role of dietary plants and phytophenols in the prevention and management of kidney stones. Further investigations are required to confirm the safety and efficacy of these compounds.

Targeting urinary calcium oxalate crystallization with inulin-type AOFOS from Aspidopterys obcordata Hemsl. for the management of rat urolithiasis

Abstract:Ethnopharmacological relevance: Calcium oxalate crystals play a key role in the development and recurrence of kidney stones (also known as urolithiasis); thus, inhibiting the formation of these crystals is a central focus of urolithiasis prevention and treatment. Previously, we reported the noteworthy in vitro inhibitory effects of Aspidopterys obcordata fructo oligosaccharide (AOFOS), an active polysaccharide of the traditional Dai medicine Aspidopterys obcordata Hemsl. (commonly known as Hei Gai Guan), on the growth of calcium oxalate crystals. Aim of the study: To investigated the effectiveness and mechanism of AOFOS in treating kidney stones. Materials and methods: A kidney stones rats model was developed, followed by examining AOFOS transport dynamics and effectiveness in live rats. Additionally, a correlation between the polysaccharide and calcium oxalate crystals was studied by combining crystallization experiments with density functional theory calculations. Results: The results showed that the polysaccharide was transported to the urinary system. Furthermore, their accumulation was inhibited by controlling their crystallization and modulating calcium ion and oxalate properties in the urine. Consequently, this approach helped effectively prevent kidney stone formation in the rats. Conclusions: The present study emphasized the role of the polysaccharide AOFOS in modulating crystal properties and controlling crystal growth, providing valuable insights into their potential therapeutic use in managing kidney stone formation.
Cymbopogon proximus and Petroselinum crispum seed ethanolic extract/Gum Arabic nanogel emulsion: Preventing ethylene glycol and ammonium chloride-induced urolithiasis in rats

Abstract:Urolithiasis is a prevalent urological disorder that contributes significantly to global morbidity. This study aimed to assess the anti-urolithic effects of Cymbopogon proximus (Halfa Bar) and Petroselinum crispum (parsley) seed ethanolic extract /Gum Arabic (GA) emulsion, and its nanogel form against ethylene glycol (EG) and ammonium chloride (AC)-induced experimental urolithiasis in rats. Rats were divided into four groups: group 1 served as the normal control, group 2 received EG with AC in drinking water for 14 days to induce urolithiasis, groups 3 and 4 were orally administered emulsion (600 mg/kg/day) and nanogel emulsion (600 mg/kg/day) for 7 days, followed by co-administration with EG and AC in drinking water for 14 days. Urolithiatic rats exhibited a significant decrease in urinary excreted magnesium, and non-enzymic antioxidant glutathione and catalase activity. Moreover, they showed an increase in oxalate crystal numbers and various urolithiasis promoters, including excreted calcium, oxalate, phosphate, and uric acid. Renal function parameters and lipid peroxidation were intensified. Treatment with either emulsion or nanogel emulsion significantly elevated urolithiasis inhibitors, excreted magnesium, glutathione levels, and catalase activities. Reduced oxalate crystal numbers, urolithiasis promoters' excretion, renal function parameters, and lipid peroxidation while improving histopathological changes. Moreover, it decreased renal crystal deposition score and the expression of Tumer necrosis factor-α (TNF-α) and cleaved caspase-3. Notably, nanogel emulsion showed superior effects compared to the emulsion. Cymbopogon proximus (C. proximus ) and Petroselinum crispum (P. crispum ) seed ethanolic extracts/GA nanogel emulsion demonstrated protective effects against ethylene glycol induced renal stones by mitigating kidney dysfunction, oxalate crystal formation, and histological alterations.

Abstract:In the phytochemical screening, it was found that Dolichos Biflorus seed extract contained the presence of tannins steroids, protein, flavonoids, terpenoids, mucilage, saponin, and carbohydrate, but no alkaloids fixed oil. So, this plant has highly diuretic activity. Which can use for the treatment of urolithiasis. Objective: The Present study aims to investigate the anti-Urolithiatic activity of Dolichos Biflorus methanolic seed extract in a rat model. Methods: Dolichos Biflorus Seeds Extract were evaluated for anti-Urolithiatic activity. Urolithiasis has been induced in Wistar rats through Ethylene glycol (0.75%v/v, p.o). Upon completion of the experiment, animals in all groups have been sacrificed and biochemical parameters such as; calcium, creatinine, Phosphorus, Uric acid, Alkaline Phosphatase BUN (Blood Urea Nitrogen), and histology of Kidney have been observed. Furthermore, Potassium, Oxalate levels, and Alanine Amino Transferases were measured. Results: The extract of Dolichos Biflorus was proven to be safe in the toxicity findings. It has been shown that in-vivo significant effect of plant extract was able to manage the urolithiatic markers such as calcium, creatinine, phosphorus, uric acid, alkaline phosphatase, BUN (Blood Urea Nitrogen), and potassium. In the urolithiatic rats it has been observed that abundant crystal depositions, renal epithelial cells had more tubular dilatation and damage shown by large spaces in the tissue, renal stone deposition damages the renal tissue and detoriate the renal function. Urolithiatic markers such as calcium, creatinine, phosphorus, uric acid, alkaline phosphatase, BUN (Blood Urea Nitrogen), and potassium were normal in animals receiving plant extract and prevent renal cell injury. Conclusion: All its findings and phytoconstituents existing inside the extract must stay the viable chemical materials involved in the prevention of Urolithiatic.

Abstract:Objective: This study aimed to evaluate the potential antiurolithiatic activity of the aqueous extract of *Catharanthus roseus* in a rat model of ethylene-glycol-induced urolithiasis. Methods: Thirty male Wistar rats were divided into five groups: Group I served as the control, Group II received ethylene glycol (0.75% v/v) in drinking water for the induction of urolithiasis, Group III received standard treatment (cystone), and Group IV was treated with the aqueous extract of Catharanthus roseus (200 mg/kg). Throughout the experimental period, various parameters such as urinary calcium, phosphate, oxalate levels, and kidney histopathology were assessed to evaluate the antiurolithiatic potential of Catharanthus roseus. Results: The results demonstrated a significant reduction in urinary calculi formation, lowered calcium and oxalate excretion, and ameliorated histological changes in the kidneys of rats treated with the aqueous extract of Catharanthus roseus. Conclusion: Our findings suggest that the aqueous extract of *Catharanthus roseus* possesses promising antiurolithiatic activity, potentially attributed to its ability to modulate urinary stone-forming parameters. Further studies are warranted to elucidate the underlying mechanisms and explore the therapeutic potential of *Catharanthus roseus* in managing urolithiasis.
Total flavonoids of Desmodium styracifolium attenuates the formation of hydroxy-l-proline-induced calcium oxalate urolithiasis in rats

Abstract:Desmosium styracifolium (D. styracifolium), which is considered as a Chinese herbal medicine, has been reported to treat the kidney stone diseases. However, the potential phytochemically active components and the underlying mechanisms associated with its efficacy in targeting urolithiasis remain to be elucidated. This study aims to investigate the anti-urolithiatic effect of total flavonoids of D. styracifolium (TFDS) on calcium oxalate (CaOx) renal stones in Sprague–Dawley rats. Animal models of CaOx urolithiasis were established in male Sprague–Dawley rats by adding 5% w/w hydroxy-l-proline (HLP) in regular rat chow. The TFDS orally at 100, 400 mg/kg, respectively, were administered along with HLP for 28 days. At the end of 28 days of treatment, urine and serum samples were collected for crystalluria determination and various biochemical analysis. Kidney tissues were isolated and processed for antioxidant parameters measurement and histopathological examinations. HLP-induced hyperoxaluria alone reliably caused CaOx nephrolithiasis in rats. We showed that TFDS significantly reduced crystalluria and CaOx crystal deposits in the kidney sections as compared to untreated HLP group. Also, TFDS was observed to decrease urinary oxalate excretion, alleviate the pro-acidosis condition, improve the impaired renal functions and renal epithelial cell injury. Moreover, TFDS protected against the oxidative stress changes via reducing MDA content, increasing CAT and GSH-Px activities in renal homogenate, as well as attenuating the expression of MCP-1, OPN and TGF-β proteins. These results indicated that TFDS had beneficial effect on inhibition of CaOx formation in the rat kidney probably through a combination of antioxidant, anti-inflammatory, urine alkalinizing activities, and lowering the concentration of urinary stone-forming constituents. Thus, TFDS might have clinical implications in preventing oxidative renal cell injury and, ultimately, kidney stone formation. The data provide a rationale for the medicinal use of TFDS in nephrolithiasis and identify this agent as a potential source of new antiurolithic drugs.

Metabolic and Network Pharmacological Analyses of the Therapeutic Effect of Grona styracifolia on Calcium Oxalate-Induced Renal Injury

Abstract:Grona styracifolia (Osbeck) Merr. (GS), a popular folk medicine, is clinically applied to treat nephrolithiasis. In this study, a urinary metabolic analysis was performed in a mouse model of renal calcium oxalate (CaOx) crystal deposition to identify the differentially altered metabolites in mice with oxalate-induced renal injury and explore the therapeutic mechanisms of GS against nephrolithiasis. Twenty-four mice were randomly divided into the control, oxalate and GS-treated groups. A metabolomics approach based on ultra-high-performance liquid chromatography coupled with quadrupole-time-of-flight mass spectrometry (UHPLC-Q-TOF/MS) was used to analyze the metabolic profiles of the urine samples. In addition, network pharmacology analysis was performed with different databases. As a result, the protective effects of GS were verified by measuring biochemical parameters and detecting crystal deposition. Fifteen metabolites were identified as the differentially altered metabolites in mice with crystal-induced renal injury. Most were involved in amino acid and fatty acid metabolism. Thirteen of these metabolites showed a reversal trend following GS treatment. A component-target-metabolite network was further constructed and nine overlapping target proteins of GS and the differentially altered metabolites were discovered. Among these proteins, the expression of estrogen receptor 2 (ESR2) in renal tissues was significantly down-regulated while androgen receptor (AR) expression was obviously increased in the oxalate group compared with the control group. These changes were reversed by the GS treatment. In conclusion, GS exerts its therapeutic effect by regulating multiple metabolic pathways and the expression of ESR and AR in mice with oxalate-induced renal injury.

Plant-based therapies for urolithiasis: a systematic review of clinical and preclinical studies

Abstract:Purpose: Urolithiasis, the formation of kidney stones, is a common and severe condition. Despite advances in understanding its pathophysiology, affordable treatment options are needed worldwide. Hence, the interest is in herbal medicines as alternative or supplementary therapy for urinary stone disease. This review explores the use of plant extracts and phytochemicals in preventing and treating urolithiasis. Methods: Following PRISMA standards, we systematically reviewed the literature on PubMed/Medline, focusing on herbal items evaluated in in vivo models, in vitro studies, and clinical trials related to nephrolithiasis/urolithiasis. We searched English language publications from January 2021 to December 2023. Studies assessing plant extracts and phytochemicals' therapeutic potential in urolithiasis were included. Data extracted included study design, stone type, plant type, part of plant used, solvent type, main findings, and study references. Results: A total of 64 studies were included. Most studies used ethylene glycol to induce hyperoxaluria and nephrolithiasis in rat models. Various extraction methods were used to extract bioactive compounds from different plant parts. Several plants and phytochemicals, including Alhagi maurorum, Aerva lanata, Dolichos biflorus, Cucumis melo, and quercetin, demonstrated potential effectiveness in reducing stone formation, size, and number. Conclusions: Natural substances offer an alternative or supplementary approach to current treatments, potentially reducing pain and improving the quality of life for urolithiasis patients. However, further research is needed to clarify their mechanisms of action and optimize their therapeutic use. The potential of plant-based therapies in treating urolithiasis is promising, and ongoing research is expected to lead to treatment advancements benefiting patients globally.

Abstract:Allium jesdianum, a plant utilized in traditional Iranian medicine, has been historically recognized for its efficacy in countering urolithiasis and alleviating kidney pain. In this study, we investigated the hydroalcoholic extract of A. jesdianum for its potential inhibitory effects on calcium oxalate crystallization (in vitro). The study employed spectrophotometric analysis and microscopic observation to quantify sodium oxalate-induced crystallization, nucleation, and aggregation. The extract of A. jesdianum at concentration of 32 mg/mL exhibited significant potential in inhibiting crystal growth, aggregation, and reducing crystal particle size. Phytochemical analysis of the extract disclosed the presence of three known phenolic and two identified steroid compounds, offering initial insights into its bioactive constituents. A. jesdianum emerges as a promising candidate for the development of plant-based oxalate anticrystallization drugs. The therapeutic mechanisms might be attributed to pain control and the presence of phenolic compounds acting as free radical scavengers, disrupting stone formation signaling pathways. However, further investigation is required to elucidate specific chemical structures related to these activities.
