Abstract:The potential of forskolin as a research tool is just beginning to be realized. Its use has revealed further subtleties to the control of cyclic AMP generation by adenylate cyclase in both solubilized and membrane preparations and in intact cells. It appears that an effect of forskolin will become one of the criteria for implicating adenylate cyclase in any physiological or biochemical response or, conversely, in ruling out an involvement of cyclic AMP. Clinical applications of forskolin as a hypotensive, spasmolytic, lipolytic, or antithrombotic agents or for the treatment of glaucoma or cardiac insufficiency remain other challenges for the future.
The feasibility and acceptabililty of video directly observed therapy (v-DOT) for achieving mastery of inhaler and nasal spray technique: A qualitative exploration.

Abstract:Background: A qualitative exploration of a randomised pilot study to compare a novel augmented teaching method (video directly observed therapy (v-DOT)) with standard training in achieving mastery of inhaler+/-nasal spray technique. Our aim was to evaluate the feasibility and acceptability of using v-DOT. Methods: Semi-structured interviews conducted 3 months after mastery of technique achieved. Interviews conducted with parent(s) of study participants +/- study participant (age dependent). Results: 12 semi-structured interviews have been conducted to date. A selection of illustrative quotes are shown. Parents and participants found the experience rewarding. Part of this appeared to result from receiving itertive feedback: 'Getting feedback and having time to kind of work on the steps was helpful.' Parents also felt that the intervention helped in assuming more responsibility and building confidence in asthma management: 'He definitely has become more confident in doing it himself. I think he also has a good insight now into how serious asthma is.' 'It was educational, and it held you accountable because that was a new habit we were starting.' Parents were also encouraged that the intervention translated into clinical benefit: ' Well, I can kind of count on one hand the number of time he has used the blue inhaler from we have started this. So something is working by just giving him the brown everyday and having a routine now.' Conclusion: v-DOT was deemed acceptable and feasible by study participants. Parents and participants felt that it helped to build confidence in optimising technique and translated into clinical benefit.

Abstract:Background: Adherence to pharmacotherapy and use of the correct inhaler technique are important basic principles of asthma management. Video- or remote-direct observation of therapy (v-DOT) could be a feasible approach to facilitate monitoring and supervising therapy, supporting the delivery of standard care. Objective: To explore the utility and the feasibility of v-DOT to monitor inhaler technique and adherence to treatment in adults attending the asthma outpatient service in a tertiary hospital in Northern Ireland. Method: The project evaluated use of the technology with 10 asthma patients. Patient and clinician feedback was obtained, in addition to measures of patient engagement and disease-specific clinical markers to assess the feasibility and utility of v-DOT technology in this group of patients. Results: The engagement rate with v-DOT for participating patients averaged 78% (actual video uploads vs expected video uploads) over a median 7 week usage period. Although 50% of patients reported a technical issue at some stage during the usage period, all patients and clinicians reported that the technology was easy to use and that they were satisfied with the outcomes. A range of positive impacts were observed, including optimised inhaler technique and an observed improvement in lung function. An increase in asthma control test scores aligned with clinical aims to promote adherence and alleviate symptoms. Conclusion: The v-DOT technology was shown to be a feasible method of assessing inhaler technique and monitoring adherence in this small group of adult asthma patients. A range of positive impacts for participating patients and clinicians were observed. Not all patients invited to join the project agreed to participate or engage with using the technology, highlighting that in this setting, digital modes of delivering care provide only one of the approaches in the necessary "tool kit" for clinicians and patients.

Abstract:PURPOSE:As stated in the Global Initiative for Asthma, there are still some asthmatic patients who have not achieved asthma control. Mobile is a useful tool for asthma management. We aimed to compare the advantages of mobile management with traditional management in improving adherence and control of asthma.METHODS:In this prospective, multicentre, randomized, controlled and parallel-group study, we enrolled patients with poor adherence and uncontrolled asthma at 32 hospitals in 28 provinces in China. Patients were randomly assigned to the mobile management or traditional management groups for 12 months. The primary endpoint was the proportion of patients with good adherence (Medication Adherence Report Scale for Asthma [MARS-A] score ≥ 45) for 6 months. This study is registered at ClinicalTrials.gov (NCT02917174).RESULTS:Between April 2017 and April 2018, 923 patients were eligible for randomization (mobile group, n = 461; traditional group, n = 462). Dropout was 84 (18.2%) in the mobile management group and 113 (24.4%) patients in the traditional management group. The proportion of patients with good adherence was significantly higher in the mobile management group than in the traditional management group (66.0% vs. 58.99%, P = 0.048). The mobile management group showed higher mean MARS-A score (at 1, 6, 9, and 12 months) and asthma control test scores (at 6 and 9 months), and lower total lost rate to follow-up within 12 months than the traditional management group.CONCLUSIONS:Mobile asthma management can improve adherence and asthma control compared to traditional management.TRIAL REGISTRATION:ClinicalTrials.gov Identifier: NCT02917174.
Novel electronic adherence monitoring devices in children with asthma: a mixed-methods study

Abstract:Introduction Adherence monitoring to inhaled corticosteroids is an essential component of asthma management. Electronic monitoring devices (EMD) provide objective data on date, time and number of actuations. However, most give no information on inhalation. Novel EMD (NEMD) platforms have the potential to monitor both activation and inhalation. Aim To assess the feasibility of NEMDs, in terms of usability, acceptability to patients and healthcare professionals and accuracy. Methods This was an open-label, prospective, mixed-methods, pragmatic randomised study. Children with asthma attending specialist tertiary care were randomised to one of four NEMD: Remote Directly Observed Therapy (R-DOT), Hailie Smartinhaler, INhaler Compliance Assessment device (INCA) and the Rafi-tone App. Following monitoring, participants were invited to focus groups or one-to-one interviews. Usability and acceptability were evaluated using themes identified from the focus groups and interviews. Adherence accuracy was determined using adherence data from each NEMD. Results Thirty-five children were recruited; 18 (51%), (11 males, median age 13.5 (7–16) years) completed monitoring, 14 (78%) provided feedback. Participants identified various features such as ease of use and minimal effort as desirable criteria for an NEMD. The Hailie and INCA fulfilled these criteria and were able to record both actuation and inhalation. Negative themes included a ‘Big Brother’ effect and costs. Conclusion There was no ‘one size fits all’, as participants identified advantages and disadvantages for each NEMD. Devices that can easily calculate adherence to activation and inhalation have the potential to have greatest utility in clinical practice. Each NEMD has different functionality and therefore choice of platform should be determined by the needs of the patient and healthcare professional.

Abstract:Aim of study: This research study aims to compare between two different counseling approaches; traditional verbal counseling vs. advanced counseling (in which we used the acoustic Flo-tone training device and its smartphone application combined with traditional verbal counseling) to determine the most beneficial counseling approach for asthmatic children who use metered-dose inhaler (MDI) with spacers concerning inhalation duration and inhalation technique mistakes. Methods: A total of 100 asthmatic children (8-18) years old were randomized into two groups (a control group, and an advanced group). Each group included 50 subjects. Every subject received 3 counseling meetings, one each month. Asthmatic children in the control group were trained on inhalation technique from MDI + spacer verbally (traditional counseling), while asthmatic children in advanced group were trained on inhalation technique from MDI + spacer verbally and by advanced counseling (whistling Flo-tone + smartphone application). At each visit mistakes in inhalation technique steps were; detected, corrected, and recorded and the inhalation duration was measured for every child in each group. Results: In both study groups, the total mean number of inhalation technique mistakes decreased significantly (p < 0.05) from visit 2, also the total mean inhalation durations in seconds showed a significant increase (p < 0.05) from visit 2. A significant (p < 0.05) reduction in the total mean number of mistakes and a significant (p < 0.05) increase in total mean inhalation durations were observed from visit 2 in advanced group compared to control group. Conclusion: Combination between traditional verbal and advanced counseling methods resulted in significant (P < 0.05) improvements in the number of inhalation technique mistakes and inhalation durations from MDI with spacer in children compared to using traditional verbal counseling alone.

Abstract:Background: The albuterol Digihaler (albuterol 90 μg/dose) transmits data wirelessly to a smart device application, which synchronizes with a Digital Health Platform to store and transfer data to a web-based Dashboard. The Reliever Digihaler System (RDS) comprises the albuterol Digihaler, application, Digital Health Platform and Dashboard. This allows patients and health care professionals to review reliever inhaler usage and inhalation quality to aid clinical decision making. Objective: To demonstrate the effectiveness, as measured by change in asthma control, of the RDS compared with standard of care. Methods: In this 12-week study, participants aged 13 years or older with suboptimal asthma control (Asthma Control Test [ACT] score < 19) were randomized to use either RDS or standard of care albuterol reliever inhalers. The health care professionals were recommended at study start to check each participant's inhalation data (including inhalation quantity and quality parameters) 1 or more times per week. Primary outcome was the proportion of participants achieving clinically meaningful improvement in asthma control (ACT score ≥ 20 at week 12 and/or increase ≥ 3 units from baseline). Bayesian statistical analysis provided a posterior probability distribution for odds ratios with corresponding credible intervals. Results: Participants using the RDS (n = 167) had an 85.3% probability of greater odds of clinically meaningful asthma control improvements than those using SoC (n = 166) after 3 months (mean odds ratio 1.33; 95% credible interval 0.813-2.050). Conclusions: In this study, participants using the RDS had greater odds of clinically meaningful improvements in asthma control versus SoC after 3 months. Further investigation of the potential of the RDS to help improve asthma management is warranted.
Assessing and addressing barriers towards MDI use in acute asthma exacerbations at a tertiary pediatric ED in the United Arab Emirates

Abstract:Asthma is one of the most common causes of emergency department (ED) visits in children. Therapy delivered through a meter dose inhaler with spacer (MDI + S) is equally as effective as nebulization in mild and moderate asthma exacerbations but was not routinely prescribed in the ED at the largest tertiary center for pediatrics in the United Arab Emirates (UAE). Phase 1 of this cohort study involved a validated survey to evaluate physicians' knowledge, attitudes and perceptions towards MDI therapy. While 62% of physicians reported that MDI + S was equally effective as nebulizers and 82% believed that they had sufficient knowledge with regard to its use, only 28% prescribed it. Perceived barriers to change of practice included: Lack of clinical practice guidelines (CPG), poor knowledge amongst nurses and physicians, caregivers' reluctance and a difficult prescription process. Phase 2 consisted of administering the same survey after completing interventions to address the aforementioned barriers. Comparisons were made between the subgroups within phase 1 and statistically significant differences were noted with a p value < .05. The number of physicians who prescribed MDI + S increased from 28% to 41% (p value = .046). Moreover, physicians who believed that convincing parents to use MDI + S therapy would be easy, increased from 35% to 66% (p value < .0001). In conclusion, more physicians reported prescribing MDI + S in Phase 2 while concerns about barriers that exist to change in practice remained similar in both phases showing that consistent and prolonged advocacy is required to achieve long-term compliance.
Prospective single-arm interventional pilot study to assess a smartphone-based system for measuring and supporting adherence to medication

Abstract:OBJECTIVES: Suboptimal medication adherence for infectious diseases such as tuberculosis (TB) results in poor clinical outcomes and ongoing infectivity. Directly observed therapy (DOT) is now standard of care for TB treatment monitoring but has a number of limitations. We aimed to develop and evaluate a smartphone-based system to facilitate remotely observed therapy via transmission of videos rather than in-person observation.DESIGN: We developed an integrated smartphone and web-based system (Mobile Interactive Supervised Therapy, MIST) to provide regular medication reminders and facilitate video recording of pill ingestion at predetermined timings each day, for upload and later review by a healthcare worker. We evaluated the system in a single arm, prospective study of adherence to a dietary supplement. Healthy volunteers were recruited through an online portal. Entry criteria included age ≥21 and owning an iOS or Android-based device. Participants took a dietary supplement pill once, twice or three-times a day for 2 months. We instructed them to video each pill taking episode using the system.OUTCOME: Adherence as measured by the smartphone system and by pill count.RESULTS: 42 eligible participants were recruited (median age 24; 86% students). Videos were classified as received-confirmed pill intake (3475, 82.7% of the 4200 videos expected), received-uncertain pill intake (16, <1%), received-fake pill intake (31, <1%), not received-technical issues (223, 5.3%) or not received-assumed non-adherence (455, 10.8%). Overall median estimated participant adherence by MIST was 90.0%, similar to that obtained by pill count (93.8%). There was a good relationship between participant adherence as measured by MIST and by pill count (Spearmans rs 0.66, p<0.001).CONCLUSIONS: We have demonstrated the feasibility, acceptability and accuracy of a smartphone-based adherence support and monitoring system. The system has the potential to supplement and support the provision of DOT for TB and also to improve adherence in other conditions such as HIV and hepatitis C.

Abstract:Background: Poor adherence to inhaled corticosteroids (ICS) is a significant challenge in pediatric asthma, contributing to health inequities. Text-message reminders for ICS therapy are an evidence-based approach that improves pediatric asthma medication adherence, yet has not been widely adopted into practice, partly due to lack of (1) participant input on design and implementation and (2) use of sustainable community linkages. Remote Asthma LinkTM (RAL) seeks to fill this gap as a school-linked text-message intervention wherein parents of children with poorly controlled asthma received daily, 2-way text-message reminders for preventive inhaler use. Responses were shared with school nurses who conducted remote check-ins with families. Enrolled children, largely from underserved backgrounds, experienced improvements in medication adherence and asthma health outcomes. While initial results were promising, we have yet to elicit participant input to refine the protocol for more widespread implementation. Objective: Examine participant perspectives on barriers and facilitators of RAL implementation. Methods: Semistructured interviews were conducted May–June 2022 with intervention participants: 10 parents, 7 school nurses, and 4 pediatric providers ( n = 21) until thematic saturation was reached. Interview transcripts were coded using thematic analysis. Results: Several facilitators for RAL implementation were identified, including ease of use and accessibility, personal connection to the school nurse, and receipt of a visual notification for habit formation. Barriers included challenges with school nurses reaching parents, poor understanding of program expectations, and lack of reimbursement structure. Participant-proposed solutions to barriers included utilizing alternate communication methods (eg, social media), educational sessions, and meeting with payors to consider reimbursement models. Conclusion: RAL is a school-linked text-message intervention demonstrating promise in improving outcomes and equity in asthma care. Key implementation facilitators, barriers, and proposed solutions will inform protocol adaptations to promote successful implementation of this and other text-message interventions into clinical practice.

Abstract:Purpose: Currently, 7.1 million children in the United States have asthma. Nonadherence to daily controller asthma medication is common, leading to more severe symptoms, overuse of rescue medication, and increased hospitalizations. The purpose of this study was to develop and evaluate the feasibility and acceptability of a novel mHealth management system composed of a sensored device, which is connected to mobile phone app that is designed to monitor and improve asthma medication adherence. Patients and methods: The asthma management system was designed using well-established behavioral theory. Seven adolescents aged 11-18 years were enrolled and given an adherence sensor, and four of those also received a mobile phone app with game features and reminders. Five patients completed the study, and one was lost to follow-up in each group. Mobile app users and their parents participated in focus groups to assess patient preferences. Feasibility was assessed by the ability of sensors to capture real-time medication data. Acceptability was assessed by patient questionnaire and focus group analysis. Results: Successful upload of real-time data from six of seven inhaler sensors to the HIPAA-compliant server demonstrates the feasibility of at-home patient monitoring using the sensor device. All three mobile app users who completed the study reported interest in continued use of the management system and would recommend the app to friends. Unstructured interviews and focus groups revealed that patients felt that the intervention helped their sense of asthma control. Conclusion: This study demonstrates the feasibility of using the sensor device to remotely monitor real-time medication usage, and user feedback demonstrates the acceptability of the intervention for patient use. The findings provide guidance for the improvement of study design and technology development. Further research is needed to assess the efficacy of the intervention.

Abstract:The kissing balloon technique is a well-documented approach used in coronary and peripheral artery diseases with bifurcation lesions. Intervention in either branch of a bifurcation lesion frequently leads to a snow ploughing effect with shifting of plaque, compromising the opposing branch. The kissing balloon technique, which means dilatation of the main branch and the side branch at the same time, could avoid plaque shifting and restore main branch geometry. Similar situations may be encountered in percutaneous transluminal angioplasty (PTA) of radiocephalic arteriovenous fistula (AVF) at the anastomosis site. Balloon dilatation of either radial artery or cephalic vein could result in plaque shifting to the opposing vessel. Here, we report a successful PTA using the kissing balloon technique for the anastomotic site in a patient with a radiocephalic AVF for haemodialysis (HD). Percutaneous transluminal angioplasty was performed in a 3 month old AVF for primary failure with inadequate blood flow during dialysis in a 56 year old man with a history of stroke, hypertension and uraemia. A 6.0-F sheath was inserted into the left radial artery. Fistulography revealed left radiocephalic AVF with diffuse stenosis over the arterial anastomosis and juxta-anastomotic cephalic vein. A bifurcation lesion was identified (Fig. 1a). The radial artery was wired with a 0.014 Shinobi wire and the cephalic vein with a 0.014 PT2 wire. The kissing balloon technique was used to dilate both arterial and venous lesions simultaneously, with a 20 mm ¥ 2.0 mm Maverick balloon (Fig. 1b, black arrow) and a 40 mm ¥ 3.0 mm Fox balloon, (Fig. 1b, white arrow), respectively. Abstract:Background United States (US) asthma medication adherence = 22%. Medication non-adherence impacts asthma control. Digital therapeutics (DTx) may improve medication adherence and asthma control. Objectives Main objectives: determine if Aptar Digital Health (ADH) respiratory platform, a DTx, improves asthma control and controller medication adherence and decreases rescue medication use. Methods Prospective, single cohort study. 104 adult participants utilized ADH respiratory platform (HeroTracker inhaler sensor and BreatheSmart mobile app) for 3-months. HeroTracker tracked controller medication adherence and rescue medication use. Asthma Control Test (ACT) and post-study platform survey were completed using BreathSmart app. Change in controller medication adherence, ACT score, and rescue medication use between baseline, T0 and 3-months, T3 were calculated. ADH respiratory platform feedback was analysed. Results ACT score increased significantly between T0 and T3 (T0: 16.5 ± 4.7; T0 to T3: + 2.8, 95% CI (2.0, 3.6), p <0.001), indicating trend in improved asthma control. Rescue medication use decreased 44% between T0 and T3 ((95% CI: 14.1, 63.5), p=.008). Controller medication adherence at T3 was 45% higher than US asthma medication adherence. 40 participants completed platform feedback survey. 82.5% reported platform was very/somewhat easy to use. 92.5% reported alerts were very/somewhat helpful. Mean platform score = 7.8 (1-10; 10=high). Conclusions ADH respiratory platform use resulted in improved asthma control and controller medication adherence and decreased rescue medication use. ADH platform feedback was positive.

Abstract:Beta-amino alcohols derived from alpha-amino acids have been extensively used as a powerful source of chirality. Transforming the alcohol moiety into a good leaving group has allowed the rearrangement of these beta-amino alcohols and the introduction of a large number of nucleophiles through the anchimeric participation of the nitrogen atom. An overview on the recent progress realized on the rearrangement of these beta-amino alcohols in the presence of (CF(3)CO)(2)O and H(2)SO(4) is reported.

Abstract:Cloud storage is considered to be the most critical factor in decision making for users as it largely scales down the infrastructure in terms of size, cost and design. Considering factors such as local storage c ost, maintenance a single server model can support multi ple users on a needed basis. This raises concerns f or integrity verification i.e., assuring the correctne ss of the data stored available in cloud. The propo sed auditing algorithm suggests and investigates digita l signature for integrity verification. A Modified Version of Elliptic curve digital Signature Algorit hm is proposed for auditing the task. The main focu s of this study is to address problems such as privacy p reserving, public auditing. In addition, the perfor mance of the auditing task is optimized. Data dynamics ha ve been modeled through various data operations suc h as block insertion, deletion and block modification . Extensive theoretical and experimental analysis presented in the paper shows that security, perform ance of the proposed algorithm are improved in term s of verification time of the auditing process.
