Abstract:Background: Adherence to pharmacotherapy and use of the correct inhaler technique are important basic principles of asthma management. Video- or remote-direct observation of therapy (v-DOT) could be a feasible approach to facilitate monitoring and supervising therapy, supporting the delivery of standard care. Objective: To explore the utility and the feasibility of v-DOT to monitor inhaler technique and adherence to treatment in adults attending the asthma outpatient service in a tertiary hospital in Northern Ireland. Method: The project evaluated use of the technology with 10 asthma patients. Patient and clinician feedback was obtained, in addition to measures of patient engagement and disease-specific clinical markers to assess the feasibility and utility of v-DOT technology in this group of patients. Results: The engagement rate with v-DOT for participating patients averaged 78% (actual video uploads vs expected video uploads) over a median 7 week usage period. Although 50% of patients reported a technical issue at some stage during the usage period, all patients and clinicians reported that the technology was easy to use and that they were satisfied with the outcomes. A range of positive impacts were observed, including optimised inhaler technique and an observed improvement in lung function. An increase in asthma control test scores aligned with clinical aims to promote adherence and alleviate symptoms. Conclusion: The v-DOT technology was shown to be a feasible method of assessing inhaler technique and monitoring adherence in this small group of adult asthma patients. A range of positive impacts for participating patients and clinicians were observed. Not all patients invited to join the project agreed to participate or engage with using the technology, highlighting that in this setting, digital modes of delivering care provide only one of the approaches in the necessary "tool kit" for clinicians and patients.

Abstract:Background: A qualitative exploration of a randomised pilot study to compare a novel augmented teaching method (video directly observed therapy (v-DOT)) with standard training in achieving mastery of inhaler+/-nasal spray technique. Our aim was to evaluate the feasibility and acceptability of using v-DOT. Methods: Semi-structured interviews conducted 3 months after mastery of technique achieved. Interviews conducted with parent(s) of study participants +/- study participant (age dependent). Results: 12 semi-structured interviews have been conducted to date. A selection of illustrative quotes are shown. Parents and participants found the experience rewarding. Part of this appeared to result from receiving itertive feedback: 'Getting feedback and having time to kind of work on the steps was helpful.' Parents also felt that the intervention helped in assuming more responsibility and building confidence in asthma management: 'He definitely has become more confident in doing it himself. I think he also has a good insight now into how serious asthma is.' 'It was educational, and it held you accountable because that was a new habit we were starting.' Parents were also encouraged that the intervention translated into clinical benefit: ' Well, I can kind of count on one hand the number of time he has used the blue inhaler from we have started this. So something is working by just giving him the brown everyday and having a routine now.' Conclusion: v-DOT was deemed acceptable and feasible by study participants. Parents and participants felt that it helped to build confidence in optimising technique and translated into clinical benefit.

Abstract:The potential of forskolin as a research tool is just beginning to be realized. Its use has revealed further subtleties to the control of cyclic AMP generation by adenylate cyclase in both solubilized and membrane preparations and in intact cells. It appears that an effect of forskolin will become one of the criteria for implicating adenylate cyclase in any physiological or biochemical response or, conversely, in ruling out an involvement of cyclic AMP. Clinical applications of forskolin as a hypotensive, spasmolytic, lipolytic, or antithrombotic agents or for the treatment of glaucoma or cardiac insufficiency remain other challenges for the future.

Abstract:Beta-amino alcohols derived from alpha-amino acids have been extensively used as a powerful source of chirality. Transforming the alcohol moiety into a good leaving group has allowed the rearrangement of these beta-amino alcohols and the introduction of a large number of nucleophiles through the anchimeric participation of the nitrogen atom. An overview on the recent progress realized on the rearrangement of these beta-amino alcohols in the presence of (CF(3)CO)(2)O and H(2)SO(4) is reported.

Abstract:The incidence of pulmonary arterial hypertension (PAH) in 39 patients with angiographically proven chronic rheumatic mitral regurgitation (MR) is reported. The overall incidence of PAH were found to be 76.9%. There were 51.3% patients with mild, 15.4% with moderate and 10.2% with severe PAH. Pulmonary arterial hypertension was present in 93% of patients below 20 years of age. Clinical and radiological assessment of PAH did not show any correlation with the haemodynamic data. Electrocardiographic right ventricular hypertrophy was seen in 4, and biventricular hypertrophy in 5 patients. All of them had moderate to severe PAH. Echocardiographic left atrial (LA) size showed an inverse correlation with mean pulmonary artery pressure (PAP) (p less than 0.001), r = 0.63). Presence of mid-systolic notch in pulmonary valve echoes indicated mean PAP more than 37mmHg (mean 54.1 +/- 19.1). Fifty-six percent patients with mid-systolic notch had moderate to severe PAH. Sixteen (94%) out of 17 patients with raised left ventricular end diastolic pressure had elevated mean PAP, and 30% of them had moderate to severe PAH. This data shows that pulmonary hypertension is relatively common in our patients with chronic rheumatic MR, and that the severity of PAH correlates well.

Abstract:The kissing balloon technique is a well-documented approach used in coronary and peripheral artery diseases with bifurcation lesions. Intervention in either branch of a bifurcation lesion frequently leads to a snow ploughing effect with shifting of plaque, compromising the opposing branch. The kissing balloon technique, which means dilatation of the main branch and the side branch at the same time, could avoid plaque shifting and restore main branch geometry. Similar situations may be encountered in percutaneous transluminal angioplasty (PTA) of radiocephalic arteriovenous fistula (AVF) at the anastomosis site. Balloon dilatation of either radial artery or cephalic vein could result in plaque shifting to the opposing vessel. Here, we report a successful PTA using the kissing balloon technique for the anastomotic site in a patient with a radiocephalic AVF for haemodialysis (HD). Percutaneous transluminal angioplasty was performed in a 3 month old AVF for primary failure with inadequate blood flow during dialysis in a 56 year old man with a history of stroke, hypertension and uraemia. A 6.0-F sheath was inserted into the left radial artery. Fistulography revealed left radiocephalic AVF with diffuse stenosis over the arterial anastomosis and juxta-anastomotic cephalic vein. A bifurcation lesion was identified (Fig. 1a). The radial artery was wired with a 0.014 Shinobi wire and the cephalic vein with a 0.014 PT2 wire. Abstract:Impressive advances in inhalation therapy for patients with asthma and chronic obstructive pulmonary disease (COPD) have occurred in recent years. However, important gaps in care remain, particularly relating to poor adherence to inhaled therapies. Digital inhaler health platforms which incorporate digital inhalers to monitor time and date of dosing are an effective disease and medication management tool, promoting collaborative care between clinicians and patients, and providing more in-depth understanding of actual inhaler use. With advances in technology, nearly all inhalers can be digitalized with add-on or embedded sensors to record and transmit data quantitating inhaler actuations, and some have additional capabilities to evaluate inhaler technique. In addition to providing an objective and readily available measure of adherence, they allow patients to interact with the device directly or through their self-management smartphone application such as via alerts and recording of health status. Clinicians can access these data remotely and during patient encounters, to better inform them about disease status and medication adherence and inhaler technique. The ability for remote patient monitoring is accelerating interest in and the use of these devices in clinical practice and research settings. More than 20 clinical studies of digital inhalers in asthma or COPD collectively show improvement in medication adherence, exacerbation risk, and patient outcomes with digital inhalers. These studies support previous findings about patient inhaler use and behaviors, but with greater granularity, and reveal some new findings about patient medication-taking behaviors. Digital devices that record inspiratory flows with inhaler use can guide proper inhaler technique and may prove to be a clinically useful lung function measure. Abstract:As the age of pregnant women and prevalence of obesity and diabetes are increasing, so is the prevalence of medical disorders during pregnancy, particularly hypertension and the associated CKD. Pregnancy can worsen kidney function in women with severe disease, and hypertension puts them at risk for pre-eclampsia and the associated complications. There are no specific guidelines for hypertension management in this population, and tight control will not prevent pre-eclampsia. Women with end-stage kidney disease should be placed on intense dialysis regimens to improve obstetric outcomes, and angiotensin-converting enzyme inhibitors and angiotensin receptor blockers are best avoided. This article will review the rationale for a management plan that includes a multidisciplinary team to discuss risks and develop a plan before conception, antepartum monitoring for maternal and fetal morbidity, individualization of medical management using medications with established records during pregnancy, and balancing the level of blood pressure control proved to protect kidney function against the potential effects that aggressive blood pressure control could have on the fetal-placental unit.

Abstract:Modernizing inhaled medications through digital technology can help address persistent problems of non-adherence and poor inhaler technique in patients with obstructive lung diseases. With a growing body of supportive clinical studies, advances in digital inhaler sensors and platforms, greater support from payers and healthcare organizations, significant growth with these technologies is expected. While all digital (smart) inhalers record adherence, these are distinguished by their compatibility with commercial inhalers, capabilities to guide inhaler technique, use of patient-reported outcomes, and user-friendliness for both the healthcare professional (HCP) and patient. Due to the complexity and novelty of employing digital inhalers, collaboration with multiple entities within health systems is necessary and a well-planned integration is needed. For HCPs and patients, cybersecurity and privacy are critical, it will require review by each healthcare organization. In the US, some payers reimburse for remote monitoring using digital inhalers, but reimbursement is currently unavailable in other countries. There are several models for remote patient care, as employing an active, ongoing digital interface between the HCP and patient or they may choose to only review data at clinical encounters. Personalization of therapies and feedback are key to success. While digital inhaler malfunction uncommonly occurs, patient attrition over a year is significant. Some patients will be challenged to use digital platforms or have the necessary technology. Additional research is needed to address cost-effectiveness, in vivo accuracy of inspiratory measurement capable devices, ability to teach inhaler technique, their application for monitoring lung function, and lastly real-world adoption and implementation in routine clinical practice.

Abstract:Introduction: Digitalised care pathways are increasingly used to enhance inhaler adherence remotely in COPD. Objective: To explore the impact of electronic inhaler adherence monitoring (EIM) and interventions on maintenance inhaler adherence rate (AR) and technique. Methods: Studies assessing remote inhaler adherence support among people with COPD were included. Pairwise and proportional meta-analysis (random effect models) was used. Results: Of 1512 studies,11 articles(n=1616) reported inhaler AR over median 3.2[1-6] months. Smart inhalers with audio-visual reminders (±biofeedback and education apps) achieved mean AR (%EIM-actuations taken as prescribed) of 64% (95%CI 43-85) but mean actual AR (%actuations in correct technique) of 32% (95%CI 0.02-64.06). Only 7% reached the target actual AR≥80%. Personalised intervention by healthcare providers alongside EIM improved AR by MD of 22% (95%CI 9-35) versus EIM alone (see Figure 1C). 85% of patients favored inhaler app reminders but 18% (95%CI 13-24, n=423) faced technical issues. Conclusion: EIM can measure and enhance inhaler adherence in COPD patients but addressing inhaler technique requires further attention.

Abstract:Maintenance therapy delivered via inhaler is central to asthma and chronic obstructive pulmonary disease (COPD) management. Poor adherence to inhaled medication and errors in inhalation technique have long represented major barriers to the optimal management of these chronic conditions. Technological innovations may provide a means of overcoming these barriers. This narrative review examines ongoing advances in digital technologies relevant to asthma and COPD with the potential to inform clinical decision-making and improve patient care. Digital inhaler devices linked to mobile apps can help bring about changes in patients' behaviors and attitudes towards disease management, particularly when they build in elements of interactivity and gamification. They can also support ongoing technique education, empowering patients and helping providers maximize the value of consultations and develop effective action plans informed by insights into the patient's inhaler use patterns and their respiratory health. When combined with innovative techniques such as machine learning, digital devices have the potential to predict exacerbations and prompt pre-emptive intervention. Finally, digital devices may support an advanced precision medicine approach to respiratory disease management and help support shared decision-making. Further work is needed to increase uptake of digital devices and integrate their use into care pathways before their full potential in personalized asthma and COPD management can be realized.
Assessment of Inhalation Technique in Clinical and Functional Control of Asthma and Chronic Obstructive Pulmonary Disease

Abstract:Introduction: Chronic obstructive pulmonary disease and asthma affect almost 300 million individuals. Inhaled therapy is often associated with technical errors reducing efficacy and compliance. Objective: To evaluate the inhalation technique and its relation with clinical and functional control in asthma and chronic obstructive pulmonary disease. Material and methods: Analytical cross-sectional study including patients with asthma and chronic obstructive pulmonary disease treated with any type of inhaler device. Demographic data and inquiry about previous teaching of inhalation technique were collected in all participants. Inhalation technique was evaluated in: Step 1 - device activation; Step 2 - previous expiration; Step 3 - inspiration; Step 4 - end inspiratory pause. Clinical control was assessed from the questionnaires Asthma Control Test, Control of Allergic Rhinitis and Asthma Test, modified Medical Research Council and Chronic Obstructive Pulmonary Disease Assessment Test. Spirometric evaluation was performed in all participants. Results: From a total of 62 subjects, 74.19% made at least one error, mainly during step 2 (53.2%). Previous education on inhalation technique was associated with lower number of errors (p = 0.014). There was no association between number of errors and age (p = 0.321), years of diagnosis (p = 0.119) or spirometric evaluation (p > 0.05). In asthma an association was found between number of errors and Asthma Control Test (p = 0.032) and Control of Allergic Rhinitis and Asthma Test (p = 0.008). Discussion and conclusion: Teaching inhalation technique has a positive impact on its future performance. Most patients make mistakes, affecting clinical control in asthma, although in chronic obstructive pulmonary disease no relation was found. This is an ongoing work that aims to reevaluate inhalation technique after patients' education and its further impact.

Determinants of poor inhaler technique and poor therapy adherence in obstructive lung diseases: a cross-sectional study in community pharmacies

Abstract:Background: Correct inhaler use can be challenging in real life, with incorrect use resulting in poor symptom control. The aim of this study was to examine factors associated with poor inhaler technique and poor therapy adherence among patients with obstructive lung disease in community pharmacies. Methods: A cross-sectional study was conducted in patients with obstructive lung diseases in nine Belgian community pharmacies. Logistic regression analyses identified factors associated with poor inhaler technique and poor therapy adherence (assessed by the Test of Adherence to Inhalers and the modified Medication Possession Ratio). Results: Seventy obstructively impaired community patients (median age 64 y, 56% females) were included and the technique of 122 inhalers was assessed. Inhaler technique scored generally poor, with half of patients making critical errors in using at least one of their inhalers. In multivariable analysis, the use of multiple devices (adjusted OR, aOR 11.68; 95% CI 3.29 to 41.51) and a diagnosis of asthma-Chronic Obstructive Pulmonary Disease overlap (ACO; aOR 7.06; 95% CI 1.15 to 43.35), were associated with making critical errors in inhaler technique independent of quality of life. Non-adherence occurred in more than one-third of patients, and occurred in up to one half of the patients when also taking overuse into account. In multivariable analysis for therapy adherence, current smoking was associated with poor therapy adherence (aOR 0.15; 95% CI 0.02 to 0.96) independently of age and poor treatment knowledge. Therapy adherence was poor in patients with asthma compared with those with ACO. Current smokers and highly educated patients seemed to be at increased risk for inhaler overuse. Conclusions: Given the important role of a correct inhaler technique and therapy adherence in disease control, these findings emphasise the need for patient education and aiming uniformity in the inhaler device. Abstract:This review addresses the ongoing challenges in asthma management, particularly focusing on patient adherence to inhaler therapy. Asthma, a chronic condition characterized by variable respiratory symptoms and airflow obstruction, can lead to significant morbidity and mortality if not properly managed. Despite advances in inhaler technology and therapeutic options, non-adherence remains a significant barrier to optimal asthma control. This review explores both intentional and unintentional non-adherence, influenced by factors such as age, socioeconomic status, and the complexity of inhaler devices. The Global Initiative for Asthma (GINA) provides guidelines aimed at improving adherence through targeted interventions, and this review examines their application. Common inhaler technique errors, including incorrect inhalation speed, not exhaling before inhaling, and failure to hold breath post-inhalation, are identified as major contributors to inadequate asthma control. Furthermore, the review explores the emerging role of electronic monitoring devices (EMDs), such as CapMedic and DigiHaler, which offer real-time feedback to enhance inhaler technique and adherence. The role of biomarkers in assessing adherence and the potential of personalized treatment strategies, including biologic therapies, are also discussed. Overall, addressing adherence requires a comprehensive approach that integrates patient education, tailored interventions, and technological innovations to achieve better clinical outcomes in asthma management.

Uncovering patterns of inhaler technique and reliever use: the value of objective, personalized data from a digital inhaler

Abstract:Electronic inhalers provide information about patterns of routine inhaler use. During a 12-week study, 360 asthma patients using albuterol Digihaler generated 53,083 inhaler events that were retrospectively analyzed. A total of 41,528 (78%) of the recorded inhalation events were suitable for flow analysis (having a PIF ≥ 18 L/min and <120 L/min). Median PIF, inhalation volume, inhalation duration, and time to PIF for these events steadily decreased between the first and last 10 days of the study, by 5.1%, 12.6%, 15.9%, and 6.4%, respectively. Continuous short-acting beta 2 -agonist (SABA) overuse, defined as ≥2 SABA inhalations/week throughout the study period, was seen in 29% ( n = 104) of patients. Of 260 patients with ≥1 instance of acute short-term SABA overuse, 55 (21%) had a confirmed exacerbation. Electronic recording of real-life inhaler use can capture valuable, objective information that could inform disease management and clinical decision-making.

Inhaler Technique and Adherence to Inhaled Medications among Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease in Vietnam

Abstract:Sub-optimal chronic obstructive pulmonary disease (COPD) management has been found largely due to patients’ medication non-adherence and incorrect inhaler technique. This study aimed to examine inhaler use technique and medication adherence among Vietnamese COPD patients as well as potential associated factors. A cross-sectional study involving 70 COPD exacerbators was conducted. Inhaler technique and adherence were evaluated by the 10-item and 12-item Test of Adherence to Inhaler (TAI). Data on the history of COPD, home prescription of inhalers and duration of hospitalization were also collected. Generalized linear regression models were used to determine the associated factors with inhaler use and medication adherence. The results showed that the proportion of patients with good inhaler technique was 22.7% for metered-dose inhalers (MDI), 30.4% for dry powder inhalers (DPI) and 31.8% for soft-mist inhalers (SMI). Full exhalation was the most common mistake. The rates of non-compliance patterns were: “ignorant” (77.1%), “sporadic” (58.6%), and “deliberate” (55.7%). Worse dyspnea, greater health condition impairment, and an increased frequency of exacerbations and hospitalizations were found to be associated negatively with correct inhaler use and treatment adherence. Instructions to COPD patients about using inhalers should focus on correct inhaler technique and adherence even when feeling healthy.

Abstract:The thymus has been established as an essential organ for the normal development of immune function in young mammals. Recent evidence suggests that the thymus also plays an active role in the maintenance of the immune system through life.’ To perform its role as a primary lymphoid organ, the thymus must attract extrinsic stem cells or prothymocytes. Precursor cells migrate from the bone marrow during the neonatal period throughout adulthood to carry out this function?’ A blind well assay system was developed to examine the “bone marrow-thymus axis” in vitro. It was used to demonstrate that a subset of murine bone marrow cells is capable of migrating through a Nuclepore membrane toward a supernatant derived from cultured newborn mouse thymuses. The migrated cells are enriched for a “null” cell population, that is, Thy 1 and surface immunoglobulin negative cells.4 When injected into an irradiated host, these cells are capable of repopulating the thymus in a significantly higher percentage than do nonmigration-enriched bone marrow cells. This significant in vivo thymus homing suggests that selective migration to thymus supernatant enriches for a prethymic cell population? In the present study the effect of sound stress on the bone marrow-thymus axis was examined using the in vitro migration assay. Histologic sections and weights of thymus, spleen, and adrenal glands were also examined for alterations resulting from the sound stress. Young adult female CBA/J mice were exposed to a 2-second, high intensity sound ( 110 db at 2900 Hz, variable interval of 1 minute) 2 hours daily for 5 days. Twentyfour hours later, the femurs and tibias were dissected from the animals; bone marrow cells were separated and suspended in media. In vitro migration assays were carried out using blind well chambers. Thymus supernatant from 3-day-old mice was used as the attractant in the bottom wells separated from the bone marrow cells in the top wells by Nuclepore membranes having 5-ym diameter pores. After 90 minutes of incubation, cells migrating to the bottom chambers were collected, counted, and compared to the number of migrated cells from the bone marrow of nonstressed mice. Migrations to Iscove’s media alone were carried out simultaneously to calculate the nonspecific background migration. The in vitro assay showed a significant decrease in the percentage of migration of bone marrow cells from experimental sound-stressed animals when compared with control nonstressed age-matched mice (TABLE 1). When thymus, spleen, and adrenal glands of the experimental animals were compared with those of the control animals, no significant changes in organ weights (TABLE 2) and no apparent histologic changes
