Abstract:AIM To evaluate the feasibility and safety of trans-anal minimally invasive surgery (TAMIS) from single institute in China. METHODS A retrospective review was conducted for patients with rectal neoplasia, who underwent TAMIS using single incision laparoscopic surgery-Port from January 2013 till January 2016 by a group of colorectal surgeons from Gastrointestinal Center Unit., Peking University Cancer Hospital. Patients' demographic data, surgical related information, post-operational pathology, as well as perioperative follow-up were all collected. RESULTS Twenty-five patients with rectal neoplasia were identified consequently. Complete full-thickness excision was achieved in all cases without conversion. 22 (88%) cases had rectal malignancies [6 were adenocarcinomas and 16 were neuroendocrine tumors (NET)], while 3 patients had adenomas. Mean surgical duration was 61.3 min, and mean post-operative stay were 2.7 d. Post-operational examination demonstrated 5 cases had positive resection margin: 2 adenocarcinoma cases and 1 NET case with positive lateral margin, and the other 2 NET cases with positive basal margin. The curve of operation time for TAMIS cases suggested a minimum of 10 cases for a laparoscopic surgeon proficient with this technique. CONCLUSION TAMIS was demonstrated to be reproducible and safe, with a relatively short learning process for laparoscopic surgeons in selected cases for rectal neoplasia. Longterm oncological outcome needs to be determined by further investigation.

Abstract:The ability of plasmacytoid dendritic cells (pDCs) to promote plasma cell differentiation and immunoglobulin (Ig) secretion through the production of type I interferon and interleukin-6 has been well documented, although the role of additional factors, including tumor necrosis factor receptor-ligand interactions, has not been addressed. On stimulation with the Toll-like receptor ligand CpG (B type, 2006) we found that pDCs exhibit strong and stable expression of CD70, a tumor necrosis factor family ligand that binds to its receptor CD27 expressed on memory B cells and promotes plasma cell differentiation and Ig secretion. Using a pDC/B-cell coculture system, we found that CpG-stimulated pDCs can induce the proliferation of CD40L-activated human peripheral B cells and Ig secretion. This occurs independently of interferon and residual CpG, and requires physical contact between pDCs and B cells. CpG-stimulated pDCs can induce the proliferation of both naive and memory B cells, although Ig secretion is restricted to the memory subset. Blocking the interaction of CD70 with CD27 using an antagonist anti-CD70 antibody reduces the induction of B-cell proliferation and IgG secretion by CpG-stimulated pDCs. We have therefore identified CD70 as an important factor in the regulation of B-cell growth and differentiation by pDCs.
A Single-Center Study on the Comparison of Short-Term Efficacy of Robotic and Laparoscopic Radical Resection of Middle and Low Rectal Cancer with Da Vinci (Xi)

Abstract:Abstract OBJECTIVE Robotic minimally invasive surgery is an important trend in the development of contemporary surgical technology. However, due to the current clinical application of robotic surgery system is in the stage of development, its status in the surgical treatment of middle and low rectal cancer has not been confirmed. The aim of this study was to compare and analyze the clinical efficacy and functional results of da Vinci (Xi)-assisted and traditional laparoscopic surgery in the treatment of middle and low rectal cancer. METHODS 82 patients with middle and low rectal cancer (tumor lower margin ≤ 10cm) were selected from the affiliated Hospital of Xuzhou Medical University from August 2020 to December 2020. According to the sequential principle, they were randomly divided into robot group (n = 41) and laparoscopy group (n = 41). Robot-assisted radical resection and laparoscopy-assisted radical resection of middle and low rectal cancer were performed respectively. The relevant clinical and functional indexes of the two groups were compared. RESULTS There was no significant difference in preoperative data between the two groups (P > 0.05). All patients completed the operation successfully and no one was converted to open surgery. The circumferential incisal margin was negative in both groups, and the resected mesorectum was intact in robot group and laparoscopy group. Compared with the laparoscopy group, the average intraoperative blood loss (55.2 ±29.8mLvs.109.5 ±58.5ml), postoperative gastrointestinal function recovery time (38.8 ±11.2hvs.50.7± 13.3h), diet recovery time (2.6 ±0.8dvs.3.4 ±1.7d) and catheter removal time (3.3 ±1.4dvs.6.7±1.9d) in the robot group were significantly reduced. The number of lymph node dissection and the rate of sphincter preservation (55% vs.35%) were better than those in the laparoscopy group, but the operation time (239.8 ±29.6 6.2vs.14.6 vs. 141.1 ±18.5 min) and the average hospitalization cost in the laparoscopy group were higher than those in the laparoscopy group (81000 ±13000 yuan vs.52000 ±8000 yuan). There was no significant difference in the mode of operation, the length of specimen resection, postoperative pathology, the grade of integrity of total mesorectal resection, postoperative hospital stay and the incidence of postoperative complications between the two groups. In addition, we found that patients in the robot group had higher satisfaction with defecation function, voiding function and sexual function. CONCLUSION Robotic and laparoscopic surgery for middle and low rectal cancer had comparable surgical safety and immediate postoperative outcomes, but robotic surgery was superior to conventional laparoscopic surgery in terms of the number of lymph nodes cleared, anal preservation rate, and postoperative recovery of gastrointestinal function for faecal, urinary, and sexual function preservation.

Abstract:In this study, surgery was performed using this procedure‐based and close follow‐up showed that robotic surgery showed corresponding advantages over conventional laparoscopic resection in terms of protection of the pelvic autonomic nerves and achieved better late results. Backgrounds This study aims to evaluate the clinical efficacy and functional outcomes of DA Vinci (Xi)‐assisted surgery compared to conventional laparoscopic surgery for middle and low rectal cancer, focusing on oncologic cure and functional preservation. Methods Between December 2020 and June 2021, 102 patients with middle and low rectal cancer (tumour lower margin ≤10 cm) were enrolled at the affiliated Hospital of Xuzhou Medical University. Participants were divided into two groups: robot‐assisted (n = 51) and laparoscopy‐assisted (n = 51). Each group underwent a radical resection using their assigned method. Clinical and functional outcomes were analysed post‐surgery. Results Preoperative data did not differ significantly between groups (P > 0.05). All surgeries were successfully completed without conversion to open surgery. The robotic group experienced significantly less intraoperative blood loss (55.2 ± 29.8 mL vs. 109.5 ± 58.5 mL) and faster recovery in gastrointestinal function (35.1 ± 9.4 h vs. 40.7 ± 1.9 h), diet recovery (2.1 ± 0.8 days vs. 2.9 ± 0.4 days), and catheter removal (2.9 ± 2.7 days vs. 5.3 ± 2.1 days). The robotic group also dissected more lymph nodes (23 ± 6 vs. 15 ± 4). However, they had longer operative times (239.8 ± 29.6 min vs. 141.1 ± 18.5 min) and higher hospital costs. Satisfaction levels regarding defecation, voiding, and sexual functions were notably higher in the robotic group. Conclusion No significant differences in surgical safety or immediate postoperative outcomes were observed between robotic and laparoscopic approaches. However, robotic surgery demonstrated superior lymph node dissection, anal function preservation, and gastrointestinal recovery, enhancing overall functional outcomes.

Abstract:Objective To compare the techniques and outcomes of conventional laparoscopic resection for colorectal cancer and robotic assisted resection for colorectal cancer.Methods From March 2010 to October 2010,10 patients with tumor on the ascending colon,left colon,sigmoid colon and rectum were treated by conventional laparoscopic techniques and Da Vinci robot-assisted surgery.The surgeries included right hemicolectomy in 2 cases,left hemicolectomy in 2 cases,low anterior resection of upper rectum on rectal cancer in 2 cases,and low anterior resection of the rectum on lower rectal cancer in 2 cases.The operation time,blood loss,postoperative recovery and evaluation of oncology treatment were compared between the two groups. Results All surgeries were successfully done without conversion.The operation time was longer in robot-assisted surgery group than in laparoscopic surgery group,but the blood loss was less than in laparoscopic surgery group. The postoperative recovery was similar between the two groups.Postoperative pathological examination of tumor specimens showed radical resection in all patients,with one case of low rectal cancer had damaged mesorectum after treated by laparoscopic technique.Conclusion The Da Vinci robot-assisted resection of colorectal cancer can better reflect the principles of damage control of minimally invasive surgery and cancer treatment guidelines;it especially has a promising future in resection of narrow pelvic rectal cancer.

Abstract:Transanal minimally invasive surgery (TAMIS) is mainly used for benign tumors of the rectum, but there are few reports on treating malignant tumors of the rectum with a large volume. The aim of this study was to evaluate the feasibility and safety of TAMIS for resection of rectal malignant tumors. A 57-year-old patient was pathologically diagnosed as rectal malignancy before surgery. Computed tomography (CT), magnetic resonance imaging (MRI), fludeoxyglucose-18F (18F-FDG), and endoscopic mucosal biopsy were performed to assess the tumor location and preoperative size. There were no contraindications in all preoperative examinations. We applied TAMIS technology to perform the operation on this patient, And the operative time, the amount of blood loss, the length of hospital stay, the cost of hospital stay, surgical complications, postoperative complications and other relevant data were all collected. The operation was successful, the operation time was 45 min, 10 mL of intraoperative blood loss, and the length of stay was 3 days, a tumor of a maximum diameter of 4 cm being completely removed. There were no related complications or recurrence during postoperative follow-up. The pathological results were tubular villous adenoma with low-grade intraepithelial neoplasia, and a focal area of high-grade intraepithelial neoplasia. The pathological stage was T1N0M0. At 3-month follow-up there were no signs of recurrence. The patient was followed up for 5 years after the operation and there was no tumor recurrence or metastasis in other parts and no other discomfort. TAMIS is less commonly used in rectal malignancies, especially for tumors with larger diameters. We successfully performed complete resection of a 4-cm rectal malignant tumor with TAMIS. Given its low risk, low cost, simple operation, and few complications, TAMIS can be used for more indications of rectum diseases.
A Comparison of Conventional and Organic Milk Production Systems in the U

Abstract:Organic milk production is one of the fastest growing segments of organic agriculture in the U.S., but little is known about the relative costs and returns of organic and conventional dairies. This study utilizes a nationwide survey of dairy operations for 2005 that includes a targeted sample of organic dairies. Treatment-effect sample-selection models were specified to isolate the impact of choosing the organic approach on various levels of milk production costs. Size and location of dairy operation were among the primary factors affecting choice of the organic approach and milk production costs. Organic dairies had production costs about $5 to $7 per cwt higher than conventional dairies and received an average milk price premium of $6.69 per cwt. Results suggest that there may be incentives for small conventional dairies to covert to the organic approach, but probably not for startup organic dairies unless they can enter at a much larger scale than the current industry norm. *The authors are with the U.S. Department of Agriculture, Economic Research Service. A Comparison of Conventional and Organic Milk Production Systems in the U.S. Organic milk production is one the fastest growing segments of organic agriculture in the U.S. Between 2000 and 2005 the number of certified organic milk cows on U.S. farms increased by an average of about 25 percent each year, from 38,000 to more than 86,000 (USDA, Economic Research Service, a). Many of these cows are on small dairy operations that have switched to the organic approach with the hope of improving farm profitability. Despite the growing number of organic dairy operations there is little information about the relative costs and returns of organic and conventional milk production and the farm characteristics of those that have chosen the organic approach. Studies by Dalton et al., Butler, and Barham et al. are among the few examples of information on this subject, but nothing is available on a national basis. Organic milk production systems rely on ecologically based practices that virtually prohibit the use of antibiotics and hormones in the cow herd and the use of synthetic chemicals in the production of cattle feed. Organic milk production systems also attempt to accommodate the animals’ natural nutritional and behavioral requirements, for example ensuring that dairy cows have access to pasture (Greene and Kremen). These requirements add to production costs and create obstacles to widespread adoption, such as higher managerial costs and risks of shifting to a new way of farming, and significant time and costs associated with the transition to organic production. Abstract:Robotic transanal minimally invasive surgery (R-TAMIS) was introduced in 2012 for the excision of benign rectal polyps and low grade rectal cancer. Ergonomic improvements over traditional laparoscopic TAMIS (L-TAMIS) include increased dexterity within a small operative field, with possibility of better surgical precision. We aim to collate the existing data surrounding the use of R-TAMIS to treat rectal neoplasms from cohort studies and larger case series, providing a foundation for future, large-scale, comparative studies.

Abstract:This is a video vignette of a 57‐year‐old asymptomatic female patient. The patient underwent a screening colonoscopy which revealed a 10 mm scar in the rectum. Biopsy resulted in a well‐differentiated tubular adenocarcinoma. Computed tomography and pelvic magnetic resonance imaging confirmed tumor characteristics without distant or lymph nodal metastasis. A minimally invasive robotic transanal resection using the Da Vinci Xi platform was performed, achieving full‐thickness lesion excision with uneventful recovery. Histopathology revealed intramucosal adenocarcinoma with free margins. Local resection is advocated for selected T1 lesions and demands a thorough preoperative assessment. Robotic‐assisted surgery presents a valuable alternative for early rectal adenocarcinoma management.

Robot-assisted Versus Laparoscopic Surgery for Rectal Cancer: A Systematic Review and Meta-Analysis

Abstract:This study aimed to compare clinical and oncological outcomes of robot-assisted and laparoscopic surgery for rectal cancer.We searched PubMed/Medline, Embase, the Cochrane Library, Yahoo, and Google Scholar databases for relevant articles published up to 2017. Studies based on comparability between robot-assisted and laparoscopic surgery for rectal cancer were designated. Clinical outcomes included operative time, conversion to open surgery, estimated blood loss (EBL), bowel function recovery time, length of hospital stay (LOS), anastomosis leak, and postoperative complications. Oncological outcomes comprised the number of lymph nodes extracted, the positive circumferential margin (PCRM), and the distal resection margin (DRM).Twenty studies were designated totaling 5496 patients, comprising a robot-assisted surgery patient group (n = 2168, 39.4%) and a laparoscopic surgery patient group (n = 3328, 60.6%). The robot-assisted surgery group was associated with longer operative time (odds ratio [OR] 0.48, 95% confidence interval [CI]; 0.14, 0.82), lower conversion to open surgery rate (OR 0.55, 95% CI; 0.44, 0.69), shorter LOS (OR - 0.15, 95% CI; -0.30, 0.00), faster bowel function recovery (OR - 0.38, 95% CI; -0.74, -0.02), and lower postoperative complications (OR 0.79, 95% CI; 0.65, 0.97). EBL, anastomosis leak rate, and oncological outcomes including the number of lymph nodes extracted, the DRM, and the PCRM showed no significant differences between groups.Robot-assisted surgery for rectal cancer showed longer operative time, lower conversion, faster bowel function recovery rates, and shorter hospital stay, and similar oncological outcomes compared to laparoscopic surgery.

Abstract:Objective:To investigate the feasibility and safety of robotic colorectal resection with hand-sewn anastomosis of the digestive tract and nature orifice specimen extraction.Methods:The clinical date of 10 patients diagnosed with colorectal tumor who were treated with robotic colorectal resection with hand-sewn anastomosis of the digestive tract and nature orifice specimen extraction from May 2020 to July 2022 at the Division of Gastrointestinal and Pancreatic Surgery, Zhejiang Provincial People’s Hospital were retrospectively studied.Results:There were 4 males and 6 females, with the age range from 33 to 82 years, received robotic colorectal resection with hand-sewn anastomosis of the digestive tract and nature orifice specimen extraction. The surgical specimen is removed through the rectum. The median operative time was 120(ranged 100 to 190)minutes. The intraoperative blood loss was 40(ranged 10 to 100)ml and there was no intraoperative blood transfusion. The median postoperative hospital stay was 10(ranged 8 to 18)days. There were no transit open surgery, re-operation and mortality. Perioperative complications (Clavien-Dindo grade): 1 case of grade Ⅲ, 2 cases of grade Ⅱ, 1 case of grade Ⅰ; 1 case developed anastomotic leakage, successful through conservative treatment.Conclusion:Robotic colorectal resection with hand-sewn anastomosis of the digestive tract and nature orifice specimen extraction was effective and safe to treat colorectal tumor with the improvement of intra-abdominal anastomosis technology.
Short-term and Long-Term Outcomes of Robotic Rectal Surgery-from the Real Word Data of 1145 Consecutive Cases in China.

Abstract:Background Due to a limited patient sample size, substantial data on robotic rectal resection (RRR) is lacking. Here, we reported a large consecutive cases from the real word data to assess the safety and efficacy of RRR. Methods From September 2010 to June 2017, a total of 1145 consecutive RRR procedures were performed in patients with stage I–IV disease. We conducted an analysis based on information from a prospectively designed database to evaluate surgical outcomes, urogenital function, and long-term oncological outcomes. Results Of three types of RRR performed, 227 (24.2%) were abdominoperineal resections, 865 (75.5%) were anterior resections, and 3 (0.3%) were Hartmann. Conversion to an open procedure occurred in 5.9% of patients. The overall positive circumferential margin rate was 1.3%. Surgical complication rate and mortality were 16.2% and 0.8% within 30 days of surgery, respectively. Mean hospital stay after surgery and hospital cost were 6.3 ± 2.9 days and 10442.5 ± 3321.5 US dollars, respectively. Risk factors for surgical complications included male gender, tumor location (mid-low rectum), combined organ resection, and clinical T category (cT3–4). Urinary function and general sexual satisfaction decreased significantly 1 month after surgery for both sexes. Subsequently, both parameters increased progressively, and the values 1 year after surgery were comparable to those measured before surgery. At a median follow-up of 34.6 months, local recurrence and distant metastases occurred in 2.3% and 21.1% of patients, respectively. Conclusions Robotic rectal resection was safe with preserved urogenital function and arrived equivalent oncological outcomes in a nonselected group of patients with rectal cancer.

Abstract:3603 Background: Currently, robotic surgery for rectal cancer using da Vinci System is common. However, there is almost no clinical trial reported. This randomized controlled trial aims to compare the safety and efficacy of robotic, laparoscopic and open abdominoperineal resection (APR) for low rectal cancer. Methods: From September 2013 to August 2016, patients aged from 18 to 75 years, with low rectal cancer within 5 cm from anal verge, clinical T1 to T3, no distant metastases, were randomly assigned to receive either robotic procedures (RAP), laparoscopic procedures (LAP) or open surgery (OS) for APR in 1:1:1 ratio. The primary endpoint was postoperative complication rate. This study is registered with ClinicalTrials.gov (NCT01985698). Results: Totally 406 patients were randomly assigned. Actually, 135 finished RAP, 131 finished LAP, and 137 finished OS (including 4 convert from LAP to OS). RAP had significantly lower postoperative complication rate (11.1%) than both LAP (21.4%, P = 0.023) and OS (27.7%, P = 0.001). Also, RAP reduced intraoperative hemorrhage (median [interquartile range], 100 [90-110] ml) than LAP (130 [100-150] ml, P < 0.001) and OS (150 [120-260] ml, P < 0.001). And RAP promoted postoperative recovery, with shorter days to first flatus (1.0 [1.0-2.0] day) than LAP (2.0 [2.0-3.0] day, P < 0.001) and OS (3.0 [2.0-4.0] day, P < 0.001), shorter days to first automatic urination (2.0 [2.0-3.0] day) than LAP (3.0 [2.0-4.0] day, P < 0.001) and OS (3.0 [2.0-4.0] day, P < 0.001), and shorter days to discharge (5.0 [5.0-6.0] days) than LAP (6.0 [5.0-7.0] days, P < 0.001) and OS (6.0 [5.0-7.0] day, P = 0.005). There was no significant difference in open conversion rate, resection margin involvement (including circumferential resection margin), number of lymph node harvested and pathological tumor stage. Conclusions: Robotic APR was safer, and reproduce equivalent surgical quality of conventional laparoscopic and open surgery. Also, it provided less injury and faster functional recovery. Abstract:Transanal minimally invasive surgery (TAMIS) is a kind of minimally invasive surgery that local resection or total mesorectal excision for rectal neoplasm is performed through the use of multichannel port(single port) transanally. Compared to transanal endoscopic microsurgery(TEM) approach, TAMIS offers an alternative to TEM for rectal neoplasm, and shows the advantage of lower cost and shorter learning curve. TAMIS approach has been used not only in the local resection of rectal neoplasm but also in transanal total mesorectal excision (transanal TME), which is also called TAMIS-TME, in recent four years. The safety and efficacy of TAMIS approach has been shown in the currently published literatures. However, TAMIS approach has to wait for more evidence-based data with larger-scale and longer follow-up to get its validation.

Abstract:BackgroundIn recent years, robot-assisted surgery using the da Vinci System® has been proposed as an alternative to traditional open or laparoscopic procedures. The aim of this study was to compare the short-term outcomes for open, laparoscopic, and robot-assisted rectal resection for cancer.MethodsTwo hundred sixty-three patients with rectal cancer who underwent curative resection between 2007 and 2009 were included. Patients were classified into an open surgery group (OS, n = 88), a laparoscopic surgery group (LAP, n = 123), and a robot-assisted group (RAP, n = 52). Data analyzed include operating time, length of recovery, methods of specimen extraction, quality of total mesorectal excision, and morbidity.ResultsThe mean operating time was 233.8 ± 59.2 min for the OS group, 158.1 ± 49.2 min for the LAP group, and 232.6 ± 52.4 min for the RAP group (p < 0.001). Patients from the LAP and RAP groups recovered significantly faster than did those from the OS group (p < 0.05). The proportion of operations performed through a natural orifice (intracorporeal anastomosis with transanal or transvaginal retrieval of specimens) was significantly higher in the RAP group (p < 0.001). The specimen quality—with a distal resection margin, harvested lymph nodes, and circumferential margin—did not differ among the three groups. The overall complication rates were 20.5, 12.2, and 19.2% in the OS, LAP, and RAP groups, respectively (p = 0.229).ConclusionsRAP and LAP reproduce the equivalent short-term results of standard OS while providing the advantages of minimal access. For the experienced laparoscopic colorectal oncologist, use of the da Vinci robot resulted in no significant short-term clinical benefit over the conventional laparoscopic approach.

Abstract:Purpose: To compare outcomes and complications of Descemet stripping automated endothelial keratoplasty (DSAEK) in complicated cases with intraocular lens (IOL) exchange, aphakia, or anterior chamber intraocular lens (ACIOL) implants with a group of noncomplicated DSAEK cases. Methods: Of the 30 complicated DSAEK cases, 14 eyes underwent concurrent IOL exchange, 5 ACIOLs were not removed, 5 eyes remained aphakic, and 5 eyes had IOL exchange done before or after DSAEK. One eye had an iris-supported phakic IOL removed, followed by cataract extraction with IOL implantation at the time of DSAEK. The comparison group included 109 consecutive DSAEK cases with a history of Fuchs dystrophy or pseudophakic bullous keratopathy. Results: In the complicated group with significant ocular comorbidities, 27.6% achieved best-corrected visual acuity (BCVA) ≥20/40 and 60% had a final BCVA ≥20/70. In the comparison group of patients without visually significant comorbidities, 94.4% of eyes achieved BCVA ≥20/40 with no complication of graft detachments. Of the 30 complicated eyes, 5 (16.7%) had graft detachments and 5 (16.7%) developed IOL dislocations. All grafts remained clear at the last follow-up visit, except 3 cases (10%) in the complicated group, 2 of which were because of primary graft failure and required penetrating keratoplasty. Conclusions: In comparison with uncomplicated DSAEK cases, higher graft and IOL dislocations were observed in cases involving IOL exchanges, ACIOLs, or aphakia.

Abstract:Transanal minimally invasive surgery (TAMIS) can be utilized to manage a wide variety of rectal lesions but can be technically demanding with traditional laparoscopic equipment. Robotic platforms such as the da Vinci Single Port system can reduce the technical barriers of TAMIS and allow more complicated lesions to be addressed. Robotic TAMIS with the SP system follows similar indications for local excision of benign and malignant lesions as conventional TAMIS or even transanal endoscopic microsurgery. We describe our initial experience using the SP system and provide technical suggestions for how to incorporate this technology. We also address innovations in flexible endoscopic robotic surgery that we anticipate will allow for increased use of organ preservation of the colon and rectum, as well as possibly expand the use of natural orifice surgery.
Short-term outcomes of robotic vs. laparoscopic surgery for rectal cancer after neoadjuvant therapy: a meta-analysis

Abstract:Background The effect of robotic surgery (RS) for rectal cancer after neoadjuvant therapy is still controversial, and a comprehensive search and analysis of the current relevant evidence is necessary. Our study aimed to evaluate the efficacy of RS for rectal cancer after neoadjuvant therapy compared with conventional laparoscopic surgery (LS). Methods Up to August 23, 2023, Embase, PubMed, Web of Science, and Cochrane databases were searched for studies of RS for rectal cancer after neoadjuvant therapy. Odds ratio (OR) or mean difference (MD) was used to calculate the effect sizes using RevMan 5.3. Results A total of 12 studies reporting on 11,686 participants were included. Compared with LS, RS increased the operative time (MD 35.16 min; 95% CI: 16.24, 54.07), but it did significantly reduce the risk of the conversion to open surgery (OR 0.46, 95% CI 0.40, 0.53) and improved the TME incomplete rate (OR 0.40, 95% CI 0.17, 0.93). Moreover, there were no difference in total postoperative complications (OR 1.13, 95% CI 0.84, 1.52), circumferential resection margin positivity (OR 0.90, 95% CI 0.63, 1.27), distal margin positive (OR 0.60, 95% CI 0.29, 1.22), blood loss (MD −11.57 ml; 95% CI: −39.09, 15.94), length of hospital stay (MD −0.08 days; 95% CI: −1.26, 1.10), mortality (OR 0.59, 95% CI 0.29, 1.21), lymph node harvested (MD 0.69.; 95% CI: −0.43, 1.82), and the time of first flatus (MD −0.47 days; 95% CI: −1.19, 0.25) between the two groups. Conclusions RS was associated with superiority over LS in reducing the risk of the conversion to open surgery and improving TME incomplete rate, which suggested that RS could be an effective method for treating rectal cancer after neoadjuvant therapy.
