Abstract:The dispersion relation and the power spectrum of the surface modes on a surface-active, freely standing film of a concentrated polymer solution are studied with a two-component fluid model of a viscoelastic material. The diagram of bending modes is obtained from an asymptotic analysis of the dispersion equation when the bending rigidity modulus is the main elastic effect on the layer interface. The resulting dynamical structure factor provides the characteristic squeezing and undulation surface modes of the interfaces driven by thermal fluctuations or by a weak external perturbation. The effect of interfacial and bulk elastic properties on the power spectrum of the scattered light is studied. In the regime of an elastic solution, finite-thickness effects and bulk elastic properties of the layer sustain a train of elastic peaks when the wavelength of thermal fluctuations is comparable to the thickness of the layer. Interfacial elasticity properties increase the strength and shift all resonance frequency peaks producing a less intense quasielastic spectrum around zero frequency.
Predicting Intensive Care Unit Length of Stay and Mortality Using Patient Vital Signs: Machine Learning Model Development and Validation

Abstract:Background Patient monitoring is vital in all stages of care. In particular, intensive care unit (ICU) patient monitoring has the potential to reduce complications and morbidity, and to increase the quality of care by enabling hospitals to deliver higher-quality, cost-effective patient care, and improve the quality of medical services in the ICU. Objective We here report the development and validation of ICU length of stay and mortality prediction models. The models will be used in an intelligent ICU patient monitoring module of an Intelligent Remote Patient Monitoring (IRPM) framework that monitors the health status of patients, and generates timely alerts, maneuver guidance, or reports when adverse medical conditions are predicted. Methods We utilized the publicly available Medical Information Mart for Intensive Care (MIMIC) database to extract ICU stay data for adult patients to build two prediction models: one for mortality prediction and another for ICU length of stay. For the mortality model, we applied six commonly used machine learning (ML) binary classification algorithms for predicting the discharge status (survived or not). For the length of stay model, we applied the same six ML algorithms for binary classification using the median patient population ICU stay of 2.64 days. For the regression-based classification, we used two ML algorithms for predicting the number of days. We built two variations of each prediction model: one using 12 baseline demographic and vital sign features, and the other based on our proposed quantiles approach, in which we use 21 extra features engineered from the baseline vital sign features, including their modified means, standard deviations, and quantile percentages. Results We could perform predictive modeling with minimal features while maintaining reasonable performance using the quantiles approach. The best accuracy achieved in the mortality model was approximately 89% using the random forest algorithm. The highest accuracy achieved in the length of stay model, based on the population median ICU stay (2.64 days), was approximately 65% using the random forest algorithm. Conclusions The novelty in our approach is that we built models to predict ICU length of stay and mortality with reasonable accuracy based on a combination of ML and the quantiles approach that utilizes only vital signs available from the patient’s profile without the need to use any external features. This approach is based on feature engineering of the vital signs by including their modified means, standard deviations, and quantile percentages of the original features, which provided a richer dataset to achieve better predictive power in our models.

Dynamic survival prediction in intensive care units from heterogeneous time series without the need for variable selection or pre-processing

Abstract:We present a machine learning pipeline and model that uses the entire uncurated EHR for prediction of in-hospital mortality at arbitrary time intervals, using all available chart, lab and output events, without the need for pre-processing or feature engineering. Data for more than 45,000 American ICU patients from the MIMIC-III database were used to develop an ICU mortality prediction model. All chart, lab and output events were treated by the model in the same manner inspired by Natural Language Processing (NLP). Patient events were discretized by percentile and mapped to learnt embeddings before being passed to a Recurrent Neural Network (RNN) to provide early prediction of in-patient mortality risk. We compared mortality predictions with the Simplified Acute Physiology Score II (SAPS II) and the Oxford Acute Severity of Illness Score (OASIS). Data were split into an independent test set (10%) and a ten-fold cross-validation was carried out during training to avoid overfitting. 13,233 distinct variables with heterogeneous data types were included without manual selection or pre-processing. Recordings in the first few hours of a patient's stay were found to be strongly predictive of mortality, outperforming models using SAPS II and OASIS scores within just 2 hours and achieving a state of the art Area Under the Receiver Operating Characteristic (AUROC) value of 0.80 (95% CI 0.79-0.80) at 12 hours vs 0.70 and 0.66 for SAPS II and OASIS at 24 hours respectively. Our model achieves a very strong performance of AUROC 0.86 (95% CI 0.85-0.86) for in-patient mortality prediction after 48 hours on the MIMIC-III dataset. Predictive performance increases over the first 48 hours of the ICU stay, but suffers from diminishing returns, providing rationale for time-limited trials of critical care and suggesting that the timing of decision making can be optimised and individualised.

Predicting Abnormalities in Laboratory Values of Patients in the Intensive Care Unit Using Different Deep Learning Models: Comparative Study

Abstract:Background: In recent years, the volume of medical knowledge and health data has increased rapidly. For example, the increased availability of electronic health records (EHRs) provides accurate, up-to-date, and complete information about patients at the point of care and enables medical staff to have quick access to patient records for more coordinated and efficient care. With this increase in knowledge, the complexity of accurate, evidence-based medicine tends to grow all the time. Health care workers must deal with an increasing amount of data and documentation. Meanwhile, relevant patient data are frequently overshadowed by a layer of less relevant data, causing medical staff to often miss important values or abnormal trends and their importance to the progression of the patient's case. Objective: The goal of this work is to analyze the current laboratory results for patients in the intensive care unit (ICU) and classify which of these lab values could be abnormal the next time the test is done. Detecting near-future abnormalities can be useful to support clinicians in their decision-making process in the ICU by drawing their attention to the important values and focus on future lab testing, saving them both time and money. Additionally, it will give doctors more time to spend with patients, rather than skimming through a long list of lab values. Methods: We used Structured Query Language to extract 25 lab values for mechanically ventilated patients in the ICU from the MIMIC-III and eICU data sets. Additionally, we applied time-windowed sampling and holding, and a support vector machine to fill in the missing values in the sparse time series, as well as the Tukey range to detect and delete anomalies. Then, we used the data to train 4 deep learning models for time series classification, as well as a gradient boosting-based algorithm and compared their performance on both data sets. Results: The models tested in this work (deep neural networks and gradient boosting), combined with the preprocessing pipeline, achieved an accuracy of at least 80% on the multilabel classification task. Moreover, the model based on the multiple convolutional neural network outperformed the other algorithms on both data sets, with the accuracy exceeding 89%. Conclusions: In this work, we show that using machine learning and deep neural networks to predict near-future abnormalities in lab values can achieve satisfactory results. Our system was trained, validated, and tested on 2 well-known data sets to ensure that our system bridged the reality gap as much as possible. Finally, the model can be used in combination with our preprocessing pipeline on real-life EHRs to improve patients' diagnosis and treatment.

Abstract:INTRODUCTION: Assessing vital sign measurements within hospital settings presents a valuable opportunity for data analysis and knowledge extraction. By generating adaptable, personalized prediction models of patient vital signs, these models can yield clinically relevant insights not achievable through population-based models. This study aims to compare several statistical forecasting models to determine their real-life applicability.OBJECTIVES: The primary objectives of this paper are to evaluate whether the following measurements: blood pressure, oxygen saturation, temperature and heart rate can predict deterioration in Intensive Care Unit (ICU) patients. Additionally, we aim to identify which of these measurements contributes most significantly to our prediction. Lastly, we seek to determine the most accurate data mining technique for real-life data applications.METHODS: This retrospective chart review study utilized data from patients admitted to the ICU at a tertiary hospital between January and December, 2019. Data mining techniques for prediction included logistic regression, support vector machine classifier, k-nearest neighbors (KNN), gradient boosting classifier, and Naive Bayes classifier. A comprehensive comparison of these techniques was performed, focusing on accuracy, precision, recall, and F-measure.RESULTS: To achieve the research objectives, the SelectKBest class was applied to extract the most contributory features for prediction. Blood pressure ranked first with a score of 9.98, followed by respiratory rate, temperature, and heart rate. Analysis of 653 patient records indicated that 129 patients expired, while 542 patients were discharged either to their homes or other facilities. Among the five training models, two demonstrated the highest accuracy in predicting patient deterioration or survival at 88.83% and 84.72%, respectively. The gradient boosting classifier accurately predicted 115 out of 129 expired patients, while the KNN correctly predicted 109 out of 129 expired patients.CONCLUSION: Machine learning has the potential to enhance clinical deterioration prediction compared to traditional methods. This allows healthcare professionals to implement preventative measures and improve patients' quality of life, ultimately increasing average life expectancy. Although our research focused exclusively on ICU patients, data mining techniques can be applied in various contexts both within and outside the hospital setting.
Vital Sign Forecasting for Sepsis Patients in ICUs

Abstract:Sepsis and septic shock are a critical medical condition affecting millions globally, with a substantial mortality rate. This paper uses state-of-the-art deep learning (DL) architectures to introduce a multi-step forecasting system to predict vital signs indicative of septic shock progression in Intensive Care Units (ICUs). Our approach utilizes a short window of historical vital sign data to forecast future physiological conditions. We introduce a DL-based vital sign forecasting system that predicts up to 3 hours of future vital signs from 6 hours of past data. We further adopt the DILATE loss function to capture better the shape and temporal dynamics of vital signs, which are critical for clinical decision-making. We compare three DL models, N-BEATS, N-HiTS, and Temporal Fusion Transformer (TFT), using the publicly available eICU Collaborative Research Database (eICU-CRD), highlighting their forecasting capabilities in a critical care setting. We evaluate the performance of our models using mean squared error (MSE) and dynamic time warping (DTW) metrics. Our findings show that while TFT excels in capturing overall trends, N-HiTS is superior in retaining short-term fluctuations within a predefined range. This paper demonstrates the potential of deep learning in transforming the monitoring systems in ICUs, potentially leading to significant improvements in patient care and outcomes by accurately forecasting vital signs to assist healthcare providers in detecting early signs of physiological instability and anticipating septic shock.

Abstract:Early detection of patient deterioration in the Intensive Care Unit (ICU) can play a crucial role in improving patient outcomes. Conventional severity scales currently used to predict patient deterioration are based on a number of factors, the majority of which consist of multiple investigations. Recent advancements in machine learning (ML) within the healthcare domain offer the potential to alleviate the burden of continuous patient monitoring. In this study, we propose an optimized ML model designed to leverage variations in vital signs observed during the final 24 hours of an ICU stay for outcome predictions. Further, we elucidate the relative contributions of distinct vital parameters to these outcomes The dataset compiled in real-time encompasses six pivotal vital parameters: systolic (0) and diastolic (1) blood pressure, pulse rate (2), respiratory rate (3), oxygen saturation (SpO2) (4), and temperature (5). Of these vital parameters, systolic blood pressure emerges as the most significant predictor associated with mortality prediction. Using a fivefold cross-validation method, several ML classifiers are used to categorize the last 24 hours of time series data after ICU admission into three groups: recovery, death, and intubation. Notably, the optimized Gradient Boosting classifier exhibited the highest performance in detecting mortality, achieving an area under the receiver-operator curve (AUC) of 0.95. Through the integration of electronic health records with this ML software, there is the promise of early notifications regarding adverse outcomes, potentially several hours before the onset of hemodynamic instability.
Forecasting acute kidney injury and resource utilization in ICU patients using longitudinal, multimodal models

Abstract:Background: Advances in artificial intelligence (AI) have realized the potential of revolutionizing healthcare, such as predicting disease progression via longitudinal inspection of Electronic Health Records (EHRs) and lab tests from patients admitted to Intensive Care Units (ICU). Although substantial literature exists addressing broad subjects, including the prediction of mortality, length-of-stay, and readmission, studies focusing on forecasting Acute Kidney Injury (AKI), specifically dialysis anticipation like Continuous Renal Replacement Therapy (CRRT) are scarce. The technicality of how to implement AI remains elusive. Objective: This study aims to elucidate the important factors and methods that are required to develop effective predictive models of AKI and CRRT for patients admitted to ICU, using EHRs in the Medical Information Mart for Intensive Care (MIMIC) database. Methods: We conducted a comprehensive comparative analysis of established predictive models, considering both time-series measurements and clinical notes from MIMIC-IV databases. Subsequently, we proposed a novel multi-modal model which integrates embeddings of top-performing unimodal models, including Long Short-Term Memory (LSTM) and BioMedBERT, and leverages both unstructured clinical notes and structured time series measurements derived from EHRs to enable the early prediction of AKI and CRRT. Results: Our multimodal model achieved a lead time of at least 12 h ahead of clinical manifestation, with an Area Under the Receiver Operating Characteristic Curve (AUROC) of 0.888 for AKI and 0.997 for CRRT, as well as an Area Under the Precision Recall Curve (AUPRC) of 0.727 for AKI and 0.840 for CRRT, respectively, which significantly outperformed the baseline models. Additionally, we performed a SHapley Additive exPlanation (SHAP) analysis using the expected gradients algorithm, which highlighted important, previously underappreciated predictive features for AKI and CRRT. Conclusion: Our study revealed the importance and the technicality of applying longitudinal, multimodal modeling to improve early prediction of AKI and CRRT, offering insights for timely interventions. The performance and interpretability of our model indicate its potential for further assessment towards clinical applications, to ultimately optimize AKI management and enhance patient outcomes.
Early prediction of circulatory failure in the intensive care unit using machine learning

Abstract:Intensive-care clinicians are presented with large quantities of measurements from multiple monitoring systems. The limited ability of humans to process complex information hinders early recognition of patient deterioration, and high numbers of monitoring alarms lead to alarm fatigue. We used machine learning to develop an early-warning system that integrates measurements from multiple organ systems using a high-resolution database with 240 patient-years of data. It predicts 90% of circulatory-failure events in the test set, with 82% identified more than 2 h in advance, resulting in an area under the receiver operating characteristic curve of 0.94 and an area under the precision-recall curve of 0.63. On average, the system raises 0.05 alarms per patient and hour. The model was externally validated in an independent patient cohort. Our model provides early identification of patients at risk for circulatory failure with a much lower false-alarm rate than conventional threshold-based systems.

Early hospital mortality prediction using vital signals

Abstract:Early hospital mortality prediction is critical as intensivists strive to make efficient medical decisions about the severely ill patients staying in intensive care units. As a result, various methods have been developed to address this problem based on clinical records. However, some of the laboratory test results are time-consuming and need to be processed. In this paper, we propose a novel method to predict mortality using features extracted from the heart signals of patients within the first hour of ICU admission. In order to predict the risk, quantitative features have been computed based on the heart rate signals of ICU patients. Each signal is described in terms of 12 statistical and signal-based features. The extracted features are fed into eight classifiers: decision tree, linear discriminant, logistic regression, support vector machine (SVM), random forest, boosted trees, Gaussian SVM, and K-nearest neighborhood (K-NN). To derive insight into the performance of the proposed method, several experiments have been conducted using the well-known clinical dataset named Medical Information Mart for Intensive Care III (MIMIC-III). The experimental results demonstrate the capability of the proposed method in terms of precision, recall, F1-score, and area under the receiver operating characteristic curve (AUC). The decision tree classifier satisfies both accuracy and interpretability better than the other classifiers, producing an F1-score and AUC equal to 0.91 and 0.93, respectively. It indicates that heart rate signals can be used for predicting mortality in patients in the ICU, achieving a comparable performance with existing predictions that rely on high dimensional features from clinical records which need to be processed and may contain missing information.

Abstract:Background : Acute heart failure (AHF) in the intensive care unit (ICU) is characterized by its criticality, rapid progression, complex and changeable condition, and its pathophysiological process involves the interaction of multiple organs and systems. This makes it difficult to predict in-hospital mortality events comprehensively and accurately. Traditional analysis methods based on statistics and machine learning suffer from insufficient model performance, poor accuracy caused by prior dependence, and difficulty in adequately considering the complex relationships between multiple risk factors. Therefore, the application of deep neural network (DNN) techniques to the specific scenario, predicting mortality events of patients with AHF under intensive care, has become a research frontier. Methods : This research utilized the MIMIC-IV critical care database as the primary data source and employed the synthetic minority over-sampling technique (SMOTE) to balance the dataset. Deep neural network models—backpropagation neural network (BPNN) and recurrent neural network (RNN), which are based on electronic medical record data mining, were employed to investigate the in-hospital death event judgment task of patients with AHF under intensive care. Additionally, multiple single machine learning models and ensemble learning models were constructed for comparative experiments. Moreover, we achieved various optimal performance combinations by modifying the classification threshold of deep neural network models to address the diverse real-world requirements in the ICU. Finally, we conducted an interpretable deep model using SHapley Additive exPlanations (SHAP) to uncover the most influential medical record features for each patient from the aspects of global and local interpretation. Results : In terms of model performance in this scenario, deep neural network models outperform both single machine learning models and ensemble learning models, achieving the highest Accuracy, Precision, Recall, F1 value, and Area under the ROC curve, which can reach 0.949, 0.925, 0.983, 0.953, and 0.987 respectively. SHAP value analysis revealed that the ICU scores (APSIII, OASIS, SOFA) are significantly correlated with the occurrence of in-hospital fatal events. Conclusions : Our study underscores that DNN-based mortality event classifier offers a novel intelligent approach for forecasting and assessing the prognosis of AHF patients in the ICU. Additionally, the ICU scores stand out as the most predictive features, which implies that in the decision-making process of the models, ICU scores can provide the most crucial information, making the greatest positive or negative contribution to influence the incidence of in-hospital mortality among patients with acute heart failure.

Abstract:Background and Objective Vital sign monitoring in the Intensive Care Unit (ICU) is crucial for enabling prompt interventions for patients. This underscores the need for an accurate predictive system. Therefore, this study proposes a novel deep learning approach for forecasting Heart Rate (HR), Systolic Blood Pressure (SBP), and Diastolic Blood Pressure (DBP) in the ICU. Methods We extracted 24,886 ICU stays from the MIMIC-III database which contains data from over 46 thousand patients, to train and test the model. The model proposed in this study, Transformer-based Diffusion Probabilistic Model for Sparse Time Series Forecasting (TDSTF), merges Transformer and diffusion models to forecast vital signs. The TDSTF model showed state-of-the-art performance in predicting vital signs in the ICU, outperforming other models' ability to predict distributions of vital signs and being more computationally efficient. Results The results of the study showed that TDSTF achieved a Standardized Average Continuous Ranked Probability Score (SACRPS) of 0.4438 and a Mean Squared Error (MSE) of 0.4168, an improvement of 18.9% and 34.3% over the best baseline model, respectively. The inference speed of TDSTF is more than 17 times faster than the best baseline model. Conclusion TDSTF is an effective and efficient solution for forecasting vital signs in the ICU, and it shows a significant improvement compared to other models in the field.

Indications of energetic consequences of decoherence at short times for scattering from open quantum systems

Abstract:Decoherence of quantum entangled particles is observed in most systems, and is usually caused by system-environment interactions. Disentangling two subsystems A and B of a quantum systemAB is tantamount to erasure of quantum phase relations between A and B. It is widely believed that this erasure is an innocuous process, which e.g. does not affect the energies of A and B. Surprisingly, recent theoretical investigations by different groups showed that disentangling two systems, i.e. their decoherence, can cause an increase of their energies. Applying this result to the context of neutronCompton scattering from H2 molecules, we provide for the first time experimental evidence which supports this prediction. The results reveal that the neutron-proton collision leading to the cleavage of the H-H bond in the sub-femtosecond timescale is accompanied by larger energy transfer (by about 3%) than conventional theory predicts. It is proposed to interpreted the results by considering the neutron-proton collisional system as an entangled open quantum system being subject to decoherence owing to the interactions with the “environment” (i.e., two electrons plus second proton of H2).
Explainable time-series deep learning models for the prediction of mortality, prolonged length of stay and 30-day readmission in intensive care patients

Abstract:Background In-hospital mortality, prolonged length of stay (LOS), and 30-day readmission are common outcomes in the intensive care unit (ICU). Traditional scoring systems and machine learning models for predicting these outcomes usually ignore the characteristics of ICU data, which are time-series forms. We aimed to use time-series deep learning models with the selective combination of three widely used scoring systems to predict these outcomes. Materials and methods A retrospective cohort study was conducted on 40,083 patients in ICU from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. Three deep learning models, namely, recurrent neural network (RNN), gated recurrent unit (GRU), and long short-term memory (LSTM) with attention mechanisms, were trained for the prediction of in-hospital mortality, prolonged LOS, and 30-day readmission with variables collected during the initial 24 h after ICU admission or the last 24 h before discharge. The inclusion of variables was based on three widely used scoring systems, namely, APACHE II, SOFA, and SAPS II, and the predictors consisted of time-series vital signs, laboratory tests, medication, and procedures. The patients were randomly divided into a training set (80%) and a test set (20%), which were used for model development and model evaluation, respectively. The area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and Brier scores were used to evaluate model performance. Variable significance was identified through attention mechanisms. Results A total of 33 variables for 40,083 patients were enrolled for mortality and prolonged LOS prediction and 36,180 for readmission prediction. The rates of occurrence of the three outcomes were 9.74%, 27.54%, and 11.79%, respectively. In each of the three outcomes, the performance of RNN, GRU, and LSTM did not differ greatly. Mortality prediction models, prolonged LOS prediction models, and readmission prediction models achieved AUCs of 0.870 ± 0.001, 0.765 ± 0.003, and 0.635 ± 0.018, respectively. The top significant variables co-selected by the three deep learning models were Glasgow Coma Scale (GCS), age, blood urea nitrogen, and norepinephrine for mortality; GCS, invasive ventilation, and blood urea nitrogen for prolonged LOS; and blood urea nitrogen, GCS, and ethnicity for readmission. Conclusion The prognostic prediction models established in our study achieved good performance in predicting common outcomes of patients in ICU, especially in mortality prediction. In addition, GCS and blood urea nitrogen were identified as the most important factors strongly associated with adverse ICU events.

Deep Learning Model Utilization for Mortality Prediction in Mechanically Ventilated ICU Patients

Abstract:Background: The requirement for mechanical ventilation has increased in recent years. Patients in the intensive care unit (ICU) who undergo mechanical ventilation often experience serious illness, contributing to a high risk of mortality. Predicting mortality for mechanically ventilated ICU patients helps physicians implement targeted treatments to mitigate risk. Methods: We extracted medical information of patients with invasive mechanical ventilation during ICU admission from the Medical Information Mart for Intensive Care III (MIMIC-III) dataset. This information includes demographics, disease severity, diagnosis, and laboratory test results. Patients who met the inclusion criteria were randomly divided into the training set (n=11,549, 70%), the test set (n=2,475, 15%), and the validation set (n=2,475, 15%). The Synthetic Minority Over-sampling Technique (SMOTE) was utilized to resolve the imbalanced dataset. After literature research, clinical expertise and an ablation study, we selected 12 variables which is fewer than the 66 features in the best existing literature. We proposed a deep learning model to predict the ICU mortality of mechanically ventilated patients, and established 7 baseline machine learning (ML) models for comparison, including K-nearest Neighbors (KNN), Logistic Regression, Decision Tree, Random Forest, Bagging, XGBoost, and Support Vector Machine (SVM). Area under the Receiver Operating Characteristic Curve (AUROC) was used as an evaluation metric for model performance. Results: Using 16,499 mechanically ventilated patients from the MIMIC-III database, the Neural Network model outperformed existing literature by 7.06%. It achieved an AUROC score of 0.879 (95% Confidence Interval (CI) [0.861-0.896]), an accuracy of 0.859 on the test set, and was well-calibrated with a Brier score of 0.0974, significantly exceeding previous best results. Conclusions: The proposed model demonstrated an exceptional ability to predict ICU mortality among mechanically ventilated patients. The SHAP analysis showed respiratory failure is a significant indicator of mortality prediction compared to other related respiratory dysfunction diseases. We also incorporated mechanical ventilation duration variable for the first time in our prediction model. We observed that patients with higher mortality rates tended to have longer mechanical ventilation times. This highlights the model's potential in guiding clinical decisions by indicating that longer mechanical ventilation may not necessarily enhance patient survival chances.

Abstract:Background In intensive care units (ICUs), accurate mortality prediction is crucial for effective patient management and resource allocation. The Simplified Acute Physiology Score II (SAPS-2), though commonly used, relies heavily on comprehensive clinical data and blood samples. This study sought to develop an artificial intelligence (AI) model utilizing key hemodynamic parameters to predict ICU mortality within the first 24 h and assess its performance relative to SAPS-2. Methods We conducted an analysis of select hemodynamic parameters and the structure of heart rate curves to identify potential predictors of ICU mortality. A machine-learning model was subsequently trained and validated on distinct patient cohorts. The AI algorithm's performance was then compared to the SAPS-2, focusing on classification accuracy, calibration, and generalizability. Measurements and main results The study included 1298 ICU admissions from March 27th, 2015, to March 27th, 2017. An additional cohort from 2022 to 2023 comprised 590 patients, resulting in a total dataset of 1888 patients. The observed mortality rate stood at 24.0%. Key determinants of mortality were the Glasgow Coma Scale score, heart rate complexity, patient age, duration of diastolic blood pressure below 50 mmHg, heart rate variability, and specific mean and systolic blood pressure thresholds. The AI model, informed by these determinants, exhibited a performance profile in predicting mortality that was comparable, if not superior, to the SAPS-2. Conclusions The AI model, which integrates heart rate and blood pressure curve analyses with basic clinical parameters, provides a methodological approach to predict in-hospital mortality in ICU patients. This model offers an alternative to existing tools that depend on extensive clinical data and laboratory inputs. Its potential integration into ICU monitoring systems may facilitate more streamlined mortality prediction processes.

Abstract:Detecting and predicting septic shock early is crucial for the best possible outcome for patients. Accurately forecasting the vital signs of patients with sepsis provides valuable insights to clinicians for timely interventions, such as administering stabilizing drugs or optimizing infusion strategies. Our research examines N-BEATS, an interpretable deep-learning forecasting model that can forecast 3 hours of vital signs for sepsis patients in intensive care units (ICUs). In this work, we use the N-BEATS interpretable configuration to forecast the vital sign trends and compare them with the actual trend to understand better the patient's changing condition and the effects of infused drugs on their vital signs. We evaluate our approach using the publicly available eICU Collaborative Research Database dataset and rigorously evaluate the vital sign forecasts using out-of-sample evaluation criteria. We present the performance of our model using error metrics, including mean squared error (MSE), mean average percentage error (MAPE), and dynamic time warping (DTW), where the best scores achieved are 18.52e-4, 7.60, and 17.63e-3, respectively. We analyze the samples where the forecasted trend does not match the actual trend and study the impact of infused drugs on changing the actual vital signs compared to the forecasted trend. Additionally, we examined the mortality rates of patients where the actual trend and the forecasted trend did not match. We observed that the mortality rate was higher (92%) when the actual and forecasted trends closely matched, compared to when they were not similar (84%).

Abstract:Background: Cardiac arrest is a leading cause of mortality prior to discharge for children admitted to the pediatric intensive care unit. To address this problem, we used machine learning to predict cardiac arrest up to three hours in advance. Methods: Our data consists of 240 Hz ECG waveform data, 0.5 Hz physiological time series data, medications, and demographics from 1,145 patients in the pediatric intensive care unit at the Johns Hopkins Hospital, 15 of whom experienced a cardiac arrest. The data were divided into training, validating, and testing sets, and features were generated every five minutes. 23 heart rate variability (HRV) metrics were determined from ECG waveforms. 96 summary statistics were calculated for 12 vital signs, such as respiratory rate and blood pressure. Medications were classified into 42 therapeutic drug classes. Binary features were generated to indicate the administration of these different drugs. Next, six machine learning models were evaluated: logistic regression, support vector machine, random forest, XGBoost, LightGBM, and a soft voting ensemble. Results: XGBoost performed the best, with 0.971 auROC, 0.797 auPRC, 99.5% sensitivity, and 69.6% specificity on an independent test set. Conclusion: We have created high-performing models that identify signatures of in-hospital cardiac arrest (IHCA) that may not be evident to clinicians. These signatures include a combination of heart rate variability metrics, vital signs data, and therapeutic drug classes. These machine learning models can predict IHCA up to three hours prior to onset with high performance, allowing clinicians to intervene earlier, improving patient outcomes.

Abstract:BACKGROUND:Bedside monitors in the ICU routinely measure and collect patients' physiologic data in real time to continuously assess the health status of patients who are critically ill. With the advent of increased computational power and the ability to store and rapidly process big data sets in recent years, these physiologic data show promise in identifying specific outcomes and/or events during patients' ICU hospitalization.METHODS:We introduced a methodology designed to automatically extract information from continuous-in-time vital sign data collected from bedside monitors to predict if a patient will experience a prolonged stay (length of stay) on mechanical ventilation, defined as >4 d, in a pediatric ICU.RESULTS:Continuous-in-time vital signs information and clinical history data were retrospectively collected for 284 ICU subjects from their first 24 h on mechanical ventilation from a medical-surgical pediatric ICU at Boston Children's Hospital. Multiple machine learning models were trained on multiple subsets of these subjects to predict the likelihood that each of these subjects would experience a long stay. We evaluated the predictive power of our models strictly on unseen hold-out validation sets of subjects. Our methodology achieved model performance of >83% (area under the curve) by using only vital sign information as input, and performances of 90% (area under the curve) by combining vital sign information with subjects' static clinical data readily available in electronic health records. We implemented this approach on 300 independently trained experiments with different choices of training and hold-out validation sets to ensure the consistency and robustness of our results in our study sample. The predictive power of our approach outperformed recent efforts that used deep learning to predict a similar task.CONCLUSIONS:Our proposed workflow may prove useful in the design of scalable approaches for real-time predictive systems in ICU environments, exploiting real-time vital sign information from bedside monitors.
