Abstract:Background: Despite initial recovery from critical illness patients requiring intensive care unit (ICU) admission, many patients remain risk of subsequent deterioration or death. And unplanned Readmission to the ICU has been associated with worsening of the original disease process, higher hospital costs, and increased in hospital mortality. The purpose of this study was to identify the risk factors and assess the performance of ML and DL models that predict readmission or death within 7 days after discharge from the ICU. Methods: We defined as a readmission to the ICU or unexpected death within 7 days after discharge from the ICU to the ward. This study introduces a clinical decision support system and verifies it for predicting real-time readmission or death occurrences within the ICU by using MIMIC-IV. We develop various machine learning (ML) and deep learning (DL) models for predicting the readmission or death in real time using the MIMIC-IV. To address the challenge of missing values in time series, we propose a novel recurrent neural network model, GRU-D++. Results: There were a total of 3398 cases in the death or readmission within 7 days after ICU discharge, and 46,863 cases were successfully discharged. Among the ML models, the RF model showed an accuracy of AUROC 0.799. The fine-tuned GRU-D++ model(DL model) demonstrated superior performances, achieving an AUROC of 0.800. The proposed GRU-D++, designed to consider temporal information and missing values, stands out for its predictive accuracy. Conclusion: Our findings suggest that this model could be a tool to early detect and treat patient deterioration after ICU discharge and prevent ICU readmission.

Abstract:The intensive care unit (ICU) comprises a complex hospital environment, where decisions made by clinicians have a high level of risk for the patients' lives. A comprehensive care pathway must then be followed to reduce p complications. Uncertain, competing and unplanned aspects within this environment increase the difficulty in uniformly implementing the care pathway. Readmission contributes to this pathway's difficulty, occurring when patients are admitted again to the ICU in a short timeframe, resulting in high mortality rates and high resource utilisation. Several works have tried to predict readmission through patients' medical information. Although they have some level of success while predicting readmission, those works do not properly assess, characterise and understand readmission prediction. This work proposes a standardised and explainable machine learning pipeline to model patient readmission on a multicentric database (i.e., the eICU cohort with 166,355 patients, 200,859 admissions and 6,021 readmissions) while validating it on monocentric (i.e., the MIMIC IV cohort with 382,278 patients, 523,740 admissions and 5,984 readmissions) and multicentric settings. Our machine learning pipeline achieved predictive performance in terms of the area of the receiver operating characteristic curve (AUC) up to 0.7 with a Random Forest classification model, yielding an overall good calibration and consistency on validation sets. From explanations provided by the constructed models, we could also derive a set of insightful conclusions, primarily on variables related to vital signs and blood tests (e.g., albumin, blood urea nitrogen and hemoglobin levels), demographics (e.g., age, and admission height and weight), and ICU-associated variables (e.g., unit type). These insights provide an invaluable source of information during clinicians' decision-making while discharging ICU patients.

Abstract:Background In-hospital mortality, prolonged length of stay (LOS), and 30-day readmission are common outcomes in the intensive care unit (ICU). Traditional scoring systems and machine learning models for predicting these outcomes usually ignore the characteristics of ICU data, which are time-series forms. We aimed to use time-series deep learning models with the selective combination of three widely used scoring systems to predict these outcomes. Materials and methods A retrospective cohort study was conducted on 40,083 patients in ICU from the Medical Information Mart for Intensive Care-IV (MIMIC-IV) database. Three deep learning models, namely, recurrent neural network (RNN), gated recurrent unit (GRU), and long short-term memory (LSTM) with attention mechanisms, were trained for the prediction of in-hospital mortality, prolonged LOS, and 30-day readmission with variables collected during the initial 24 h after ICU admission or the last 24 h before discharge. The inclusion of variables was based on three widely used scoring systems, namely, APACHE II, SOFA, and SAPS II, and the predictors consisted of time-series vital signs, laboratory tests, medication, and procedures. The patients were randomly divided into a training set (80%) and a test set (20%), which were used for model development and model evaluation, respectively. The area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and Brier scores were used to evaluate model performance. Variable significance was identified through attention mechanisms. Results A total of 33 variables for 40,083 patients were enrolled for mortality and prolonged LOS prediction and 36,180 for readmission prediction. The rates of occurrence of the three outcomes were 9.74%, 27.54%, and 11.79%, respectively. In each of the three outcomes, the performance of RNN, GRU, and LSTM did not differ greatly. Mortality prediction models, prolonged LOS prediction models, and readmission prediction models achieved AUCs of 0.870 ± 0.001, 0.765 ± 0.003, and 0.635 ± 0.018, respectively. The top significant variables co-selected by the three deep learning models were Glasgow Coma Scale (GCS), age, blood urea nitrogen, and norepinephrine for mortality; GCS, invasive ventilation, and blood urea nitrogen for prolonged LOS; and blood urea nitrogen, GCS, and ethnicity for readmission. Conclusion The prognostic prediction models established in our study achieved good performance in predicting common outcomes of patients in ICU, especially in mortality prediction. In addition, GCS and blood urea nitrogen were identified as the most important factors strongly associated with adverse ICU events.

Abstract:Readmission to the Coronary Care Unit (CCU) has significant implications for patient outcomes and healthcare expenditure, emphasizing the urgency to accurately identify patients at high readmission risk. This study aims to construct and externally validate a predictive model for CCU readmission using machine learning (ML) algorithms across multiple hospitals. Patient information, including demographics, medical history, and laboratory test results were collected from electronic health record system and contributed to a total of 40 features. Three ML models, Logistic Regression, Random Forest, and Gradient Boosting were employed to estimate the readmission risk. The gradient boosting model was selected demonstrated superior performance with an Area Under the Receiver Operating Characteristic Curve (AUC) of 0.887 in the internal validation set. Further external validation in hold-out test set and three other medical centers upheld the model’s robustness with consistent high AUCs, ranging from 0.852 to 0.879. The results endorse the integration of ML algorithms in healthcare to enhance patient risk stratification, potentially optimizing clinical interventions and diminishing the burden of CCU readmissions.

A Machine Learning Based Discharge Prediction of Cardiovascular Diseases Patients in Intensive Care Units

Abstract:This paper targets a major challenge of how to effectively allocate medical resources in intensive care units (ICUs). We trained multiple regression models using the Medical Information Mart for Intensive Care III (MIMIC III) database recorded in the period between 2001 and 2012. The training and validation dataset included pneumonia, sepsis, congestive heart failure, hypotension, chest pain, coronary artery disease, fever, respiratory failure, acute coronary syndrome, shortness of breath, seizure and transient ischemic attack, and aortic stenosis patients' recorded data. Then we tested the models on the unseen data of patients diagnosed with coronary artery disease, congestive heart failure or acute coronary syndrome. We included the admission characteristics, clinical prescriptions, physiological measurements, and discharge characteristics of those patients. We assessed the models' performance using mean residuals and running times as metrics. We ran multiple experiments to study the data partition's impact on the learning phase. The total running time of our best-evaluated model is 123,450.9 mS. The best model gives an average accuracy of 98%, highlighting the location of discharge, initial diagnosis, location of admission, drug therapy, length of stay and internal transfers as the most influencing patterns to decide a patient's readiness for discharge.

Abstract:Background Patient monitoring is vital in all stages of care. In particular, intensive care unit (ICU) patient monitoring has the potential to reduce complications and morbidity, and to increase the quality of care by enabling hospitals to deliver higher-quality, cost-effective patient care, and improve the quality of medical services in the ICU. Objective We here report the development and validation of ICU length of stay and mortality prediction models. The models will be used in an intelligent ICU patient monitoring module of an Intelligent Remote Patient Monitoring (IRPM) framework that monitors the health status of patients, and generates timely alerts, maneuver guidance, or reports when adverse medical conditions are predicted. Methods We utilized the publicly available Medical Information Mart for Intensive Care (MIMIC) database to extract ICU stay data for adult patients to build two prediction models: one for mortality prediction and another for ICU length of stay. For the mortality model, we applied six commonly used machine learning (ML) binary classification algorithms for predicting the discharge status (survived or not). For the length of stay model, we applied the same six ML algorithms for binary classification using the median patient population ICU stay of 2.64 days. For the regression-based classification, we used two ML algorithms for predicting the number of days. We built two variations of each prediction model: one using 12 baseline demographic and vital sign features, and the other based on our proposed quantiles approach, in which we use 21 extra features engineered from the baseline vital sign features, including their modified means, standard deviations, and quantile percentages. Results We could perform predictive modeling with minimal features while maintaining reasonable performance using the quantiles approach. The best accuracy achieved in the mortality model was approximately 89% using the random forest algorithm. The highest accuracy achieved in the length of stay model, based on the population median ICU stay (2.64 days), was approximately 65% using the random forest algorithm. Conclusions The novelty in our approach is that we built models to predict ICU length of stay and mortality with reasonable accuracy based on a combination of ML and the quantiles approach that utilizes only vital signs available from the patient’s profile without the need to use any external features. This approach is based on feature engineering of the vital signs by including their modified means, standard deviations, and quantile percentages of the original features, which provided a richer dataset to achieve better predictive power in our models.

[Prediction of Intensive Care Unit Readmission for Critically Ill Patients Based on Ensemble Learning].

Abstract:OBJECTIVE:To develop machine learning models for predicting intensive care unit (ICU) readmission using ensemble learning algorithms.METHODS:A publicly accessible American ICU database, medical information mart for intensive care (MIMIC)-Ⅲ as the data source was used, and the patients were selected by the inclusion and exclusion criteria. A set of variables that had the predictive ability of outcome including demographics, vital signs, laboratory tests, and comorbidities of patients were extracted from the dataset. We built the ICU readmission prediction models based on ensemble learning methods including random forest, adaptive boosting (AdaBoost), and gradient boosting decision tree (GBDT), and compared the prediction performance of the machine learning models with a conventional Logistic regression model. Five-fold cross validation was used to train and validate the prediction models. Average sensitivity, positive prediction value, negative prediction value, false positive rate, false negative rate, area under the receiver operating characteristic curve (AUROC) and Brier score were used as performance measures. After constructing the prediction models, top 10 predictive variables based on importance ranking were identified by the model with the best discrimination.RESULTS:Among these ICU readmission prediction models, GBDT (AUROC=0.858) had better performance than random forest (AUROC=0.827), and was slightly superior to AdaBoost (AUROC=0.851) in terms of AUROC. Compared with Logistic regression (AUROC=0.810), the discrimination of the three ensemble learning models was much better. The feature importance provided by GBDT showed that the top ranking variables included vital signs and laboratory tests. The patients with ICU readmission had higher mean arterial pressure, systolic blood pressure, diastolic blood pressure, and heart rate than the patients without ICU readmission. Meanwhile, the patients readmitted to ICU experienced lower urine output and higher serum creatinine. Overall, the patients having repeated admissions during their hospitalization showed worse heart function and renal function compared with the patients without ICU readmission.CONCLUSION:The ensemble learning based ICU readmission prediction models had better performance than Logistic regression model. Such ensemble learning models have the potential to aid ICU physicians in identifying those patients with high risk of ICU readmission and thus help improve overall clinical outcomes.

Abstract:Abstract Background Intensive Care Unit (ICU) readmissions in patients with heart failure (HF) result in a significant risk of death and financial burden for patients and healthcare systems. Prediction of at-risk patients for readmission allows for targeted interventions that reduce morbidity and mortality. Methods and results We presented a process mining/deep learning approach for the prediction of unplanned 30-day readmission of ICU patients with HF. A patient’s health records can be understood as a sequence of observations called event logs; used to discover a process model. Time information was extracted using the DREAM (Decay Replay Mining) algorithm. Demographic information and severity scores upon admission were then combined with the time information and fed to a neural network (NN) model to further enhance the prediction efficiency. Additionally, several machine learning (ML) algorithms were developed to be used as the baseline models for the comparison of the results. Results By using the Medical Information Mart for Intensive Care III (MIMIC-III) dataset of 3411 ICU patients with HF, our proposed model yielded an area under the receiver operating characteristics (AUROC) of 0.930, 95% confidence interval of [0.898–0.960], the precision of 0.886, sensitivity of 0.805, accuracy of 0.841, and F-score of 0.800 which were far better than the results of the best baseline model and the existing literature. Conclusions The proposed approach was capable of modeling the time-related variables and incorporating the medical history of patients from prior hospital visits for prediction. Thus, our approach significantly improved the outcome prediction compared to that of other ML-based models and health calculators.

Abstract:Background: Patients in serious condition due to COVID-19 often require special care in intensive care units (ICUs). This disease has affected over 758 million people and resulted in 6.8 million deaths worldwide. Additionally, the progression of the disease may vary from individual to individual, that is, it is essential to identify the clinical parameters that indicate a good prognosis for the patient. Machine learning (ML) algorithms have been used for analyzing complex medical data and identifying prognostic indicators. However, there is still an urgent need for a model to elucidate the predictors related to patient outcomes. Therefore, this research aimed to verify, through ML, the variables involved in the discharge of patients admitted to the ICU due to COVID-19. Methods: In this study, 126 variables were collected with information on demography, hospital length stay and outcome, chronic diseases and tumors, comorbidities and risk factors, complications and adverse events, health care, and vital indicators of patients admitted to an ICU in southern Brazil. These variables were filtered and then selected by a ML algorithm known as decision trees to identify the optimal set of variables for predicting patient discharge using logistic regression. Finally, a confusion matrix was performed to evaluate the model's performance for the selected variables. Results: Of the 532 patients evaluated, 180 were discharged: female (16.92%), with a central venous catheter (23.68%), with a bladder catheter (26.13%), and with an average of 8.46- and 23.65-days using bladder catheter and submitted to mechanical ventilation, respectively. In addition, the chances of discharge increase by 14% for each additional day in the hospital, by 136% for female patients, 716% when there is no bladder catheter, and 737% when no central venous catheter is used. However, the chances of discharge decrease by 3% for each additional year of age and by 9% for each other day of mechanical ventilation. The performance of the training data presented a balanced accuracy of 0.81, sensitivity of 0.74, specificity of 0.88, and the kappa value was 0.64. The test performance had a balanced accuracy of 0.85, sensitivity 0.75, specificity 0.95, and kappa value of 0.73. The McNemar test found that there were no significant differences in the error rates in the training and test data, suggesting good classification. This work showed that female, the absence of a central venous catheter and bladder catheter, shorter mechanical ventilation, and bladder catheter duration were associated with a greater chance of hospital discharge. These results may help develop measures that lead to a good prognosis for the patient.
Interpretable Machine Learning-Based Clinical Predictive Model for Early Readmission in Patients with Cardiogenic Shock

Abstract:Background/Purpose: Cardiogenic shock (CS) is a critical condition characterized by low cardiac output leading to end-organ hypoperfusion and often multisystem organ failure, affecting up to 50,000 people annually in the United States. Acute myocardial infarction (AMI) is the primary cause, responsible for 81% of CS cases. Despite advancements in reperfusion therapies improving survival, in-hospital mortality remains high at 40%-67%, with 18.6% of survivors readmitted within 30 days. Traditional methods struggle to quantify and process the complex interactions among various risk factors, making prediction of readmissions challenging. Machine learning (ML) offers a promising solution by capturing intricate patterns and non-linear relationships among numerous variables. This study aims to develop an ML-based prediction model for 7-day and 30-day readmission rates in CS patients using the 2019 National Readmission Database (NRD). Additionally, the study utilizes SHapley Additive exPlanations (SHAP) to interpret the outcomes of the applied machine learning methods. Method: We conducted a retrospective study using the NRD for 2019. Index hospitalizations were identified by non-elective admissions with a primary ICD-10 diagnosis of cardiogenic shock. Exclusions included patients under 18, missing length of stay or days to event data, and same-day transfers. The primary outcome was readmission within 7 and 30 days post-discharge. Welch's t-test compared continuous variables. Various ML models were evaluated for their predictive performance, and SHAP values were used to interpret the most influential features. Results: The study included 97,653 adults hospitalized for CS, with a mean age of 65.8 years and 38.4% being female. The in-hospital mortality rate was 33.7%. Among 51,976 index hospitalizations, 8.3% were readmitted within 7 days, and 21.02% within 30 days. Significant predictors of higher readmission rates included younger age, lower income, Medicaid insurance, CKD3, drug abuse, chronic pulmonary disease, PHTN, depression, leukemia, lymphoma, discharge against medical advice, and certain hospital characteristics. The FT-Transformer (a specialized deep neural network approach for tabular data) model achieved the highest AUCs of 0.76 and 0.78 for 7-day and 30-day readmissions, respectively, outperforming traditional methods like Logistic Regression (AUCs: 0.60 and 0.63). SHAP analysis revealed a wide array of features contributing to readmission predictions at both the population and individual levels. For the general population, the top features included APRDRG, DRG_NoPOA, age, chronic kidney disease, length of stay, number of ICD-10 codes, and disposition at discharge. In contrast, for an individual patient, the most influential feature for predicting 7-day readmission may differ, though there are some overlaps. This highlights the potential of personalized medicine, where individual risk factors are weighted differently compared to the general population, providing tailored insights for targeted interventions. Conclusion: This study demonstrates that advanced ML models, particularly the FT-Transformer and Random Forest, significantly outperform traditional methods in predicting readmissions in CS patients. The use of SHAP values enhances the interpretability of these models, providing actionable insights for healthcare providers. The differentiation between general population feature contributions and individual-specific factors underscores the importance of personalized medicine. By understanding individual risk profiles, healthcare providers can implement more precise and effective interventions, ultimately aiming to reduce readmissions and optimize healthcare outcomes.

Abstract:A hospital readmission is when a patient who was discharged from the hospital is admitted again for the same or related care within a certain period. Hospital readmissions are a significant problem in the healthcare domain, as they lead to increased hospitalization costs, decreased patient satisfaction, and increased risk of adverse outcomes such as infections, medication errors, and even death. The problem of hospital readmissions is particularly acute in intensive care units (ICUs), due to the severity of the patients' conditions, and the substantial risk of complications. Predicting Unplanned Readmissions in ICUs is a challenging task, as it involves analyzing different data modalities, such as static data, unstructured free text, sequences of diagnoses and procedures, and multivariate time-series. Here, we investigate the effectiveness of each data modality separately, then alongside with others, using state-of-the-art machine learning approaches in time-series analysis and natural language processing. Using our evaluation process, we are able to determine the contribution of each data modality, and for the first time in the context of readmission, establish a hierarchy of their predictive value. Additionally, we demonstrate the impact of Temporal ions in enhancing the performance of time-series approaches to readmission prediction. Due to conflicting definitions in the literature, we also provide a clear definition of the term Unplanned Readmission to enhance reproducibility and consistency of future research and to prevent any potential misunderstandings that could result from diverse interpretations of the term. Our experimental results on a large benchmark clinical data set show that Discharge Notes written by physicians, have better capabilities for readmission prediction than all other modalities.

Abstract:Introduction: Despite significant therapeutic advancements, Atherosclerotic Cardiovascular Disease (ASCVD) patients require frequent hospitalization. Machine learning (ML) algorithms present an opportunity to develop improved and more generalizable prediction models for 30-day hospital readmission due to ASCVD. Objectives: The current study aims to leverage electronic health record (EHR) data to predict 30-day hospital readmission in high-risk ASCVD patients. Methods: This study utilized the MIMIC III database (a publicly available large, single-centre critical care database of 46,520 patients) comprising of deidentified ~60,000 ICU admissions. Adult patients with the first admission due to ASCVD event and whose length of stay (LOS) in the ICU was >48 hours were included. To develop a prediction model, features representing groups of diagnosis data along with demographics and length of stay in the hospital were used. The performance of ML models was evaluated using the area under the receiver operating characteristic curves (AUCs). Results: Our cohort consisted of 22,666 admissions (mean age 70.1 years) due to an ASCVD event. Out of the 9022 readmissions within 180 days, 60.5% were observed within 30 days of discharge. Among the 17 prospective predictors age, LOS and first hospitalization due to coronary atherosclerosis, heart failure, and haemorrhage issues were the most important factors to predict readmission within 30 days (Fig A). Random Forest was the best performing model with an AUC of 0.66 (Fig B). Conclusions: This model can be helpful in predicting the readmission of high-risk patients after the first ASCVD event which might address the huge unmet need of aiding healthcare resource planning, better patient care, and prevention of rehospitalization and death by intensifying treatment interventions in those patients and taking appropriate decisions to discharge patients based on their age, LOS and ASCVD events resulting in first hopitalization.

Forecasting Patient Early Readmission from Irish Hospital Discharge Records Using Conventional Machine Learning Models

Abstract:Background/Objectives: Predicting patient readmission is an important task for healthcare risk management, as it can help prevent adverse events, reduce costs, and improve patient outcomes. In this paper, we compare various conventional machine learning models and deep learning models on a multimodal dataset of electronic discharge records from an Irish acute hospital. Methods: We evaluate the effectiveness of several widely used machine learning models that leverage patient demographics, historical hospitalization records, and clinical diagnosis codes to forecast future clinical risks. Our work focuses on addressing two key challenges in the medical fields, data imbalance and the variety of data types, in order to boost the performance of machine learning algorithms. Furthermore, we also employ SHapley Additive Explanations (SHAP) value visualization to interpret the model predictions and identify both the key data features and disease codes associated with readmission risks, identifying a specific set of diagnosis codes that are significant predictors of readmission within 30 days. Results: Through extensive benchmarking and the application of a variety of feature engineering techniques, we successfully improved the area under the curve (AUROC) score from 0.628 to 0.7 across our models on the test dataset. We also revealed that specific diagnoses, including cancer, COPD, and certain social factors, are significant predictors of 30-day readmission risk. Conversely, bacterial carrier status appeared to have minimal impact due to lower case frequencies. Conclusions: Our study demonstrates how we effectively utilize routinely collected hospital data to forecast patient readmission through the use of conventional machine learning while applying explainable AI techniques to explore the correlation between data features and patient readmission rate.

Abstract:Predicting clinical patients' vital signs is a leading critical issue in intensive care units (ICUs) related studies. Early prediction of the mortality of ICU patients can reduce the overall mortality and cost of complication treatment. Some studies have predicted mortality based on electronic health record (EHR) data by using machine learning models. However, the semi-structured data (i.e., patients' diagnosis data and inspection reports) is rarely used in these models. This study utilized data from the Medical Information Mart for Intensive Care III. We used a Latent Dirichlet Allocation (LDA) model to classify text in the semi-structured data of some particular topics and established and compared the classification and regression trees (CART), logistic regression (LR), multivariate adaptive regression splines (MARS), random forest (RF), and gradient boosting (GB). A total of 46,520 ICU Patients were included, with 11.5% mortality in the Medical Information Mart for Intensive Care III group. Our results revealed that the semi-structured data (diagnosis data and inspection reports) of ICU patients contain useful information that can assist clinical doctors in making critical clinical decisions. In addition, in our comparison of five machine learning models (CART, LR, MARS, RF, and GB), the GB model showed the best performance with the highest area under the receiver operating characteristic curve (AUROC) (0.9280), specificity (93.16%), and sensitivity (83.25%). The RF, LR, and MARS models showed better performance (AUROC are 0.9096, 0.8987, and 0.8935, respectively) than the CART (0.8511). The GB model showed better performance than other machine learning models (CART, LR, MARS, and RF) in predicting the mortality of patients in the intensive care unit. The analysis results could be used to develop a clinically useful decision support system.

Abstract:Introduction and aim The prediction of sepsis mortality of intensive care unit (ICU) observations using machine learning (ML) methods are hypothesized to yield better or as good as performance compared to the prognostic scores. This paper aims to show that the accuracy of ML in mortality estimation may be superior and supportive knowledge to SAPS II, APACHE II and SOFA (traditional) scores. Patients and methods The retrospective collection of data from the patients ( n = 200) admitted to ICU of Acibadem Hospital, Istanbul-Turkey, between 2015 and 2020 is utilized to detect the sepsis mortality risk using eight ML methods along with the traditional prognostic scores. The mortality as decisive indicator is evaluated according to the explanatory variables included to quantify the traditional scores. In the calibration of the data, five different predetermined split of the data is used for training and the validation of the ML methods. The efficiency of the prediction results of ML methods and the ICU scoring methods are investigated by AUC-ROC curves and other ML accuracy indicators. Consecutive processes of Box-Cox and Min-Max transformations on data, and parameter optimization (Grid Search) are performed to increase the efficiency of ML methods. Results The accuracy in the mortality prediction is achieved the best by Multi-Layer Perceptron method compared to SAPS II and APACHE II methods, and is as good the one with what SOFA predicts. Prediction accuracies (AUC) in ML methods ranges between 75% -86%, 63%–82% and 73%–81% compared to the ones obtained for these three scores, respectively. Additionally, the outcomes show that non-tree based algorithms implemented first time in this paper can be utilized as ML tool in predicting mortality even for small sample. Conclusion The outcomes of this study have clinical merits in evaluating on the potential use of ML methods in predicting ICU mortality superior to traditional scores APACHE II, SAPS II and as good as SOFA. Additionally, it highlights which of the variables contributing to the risk of sepsis mortality should be taken as apriori information in treating the patients.

Abstract:Readmission to intensive care units (ICU) in sepsis patients is costly and associated with poor patient outcomes. Prediction of ICU readmission in patients with sepsis is of particular interest for physicians as they can take early interventions, which would help to optimize the ICU discharge time and avoid deterioration of patient conditions. In this study, we used an openly accessible database to identify patients with sepsis and built models for 30-day ICU readmission prediction. In order to reduce the high dimensionality of the dataset and avoid model overfitting, we adopted two feature selection approaches, sequential forward selection and recursive feature elimination to identify a subset of the most predictive features. After feature selection, we built ICU readmission prediction models for sepsis patients using eXtreme Gradient Boosting (XGBoost), Multi-Layer Perceptron and Logistic Regression. We compared prediction performance of the three models and found that XGBoost-based model has the best discrimination. This model has the potential to aid ICU physicians in identifying sepsis patients with a high risk of ICU readmission and thus may help improve the clinical outcomes.

Abstract:Early detection of patient deterioration in the Intensive Care Unit (ICU) can play a crucial role in improving patient outcomes. Conventional severity scales currently used to predict patient deterioration are based on a number of factors, the majority of which consist of multiple investigations. Recent advancements in machine learning (ML) within the healthcare domain offer the potential to alleviate the burden of continuous patient monitoring. In this study, we propose an optimized ML model designed to leverage variations in vital signs observed during the final 24 hours of an ICU stay for outcome predictions. Further, we elucidate the relative contributions of distinct vital parameters to these outcomes The dataset compiled in real-time encompasses six pivotal vital parameters: systolic (0) and diastolic (1) blood pressure, pulse rate (2), respiratory rate (3), oxygen saturation (SpO2) (4), and temperature (5). Of these vital parameters, systolic blood pressure emerges as the most significant predictor associated with mortality prediction. Using a fivefold cross-validation method, several ML classifiers are used to categorize the last 24 hours of time series data after ICU admission into three groups: recovery, death, and intubation. Notably, the optimized Gradient Boosting classifier exhibited the highest performance in detecting mortality, achieving an area under the receiver-operator curve (AUC) of 0.95. Through the integration of electronic health records with this ML software, there is the promise of early notifications regarding adverse outcomes, potentially several hours before the onset of hemodynamic instability.

Abstract:Background: In the ICU, patients with acute myocardial infarction and cardiogenic shock (AMI-CS) often face high mortality rates, making timely and precise mortality risk prediction crucial for clinical decision-making. Despite existing models, machine learning algorithms hold the potential for improved predictive accuracy. Methods: In this study, a predictive model was developed using the MIMIC-IV database, with external validation performed on the eICU-CRD database. We included ICU patients diagnosed with AMI-CS. Feature selection was conducted using the Boruta algorithm, followed by the construction and comparison of four machine learning models: Logistic Regression (LR), eXtreme Gradient Boosting (XGBoost), Adaptive Boosting (AdaBoost), and Gaussian Naive Bayes (GNB). Model performance was evaluated based on metrics such as AUC (Area Under the Curve), accuracy, sensitivity, specificity, and so on. The SHAP method was employed to visualize and interpret the importance of model features. Finally, we constructed an online prediction model and conducted external validation in the eICU-CRD database. Results: In this study, a total of 570 and 391 patients with AMI-CS were included from the MIMIC-IV and eICU-CRD databases, respectively. Among all machine learning algorithms evaluated, LR exhibited the best performance with a validation set AUC of 0.841(XGBoost: 0.835, AdaBoost: 0.839, GNB: 0.826). The model incorporated five variables: prothrombin time, blood urea nitrogen, age, beta-blockers and Angiotensin-Converting Enzyme Inhibitors or Angiotensin II Receptor Blockers. SHAP plots are employed to visualize the importance of model features and to interpret the results. An online prediction tool was developed, externally validated with the eICU-CRD database, achieving an AUC of 0.755. Conclusion: Employing the LR algorithm, we developed a predictive model for assessing the mortality risk among AMI-CS patients in the ICU setting. Through model predictions, this facilitates early detection of high-risk individuals, ensures judicious allocation of healthcare resources.
