Abstract:This study with a T-52 class wheelchair racing athlete aimed to combine quantitative biomechanical measurements to the athlete's perception to design and test different prototypes of a new kind of rigid gloves. Three personalized rigid gloves with various, fixed wrist extension angles were prototyped and tested on a treadmill in a biomechanics laboratory. The prototype with 45° wrist extension was the athlete's favourite as it reduced his perception of effort. Biomechanical assessment and user-experience data indicated that his favourite prototype increased wrist stability throughout the propulsion cycle while maintaining a very similar propulsion technique to the athlete's prior soft gloves. Moreover, the inclusion of an innovative attachment system on the new gloves allowed the athlete to put his gloves on by himself, eliminating the need for external assistance and thus significantly increasing his autonomy. This multidisciplinary approach helped to prototype and develop a new rigid personalized gloves concept and is clearly a promising avenue to tailor adaptive sports equipment to an athlete's needs.

Abstract:Wheelchair sports are an important tool in the rehabilitation of people with severe chronic disabilities and have been a driving force for innovation in technology and practice. In this paper, we will present an overview of the adaptive technology used in Paralympic sports with a special focus on wheeled technology and the impact of design on performance (defined as achieving the greatest level of athletic ability and minimizing the risk of injury). Many advances in manual wheelchairs trace their origins to wheelchair sports. Features of wheelchairs that were used for racing and basketball 25 or more years ago have become integral to the manual wheelchairs that people now use every day; moreover, the current components used on ultralight wheelchairs also have benefitted from technological advances developed for sports wheelchairs. For example, the wheels now used on chairs for daily mobility incorporate many of the components first developed for sports chairs. Also, advances in manufacturing and the availability of aerospace materials have driven current wheelchair design and manufacture. Basic principles of sports wheelchair design are universal across sports and include fit; minimizing weight while maintaining high stiffness; minimizing rolling resistance; and optimizing the sports-specific design of the chair. However, a well-designed and fitted wheelchair is not sufficient for optimal sports performance: the athlete must be well trained, skilled, and use effective biomechanics because wheelchair athletes face some unique biomechanical challenges.

Abstract:As participation in wheelchair sports increases, the need of quantitative assessment of biomechanical performance indicators and of sports- and population-specific training protocols has become central. The present study focuses on junior wheelchair basketball and aims at (i) proposing a method to identify biomechanical performance indicators of wheelchair propulsion using an instrumented in-field test and (ii) developing a training program specific for the considered population and assessing its efficacy using the proposed method. Twelve athletes (10 M, 2 F, age = 17.1 ± 2.7 years, years of practice = 4.5 ± 1.8) equipped with wheelchair- and wrist-mounted inertial sensors performed a 20-metre sprint test. Biomechanical parameters related to propulsion timing, progression force, and coordination were estimated from the measured accelerations and used in a regression model where the time to complete the test was set as dependent variable. Force- and coordination-related parameters accounted for 80% of the dependent variable variance. Based on these results, a training program was designed and administered for three months to six of the athletes (the others acting as control group). The biomechanical indicators proved to be effective in providing additional information about the wheelchair propulsion technique with respect to the final test outcome and demonstrated the efficacy of the developed program.

Abstract:The difficulty of designing equipment for individuals who have some degree of special needs is that each individual has a unique clinical picture, hampering the development of new technologies. In this sense, this article explores the ways in which the design and evaluation of virtual devices can be combined with new prototyping techniques to create and develop equipment that will allow wheelchair accessibility to be improved. To this end, a computer vision control structure and a virtual software were created to assist in the development of a wheelchair coupling project, in order to study and evaluate the functional characteristics of the device. In this way, the present work incorporates a virtual system that represents the automation and mechanical design of new wheelchairs. Thus defining a prototyping methodology with the aid of virtual environments, based on movement controls adaptive to the parameters stipulated in real environments. Therefore, the present work results in a simulator for an electric wheelchair docking platform for multiple disabilities, which allows the validation of the project, the acquisition of development needs, the proposition of improvements in the preliminary acquisition platform and a system of computer vision control.

Abstract:Abstract The equipment for evaluating the propulsion of a wheelchair is very complex and expensive. To validate a new dynamometer prototype for assessing the propulsion capacity of wheelchairs, 21 healthy subjects (age: 20.9±2.4 yr; weight: 68.9±7.9 kg; height: 174.0±7.1 cm; BMI: 22.7±2.5 kg·m−2) who do not normally require wheelchairs performed a sprint protocol for 20 s after a 1-min warm-up. The power and rotation data acquired by the prototype (both right and left sides) were compared with those of a reference system via high-speed videography (240 fps). The results showed high levels of accordance (95% CI), excellent values for the intraclass correlation coefficient (ICC: .99; P<0.00), no significant differences in the rotation (P=0.91) and power (P=0.94) between the methods. The proposed equipment met the validation criteria and thus can be applied as a new tool for assessing wheelchair propulsion.

Personalized Tests in Paralympic Athletes: Aerobic and Anaerobic Performance Profile of Elite Wheelchair Rugby Players

Abstract:In Paralympic sports, the goal of functional classifications is to minimize the impact of impairment on the outcome of the competition. The present cross-sectional study aimed to investigate aerobic and anaerobic personalized tests in Paralympic athletes and to correlate them with the classification of the international wheelchair rugby federation (IWRF). Sixteen elite players of the Italian wheelchair rugby team volunteered for the study. Aerobic (incremental test to exhaustion) and anaerobic (Wingate 30s all-out test, 5 and 10-meter sprint test, shuttle test, isometric test) sport-performance measurements were correlated singularly or grouped (Z scores) with the classification point. Moreover, a multivariate permutation-based ranking analysis investigated possible differences in the overall level of performance among the adjacent classified groups of players, considering the scores of each test. A statistically significant correlation between the performance parameters and the IWRF functional classification considering both aerobic and anaerobic personalized tests was detected (0.58 ≤ r ≤ 0.88; 0.0260 ≤ p ≤ 0.0001). The multivariate permutation-based ranking analysis showed differences only for the low-pointers versus mid-pointers (p = 0.0195) and high-pointers (p = 0.0075). Although single performance parameters correlated with athletes' classification point, results of the multivariate permutation-based ranking analysis seem to suggest considering only the most significant anaerobic and sport-specific performance parameters among athletes. These should be combined with the physical assessment and the qualitative observation, which are already part of the classification process to improve its effectiveness.

Abstract:Continuous Passive Motion - CPM - is a therapeutic method that provides fast and safe patient rehabilitation. Considering the fact that the equipment performs the movement along with the patient, who also does it but passively, this treatment procedure can immediately be applied in the postoperative care period. Although this technique makes recovery faster, as strains will be spared from ligaments, muscles and tendons, not all clinics can offer CPM therapy due to its high cost. Bearing in mind the existence of such equipment that performs the aforementioned motion, the objective of this work was the development and manufacture of a prototype device for continuous passive shoulder movement, with costs reduced to an approximate value of USD$730.00 compared to the average price in the market which is around USD$1510.00. This difference in values would make it affordable for a greater majority of clinics and offices to purchase them and, thus, promote the accelerated timely recovery of the limb after operation, for a larger number of patients, allowing these patients to return to their daily tasks in shorter period of time. For the execution of the prototype, specific mechanical manufacturing methods, such us turning, milling and welding of materials, were applied in order to make the equipment safe and effective, provide comfort for the patient and, above all, make the device affordable for most physiotherapy clinics. The material used for the production was stainless steel, cables, pulleys, tilting automation kit and inductive sensors.
A Portable and Cost-Effective Upper Extremity Rehabilitation System for Individuals with Upper Limb Motor Deficits

Abstract:Long-term rehabilitation opportunities are critical for millions of individuals with chronic upper limb motor deficits striving to improve their motor performance through self-managed rehabilitation programs. However, there is minimal professional support of rehabilitation across the lifespan. In this paper, we introduce an upper extremity rehabilitation system, the Quality of Movement Feedback-Oriented Measurement System (QM-FOrMS), by integrating cost-effective portable sensors and clinically verified motion quality analysis towards individuals with upper limb motor deficits. Specifically, QM-FOrMS is comprised of an eTextile pressure sensitive mat, named Smart Mat, a sensory can, named Smart Can, and a mobile device. A personalizable and adaptive upper limb rehabilitation program is developed, including both unilateral and bilateral functional activities which can be selected from a list or custom designed to further tailor the program to the individual. Quantitative evaluation of the motor performance from the QM-FOrMS is derived from fine-grained kinematic measurements. We ran a pilot study with three groups, including five baseline subjects (i.e., healthy young adults), six older adults and four individuals with movement impairment. The experimental results show that QM-FOrMS can provide the detailed feature during the unattended rehabilitation exercise, and proposed metrics can distinguish the evaluation results across group.

Abstract:Introduction Currently studies indicate the need to incorporate the user`s perspective in the testing of new assistive technologies. The objective of this paper is to test a baropodometric insole prototype for monitoring and treatment weight-bearing asymmetry, according to the Participatory Design. Methods We used a qualitative case study approach during the testing phase of the baropodometric insole prototype. The focus group approach addressed topics related to the experience and accessibility of the potential user in conjunction with professionals, researchers, and physiotherapy students. Facilitators, barriers, and requirements for the device were collected through audio recordings of the discussions during and after prototype testing. Results Key steps in the prototype testing process were divided into (1) Test of the prototype according to the Participatory Design, divided into Who, When, How, and Why the potential user was involved in the study; and (2) Facilitators, barriers and requirements to improve the prototype. Conclusions The baropodometric insole prototype can be seen as a promising device for monitoring and treating weight-bearing asymmetry.

Abstract:Summary An increase in total Na, due in large part to an increase in cell Na, was measured in the freshly excised rat tail artery during the course of DOCA-saline treatment. Since this change was associated with a fall in cell K, was first observed at 2 weeks, coincident with the rise in blood pressure, and was not sustained during subsequent immersion of the artery at zero pressure, it probably reflects the high in vivo intravascular pressure. In the incubated artery, cell Na is significantly reduced early n the course of treatment, while cell K falls late. Thus, Na transport in the artery is under direct attack from the start, but it is suggested that this leads to hypertrophy rather than to vasoconstriction.

Abstract:A large percentage of the world population is currently affected by functional impairments. In this context, the prescription of assistive technology products and services contributes to an individual's quality of life. However, high abandonment rates of such devices have been reported and are commonly associated with the non-correspondence to users' needs. The design of customized products requires a systematization level that is not well achieved by traditional methods, and customization-oriented approaches usually promote restricted user participation. This paper introduces an iterative participatory procedure focused on both early and conception stages of the design of assistive technology. It integrates data collection tools with field-specific assessment protocols, the house of quality technique for decision-making, and an effective participation of potential users. Due to the lack of functional adequacy observed in shower chairs for adults, such equipment was selected for a case study. The procedure comprises an iterative, spiral-shaped flowchart interspersed with interdisciplinary tasks, and the case study revealed complex and intricate interactions with the final user in both phase of design cross-domain establishment and different conceptual design presentations. The use of design techniques integrated with assistive technology assessment protocols provided greater fluidity in the communication between the interdisciplinary design team, represented by researchers in engineering design and occupational therapy. Three versions of the conceptual design were generated through the implementation of user feedback and technical improvements, showing a good correspondence with the initially gathered requirements and being satisfactorily evaluated by health professionals.

Abstract:Objective: to develop an algorithm for customized selection of a wheelchair for the disabled persons with limited mobility followed by design of a software allowing to automate the process of wheelchair selection. Materials and methods. Analytical review of English literature was performed to address the issues in prescription and utilization of the assistive devices and technologies (ADT). The regulatory documents governing the ADT prescription in the Russian Federation (RF) were studied. Russian market of the wheelchairs for the disabled persons was analyzed. Ten disabled persons with traumatic spinal cord injury to the department of medical rehabilitation of the Centre were examined to pilot the software for automated wheelchair selection. Results. After the literature review results were structured, the major groups of factors that should be considered when selecting a wheelchair were identified: 1) anthropometric measurement, 2) physical and functional capabilities of the disabled person, 3) level of everyday activity, 4) availability and physical capabilities of the caregivers, 5) particularities of the environmental factors. The algorithm for customized selection of wheelchair was designed. The database comprising the technical characteristics of 141 wheelchair models that are taken as a basis of the software was developed. IBM-compatible computer was used to develop the software. Programming language: Delphi, data base management system: Microsoft Access 2013. Piloting results showed the effectiveness of the software. In all cases appropriate wheelchairs were selected. Conclusion. Within current study the software for automated process of customized wheelchair selection was piloted. The software takes into consideration the level of activities and participation of the disabled persons and significant environmental factors which allows to customize the ADT prescription.

Abstract:In this paper, a design and construction of a cycling-based wheelchair prototype with leg-exoskeletons are proposed with product considerations for persons who have lower limb problems. For the conceptual and mechanical design, an exoskeleton linkage attached to wheelchair is designed carefully for comfortable considerations to users. Particularly, functional flowchart and control is proposed to enable the prototype to help users to move around and to exercise simultaneously. In addition, two prototypes built in the years of 2015 and 2017 are shown and compared to describe the characteristics of the solutions. At last, the product considerations are analyzed according to our design.

Abstract:Abstract This article presents a design of an experiment for investigating the effect of changing the geometry of Paralympic alpine sit-ski poles/outriggers in the LW 10-12 class. An experiment design for mapping an individual athlete’s performance parameters has been developed, with a resolution for finding the optimal outrigger geometry. By prototyping an adjustable experiment setup with implemented sensor systems, the performance increase can be analysed and implemented in new equipment. Results show that changing double poling geometry provides a substantial performance increase, regarding time and propulsive force.

Abstract:This article introduces a method to extract the speed and density of microparticles in real time at several kHz using an asynchronous event-based camera mounted on a full-field optical coherence tomography (FF-OCT) setup. These cameras detect significant amplitude changes, allowing scene-driven acquisitions. They are composed of an array of autonomously operating pixels. Events are triggered when an illuminance change at the pixel level is significant at 1μs time precision. The event-driven FF-OCT algorithm relies on a time-based optical flow computation to operate directly on incoming events and updates the estimation of velocity, direction and density while reducing both computation and data load. We show that for fast moving microparticles in a range of 0.4 - 6.5mm/s, the method performs faster and more efficiently than existing techniques in real time. The target application of this work is to evaluate erythrocyte dynamics at the microvascular level in vivo with a high temporal resolution.

Abstract:Athlete impairment level is an important factor in wheelchair mobility performance (WMP) in sports. Classification systems, aimed to compensate impairment level effects on performance, vary between sports. Improved understanding of resemblances and differences in WMP between sports could aid in optimizing the classification methodology. Furthermore, increased performance insight could be applied in training and wheelchair optimization. The wearable sensor-based wheelchair mobility performance monitor (WMPM) was used to measure WMP of wheelchair basketball, rugby and tennis athletes of (inter-)national level during match-play. As hypothesized, wheelchair basketball athletes show the highest average WMP levels and wheelchair rugby the lowest, whereas wheelchair tennis athletes range in between for most outcomes. Based on WMP profiles, wheelchair basketball requires the highest performance intensity, whereas in wheelchair tennis, maneuverability is the key performance factor. In wheelchair rugby, WMP levels show the highest variation comparable to the high variation in athletes’ impairment levels. These insights could be used to direct classification and training guidelines, with more emphasis on intensity for wheelchair basketball, focus on maneuverability for wheelchair tennis and impairment-level based training programs for wheelchair rugby. Wearable technology use seems a prerequisite for further development of wheelchair sports, on the sports level (classification) and on individual level (training and wheelchair configuration).
