Abstract:Objective: This study aimed to investigate how informal primary caregivers of individuals with dementia living in the community cope with caring-related measures of anxiety, depression, burden and quality of life. Participants and Design: Participants included 60 informal caregivers (23 males and 37 females) of community-dwelling individuals with dementia who attended a state-run geriatric day hospital in Malta. Caregiver measures included the Hospital Anxiety and Depression Scale, the World Health Organization Quality of Life–BREF and Brief COPE questionnaires, and Zarit Burden Interview. The Mini Mental State Examination and Barthel Index of Activities of Daily Living scores were used to determine the degree of cognitive impairment and performance in activities of daily living of care-recipients. Results: Informal caregivers experienced anxiety and depression with both emotional distress states negatively affecting all quality of life domains. Depression and burden experienced by informal primary caregivers showed a strong association with individuals with dementia-related variables such as age, cognitive impairment and activities of daily living scores. The use of dysfunctional coping strategies was found to be related to caregivers’ emotional distress, low quality of life and burden. Conclusion: The findings indicate that informal primary caregivers experienced anxiety and depression, had a lower quality of life, and feel burdened during their caring role. However, caregivers making use of emotion-focused coping strategies were found to be protected against emotional distress. The results highlight the need of providing support services aimed at promoting the psychological wellbeing of informal carers of community-dwelling individuals with dementia.

Abstract:Objectives: To assess health-related quality of life (QoL) in caregivers of elderly patients with chronic disabilities receiving, or not receiving, social worker support. Methods: This multicenter open-label randomized study assigned caregivers to receive an information booklet, exclusively, or with social worker support. Caregivers completed Short Form-36 (SF-36) and Hospital Anxiety Depression Scale quarterly, and Zarit Burden Interview each semester, for 24 months. We reported caregiver QoL mean changes at 12 and 24 months (M12, M24). Longitudinal QoL analysis up to M24 used mixed models for repeated measures (MMRM). Results: Among the 179 caregivers randomized from 2015 to 2019, the SF-36 physical and mental component summary showed no significant changes at M12 and M24, in terms of neither anxiety nor burden. However, depression significantly increased (M12: 1.4 ± 4.0; M24: 1.7 ± 4.1) with significant adjusted mean increase using MMRM at M24: 3.4 [0.6–2.5] in the control group, exclusively. Conclusion: These findings call for better recognition of the social support to prevent caregiver QoL deterioration and alleviate their depression early in the course of the disease. The Actions of Propofol on &ggr;-Aminobutyric Acid-A and Glycine Receptors in Acutely Dissociated Spinal Dorsal Horn Neurons of the Rat

Abstract:The spinal cord plays an important role in modulating anesthetic-induced suppression of nociceptive transmission. To gain some insight into the anesthetic mechanisms of propofol at the spinal level, we investigated the direct action of propofol and its modulation on the &ggr;-aminobutyric acid-A receptor (GABAAR) and the glycine receptor (GlyR) in acutely dissociated rat spinal dorsal horn neurons by using whole-cell patch-clamp electrophysiology. Propofol induced Cl− currents (ICl), which were sensitive to bicuculline and, to a lesser extent, to strychnine. The activation, desensitization, and deactivation of propofol-induced ICl were slower than those of GABA- and glycine-induced ICl. In addition, this study revealed similar modulatory actions of propofol on GABAAR and GlyR. Propofol potentiated both GABA- and glycine-induced ICl at small con-centrations and inhibited both GABA- and glycine-induced ICl at large concentrations. The potentiation of propofol on ICl was caused by slowing current desensitization and deactivation, whereas the inhibition actions might be involved in the cross-desensitization between GABA- and propofol-induced ICl and the cross-inhibition between the GABAAR and GlyR. The results suggest that propofol facilitation of GABAAR and GlyR at the spinal level could contribute significantly to general anesthetic-induced analgesia and anesthesia.

Abstract:Aging has been a growing source of increased opportunities raised by longevity, but also a growing source of concern,particularly with regard to the mental health and the well-being of elderly people. As such, it is highly relevant to have a better understanding of the relationship between workplace related psychosocial factors and quality of life, not only among working population, but also among retired individuals. The main objective of this study is to better understand and characterize the relation between quality of life (QoL), work-related psychosocial conditions, and social support for both working and retired adults. Participants were 1.330, 62.2% of which female, with ages ranging between 55 and 75 years old. 47% of the respondents were working and 46 % were retired. Three structural equation models were tested, using the total sample and by labor situation (working and retired). The results showed that the stress management (with a greater effect in retired participants) and the social support (with a greater effect in working participants) have a positive influence on the QoL factor. Better work-related psychosocial conditions and higher social support were predictors of higher/better QoL. Our study allowed us to conclude that QoL is influenced by working situation, social support and work-related psychosocial conditions. This is a powerful message to policy makers in view of the growth of older populations.

Abstract:Background: In view of demographic change, there is a growing need for qualified nursing professionals. More and more people in need of care prefer to be cared for at home. A shortage of skilled workers and constantly changing and increasing professional requirements are some of the consequences. The aim of this study is to generate further insights into work-related psychosocial stress factors and the resources of outpatient caregivers in their subjective perceptions to derive approaches for possible health-promoting interventions for practice and research that promote healthier and more sustainable work. In addition, coping strategies and needs should be inquired about in order to determine support offers. Methods: Using a semi-structured interview guide, thirty outpatient nursing professionals in Germany were interviewed in 2022. The subject of this interview was the psychosocial working conditions and resources in outpatient care. The collected data were evaluated by means of the Kuckartz qualitative content analysis. Results: This qualitative study identified work-related job demands and resources in outpatient care. The way nurses are treated in the companies they work for and the appreciation of patients were explored as important resources. Physical demands, the time factor, and dealing with personal fates are primarily mentioned as stress factors. The learning effect plays an important role in dealing with these. Conclusion: This study contributes to a better understanding of current job strain and resources as well as job satisfaction in outpatient care. In any case, it became clear that the nursing staff love their profession, which they see more as a vocation. Future quantitative studies are necessary to build on these findings. First ideas of implications for health promotion measures in research and practice have already been derived.

Abstract:Background: Caring for individuals with mental disorders poses significant challenges for caregivers, often leading to compromised quality of life and mental health issues such as stress, anxiety, and depression. This study aims to assess the extent of these challenges among caregivers in Greece, identifying which demographic factors influence their well-being. Method: A total of 157 caregivers were surveyed using the SF-12 Health Survey for quality-of-life assessment and the DASS-21 questionnaire for evaluating stress, anxiety, and depression symptoms. t-tests, Kruskal–Wallis tests, Pearson's correlation coefficients, and regression analyses were applied to understand the associations between demographics, quality of life, and mental health outcomes. Results: The study found that caregivers, especially women and younger individuals, faced high levels of mental health challenges. Marital status, educational level, and employment status also significantly influenced caregivers' well-being. Depression was the most significant factor negatively correlating with the mental component of quality of life. The magnitude of the burden experienced by caregivers highlighted the urgency for targeted social and financial support, as well as strategic treatment programs that consider caregiver well-being. Conclusions: Caregivers of individuals with mental disorders endure significant stress, anxiety, and depression, influencing their quality of life. Demographic factors such as age, gender, marital status, education, and employment status have notable impacts. Findings emphasize the need for society-wide recognition of caregivers' roles and the creation of comprehensive support and intervention programs to alleviate their burden, particularly in the context of the COVID-19 pandemic.

Abstract:So far, limited research has been carried out to better understand the interplay between the emotions, the use of emotion regulation strategies, and the well-being of professional caregivers of People with Dementia (PwD). This pilot study ( N = 43 professional caregivers) aimed to (1) describe the type and frequency of emotions experienced at work; (2) analyze the associations between experienced emotions, emotion regulation strategies, and well-being; and (3) test whether the use of specific emotion regulation strategies moderates the relationship between experienced emotions and emotional exhaustion. In the challenging context of professionally caring for PwD, results suggest that (1) caregivers experience positive emotions more frequently than negative emotions; (2) caregivers using relatively inappropriate regulation strategies are more likely to experience negative emotions, less likely to experience positive emotions, and have poorer physical and mental health; and (3) expressive suppression significantly moderates the relationship between positive experienced emotions and emotional exhaustion.

Abstract:Background: Carers of people with Alzheimer's disease often have a high degree of commitment and dedication which may also compromise physical and emotional, leisure, and occupational self-care. This study aimed to explore health-related quality of life (HRQoL) and psychoemotional variables in caregivers with and without caregiver overload and its relationship. Methods: A single-measure cross-sectional correlational study was carried out involving 59 informal caregivers of people with Alzheimer's disease with a mean age of 59.30 (±10.58). The participants completed the adult HRQoL questionnaires (EQ-5D-3L), Zarit Burden Inventory test, General Happiness Questionnaire, Satisfaction with Life Scale, Rosenberg self-esteem scale, Occupational Balance Questionnaire (OBQ-E), International Fitness Scale (IFIS), Family Apgar scale, and Duke-UNC-11 Functional Social Support Questionnaire. Results: A significantly higher level of HRQoL (p = 0.029) in subjective happiness (p = 0.018), perceived social support (p = 0.046), avoidance (p = 0.034), occupational balance (p = 0.002), life satisfaction (p = 0.037), and self-perceived physical fitness (p = 0.021) was found in caregivers without perceived overload. Also, HRQoL was directly associated with self-perceived physical fitness (β = 0.534; p < 0.001) and occupational balance (β = 0.375; p < 0.001) and self-esteem (β = 0.249; p < 0.016). Conclusions: Caregivers who do not perceive overload have better levels of HRQoL and psychoemotional variables, establishing a relationship between HRQoL with self-perceived physical fitness, occupational balance, and self-esteem.

Abstract:Background: Palliative care, a crucial aspect of healthcare, faces challenges in psychosocial and emotional management among nurses. With an increasing need for palliative services globally, addressing the emotional well-being of nurses becomes pivotal. Aim: To explore nurses' psychosocial and emotional work experiences in a palliative care department. The focus is on understanding the challenges, identifying coping strategies, and assessing the impact on professional and private life when facing those experiences. Methods: A phenomenologic-hermeneutical study involving semi-structured interviews was conducted to comprehend the lived experiences of ten nurses working in a hospital's palliative care department in Spain. Hermeneutic analysis was employed to extract patterns and insights from their experiences. The COREQ checklist was used to report this study. Findings: Palliative care nurses perceived insufficient preparation in emotional management, grappling with complex family interactions and unique work dynamics. They highlighted the significance of self-protection strategies, experience, clinical sessions, and external resources. Limited training in emotional resilience and challenges posed by the COVID-19 pandemic emerged as organisational barriers. Nurses expressed a desire for enhanced training and resources. Conclusion: The study uncovered a deficiency in the emotional preparedness of palliative care nurses, impacting their professional and personal lives. Despite positive aspects, the emotional toll necessitates attention and intervention. Implications for nursing policy: Comprehensive emotional training for palliative care nurses, addressing barriers, providing resources, and acknowledging emotional labour is necessary. Reinstating paused training sessions and considering specific challenges like those posed by the pandemic are vital. Supporting nurses in their professional and private lives is crucial for sustaining quality care in palliative care departments.

Abstract:Abstract Background. Better knowledge on caregivers is required to address their needs with dedicated support. The results from the ICE study explored the health-related quality of life (HRQoL) in caregivers of elderly with chronic disabilities, after social intervention or not. Methods. Caregivers of patients (≥60 years) with cancer, neurodegenerative disease, stroke, or age-related macular degeneration were randomized 1:1 in the supportive intervention group (support from social workers and information booklet) or in the control group (information booklet only), and completed questionnaires (short form-36 questionnaire (SF-36), Hospital Anxiety Depression Scale (HADS), and Zarit Burden Interview (ZBI) quarterly (6-monthly for ZBI) during two years. Caregiver characteristics, and caregiver HRQoL assessment through SF-36, HADS, ZBI scores at baseline, at 12 months (M12) and 24 months (M24) on an individual and an aggregate level, were described using paired-t test comparisons, and minimal important difference. Longitudinal assessments of caregiver HRQoL over the two first years were reported. Results. From 2015 to 2019, 179 caregivers supported patients with cancer (n=92, 52%), Alzheimer (n=46, 26%), Parkinson (n=20, 11%) were randomized. No evidence differences in SF-36 PCS and MCS summary scores mean change over time, neither in HADS anxiety and burden ZBI scores were shown. However, HADS depression mean changes showed a clinically significant increase of 1.4 (4.0) at M12, and 1.7 (4.1) at M24 in control group. Conclusion. Appropriate support need to be proposed to improve the global caregiver HRQoL, and reduce depression at 1 and 2 years. Psychological stress of general practitioners in the care of patients with palliative care needs: an exploratory study

Abstract:Background: In Germany, general practitioners play a pivotal role in palliative care provision. Caring for patients with palliative care needs can be a burden for general practitioners, highlighting the importance of self-care and mental health support. This study aimed to explore the role of palliative care in general practitioners' daily work, the stressors they experience, their coping mechanisms, and the potential benefits of Advance Care Planning in this context. Methods: An exploratory approach was employed, combining a short quantitative survey with qualitative interviews. The analysis was based on a structuring qualitative content analysis, following a deductive-inductive procedure and integrating the Stress-Strain Model and Lazarus' Transactional Model of Stress and Coping. We recruited eleven general practitioners to take part in the study. Results: General practitioners viewed palliative care as integral to their practice but faced challenges such as time constraints and perceived expertise gaps. Societal taboos often hindered conversations on the topic of death. Most general practitioners waited for their patients to initiate the topic. Some general practitioners viewed aspects of palliative care as potentially distressing. They used problem-focused (avoiding negative stressors, structuring their daily schedules) and emotion-focused (discussions with colleagues) coping strategies. Still, general practitioners indicated a desire for specific psychological support options. Advance Care Planning, though relatively unfamiliar, was acknowledged as valuable for end-of-life conversations. Conclusions: Palliative care can be associated with negative psychological stress for general practitioners, often coming from external factors. Despite individual coping strategies in place, it is advisable to explore concepts for professional psychological relief. Trial registration: Not registered.

Abstract:Background Traditionally, the psychological well-being of healthcare workers has been taken for granted — it has even been considered a part of the requirements that were demanded of them. When these professionals have experienced suffering and psychological depletion, they have been held accountable for this suffering, adopting an individualistic and reductionist viewpoint focused only on the professional. This approach has become obsolete due to its proven ineffectiveness, especially from an ethics of responsibility and organization viewpoint. Context The psychological well-being of the healthcare worker (and its opposites: suffering, exhaustion, and disenchantment) is advantageous to the professional's commitment to the institution, to their work performance, and to their personal life. Objective The objective of this paper is to reflect on the psychological suffering of the palliative care professional. Method We will reflect on the three levels of responsibility that influence such suffering (micro-meso-macro-ethical; worker-environment-institution). Results We will propose a global strategy for the care of psychological well-being supported by scientific evidence and key references. Significance of results We conclude with some contributions on what we have learned and still have to learn on this topic.

[End of life conflicts: perceptions of health professionals in French speaking Valais]

Abstract:The aim of the study was to identify the perception health professionals in palliative care had of end of life conflicts. The nature of t he conflicts,their prevalence, intensity, causes andeffects were specially addressed. Furthermore proposals of possible solutions of conflicts were investigated. A cross-sectional study was done through a self-administered questionnaire among 460 health professionals in the French Speaking Valais. The health professionals worked either in acute care hospitals, chronic health institutions, and old people nursing homes or in ambulatory palliative care units. Among the target population 233 persons replied to the questionnaire (50.6%). Among the respondents 82.4% reported to be confronted to end of life conflicts. Conflicts were quite rare (66%) and of medium or high intensity (86%). Conflicts were of medical nature (74%) or related to ethic issues (64%). Conflicts occurred among family members (68%), medical doctors and nurses (49%) or patients et medical staff (16%). Conflicts were due mainly (50-60%) to lack of communication, ambivalence among parties, differences in value systems. Mentioned (50-60%) consequences of conflicts were the waste of resources, the physical and the psychological burnout. Possible identified solutions to conflicts were interdisciplinary seminars, internal supervision procedure and the sharing of experiences. Neither age, nor gender, professional experience, working par place, training were predictive of exposure toconflicts. End of life conflicts are a common reality. They may compromise the quality of care and may contribute to high economic and social costs. They should be put on the public health agenda as a high priority.

Abstract:Palliative care and oncology generate a risk of burnout and psychological distress in professionals. The purpose of this study is to identify both psychopathological and positive factors related to mental health at work. It aims (i) to explore the extent to which these professionals are confronted with suffering, illness, and death; and to explore the prevalence of psychological distress and/or burnout, (ii) to identify potential determinants of burnout and psychological wellbeing at work, (iii) to develop an integrative model of mental health; and to identify frequency and impact of confrontations with death, and (iv) to identify profiles of professionals are at risk of developing a mental health disorder or, conversely, characterized by wellbeing.

"We Won't Retire Without Skeletons in the Closet": Healthcare-Related Regrets Among Physicians and Nurses in German-Speaking Swiss Hospitals

Abstract:Physicians and nurses are expected to systematically provide high-quality healthcare in a context marked by complexity, time pressure, heavy workload, and the influence of nonclinical factors on clinical decisions. Therefore, healthcare professionals must eventually deal with unfortunate events to which regret is a typical emotional reaction. Using semistructured interviews, 11 physicians and 13 nurses working in two different hospitals in the German-speaking part of Switzerland reported a total of 48 healthcare-related regret experiences. Intense feelings of healthcare-related regrets had far-reaching repercussions on participants' health, work-life balance, and medical practice. Besides active compensation strategies, social capital was the most important coping resource. Receiving superiors' support was crucial for reaffirming professional identity and helped prevent healthcare professionals from quitting their job. Findings suggest that training targeting emotional coping could be beneficial for quality of life and may ultimately lead to lower job turnover among healthcare professionals.

Abstract:Twin and family studies provide evidence of a genetic component in migraine, in particular migraine with aura (MA). Familial hemiplegic migraine (FHM) is a rare monogenic subtype of MA for which three causative genes have been identified: CACNA1A (FHM1), ATP1A2 (FHM2), and SCN1A (FHM3). Mutations in these genes are also found in some patients with sporadic hemiplegic migraine. Linkage studies have identified several gene loci for the more common forms of migraine; however, identification of the respective causative genes is still pending. This review summarizes recent developments in the genetics of migraine and their implications for molecular genetic testing. We further discuss the roles of CACNA1A, ATP1A2, and SCN1A in the pathophysiology of cortical spreading depression, which is the likely correlate of migraine aura.
New method to determine oxygen cost for contractility.

Abstract:We developed a new method to determine the oxygen cost for myocardial contractility and applied it to epinephrine in the excised cross-circulated dog heart. We utilized the relation between myocardial oxygen consumption (VO2) and the systolic pressure-volume area (PVA) which represents the total mechanical energy generated by contraction. We first obtained a reference VO2-PVA relation in a baseline contractile state. Then, the end-diastolic and stroke volumes were fixed constant and a global index of ventricular contractility, Emax, was enhanced by infusing epinephrine. The VO2-PVA data point was shifted linearly right-upward with the increases in Emax. From the slopes of both the reference VO2-PVA relation line and the regression line of VO2 on PVA during the gradually increased Emax, we calculated the oxygen cost for contractility, i.e., the ratio of the elevation of the VO2-PVA relation to enhanced Emax in each heart. The ratio was 0.00095 +/- 0.00013 ml O2.ml.mmHg-1.beat-1.100 g LV-2. The result indicates that the oxygen cost for contractility can be reliably and efficiently determined by this new method.

Abstract:In a context marked by negative health indicators that make structural aspects more salient, this paper aimed at understanding and explaining the processes and determinants at work that positively and negatively interfere with the professionals’ health in the French public nursing home environment. To this purpose, the qualitative approach by grounded theory was chosen. In total, 90 semi-structured interviews were recorded and 43 were transcribed; in addition, 10 observations of 46 participations in meetings and working groups were carried out in four public service and hospital establishments. Our results indicate that the role of health workers, its definition, and its execution are fundamental to the understanding of their health at work. Two protective and constructive processes are involved in the maintenance and development of the professionals’ health in this work, with considerable confrontations with death and suffering: individual and collective control of emotional and cognitive commitment, and the development of resources for formation, information, and cooperation. Nonetheless, they are jeopardized when a lasting imbalance is generated between the work’s demands and the available resources. This leads to a loss spiral in organizational, inter-individual, and individual resources that makes it difficult to sustain work.

Abstract:INTRODUCTION Recent studies showed that physicians' well-being, happiness, and sense of meaning can easily be challenged and even threatened, while their coping mechanisms are often insufficient to prevent exhaustion and burnout. The aim of this case series is to assess these components; to see how they are interconnected and to compare results with previous studies. METHODS Case series. The Meaning of Life Questionnaire (MLQ), the Job-related Affective Well-being Scale (JAWS), the Brief-COPE, the Subjective Happiness Scale (SHS), the Self-Compassion Scale (SCS-SF), the Perceived Stress Scale (PSS 10), the Depression Anxiety Stress Scale (DASS-10) are used. RESULTS Results of self-reported questionnaires are shown. Individual results from a case report are compared with results from one previous research in the literature together with correlations between questionnaires from previous research. CONCLUSIONS Physicians' timely awareness of their job-related wellbeing, perceived stress, happiness, depression and anxiety symptoms, self-compassion, and sense of meaning in life, together with their use of adaptive coping strategies can be useful for preventive purposes.
