Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.

Abstract:Purpose To investigate differences in the objective and subjective visual quality of patients with high myopia following implantable collamer lens V4c (ICL V4c) versus small incision lenticule extraction (SMILE) early postoperatively.Methods This prospective analysis included 94 eyes of 57 patients (8 males) in the ICL V4c group (mean spherical equivalent (SE), -8.07 +/- 1.03 dioptres (D)) and 103 eyes of 57 patients (13 males) in the SMILE group (mean SE, -7.85 +/- 0.78 D). Refractive parameters and wavefront aberrations were recorded preoperatively and 6 months postoperatively. Subjective visual quality was scored with the Quality of Vision questionnaire, and operation satisfaction was evaluated 6 months postoperatively.Results At 6 months postoperatively, the efficacy index was 1.17 +/- 0.16 in the ICL and 1.09 +/- 0.20 in the SMILE group; the safety index was 1.21 +/- 0.18 in the ICL and 1.12 +/- 0.16 in the SMILE group. A significant increase in total higher-order aberrations (HOAs) in both the ICL and SMILE groups was observed, but to a lesser degree in ICL (p < 0.05). In the ICL group, trefoil significantly increased (p < 0.05), whereas in the SMILE group, coma, trefoil and spherical aberration significantly increased (p < 0.05). Glare, haloes and blurred vision had the highest incidences after both types of surgery, and postoperative haloes were significantly more common following ICL; 96.1% of ICL-treated and 94.9% of SMILE-treated patients were satisfied with their visual outcomes.Conclusion ICL V4c caused lower HOAs induction than SMILE. Despite the postoperative visual disturbances, both ICL and SMILE provided good efficacy, safety, predictability and high satisfaction in correcting high myopia.

Abstract:To compare the 4-year visual outcomes of implantable collamer lens V4c (EVO-ICL) implantation and small incision lenticule extraction (SMILE) for high myopia and astigmatism. This retrospective case study included 64 eyes of 40 patients. These patients with preoperative manifest refraction spherical equivalent (SE) between − 6.00 and − 10.00 diopters (D) were screened from the database of SMILE and EVO-ICL implantation procedures in 2015. The ICL group [32 eyes of 19 patients (mean age, 29.6 ± 6.3 years); mean SE, -8.71 ± 1.06 D] and SMILE group [32 eyes of 21 patients (mean age, 27.7 ± 5.6 years); mean SE, -8.35 ± 0.65D] were compared. All patients were then prospectively examined at a four-year follow-up for routine postoperative examinations, higher-order ocular aberrations, retinal image quality and a questionnaire. The safety indexes were 1.15 ± 0.14 and 1.22 ± 0.21 (P = 0.36) for the SMILE and ICL groups, respectively. No eyes lost two or more lines of CDVA in either group. Abstract:Purpose To compare one-year visual outcomes and optical quality after femtosecond laser small incision lenticule extraction (SMILE) and Visian Implantable Collamer Lens with a central hole (ICL V4c) implantation for high myopia. Methods This prospective non-randomized study included 37 eyes of 20 patients who underwent SMILE (G-S) and 39 eyes of 20 patients who underwent ICL V4c implantation (G-V). Spherical equivalent (SE), uncorrected distance visual acuity, corrected distance visual acuity, retinal image quality and intraocular scattering were evaluated at one year after surgery. Each model was adjusted for age, gender, eye and preoperative SE during treatment. Results At the one-year follow-up, G-S and G-V showed similar safety index (1.13 +/- 0.13 and 1.11 +/- 0.15, respectively) and efficacy index (1.05 +/- 0.14 and 1.06 +/- 0.15, respectively). All 21 eyes with Toric ICL (TICL) had a postoperative astigmatism of <= 0.5 dioptres (D), while the astigmatism was <= 0.5 D in all G-S eyes. Thirty-six G-S eyes (97%) and 35 G-V eyes (90%) were within +/- 0.5 D of the target SE. The changes in the modulation transfer function cut-off frequency (MTFcut-off) and the objective scatter index from before operation to one year following the operation were not significantly different between the groups (p = 0.523 and 0.826, respectively). Conclusion Both SMILE and ICL V4c implantation provide good safety, efficacy, predictability, and stability in correcting high myopia. Optical quality including intraocular scattering was great and relatively stable in both groups during the one-year observation period.

Abstract:Purpose:To investigate the 2-year visual quality of Evolution Implantable Collamer Lens (EVO-ICL) and small incision lenticule extraction (SMILE) for the correction of low myopia. Methods:In this prospective study, we included 25 eyes of 25 patients (7 men) who underwent EVO-ICL and 36 eyes of 36 patients (16 men) who underwent SMILE between January 2018 and December 2018. Subjective and objective visual outcomes were compared between ICL and SMILE. All patients were followed for 2 years. Results:At the postoperative 2-year visit, the percentage of patients with uncorrected distance visual acuity (UDVA) greater than or equal to preoperative corrected distance visual acuity (CDVA) was comparable in the ICL group (80%, 20/25) and SMILE group (88.89%, 32/36). Spherical equivalent (SE) was within ± 0.50 D in 96% (24/25) of the ICL group and 94.44% (34/36) of the SMILE group. No eyes lost more than 2 lines of CDVA. Postoperative high-order aberrations (HOAs) were significantly increased in the ICL group (p < 0.01) and in the SMILE group (p < 0.01). The most common visual complaint was halo after ICL and starburst after SMILE. There was no correlation between HOAs and visual complaints (p > 0.05). Conclusion:Evolution Implantable Collamer Lens provides comparable safety, efficacy, long-term visual stability, and high patient satisfaction when compared to SMILE in correcting low myopia. EVO-ICL could be a favorable alternative for low myopia. Abstract:Purpose To investigate the long-term visual quality and stability of implantable collamer lens (ICL) and laser refractive surgery (LRS) for myopia. Methods This study comprised 52 eyes of 26 high-myopia anisometropia patients who were suitable for surgical treatment. In each patient, the higher-myopia eye was implanted with ICL and the lower-myopia eye was treated with LRS. The patients were followed for 3 years. During that time period, uncorrected (UDVA) and corrected distance visual acuity (CDVA), refraction, wavefront aberration and visual quality were evaluated. Results The spherical equivalent refractive error changed from -14.11 +/- 3.39 D preoperatively to -1.27 +/- 1.05 D 3 years after ICL implantation and from -8.75 +/- 2.76 D to -1.12 +/- 1.30 D after LRS. The changes in refractive error from 1 month to 3 years were -0.52 and -0.77 D for the ICL and LRS groups, respectively. The safety indices (postoperative CDVA/preoperative CDVA) were 1.84 +/- 1.00 and 1.32 +/- 0.40, and the efficacy indices (postoperative UDVA/preoperative CDVA) were 1.40 +/- 1.10 and 1.11 +/- 0.44, respectively. The postoperative coma, spherical and total higher-order aberrations in the ICL group were lower than those in the LRS group. Conclusion Both ICL implantation and LRS are safe and effective procedures for myopia with suitable indications, but ICL implantation is more stable. Fewer induced aberrations are gained after ICL implantation.

Abstract:PurposeTo investigate visual outcomes early after implantable collamer lens (ICL) V4c implantation between patients with fully corrected and under-corrected spectacles preoperatively.MethodsPatients who implanted ICL V4c were divided into the full correction (46 eyes/23 patients) and under-correction groups (48 eyes/24 patients) based on preoperative differences between the spherical diopter of the spectacles and the actual spherical diopter. Refractive outcomes, scotopic pupil size, higher-order aberrations, and subjective visual outcomes as assessed using a validated questionnaire were compared between the two groups 3 months postoperatively. Moreover, the relationships between the severity of haloes and postoperative ocular or ICL parameters were analyzed.ResultsAt the 3-month follow-up, the efficacy indices in the full correction and under-correction groups were 0.99 +/- 0.12 and 1.00 +/- 0.10, respectively; the safety indices were 1.15 +/- 0.16 and 1.15 +/- 0.15, respectively. Total-eye spherical aberration (p < 0.0001) and internal spherical aberration (p = 0.0005) were significantly different pre- and post-operatively in the under-correction group, while no differences were found in the full correction group. Total-eye spherical aberration (p = 0.002) and the severity of haloes (p = 0.03) were postoperatively different between the two groups. The severity of haloes was associated with postoperative spherical aberration (total-eye spherical aberration: r = -0.32, p = 0.002; internal spherical aberration: r = -0.24, p = 0.02).ConclusionGood efficacy, safety, predictability, and stability were obtained early after surgery regardless of preoperative spectacle correction. Patients in the under-correction group possessed a shift to negative spherical aberration and reported greater severity of haloes at the 3-month follow-up. Haloes were the most common visual symptoms after ICL V4c implantation and the severity of them was correlated with postoperative spherical aberration.

Abstract:Behavioral and psychiatric symptoms frequently accompany the cognitive deterioration of dementia occurring in up to 80% of both community-based and nursing home patients. In Alzheimer's disease (AD), behavioral complications may develop at any stage of the disease process and tend to follow a more unpredictable course than the core cognitive symptoms. Over the past 15 years, our knowledge of the biologic and neurochemical basis of dementia, and of AD in particular, has increased significantly. Great emphasis has been placed on the cholinergic system abnormalities in the context of the cognitive impairment in AD and on the psychopharmalogic enhancement of cholinergic transmission in AD. However deficits in other neurotransmitter systems, such as the noradrenergic, serotonergic, and dopaminergic systems, also occur and may contribute to the core cognitive symptoms of AD. More recently, there has been increased interest in the biologic and neurochemical basis of behavioral and psychiatric disturbances in dementia. This article reviews the evidence for biologic and neurochemical correlates of psychosis and agitation in dementia, and discusses the treatment implications for these findings. Abstract:This study sought to compare the visual quality between intraocular collamer lens (EVO-ICL) implantation and small-incision lenticule extraction (SMILE) and determine the appropriate surgical method to treat patients with high myopia (− 6.25 to − 10 D). A total of 48 eyes underwent EVO-ICL implantation and another 48 eyes underwent SMILE. The uncorrected distance visual acuity (UDVA), best-corrected distance visual acuity (BCVA) and equivalent spherical degree were compared across the SMILE (− 6.25 to − 10 D) and EVO-ICL (− 6.25 to − 10 D) implantation groups. Preoperative and postoperative visual quality parameters were compared between and within groups. The OQAS II values (OV 100%) one week and one month after surgery and the modulation transfer function (MTF), OV 20% and OV 9% values one week after surgery in the SMILE group were lower than the respective preoperative values. The objective scatter index (OSI) value increased one week as well as one month after surgery compared with the preoperative values. The MTF cut-off value of the SMILE group was lower than that of the EVO-ICL implantation group three months after surgery. For patients with high myopia, the postoperative visual quality of EVO- ICL implantation was slightly better than that of SMILE.

Abstract:Purpose: To investigate the clinical outcomes in small incision lenticule extraction (SMILE) and EVO implantable Collamer lens (ICL)-treated high myopia. Methods: Thirty-three SMILE-treated and 32 EVO ICL-treated patients were included and followed up for 6 months. Subjective refraction, contrast sensitivity, and disk halo size were measured preoperatively and postoperatively. Patient-reported outcomes (PROs) were obtained at the final visit. Results: Significant differences in efficacy and safety indices were observed between the SMILE and EVOICL groups at 6months postoperatively (P< 0.05). In the SMILE group, the mesopic contrast sensitivity at 2.2 cycles per degree (cpd) and photopic contrast sensitivity at 0.5, 3.4, and 7.1 cpd were significantly improved. In the EVO ICL group, the mesopic contrast sensitivity at 7.1 cpd and photopic contrast sensitivity at 0.5, 7.1, and 14.6 cpd were significantly improved. The halo radii after SMILE were significantly increased at 1 week, showed a decreasing trend at 1 month, returned to baseline at 3 months, and progressed stably at 6 months. However, it was unchanged in the EVO ICL group. Regarding subjective experience, haloeswere the most commondisturbance with mild and little bothersomeness after EVO ICL in contrast to starbursts after SMILE. Conclusions: EVO ICL implantation yielded better visual outcomes, improved contrast sensitivity particularly at high spatial frequencies, had a stabler disk halo size, and increased incidence of haloes, with less influence than that of SMILE.
Implantation of posterior chamber phakic intraocular lens for myopia and hyperopia - long-term clinical outcomes

Abstract:Purpose: To evaluate long-term refractive outcomes of implantable collamer lens (ICL) implantation and late postoperative complications. Methods: We assessed outcomes of patients who underwent ICL implantation (type ICM V4 for myopia, ICH V3 for hyperopia, TICM V4 for astigmatism) in our department between 1998 and 2013. It comprised 62 eyes (40 myopic and 22 hyperopic). The average follow-up period was 10.5 years. We evaluated: uncorrected and best-corrected visual acuity (UCVA and BCVA), spherical equivalent (SE), ICL vault, endothelial cell density and late postoperative complications. Results: In myopes, the average UCVA was 1.0±0.37 and BCVA 1.18±0.38, in hyperopes 0.78±0.19 and 1.14±0.18, respectively. The average SE in myopes, whose target refraction was emmetropia, was -0.6±0.83 Dsf, in hyperopes +0.73±0.93. Central ICL vault was 206.16μm±105.94, (range 10-427) in myopes, 195.5μm±109.09, (range 20-404) in hyperopes. The most common late postoperative complication was cataract formation. Three myopic eyes (7.5%) developed symptomatic anterior subcapsular opacities with loss of at least two lines of BCVA. Cataract significantly affecting visual acuity occurred in 5 myopic eyes (12.5%) and 2 hyperopic eyes (9.09%). In these eyes, ICL removal and cataract surgery was performed. Conclusions: In our experience, ICL implantation in moderate and high ametropia was effective and relatively safe. The most common late complication was cataract formation. This complication can be managed effectively surgically with good refractive outcomes without loss of BCVA.
Spherical implantable collamer lenses for myopia and hyperopia: 126 eyes with 1-year follow up

Abstract:Background: To report on 1-year follow up of a series of 126 eyes implanted with a spherical implantable collamer lens (ICL). Methods: Retrospective case note review of 126 eyes (68 patients) undergoing ICL placement by a single surgeon (TG) to correct preoperative mean spherical equivalents between +8.88 and -20.50 D. Main outcome measures included uncorrected visual acuity (UCVA), refraction, best spectacle-corrected VA, unaided binocular vision, adverse events, operative/postoperative complications and patient symptoms. Results: Complete data were available at 1 year on 121 of the 126 eyes (65 out of 68 patients). At 12 months 86% (104/121) had postoperative UCVA better than or equal to preoperative best spectacle-corrected VA, and 98% (118/121) had UCVA within one line of preoperative best spectacle-corrected VA. Ninety-six per cent (116/121) had postoperative UCVA of 6/9 or better, and 100% had postoperative best-corrected visual acuity of 6/12 or better. Ninety-eight per cent (64/65) patients had unaided binocular vision at 1 year of 6/7.5 or better. Average myopia treated was -8.83 D (116 patients), and average hyperopia +7.14 D (10 patients). Two ICLs were replaced because of high vaulting; one eye had increased intraocular pressure that resolved with smaller ICL placement. Four patients had visually insignificant pigment deposits on the ICL at 3/12, but no pigmentary glaucoma. Five patients noticed haloes around lights at night, none preventing driving. Conclusions: One-year results from this study, the largest reported clinical investigation of the use of ICLs in New Zealand, support the safety, efficacy and predictability of ICL to treat both hyperopic and myopic spherical refractive errors.
Visual Outcomes of Visian ICL Implantation for High Myopia in Patients with Shallow Anterior Chamber Depth

Abstract:Background: High myopia with shallow anterior chamber depth (ACD less than 2.8 mm) is not rare. This observational study aims to evaluate visual outcomes after implantation of the Visian Implantable Collamer Lens with a central hole (ICL V4c) in these patients.Methods: A prospective cohort of consecutive 51 eyes of 31 patients (20 to 42 years old) was followed for at least 12 months (average 15.35 ± 4.90 months, rangers from 12 to 25 months). The preoperative ACD was 2.74 ± 0.04 mm (2.65 to 2.79 mm). Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), intraocular pressure (IOP), manifest refraction, vault, and endothelial cell density (ECD) were measured during the follow-ups after surgery.Results: All surgeries were performed safely and no complication was observed during the follow-up. At the last follow-up, the safety index (postoperative CDVA / preoperative CDVA) was 1.33 ± 0.60 and the efficacy index (postoperative UDVA / preoperative CDVA) was 1.14 ± 0.54. After the surgery, no eye had decreased CDVA and 59% (30 eyes) of the eyes gained at least one line. Forty-seven eyes (92%) were within ± 1.0 D and 35 eyes (69%) were within ± 0.5 D of the attempted refraction. The mean postoperative vault was 380.00 ± 152.84 µm (90 to 700µm). The ECD was reduced by 8.38 ± 0.06% as compared to the preoperative value (p < 0.001). No significant change was observed in IOP (p = 0.061) at the last follow-up. Ultrasound Biomicroscopy (UBM) showed none of the eyes had trabecular-iris angle closed.Conclusions: In this prospective observational study, ICL V4c implantation in patients with high myopia and shallow ACD achieved satisfying and stable visual outcomes. Its long-term safety and stability require further investigation.Trial Registration: This trial was retrospectively registered on 05/08/2018 under the number (ChiCTR1800017594)
Small Incision Lenticule Extraction (SMILE) for Moderate and High Myopia: Seven-Year Outcomes of Refraction, Corneal Tomography, and Wavefront Aberrations

Abstract:Purpose . To investigate the long-term outcomes of refraction, corneal tomography, and wavefront aberrations after small incision lenticule extraction (SMILE) for moderate and high myopia. Methods . Prospective, nonconsecutive case series. A total of 26 patients (26 eyes) who underwent SMILE from May 2010 to March 2013 at the Fudan University Eye and ENT Hospital (Shanghai, China) were enrolled. The periods of follow-up were 1 month, 1 year, 5 years, and 7 years after surgery. The routine eye examinations included uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), manifest refraction, and corneal tomography. Results . All surgeries were executed without any complications. At the final visit, an UDVA of 20/20 or better was achieved in 26 eyes (100%) and 11 eyes (42%) exhibited no change in CDVA. 9 eyes (35%) gained one line, 6 eyes (23%) gained two lines, and no eyes lost CDVA. 24 eyes (92%) and 26 eyes (100%) were within ±0.5 D and ±1.00 D of the target refraction, respectively. A mean refractive regression of −0.17 D was observed between 1 month and 7 years postoperatively. Mean corneal front curvature (MCFC) was significantly decreased between pre- and post-SMILE surgery ( P < 0.0001 ). Higher-order aberrations (HOAs) and vertical coma were significantly increased after SMILE compared to those measured before surgery (all P < 0.001 ). There were no significant differences in trefoil and spherical aberration between pre- and post-SMILE surgery (all P > 0.05 ). Conclusion . SMILE is an effective, safe, and stable procedure for moderate and high myopia, with relatively constant corneal stability and wavefront aberrations. Comparative Study of Implantable Collamer Lens Implantation in Treating Four Degrees of Myopia: Six-Month Observation of Visual Results, Higher-Order Aberrations, and Amplitude of Accommodation

Abstract:Purpose: To compare the visual and refractive outcomes, higher-order aberrations (HOAs), and amplitude of accommodation (AA) after implantable collamer lens (ICL) model V4c implantation in four degrees of myopia. Methods: One hundred and thirty-seven myopic eyes (137 patients) undergoing ICL implantation were included and divided into four groups: Group 1 with spherical equivalent (SE) <=-6.0D, Group 2 with SE from -6.13D to -9.0D, Group 3 with SE from -9.13D to -12.0D, and Group 4 with SE from -12.13D to -18.0D. The postoperative visits were scheduled at 1 day, 1 week, 1 month, 3 months, and 6 months. Visual and refractive outcomes, HOAs and AA were observed and compared. Results: At 6 months postoperatively, the uncorrected distance visual acuity (UDVA) in Group 4 was worse than the values in the other groups (all p < .05). Meanwhile, Group 4 had more eyes with corrected distance visual acuity (CDVA) improvement than the other groups. Furthermore, 96%, 100%, 100%, and 81% of eyes had an SE within +/- 0.5D in Group 1, Group 2, Group 3, and Group 4 (p < .05 between Group 4 and the other groups), respectively. The postoperative UDVA and SE remained stable in all groups. No significant change in total HOAs was found between preoperative and postoperative values for each group. However, quatrefoil increased in each group, whereas trefoil was induced in all groups except for Group 1. Negative spherical aberration was induced in Group 3 and Group 4. AA significantly decreased 1 week postoperatively and gradually improved in each group. Although AA improved to the preoperative level in Group 1 at 3 months postoperatively, it was still lower than the preoperative level in the other groups. Conclusions: In treating different degrees of myopia with ICL implantation, differences were observed in terms of CDVA improvement, SE predictability, HOAs induction, and accommodation recovery.
Implantable Collamer Lens® for Management of Pseudophakic Ametropia in Eyes With a Spectrum of Previous Corneal Surgery

Abstract:Purpose: To assess the visual outcomes, predictability, and safety of the Implantable Collamer Lens (ICL) (STAAR Surgical, Inc., Monrovia, CA) for the management of residual pseudophakic refractive error. Methods: Forty-nine pseudophakic eyes of 40 patients in which myopic, hyperopic, or toric ICLs were implanted were retrospectively analyzed. Twenty-nine eyes had been implanted with a monofocal intraocular lens (IOL) and 20 eyes had a multifocal IOL. All IOLs were implanted in the capsular bag. Of the 49 eyes included, 6 had residual refractive error after phacoemulsification without corneal pathology or surgical alteration (virgin cornea group), 12 had LASIK or photorefractive keratectomy (PRK) (excimer laser group), 8 had radial keratotomy, 5 had intrastromal corneal ring segments (ICRS) implantation, 11 had penetrating keratoplasty, and 7 had deep anterior lamellar keratoplasty (DALK). Uncorrected and corrected (CDVA) distance visual acuity and manifest refraction were evaluated. Results: The efficacy/safety indices were 0.92/1.10, 0.98/1.13, 1.04/1.11, 0.90/1.13, 0.79/1.17, and 0.71/1.23 for the virgin cornea, excimer laser, radial keratotomy, ICRS, penetrating keratoplasty, and DALK groups, respectively. No eye lost one or more lines of CDVA. The virgin cornea, excimer laser, and radial keratotomy groups showed better predictability and accuracy, with 96.2% spherical equivalent within ±1.00 diopters (D). The ICRS, penetrating keratoplasty, and DALK groups demonstrated approximately 50% spherical equivalent within ±1.00 D. There were no intraoperative or postoperative complications. Conclusions: Good refractive outcomes and a strong safety record support the use of the ICL for the correction of residual refractive error in pseudophakic eyes with previous corneal surgery. Abstract:Purpose: To assess Visian Implantable Collamer Lens (ICL) (STAAR Surgical, Monrovia, CA) implantation in the ciliary sulcus to correct pseudophakic ametropia in patients who are not candidates for a keratorefractive procedure. Methods: The authors performed a prospective non-comparative case series study of 18 patients (age: 48 to 61 years) with refractive surprise after phacoemulsification. Patients underwent implantation of a piggyback collagen copolymer lens: V4C design in 16 myopic eyes and V4B design in 2 hyperopic eyes. The position and vault of the ICLs were documented at all control visits clinically and with Pentacam (Oculus Optikgeräte, Wetzlar, Germany). Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction spherical equivalent (MRSE), intraocular pressure (IOP), and endothelial cell count were recorded at baseline and 1 week and 1, 6, 12, and 18 months postoperatively. Results: The MRSE improved from -3.08 ± 2.37 diopters (D) preoperatively to -0.44 ± -0.23 D postoperatively, corrected with a mean ICL power of -3.20 ± 2.90 D. The mean UDVA improved from 1.03 ± 0.12 logMAR preoperatively to 0.05 ± 0.06 logMAR postoperatively (P = .00), whereas CDVA improved from 0.47 ± 0.03 logMAR preoperatively to -0.006 ± 0.02 logMAR (P = .001) postoperatively. None of the cases developed interlenticular opacification throughout the 18-month follow-up. The mean ICL vault measured by Scheimpflug tomography was 451.27 ± 178.5 µm. Acute IOP elevation with anterior uveitis developed in 2 eyes and was controlled by topical steroids and a beta-blocker. Conclusions: Sulcus implantation of the secondary ICL to correct pseudophakic refractive error was safe, predictable, and well tolerated in all studied eyes. [J Refract Surg. 2017;33(8):532-537.].
Implantable Collamer Lens Versus Iris-Fixed Phakic Intraocular Lens Implantation To Correct Myopia: A Meta-Analysis

Abstract:This study is a meta-analysis comparing the efficacy, predictability, and safety of correcting myopia via implantation of two types of phakic intraocular lens (PIOLs): the implantable collamer lens (ICL) and iris-fixed PIOL. The Cochrane library, Pubmed, and EMBASE were searched. Study selection, data exclusion, and quality assessment were performed by two independent observers. The pooled relative risk (RR), pooled standardized mean difference (SMD), and their 95% confidence intervals (CIs) were used to compare lenses. Seven studies, involving 511 eyes, were included. The pooled SMD in postoperative uncorrected distance visual acuity (UDVA) comparing ICLs to iris-fixed PIOLs was -0.22 (95% CI, -0.58 to 0.13; P = .22). The pooled RR values of UDVA of 20/20 or better and of 20/40 or better comparing ICLs to iris-fixed PIOLs were 1.15 (95% CI, 0.89 to 1.47; P = .29) and 1.01 (95% CI, 0.95 to 1.08; P = .75), respectively. The pooled RR of loss of best spectacle-corrected visual acuity (BSCVA) and gain in BSCVA comparing ICLs to iris-fixed PIOLs were 1.20 (95% CI, 0.24 to 6.00; P = .82) and 1.14 (95% CI, 0.89 to 1.48; P = .31), respectively. The pooled RR comparing ICLs to iris-fixed PIOLs was 0.78 (95% CI, 0.29 to 2.12; P = .63) for all reported complications and 2.80 (95% CI, 1.04 to 7.52; P = .04) for severe complications. The pooled RR of achieving a result within +/- 0.5 D (diopter) of the intended target comparing ICLs to iris-fixed PIOLs was 1.35 (95% CI, 1.04 to 1.77; P = .03). Overall, there is no significant difference in efficacy between the two types of PIOLs or in safety, except that the ICL is associated with a greater incidence of severe complications, especially anterior subcapsular cataract, primarily in the Version 2 and Version 3 groups. However, ICL has better predictability.
