Abstract:Abstract OBJECTIVES To investigate the post-operative outcomes of lung resection in patients with SARS-CoV-2 infection and determine the optimal timing of surgery. METHODS This retrospective, single-center cohort study included patients who underwent lung resection between June 2021 and June 2022. Patients were divided into the COVID-19 and non-COVID-19 groups based on their pre-operative SARS-CoV-2 infection history, and post-operative outcomes were compared. Logistic regression analysis was conducted to identify the risk factors of complications after lung resection surgery. RESULTS In total, 1,194 patients were enrolled, of whom, 79 had a history of SARS-CoV-2 infection. In the COVID-19 group, 66 patients (90.4%) have received at least 1 vaccination dose. The average interval between infection and surgery was 67 days, with no significant impact on post-operative outcomes. Regarding post-operative outcomes, there were no significant differences in major complication rate (6.3% vs 5.4%, P = 0.613), respiratory complication rate (19.0% vs 12.2%, P = 0.079), or length of stays (4.9 ± 3.4 vs 5.0 ± 5.6, P = 0.992) between the two groups. Multivariate logistic regression analysis revealed that age, male sex, poor pulmonary function test, open surgery, and extensive lung resection were risk factors for postoperative complications, while preoperative COVID-19 infection status was not a statistically significant risk factor. CONCLUSIONS In the post-vaccination era, lung resection surgery can be safely performed shortly after SARS-CoV-2 infection, even within 4 weeks of infection.

Abstract:Objective: This study aimed to investigate the clinical manifestations and prognosis of lung transplant (LTx) recipients infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) during the coronavirus disease (COVID-19) pandemic. Methods: The research participants were LTx recipients who underwent surgery and were regularly followed up at our center. From 1 December 2022 to 28 February 2023, during the COVID-19 pandemic in China, research participants were interviewed either online or in person. SARS-CoV-2 nucleic acid or self-tested antigens were detected according to accessibility. Diagnosis and treatment were performed according to the Diagnosis and Treatment Plan for COVID-19 (10th edition) issued by the National Health Commission of the People's Republic of China. Hospitalized patients underwent chest imaging examinations, routine blood tests, biomarkers for infection and inflammation, and biochemical tests, all of which were taken and recorded. Data were analyzed to describe the features of COVID-19 in LTx recipients. Results: In total, 52 patients were enrolled in this study, comprising 48 men and 4 women, with a mean age of 51.71 ± 11.67 years. By 1 December 2022, the mean survival period was 33.87 ± 25.97 months, of which 84.61% of the patients (44/52) had a survival period longer than 12 months. The SARS-CoV-2 infection rate in these LTx recipients was 82.69% (43/52), with 3.85% (2/52) of the infected recipients being asymptomatic, 50.00% (26/52) of the infected recipients experiencing mild COVID-19, 11.54% (6/52) having moderate COVID-19, and 17.31% (9/52) having severe or critical COVID-19. The mortality rate among severe and critical patients was 66.67% (6/9). Conclusion: LTx recipients in this cohort exhibited a notable susceptibility to SARS-CoV-2, with 82.69% of individuals diagnosed with COVID-19. Moreover, the mortality rate among critically ill patients was high.

Abstract:Aim:To analyze the possible risk factors of delayed virus clearance in hospitalized patients with coronavirus disease 2019 (COVID-19).Methods:Retrospective analysis of patients with COVID-19 admitted to the isolation wards from our hospital from 19th Jan 2020 to 18th March 2020. We were collected patient's data including demographic, epidemiologic, and clinical information, as well as laboratory and radiologic findings. The possible confounding risk factors for prolonged viral RNA shedding of COVID-19 during hospitalization were explored by univariate analysis and any variables with a p value less than 0.05 after univariate analysis were included in a subsequent multivariate logistic regression model analysis.Results:The 104 patients included 30 mild patients and 74 severe or critically ill patients. The median duration of viral RNA positivity in sputum was 11 days, and the longest duration of viral RNA positivity was 49 days after admission. Multivariate analysis shown that the used with darunavir/cobicistat treatment (odds ratio [OR]: 4.25, 95% confidence interval [CI]: 1.25-14.42, p = 0.020), duration of fever (OR: 1.15, 95% CI: 1.03-1.30, p = 0.015) and time to radiological improvement (OR: 1.14, 95% CI: 1.01-1.30, p = 0.033) were associated with delayed clearance of SARS-CoV-2 in sputum from COVID-19 patients. Then adjusted in the multivariate binary logistic regression analysis model in severe COVID-19 and found that critical COVID-19 patients (OR: 13.25, 95% CI: 1.45-12.07, p = 0.022), lower virus cycle threshold (CT) values of RT-PCR (OR: 0.96, 95% CI: 0.93-0.99, p = 0.004) and used with darunavir/cobicistat treatment (OR: 8.44, 95% CI: 2.21-32.28, p = 0.022) were associated with delayed clearance of SARS-CoV-2 in sputum from COVID-19 patients.Conclude:Clearance of viral RNA in sputum was delayed in severe COVID-19 patients, especially with lower virus CT value. And antivirals with darunavir/cobicistat has little advantage in eliminating SARS-CoV-2.

Abstract:Undetected respiratory infections may adversely affect the intrapulmonary resistance after Stage 2 or Stage 3 Fontan palliation. A few studies describe a higher risk for viral pneumonia during respiratory virus season, but none of them have focused on the effect of symptomatic viral pneumonia on in-hospital clinical course after bidirectional Glenn shunt. We analysed 77 patients who underwent bidirectional Glenn shunt surgery. Six patients were detected with pneumonia and proof of viral ribonucleic acid in tracheal mucus in the very early postoperative time. We compared them retrospectively to the remaining 71 patients regarding preoperative inflammatory signs, mortality, paediatric ICU length of stay, and ventilation time. The infection rate was not seasonal dependent. Ventilation time was significantly elongated in the pneumonia group (558 h ± 634 vs. 8.7 h ± 1.9; p < 0.0001) and so was the paediatric ICU length of stay (29 days ± 26 vs. 3 days±1; p = 0.007). Significantly more patients in the pneumonia group required extracorporeal cardiac life support postoperatively. The mortality was significantly increased in patients with pneumonia. Even subclinical viral pneumonia may cause ventilation-to-perfusion mismatch by raising intrapulmonary resistance. Recorded parameters of postoperative paediatric ICU therapy showed a significant impact of a viral pneumonia on patients after bidirectional Glenn shunt. The respiratory syncytial virus vaccination does not protect these patients from infection with other respiratory viruses. The focus should be put on preoperative diagnosis of pulmonary infections in the vulnerable group of patients with univentricular hearts.

Effects of pre-operative isolation on postoperative pulmonary complications after elective surgery: an international prospective cohort study

Abstract:We aimed to determine the impact of pre-operative isolation on postoperative pulmonary complications after elective surgery during the global SARS-CoV-2 pandemic. We performed an international prospective cohort study including patients undergoing elective surgery in October 2020. Isolation was defined as the period before surgery during which patients did not leave their house or receive visitors from outside their household. The primary outcome was postoperative pulmonary complications, adjusted in multivariable models for measured confounders. Pre-defined sub-group analyses were performed for the primary outcome. A total of 96,454 patients from 114 countries were included and overall, 26,948 (27.9%) patients isolated before surgery. Postoperative pulmonary complications were recorded in 1947 (2.0%) patients of which 227 (11.7%) were associated with SARS-CoV-2 infection. Patients who isolated pre-operatively were older, had more respiratory comorbidities and were more commonly from areas of high SARS-CoV-2 incidence and high-income countries. Although the overall rates of postoperative pulmonary complications were similar in those that isolated and those that did not (2.1% vs 2.0%, respectively), isolation was associated with higher rates of postoperative pulmonary complications after adjustment (adjusted OR 1.20, 95%CI 1.05-1.36, p = 0.005). Sensitivity analyses revealed no further differences when patients were categorised by: pre-operative testing; use of COVID-19-free pathways; or community SARS-CoV-2 prevalence. The rate of postoperative pulmonary complications increased with periods of isolation longer than 3 days, with an OR (95%CI) at 4-7 days or ≥ 8 days of 1.25 (1.04-1.48), p = 0.015 and 1.31 (1.11-1.55), p = 0.001, respectively. Isolation before elective surgery might be associated with a small but clinically important increased risk of postoperative pulmonary complications. Longer periods of isolation showed no reduction in the risk of postoperative pulmonary complications. These findings have significant implications for global provision of elective surgical care.

Abstract:Purpose: Although patients with SARS-CoV-2 infection have several risk factors for ventilator-associated lower respiratory tract infections (VA-LRTI), the reported incidence of hospital-acquired infections is low. We aimed to determine the relationship between SARS-CoV-2 pneumonia, as compared to influenza pneumonia or no viral infection, and the incidence of VA-LRTI. Methods: Multicenter retrospective European cohort performed in 36 ICUs. All adult patients receiving invasive mechanical ventilation > 48 h were eligible if they had: SARS-CoV-2 pneumonia, influenza pneumonia, or no viral infection at ICU admission. VA-LRTI, including ventilator-associated tracheobronchitis (VAT) and ventilator-associated pneumonia (VAP), were diagnosed using clinical, radiological and quantitative microbiological criteria. All VA-LRTI were prospectively identified, and chest-X rays were analyzed by at least two physicians. Cumulative incidence of first episodes of VA-LRTI was estimated using the Kalbfleisch and Prentice method, and compared using Fine-and Gray models. Results: 1576 patients were included (568 in SARS-CoV-2, 482 in influenza, and 526 in no viral infection groups). VA-LRTI incidence was significantly higher in SARS-CoV-2 patients (287, 50.5%), as compared to influenza patients (146, 30.3%, adjusted sub hazard ratio (sHR) 1.60 (95% confidence interval (CI) 1.26 to 2.04)) or patients with no viral infection (133, 25.3%, adjusted sHR 1.7 (95% CI 1.2 to 2.39)). Gram-negative bacilli were responsible for a large proportion (82% to 89.7%) of VA-LRTI, mainly Pseudomonas aeruginosa, Enterobacter spp., and Klebsiella spp. Conclusions: The incidence of VA-LRTI is significantly higher in patients with SARS-CoV-2 infection, as compared to patients with influenza pneumonia, or no viral infection after statistical adjustment, but residual confounding may still play a role in the effect estimates.

Abstract:Objectives: Pulmonary complication remains common morbidity after thoracoscopic minimally invasive oesophagectomy (MIE), partially due to lung injury caused by one-lung ventilation (OLV). We attempted two-lung ventilation (TLV) with low tidal volume in semi-prone position assisted by artificial CO2 pneumothorax. This study aimed to compare the impact of two ventilation strategies on postoperative pulmonary complications. Methods: From March 2011 to August 2013, 200 patients underwent thoracoscopic MIE. The initial 100 patients performed with OLV (succeeded in 99, with 1 case conversion), and the later half performed with TLV (succeeded in 99, with 1 case conversion). During thoracic stage, patients in the OLV group were intubated with a double-lumen endotracheal tube and given 8 ml/kg tidal volume; while in the TLV group, patients were intubated with a single-lumen endotracheal tube and also given 8 ml/kg tidal volume. An artificial CO2 pneumothorax was achieved at pressure of 8 mmHg. Meanwhile, a pre-placed bronchial blocker was used in case of conversion to thoracotomy. During the abdominal and cervical stage, all patients performed two-lung ventilation. Patients' demographics and clinical outcomes were recorded and statistically compared. Results: No significant difference was observed between the two groups in demographics, blood loss, lymph nodes harvested, postoperative hospital stay, total complications or 30-day mortality. The thoracic operative time was longer in the TLV group than in OLV (85.4 ± 13.4 min vs 74.8 ± 11.2 min, P = 0.000). The occurrence of pulmonary complications in the TLV group was lower (9.1% vs 20.2%, P = 0.027). Reintubation for ARDS/ALI was given in 12 patients (3 in TLV and 9 in OLV). The oxygenation index was significantly higher in the TLV group than OLV group on POD 1 (319.2 ± 31.3 vs 298.2 ± 29.9, P = 0.000) and POD 2 (330.8 ± 33.3 vs 318.0 ± 32.4, P = 0.006). Conclusions: Two-lung ventilation with low tidal volume during MIE in semi-prone position appears to be feasible, safe and effective. It may attenuate postoperative pulmonary complications. Additional RCT studies are required to confirm this conclusion. Disclosure: No significant relationships.

Abstract:ObjectiveThe aim of this study was to determine whether a AOI strategy on non-ventilated lung could reduce the regional and systemic proinflammatory cytokine and oxidative stress response associated with esophagectomy, and to evaluate whether AOI can be used as a novel lung protective ventilation strategy. Its impact on oxygenation after OLV, surfactant protein A, B, C (SP-A, B, C), postoperative hospital stay and postoperative pulmonary complications (PPCs) was also evaluated.MethodsFifty-four adults (ASA II-III) undergoing esophagectomy with OLV were enrolled in the study. Patients were randomly assigned into 2 groups: control group (group C) and treated group (group T). Group C was treated with traditional OLV mode,while group T was given AOI of 5 L/min oxygen on the non-ventilated lung immediately at the beginning of OLV. Arterial blood gas was analyzed before and after OLV. A bronchoalveolar lavage(BAL) was performed after OLV on the non-ventilated lung. Proinflammatory cytokine, oxidative stress markers(TNF-α, NF-κB,sICAM-1,IL-6,IL-10,SOD,MDA) and SP-A, B, C were analyzed in serum and BALF as the primary endpoint.The clinical outcome determined by PPCs was assessed as the secondary endpoint.ResultsPatients with AOI had better oxygenation in the recovery period, oxygenation index(OI) (394[367–426] and 478[440–497]mmHg, respectively) of group T at T2 and T3 were significantly higher than those (332[206–434] and 437[331–512]mmHg, respectively) of group C. OLV resulted in an increase in the measured inflammatory markers in both groups, however, the increase of inflammatory markers upon OLV in the group C was significantly higher than those of group T. OLV resulted in an increase in the measured SP-A, B, C in serum of both groups. However, the levels of SP-A, B, C of group T were lower than those of group C in serum after OLV, and the results in BALF were the opposite. The BALF levels of SOD(23.88[14.70–33.93]U/ml) of group T were higher than those(15.99[10.33–24.16] U/ml) of group C, while the levels of MDA in both serum and BALF of group T(8.60[4.14–9.85] and 1.88[1.33–3.08]nmol/ml, respectively) were all lower than those of group C (11.10[6.57–13.75] and 1.280[1.01–1.83]nmol/ml) after OLV. There was no statistical difference between the two groups in terms of postoperative hospital stay and the incidence of PPCs.ConclusionAOI on non-ventilated lung during OLV can improve the oxygenation function after OLV, relieve the inflammatory and oxidative stress response in the systemic and non-ventilated lung after OLV associated with esophagectomy.Trial registrationChiCTR-IOR-17011037. Abstract:Background During one-lung ventilation (OLV), positive end-expiratory pressure (PEEP) can improve lung aeration, but might over-distend lung units and increase intrapulmonary shunt. We hypothesized that higher PEEP shifts pulmonary perfusion from the ventilated to the non-ventilated lung, resulting in a U-shaped relationship with intrapulmonary shunt during OLV. Methods In nine anesthetized female pigs, a thoracotomy was performed and intravenous lipopolysaccharide infused to mimic the inflammatory response of thoracic surgery. Animals underwent OLV in supine position with PEEP of 0 cmH2O, 5 cmH2O, titrated to best respiratory system compliance, and 15 cmH2O (PEEP0, PEEP5, PEEPtitr, and PEEP15, respectively, 45 min each, Latin square sequence). Respiratory, hemodynamic, and gas exchange variables were measured. The distributions of perfusion and ventilation were determined by i.v. fluorescent microspheres and computed tomography, respectively. Results Compared to two lung ventilation, the driving pressure increased with OLV, irrespective of the PEEP level. During OLV, cardiac output was lower at PEEP15 (5.5 ± 1.5 l/min) than PEEP0 (7.6 ± 3 l/min) and PEEP5 (7.4 ± 2.9 l/min; P=0.004), while the intrapulmonary shunt was highest at PEEP0 (PEEP0: 48.1 ± 14.4 %; PEEP5: 42.4 ± 14.8 %; PEEPtitr: 37.8 ± 11.0 %; PEEP15: 39.0 ± 10.7 %; P=0.027). The relative perfusion of the ventilated lung did not differ among PEEP levels (PEEP0: 65.0 ± 10.6 %; PEEP5:68.7 ± 8.7 %; PEEPtitr: 68.2 ± 10.5 %; PEEP15: 58.4 ± 12.8%; P=0.096), but the centers of relative perfusion and ventilation in the ventilated lung shifted from ventral to dorsal, and from cranial to caudal zones with increasing PEEP. Conclusion In this experimental model of thoracic surgery, higher PEEP during OLV did not shift the perfusion from the ventilated to the non-ventilated lung, thus not increasing intrapulmonary shunt. Abstract:Objective To investigate the influence of combined general-thoracic epidural anaesthesia on intrapulmonary shunt and blood oxygenation during one-lung ventilation.MethodsSixty patients with cancer of esophagus and ASA Ⅰ-Ⅱ grade were randomly divided into control group(n=30)and experimental group(n=30).The patients in experimental group received combined general-thoracic epidural anaesthesia and the patients in control group received general anaesthesia only.Arterial and venous blood gas analyses were recorded to calculate Qs/Qt and PaO2 when two lung ventilation after induction of anaesthesia(S1),30 min after one lung ventilation(S2)and 60 min after one lung ventilation(S3).Results PaO2 decreased significatly while Qs/Qt rised significatly at S2,S3 than S1 in all patients(all P0.05).There were no significant differences of Qs/Qt,PaO2 and PaCO2 between two groups(P0.05).Conclusion Thoracic epidural anaesthesia does not significantly influence intrapulmonary shunt and blood oxygenation during one lung ventilation.
Optimum timing of lung resection surgery following SARS‐CoV‐2 infection for non‐small cell lung cancer

Abstract:Background The impact of severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) infection on postoperative recovery of non‐small cell lung cancer (NSCLC) is need to be understood, thereby informing the optimal timing of surgical decision‐making during the COVID‐19 pandemic for NSCLC patients. This study reports the postoperative outcomes of surgical NSCLC patients with preoperative SARS‐CoV‐2 infection. Method This single‐center retrospective cohort study included 241 NSCLC patients who underwent lobectomy or sub‐lobectomy between December 1, 2022 and February 14, 2023. Surgical outcomes of patients with preoperative SARS‐CoV‐2 infection (stratified by the time from diagnosis of SARS‐CoV‐2 infection to surgery) were compared with those without preoperative SARS‐CoV‐2 infection. The primary outcomes were total postoperative complications and postoperative pulmonary complications (PPCs), the secondary outcomes included operation time, total postoperative drainage and time, length of hospital stay (LOS), 30‐day and 90‐day postoperative symptoms. Results This study included 153 (63.5%) patients with preoperative SARS‐CoV‐2 infection and 88 (36.5%) patients without previous SARS‐CoV‐2 infection. In patients with a preoperative SARS‐CoV‐2 diagnosis, the incidence of total postoperative complications (OR, 3.00; 95% CI, 1.12–8.01; p = 0.028) and PPCs (OR, 4.20; 95% CI, 1.11–15.91; p = 0.035) both increased in patients infected having surgery within 2 weeks compared with non‐infection before surgery. However, patients who underwent lung resection more than 2 weeks after SARS‐CoV‐2 diagnosis had a similar risk of postoperative complications and surgical outcomes with those non‐infection before surgery. Conclusion This is the first study to provide evidence regarding the optimum timing of lung resection surgery and assessing early outcomes after surgery in NSCLC patients with SARS‐CoV‐2 infection. Our study documents that the SARS‐CoV‐2 infection did not complicate surgical procedures for lung cancer, and suggest that lung surgery should be postponed at least 2 weeks after SARS‐CoV‐2 infection for NSCLC patients.

Abstract:Background: Six months since the outbreak of coronavirus disease (COVID-19), the pandemic continues to grow worldwide, although the outbreak in Wuhan, the worst-hit area, has been controlled. Thus, based on the clinical experience in Wuhan, we hypothesized that there is a relationship between the patient's CO 2 levels and prognosis. Methods: COVID-19 patients' information was retrospectively collected from medical records at the Leishenshan Hospital, Wuhan. Logistic and Cox regression analyses were conducted to determine the correlation between decreased CO 2 levels and disease severity or mortality risk. The Kaplan-Meier curve analysis was coupled with the log-rank test to understand COVID-19 progression in patients with decreased CO 2 levels. Curve fitting was used to confirm the correlation between computed tomography scores and CO 2 levels. Results: Cox regression analysis showed that the mortality risk of COVID-19 patients correlated with decreased CO 2 levels. The adjusted hazard ratios for decreased CO 2 levels in COVID-19 patients were 8.710 [95% confidence interval (CI): 2.773–27.365, P < 0.001], and 4.754 (95% CI: 1.380–16.370, P = 0.013). The adjusted odds ratio was 0.950 (95% CI: 0.431–2.094, P = 0.900). The Kaplan-Meier survival curves demonstrated that patients with decreased CO 2 levels had a higher risk of mortality. Conclusions: Decreased CO 2 levels increased the mortality risk of COVID-19 patients, which might be caused by hyperventilation during mechanical ventilation. This finding provides important insights for clinical treatment recommendations.

Abstract:Background A history of SARS-CoV-2 infection has been reported to be associated with an increased risk of postoperative pulmonary complications (PPCs). Even mild PPCs can elevate the rates of early postoperative mortality, intensive care unit (ICU) admission and prolong the length of ICU and/or hospital stays. Consequently, it is crucial to develop perioperative management strategies that can mitigate these increased risks in surgical patients who have recently been infected with SARS-CoV-2. Accumulating evidence suggests that nitric oxide (NO) inhalation might be effective in treating COVID-19. NO functions in COVID-19 by promoting vasodilation, anticoagulation, anti-inflammatory and antiviral effects. Therefore, our study hypothesises that the perioperative use of NO can effectively reduce PPCs in patients with recent SARS-CoV-2 infection. Method and analysis A prospective, double-blind, single-centre, randomised controlled trial is proposed. The trial aims to include participants who are planning to undergo surgery with general anaesthesia and have been recently infected with SARS-CoV-2 (within 7 weeks). Stratified allocation of eligible patients will be performed at a 1:1 ratio based on the predicted risk of PPCs using the Assess Respiratory Risk in Surgical Patients in Catalonia risk index and the time interval between infection and surgery. The primary outcome of the study will be the presence of PPCs within the first 7 days following surgery, including respiratory infection, respiratory failure, pleural effusion, atelectasis, pneumothorax, bronchospasm and aspiration pneumonitis. The primary outcome will be reported as counts (percentage) and will be compared using a two-proportion 2 test. The common effect across all primary components will be estimated using a multiple generalised linear model. Ethics and dissemination The trial is approved by the Institutional Review Board of Xijing Hospital (KY20232058-F1). The findings, including positive, negative and inconclusive results, will be published in scientific journals with peer-review processes. Trial registration number NCT05721144.

Abstract:Coronavirus disease (COVID-19) is a severe infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that binds to the cells; angiotensin converting enzyme 2 (ACE2) receptor. In the first severe case of COVID-19 in Shenzhen city, we found that in addition to the typical clinical manifestations, our patient presented hemoptysis, refractory hypoxemia and pulmonary fibrosis-like changes on computed tomography (CT) involving alveoli and pulmonary interstitium in the early stage and acute pulmonary hypertension and right heart failure in the later stage, which were not completely justified by myocarditis, acute respiratory distress syndrome (ARDS), pulmonary fibrosis and high PEEP level. The lung compliance deterioration of this patient was not as serious as we expected, indicating classic ARDS was not existed. Simultaneously, the first autopsy report of COVID-19 in China showed normal-structured alveoli and massive thick excretion in the airway. Then, we speculated that the virus not only attacked alveolar epithelial cells, but also affected pulmonary vascular endothelial cells. Imbalance in the ACE2-RAAS- bradykinin axis and the cytokine storm could be an important mechanism leading to pathophysiological changes in pulmonary vascular and secondary refractory hypoxemia. Pulmonary vasculitis or capillaritis associated to immune damage and an inflammatory storm could exist in COVID-19 because of ground-glass opacities in the subpleural area, which are similar to connective tissue disease associated interstitial lung disease (CTD-ILD). Thus, this case elucidates new treatment measures for COVID-19.

Abstract:A 56-year-old woman presented at our emergency department with nausea and acute chest pain that radiated to her interscapular region and left leg. She had experienced no syncope, vomiting, or shortness of breath. Her medical history included smoking and a myocardial infarction 16 years earlier. Her family history included coronary artery disease and an aortic aneurysm. On presentation, her physical indings were unremarkable except for hypertension (160/110 mmHg). She was admitted to the critical care unit and given intravenous nitroprusside and metoprolol for blood-pressure control. Because aortic dissection was suspected, she underwent contrast-enhanced computed tomography (CT), which revealed dilation of the sinuses of Valsalva (maximum diameter, 5.6 cm) and effacement of the sinotubular junction (Fig. 1). A Stanford type B aortic dissection was also present; the intimal flap began just distal to the origin of the left subclavian artery, involved the entire descending thoracoabdominal aorta, and extended distally into both common iliac arteries (Fig. 2). At the L4, L5, S1, and S2 levels, the dural sac was enlarged (maximum diameter, 2.7 cm; dural sac ratio >0.98) (Fig. 3). On the basis of these indings alone, Marfan syndrome was diagnosed. An aortic stent was placed at the origin of the dissection, and the patient recovered uneventfully. Abstract:BACKGROUND: One-lung ventilation (OLV) is frequently applied during video-assisted thoracoscopic surgery (VATS) airway management to collapse and isolate the nondependent lung (NL). OLV can give rise to hypoxemia as a result of the pulmonary shunting produced. Our study aimed to assess the influence of continuous positive airway pressure (CPAP) combined with small-tidal-volume ventilation on improving arterial oxygenation and decreasing pulmonary shunt rate (Q S /Q T ) without compromising surgical field exposure during OLV. METHODS: Forty-eight patients undergoing scheduled VATS lobectomy were enrolled in this research and allocated into three groups at random: C group (conventional ventilation, no NL ventilation intervention was performed), LP group (NL was ventilated with lower CPAP [2 cmH 2 O] and a 40–60 mL tidal volume [TV]), and HP group (NL was ventilated with higher CPAP [5 cmH 2 O] and a 60–80 mL TV). Record the blood gas analysis data and calculate the Q S /Q T at the following time: at the beginning of the OLV (T0), 30 min after OLV (T1), and 60 min after OLV (T2). Surgeons blinded to ventilation techniques were invited to evaluate the surgical fields. RESULTS: The demography data of the three groups were consistent with the surgical data. At T1, PaO 2 in the HP group was substantially higher compared to the C group ( P < 0.05), while there was no significant difference in the LP group ( P > 0.05). At T1-T2, PaCO 2 in the LP and HP groups was significantly less than that in the C group ( P < 0.05). At T1, the Q S /Q T values of groups C, LP, and HP were 29.54 ± 6.89%, 22.66 ± 2.08%, and 19.64 ± 5.76%, respectively, and the Q S /Q T values in the LP and HP groups markedly reduced ( P < 0.01). The surgical field’s evaluation by the surgeon among the three groups was not notable ( P > 0.05). CONCLUSION: CPAP combined with small-tidal-volume ventilation effectively improved arterial oxygenation and reduced Q S /Q T and PaCO 2 without compromising surgical field exposure during OLV. Among them, 5 cmH 2 O CPAP + 60–80 ml TV ventilation had a better effect on improving oxygenation.

Abstract:TYPE: Case Report Publication TOPIC: Transplantation INTRODUCTION: Coronavirus disease 2019 (COVID-19) related severe acute respiratory distress syndrome (ARDS) cases have an extremely high mortality rate, even with the support of mechanical ventilation (MV) and extracorporeal membrane oxygenation (ECMO). This report provides initial experience of bilateral lung transplantation (LT) as a salvage therapy for patients with progressive deterioration of lung function and multiple negative nucleic acid tests post-COVID-19. CASE PRESENTATION: A 58-year-old man had fever and cough for 1 day and a history of contact with a COVID-19 patient on January 20, presented to local hospital in Jiangsu Province, on January 23. COVID-19 was confirmed and progressively refractory hypoxemia was presented. MV support started from February 7 followed by tracheostomy. Convalescent plasma was infused. The patient still progressed to severe ARDS, although nucleic acid tests were negative. Veno-venous (V-V) ECMO was adopted to support the patient's life and LT was considered. On February 28, the patient manifested an unstable hemodynamic status and an urgent LT was performed with an additional V-A ECMO intraoperatively. ECMO was weaned off 37 hours post-surgery. The patient had intermittently weaned off MV since postoperative day 3 and the rehabilitation program was initiated. DISCUSSION: This case inspires further consideration of candidate evaluation, protection of medical team and research on post-transplantation physical dynamics (Table 1). More evidence are needed to evaluate this salvage therapy in terms of the short-term and long-term benefits. CONCLUSIONS: Although cases are limited, it provides initial experience on how to prepare both the medical team and the patient for the high-risk LT for ARDS post-COVID-19. Short-Term Outcomes of Patients With COVID-19 Undergoing Invasive Mechanical Ventilation: A Retrospective Observational Study From Wuhan, China

Abstract:Background: COVID-19 has spread rapidly worldwide. Many patients require mechanical ventilation. The goal of this study was to investigate the clinical course and outcomes of patients with COVID-19 undergoing mechanical ventilation and identify factors associated with death. Methods: Eighty-three consecutive critically ill patients with confirmed COVID-19 undergoing invasive mechanical ventilation were included in this retrospective, single-center, observational study from January 31 to March 15, 2020. Demographic, clinical, laboratory, radiological, and mechanical ventilation data were collected and analyzed. The primary outcome was 28-day mortality after endotracheal intubation. The secondary outcomes included the incidences of SARS-CoV-2-related cardiac, liver, and kidney injury. Results: Seventy-four out of 83 (89.2%) patients achieved oxygen saturation above 93% after intubation. Forty-nine out of 83 (59%) patients died and 34 (41%) patients survived after 28 days of observation. Multivariable regression showed increasing odds of death associated with cardiac injury (odds ratio 15.60, 95% CI 4.20-74.43), liver injury (5.40, 1.46-23.56), and kidney injury (8.39, 1.63-61.41), and decreasing odds of death associated with the higher PaO2/FiO2 ratio before intubation (0.97, 0.95-0.99). PaO2/FiO2 ratio before intubation demonstrated a positive linear correlation with platelet count (r = 0.424, P = 0.001), and negative linear correlation with troponin I (r = -0.395, P = 0.008). Conclusions: Cardiac, liver, and kidney injury may be associated with death for critically ill patients with COVID-19 undergoing invasive mechanical ventilation. The severity of pre-intubation hypoxia may be associated with a poorer outcome of patients with COVID-19 undergoing invasive mechanical ventilation. Larger, multi-institutional, prospective studies should be conducted to confirm these preliminary results.
