Abstract:Purpose: Brachytherapy plays a crucial role in the standard of care for locally advanced gynecological malignancies. In this report, we present the experience from a tertiary teaching hospital, which is a referral center for image-guided brachytherapy (IGBT) in the management of locally advanced gynecological malignancies. Material and methods: This was a retrospective study of 130 patients referred to our hospital for IGBT after receiving initial external beam radiotherapy in their primary healthcare facilities, from January 2021 till January 2023. CT-based planning was done to delineate high-risk clinical target volume (HR-CTV). Dose of 6-7.5 Gy in 3-4 fractions was prescribed. Overall treatment time (OTT) was calculated, and patients were assessed for clinical response and toxicity after three months. Results: All patients received IGBT using an intra-cavitary or interstitial technique. The D90 HR-CTV mean EQD2 dose was 28.34 ±2.78 Gy. The mean EQD2 dose to 2 cc of the bladder, rectum, and sigmoid was 18.31 ±5.19 Gy, 14.14 ±5.76 Gy, and 17.43 ±4.75 Gy, respectively. The median interval time between the last fraction of external beam radiation therapy (EBRT) and first evaluation in the hospital was 19 (range, 13-28) days (interquartile range [IQR]). The median time between the completion of chemoradiation and brachytherapy procedure was 25 (range, 19-33) days (IQR). The mean overall treatment time (OTT) was 63.5 ±14.7 days. Conclusions: This study highlights the established advantages of image-guided interstitial brachytherapy and associated challenges. To optimize the overall treatment duration, it is imperative to prioritize and update the referral processes for brachytherapy centers.
Brachytherapy Workflow Practices: Analysis of Different Workflow Scenarios in Patients With Cervical Cancer and Impact on IGBT Implementation-An IAEA Study

Abstract:Purpose: The workflow of brachytherapy (BT) is an essential aspect of treatment to consider in image-guided brachytherapy (IGBT). It has an overarching effect influencing patient throughput and the number of cancer treatments that can be performed as it occupies equipment, space, and personnel. There is limited research addressing this issue. Under the International Atomic Energy Agency's Coordinated Research Activity titled IGBT for cervix cancer: An implementation study, our study analyzes various scenarios in the clinical workflow of BT delivery for cervical cancer. It aims to determine the extent to which these scenarios allow the routine implementation of IGBT. With this information, current barriers and individualized adaptations to efficient workflows can be identified to enhance the global application of IGBT, leading to better cervical cancer treatment. Materials and methods: A web-based poll of questions regarding practices in BT workflow was presented to 62 participants from low-, lower middle-, upper middle-, and high-income countries (19 countries). Results: This study highlighted diversity in BT practices across countries, income levels, and regions. It identified variations in workflow, patient throughput, and resource availability, which can have implications for the efficiency and quality of BT treatments. Scenario A, utilizing multiple locations for the steps of the BT procedure, was the most commonly used. The availability of resources, such as imaging devices and trained personnel, varied among the participating centers and remained challenging for IGBT implementation and sustainability. Conclusion: The design of the BT facility plays a vital role in improving efficiency, with a dedicated BT suite contributing to an efficient workflow but limiting patient throughput, especially for high-volume centers. Although IGBT is effective, its implementation requires consideration of various logistical challenges and should be individualized.

Abstract:Background A survey of medical physics residency program directors was conducted in Spring 2021 to examine the current state of brachytherapy (BT) training during residency. In this related work, a subsequent survey of therapeutic medical physics residents in 2022 was conducted to assess the confidence and experience of the trainees. Concerns for access to high‐quality and diverse training in BT have escalated in importance due to recent declines in BT utilization. Methods A survey consisting of 26 questions was designed by a working unit of the Brachytherapy Subcommittee of the American Association of Physicists in Medicine (AAPM) and approved for distribution by the Executive Committee of the AAPM. The survey was distributed to current trainees and recent graduates of the Commission on Accreditation of Medical Physics Education Programs accredited therapeutic medical physics residency programs by the AAPM. The participant response was anonymously recorded in an online platform and subsequently analyzed using spreadsheet software. Results The survey was distributed to 796 current medical physics residents or recent graduates over the course of 6 weeks in February and March of 2022. The survey received 736 views and a total of 182 responses were collected, with 165 respondents completing the survey in full. Among those responses, 110 had completed their residency training, with program start dates ranging from calendar years 2015 to 2021. Individual responses from the survey takers (including partial survey submissions) were evaluated and analyzed to compile results. Conclusions Medical physics residents reported the highest levels of confidence and caseload for gynecological BT procedures when compared with other surveyed treatment techniques. This indicates opportunities to improve training and increase access to clinical caseload are needed in order to improve competency and confidence. Time constraints (clinical and rotation‐based) were indicated as impediments to BT proficiency. Medical physics residents reported enthusiasm for additional training opportunities in BT, and it is evident that additional structure and programs are required to ensure adequate access to BT training during residency.

A Novel Reconstruction and Tracking of 3D-Articulated Human Body from 2D Point Correspondences of a Monocular Image Sequence

Abstract:A novel method is proposed to estimate the 3D relative positions of an articulated body from point correspondences in an uncalibrated monocular image sequence. It is based on a camera perspective model. Unlike previous approaches, our proposed method does not require camera parameters or a manual specification of the 3D pose at the first frame, nor does it require the assumption that at least one predefined segment in every frame is parallel to the image plane. Our work assumes a simpler assumption, for example, the actor stands vertically parallel to the image plane and not all of his/her joints lie on a plane parallel to the image plane in the first frame. Input into our algorithm consists of a topological skeleton model and 2D position data on the joints of a human actor. By geometric constraint of body parts in the skeleton model, 3D relative coordinates of the model are obtained. This reconstruction from 2D to 3D is an ill-posed problem due to non-uniqueness of solutions. Therefore, we introduced a technique based on the concept of multiple hypothesis tracking (MHT) with a motion-smoothness function between consecutive frames to automatically find the optimal solution for this ill-posed problem. Since reconstruction configurations are obtained from our closed-form equation, our technique is very efficient. Very accurate results were attained for both synthesized and real-world image sequences. We also compared our technique with both scaled-orthographic and existing perspective approaches. Our proposed method outperformed other approaches, especially in scenes with strong perspective effects and difficult poses.

Abstract:Abstract This study assessed the significance of hands-on-training (HoT) and questionnaire-based surveys on 3D image-guided brachytherapy (3D-IGBT) and a combination of intracavitary and interstitial brachytherapy, the so-called ‘hybrid’ BT (HBT), in uterine cervical cancer. In October 2023, 29 radiation oncologists, nurses, radiologic technologists and medical physicists from 10 Japanese facilities participated in an HoT on 3D-IGBT and HBT. Questionnaires were distributed to each participant before and after the HoT, and feedback was obtained through online channels. The questionnaire response rate was 83% (24/29), with at least one participant responding from each facility. ‘Insertion of applicators and needles’, ‘human resource shortage’ and ‘pain relief and sedation’ were the primary concerns of radiation oncologists. ‘Applicator reconstruction’, ‘ optimization of dwell positions’, ‘ treatment planning’ and ‘ human resource shortages ’ were the primary concerns of radiological technologists and medical physicists. The HoT content was adjusted according to the results of preliminary surveys. The concerns expressed by the participants were addressed during the lectures and practical training. Significant reductions in anxiety were observed toward all items of the 10-point self-assessment after the HoT, regardless of the profession. The average score on satisfaction with the HoT (on a 10-point scale) was 9.52 (minimum of 8 and maximum of 10). In conclusion, HoT tailored in response to a pre-questionnaire-based survey effectively reduced participants’ anxiety regarding the implementation of 3D-IGBT and HBT.

Abstract:Purpose: A prospective phase 2 study was conducted to evaluate the feasibility and safety of single-application multifractionated (SA-MF), high-dose-rate (HDR), image guided adaptive brachytherapy (IGABT) for cervical cancer. Methods and materials: Patients (N = 41) with International Federation of Gynaecology and Obstetrics 2009 stage IIB-IVA disease recruited between 2017 and 2019 underwent SA-MF. After completion of external beam radiation therapy of 50 Gy in 25 fractions, patients received magnetic resonance IGABT. The IGABT protocol consisted of a single brachytherapy (BT) application and treatment with 3 fractions of HDR (9 Gy on day 1; 2 fractions of 7 Gy with a minimum 6-hour gap on day 2) after achieving planning aims of the high-risk clinical target volume (HRCTV) receiving >84 Gy EQD2 and 2 cm3 of the bladder and rectum/sigmoid receiving ≤85 Gy and <71 Gy, respectively. Interfraction variation was addressed by performing computed tomography planning and coregistration using a mutual information-based coordinate system on day 2 before the second fraction. Organ at risk contouring was done on computed tomography, and doses were re-evaluated and reoptimized if required. Results: Thirty-eight patients were treated as per the protocol. All patients underwent Intracavitary + Interstitial BT with needles (median, 4; range, 3-11). The mean ± standard deviation HRCTV volume was 41 ± 21 cm3 and HRCTV D90 dose was 87.2 ± 3.6Gy. The 0.1 cm3 and 2 cm3 to bladder, rectum, and sigmoid were -103.2 ± 10.6 Gy and -84.6 ± 6.8 Gy, 82.2 ± 9.5 Gy and -68.3 ± 5.7 Gy, and 83.5 ± 9.8 Gy and -69.5 ± 5.9 Gy, respectively. Six patients required reoptimization before the second fraction to meet planning aims. Mean overall treatment time was 47 ± 6 days. With a median follow up of 22 months (range, 2-37), 2-year local control and disease-free and overall survival were 90.1%, 85%, and 94.5%, respectively. So far 1 patient with grade II and 2 patients with grade III rectal toxicities have been reported. Conclusion: Magnetic resonance IGABT with SA-MF BT was feasible in 95% of patients. The dosimetric parameters and clinical results achieved so far look promising.

Abstract:Gynecologic malignancies are a leading cause of death in women worldwide. Standard treatment for many primary and recurrent gynecologic cancer cases includes a combination of external beam radiation, followed by brachytherapy. Magnetic Resonance Imaging (MRI) is benefitial in diagnostic evaluation, in mapping the tumor location to tailor radiation dose, and in monitoring the tumor response to treatment. Initial studies of MR-guidance in gynecologic brachtherapy demonstrate the ability to optimize tumor coverage and reduce radiation dose to normal tissues, resulting in improved outcomes for patients. In this article we describe a methodology to aid applicator placement and treatment planning for 3 Tesla (3T) MR-guided brachytherapy that was developed specifically for gynecologic cancers. This has been used in 18 cases to date in the Advanced Multimodality Image Guided Operating suite at Brigham and Women's Hospital. It is comprised of state of the art methods for MR imaging, image analysis, and treatment planning. An MR sequence using 3D-balanced steady state free precession in a 3T MR scan was identified as the best sequence for catheter identification with ballooning artifact at the tip. 3D treatment planning was performed using MR images. Item in development include a software module designed to support virtual needle trajectory planning that includes probabilistic bias correction, graph based segmentation, and image registration algorithms. The results demonstrate that 3T MR has a role in gynecologic brachytherapy. These novel developments improve targeted treatment to the tumor while sparing the normal tissues.
Challenges in the Sustainability of Brachytherapy Service in Contemporary Radiotherapy

Abstract:Brachytherapy has a long history of delivering a highly conformal radiation dose to the target volume with sparing of adjacent normal tissue and has an irreplaceable role in certain cancers, such as cervical and prostate cancers. There have been futile attempts to replace brachytherapy with other radiation techniques. Despite that there are multifaceted challenges in preserving this dying art, from establishment, to a trained workforce, to maintenance of the equipment and source replacement costs. Here we focus on the challenges to access brachytherapy, the availability and distribution of care across the globe and appropriate training leading to proper implementation of the procedure. Brachytherapy holds a significant place in the treatment armamentarium of most common cancers, such as cervical, prostate, head and neck and skin cancers. However, there is an uneven distribution of brachytherapy facilities, not only across the globe, but also at a national level, with a larger proportion of facilities concentrated in certain regions, more so in low and low-middle income countries. The regions with the highest incidence of cervical cancer have the least access to brachytherapy facilities. Attempts to bridge the gap are essential and should be focused on uniform distribution and access to care, improving training of the workforce through specialised training programmes, reducing the cost of care, planning to reduce the recurring cost, generating evidence and research guidelines, renewing interest in brachytherapy through rebranding, use of social media and building an attainable long-term roadmap.

Abstract:Simulation is a technique used in healthcare to replicate clinical scenarios and improve patient safety, efficacy, and efficiency. Simulation-based medical education facilitates training and assessment in healthcare without increasing risk to patients, supported by ample evidence from surgical/procedural specialties. Simulation in radiation oncology has been leveraged to an extent, with successful examples of both screen-based and hands-on simulators that have improved confidence and performance in trainees. In the current era, evidence substantiates a significant deficit in brachytherapy procedure education, with radiation oncology residents reporting low confidence in this procedural skill, largely attributable to insufficient caseloads at some centers. Simulation-based medical education can facilitate structured training and competency-based assessment in brachytherapy skills. This review discusses existing advances and future directions in brachytherapy simulation, using examples from simulation in surgical specialties.

Abstract:Background and purpose Radiation oncology is an essential component of therapeutic oncology and necessitates well-trained personnel. Multicatheter brachytherapy (MCBT) is one radiotherapeutic option for early-stage breast cancer treatment. However, specialized hands-on training for MCBT is not currently included in the curriculum for residents. A recently developed hands-on brachytherapy workshop has demonstrated promising results in enhancing knowledge and practical skills. Nevertheless, these simulation-based teaching formats necessitate more time and financial resources. Our analyses include computational models for the implementation and delivery of this workshop and can serve as a basis for similar educational initiatives. Methods This study aimed to assess the cost-effectiveness of a previously developed and evaluated breast brachytherapy simulation workshop. Using a micro-costing approach, we estimated costs at a detailed level by considering supplies, soft- and hardware, and personnel time for each task. This method also allows for a comprehensive evaluation of the costs associated with implementing new medical techniques. The workshop costs were divided into two categories: development and workshop execution. The cost analysis was conducted on a per-participant basis, and the impact on knowledge improvement was measured using a questionnaire. Results The total workshop costs were determined by considering the initial workshop setup expenses including the development and conceptualization of the course with all involved collaborators, as well as the costs incurred for each individual course. The workshop was found to be financially efficient, with a per-participant cost of € 39, considering the industrial sponsorship provided for brachytherapy equipment. In addition, we assessed the workshop's efficacy by analyzing participant feedback using Likert scale evaluations. The findings indicated a notable enhancement in both theoretical and practical skills among the participants. Moreover, the cost-to-benefit ratio (CBFR) analysis demonstrated a CBFR of € 13.53 for each Likert point increment. Conclusion The hands-on brachytherapy workshop proved to be a valuable and approximately cost-effective educational program, leading to a significant enhancement in the knowledge and skills of the participants. Without the support of industrial sponsorship, the costs would have been unattainable.

Abstract:Venlafaxine is a widely used serotonin- and norepinephrine-reuptake inhibitor-type antidepressant that causes serious adverse effects in at least 5% of cases. Serious withdrawal symptoms may occur within hours of cessation or reduction of the usual dosage and may affect motor and coordination skills to such a degree that patients should be explicitly urged either to adhere to a strict medication routine or not to drive a car. Recent clinical evidence about withdrawal symptoms is presented that may indicate incidents in noradrenergic activity irrespective of dosage.
Assessment of iodine nutrition status among school age children of Nepal by urinary iodine assay.

Abstract:The present study was undertaken to evaluate the iodine status of Nepalese school age children by measuring urinary iodine excretion (UIE). A population based cross-sectional study was conducted during November-December 2006 among 1,094 school age children. Spot urine samples were collected from all children and UIE was measured during February to March 2007 by an ammonium persulfate digestion microplate (APDM) method. The median UIE at the national level was 193.10 microg/l, indicating adequate iodine intake in Nepalese schoolchildren. The proportion of the population having UIE below 50 microg/l and below 100 microg/l were 4.5% and 22.0%, respectively. Determination of precision of the method was done following calculation of the inter- and intra-assay coefficient of variation (CV). At low, medium and high concentrations of urinary iodine the intra-assay CVs were 6.3, 1.8 and 1.9%, respectively. The inter-assay CVs for low, medium and high concentrations of urinary iodine were 11.9, 4.9 and 6.2%, respectively. Therefore, current iodine nutrition status is at satisfactory levels in Nepal. An effective monitoring program must be continued to ensure optimal iodine status and prevent the population from developing iodine deficiency disorder (IDD).
Three Dimensional Printed Individual Template Based (192)ir High-Dose Rate Interstitial Brachytherapy for Central Recurrent Gynecologic Cancer: A Phase II Clinical Trial

Abstract:To evaluate the feasibility, safety, and accuracy of three-dimensional (3D)-printed individual template (3D-PIT) based with HDR interstitial brachytherapy (HDR-IB) for pelvic central recurrent gynecologic cancer (CR-GYN). 32 patients diagnosed CR-GYN in our institution were received 192Ir HDR-IB. 3D-PIT technique was carried out for all these patients in our center. They underwent procedure that firstly simulation; then preplan; thirdly 3D template design and printing and fourthly interstitial 192Ir HDR brachytherapy operation. There are two types of individual 3D-PIT designed. Type I: transvaginal template/applicator, and Type II: transvaginal combined transperineal template. The prescription dose to GTV was 10-36Gy, 5-6Gy/fraction, 2-6f. Dose parameters were D90, D100, V100, conformal index, external index and homogeneity index of the target volume, and the dose received by 2cm3 of normal tissue of organs at risk (rectum, bladder, and intestine) were recorded. We compared the dose parameters of the treatment plan with the preplan. The short-term efficacy was evaluated by RECIST v1.1, and the adverse event was evaluated by CTCAE V4.0. In total 32 patients, 205 needles were inserted for 153 applications, The mean V100, D100, and D90 per fraction were 88.9%±9.8%,3.45Gy±0.54Gy, and 5.79 Gy±0.32Gy respectively; the mean D2cc of rectum, bladder and colon per fraction were 3.14Gy±0.23Gy, 3.76Gy±0.72Gy and 2.47Gy±0.34Gy respectively. The mean CI, EI, and HI were 0.59±0.12, 0.31±0.16, and 0.47±0.22. Dosimetry parameters comparison between preplan and treatment plan of 20/32 patients with Type II 3D-PIT based HDR-IB show that there were no statistically significant (P > 0.05) difference in GTV volume, V100, D100, D90, CI and HI. Treatment plan EI was lower than preplan, and the difference was statistically significant (P = 0.022). Median follow up time was 10 months (3–28 months). No severe complications of procedures occurred. Grade 3 or 4 late toxicities (fistula ) were observed in 3 patients (9%). The local response rate (CR + PR) was 84.4% (27/32) one month later completion of treatment. In addition, the median TTP was 15.4 months (95% CI 11.3- 19.6ms), 1 year and 2 year local control rate were 51.7% and 51.7%. 3D-PIT based with CT-simulator for HDR-IB for central r-GYN has good dosimetry parameters consistently, was clinically feasible and efficient, and complications did not increased significantly.

Abstract:PURPOSE To evaluate clinical outcomes of CT-based adaptive intracavitary and interstitial brachytherapy (IC followed by IC-ISBT) in locally advanced cervical cancer (LACC) in resource-constrained settings. METHODS AND MATERIALS LACC patients treated with adaptive brachytherapy techniques were analyzed to evaluate treatment characteristics and clinical outcomes. The Kaplan-Meier method was used for survival analysis, and the log-rank test for univariate analysis. RESULTS Out of 141 eligible patients with LACC, 87 (61.7%) patients received external beam radiotherapy (EBRT) in referral hospitals, while 54 (38.3%) were treated at our center. We divided our cohort into two groups: poor EBRT responder group ( n = 70 [49.6%]) where IC-ISBT was adapted to achieve optimum tumor doses and OAR optimization group 71 (50.4%) where IC-ISBT was performed to reduce OAR doses. Median HRCTV-D90 dose was 88 Gy (range 70–109 Gy) with median HRCTV volume 33cc (range 15–96). Median D2cc doses to OARs were 90 Gy (range 70–107), 71 Gy (range 55–105) and 70 Gy (range 47–90) to bladder, rectum and sigmoid, respectively. At median follow-up of 32 months, 3-year local control (LC), locoregional control (LRC), disease-free survival (DFS) and overall survival (OS) were 83%, 75%, 64% and 72%, respectively. Subgroup analysis revealed significantly better outcomes for OAR optimization compared to poor EBRT responders, with 3-year LC (95% vs. 70.1%, p < 0.001), LRC (87.3% vs. 62.7%, p < 0.001), DFS (79.2% vs. 49.4%, p < 0.001), and OS (86.2% vs. 57.4%, p < 0.001) CONCLUSION In resource-constrained settings, implementation of Adaptive IC-ISBT is a viable alternative for optimizing OAR doses in LACC. However proactive approach employing IC-ISBT for tumor dose-escalation from first fraction of BT is warranted for improving LC in poor EBRT responders.

The Preliminary Results of 3-Dimensional Printed Individual Template Assisted 192ir High-Dose Rate Interstitial Brachytherapy for Central Recurrent Gynecologic Cancer

Abstract:Objective: To evaluate the feasibility and safety of high dose rate interstitial brachytherapy (HDR-IB) assisted with 3-dimensional printing individual template (3D-PIT) for central pelvic recurrent gynecologic cancer (CR-GYN). Methods: Totally 32 patients diagnosed with CR-GYN received iridium-192(192Ir) HDR-IB assisted with 3D-PIT that was classified in 2 types(Type I: transvaginal template/ applicator, and Type II: transvaginal combined transperineal template). The prescribed dose to gross tumor volume (GTV) was 10-36 Gy in 2-6 fractions. We rely on a few dosimetric parameters for quality control. The short-term efficacy was evaluated by RECIST v1.1, and the adverse event was evaluated by CTCAE V4.0. Results: The median V100, D100 and D90 of per fraction among all the patients were 88.9%±9.8%, 3.45Gy±0.54 Gy, and 5.79Gy±0.32 Gy, respectively. Dosimetric comparison between preplan and treatment plan of 20/32 patients with Type II 3D-PIT showed no significant difference in GTV volume, V100, D100, D90, conformation index (CI) and homogeneity index (HI). No severe treatment complications occurred. Grade 3 or 4 late toxicities (fistula) were observed in 3 patients (9%). The local response rate (complete remission, CR + partial remission, PR) was 84.4% (27/32) 1 month after completion of treatment. The median time to progression (TTP) was 15.4 months (95% CI 11.3- 19.6 months), 1-year local control (LC) rate were 51.7%. Conclusions: HDR-IB assisted by 3D-PIT was a reliable modality for CR-GYN due to the clinical feasibility and accepted complications.

Abstract:Subjects that Freud excluded or incompletely explored have been sites of theoretical expansion in over a century of observation: the role of the other, the self, the preoedipal period, action, the countertransference, limits to neutrality/anonymity/abstinence, the loci of the analytic drama, effects beyond interpretation, agency, and basic needs (versus wishes). These developments have led to conflicting theories and sect-like groupings within the field. Group psychological processes underlying this are discussed; and a broad and inclusive view of psychoanalysis is proposed under the heading of the study of the workings of mind. Additionally, substantial integrative proposals are offered with respect to the central tasks of individual development, theories of mind, the relational turn, and aspects of technique.
Patterns of practice survey for cervical cancer brachytherapy in Morocco

Abstract:PURPOSE This study surveyed radiation oncologists in Morocco to explore current practices and perspectives on brachytherapy for cervix cancer. METHODS AND MATERIALS A 37-question survey was conducted in April 2023 among 165 Moroccan radiation oncologists using Google Forms. RESULTS Of the 93 respondents, 39% treated over 20 patients in 2022 using 3D image-guided brachytherapy (BT) through the HDR technique; 2D techniques were not reported in the last five years. Intracavitary BT is uniformly applied with a tandem and ovoid applicator. Only 14% utilized interstitial needles for hybrid BT. Iridium-192 was the primary radioactive source (63%), followed by cobalt (37%). Ultrasound-guided 47% of applicator insertions. All used CT scans for planning, but only 6% used MRI fusion due to limited availability. Guidelines for target volume and dose prescription were mostly based on GEC-ESTRO recommendations (74%), followed by Manchester Point A (30.4%) and ABS (11%). Over 90% delineated CTV-HR and CTV-IR; 30% delineated GTV. All marked the bladder and rectum, while 52% marked the sigmoid , 5% the small bowel, and 3% the recto-vaginal point. For dosimetry, 12% used ICRU 89 points, 54% used dose-volume histograms (DVH), and 36% used both. Most reported EQD2cc for OARs for the rectum and bladder, with nine still using ICRU point doses. The most common fractionation schema was 7 Gy in four fractions (60%) and 7 Gy in three fractions (55%). CONCLUSIONS Brachytherapy remains essential for treating cervical cancer in Morocco. Key areas for improvement include MRI fusion-guided brachytherapy, access to advanced applicators, expanding interstitial techniques, and professional training and national referential.

Abstract:To report the physical dosimetry study and preliminary results of a prospective study evaluating 3D-printing template assisted with CT-guidance for 125I seed brachytherapy to treat recurrent or metastatic gynecological carcinoma. In this retrospective study, with median follow-up of 10 months for all patients, 95 patients were enrolled and analyzed; inclusion criteria were Karnofsky performance status 60 to 100, expected overall survival >3 months, patients were pathologically or radiologically confirmed to be recurrent or metastatic gynecological carcinoma and the tumors were unresectable due to individual rejection or surgeon consultation. All patients have accepted 3D-printing template assisted with CT-guidance for 125I seed brachytherapy. Kaplan-Meier survival analysis was used to estimate local control (LC), overall survival (OS), and progression-free survival (PFS). We analyzed the confounding factors that affected LC, PFS, and OS by using Cox proportional hazards regression model. Acute and late toxic effects were prospectively assigned by using the Radiation Therapy Oncology Group (RTOG) and European Organization for Research and Treatment of Cancer (EORTC) criteria. Of 87 patients (median [range] age, 53 [29-92] years; vulvar cancer, 2 [2%]; endometrial cancer, 10 [11%]; cervical cancer, 72 [83%]; ovarian cancer, 3 [3%]), 32 (37%) were alive at last follow-up. 9 (10%) patients adopted secondary RIS implantation on different sites. 75% quantile LC was 16 months (1-year LC, 85%; 95%CI, 77%-93%). Median PFS was 12 months (1-year PFS, 48%; 95%CI, 36%-59%). Median OS was 15 months (1-year OS, 65%; 95%CI, 54%-76%). On multivariate analysis, GTV D100 >60Gy independently correlative for good LC (hazard ratio [HR], 0.139; 95% CI, 0.037-0.527; P=.004), PFS (HR, 0.346; 95% CI, 0.180-0.666; P=.001) and OS (HR, 0.185; 95% CI, 0. 083-0.416; P<.001). Rates of grade 2 acute radiothermitis, vomiting, proctitis and hematuresis were 2 (2%), 1 (1%), 3 (3%) and 2 (2%). Acute grade 3 proctitis occurred in 1 (1%) patient. Late toxic effects were uncommon: 1 (1%) patient developed an hematuresis (grade 2) and 1 (1%) patient developed an rectovaginal fistula. There were no acute or late grade 5 toxic effects. 125I seed brachytherapy to treat recurrent or metastatic gynecologic carcinoma showed promising clinical outcomes and rates of toxic effects. Further optimization of 125I seed brachytherapy is needed in order to improve the accuracy of treatment.

Abstract:Objective: Overall treatment time (OTT) is an important index for local control in patients with locally advanced‌ cervical cancer treated with definitive chemoradiation (External Beam Radiation Therapy (EBRT), Brachytherapy (BT) and concomittant chemotherapy). This study aimed to evaluate the efficiency and safety of reducing OTT by shortening the brachytherapy duration to one week in the intervention group copared to three weeks in the control group. Method: The study was a non-randomized open-label phase II clinical trial, carried out on 49 cervical cancer patients (26 in intervention grouop and 23 in control group) who received EBRT concomitant with Cisplatin, followed by brachytherap in order to deliver 60 Gy equivalent total doses in 2-Gy fractions (EQD2) to Intermediate Risk-Clinical Tumor Volume (IR-CTV) and 85-90 Gy EQD2 to High Risk-Clinical Tumor Volume (HR-CTV). In the intervention group, all brachytherapy sessions were performed in 1 week, while for the control group, it was administrated in 3 consecutive weeks. The participants were followed (Minimum follow up time was 6 month and median follow up time was 10 month) to assess response and toxicity of the treatment. Results: Overall, more than 95% of study participants had a complete response and more than 4.0% reported partial response, and no treatment failure was observed. The complete response in intervention and control groups was 96.1% and 95.6%, respectively (P value > 0.05). There was no difference in acute toxicity between the two groups. Conclusion: considering that short course brachytherapy was non inferior to conventional course from point of Response Rate and Side Effects during follow up time; so this strategy can be considered as an option for reducing the OTT which can at least cause decreasing the costs. Studies with larger sample size and phase 3 are recommended.
