Abstract:OBJECTIVE:To evaluate the effect of distraction osteogenesis for severe micrognathia by comparing the pre- and post-operative profile and mentolabial relationship.METHODS:16 cases underwent temporal-mandibular joint plasty and temporal fasciomuscular flap transfer. The mandibular distraction began at the 5th postoperative day at a rate of 0.8 mm a day, two times a day. Bony and soft tissue cephalometry were performed before and after operation. T-test was used to study the change after distraction osteogenesis.RESULTS:There were significant differences in facial convexity, lower facial height, lower lip length, inter-labial distance, the ratio of lip to mental, the distance from lip to esthetic plane, the depth of mentolabial crease and the thickness of mental soft tissue.CONCLUSIONS:Mandibular distraction osteogenesis can markedly improve the soft tissue profile of the middle and lower face for severe micrognathia.
Simultaneous correction of bilateral temporomandibular joint ankylosis with mandibular micrognathia using internal distraction osteogenesis and 3-dimensional craniomaxillofacial models.

Abstract:The present study evaluated the simultaneous correction of bilateral temporomandibular joint ankylosis with mandibular micrognathia using internal distraction osteogenesis (DO) with the help of a 3-dimensional craniomaxillofacial model technique.A total of 16 patients (age 18 to 43 years) with bilateral temporomandibular joint ankylosis and mandibular micrognathia were included in the present study. Obstructive sleep apnea and hypopnea syndrome was diagnosed in all patients preoperatively. Three-dimensional craniomaxillofacial models of the 16 patients were constructed using computed tomography and a rapid prototype technique. Simulation surgery and individual internal DO was performed on the models. The treatment included simultaneous DO of the mandibular body and transport DO for temporomandibular joint arthroplasty. The distraction was started on the seventh day after surgery. The distraction rate was 0.8 mm/day. The patients began active mouth opening postoperatively. Distracters were kept in place for 4 months after distraction completion and then removed. Polysomnography, cephalometry, and computed tomography were performed at 6 months postoperatively.The obstructive sleep apnea and hypopnea syndrome was cured, and the micrognathia was corrected in all patients. The average mouth opening increased from 4.6 mm preoperatively to 33.5 mm postoperatively. The average range of the sella-nasion-supramental angle increased from 68.7 degrees preoperatively to 77.6 degrees postoperatively. Bone formation in the distraction gaps was observed. The follow-up period was 29.7 months (range 6 to 52). No complications or recurrence of temporomandibular joint ankylosis or micrognathia occurred in any patient during the follow-up period.Bilateral temporomandibular joint ankylosis accompanied by mandibular micrognathia and obstructive sleep apnea and hypopnea syndrome can be corrected effectively by simultaneous internal DO. The application of preoperative simulation surgery using 3-dimensional craniomaxillofacial model has many advantages for planning the surgical method and precise operation. Our preliminary results have shown that it is a safe, effective, and feasible technique.
Study in sEMG of masticatory muscles before and after occlusal rehabilitation in patients without occlusal support

Abstract:Objective: To investigate the changes in surface electromyography(sEMG) activity of masticatory muscles before and after occlusal rehabilitation in patients without occlusal support.Methods: 24 patients without occlusal support were selected and conducted with regularly occlusal rehabilitation.The sEMG of masseter muscle(MM) and anterior temporalis muscle(TA) at mandibular postural position(MPP) and during clenching at intercuspal position(ICP) were separately recorded in different stages.The changes of the mean amplitude(MA) of sEMG,the asymmetry indexes(AsI) of bilateral MM,TA and total masticatory muscles(AsMM,AsTA,Astot),activity indexes(AcI) of MM/TA in each stage were analyzed.Results: 1.The changes in sEMG activity before and after restoration at MPP: MA and AsI of MM had no significant difference in different stages(P > 0.05).MA and AsI of TA after using 3 months were significantly higher than using early stage.AcI of MM/TA was negative at every stage(P < 0.05).2.The changes in sEMG activity before and after restoration at MPP: MA of MM and TA was significantly increased after using three months(P <0.05).Each AsI was close to first wearing(P>0.05),but significantly lower than 1 week and 1 month after restoration(P<0.05).AcI of MM/TA were close to 0.Conclusion: The sEMG of masticatory muscles changed at different stages of prothetic rehabilitation in patients without occlusal support.sEMG can provide reference for evaluation of effect of occlusal rehabilitation.
Changes in masseter muscle morphology after surgical-orthodontic treatment in patients with skeletal Class III malocclusion with mandibular asymmetry: The automatic masseter muscle segmentation model

Abstract:Introduction: This study evaluated the masseter muscle changes after surgical-orthodontic treatment in patients with a skeletal Class III malocclusion using automatic segmentation. Methods: Images of 120 patients with skeletal Class III malocclusion were obtained and reconstructed at T0 (pretreatment), T1 (presurgery), and T2 (6-12-month postsurgery). The patients were divided into symmetrical and asymmetrical groups. The volume, major axis length, maximum cross-sectional area, horizontal cross-sectional area 5 mm above the mandibular foramen (CSAF), and orientation were calculated automatically. Results: In the asymmetrical group, the volume and major axis length on the deviated side were lower than on the nondeviated side at T0, T1, and T2 ( P < 0.05). There were no significant differences in maximum cross-sectional area and CSAF bilaterally. The orientation was coronally more vertical and sagittally more forward on the deviated side (both P < 0.001). In the symmetrical group, there were no significant bilateral differences at T0, T1, and T2. The volume, major axis length, and CSAF decreased, and the coronal orientation was more vertical on the nondeviated side at T2 than at T0 in both groups ( P < 0.05). The coronal plane orientation was more inclined on the deviated side at T2 than at T0 in the asymmetrical group ( P < 0.05). Conclusions: The smaller volume on the deviated side at T2 indicates the need for myofunctional training after surgery. The masseter muscle volume and the cross-sectional area did not recover to the preorthodontic levels. Studies with longer follow-up durations are needed to confirm these findings.

Abstract:Objective Patients with type IIA craniofacial microsomia (CFM) may benefit from mandibular distraction osteogenesis (MDO) treatment during childhood; however, remodelling of the mandible during the consolidation phase, which may affect the short‐term outcomes of MDO, has not yet been quantitatively analysed using computed tomography. Therefore, we aimed to investigate bone remodelling of the mandible in children with type IIA CFM treated with MDO before distractor removal and the factors that influence ramus vertical elongation efficiency. Materials and Methods Twenty‐three children with unilateral CFM were studied between 2020 and 2024. Longitudinal computed tomography data (preoperative, end of active phase and at pre‐distractor removal) were analysed. Condyle positions and the mandibular cant were analysed using a paired‐sample t test. The relapse rates of vertical lengthening and mandibular cant were calculated. The correlation between distraction efficiency and preoperative craniofacial morphology was analysed. Results The condyle on the affected side moved upwards and backwards by 28.84 ± 4.08 and 2.85 ± 4.33 mm, respectively during the active phase but lost 7.66 ± 2.64 mm of vertical extension during the consolidation phase. The relapse rates for vertical extension of the condyle and occlusal plane were 27% and 35%, respectively. The ratio of mandibular ramus height was positively related to EV. Conclusions In children with CFM, attention should be paid to vertical elongation instability and relapse of mandibular inclination during consolidation. Severe mandibular ramus hypoplasia is a preoperative risk factor for vertical skeletal relapse during consolidation. Further efforts are required to reduce the stress that leads to relapse.

Assessment of Masseter Volume and Postoperative Stability After Orthognathic Surgery in Patients With Skeletal Class III Malocclusion With Facial Asymmetry

Abstract:Objective: This study aimed to evaluate the effect of orthognathic surgery on masseter volume in patients with skeletal Class III malocclusion with facial asymmetry and the effect of masseter volume on stability in orthognathic surgery. Methods: This research studied 16 patients with Class III malocclusion with facial asymmetry who received combined orthodontic-orthognathic treatment and underwent craniofacial computed tomography (CT) before (T0), 2 weeks after (T1), and 6 months after (T2) surgery. Three-dimensional (3D) CT images were retrospectively analyzed, using 3D volume reconstruction to obtain the masseter volume and examine the impact of the masseter volume on stability in orthognathic surgery. Results: A statistically significant difference ( P < 0.05) in the volume of the masseter was found up to 6 months after orthognathic surgery compared with the preoperative period, and the reduction in the masticatory muscle volume on the lengthened side is greater than on the shortened side ( P < 0.05). The volume of both masseters differed according to facial asymmetry, and the difference was significantly reduced after orthognathic surgery ( P < 0.05). During the period time (T1-T2), cephalometric maxillary marker points were not significantly different ( P > 0.05), and mandibular marker points were significantly anteriorly shifted ( P < 0.05). There was an association between the masseter volume and anterior shift of point B (R > 0.5, P < 0.05), the upward and anterior shifts of the gonion point differed between the lengthened and shortened sides ( P < 0.05). Conclusion: The size of the masseter becomes smaller 6 months after orthognathic surgery, and orthognathic surgery improves both bone and soft tissue symmetry. A larger sagittal relapse of mandibular setback occurred in patients with greater masseter volume. Considering these alterations may be helpful in planning orthognathic surgery.
[Distraction Osteogenesis of the Mandibular Ramus in Hemifacial Microsomia].

Abstract:OBJECTIVE:To study the application of distraction osteogenesis (DO) of the mandibular ramus in hemifacial microsomia.METHODS:From 1999 to 2006, 15 patients with hemifacial microsomia were treated with DO of the ramus, including 12 cases of children and juveniles, 3 cases of grown-ups. Maxillary Le Fort I osteotomy, unilateral sagittal split ramus osteotomy, genioplasty and free scapular flaps were selected to perform on the first or secondary stage of treatment.RESULTS:Successful distraction was achieved in all patients with an average distraction distance of 21.25 mm. The distractor was removed after consolidation proved by clinical examination and X-ray. Facial appearance and and occlusal relationship were improved greatly. The occlusal plane was almost normal. There was no infection, malunion or permanent injury of inferior alveolar nerve.CONCLUSIONS:DO of the ramus is very effective for the correction of severe hemifacial microsomia. It has much better therapeutic effect than traditional orthognathic procedures.

Abstract:Background: Orthodontic and surgical technical advances in recent years have resulted in treatment opportunities for a whole range of craniofacial skeletal disorders either in the adolescent or adult patient. In the growing child, these can include myofunctional orthodontic appliance therapy or distraction osteogenesis procedures, while in the adult, the mainstay approach revolves around orthognathic surgery. The literature agrees that for a change in craniofacial morphology to remain stable, the muscles acting upon the facial skeleton must be capable of adaptation in their structure and, therefore, their function. Failure of the muscles to adapt to the change in their length or orientation will place undesirable forces on the muscle attachments leading to potential instability of the skeleton. Adaptation can occur through various processes including those within the neuromuscular feedback mechanism, through changes within muscle structure or through altered muscle physiology, and through changes at the muscle/bone interface. It is now accepted that because there is no single method of assessing masticatory function, several measures should be taken, and whenever possible, simultaneously. Methods: This investigation was designed to apply several, newly developed and more sophisticated methods of measuring muscle structure and function to a situation where adaptation of muscle is pivotal to the success of a therapeutic approach. Patients attending the combined orthodontic/orthognathic surgery clinic at the Clitrofa - Centro Médico, Dentário e Cirúrgico, in Trofa, Portugal, were screened. Ten patients scheduled for a bimaxillary osteotomy involving a combination of maxillary Le Fort I impaction procedure coupled with a sagittal split advancement of the mandible were selected to form the study group. The patients have MRI of the masseter muscle to evaluate the masseter muscle volume and fibre orientation changes. This exam was taken before surgery (T0), 6 to 12 months after surgery (T1), and 3 years after surgery (T2), by two independent observers, according to the protocol jointly developed between the Eastman Dental Institute - University of London and the MRI Centre - Department of Radiology at John Radcliffe Hospital - University of Oxford. Results: Significant differences (p < 0.05) have been identified between Time 0 (pre-op) and Time 1 (6-12 months post-op) regarding the masseter area (mm2). The differences against Time 0 (pre-op) seem to disappear at Time 2 (3 years post-op). Conclusions: MRI therefore seems to be a valid tool for measuring differences in the masseter muscle area and volume associated with high-severity occlusal deformities, although showing not to be as efficient in detecting the same differences in cases of low-severity occlusal deformities.

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>Focused on the measurement of the oropharynx and laryngopharynx, the aim of this study was to explore how mandibular distraction osteogenesis affects the morphological changes of the oropharynx and laryngopharynx.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Method</h3><p>We designed a retrospective study of airway parameters measurement in CFM patients undertaking the mandibular distraction osteogenesis treatment. The indication of the surgical treatment was the obvious facial asymmetry. Mimics 17.0(Materialise Inc. Belgium) was used to build 3D models of the oro- and laryngopharynx. The minimum cross-sectional area (CSA), mean-CSA, volume, length, and surface area were measured in the 3D models. All data were entered into SPSS v23.0 (SPSS Inc. USA), statistical differences pre-and postoperatively were assessed by using Wilcoxon test.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Thirty-two patients diagnosed with CFM were included in this study. Five were classified as type IIa,21 as type IIb and six as type III using the Pruzanksy-Kaban classification. The average age of the patients was 8.34 years old; 14 were females and 18 were males. The average traction time was 35.59 days and the average length of traction was 35.81 mm. The total volume of the airway was significantly increased after MDO (P=0.001). In the oropharynx and laryngopharynx segments, the mean-CSA, minimum CSA and surface area were significantly different in statistical results. From the results, the expansion of oropharynx and laryngopharynx were found after MDO.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>As one of surgery treatment, MDO could expand the volume of oropharynx and laryngopharynx in 3D CT models. It was found the increase in the mean-CSA, minimum CSA and surface area in oropharynx and laryngopharynx after MDO in measurement as well. Thus, the MDO is considered as a clinically useful treatment to change oropharynx and laryngopharynx structure.</p>

A Modified Protocol to Evaluate Masseter Morphology Based on Automatic Segmentation and Registration

Abstract:The purpose of the current study was to develop a modified protocol of evaluating the masseter morphology based on graph-cut automatic segmentation. The protocol was applied in the study on pre and postoperative masseter variation in patients receiving mandibular angle reduction. Computed tomography (CT) was used for the image data analysis. The masseter boundary was automatically segmentate on slices. The bony structure of mandible was segmented by the threshold restrain and three-dimensional (3D) reconstructed using the matching cubes algorithm. The pre- and postoperative images of the bony and masseter structures are overlapped in on coordinate system via the ICP registration. The masseter variation was reflected by the measures of the total volume and the 3D surface meshes through comparison of pre- vs. post-operative data. The average volume of the masseter was 32270.5±9354.1 mm <sup xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">3</sup> prior to the surgery, and decreased to 26376.89±7571.4 mm <sup xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">3</sup> and 26650.7±7179.3 mm <sup xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">3</sup> at 6 and 24 months after surgery, respectively. The variation on the cross section at 6 months after the surgery was 6.4±1.5 mm on the angular plane, 2.2±0.9 mm on the occlusion plan and 0.1±0.1 mm on the zygomatic plane. At 24 months after the surgery, the variation was 6.0±1.1 mm on the angular plane, 0.9±0.5mm on the occlusion plane and 0 mm on the zygomatic plane. The 3D color map of the meshes showed that the variation occurred mainly at angular and lower ramous area. The modified protocol was feasible for the evaluation of the masseter morphological variation. 3D comparison between the masseter meshes with representative color map is a convenient and time-saving method to evaluate the morphological variation after mandibular angle reduction.
[Application of Distraction Osteogenesis in Correction of Hemifacial Microsomia].

Abstract:OBJECTIVE:To study the effect of distraction osteogenesis of mandibular ramus in correcting hemifacial microsomia.METHODS:From December 1997 to March 2000 two kinds of mandibular distractors made in Germany and China were used in 11 cases with hemifacial microsomia aged 4.5 approximately 23 years (on average 14.1 years). Orthodontic treatment was given during the distraction. LeFort I osteotomy and genioplasty were performed in 5 cases and 4 cases respectively.RESULTS:The treated mandibular rami were lengthened 15 approximately 30 mm. No infection and dysosteogenesis was found. In only one case, the distractor failed to function at the third day of treatment and was replaced. The distractor was removed after 3 months while new bone was formed in all cases. All of the patients and their family members expressed their satisfaction with the results of treatment.CONCLUSION:Mandibular distraction osteogenesis is the most effective procedure for correcting hemifacial microsomia.
[The Treatment of Mandibular Micrognathia Secondary to Temporomandibular Joint Ankylosis with Distraction Osteogenesis].

Abstract:OBJECTIVE:To evaluate the clinical effect of distraction osteogenesis for patients with mandibular micrognathia secondary to temporomandibular joint (TMJ) ankylosis.METHODS:43 patients (aged from 2 to 61 years old) with mandibular micrognathia were treated with mandibular distraction osteogenesis. Two types of mechanical distraction were utilized in this study. Ten patients (age ranged from 2 to 16-years-old, mean age 7.6 years old) with severe micrognathia underwent bilateral mandibular distraction with rigid external distraction (RED) device. Other 33 patients were treated with unilateral(6 cases) or bilateral (27 cases) mandibular distraction using internal distraction device. Distraction was started on the 4th to 8th day after operation and distraction rate was 0.25 mm every time, four times a day. Distractor was removed after 3 to 6 months of consolidation period.RESULTS:Eighty sides of mandible in 43 patients were lengthened. The mean distraction distance was 23.2 mm (ranged from 14 to 35 mm). After distraction, the average posterior airway space (PAS) was enlarged from 4.9 mm to 10.4 mm and average angle of sella-nasion-point B (SNB) was increased from 64.2 degrees to 74.5 degrees. The apnea hypopnea index (AHI) was decrease significantly. The profile was improved and OSA was improved effectively in each patient. No complication occurred during treatment. No persistent numbness of lower lip was observed. All patients were satisfied with the results. After a mean follow-up period of 20.3 months(5 to 103 months) , the result was stable and no obvious relapse of micrognathia was observed.CONCLUSIONS:Distraction osteogenesis is an effective way in correction of mandibular micrognathia secondary to TMJ ankylosis. RED is a new method for treatment of children and adolescence with severe mandibular micrognathia. The procedure is simple and safe with stable result.
Distraction Osteotomy Combined With Orthodontics: An Effective Way to Decrease Short-term Recurrence in Treating Child Craniofacial Microsomia

Abstract:Background: This study aims to investigate strategies for reducing early relapse following mandibular distraction osteotomy (MDO) in craniofacial microsomia (CFM) patients. Methods: Forty CFM patients were divided into 2 groups: 20 underwent MDO alone, while 20 received MDO combined with orthodontic treatment (MDO-O). Preoperative, prefixation, and postfixation CT data in DICOM format were collected and reconstructed using Proplan software. Spatial geometric analysis was conducted to assess the inclination angle of the occlusal plane (OP), and OP deviation was directly measured using the software. Results: Patients in the MDO-O group demonstrated a statistically significant improvement in OP inclination at the end of the fixation period compared with those who underwent MDO alone ( P =0.003). Conclusions: Combining MDO with a bite plate and bite glue sticks in CFM patients during the mixed dentition period enhances OP outcomes, effectively reducing short-term recurrence.

Abstract:Background: The aim of this study was to present comprehensive skeletal, dental and facial aesthetic outcomes and long-term stability of maxillary anterior segmental distraction osteogenesis (MASDO) for the treatment of maxillary hypoplasia in CLP. Materials and methods: Six patients with maxillary hypoplasia treated with MASDO by a miniscrew assisted intraoral tooth-borne distractor were included. Cephalometric radiographs were obtained before distraction (T1), after the consolidation period (T2) and after orthodontic treatment or before orthognathic surgery (T3). Thirty-one cephalometric variables (12 skeletal, 9 dental, and 10 soft tissue variables) were used to evaluate changes in the dentofacial structures and the soft tissue profile. Friedman and Wilcoxon tests were applied to identify significant differences in hard and soft tissue changes during the T1-T2, T2-T3, and T1-T3 periods. Results: All patients successfully underwent MASDO without serious complications. From T1 to T2, forward movements of ANS and A (FH ⊥ N-A, VRL-ANS and VRL-A) were significant (p < 0.05). Significant increases in SNA and ANB were noted. Significant upward movement of points ANS (CFH-ANS) and A (CFH-A) was observed (p < 0.05). After distraction, a significant decrease in overjet and an increase in overbite were obtained (p < 0.05). Anterior tipping of the upper incisors (U1/ANS-PNS and U1/SN) was observed (p < 0.05). The soft tissue points of Pn, Sn, Ss, and ls showed significant anterior movement (p < 0.05). In addition, a significant increase in the nasolabial angle was measured (p < 0.05). All of the above data showed no statistically significant changes between T2 and T3 (p > 0.05). Conclusion: MASDO using a miniscrew assisted tooth-borne distractor presented significant maxillary advancement and favorable long-term stability in treating CLP patients with maxillary hypoplasia.
Mandibular Distraction Osteogenesis in Children and Adolescents: A Clinical and Radiographic Research Using Cone-Beam Computed Tomography

Abstract:Aim: The aim of this cross-sectional study was to evaluate, via cone-beam computed tomography, the long-term postoperative outcome in children treated with mandibular distraction osteogenesis. Materials and Methods: All young patients treated with mandibular distraction osteogenesis (MDO), during a 16-year period, at the University Department of Oral and Maxillofacial Surgery of a Pediatric Hospital, were recalled, and various clinical and radiographic parameters were recorded. Results: Eleven patients were included: 5 with hemifacial microsomia (HFM) and 6 with mandibular micrognathia. In all cases, MDO had been successful in regular follow-up and decannulation, soon after MDO, was achieved in all tracheostomy cases. The long-term result in cases of HFM was found stable, functionally and esthetically accepted, although less satisfactory than in regular follow-up; in micrognathia patients, relapse of different degrees was registered in 4 of 6 cases, without any need for tracheostomy though. Detailed and accurate information was obtained by cone-beam computed tomography (CBCT). The shape of the regenerated bone was irregular in HFM cases and relatively normal in the micrognathia cases. Quality of the regenerated bone was normal in all patients. The irregular shape registered in HFM cases did not compromise a safe orthognathic operation. Conclusions: Distraction osteogenesis remains an early treatment choice in cases of mandibular deformities. Long-term findings showed that there is a degree of relapse with growth, which was more obvious in mandibular micrognathia cases. Computed tomography contributes to detailed evaluation of changes at the distraction site.

Abstract:The evidence surveyed by Blair and Connelly (1996) now provides a clearly outlined challenge for action in applying physical activity for health promotion. Strategies to choose are single risk or broad spectrum within an individual or community approach. The 20-year results of the broad spectrum, community intervention in North Karelia, Finland more than halved the coronary mortality of participants aged 35-64 years (Puska, Tuomilehto, Vartiainen, Korhonen, & Torppa, 1995). Many facets of human lives, including physical activity, were charted in another project: the 11-year follow-up of Finnish municipal employees aged 45-58 years at entry into the project (Tuomi, 1991). In this project, physical activity showed marked positive associations with work capacity, lifestyle, and well-being. Thus, in addition to reducing morbidity and mortality, physical activity and fitness also promote mental and social health and improve the quality of life.

Abstract:OBJECTIVE:To study the effects of unilateral mandibular distraction osteogenesis(MDO) on the temporomandibular joint(TMJ).METHODS:Ten rhesus monkeys were used to perform right MDO. After 5 days, latency period, the distractor was initiated at the rate of 0.5 mmx2/day, totally 15 days. When MDO completed, and 2,4,6,and 12 weeks after MDO, X-ray films of bilateral TMJ were taken and one group of animals was sacrificed. Bilateral TMJ were obtained and paraffin slices were made HE and immunohistochemistry stains of TGF-beta1 were done on the slices.RESULTS:After MDO, the monkeys' mandible midline obviously deflected to the left side. The front teeth had open bite and reverse bite. The right molars were severely open bite and the left molars were reverse bite. From the X-ray film, we can see that the right condylar process moved backward and upward in the acetabulum, while the left condylar process moved forward and downward. On the slices, both sides of the TMJ showed different degrees of degeneration and proliferation, but the changes on the right side were more severe and obvious than the left side, and had involved all the cartilage's layers and the bone tissues under the cartilage. In some animals, there even appeared cartilage erosion and light histological changes of acetabulum. During the consolidation period, the open bite gradually closed, and the condylar process gradually moved to normal position. Twelve weeks after MDO, the open bite at the right side disappeared, but the front teeth open bite and reverse bite still existed. The right condylar process still lay a little backward and had some degree of degeneration. The left TMJ had the normal position and histology except for cartilage hypertrophy layer thickness. At different times during consolidation period, the positive stains of TGF-beta1 in condylar process on both sides were all more obvious than that of the control group.CONCLUSION:MDO can cause TMJ position change and light histological degeneration, but these changes can gradually rehabilitate in the future. TGF-beta1 may play a role in the reconstructive process of TMJ degeneration.

Abstract:Background Facial esthetics is a major concern of orthodontic patients. This study aims to evaluate orthodontic treatment-related thickness changes of the masseter muscles and surrounding soft tissues and the potential factors that would influence these changes during orthodontic treatment in female adults. Methods Forty-two female adult patients were included in this retrospective study and were divided into extraction ( n = 22) and nonextraction ( n = 20) groups. Pretreatment and posttreatment cone-beam computed tomography (CBCT) images were superimposed and reconstructed. The thickness changes of the masseter area of facial soft tissue (MAS), masseter muscles (MM) and surrounding fat tissue (FT) were measured. Pretreatment age, treatment duration, sagittal relationship (ANB), and vertical relationship (Frankfort-mandibular plane angle, FMA)-related MAS, MM and FT changes were compared between extraction and nonextraction groups. Spearman’s correlation coefficient was calculated between the above variables. Regression analysis was conducted to confirm the causal relations of the variables. Results The thickness of MAS and MM significantly decreased in both groups, with larger decreases (> 1 mm) in the extraction group. There were strong correlations ( r > 0.7) between the thickness decrease in MAS and MM in both groups and moderate correlations ( r > 0.4) between MAS and FT in the nonextraction group. A significantly greater decrease of MAS and MM were found to be moderately correlated with a smaller FMA ( r > 0.4) in the extraction group. Scatter plots and regression analysis confirmed these correlations. Conclusions Masseter muscles and the surrounding soft tissue exhibited a significant decrease in thickness during orthodontic treatment in female adults. Low-angle patients experienced a greater decrease in soft tissue thickness in the masseter area in the extraction case. But the thickness changes were clinically very small in most patients.
