Abstract:Cardiopulmonary bypass (CPB) is frequently employed for cardiac surgery, and selecting a suitable priming fluid is a prerequisite for CPB. Currently, the commonly used priming fluids in clinics are classified as crystalloids and colloids, including balanced crystalloids, albumin, dextran, gelatin and hydroxyethyl starch (HES). This network meta-analysis compared the effects of eight fluids used during CPB in adults to determine optimal priming fluid during CPB surgery. Randomised controlled trials assessing priming fluids for CPB in adult cardiac surgery published before 13 April 2023 were searched across Ovid MEDLINE(R) ALL, OVID EMbase, and Cochrane Central Register of Controlled Trials. Various priming fluids were classified into eight categories, including balanced crystalloids, 0.9% NaCl, iso-oncotic human albumin, hyperoncotic human albumin, HES with molecular weight 130k, HES with molecular weight 200k, gelatin and dextran. The NMA of platelet counts revealed no significant differences in any result. In direct comparison results, only the comparison of HES with molecular weight 130k vs. gelatin (standard mean difference = −0.40, 95% confidence interval [95%CI: −0.63, −0.16) revealed a significant difference. According to the SUCRA, balanced crystalloids had the highest platelet count, followed by gelatin, and HES with a molecular weight of 130k had the lowest platelet, followed by HES with a molecular weight of 200k. Patients using dextran have a low mortality rate and a short mean CPB time, the use of balanced crystalloids is beneficial in terms of platelet count, and HES with molecular weight 130k is beneficial for postoperative urine volume at 24h. However, all priming fluids have pros and cons quite, and the optimal choice of priming fluids remains unsupported by current evidences. When performing CPB surgery, the type of priming fluid should be selected according to the actual situation in CPB for adult cardiac surgery. When dextran was used as the CPB priming fluid, patients had the lowest mortality and shortest mean CPB time. With iso-oncotic HA, patients had the shortest length of ICU stay, the least blood loss 24h after surgery, and the lowest chest tube output 24h after surgery. The use of balanced crystalloids was beneficial for platelet count, the use of L-HES was beneficial for urine output 24h after surgery, and the use of H-HES resulted in the shortest hospital stay. In summary, each of these fluids has pros and cons quite, and an optimal choice of priming fluids during CPB surgery remains unsupported by current evidence. When performing CPB surgery, the type of priming fluids should be selected according to the actual condition of the patient's body.

Effects of Hydroxyethyl Starch 130/0.4 on Pulmonary Capillary Leakage and Cytokines Production and NF-κB Activation in CLP-Induced Sepsis in Rats

Abstract:Sepsis and resulting multiple system organ failure are the leading causes of mortality in intensive care units. Hydroxyethyl starch (HES) 130/0.4 was a novel preparation, developed to improve the pharmacokinetics of current medium molecular weight HES solutions. This study was designed to explore the effects of HES 130/0.4 on pulmonary capillary permeability (PCP), production of cytokines, and activation of transcription factor in septic rats induced by cecal ligation and puncture (CLP).Adult male Sprague Dawley rats were randomly divided into six groups (six rats/group): saline controls (30 ml/kg); CLP plus saline (30 ml/kg); CLP plus HES (7.5, 15, or 30 ml/kg, respectively), and HES alone (30 ml/kg). Mean arterial blood pressure and heart rate were monitored during the experiment process. Myeloperoxidase (MPO) activity, wet/dry weight ratio, PCP, tumor necrosis factor alpha (TNF-alpha), interleukin (IL)-6, IL-10, and nuclear factor-kappa B (NF-kappaB) were investigated at 6 h.We demonstrated that CLP could provoke significant injury in lung, characterized by increase in PCP, wet/dry weight ratio, MPO activity, TNF-alpha, IL-6, and IL-10 level, and NF-kappaB activation. Without obvious influence on systemic macro-hemodynamics, HES 15 ml/kg and 30 ml/kg significantly reduced CLP-induced elevation of pulmonary capillary permeability, wet/dry weight ratio, and production of IL-6. Meanwhile, HES 15 ml/kg increased IL-10 level and HES 7.5, 15, and 30 ml/kg suppressed MPO activity, TNF-alpha level, and NF-kappaB activation.HES 130/0.4 can inhibit CLP-induced PCP by attenuating pulmonary inflammation and NF-kappaB activation in vivo.

Abstract:OBJECTIVE:To investigate the relationship between phospholipase D (PLD) activation and neutrophil priming induced by cardiopulmonary bypass (CPB), and try to clarify whether CPB-induced systemic inflammatory response can be attenuated by inhibiting neutrophilic PLD activation.METHODS:Neutrophils were isolated from arterial blood of 8 patients undergoing valve replacement before operation and 30 min after initiation of CPB respectively. Both the preoperative and CPB-stirred neutrophils were subdivided into 5 groups by receiving different experimental interventions: (1) bacterial lipopolysaccharide (LPS, 10 ng x ml(-1)), (2) N-formylmethionylphenylalanine (fMLP, 1 micromol x L(-1)), (3) LPS+fMLP, (4) 1-butanol (0.5%)+LPS+fMLP, (5) vehicle. Elastase and myeloperoxidase (MPO) release was measured for the parameters of neutrophil activation, neutrophil PLD activity was determined by quantitation of choline produced from the stable product of phosphatidylcholine catalyzed by PLD.RESULTS:(1) Preoperative neutrophils treated with LPS+fMLP presented significantly higher PLD activity (13.48+/-2.61 nmol choline x h(-1) x mg(-1)) and released more elastase and MPO than cells treated with vehicle (PLD activity 3.70+/-0.49 nmol choline x h(-1) x mg(-1)), P<0.01), LPS (P<0.01) and fMLP respectively. In 1-butanol+LPS+fMLP group, PLD activity of preoperative neutrophils was lower than that in LPS+fMLP group (P<0.01), besides the release of elastase and MPO decreased sharply below both LPS+fMLP and fMLP groups (P<0.01). In LPS group, PLD activity was higher (P<0.01), while elastase and MPO release did not differ from control. fMLP group presented PLD activity, elastase and MPO release higher than control (P<0.01); nevertheless, lower than LPS+fMLP group (P<0.01). (2) CPB-stirred neutrophils presented prominent PLD activity increment, and even the control level was 3.59-fold of the pre-operative control (P<0.01). PLD activity in LPS+fMLP group was higher than that in other groups. Notably, PLD activity was even nonstatistically lower in 1-butanol+LPS+fMLP group than that in LPS or fMLP group. CPB-stirred neutrophils in LPS+fMLP group released more elastase and MPO than control, LPS, and 1-butanol+LPS+fMLP groups did (P<0.01); however, neither of the release was statistically different from that of fMLP group.CONCLUSIONS:Cardiopulmonary bypass enables neutrophil priming accompanied with significant increase in PLD activity. Inhibition of neutrophil PLD activation attenuates its priming and may alleviate CPB-induced systemic inflammatory reaction.

Abstract:Objective To explore the efficiency and safety of 4％ succinylated gelatin as priming solution instead of fresh frozen plasma during cardiopulmonary bypass for cyanotic heart diseases in children.Methods A total of eighty cyanotic infant patients undergoing heart surgery with extracorporeal circulation (ECC) were randomly divided into two groups (n =40 each group).Either 4％ succinylated gelatin (gelatin group) or fresh frozen plasma (plasma group) was primed for ECC respectively.Rapid-thromboelastography (r-TEG) was performed after anesthesia induction and protamine administration.The postoperative parameters of renohepatic function,bleeding volume,blood product dosage and clinical recovery time were recorded.And the association between characteristics,hemostatic variables and postoperative blood loss volume were also analyzed.Results No patient developed obvious renal or liver dysfunction.Compared with gelatin group (1 355 ± 338 mg/L),plasma group showed a significantly elevated level of fibrinogen (1 672 ± 390 mg/L) (P =0.012).However,there was no clinical efficiency of adding plasma in ECC circuit since bleeding volume(21.5 ± 8.8 vs 22.9 ± 8.5,P =0.371),blood product dosage(PLT 1.0 u vs 1.0 u P =0.253;RBC 1.0 u vs 1.5 u,P =0.483 ; FFP 33.9 ± 10.9 ml/kg vs 35.4 ± 11.8 ml/kg,P =0.268) and clinical recovery time(ventilation time 39.3 h vs 37.5 h,P =0.256; ICU stay 61.0 vs 60.5 h,P =0.264;hospitalization time 14 vs 14 d,P =0.430) were comparable between two groups.The maximal amplitude (MA) after bypass was correlated with early blood loss volume after surgery.Conclusions Replacement of plasma with artificial colloid-like gelatinin for ECC circuit priming is both safe and effective for infants with congenital heart disease.And r-TEG may be used as a tool for evaluating hemostatic function after heart surgery in infant patients.
Impact of Crystalloid or Albumin Priming of the Heart-Lung Machine on Inhospital Outcome after Coronary Artery Bypass Surgery

Abstract:Background: Crystalloid priming is a cost-effective, free from immunological reactions, and independent from human plasma delivery. However, there is some debate on the negative impact of low plasma colloid pressure and higher incidence of systemic inflammatory response syndrome (SIRS). The aim of the study was to rule out any adverse effects of crystalloid priming on the postoperative outcome. Methods: We investigated 520 consecutive patients, including emergencies, who had isolated on-pump coronary artery bypass grafting in 2009 by retrospective analysis in our clinic. Crystalloid priming (n = 294) was introduced as an alternative to albumin (n = 226). Reviewing patient charts and IT-based data generated a dataset of perioperative parameters. Results: There were no differences with respect to demographical data and preexisting comorbidities between both groups. Despite equal perfusion times, more volume had to be substituted during extracorporeal circulation following crystalloid priming. However, this did not influence the inhospital outcomes. According to the definition of the "Sepsis-3 Guidelines," the incidence of SIRS was similar. There was no difference in the need for a vasopressor treatment, and only transient higher serum lactate levels were found in the crystalloid group. The incidence of neurologic and organ-related adverse events, as well as 30-day mortality was comparable. Conclusion: The use of crystalloid priming is safe in coronary artery bypass grafting surgery in adults. However, there might be a greater need for crystalloid fluids during surgery.

Abstract:Acute microcirculatory perfusion disturbances and organ edema are important factors leading to organ dysfunction during cardiac surgery with cardiopulmonary bypass (CPB). Priming of the CPB system with crystalloid or colloid fluids, which inevitably leads to hemodilution, could contribute to this effect. However, there is yet no optimal evidence-based strategy for this type of priming. Hence, we will investigate different priming strategies to reduce hemodilution and preserve microcirculatory perfusion.

Abstract:OBJECTIVES:To assess the efficacy and safety of albumin as pump priming fluid in cardiac surgery.DESIGN:Meta-analysis of randomized controlled trials.SETTING:Each study was conducted in a surgical center or intensive care unit.PARTICIPANTS:Adult and pediatric patients undergoing cardiac surgery with cardiopulmonary bypass who received circuit priming fluids.INTERVENTIONS:Extracorporeal circuit priming with either albumin or crystalloid.MEASUREMENTS AND RESULTS:Fourteen eligible randomized controlled trials with 741 patients were included in the present meta-analysis. Albumin prime had lower bleeding (CI -202.20 to -142.88 mL, p < 0.00001) and showed a greater advantage in preserving platelet counts (CI 14.85-21.48 × 103 mm-3, p < 0.00001), maintaining colloid osmotic pressure and sustaining negative fluid balance. No significant differences were found in the remaining study outcomes.CONCLUSIONS:Albumin was shown to be safe and efficacious in extracorporeal circulation perfusion. However, its clinical advantages were not clearly highlighted, as there were no significant differences in the number of deaths, length of hospital stay, or intensive care unit duration. The results should be interpreted cautiously, as most included studies were small in scale, and the total number of participants was limited.

Abstract:Effects on platelet number and function are a central concern in choice of priming uid on congenital cardiac patients. Aim to compare between albumin and crystalloid with FFPas priming solution in cardiopulmonary bypass for paediatric congenital cyanotic heart disease patients (CHD). A total of 66 CHD paediatric patients were divided into two groups to received either albumin or crystalloid solution +FFP as priming uid. The primary endpoint was assessment of haemoglobin, platelet count and urine output immediately before induction and after cessation of CPB. Secondary endpoints considered blood loss and consumption of blood products in post operative 24 hrs period. Albumin 115151.52±29487.41: Gr.Cr +FFP 139393±23040.64; p 0.000), post CBP urine(Gr.Albumin 869.94±282.67:Gr.Cr+FFP 1051.67±235.48;p 0.006), left drain collection,(Gr albumin 25.30±4.49: Gr Cr 43.03± 7.88;p0.05) right drain collection Gr Albumin 15.15±3.36:Gr Cr 38.33± 7.01; p 0.005), PRBC transfused (Gr Albumin 50.91±13.77:Gr Cr+FFP 114.27±19.05 ;p0.009), and platelet transfusion(Gr Albumin 75.8%:Gr Cr+FFP 24.2%; p 0.003) . Albumin in the prime may attenuate the extravasations of uid out of the vascular space, Moreover systemic inammatory response associated trans-capillary leakage of plasma proteins is possible to prevent by increase serum albumin levels. To conclude albumin as priming solution revealed better than crystalloid +FFPin cardiopulmonary bypass surgery in paediatric age group.

Abstract:Background: In cardiac surgery with cardiopulmonary bypass (CPB), large amounts of fluids are administered. CPB priming with crystalloid solution causes marked hemodilution and fluid extravasation. Colloid solutions may reduce fluid overload because they have a better volume expansion effect than crystalloids. The European Medicines Agency does not recommend the use of hydroxyethyl starch solutions (HES) due to harmful renal effects. Albumin solution does not impair blood coagulation but the findings on kidney function are conflicting. On the other hand, albumin may reduce endothelial glycocalyx destruction and decrease platelet count during CPB. No large randomized, double-blind, clinical trials have compared albumin solution to crystalloid solution in cardiac surgery. Methods/design: In this single-center, double-blind, randomized controlled trial comprising 1386 adult cardiac surgery patients, 4% albumin solution will be compared to Ringer's acetate solution in CPB priming and volume replacement up to 3200 mL during surgery and the first 24 h of intensive care unit stay. The primary efficacy outcome is the number of patients with at least one major adverse event (MAE) during 90 postoperative days (all-cause death, acute myocardial injury, acute heart failure or low output syndrome, resternotomy, stroke, major arrhythmia, major bleeding, infection compromising post-procedural rehabilitation, acute kidney injury). Secondary outcomes are total number of MAEs, incidence of major adverse cardiac events (MACE; cardiac death, acute myocardial injury, acute heart failure, arrhythmia), amount of each type of blood product transfused (red blood cells, fresh frozen plasma, platelets), total fluid balance at the end of the intervention period, total measured blood loss, development of acute kidney injury, days alive without mechanical ventilation in 90 days, days alive outside intensive care unit at 90 days, days alive at home at 90 days, and 90-day mortality. Discussion: The findings of this study will provide new evidence regarding efficacy and safety of albumin solution in adult patients undergoing cardiac surgery with CPB. Comparison of Erythromycin Stearate and Oxytetracycline in the Treatment of Nongonococcal Urethritis: Their Efficacy against Chlamydia Trachomatis

Abstract:The effectiveness of Oxytetracycline 250 mg. 6 hourly for 2 weeks and erythromycin stearate 500 mg. 12 hourly for 2 weeks in the treatment of non-gonococcal urethritis (NGU) have been compared; cell culture for Chlamydia trachomatis was performed before and after treatment. There was no significant difference between the results obtained with the two antimicrobials. Both were clinically effective in the treatment of Chlamydia-positive NGU. Response to therapy was rapid; 2 weeks after treatment began, only 5 (14%) of 35 men treated with Oxytetracycline and 4 (13%) of 30 men treated with erythromycin stearate still had urethritis. Clinical improvement was accompanied in the majority of cases by failure to re-isolate C. trachomatis, but a small number of men who had not been exposed to the risk of reinfection yielded chlamydiae after therapy with Oxytetracycline or erythromycin stearate. The results of treatment of men with NGU from whom C. trachomatis had not been isolated were similar to those obtained in isolation-positive patients; again, there was no significant difference in the efficacy of the two antimicrobials. It is concluded that erythromycin stearate is a useful alternative to tetracyclines in the treatment of NGU.
The use of mannitol in cardiopulmonary bypass prime solution—Prospective randomized double‐blind clinical trial

Abstract:BackgroundThe optimal prime solution for the cardiopulmonary bypass (CPB) circuit in adult cardiac surgery has not yet been defined. Mannitol is widely used in the priming solution for CPB despite the fact that there is no clear consensus on the role of mannitol in cardiac surgery. The aim of this study was to investigate the effect of mannitol in the CPB prime solution. MethodsThis prospective, randomized, double‐blind study included forty patients with normal cardiac and renal functions, who underwent coronary artery bypass grafting. One group received a prime based on Ringer's acetate (n=20), and the other a prime consisting of Ringer's acetate with 200 mL mannitol (n=20). Changes in osmolality, acid‐base status, electrolytes and renal‐ related parameters were monitored. ResultsNo significant differences were found in osmolality between the Ringer's acetate group and the mannitol group at any time. The mannitol group showed a pronounced decrease in sodium, from 138.7±2.8 mmol/L at anesthesia onset, to 133.9±2.6 mmol/L after the start of CPB (p <0.001). No differences were seen in the renal parameters between the groups, apart from a short‐term effect of mannitol on peroperative urine production (p = 0.003). ConclusionWe observed no effects on osmolality of a prime solution containing mannitol compared to Ringer's acetate based prime in patients with normal cardiac and renal function. Influence of Different Volume Replacement Strategies on Systemic Inflammatory Response Syndrome after Cardiopulmonary Bypass

Abstract:Objective To compare the effect of hydroxyethylstarch(HES 130/0.4)on systemic inflammatory response syndrome(SIRS)after cardiopulmonary bypass(CPB)with gelofusine(MFG) in patients undergoing coronary artery bypass grafting(CABG). Methods Twenty-three patients scheduled for selective CABG were randomized to receive either HES 130/0.4 (n=12,groupⅠ) or MFG (group Ⅱ,n=11) as prime solution and volume replacement. Fluid therapy was given perioperatively and continued for 48 h after operation. The incidence of SIRS was recorded. The serum concentrations of C-reactive protein(CRP),interleukin (IL)-6,P-selectin and intercellular adhesion molecule-1 (ICAM-1) were measured after induction of anesthesia,at the end of CPB,as well as 2 h,24 h and 48 h after the end of CPB for both groups. Results Biometric and perioperative data of two groups were similar. There was no marked difference in the number of patients suffering from SIRS. At 24 h and 48 h after CPB,CRP levels were increased in two groups,but the difference was statistically significant only at 48 h between the two groups (P0.05). The concentration of P-selection rose in two groups at the end of CPB and continued to increase during the first 2 postoperative days without significant difference between the two groups. Serum concentrations of ICAM-1 were significantly higher in two groups at 24 h and 48 h after the end of CPB than those after induction,particularly at 48 h in group Ⅱ. Plasma IL-6 levels were elevated above the preoperative levels and there was not statistically significant between the two groups. Conclusion Compared to MFG,volume replacement with HES 130/0.4 may reduce the inflammatory response in patients undergoing CABG surgery,resulting from reduced endothelial activation and endothelial damage induced by HES 130/0.4.

Abstract:This study aimed to evaluate the effects of washed cardiopulmonary (CPB) circuit residual blood reinfusion on the postoperative clinical outcome for pediatric patients undergoing cardiac surgery. A total of 309 consecutive Chinese cardiac patients receiving CPB between October 2010 and April 2011 were prospectively analyzed. For 217 patients, CPB circuit residual blood was reinfused after the cell-saving procedure [cell-salvage group (CS)]. The remaining 92 patients were directly transfused with allogenic red blood cells (RBCs) after their operation [control group (CON)]. Assessment included perioperative transfusion of RBCs, postoperative hematocrit (HCT), chest tube drainage during the first 24 h after the operation, intrahospital mortality, respiratory morbidity, and renal dysfunction. The two groups were well matched in terms of demographics, CPB data, and complexity of surgical procedure. The patients in the CS group had a significantly higher HCT level postoperatively (p = 0.018) and a less allogenic RBCs transfusion (p = 0.000). The two groups did not differ in terms of chest tube drainage during the first 24 h postoperatively, intrahospital mortality, or respiratory morbidity. The incidence of serum creatinine (≥ 2-folds) during the first 72 h after the operation was significantly lower in the CS group (2.3 %) than in the CON group (8.7 %) (p = 0.010). Reinfusion of washed CPB circuit residual blood significantly raised the postoperative HCT level, reduced the allogeneic blood transfusion, decreased the incidence of early postoperative renal dysfunction, and did not increase the chest tube drainage after the operation in pediatric cardiac surgery.
Reducing the prime cardiopulmonary bypass volume during paediatric cardiac surgery

Abstract:Perfusion, Ahead of Print. IntroductionDespite technological advances, the use of homologous blood to prime the cardiopulmonary bypass (CPB) circuits of infants under 10 kg remains common. However, such rapid massive transfusion may increase post-CPB morbidity.MethodWe retrospectively included consecutive patients weighing 2.3–10 kg who underwent cardiac surgery under CPB. Patients were divided into two groups based on their priming volumes: low priming volume (LPV) (below the median volume) or high priming volume (HPV) (the median volume or above).ResultsThe study included 208 patients, of whom 104 had priming volumes below the median [37.9 (28.4–51.7) mL/kg] and 104 had at least the median volume. We recorded positive correlations between the priming volume, on the one hand, and the peak creatinine and CRP levels within 5 days postoperatively, the duration of intensive care unit (ICU) stay, and the mechanical ventilation time, on the other. A relationship was also observed between a higher median priming volume and the need for renal replacement therapy in the ICU and mediastinitis.ConclusionAlthough the differences in priming volume between the twogroups were small, they significantly influenced the postoperative complications. Perfusionists should seek to limit the priming volume to reduce the post-CPB inflammatory response, the duration of ICU stay, and possibly the risk of mediastinitis.

Perioperative fluid management for adult cardiac surgery: network meta-analysis pooling on twenty randomised controlled trials

Abstract:Background: The aim of this study was to evaluate colloids and crystalloids used in perioperative fluid therapy for cardiac surgery patients to further investigate the optimal management strategies of different solutions. Method: RCTs about adult surgical patients allocated to receive perioperative fluid therapy for electronic databases, including Ovid MEDLINE, EMBase, and Cochrane Central Register of Controlled Trials, were searched up to February 15, 2023. Results: None of the results based on network comparisons, including mortality, transfuse PLA, postoperative chest tube output over the first 24 h following surgery, and length of hospital stay, were statistically significant. Due to the small number of included studies, the results, including acute kidney injury, serum creatinine, serum microglobulin, and blood urea nitrogen, are from the direct comparison. For transfusion of RBCs, significant differences were observed in the comparisons of 3% gelatine vs. 6% HES 200/0.5, 4% albumin vs. 5% albumin, 4% gelatine vs. 5% albumin, 5% albumin vs. 6% HES 200/0.5, and 6% HES 130/0.4 vs. 6% HES 200/0.5. In transfusion of FFP, significant differences were observed in comparisons of 3% gelatine vs. 4% gelatine, 3% gelatine vs. 6% HES 200/0.5, 5% albumin vs. 6% HES 200/0.5, 4% gelatine vs. 5% albumin, 4% gelatine vs. 6% HES 200/0.4, and 6% HES 130/0.4 vs. 6% HES 200/0.5. For urinary output at 24 h after surgery, the results are deposited in the main text. Conclusion: This study showed that 3% gelatin and 5% albumin can reduce the transfuse RBC and FFP. In addition, the use of hypertonic saline solution can increase urine output, and 5% albumin and 6% HES can shorten the length of ICU stay. However, none of the perioperative fluids showed an objective advantage in various outcomes, including mortality, transfuse PLA, postoperative chest tube output over the first 24 h following surgery, and length of hospital stay. The reliable and sufficient evidences on the injury of the kidney, including acute kidney injury, serum creatinine, serum microglobulin, and blood urea nitrogen, was still lacking. In general, perioperative fluids had advantages and disadvantages, and there were no evidences to support the recommendation of the optimal perioperative fluid for cardiac surgery.
Solar Cycle Variability of the Thermospheres of Mars and Earth

Abstract:This paper comparatively examines long-term variations in the thermospheres of Mars and Earth, from the solar maximum of 2002-2003 to the deep solar minimum of 2009-2010. Data employed include densities and exosphere temperatures derived from orbital drag analyses of the Mars Odyssey (MO) satellite for Mars, and accelerometer measurements on the CHAMP satellite for Earth. Derivation of thermosphere densities are relatively straightforward for Earth, but the Mars thermosphere data require precise orbit determination techniques and separation of such factors as solar radiation pressure and various nuances of the gravity field, as well as accounting for possible density variations connected with dust storms. Delineation of solar cycle variability in planetary thermospheres serves the important role of constraining energetics balances in first-principles general circulation models [1,2,3].

Abstract:Purpose To investigate the effects and mechanisms of methylprednisolone(MP) on alveolar-capillary permeability after hypothermic cardiopulmonary bypass(CPB). Methods Fifteen dogs were randomly divided into 3 groups.Group A was the control group without CPB;group B and group C were subjected to cardiopulmonary bypass and crossclamping the aorta for 90 minutes.After weaning from cardiopulmonary bypass,they were allowed to survive till 90 min after aortic declamping.MP(group C) or saline(group B) was injected intravenously before surgery.Intraoperative and postoperative hemodynamic studies and lung parameters were recorded.Plasma levels of MMP-9 were measured at the following time points: before MP or saline was administered,aortic declamping,30 min and 90 min after aortic declamping.Immediately after the animals were killed,the left lungs were obtained from 15 dogs and mounted in isolated perfused lung chambers for filtration coefficient(K_f) assessment. Results Compared with group B,there was a lower PVR and a higher oxygenation index(PaO_2/FiO_2) in group C after aortic declamping(P<0.05).After CPB there was a significant elevation in plasma levels of MMP-9,compared with those in group B,MP treatments were associated with lower plasma MMP-9.From isolated lung study,we found statistically significant changes in K_f between groups.In addition,statistically significant positive correlation was found between postbypass peak MMP-9 levels and lung K_f(P<0.05),and statistically significant negative correlation was found between MMP-9 and postbypass oxygenation indices. Conclusions(1) Hypothermic CPB could increase blood levels of MMP-9,and increase alveolar-capillary permeability.(2) MP of 5mg/kg could effectively reduce the release of MMP-9,and improve alveolar-capillary permeability.
Applications of Ultrafiltration of Priming Solution with Blood in Pediatric Cardiac Surgery

Abstract:Objective To discuss whether priming solution with blood can reach a physiologic state after ultrafiltration and investigate the influence of this method on electrolytes,acid-base balance and cardiopulmonary function in patients in perioperative period. Methods Forty patients with congenital heart diseases treated in our hospital from February to June 2009 were enrolled in this study.The weight of these patients was less than 8 kg.They were randomly divided into two groups: the experimental group(n=20,12 males and 8 females;age: 131.00±103.00 d;weight: 4.14±0.96 kg) and the control group(n=20,11 males and 9 females;age: 127.00±88.00 d;weight: 4.38±1.05 kg).For patients in the experimental group,the priming solution was filtered with a blood ultrafilter in the cardiopulmonary bypass(CPB) circuit for twenty minutes,while the priming solution circulated in the CPB circuit without filtration for patients in the control group before operation.Data were obtained from the priming solution before and after ultrafiltration.Blood gas analysis was done and indexes like the electrolytes were detected during the operation.Fractional shorting(FS),ejection fraction(EF) and cardiac output(CO) were measured by echocardiography.Pulmonary function change was also observed.Results No death occurred in both groups.Mechanical ventilation time for the experimental group was significantly shorter than that of the control group(2.7±0.3 d vs.4.1±0.4 d,P<0.05).After ultrafiltration for the experimental group,all indexes of the priming solution reached normal values: pH increased(from 6.89±0.22 to 7.40±0.57,P=0.001);base excess increased(from-16.12±0.98 mmol/L to +0.31±2.40 mmol/L,P=0.000);potassium concentration decreased(from 10.33±2.13 mmol/L to 4.27±0.93 mmol/L,P=0.000);interleukin-8(IL-8) decreased(from 78.40±6.10 pg/ml to 64.30±48.10 pg/ml,P=0.036);and bradykinin decreased(from 5 982±1 353 pg/ml to 531.00±35.00 pg/ml,P=0.031).The decrease of FS,EF and CO in the experimental group was smaller than that of the control group.Four hours after surgery,CO in the experimental group was significantly higher than that in the control group(2.77±0.95 L/min vs.1.66±0.75 L/min,P=0.001).Twenty-four hours after surgery,EF in the experimental group was significantly higher than that in the control group(67.44%±6.89% vs.61.17%±9.02%,P=0.003).Six hours after surgery,alveolar-arterial oxygen difference(A-aDO2) and respiratory index(RI) increased significantly(P<0.05) in both groups,and then started to drop to normal.Patients in the experimental group recovered gas exchange earlier than patients in the control group.Forty-eight hours after operation,A-aDO2 and RI in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion Ultrafiltration of priming solution may confer an advantage in maintaining more physiological conditions,reducing inflammatory mediators, and improving cardiopulmonary function after operation,which is very important in performing cardiac surgery on the new-born and infants with complex congenital heart diseases.

Abstract:OBJECTIVE To investigate the safety of hydroxyethyl starch(HES) 130/0.4 in patients undergoing coronary artery bypass grafting(CABG),and to compare its efficacy on systemic inflammatory response syndrome(SIRS) after extracorporeal circulation(ECC) with Gelofusine.METHODS Twenty-four patients scheduled for elective CABG were randomized to receive either HES 130/0.4(group Ⅰ,n=12) or Gelofusine(group Ⅱ,n=11) as prime solution and volume replacement.Fluid therapy was given perioperatively and continued for 48 h postoperation.The incidence of SIRS,postoperative clinical parameters and blood biochemistry were recorded. The serum concentrations of C-reactive protein(CRP),interleukin(IL)-6,P-selection and intercellular adhesion molecule-1(ICAM-1) were measured after induction of anesthesia,at the end of ECC,as well as 2 hours,24 hours and 48 hours after the end of ECC for both groups.RESULTS Liver enzymes in group Ⅰ were significantly higher than that in group Ⅱ(P<0.05).There was no difference in the number of patients suffering from SIRS.Twenty-four and 48h after ECC,the CRP levels were increased in two groups,but the difference was statistically significant only at 48h between the two groups(P<0.05).Serum concentrations of ICAM-1 were significantly higher in two groups at 24 hours and 48h after the end of ECC.CONCLUSION HES 130/0.4 can be safely used in patients undergoing CABG,although caution should emphasized on its potential liver damage.Compared to Gelofusine,volume replacement with HES 130/0.4 may reduce the inflammatory response in patients undergoing CABG surgery,because of reducing endothelial activation and endothelial damage induced by HES 130/0.4.
