Abstract:Purpose Several techniques for arthroscopic repair of rotator cuff defects have been introduced over the past years. Besides established techniques such as single-row repairs, new techniques such as double-row reconstructions have gained increasing interest. The present article therefore provides an overview of the currently available literature on both repair techniques with respect to several anatomical, biomechanical, clinical and structural endpoints. Methods Systematic literature review of biomechanical, clinical and radiographic studies investigating or comparing single- and double-row techniques. These results were evaluated and compared to provide an overview on benefits and drawbacks of the respective repair type. Results Reconstructions of the tendon-to-bone unit for full-thickness tears in either single- or double-row technique differ with respect to several endpoints. Double-row repair techniques provide more anatomical reconstructions of the footprint and superior initial biomechanical characteristics when compared to single-row repair. With regard to clinical results, no significant differences were found while radiological data suggest a better structural tendon integrity following double-row fixation. Conclusion Presently published clinical studies cannot emphasize a clearly superior technique at this time. Available biomechanical studies are in favour of double-row repair. Radiographic studies suggest a beneficial effect of double-row reconstruction on structural integrity of the reattached tendon or reduced recurrent defect rates, respectively.
Editorial Commentary: Bridging Graft Techniques Improve Short-Term Outcomes for Irreparable Rotator Cuff Tear Repair

Abstract:There is a growing need for nonarthroplasty treatment options for irreparable rotator cuff tears. Options include superior capsular reconstruction (SCR), tendon transfers, subacromial balloon spacer, bridge grafting, biological tuberoplasty, and partial rotator cuff repair with or without augmentation. In our experience, repair with marginal convergence techniques is superior to SCR; if there is enough tissue, repair is the preferred strategy. In an effort to improve outcomes after repair, there has been increased interest in the use of interposition or bridging graft (BG) techniques, in which an allograft or autograft is secured on the humerus laterally and to the remnant tendon medially. Interposition or bridging grafts can be used to supplement partial repair in an effort to replace the patient's own missing tissues, and restore the biomechanical force couple of the rotator cuff and create a humeral head-depressing spacer effect in the subacromial space. These techniques show promising results compared to superior capsular reconstruction. Various graft options for BG are available, including human dermal allograft, fascia lata autograft, and tenotomized biceps autograft. Multiple animal studies have demonstrated that interposition grafts can improve the biomechanical properties of the repair construct, and histological studies in animal models have shown evidence of tissue in-growth into the BG, which could lead to increased repair strength over time. Finally, recent studies suggest that a bridging graft may improve short-term outcomes compared to partial repair alone. It remains to be seen whether this difference is clinically meaningful and durable.

Abstract:The goal of massive, retracted rotator cuff repair surgery is structural healing that produces pain relief and strength restoration. Healing is difficult to achieve in patients with large, retracted, chronic, and fatty degenerated tears. High repair tension previously has been shown to be a risk factor for clinically and structurally failed repairs. Repair tension can be decreased by arthroscopic mobilization and muscle advancement (AMA) of the supraspinatus and infraspinatus muscle bellies from their scapular origins with subsequent lateralization of the muscle-tendon units toward the repair site. Linked double-row repair of the tendons after AMA has been shown to improve the outcomes of repairs in high-risk rotator cuff repairs. Surgeons should consider adding AMA to their intraoperative armamentarium for use in retracted cuff tears when traditional techniques fail to achieve a low-tension repair state.

Abstract:Adsorption isoterms and capillary condensation in an open slit with walls decorated with arrays of pillars are examined using the density functional theory. Compared with the main substrate, the pillars can have the same or different parameters in the Lennard-Jones interaction potential between them and the fluid in the slit. The roughness of the solid surface, defined as the ratio between the area of the actual surface and the area of the surface free of pillars, is controlled by the height of the pillars. It is shown that the capillary condensation pressure first increases with increasing roughness, passes through a maximum, and then decreases. The amount of adsorbed fluid at constant volume of the slit has, in general, a nonmonotonic dependence on roughness. These features of adsorption and capillary condensation are results of increased surface area and changes in the fluid-solid potential energy due to changes in roughness.

Abstract:To improve healing after rotator cuff repair, synthetic, allograft, and dermal xenograft patches have been shown to significantly improve strength and load. Recent research has shown that a bio-inductive bovine collagen implant results in improvement in healing in partial-thickness, small, medium, and even large tears, but thus far, repair of massive and revision tears with augmentation is no better than repair alone. Optimizing these challenging repairs remains difficult and elusive.

Subacromial Surgery for Irreparable Posterosuperior Rotator Cuff Tears

Abstract:The irreparable posterosuperior rotator cuff tear describes a tear of the supraspinatus and/or infraspinatus tendon that is massive, contracted, and immobile in both the anterior-posterior and medial-lateral directions. Patients with an intact subscapularis and preserved forward elevation are challenging to treat because there is not a consensus treatment algorithm. For low-demand, elderly patients, several subacromial surgical options are available that can provide pain relief without the risks or burden of rehabilitation posed by reverse total shoulder arthroplasty or a complex soft-tissue reconstruction (e.g., superior capsular reconstruction, tendon transfer, bridging grafts). Debridement, more specifically the "smooth-and-move" procedure, offers a reliable outcome with documented improvements in pain and function at long-term follow-up. Similarly, the biodegradable subacromial balloon spacer (InSpace; Stryker, Kalamazoo, MI) has been shown to significantly improve pain and function in patients who are not responsive to nonoperative treatment. Disease progression with these options is possible, with a small percentage of patients progressing to rotator cuff arthropathy. Biologic tuberoplasty and bursal acromial reconstruction are conceptually similar to the balloon spacer but instead use biologic grafts to prevent bone-to-bone contact between the humeral head and the acromion. Although there is no single gold standard treatment, the variety of surgical techniques allows patients and surgeons to effectively manage these challenging situations.

Abstract:Massive irreparable rotator cuff tears (MIRCTs) represent a vexing problem, particularly in the physiologically young patient with no glenohumeral arthritis and high functional expectations. Not even the last line of defense, the reverse shoulder arthroplasty can help all of our patients with MIRCTs and resultant pseudoparalysis, and in younger patients, joint preservation could be a preferable first line intervention. Superior capsular reconstruction (SCR) shares the joint preservation arena with an evolving multitude of options, including arthroscopic debridement, partial rotator cuff repair, biologic tuberoplasty, interposition balloon spacer, and lower trapezius tendon transfer. SCR shows promising biomechanical and clinical outcomes, including restoration of native glenohumeral biomechanics and contact pressures, achievement of humeral head depression, reduction of pain, and improved patient reported outcome measures (PROMs) and range of motion (ROM), despite a high rate of graft re-tear or discontinuity. Yet, comparable results may be achieved via simpler procedures including debridement, partial repair, tuberoplasty, and/or spacer. Ultimately, the true value of SCR, or any other technique, would be proven improvement in strength or survivorship from subsequent surgery, particularly reverse arthroplasty. Optimizing indications for these constantly evolving techniques is a moving target and essential for selecting the highest value procedures for patients based on their individual needs.

Abstract:For irreparable posterior superior rotator cuff tears (IPSRCTs), techniques other than reverse shoulder arthroplasty and tendon transfer are still under research; among these, reusing the long head of the biceps (LHB) is noteworthy. Various techniques of LHB reuse have been reported and can be divided into 3 biomechanical categories: restoration of the force chain of the rotator cuff, augmentation of the superior capsule, and combined force-chain restoration and superior capsule augmentation, among which in situ biceps tenodesis may be the simplest. LHB reuse for IPSRCTs is always combined with partial rotator cuff repair, and clinical studies have revealed that this combined treatment is effective in most cases. However, the structural and functional conditions of IPSRCTs vary. For each LHB reuse method, the most suitable conditions for its use should be explored. Likewise, for each specific rotator cuff tear condition, the best method for LHB reuse also needs to be explored. Studies of high level of evidence and long-term follow-up are needed.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10_suppl3, October 2024. Massive rotator cuff tears (RCT) defined as defects measuring >5 cm or involving two or more torn tendons. Irreparable RCT are generally large and retracted with poor or attenuated tissue quality, muscular atrophy, and fatty infiltration. Surgical options for irreparable RCT are arthroscopic debridement, partial repair, margin convergence, patch graft, tendon transfer, balloon spacer, and reverse total shoulder arthroplasty (RSA). Recently, superior capsular reconstruction (SCR) developed by a Japanese surgeon, Mihata et al, has gained attention as potential treatment for patients with irreparable RCT. Anatomically, superior capsule acts as a stabilizing structure and its crucial role in improving shoulder function by re-centering the humeral head into the glenoid socket, thus restoring the force couples to efficiently elevate the arm in case of an irreparable superior cuff tear. Indications include younger patients (aged < 65-70 years) with irreparable, massive RCT involving the supraspinatus and infraspinatus with minimal arthritis, an intact or repairable subscapularis, and a functional deltoid without bony deficiency, stiffness, or advanced arthropathy. Fascia lata is harvested as the graft with 6 – 8 mm thickness should be achieved. The graft is fixed to the superior glenoid with 2 suture anchors, and 4 suture anchors are used to secure the graft laterally to the humerus. The graft should be sutured to the remnant posterior cuff tendon such as the infraspinatus or teres minor tendon. Reconstruction of the superior capsule using fascia lata autograft has shown promising early results with improved clinical scores, range of motion, and muscle strength.

Abstract:The management of massive rotator cuff tears (MRCT) presents a unique challenge to many orthopedic specialists. Unlike tears that are predicted to do well with primary, complete repair, MRCT are affected by tissue retraction, in-elasticity, bursal scarring, muscle atrophy, and fatty degeneration; operative repair thus portends worse healing rates than smaller tears and is associated with recurrent tear rates of up to 91% based on ultrasonography and magnetic resonance imaging (MRI). Rotator cuff tears are a common condition in patients over the age of 50. Thus, multiple advances in treatment strategies have been made to combat the limited efficacy of complete or partial rotator cuff repair in the setting of a massive or irreparable rotator cuff tears. It is of utmost importance that the operating orthopedic surgeon be familiar with these various treatment modalities to best serve the patient and that they harbor these skills within their armamentarium. This article details a review of the current literature including nonoperative and operative treatments for the management of massive and irreparable rotator cuff tears. The primary objective is to propose a literature-based algorithm for the treatment of massive and often irreparable rotator cuff tears to allow for informed ease in the decision-making process.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Massive irreparable rotator cuff tears (MIRCTs) remain a challenging treatment paradigm, particularly for nonelderly patients without pseudoparalysis or arthritis.Purpose:To use a network meta-analysis to analyze comparative studies of surgical treatment options for MIRCTs in patients <70 years of age for several patient-reported outcomes, range of motion (ROM), and acromiohumeral distance (AHD).Study Design:Network meta-analysis of comparative studies; Level of evidence, 3.Methods:A systematic review of the literature, using PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, of the MEDLINE, Embase, and Cochrane Library databases was conducted from 2017 to 2022. Inclusion criteria were (1) clinical comparative studies of MIRCTs (with several study-specific criteria); (2) ≥1 outcome of interest reported on, with standard deviations; (3) minimum 1-year follow-up; and (4) mean age of <70 years for both cohorts, without arthritis or pseudoparalysis. There were 8 treatment arms compared. Outcomes of interest were the American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley score, visual analog scale for pain, AHD, and forward flexion and external rotation ROM. A frequentist approach to network meta-analysis with a random-effects model was performed using the netmeta package Version 0.9-6 in R.Results:A total of 23 studies met the inclusion criteria, with 1178 patients included in the network meta-analysis. There was a mean weighted age of 62.8 years, 568 (48.2%) men, with a mean follow-up of 28.9 months. There were no significant differences between groups in regard to sex (P = .732) or age (P = .469). For the ASES score, InSpace balloon arthroplasty (mean difference [MD], 12.34; 95% CI, 2.18 to 22.50; P = .017), arthroscopic bridging graft (aBG) (MD, 7.07; 95% CI, 0.28 to 13.85; P = .041), and long head of biceps augmented superior capsular reconstruction (BSCR) (MD, 5.16; 95% CI, 1.10 to 9.22; P = .013) resulted in the highest P-scores. For the Constant-Murley score, debridement (MD, 21.03; 95% CI, 8.98 to 33.08; P < .001) and aBG (MD, 6.97; 95% CI, 1.88 to 12.05; P = .007) resulted in the highest P-scores. For AHD, BSCR resulted in the highest P-score (MD, 1.46; 95% CI, 0.45 to 2.48; P = .005). For forward flexion ROM, debridement (MD, 45.77; 95% CI, 25.41 to 66.13; P < .001) resulted in the highest P-score, while RSA resulted in the lowest P-score (MD, –16.70; 95% CI, −31.20 to −2.20; P = .024).Conclusion:For patients <70 years with MIRCT without significant arthritis or pseudoparalysis, it appears that graft interposition repair techniques, superior capsular reconstruction using the long head of the biceps tendon, arthroscopic debridement, and balloon arthroplasty provide superiority in various outcome domains, while RSA provides the least benefit in forward flexion.

Allograft Reconstruction for Massive, Irreparable Rotator Cuff Tears

Abstract:Background There is currently no widely accepted treatment for massive, irreparable rotator cuff tears. Hypothesis Allograft reconstruction to span the remaining defect in massive, irreparable rotator cuff tears will lead to increased functional results and will demonstrate healing of the allograft on follow-up magnetic resonance imaging studies. Study Design Case series; Level of evidence, 4. Methods Between 1989 and 2003, 32 patients underwent allograft reconstruction of massive rotator cuff tears. University of California, Los Angeles shoulder scores were compared preoperatively and postoperatively and analyzed using paired Student t-tests. In addition, 15 patients underwent postoperative magnetic resonance imaging to evaluate the structural integrity of the allograft reconstruction at the greater tuberosity insertion. Results Of 32 patients, 28 were available for review at a mean follow-up of 31.3 months; 23 of the 28 patients were satisfied with their outcome. There was 1 postoperative infection and 1 acute allograft rejection. The mean University of California, Los Angeles score increased from 12.1 preoperatively to 26.1 postoperatively (P< .001). All 15 patients evaluated with postoperative magnetic resonance imaging arthrograms demonstrated complete radiographic failure of the allograft rotator cuff reconstruction. Despite radiographic failure, the mean University of California, Los Angeles score increased from 13.2 preoperatively to 28.3 postoperatively in this subset of patients. Conclusion The functional results of this reconstruction method are similar to those reported for debridement and subacromial decompression alone and are satisfactory, despite magnetic resonance imaging arthrogram evaluation demonstrating failure of the structural integrity of the allograft. Allograft reconstruction carries increased risk of infection and rejection and is technically more difficult than other less expensive treatment options with similar functional results. Therefore, we do not recommend allograft reconstruction of massive and otherwise irreparable rotator cuff tears.

Abstract:The InSpace subacromial balloon spacer is indicated for the treatment of massive irreparable rotator cuff tears. The device is placed in the subacromial space with the aim of restoring shoulder function by limiting painful acromiohumeral contact and recentering the superiorly migrated humeral head. However, controversy exists because 2 randomized controlled trials have produced conflicting findings with regards to efficacy. These findings, along with narrow indications, have led to low use of the balloon among European and Latin American surgeons. The controversy generated by conflicting randomized controlled trials poses challenges in the interpretation of cost effectiveness, especially when the role of the balloon among a plethora of other options including superior capsular reconstruction (either with long head of biceps or dermal allograft), graft augmentation, tendon transfer, biologic tuberoplasty, acromial resurfacing, and reverse shoulder arthroplasty is not well defined. However, it is my opinion that the balloon has particular value in older, low-demand patients, in whom severe comorbidities make a shorter surgical duration and quicker recovery appealing.
Hierarchically Porous Rutile Titania: Harnessing Spontaneous Compositional Change in Mixed-Metal Oxides

Abstract:In numerous applications involving fluid flow through a porous structure, performance improvements, such as better flow characteristics and increased fluid−solid contact, are obtained when multiple length scales of porosity are present in the porous medium. To this end, we have developed a route to hierarchically porous rutile titania that obviates the need for preformed or preassembled structure-directing agents. The preparation involves the successive leaching of phases and components from a dense composite of wurtzite ZnO and inverse spinel Zn2TiO4. The resulting monoliths are composed of grains of highly crystalline rutile titania and possess a hierarchy of interconnected pores with length scales of 5 μm and 100 nm. Ex situ solid-state 67Zn and 47,49Ti NMR has helped probe local environments around these elements in both the mixed and pure crystalline materials that are formed during the leaching processes.
Bone density and skeletal metabolism in patients with orthotopic ileal neobladder.

Abstract:The interposition of a bowel segment as a bladder substitute into the urinary tract may result in impaired calcium metabolism. We studied 25 male patients (aged 45 to 77 yr) who had undergone a Vescica Ileale Padovana (VIP) reconstruction following cystectomy 29 to 75 mo before. Bone mineral density of the spine and femur was measured by dual x-ray absorptiometry. Blood and 24-h urine samples were analyzed for the main parameters of bone metabolism. Sixteen healthy men were enrolled as a control group. Although blood pH did not differ between patients and control subjects, VIP subjects showed lower levels of plasma HCO3- (P < 0.005) and higher serum chloride (P < 0.001). Bone alkaline phosphatase was higher (P < 0.001), and urine calcium, phosphate, and creatinine levels were lower in VIP patients (P < 0.01, P < 0.01, and P < 0.05, respectively). Bone mineral density at the femoral neck (P < 0.03) and Ward's triangle (P < 0.05) was decreased in VIP patients. When subdivided according to time since operation, patients who had the ileal neobladder implanted for a shorter period of time showed lower blood pH (P < 0.03) and urine calcium (P < 0.05) levels and higher urinary hydroxyproline (P < 0.02). Duration of the ileal neobladder was positively correlated with PTH (r = 0.46, P < 0.03) and blood pH (r = 0.47, P < 0.02). Furthermore, pH values were positively correlated with urine calcium (r = 0.48, P < 0.02). In conclusion, in patients with ileal neobladder, a mild metabolic acidosis is responsible for an increased bone turnover and lower bone mass. Moreover, a decrease over time in the absorption capacity of the ileal pouch might result in calcium malabsorption, which represents an additional risk factor for reduced bone mass in these patients.
Ischaemic preconditioning of myocardium.

Abstract:Myocardium has the innate potential to adapt to transient sublethal ischaemia so that it becomes more resistant to a subsequent, more severe, ischaemic insult. The response is called ischaemic preconditioning and protection of the myocardium is manifested by a slowing of adenosine triphosphate decline, limitation of ischaemic necrosis and a reduction in dysrhythmia severity. Protection conferred by preconditioning occurs in two distinct temporal phases. An early phase of protection is observed immediately but wanes within two to three hours (classic preconditioning). This is followed many hours later by a second window of protection (delayed preconditioning). The cellular mechanisms underpinning both forms of protection are currently being intensively investigated. There is evidence that human myocardium can be preconditioned ex vivo and also in situ during elective procedures such as angioplasty and coronary artery bypass grafting. Furthermore, evidence points to the possibility that preconditioning occurs naturally in some ischaemic syndromes, particularly warm-up angina and preinfarction angina. Ultimately, investigation of the mechanisms of preconditioning may contribute to the development of rational therapies for protecting the ischaemic myocardium and, perhaps more importantly, enhance our understanding of the molecular basis of ischaemic heart disease.

Long Head of Biceps Tendon Transposition for Massive and Irreparable Rotator Cuff Tears: A Systematic Review and Meta-Analysis

Abstract:Superior capsular reconstruction (SCR) with long head of biceps tendon (LHBT) transposition was developed to massive and irreparable rotator cuff tears (MIRCTs); however, the outcomes of this technique remain unclear.To perform a systematic review of biomechanical outcomes and a meta-analysis of clinical outcomes after LHBT transposition for MIRCTs.We performed a systematic electronic database search on PubMed, EMBASE, and Cochrane Library. Studies of SCR with LHBT transposition were included according to the inclusion and exclusion criteria. Biomechanical studies were assessed for main results and conclusions. Included clinical studies were evaluated for quality of methodology. Data including study characteristics, cohort demographics, and outcomes were extracted. A meta-analysis was conducted of the clinical outcomes.According to our inclusion and exclusion criteria, a total of six biomechanical studies were identified and reported an overall improvement in subacromial contact pressures and prevention of superior humeral migration without limiting range of motion (ROM) after LHBT transposition for MIRCTs. A total of five clinical studies were included in the meta-analysis of LHBT transposition outcomes, consisting of 253 patients. The results indicated that compared to other surgical methods for MIRCTs, LHBT transposition had advantages of more significant improvement in ROM (forward flexion mean difference [MD] = 6.54, 95% confidence interval [CI]: 3.07-10.01; external rotation [MD = 5.15, 95%CI: 1.59-8.17]; the acromiohumeral distance [AHD] [MD = 0.90, 95%CI: 0.21-1.59]) and reducing retear rate (odds ratio = 0.27, 95%CI: 0.15-0.48). No significant difference in American Shoulder and Elbow Surgeons score, visual analogue scale score, and University of California at Los Angles score was demonstrated between these two groups for MIRCTs.In general, SCR with LHBT transposition was a reliable and economical technique for treating MIRCTs, both in terms of biomechanical and clinical outcomes, with comparable clinical outcomes, improved ROM, AHD, and reduced the retear rates compared to conventional SCR and other established techniques. More high-quality randomized controlled studies on the long-term outcomes of SCR with LHBT transposition are required to further assess.

Abstract:Repair of rotator cuff tear is challenging because poor biologic factors can negatively impact tendon healing and risk complications. Sometimes, instead of attempting complex procedures with a guarded prognosis to reconstruct or replace rotator cuff like tissue, less is more. Minimally invasive, arthroscopic subacromial balloon spacer (SBS) for massive rotator cuff tears, demonstrates excellent long-term outcomes. Radiographically, SBS outcomes may be concerning, but clinically, patients do well. If pain is the primary complaint and function is maintained, the balloon spacer is an excellent option with a fast recovery. However, if function is deficient, or if the patient prioritizes strength recovery as their primary desired outcome, then more invasive procedures are often indicated.
