Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Superior capsular reconstruction (SCR) and lower trapezius tendon transfer (LTT) have recently been used to manage irreparable posterosuperior rotator cuff tears (PSRCTs). There has been a paucity of comparative biomechanical considerations between the 2 procedures.Purpose:To compare the glenohumeral stability and biomechanical properties between SCR and LTT in PSRCTs involving the entire infraspinatus tendon region.Study Design:Controlled laboratory study.Methods:Eight fresh-frozen cadaveric shoulders were tested at 0°, 20°, and 40° of shoulder abduction. Maximum internal, external, and total humeral range of motion (ROM), superior translation of the humeral head, and subacromial contact characteristics were compared among 4 conditions: (1) intact rotator cuff, (2) PSRCTs involving the entire infraspinatus tendon region, (3) LTT using Achilles allograft (12 N and 24 N of loading), and (4) SCR using fascia lata allograft.Results:Although a decrease in total ROM was noted in LTT with 12 N compared with the tear condition, LTT with both 12 N and 24 N as well as SCR did not restrict total rotational ROM compared with the intact condition. LTT had decreased total ROM compared with tear condition at 20° of abduction (P = .042), while no significant decrease was confirmed at all abduction angles after SCR. SCR and LTT with 24 N decreased superior translation compared with the PSRCT condition at 0° and 20° of abduction (P < .037) but not significantly at 40° of abduction, whereas LTT with a 24-N load decreased glenohumeral superior translation at all abduction angles (P < .039). Both SCR and LTT decreased subacromial contact pressure compared with the tear condition (P < .014) at all abduction angles. SCR decreased subacromial contact pressure at 0° and 40° of abduction (P = .019 and P = .048, respectively) compared with LTT with 12 N of loading, while there was no difference between SCR and LTT with 24 N of loading in all abduction angles. SCR increased the contact area compared with the PSRCT condition at all abduction angles (P < .023), whereas LTT did not increase the contact area.Conclusion:SCR and LTT decreased glenohumeral superior translation and contact pressure compared with PSRCT conditions. The LTT was superior to SCR in terms of superior translation of the humeral head at a higher shoulder abduction angle, whereas the SCR showed more advantageous subacromial contact characteristics compared with LTT.Clinical Relevance:These biomechanical findings provide insights into these 2 fundamentally different procedures for the treatment of young and active patients with PSRCTs involving the entire infraspinatus tendon region.

Novel complementary peptides to target molecules.

Abstract:We generated an evolutionary computer program that generates complementary peptide (C-pep) sequences, with the potential to interact with a target peptide, by comparing several physico-chemical parameters of each pair of the complementary peptides being analyzed. We generated C-peps to target several molecules. About 30% of synthesized C-peps interfered with the function of their targets. C5a stimulates generation of TNFα and other inflammatory cytokines. Inhibition of C5a should be effective against sepsis, which impairs the status of cancer-bearing patients. One of the inhibitory C-peps of C5a, termed AcPepA, was effective in Cynomolgus monkeys intravenously infused with a lethal dose of bacterial LPS (4 mg/kg) destined to die. The monkeys were rescued by intravenous administration of 2 mg/kg/h of AcPepA. The excellent therapeutic effect of AcPepA is likely to be due to restriction of high mobility group box 1 (HMGB1) surge induced by the effect of C5a on C5L2, which is the second C5a receptor, since the released HMGB1 has the capacity to stimulate TLR4 as an endogeneous ligand resulting in further activation of inflammatory cells to release inflammatory cytokines forming a positive feedback circuit of inflammation.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10_suppl3, October 2024. Massive rotator cuff tears (RCT) defined as defects measuring >5 cm or involving two or more torn tendons. Irreparable RCT are generally large and retracted with poor or attenuated tissue quality, muscular atrophy, and fatty infiltration. Surgical options for irreparable RCT are arthroscopic debridement, partial repair, margin convergence, patch graft, tendon transfer, balloon spacer, and reverse total shoulder arthroplasty (RSA). Recently, superior capsular reconstruction (SCR) developed by a Japanese surgeon, Mihata et al, has gained attention as potential treatment for patients with irreparable RCT. Anatomically, superior capsule acts as a stabilizing structure and its crucial role in improving shoulder function by re-centering the humeral head into the glenoid socket, thus restoring the force couples to efficiently elevate the arm in case of an irreparable superior cuff tear. Indications include younger patients (aged < 65-70 years) with irreparable, massive RCT involving the supraspinatus and infraspinatus with minimal arthritis, an intact or repairable subscapularis, and a functional deltoid without bony deficiency, stiffness, or advanced arthropathy. Fascia lata is harvested as the graft with 6 – 8 mm thickness should be achieved. The graft is fixed to the superior glenoid with 2 suture anchors, and 4 suture anchors are used to secure the graft laterally to the humerus. The graft should be sutured to the remnant posterior cuff tendon such as the infraspinatus or teres minor tendon. Reconstruction of the superior capsule using fascia lata autograft has shown promising early results with improved clinical scores, range of motion, and muscle strength.

Abstract:The interposition of a bowel segment as a bladder substitute into the urinary tract may result in impaired calcium metabolism. We studied 25 male patients (aged 45 to 77 yr) who had undergone a Vescica Ileale Padovana (VIP) reconstruction following cystectomy 29 to 75 mo before. Bone mineral density of the spine and femur was measured by dual x-ray absorptiometry. Blood and 24-h urine samples were analyzed for the main parameters of bone metabolism. Sixteen healthy men were enrolled as a control group. Although blood pH did not differ between patients and control subjects, VIP subjects showed lower levels of plasma HCO3- (P < 0.005) and higher serum chloride (P < 0.001). Bone alkaline phosphatase was higher (P < 0.001), and urine calcium, phosphate, and creatinine levels were lower in VIP patients (P < 0.01, P < 0.01, and P < 0.05, respectively). Bone mineral density at the femoral neck (P < 0.03) and Ward's triangle (P < 0.05) was decreased in VIP patients. When subdivided according to time since operation, patients who had the ileal neobladder implanted for a shorter period of time showed lower blood pH (P < 0.03) and urine calcium (P < 0.05) levels and higher urinary hydroxyproline (P < 0.02). Duration of the ileal neobladder was positively correlated with PTH (r = 0.46, P < 0.03) and blood pH (r = 0.47, P < 0.02). Furthermore, pH values were positively correlated with urine calcium (r = 0.48, P < 0.02). In conclusion, in patients with ileal neobladder, a mild metabolic acidosis is responsible for an increased bone turnover and lower bone mass. Moreover, a decrease over time in the absorption capacity of the ileal pouch might result in calcium malabsorption, which represents an additional risk factor for reduced bone mass in these patients.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Both superior capsular reconstruction (SCR) and middle trapezius tendon (MTT) transfer can be performed in nonarthritic young and active patients with isolated irreparable supraspinatus tendon tears (IISTTs). However, to our knowledge, no comparative clinical studies have been conducted on these procedures.Purpose:To evaluate and compare the clinical and radiological outcomes of SCR and arthroscopic-assisted MTT transfer in patients with IISTTs.Study Design:Cohort study; Level of evidence, 3.Methods:A total of 60 patients with IISTTs were categorized into 2 groups according to the surgical procedure—Group S underwent SCR (n = 34); Group M underwent MTT transfer (n = 26). The clinical outcomes included the visual analog scale score for pain, active shoulder range of motion, American Shoulder and Elbow Surgeons score, and University of California, Los Angeles, shoulder score. Radiological outcomes included the assessment of the acromiohumeral distance, progression of cuff tear arthropathy (Hamada grade), subacromial bone erosion, and graft failure.Results:The mean follow-up time was 39.3 ± 5.2 months (range, 26-59 months) and 37.6 ± 9.8 months (range, 27-54 months) in Group S and Group M, respectively. Significant improvements in clinical outcomes were observed in both groups, while the active forward flexion (148.2°± 24.1° vs 165.9°± 8.7°; P = .003) and abduction (131°± 37.3° vs 152.5°± 17.9°; P = .035) were significantly higher in Group M at the final follow-up. No significant differences were found in the postoperative Constant, American Shoulder and Elbow Surgeons, and University of California, Los Angeles, shoulder scores between the 2 groups. Radiologically, although no difference was found in the Hamada grade at the final follow-up between the 2 groups (P = .143), the rates of acromial wear (58.8% vs 15.4%; P < .001) and graft retear (47.1% vs 7.7%; P < .001) were significantly higher in Group S than in Group M.Conclusion:Both SCR and MTT transfer improved the overall clinical outcomes of IISTTs postoperatively, whereas MTT transfer was superior to SCR in terms of active forward flexion and abduction range of motion. Although higher rates of graft failure and subacromial bone erosion were observed in Group S, no difference was found in the clinical scores between the 2 groups at the short-term follow-up. However, further well-structured, prolonged comparative trials should be conducted in the future.

Abstract:Massive irreparable rotator cuff tears (MIRCTs) represent a vexing problem, particularly in the physiologically young patient with no glenohumeral arthritis and high functional expectations. Not even the last line of defense, the reverse shoulder arthroplasty can help all of our patients with MIRCTs and resultant pseudoparalysis, and in younger patients, joint preservation could be a preferable first line intervention. Superior capsular reconstruction (SCR) shares the joint preservation arena with an evolving multitude of options, including arthroscopic debridement, partial rotator cuff repair, biologic tuberoplasty, interposition balloon spacer, and lower trapezius tendon transfer. SCR shows promising biomechanical and clinical outcomes, including restoration of native glenohumeral biomechanics and contact pressures, achievement of humeral head depression, reduction of pain, and improved patient reported outcome measures (PROMs) and range of motion (ROM), despite a high rate of graft re-tear or discontinuity. Yet, comparable results may be achieved via simpler procedures including debridement, partial repair, tuberoplasty, and/or spacer. Ultimately, the true value of SCR, or any other technique, would be proven improvement in strength or survivorship from subsequent surgery, particularly reverse arthroplasty. Optimizing indications for these constantly evolving techniques is a moving target and essential for selecting the highest value procedures for patients based on their individual needs.

Abstract:Introduction: Concomitant nerve injuries with musculoskeletal injuries present a challenging problem. The goals of nerve reconstruction for the shoulder include shoulder abduction and external rotation. When patients fail to achieve acceptable shoulder external rotation and shoulder abduction, tendon transfers such as trapezius transfer offer a reliable option in the subsequent stage. Case Presentation: A 32-year-old male presented with weak external rotation in his left shoulder, after previous axillary nerve reconstruction. He received the ipsilateral lower trapezius transfer with the aim of improving the external rotation. Discussion: The lower trapezius restores a better joint reaction force in both the compressive-distractive and anterior-posterior balancing and provides a centering force through the restoration of the anterior-posterior force couple. Conclusion: We believe that the ipsilateral lower trapezius transfer to the infraspinatus is a good outcome and is effective in improving overall shoulder stability and the shoulder external rotation moment arm or at least maintaining in neutral position with the arm fully adducted in patients with post axillary nerve injuries post unsatisfactory nerve reconstruction to increase the quality of life and activities of daily living.

Abstract:We have tested the interaction between amyloid beta protein (A beta P) and tachykinin receptors in cultured UC-11MG astrocytoma cells, which express high affinity substance P receptors and respond to substance P with an unusually large stimulation of polyphosphoinositide hydrolysis. Both the full-length A beta P (A beta P1-40) and the fragment 25-35 (A beta P25-35) did not affect the stimulation of [3H]inositolmonophosphate (InsP) formation by substance P. A beta P25-35 was also inactive when applied to the cultures 18 or 72 h prior to the assay. In addition, A beta P25-35 did not displace specifically bound [3H]SarMet substance P from its recognition sites in intact UC-11MG cells. These results suggest that, at least in this specific cell type, amyloid peptides do not interact with substance P receptors.

Abstract:Background Lower trapezius tendon (LTT) transfer has emerged as a promising surgical intervention for addressing posterosuperior irreparable rotator cuff tears (PSIRCTs) in the absence of glenohumeral arthritis. However, comparative studies evaluating primary versus secondary LTT transfers following rotator cuff repair failure are limited. Methods This retrospective study analyzed 103 patients who underwent LTT transfer for PSIRCTs without glenohumeral arthritis. Patients were categorized into Group Primary (no prior shoulder surgeries, n=83) and Group Secondary (previous rotator cuff repairs failure, n=20). Clinical outcomes were assessed using Visual Analog Scale (VAS), Constant score, American Shoulder and Elbow Surgeons (ASES) score, University of California Los Angeles (UCLA) score, and Single Assessment Numeric Evaluation (SANE) score. Radiological evaluations included acromiohumeral distance (AHD) and Hamada classification. Incidences of complications such as retears and infections were documented. Results The mean age for the Group Primary and the Group Secondary was 63.3 ± 6.3 years and 61.5 ± 4.8 years, respectively, with mean follow-up periods of 37.4 ± 15.7 months and 42.9 ± 16.8 months, respectively. Both Group Primary and Group Secondary demonstrated significant postoperative improvements in VAS, Constant, ASES, UCLA, and SANE scores, with no statistically significant differences observed between the groups. Radiologically, both groups showed a slight progression of glenohumeral arthritis with no significant differences between the groups. Retears of transferred tendon occurred at a similar rate in each group, with 18% in Group Primary and 20% in the Group Secondary Conclusion Secondary LTT transfer following rotator cuff repair failure represents an effective surgical approach for managing PSIRCTs, demonstrating comparable improvements in clinical and radiological outcomes to primary LTT transfer. These findings support the consideration of LTT transfer irrespective of prior rotator cuff repair history. Long-term follow-up studies are essential to evaluate the durability and functional outcomes over extended periods.

Abstract:Adsorption isoterms and capillary condensation in an open slit with walls decorated with arrays of pillars are examined using the density functional theory. Compared with the main substrate, the pillars can have the same or different parameters in the Lennard-Jones interaction potential between them and the fluid in the slit. The roughness of the solid surface, defined as the ratio between the area of the actual surface and the area of the surface free of pillars, is controlled by the height of the pillars. It is shown that the capillary condensation pressure first increases with increasing roughness, passes through a maximum, and then decreases. The amount of adsorbed fluid at constant volume of the slit has, in general, a nonmonotonic dependence on roughness. These features of adsorption and capillary condensation are results of increased surface area and changes in the fluid-solid potential energy due to changes in roughness.

Abstract:Six new norlignans, named sequosempervirins B–G (1–6), together with three known norlignans, agatharesinol (7), agatharesinol acetonide (8), and sugiresinol (9), were isolated from the branches and leaves of Sequoia sempervirens. Their structures were determined mainly by high‐resolution mass spectroscopy (HR‐MS), and various 1D‐ and 2D‐NMR methods, as well as, in the case of 1, by means of X‐ray diffraction. Compound 8 showed anticancer activity towards the A549 non‐small‐cell lung‐cancer cell line (IC50=27.1 μM). The acetone extract of S. sempervirens was found to be antifungal towards Candida glabrata (IC50=15.98 μg/ml), and both the acetone and MeOH extracts inhibited the proteolytic activity of cathepsin B (IC50=4.58 and 5.49 μg/ml, resp.).

Abstract:The optimal treatment for patients diagnosed with isolated supraspinatus irreparable rotator cuff tears continues to be a subject of debate. Joint-preserving methods, including partial repair, superior capsule reconstruction, balloon spacers, and tendon transfer, have been introduced. Among these options, the middle trapezius tendon (MTT) transfer has garnered attention for its potential to replace the irreparable portion of the supraspinatus tendon and provide dynamic stability to the joint. Although some reports have highlighted promising clinical outcomes of MTT, there remains a dearth of literature regarding the techniques and methods involved in the surgical procedure. This Technical Note introduces an arthroscopic-assisted technique for MTT transfer using an Achilles tendon allograft for patients diagnosed with isolated supraspinatus irreparable rotator cuff tears in lateral decubitus.
Long Head of Biceps Tendon Transposition for Massive and Irreparable Rotator Cuff Tears: A Systematic Review and Meta-Analysis

Abstract:Superior capsular reconstruction (SCR) with long head of biceps tendon (LHBT) transposition was developed to massive and irreparable rotator cuff tears (MIRCTs); however, the outcomes of this technique remain unclear.To perform a systematic review of biomechanical outcomes and a meta-analysis of clinical outcomes after LHBT transposition for MIRCTs.We performed a systematic electronic database search on PubMed, EMBASE, and Cochrane Library. Studies of SCR with LHBT transposition were included according to the inclusion and exclusion criteria. Biomechanical studies were assessed for main results and conclusions. Included clinical studies were evaluated for quality of methodology. Data including study characteristics, cohort demographics, and outcomes were extracted. A meta-analysis was conducted of the clinical outcomes.According to our inclusion and exclusion criteria, a total of six biomechanical studies were identified and reported an overall improvement in subacromial contact pressures and prevention of superior humeral migration without limiting range of motion (ROM) after LHBT transposition for MIRCTs. A total of five clinical studies were included in the meta-analysis of LHBT transposition outcomes, consisting of 253 patients. The results indicated that compared to other surgical methods for MIRCTs, LHBT transposition had advantages of more significant improvement in ROM (forward flexion mean difference [MD] = 6.54, 95% confidence interval [CI]: 3.07-10.01; external rotation [MD = 5.15, 95%CI: 1.59-8.17]; the acromiohumeral distance [AHD] [MD = 0.90, 95%CI: 0.21-1.59]) and reducing retear rate (odds ratio = 0.27, 95%CI: 0.15-0.48). No significant difference in American Shoulder and Elbow Surgeons score, visual analogue scale score, and University of California at Los Angles score was demonstrated between these two groups for MIRCTs.In general, SCR with LHBT transposition was a reliable and economical technique for treating MIRCTs, both in terms of biomechanical and clinical outcomes, with comparable clinical outcomes, improved ROM, AHD, and reduced the retear rates compared to conventional SCR and other established techniques. More high-quality randomized controlled studies on the long-term outcomes of SCR with LHBT transposition are required to further assess.

Abstract:BACKGROUND: Margin convergence (MC) and superior capsular reconstruction (SCR) are common treatment options for irreparable rotator cuff tears in younger patients, although they differ in associated costs and operative times. The purpose of this study was to compare range of motion, patient reported outcomes (PROs), and re-operation rates following MC and SCR. We hypothesized superior outcomes after SCR relative to MC regarding functional outcomes, subjective measures, and re-operation rates.METHODS: This was a multicenter retrospective review of 59 patients from three surgeons treating irreparable rotator cuff tears with either MC (n=28) or SCR (n=31) and minimum 1-year follow-up from 2014-2019. Visual Analogue Scale (VAS) for pain, Subjective Shoulder Value (SSV), active forward flexion (FF), external rotation (ER), re-tear rate, and conversion rate to reverse shoulder arthroplasty were evaluated. T-tests and Chi-Squared tests were used for continuous and categorical variables, respectively (*p<0.05).RESULTS: Baseline demographics, range of motion, and MRI findings were similar between groups. Average follow-up was 31.5 months and 17.8 months for MC and SCR groups (p<0.001). The MC and SCR groups had similar postoperative FF (151 ± 26 vs 142 ± 38 degrees; p=0.325) and ER (48 ± 12 vs 46 ± 11 degrees; p=0.284), with both groups not improving significantly from their preoperative baselines. However, both cohorts demonstrated significant improvements in VAS (MC: 7.3 to 2.5; SCR: 6.4 to 1.0) and SSV (MC: 54% to 82%; SCR: 38% to 87%). There were no significant differences in postoperative VAS, SSV, and rates of re-tear or rates of conversion to arthroplasty between groups. In patients with preoperative pseudoparesis (FF < 90°), SCR (n=9) resulted in greater postoperative FF than MC (n=5) (141 ± 38 vs 67 ± 24 degrees; p=0.002).CONCLUSION: Both MC and SCR demonstrated excellent postoperative outcomes in the setting of massive irreparable rotator cuff tear, with significant improvements in PROs and no significant differences in range of motion. Specifically for patients with preoperative pseudoparesis, SCR was more effective in restoring forward elevation. Further long-term studies are needed to compare outcomes and establish appropriate indications.

Abstract:The irreparable posterosuperior rotator cuff tear describes a tear of the supraspinatus and/or infraspinatus tendon that is massive, contracted, and immobile in both the anterior-posterior and medial-lateral directions. Patients with an intact subscapularis and preserved forward elevation are challenging to treat because there is not a consensus treatment algorithm. For low-demand, elderly patients, several subacromial surgical options are available that can provide pain relief without the risks or burden of rehabilitation posed by reverse total shoulder arthroplasty or a complex soft-tissue reconstruction (e.g., superior capsular reconstruction, tendon transfer, bridging grafts). Debridement, more specifically the "smooth-and-move" procedure, offers a reliable outcome with documented improvements in pain and function at long-term follow-up. Similarly, the biodegradable subacromial balloon spacer (InSpace; Stryker, Kalamazoo, MI) has been shown to significantly improve pain and function in patients who are not responsive to nonoperative treatment. Disease progression with these options is possible, with a small percentage of patients progressing to rotator cuff arthropathy. Biologic tuberoplasty and bursal acromial reconstruction are conceptually similar to the balloon spacer but instead use biologic grafts to prevent bone-to-bone contact between the humeral head and the acromion. Although there is no single gold standard treatment, the variety of surgical techniques allows patients and surgeons to effectively manage these challenging situations.

Abstract:Background: Large-to-massive rotator cuff tears (LMRCTs) present challenges in achieving successful repair due to factors such as muscle atrophy and tendon retraction. Arthroscopic rotator cuff repair (ARCR) with reinforcement techniques like superior capsule reconstruction (SCR) or patch graft augmentation (PGA) has emerged as a less invasive option to improve shoulder joint stability and prevent retear. This study aimed to compare the clinical and radiological outcomes of SCR and PGA as reinforcement techniques for the arthroscopic repair of LMRCTs. Methods: A single-center retrospective study was conducted on patients undergoing LMRCT repair between January 2019 and December 2021. Patients were divided into two groups: those receiving SCR (Group 1) and those receiving PGA (Group 2). Various clinical parameters including range of motion, functional scores, and radiological assessments were evaluated preoperatively and six months postoperatively. Results: Both SCR and PGA techniques demonstrated significant improvements in the range of motion and clinical scores postoperatively. However, Group 2 showed higher postoperative SST and UCLA scores compared to Group 1. Radiologically, there was a slightly higher retear rate in Group 2, although this was not statistically significant. Group 2 also had a shorter mean duration of surgery compared to Group 1. Conclusions: In the arthroscopic repair of LMRCTs, both SCR and PGA techniques exhibit favorable clinical and radiological outcomes. Despite the simplicity of PGA compared to SCR, it offers comparable results with a shorter surgical duration, making it a feasible reinforcement option for surgeons.

Abstract:The subscapularis is a dynamic structure serving as part of a shoulder force couple in the transverse plane, conferring stability, strength and optimizing motion. Full thickness, retracted anterosuperior rotator cuff tears with fatty infiltration pose a significant challenge to surgeons particularly in younger patients with high functional demands where primary repair may not be feasible. This clinical scenario is rare, and most investigations on this topic are limited to small, retrospective case-series with short-term follow-up. For patients in whom joint preservation is desired, pectoralis major transfer or latissimus dorsi transfers are the most reported surgical options with heterogeneity in techniques and clinical outcomes. Recent research describes an alternative of pectoralis minor transfer for this same purpose with promising short-term outcomes. However, as with pectoralis major transfers for management of anterosuperior irreparable, the outcome is highly dependent on the integrity of the posterosuperior rotator cuff. Thus, when faced with a patient with a massive anterosuperior rotator cuff tear with atrophy, retraction, and other demographic or medical conditions that may predispose that patient to a lower likelihood of healing and doing the extensive rehabilitation from a tendon transfer, consider the alternative of a reverse total shoulder arthroplasty. The reverse is a reliable operation that affords patients excellent pain relief, reasonable function typically lifting up to 25-30 pounds, with good patient reported outcomes even in patients as young as 60 years of age. Tendon transfers, including pectoralis minor transfers, have a role, but we need to be thoughtful about indications and counsel patients on expected outcomes and the durability of those outcomes over time.

Abstract:The incidence of a subscapularis tear combined with any other rotator cuff tear is around 19% to 49% among all rotator cuff lesions. On the contrary, less attention has been given to the treatment of anterosuperior rotator cuff tears, particularly by arthroscopic methods. Subscapularis lesions are hard to access and require advanced surgical technique along with optimum visualization to achieve an anatomic repair. Use of an anterior portal helps in obtaining a good visualization with a 30° arthroscope in viewing the tendon along its axis as well as the posterior, superior, and anterior sides. Incorporation of the biceps tendon along with supraspinatus repair helps strengthen the supraspinatus repair as well as preserve the acromiohumeral index. Double-row repair of both subscapularis and supraspinatus ensures an adequate bone bed for tendon healing.

Abstract:Rotator cuff pathology is the main cause of shoulder pain and dysfunction in older adults. When a rotator cuff tear involves the subscapularis tendon, the symptoms are usually more severe and the prognosis after surgery must be guarded. Isolated subscapularis tears represent 18% of all rotator cuff tears and arthroscopic repair is a good alternative primary treatment. However, when the tendon is deemed irreparable, tendon transfers are the only option for younger or high-functioning patients. The aim of this review is to describe the indications, biomechanical principles, and outcomes which have been reported for tendon transfers, which are available for the treatment of irreparable subscapularis tears. The best tendon to be transferred remains controversial. Pectoralis major transfer was described more than 30 years ago to treat patients with failed surgery for instability of the shoulder. It has subsequently been used extensively to manage irreparable subscapularis tendon tears in many clinical settings. Although pectoralis major reproduces the position and orientation of the subscapularis in the coronal plane, its position in the axial plane - anterior to the rib cage - is clearly different and does not allow it to function as an ideal transfer. Consistent relief of pain and moderate recovery of strength and function have been reported following the use of this transfer. In an attempt to improve on these results, latissimus dorsi tendon transfer was proposed as an alternative and the technique has evolved from an open to an arthroscopic procedure. Satisfactory relief of pain and improvements in functional shoulder scores have recently been reported following its use. Both pectoralis minor and upper trapezius transfers have also been used in these patients, but the outcomes that have been reported do not support their widespread use.
Thrombosed varicose vein in the sternocleidomastoid muscle

Abstract:A thrombosed varicose vein arising in the neck is quite rare. A 68-year-old Japanese male was referred to our hospital because of a mass in his left upper neck. The mass did not change in size in response to strain resulting from Valsalva’s manouvre. Magnetic resonance imaging (MRI) showed iso-signal intensity of the mass on T1-weighted images and a target-like signal arrangement (concentric hyper-, hypo-, hyper-signal intensity from outside to inside) on T2-weighted images. Surgical excision revealed that the tumour arose from the intramuscular small vein in the sternocleidomastoid muscle. The pathological examination revealed the mass to be a thrombosed varicose vein with capillarization in the dilated vein wall. The de-oxygenation and degradation of haemoglobin were thought to be responsible for these characteristic MRI findings. The concentric signal distribution on MRI strongly suggested this pathology.
