Abstract:OBJECTIVE:To explore the operative approach and method of internal fixation for the treatment of type-C thoracolumbar fractures. METHODS:T wenty-eight patients (male 20, female 8, ranging in age from 20 to 54 years, with an average of 38.5 years) with type-C thoracolumbar fractures were invovled in the study. Distributed segments involved T11 in 2 patients, T12 in 3 patients, L1 in 11 patients, L2 in 8 patient, L3 in 3 patients and L4 in 1 patient. Twenty-six patients were followed up (range from 12 to 20 months). According to the analysis of X-ray and CT image,height of vertebral body, Cobb angle on sagittal and coronal plane and the percentage of occupancy of vertebral canal were measured. The recovery of nerve, happening of back pain and the failure of internal fixation were observed. RESULTS:The preoperative averaging height-loss decreased from 37.4% to 6.8% and the deformation of coronal plane was completely rectified. The preoperative averaging Cobb angle on sagittal plane recovered from 22.3 degrees to 5.6 degrees and the preoperative occupancy of vertebral canal averaging recovered from 33.7% to 5.9%. The difference was statistically significant (P < 0.05). Moreover, after 1 year follow-up, the changes of the above-mentioned index was no statistically significant (P > 0.05). Except for 8 patients with complete nerve damage losing the possibility of recovery, the others with incomplete nerve damage obtained 1 to 3 degree's improvement. The ratio of back pain occurrence was 19.2%. There was no failure of internal fixation. CONCLUSION:The treatment of thoracolumbar type-C fractures with simple posterior long-segment internal fixation or posterior long-segmental fixation added by anterior autograft fusion is a reliable and effective method. The short-term therapeutic effect is satisfactory and the long-term therapeutic effect is to be further observed.
[Clinical Effect of Posterior Atlanto Axial Vertebra Internal Fixation for Treatment of Instability of Occipitocervical].

Abstract:OBJECTIVE:To explore radiographic results and clinical effects of posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical.METHODS:The clinical data of 155 patients with instability of occipitocervical treated by posterior atlanto axial vertebra internal fixation were respectively analyzed from September 2005 to January 2011. There were 68 males and 87 females, ranging in age from 6 to 75 years old with an average of 45.6 years old. Of them, 53 cases were fresh odontoid fractures(Aderson type II C), 30 cases were os odontoideum, 20 cases were old odontoid fractures, 18 cases were unstable atlas fractures, 12 cases were atlanto axial rotatory dislocation, 11 cases were atlanto axial dislocation after rheumatoid arthritis, and 11 cases were basilar invagination. Radiographic results were evaluated in terms of atlas pedicle screw fixation, bone healing and bone graft fusion. Clinical effect evaluation included relief of pain in the occipital-cervical region by VAS score and JOA score.RESULTS:Totally 300 screws were set through atlas pedicle screw fixation in 150 patients. Five patients receivde hook fixation. Postoperative CT showed ideal nailing were 275 (91.7%),acceptable nailing were 14 (4.7%) and unacceptable nailing were 11 (3.6%). All patients were followed up, and the duration ranged from 16 to 40 months with an average of 25.4 months. The fresh fractures healed and 140 cases got bone graft fusion. Preoperative VAS and JOA score were respectively improved from (7.2 +/- 1.1), (7.3 +/- 2.4) to (3.2 +/- 1.1), (13.3 +/- 2.4) at the latest follow-up.CONCLUSION:Posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical can effectively recover physiological curvature of cervical, provide mechanical stability, and obtain good clinical effect. For the young patients who require further activity, posterior fixation and non-fusion technology is a good choose, which can avoid bone graft.
Biomechanical and Clinical Study of Anterior Transarticular Screw Fixation for Treatment of Traumatic Instability of C1-C2 Joint

Abstract:Objective To evaluate the biomechanical stability of C1-C2 after anterior C1-C2 transarticular screw fixation and assess its curative effect clinically. Methods Eight human cadaveric occipitocervical spine specimens which have been harvested freshly were tested in three modes of loading,flexion-extension, lateral bending, and axial rotation, and four configurations including intact, after an odontoid fracture, and after anterior or posterior C1-C2 transarticular screw fixation. All tests were performed in load and torque control. Clinically, 20 patients who had C1 -C2 traumatic instability were instructed to be operated with anterior C1 -C2 transarticular screw fixation technique, planting cancellated granulate bone into the space between muscular tubercle of arias and odontoid bone. Results Both the anterior and posterior C1-C2 transarticular screw fixation techniques limited motion at the C1-C2 articulation at the tested directions and there were no significant differences between the two techniques. In the 20 patients, one with complete cervical cord injury died of pulmonary infection a month later, and the rest 19 were followed up about 7 months to 3 years (mean 18 months). They all had no trauma on vertebral artery and cervical cord during their cervical operations, and had aflantoaxial synostoses. Conclusion The anterior C1-C2 transarticular screw fixation not only is easy to manipulate and provides the same stability as posterior C1-C2 transarticular screw fixation, but has a less risk of injury on vertebral artery and cervical cord.
Management of hangman’s fracture with percutaneous transpedicular screw fixation

Abstract:Purpose This study describes a percutaneous technique for C2 transpedicular screw fixation and evaluates its safety and efficacy in the treatment of patients with hangman’s fracture. Methods Ten patients with hangman’s fracture were treated by percutaneous C2 transpedicular screw fixation. There are six males and four females, who were, based on the classification of Levine and Edwards, sorted as follows: type I fracture, three cases; type II, five cases; type IIa, two cases. The causes of injury were road traffic accident in six patients and falling injury in four patients. Other associated lesions included rib fractures (7 patients), head injuries (4 patients), and fractures of extremities (6 patients). Results The new technique was performed successfully in all cases. The average operation time was 98 min (range 60–130 min) and the estimated blood loss was 25 ml (range 15–40 ml). No complications such as vascular or neural structures injuries were found intraoperatively. Postoperative CT scans demonstrated that 17 (85 %) of 20 screws were placed satisfactorily, and 3 (15 %) screws showed perforations of the pedicle wall (<2 mm). These patients were asymptomatic and no further intervention was required postoperatively. After 8–25 months follow-up (mean 15.3 months), solid fusion was demonstrated by computed tomography. All cases got well-sagittal alignment and no angulation or dislocation was found at the segment of C2–C3. There was no loss of fixation. Clinical examination showed a full range of motion in the neck in all patients. Conclusions The fluoroscopically assisted percutaneous C2 transpedicular screw fixation method is a technically feasible and minimally invasive technique for hangman’s fracture.
Posterior Arthrodesis of C1-C3 for the Stabilization of Multiple Unstable Upper Cervical Fractures with Spinal Cord Compromise

Abstract:Background: Multiple fractures of the atlas and axis are rare. The management of multiple fragment axis fractures and unstable atlas fractures is still challenging for the spinal surgeon. There are no published reports of similar fractures with 3-part fracture of axis associated with an unstable atlas fracture.Case summary: We present a patient with concurrent axis and atlas fractures, which have not been reported. The patient suffered hyperextension injury with neck pain and numbness of the bilateral upper extremity associated with weakness after a 2-m fall. The axis fractures included an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3. The atlas fracture was unstable. The neurological examination manifested as central canal syndrome, which was due to the hyperextension injury of cervical spine and spondylolisthesis of C2-C3. The patient was diagnosed as multiple unstable upper cervical fractures with spinal cord compromise. We performed posterior arthrodesis of C1-C3. Postoperatively, the patient showed neurological improvement, and C1-C3 had fused at the 3-month follow-up.Conclusion: Posterior arthrodesis of C1-C3 could provide a stable fixation for the 3 parts of axis (an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3) combined an unstable atlas fracture. Both the patient and the doctor were satisfied with the results of the treatment. So posterior arthrodesis of C1-C3 is a suitable treatment option for the treatment of a concurrent unstable atlas fracture and multiple fractures of the axis.
Anterior Cervical Discectomy and Fusion Plus Plating for Unstable Hangman's Fractures

Abstract:Objective To investigate the clinical effectiveness of anterior cervical discectomy and fusion (ACDF)plus plating in the treatment of unstable Hangman's fractures.Methods A retrospective case series study was carried out to analyze 12 patients who received ACDF plus plate internal fixation for unstable Hangman's fracture admitted to Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine from July 2014 to July 2018.There were nine males and three females,aged 24-70 years with an average of 46.2 years.According to Levine-Edwards typing,there were seven patients of type Ⅱ,four patients of type Ⅱ A and one patient of type Ⅲ.There were two patients of grade D and 10 patients of grade E according to the American Spinal Injury Association (ASIA) classification.The operation time,intraoperative blood loss and the operational complications were recorded.At 3 days,1,3,6,12 and 24 months after operation,the height of intervertebral disc,physiological curvature of cervical spine,fracture healing and bone graft fusion were evaluated.The recovery of neurological function was assessed by ASIA,and the neck pain relief and function improvement was assessed by visual analogue scale (VAS) and the neck disability index (NDI).Results All patients were followed for 6-48 months(mean,27.7 months).The operation time ranged from 50-100 minutes,with an average of 72.5 minutes.The intraoperative blood loss was 30-120 ml (mean,65.8 ml).No serious complications occurred during and after operation.Two patients had dysphagia after operation,and the symptoms were relieved at the follow-up of 3 months.All patients achieved bony fusion of C2-3 within the 6 month follow-up.No subsidence or migration of cages and significant loss of intervertebral space height were observed.The patient's neck and occipital pain was relieved obviously,and the flexion,extension and rotation of cervical spine were good.The neurologic dysfunction of two patients of ASIA Grade D recovered after surgery,and the final ASIA grade was E.The VAS score was decreased from 5.0 (4.0,6.0)points preoperatively to 1.0(0,1.0)points at the last follow-up (P ＜0.01).The NDI score was improved from (28.5 ± 6.4) points preoperatively to (5.3 ± 2.7) points at the last follow-up (P＜0.01).Conclusion ACDF combined with plate internal fixation can provide immediate postoperative stability,high fusion rate,less complications,less pain,and satisfactory function recovery.
Deflective Placement of Anterior Odontoid Cannulated Screw for Treatment of Odontoid Fracture Combined with Atlantoaxial Posterior Dislocation

Abstract:Objective To investigate deflective placement of anterior odontoid cannulated screw for treatment of odontoid fracture combined with atlantoaxial posterior dislocation.Method From August 2002 to December 2011,18 cases of fresh odontoid fracture combined with atlantoaxial posterior dislocation were treated with deflective placement of anterior odontoid cannulated screw in our hospital.They were 10 males and 8 females,aged from 20 to 50 years (mean,41 years).According to the classification of Anderson-D'Alonzon,12 cases were type Ⅱ and 6 type Ⅲ; according to the classification of American Spinal Injury Association (ASIA),3 cases were grade C,10 grade D and 5 grade E.All patients underwent skull traction pre-operation to reduce dislocation.During the operation,the head was bent close to the chest in order to maintain atlantoaxial restoration and odontoid reduction.Because in this posture the jawbone blocked Kirschner wire and made the placement challenging,the anterior odontoid cannulated screw was deflected in all cases.Results In this series,the operation time ranged from 1.0 to 1.5 hours and the blood loss 50 to 100 mL (mean,75 mL).The mean time of follow-up was 15 months (range,12 to 18 months).All cases got bony union 3 to 4 months postoperation without screw failure.According to the ASIA grading for neural function,one case improved from grade C to grade E,one from grade C to grade D,9 cases from grade D to grade E,and no improvement was noted in the other cases.The difference in atlantoaxial shift distance between preoperation and postoperation was statistically significant (P ＜ 0.05).Conclusion In the treatment of odontoid fracture combined with atlantoaxial posterior dislocation,deflective placement of anterior odontoid cannulated screw may preserve the rotation function of the atlas and axis and lead to positive outcomes.
Individual Surgical Strategy Using Posterior Lag Screw-Rod Technique for Unstable Atypical Hangman's Fracture Based on Different Fracture Patterns

Abstract:Objective: The literature on surgical management of atypical hangman's fracture (AHF) is sparse. The aim of this study was to describe an individual surgical strategy using a lag screw-rod technique for treatment of unstable AHF based on different fracture patterns. Methods: A retrospective analysis of 23 patients with unstable AHF was performed. Fractures were classified into 3 patterns: A, 1 fracture line through 1 side of C2 body obliquely and another through pars interarticularis on the other side; B, 1 fracture line through 1 side of C2 body obliquely and another through contralateral lamina; C, bilateral oblique fracture lines through posterior cortex of C2 on different sides. Posterior C2-C3 pedicle screw fixation and fusion using a lag screw-rod technique with different surgical strategies for each fracture pattern was used for all patients. Complications, neck pain, neurologic status, reduction of anterior translation and angulation between C2 and C3, and fusion rate were evaluated. Results: No technique-related complications (e.g., spinal cord or nerve injury caused by malposition of screws) occurred. Mean follow-up time was 37 months. Satisfactory reduction and bony union were demonstrated on postoperative radiographs. Neck pain and neurologic deficits caused by C2 injury improved significantly in all patients after operation. No graft or implant-related complications were observed in patients during the entire follow-up period. Conclusions: Using individual surgical strategies based on different fracture patterns, the posterior C2-C3 lag screw-rod technique may be an effective and reliable option for unstable AHF.
Surgical Management of Hangman's Fracture: A Systematic Review

Abstract:Background: Hangman's fractures are bilateral fractures of the C2 pars interarticularis produced during hyperextension injuries. The Levine-Edwards classification divides these fractures into 4 categories determined by injury stability. While stable fracture patterns are typically managed conservatively, prolonged traction required in unstable fractures may be superseded by surgery in its practicality. Surgical approaches can be divided into anterior and posterior: the anterior approach allows access to the disc and is used for anterior cervical discectomy and fusion (ACDF); the posterior approach includes C2 direct pedicle screw (DPS), which preserves motion segments and may be done with a minimally invasive surgery (MIS) approach. Multilevel rod and screw fusion provide the strongest biomechanical fixation. This systematic review compares indications, complications, and functional outcomes of different approaches. Methods: A search of multiple databases with keywords "hangman fracture," "hangman's fracture," "axis fracture," and "C2 fracture" was conducted; articles were included if they described the surgical technique and included at least one of the primary outcomes: functional outcomes, complication rates, operation time, and blood loss. Results: A total of 1889 abstracts were screened, 137 full text articles were analyzed, and 36 articles were included, yielding a combined total of 627 patients. ACDF was preferred in unstable fracture patterns. Pre- and postoperative visual analog scale (VAS) scores fell in all groups, with MIS DPS producing the lowest VAS scores. Approaches had excellent neurological improvement and fusion rates. Reported complication rates were generally low; self-limiting dysphagia was most common in the anterior approach and higher volumes of blood loss occured with the posterior approach (255.9 mL in open posterior approach, 75.8 mL in MIS, and 64.3 mL in ACDF). Conclusion: All surgical methods of hangman's fracture fixation have their indications and advantages; surgeons should be equipped to perform all options. Anterior approaches may be preferred for their lower blood loss and access to the disc; however, MIS may improve outcomes in posterior approach. Clinical relevance: This systematic review can assist spinal surgeons in the selection of the most appropriate surgical option for hangman's fracture and allow surgeons to inform patients of the risks and benefits.
Dynamics and instabilities of defects in two-dimensional crystals on curved backgrounds.

Abstract:Point defects are ubiquitous in two-dimensional crystals and play a fundamental role in determining their mechanical and thermodynamical properties. When crystals are formed on a curved background, finite-length grain boundaries (scars) are generally needed to stabilize the crystal. We provide a continuum elasticity analysis of defect dynamics in curved crystals. By exploiting the fact that any point defect can be obtained as an appropriate combination of disclinations, we provide an analytical determination of the elastic spring constants of dislocations within scars and compare them with existing experimental measurements from optical microscopy. We further show that vacancies and interstitials, which are stable defects in flat crystals, are generally unstable in curved geometries. This observation explains why vacancies or interstitials are never found in equilibrium spherical crystals. We finish with some further implications for experiments and future theoretical work.
Treatment of Atlantoaxial Instability with C1-C2 Posterior Transarticular Screw Fixation

Abstract:OBJECTIVE:To evaluate the technique of C1-C2 transarticular screw fixation for atlantoaxial instability or dislocation.METHODS:Among 14 patients with atlantoaxial instability, 10 were male and 4 patients were female, with an average age of 38.6 years ranging from 17 to 62 years. All patients were treated by internal fixation with transarticular screws.RESULTS:There were 28 screws applied in 14 patients. All patients were followed up. The average follow-up period was 16 months (range, 9 to 35 months). The postoperative JOA score was ranging from 13.8 to 15.8 with the average score of (14.50 +/- 0.66) and the improved rate of (76.12 +/- 4.94)%. No spinal injury and vertical artery injury was found and osseous fusion was completed in all patients.CONCLUSION:The technique of C1-C2 transarticular screw fixation is one of the best treatments for atlantoaxial instability. Without the help of structural bone graft and aided internal fixation, morselized cancellous bone graft can acquire effective osseous fusion.
The Clinical Study of Minimally Invasive Percutaneous New Special C2 Transpedicular Lag-Screw Fixation for Treatment of Hangman's Fracture

Abstract:Objective To evaluate the clinical effects of minimally invasive percutaneous new C2 transpedicular lag-screw fixation for management of Hangman's fractures. Methods From October 2015 to June 2017, 25 patients who had Hangman's fracture were operated with minimally invasive percutaneous new C2 transpedicular lag-screw fixation assisted by infrared navigation, among them there were 18 male and 7 female, aged from 28 to 70 years [average (46.1±13.3) years]. According to the Levine-Edwards classification: 16 patients were TypeⅠ, 6 patients were TypeⅡ and 3 patients were TypeⅡA. The cause of the injury was road traffic accident in 20 patients and fall from height in 5 patients. 19 patients were simple Hangman's fracture and other associated lesions included odontoid fracture of TypeⅡ(2 patients), atlas fracture of TypeⅠ (2 patients), rupture of spleen (1 patient), and rib fractures (1 patient). According to American Association for spinal cord injury, graded as spinal cord injury D in 3 cases and E in 22 cases. The clinical outcomes were evaluated by visual analog scale (VAS) scores and related indicators, such as operation time, the range of motion (ROM) of cervical spine and ratio of bony union were recorded. The pre- and post- operative VAS scores of neck pain were compared with repeated measures analysis of variance. Results The mean follow-up time was (12.5±6.0) months, ranging from 3 to 24 months. Satisfactory reduction was obtained in all cases (minimal translation ≤ 2 mm without obvious C2,3 angulation). The average operation time was (65.9±12.1) min. At first day, one month, three months and the last follow-up, VAS scores of neck pain decreased comparing with preoperational measurements. There was significant difference between the pre-and post- operative VAS scores of neck pain (t=24.7, 25.8, 23.1, 24.1, P<0.001). According to American Association for spinal cord injury, three cases restored from D to E. There was no spinal cord or vertebral artery injury during operation and there was no screw loosing or breakage and cervical malformation happened during follow-up period. Bony fusion was achieved in all of these cases and the range of neck rotation was stored normal at the last follow-up. The average time of Bony fusion was (3.4±0.8) months. Conclusion The primary clinical efficacies of minimally invasive percutaneous new C2 transpedicular lag-screw fixation for treatment of Hangman's fracture were satisfactory, which carries precision and safety, immediate stability and more reliable. Is Anterior Cervical Discectomy and Fusion Sufficient for Neglected Unstable Hangman's Fracture? A Retrospective Case Series of Five Patients

Abstract:Introduction: This study investigates the outcomes of treating neglected unstable Hangman's fractures through a single-stage Anterior Cervical Discectomy and Fusion (ACDF) procedure with tricortical iliac crest bone grafts. Methods: Five patients with neglected unstable Hangman's fractures, treated at our institution between March 2012 and March 2017, underwent C2-C3 ACDF. Functional outcomes were assessed using the Visual Analog Scale (VAS) score and Neck Disability Index (NDI), and neurological evaluation was done using the American Spinal Injury Association (ASIA) grading system. The radiological assessment included serial plain radiographs and a computed tomography scan at a 12-month follow-up. Results: Postoperatively, C2-C3 angulation improved significantly, decreasing from 15° to 4.4°, and sagittal translation improved from 4.2 mm to 2 mm. The VAS score improved from 6.4 to 1.4 at 24 months postsurgery. Concurrently, NDI decreased from 70.4% to 14.8%. Fusion occurred in an average of 5.6 months. Neurologically, one patient improved from ASIA grade D to grade E, while the other four retained their grade E status. Conclusions: A single-stage ACDF with autologous iliac crest bone grafts is an effective surgical option for neglected type II/IIA Hangman's fractures, yielding satisfactory functional and radiological outcomes. This technique significantly corrects anterior translation and angulation, even in neglected cases, with the aid of intraoperative skull traction and plate reduction.
Combined anterior odontoid screw fixation and C1-C2 Jefferson fracture anterior fixation: a step-by-step technical note

Abstract:Background Combined triple C1-C2 fixation has been described in previous literature as a safe, effective, and minimally invasive procedure for complex atlas and odontoid fractures that allows for a greater range of motion compared to posterior approaches and atlanto-occipital fusion but is performed scarcely due to the occipital-cervical diastasis resulting from often-fractured C1 joint masses. There is no evidence-based consensus regarding treating complex atlantoaxial fractures, and surgical strategies are only based on clinical experience. Methods We report the combined triple C1-C2 fixation technique with manual reduction of the joint masses during patient positioning on the operating table, thus allowing effective stabilization in a single surgical time. We describe our experience in the management of a 75-year-old patient presenting with an acute complex type II fracture of C1, also involving one lateral mass combined with a type II odontoid fracture and occipital-cervical diastasis. Results We provide a step-by-step guide to combined triple C1-C2 anterior fixation with manual fracture reduction and describe the clinical case of an acute complex type II fracture of C1 also involving one lateral mass, combined with a type II odontoid fracture and occipital-cervical diastasis. Conclusion The combined triple C1-C2 fixation represents a safe and efficient minimally invasive anterior approach for complex type II fractures of C1 with type II odontoid fractures. Manual reduction of the joint masses during patient positioning allows effective stabilization in a single surgical time.

[Surgical treatment of atypical unstable Hangman fracture in elderly osteoporotic patients]

Abstract:Objective: To explore the clinical effect of operative reduction and internal fixation in treating atypical unstable Hangman fracture in elderly osteoporotic patients. Methods: The clinical data of 9 elderly osteoporotic patients with atypical unstable Hangman fracture treated from December 2012 to December 2017 were retrospectively analyzed. There were 5 males and 4 females with an average age of 72.7 years (range 65 to 79) old. The fracture line was located on bilateral inferior articular surface or lamina with anterior dislocation of axis in 3 cases (similar to Levine-Edwards type II); one side of the fracture line was located on the lower articular surface and opposite side involved on the upper articular surface and the body of axis with atlantoaxial lateral mass dislocation in 4 cases; the fracture line was located at the pedicle of the axis, transverse foramen and posterior inferior wall of the vertebral body, accompanying by anterior and posterior longitudinal ligaments and C₂-C₃ intervertebral disc injury resulted in C₂-C₃ intervertebral instability in 2 cases. According to ASIA grade of spinal cord injury, 2 cases were grade D and 7 cases were grade E. Preoperative JOA scores were 9 to 11 points with an average of 9.8 points. All patients underwent routinely skull traction for reduction. Three patients with anterior dislocation of axis were treated with stage I anterior-posterior internal fixation and 6 patients with simple posterior pedicle screw fixation. Postoperative JOA score, ASIA system and the imaging data were used to evaluate clinical effects. Results: All patients were followed up from 12 to 20 months with an average of 18 months. The neck pain disappeared after operation and cervical spine movement was well in all patients. The improvement rate of JOA at 3 months after operation was 75% to 87.5%. Two cases of ASIA grade D fully recovered to grade E. All fractures healed at 6 to 12 months after operation. Except one case occurred superior laryngeal nerve injury and had a complete recovery after 3 months, the other patients had no complications like spinal cord and vertebral artery injury, plate screw loosening and wound infection. Conclusions: According to the type of atypical unstable Hangman fracture in elderly osteoporotic patients, choosing a reasonable and personalized surgical plan can achieve immediate cervical stability and well reduction, which is beneficial for early ambulation and quick rehabilitation.
Surgical Options for Instability of Atlantoaxiai Complex Combined with Odontoid Fracture

Abstract:Objective To investigate 3 operative treatments and their clinical outcomes for instability of atlantoaxial complex combined with odontoid fracture.Methods From January 2002 to December 2011,54 cases of instability of atlantoaxial complex combined with odontoid fracture were treated surgically in our department.They were 39 males and 15 females,with a mean age of 36 years (range,18 to 55 years).Of them,5 fractures were obsolete and 19 fresh.According to the classification of Anderson-D'Alonzon,30 cases were type Ⅱ and 24 type Ⅲ,12 cases had atlas anterior dislocation,12 atlas posterior dislocation and 30 no obvious dislocation.The preoperative neural function by the American Spinal Injury Association (ASIA) system:grade B in 2,grade C in 5,grade D in 16 and grade E in 31 cases.According to the fracture type and individual status,30 patients underwent internal fixation with anterior odontoid cannulated screws (group A),8 patients had internal fixation with anterior C1-C2 transarticular screws (group B)and 16 patients received internal fixation with posterior C1-C2 screw-rods (group C).Results No intraoperative injury to the vertebral artery or the spinal cord occurred.The patients with atlas dislocation obtained fine reduction post-operation.The mean time of follow-up was 16 months (range,6 to 24 months).All the patients got bony union 3 to 4 months after operation except for the 2 cases in group A who showed nonunion 6 months after operation without clinical symptoms.No instrument failure was found at the final follow-up.According to the ASIA grading 6 months after operation,2 cases were rated as grade C,6 as grade D and 46 as grade E.Conclusions The above 3 surgical treatments are all effective for instability of atlantoaxial complex combined with odontoid fracture.The internal fixation with anterior odontoid cannulated screws should be the first option.Only when it is inappropriate,fixation with anterior C1-C2 transarticular screws and fixation with posterior C1-C2 screw-rods can be taken into consideration.

Abstract:Background context Odontoid fractures are among the most common cervical spine fractures in the elderly and are associated with increased morbidity and mortality. Clinical evidence suggests improved survival and quality of life after operative intervention compared to nonoperative treatment. Purpose This study seeks to examine the stability of an osteoporotic Type II odontoid fracture following posterior atlantoaxial fixation with either the Magerl transarticular fixation technique or the Harms C1 lateral mass screws C2 pedicle screw rod fixation. Study design Biomechanical cadaveric study Methods Eighteen cadaveric specimens extending from the cephalus to C7 were used in this study. Reflective marker arrays were attached to C1 and C2 and a single marker on the dens to measure movement of each during loading with C2-C3 and occiput-C1 being allowed to move freely. A biomechanical testing protocol imparted moments in flexion-extension, axial rotation, and lateral bending while a motion capture system recorded the motions of C1, C2 and the dens. The spines were instrumented with either the Harms fixation (n=9) or Magerl fixation (n=9) techniques, and a simulated Type II odontoid fracture was created. Motions of each instrumented spine were recorded for all moments, and then again after the instrumentations was removed to model the injured, non-instrumented state. Results Both Harms and Magerl posterior C1-C2 fixation allowed for C1, C2, and the dens to move as a relative unit. Without fixation the dens motion was coupled with C1. No significant differences were found in X, Y, Z translation motion of the dens, C1 or C2 during neutral zone motions between the Magerl and Harms fixation techniques. There were no significant differences found in Euler angle motion between the two techniques in either flexion-extension, axial rotation, or lateral bending motion. Conclusions Our findings suggest that both Harms and Magerl fixation can significantly reduce dens motion in Type II odontoid fractures in an osteoporotic cadaveric bone model. Clinical significance Both Harms and Magerl posterior atlantoaxial fixation techniques allowed for C1, C2, and the dens to move as a relative unit following odontoid fracture, establishing more anatomic stability to the upper cervical spine.

Abstract:Background: C2 fractures can be classified differently when dens, pedicles or body are injured. With regards to the best management of Type-II Anderson-D'Alonzo fractures, Hangman's fractures of pedicles and C2 body fractures are more debatable. However, vertebral pedicle and/or articular screw and dorsal wiring are the most common surgical posterior approaches opted for. Compared to the screw technique, dorsal wiring provides certain benefits such as a lower risk of vertebral artery injury, no need for navigation, less lateral dissection of the paraspinal muscles, shorter surgery time and lower medical costs.Case Description: Two patients with failed conservative treatment for Anderson-D'Alonzo Type-III fractures (Cases 1A and 1B), a patient suffering from a Type-II Hangman's fracture (Case 2) and a patient with failed conservative treatment for a C2 transversal body fracture (Case 3) underwent surgery at the Neurosurgery Division of the University Hospital of Modena (Italy) between July 2020 and September 2021. All patients were treated with posterior wiring with 5 mm Polyester bands, titanium-peek fixation system (Jazz-Lock system MediNext<sup>®</sup>-Implanet) inserted through the C1 posterior arch and either the C2 or C3 laminae. A fracture diastasis reduction was observed ranging between 4.5 and 1 mm. No intraoperative and post-operative complications were encountered. The duration of the period of hospitalisation ranged between 5 and 12 days. All patients who had worked prior to the traumatic event were able to return to work 18 months following surgery.Conclusions: In reducing C2 fractures, a sublaminar fixation with polyester bands and a titanium-peek fixation system can be proposed for fragile and elderly patients.

Abstract:This study aims to investigate the feasibility and efficacy of anterior atlantoaxial motion preservation fixation (AMPF) in treating axis complex fractures involving the odontoid process fracture and Hangman's fractures with C2/3 instability. A retrospective study was conducted on eight patients who underwent AMPF for axis complex fractures at the General Hospital of Central Theater Command from February 2004 to October 2021. The types of axis injuries, reasons for injuries, surgery time, intraoperative blood loss, spinal cord injury classification (American Spinal Injury Association, ASIA), as well as complications and technical notes, were documented. This study included eight cases of type II Hangman's fracture, five cases of type II and three cases of type III odontoid process fracture. Five patients experienced traffic accidents, while three patients experienced falling injuries. All patients underwent AMPF surgery with an average intraoperative blood loss of 288.75 mL and a duration of 174.5 min. Two patients experienced dysphagia 1 month after surgery. The patients were followed up for an average of 15.63 months. One case improved from C to E in terms of neurological condition, three cases improved from D to E, and four cases remained at E. Bony fusion and Atlantoaxial Motion Preservation were successfully achieved for all eight patients. AMPF is a feasible and effective way for simultaneous odontoid process fracture and Hangman's fractures with C2/3 instability, while preserving atlantoaxial movement.
