Abstract:The utilization of vast amounts of EHR data is crucial to the studies in medical informatics. Physicians are medical participants who directly record clinical data into EHR with their personal expertise, making their roles essential in follow-up data utilization, which current studies have yet to recognize. This paper proposes a physician-centered perspective for EHR data utilization and emphasizes the feasibility and potentiality of digging into physicians' latent decision patterns in EHR. To support our proposal, we design a physician-centered CDS approach named PhyC and test it on a real-world EHR dataset. Experiments show that PhyC performs significantly better in the auxiliary diagnosis of multiple diseases than globally learned models. Discussions on experimental results suggest that physician-centered data utilization can help to derive more objective CDS models, while more means for utilization need further exploration.

Abstract:Background: Increasing digitalization in the medical domain gives rise to large amounts of health care data, which has the potential to expand clinical knowledge and transform patient care if leveraged through artificial intelligence (AI). Yet, big data and AI oftentimes cannot unlock their full potential at scale, owing to nonstandardized data formats, lack of technical and semantic data interoperability, and limited cooperation between stakeholders in the health care system. Despite the existence of standardized data formats for the medical domain, such as Fast Healthcare Interoperability Resources (FHIR), their prevalence and usability for AI remain limited. Objective: In this paper, we developed a data harmonization pipeline (DHP) for clinical data sets relying on the common FHIR data standard. Methods: We validated the performance and usability of our FHIR-DHP with data from the Medical Information Mart for Intensive Care IV database. Results: We present the FHIR-DHP workflow in respect of the transformation of "raw" hospital records into a harmonized, AI-friendly data representation. The pipeline consists of the following 5 key preprocessing steps: querying of data from hospital database, FHIR mapping, syntactic validation, transfer of harmonized data into the patient-model database, and export of data in an AI-friendly format for further medical applications. A detailed example of FHIR-DHP execution was presented for clinical diagnoses records. Conclusions: Our approach enables the scalable and needs-driven data modeling of large and heterogenous clinical data sets. The FHIR-DHP is a pivotal step toward increasing cooperation, interoperability, and quality of patient care in the clinical routine and for medical research.

Abstract:Precision medicine—of which precision prescribing is a core component—is becoming a new frontier in today's healthcare. Both artificial intelligence (AI) and machine learning (ML) have the potential to enhance our understanding of data and therefore our ability to accurately diagnose and treat patients. By leveraging these technologies and processes, we can uncover associations between a person's genomic makeup and their health, identify biomarkers associated with diseases, fine-tune patient selection for clinical trials, reduce costs, and accelerate drug discovery and vaccine development. Although real-world data pose challenges in terms of collection, representation, and missing or inaccurate data sets, the integration of precision medicine into healthcare is critical. Clearly, precision medicine can benefit from health information innovations that empower decision-making at the patient level. Healthcare fusion is an example of an innovative framework and process [K Zhai et al., 2022, pp. 179–192] [K Zhai et al. ECKM 2022, 20(3), pp. 179–192]. Data science and process improvement are also expected to play a role in resource planning and operational efficiency for optimal patient-centered care. Driving this transformation are advances in 'omics' technologies, digital devices, and imaging capabilities, along with an arsenal of powerful analytics tools working across a multitude of institutions and stakeholders. Encompassing this entire ecosystem, medicine will be evidence-based and driven by three key components: (1) Data curation through clinical diagnostics and behavioral apps that capture health and disease states; (2) Individualized solutions driven by advanced data analytics and personalized therapies; and (3) Business models that deliver value and incentivize growth. The aim of this paper is to present a novel conceptual framework to leverage AI and enhance information flow to serve the patient as per components one and two.

Abstract:Background The harmonization and standardization of digital medical information for research purposes is a challenging and ongoing collaborative effort. Current research data repositories typically require extensive efforts in harmonizing and transforming original clinical data. The Fast Healthcare Interoperability Resources (FHIR) format was designed primarily to represent clinical processes; therefore, it closely resembles the clinical data model and is more widely available across modern electronic health records. However, no common standardized data format is directly suitable for statistical analyses, and data need to be preprocessed before statistical analysis. Objective This study aimed to elucidate how FHIR data can be queried directly with a preprocessing service and be used for statistical analyses. Methods We propose that the binary JavaScript Object Notation format of the PostgreSQL (PSQL) open source database is suitable for not only storing FHIR data, but also extending it with preprocessing and filtering services, which directly transform data stored in FHIR format into prepared data subsets for statistical analysis. We specified an interface for this preprocessor, implemented and deployed it at University Hospital Erlangen-Nürnberg, generated 3 sample data sets, and analyzed the available data. Results We imported real-world patient data from 2016 to 2018 into a standard PSQL database, generating a dataset of approximately 35.5 million FHIR resources, including “Patient,” “Encounter,” “Condition” (diagnoses specified using International Classification of Diseases codes), “Procedure,” and “Observation” (laboratory test results). We then integrated the developed preprocessing service with the PSQL database and the locally installed web-based KETOS analysis platform. Advanced statistical analyses were feasible using the developed framework using 3 clinically relevant scenarios (data-driven establishment of hemoglobin reference intervals, assessment of anemia prevalence in patients with cancer, and investigation of the adverse effects of drugs). Conclusions This study shows how the standard open source database PSQL can be used to store FHIR data and be integrated with a specifically developed preprocessing and analysis framework. This enables dataset generation with advanced medical criteria and the integration of subsequent statistical analysis. The web-based preprocessing service can be deployed locally at the hospital level, protecting patients’ privacy while being integrated with existing open source data analysis tools currently being developed across Germany.

Abstract:Background: In a multisite clinical research collaboration, institutions may or may not use the same common data model (CDM) to store clinical data. To overcome this challenge, we proposed to use Health Level 7's Fast Healthcare Interoperability Resources (FHIR) as a meta-CDM-a single standard to represent clinical data. Objective: In this study, we aimed to create an open-source application termed the Clinical Asset Mapping Program for FHIR (CAMP FHIR) to efficiently transform clinical data to FHIR for supporting source-agnostic CDM-to-FHIR mapping. Methods: Mapping with CAMP FHIR involves (1) mapping each source variable to its corresponding FHIR element and (2) mapping each item in the source data's value sets to the corresponding FHIR value set item for variables with strict value sets. To date, CAMP FHIR has been used to transform 108 variables from the Informatics for Integrating Biology & the Bedside (i2b2) and Patient-Centered Outcomes Research Network data models to fields across 7 FHIR resources. It is designed to allow input from any source data model and will support additional FHIR resources in the future. Results: We have used CAMP FHIR to transform data on approximately 23,000 patients with asthma from our institution's i2b2 database. Data quality and integrity were validated against the origin point of the data, our enterprise clinical data warehouse. Conclusions: We believe that CAMP FHIR can serve as an alternative to implementing new CDMs on a project-by-project basis. Moreover, the use of FHIR as a CDM could support rare data sharing opportunities, such as collaborations between academic medical centers and community hospitals. We anticipate adoption and use of CAMP FHIR to foster sharing of clinical data across institutions for downstream applications in translational research.

Abstract:Purpose of Review:This article addresses the occurrence of acute symptomatic seizures in the setting of many medical illnesses. Recent Findings:Many medical illnesses can cause seizures. Metabolic derangements, such as disorders of serum glucose metabolism, cause seizures, as well as other neurologic manifestations. Many medicines (such as antibiotics, antivirals, antidepressants, and antipsychotics), whether used at typical doses or in overdosage, can lead to acute symptomatic seizures. Summary:Acute symptomatic seizures occur most often in illnesses that directly injure the brain. Trauma, stroke, CNS tumors, and CNS infection very commonly cause seizures. However, many medical illnesses do not directly injure the brain but lead to neurologic signs and symptoms, such as seizures. Recognizing these effects, especially in critically ill patients, is important for optimizing medical care.
Interoperable synthetic health data with SyntHIR to enable the development of CDSS tools

Abstract:There is a great opportunity to use high-quality patient journals and health registers to develop machine learning-based Clinical Decision Support Systems (CDSS). To implement a CDSS tool in a clinical workflow, there is a need to integrate, validate and test this tool on the Electronic Health Record (EHR) systems used to store and manage patient data. However, it is often not possible to get the necessary access to an EHR system due to legal compliance. We propose an architecture for generating and using synthetic EHR data for CDSS tool development. The architecture is implemented in a system called SyntHIR. The SyntHIR system uses the Fast Healthcare Interoperability Resources (FHIR) standards for data interoperability, the Gretel framework for generating synthetic data, the Microsoft Azure FHIR server as the FHIR-based EHR system and SMART on FHIR framework for tool transportability. We demonstrate the usefulness of SyntHIR by developing a machine learning-based CDSS tool using data from the Norwegian Patient Register (NPR) and Norwegian Patient Prescriptions (NorPD). We demonstrate the development of the tool on the SyntHIR system and then lift it to the Open DIPS environment. In conclusion, SyntHIR provides a generic architecture for CDSS tool development using synthetic FHIR data and a testing environment before implementing it in a clinical setting. However, there is scope for improvement in terms of the quality of the synthetic data generated. Abstract:Healthcare faces significant challenges in exchanging and utilizing health information across diverse providers, necessitating innovative solutions for improved interoperability. This study presents a comprehensive exploration of scalable technical and semantic solutions for patient care integration, emphasizing the implementation of these solutions within the framework of the Fast Healthcare Interoperability Resources (FHIR) standard. Our approach revolves around the development and deployment of Technical Interoperability Suite (TIS) and Semantic Interoperability Suite (SIS) technology solutions to disparate health information systems, predominantly Electronic Health Records (EHRs) into a unified Patient Care Platform, fostering comprehensive data exchange and utilization. The integration process involves importing data from various EHR systems and transforming imported patient data into FHIR-standardized formats. The provided solution supports various functionalities, including automatic and manual importation of patient data, through standard computer-readable templates. The integration of TIS and SIS solutions is underpinned by a robust technological framework, incorporating technologies such as Typescript, Deno, and document-oriented databases such as MongoDB. The effectiveness of our interoperability solutions was validated through deployment in multinational EU projects: ADLIFE and CAREPATH. The scalability and generalizability of our approach underscore its potential for diverse healthcare settings.
Data integration between clinical research and patient care: A framework for context-depending data sharing and in silico predictions

Abstract:The transfer of new insights from basic or clinical research into clinical routine is usually a lengthy and time-consuming process. Conversely, there are still many barriers to directly provide and use routine data in the context of basic and clinical research. In particular, no coherent software solution is available that allows a convenient and immediate bidirectional transfer of data between concrete treatment contexts and research settings. Here, we present a generic framework that integrates health data (e.g., clinical, molecular) and computational analytics (e.g., model predictions, statistical evaluations, visualizations) into a clinical software solution which simultaneously supports both patient-specific healthcare decisions and research efforts, while also adhering to the requirements for data protection and data quality. Specifically, our work is based on a recently established generic data management concept, for which we designed and implemented a web-based software framework that integrates data analysis, visualization as well as computer simulation and model prediction with audit trail functionality and a regulation-compliant pseudonymization service. Within the front-end application, we established two tailored views: a clinical (i.e., treatment context) perspective focusing on patient-specific data visualization, analysis and outcome prediction and a research perspective focusing on the exploration of pseudonymized data. We illustrate the application of our generic framework by two use-cases from the field of haematology/oncology. Our implementation demonstrates the feasibility of an integrated generation and backward propagation of data analysis results and model predictions at an individual patient level into clinical decision-making processes while enabling seamless integration into a clinical information system or an electronic health record.
Preparing healthcare leaders of the digital age with an integrative artificial intelligence curriculum: a pilot study

Abstract:Artificial intelligence (AI) is rapidly being introduced into the clinical workflow of many specialties. Despite the need to train physicians who understand the utility and implications of AI and mitigate a growing skills gap, no established consensus exists on how to best introduce AI concepts to medical students during preclinical training. This study examined the effectiveness of a pilot Digital Health Scholars (DHS) non-credit enrichment elective that paralleled the Dartmouth Geisel School of Medicine's first-year preclinical curriculum with a focus on introducing AI algorithms and their applications in the concurrently occurring systems-blocks. From September 2022 to March 2023, ten self-selected first-year students enrolled in the elective curriculum run in parallel with four existing curricular blocks (Immunology, Hematology, Cardiology, and Pulmonology). Each DHS block consisted of a journal club, a live-coding demonstration, and an integration session led by a researcher in that field. Students' confidence in explaining the content objectives (high-level knowledge, implications, and limitations of AI) was measured before and after each block and compared using Mann-Whitney U tests. Students reported significant increases in confidence in describing the content objectives after all four blocks (Immunology: U = 4.5, p = 0.030; Hematology: U = 1.0, p = 0.009; Cardiology: U = 4.0, p = 0.019; Pulmonology: U = 4.0, p = 0.030) as well as an average overall satisfaction level of 4.29/5 in rating the curriculum content. Our study demonstrates that a digital health enrichment elective that runs in parallel to an institution's preclinical curriculum and embeds AI concepts into relevant clinical topics can enhance students' confidence in describing the content objectives that pertain to high-level algorithmic understanding, implications, and limitations of the studied models. Building on this elective curricular design, further studies with a larger enrollment can help determine the most effective approach in preparing future physicians for the AI-enhanced clinical workflow.

Abstract:Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on the German Medical Informatics Initiative. Future medicine will be predictive, preventive, personalized, participatory and digital. Data and knowledge at comprehensive depth and breadth need to be available for research and at the point of care as a basis for targeted diagnosis and therapy. Data integration and data sharing will be essential to achieve these goals. For this purpose, the consortium Data Integration for Future Medicine (DIFUTURE) will establish Data Integration Centers (DICs) at university medical centers. Objectives: The infrastructure envisioned by DIFUTURE will provide researchers with cross-site access to data and support physicians by innovative views on integrated data as well as by decision support components for personalized treatments. The aim of our use cases is to show that this accelerates innovation, improves health care processes and results in tangible benefits for our patients. To realize our vision, numerous challenges have to be addressed. The objective of this article is to describe our concepts and solutions on the technical and the organizational level with a specific focus on data integration and sharing. Governance and policies: Data sharing implies significant security and privacy challenges. Therefore, state-of-the-art data protection, modern IT security concepts and patient trust play a central role in our approach. We have established governance structures and policies safeguarding data use and sharing by technical and organizational measures providing highest levels of data protection. One of our central policies is that adequate methods of data sharing for each use case and project will be selected based on rigorous risk and threat analyses. Interdisciplinary groups have been installed in order to manage change. Architectural framework and methodology: The DIFUTURE Data Integration Centers will implement a three-step approach to integrating, harmonizing and sharing structured, unstructured and omics data as well as images from clinical and research environments. First, data is imported and technically harmonized using common data and interface standards (including various IHE profiles, DICOM and HL7 FHIR). Second, data is preprocessed, transformed, harmonized and enriched within a staging and working environment. Third, data is imported into common analytics platforms and data models (including i2b2 and tranSMART) and made accessible in a form compliant with the interoperability requirements defined on the national level. Secure data access and sharing will be implemented with innovative combinations of privacy-enhancing technologies (safe data, safe settings, safe outputs) and methods of distributed computing. Use cases: From the perspective of health care and medical research, our approach is disease-oriented and use-case driven, i.e. following the needs of physicians and researchers and aiming at measurable benefits for our patients. We will work on early diagnosis, tailored therapies and therapy decision tools with focuses on neurology, oncology and further disease entities. Our early uses cases will serve as blueprints for the following ones, verifying that the infrastructure developed by DIFUTURE is able to support a variety of application scenarios. Discussion: Own previous work, the use of internationally successful open source systems and a state-of-the-art software architecture are cornerstones of our approach. In the conceptual phase of the initiative, we have already prototypically implemented and tested the most important components of our architecture.
Enabling scalable clinical interpretation of ML-based phenotypes using real world data

Abstract:The availability of large and deep electronic healthcare records (EHR) datasets has the potential to enable a better understanding of real-world patient journeys, and to identify novel subgroups of patients. ML-based aggregation of EHR data is mostly tool-driven, i.e., building on available or newly developed methods. However, these methods, their input requirements, and, importantly, resulting output are frequently difficult to interpret, especially without in-depth data science or statistical training. This endangers the final step of analysis where an actionable and clinically meaningful interpretation is <a class="link-external link-http" href="http://needed.This" rel="external noopener nofollow">this http URL</a> study investigates approaches to perform patient stratification analysis at scale using large EHR datasets and multiple clustering methods for clinical research. We have developed several tools to facilitate the clinical evaluation and interpretation of unsupervised patient stratification results, namely pattern screening, meta clustering, surrogate modeling, and curation. These tools can be used at different stages within the analysis. As compared to a standard analysis approach, we demonstrate the ability to condense results and optimize analysis time. In the case of meta clustering, we demonstrate that the number of patient clusters can be reduced from 72 to 3 in one example. In another stratification result, by using surrogate models, we could quickly identify that heart failure patients were stratified if blood sodium measurements were available. As this is a routine measurement performed for all patients with heart failure, this indicated a data bias. By using further cohort and feature curation, these patients and other irrelevant features could be removed to increase the clinical meaningfulness. These examples show the effectiveness of the proposed methods and we hope to encourage further research in this field.

Abstract:Artificial Intelligence (AI) plays a crucial role in medical field and has the potential to revolutionize healthcare practices. However, the success of AI models and their impacts hinge on the synergy between AI and medical specialists, with clinicians assuming a dominant role. Unfortunately, the intricate dynamics and interactions between AI and clinicians remain undiscovered and thus hinder AI from being translated into medical practice. To address this gap, we have curated a groundbreaking database called AI.vs.Clinician. This database is the first of its kind for studying the interactions between AI and clinicians. It derives from 7,500 collaborative diagnosis records on a life-threatening medical emergency -- Sepsis -- from 14 medical centers across China. For the patient cohorts well-chosen from MIMIC databases, the AI-related information comprises the model property, feature input, diagnosis decision, and inferred probabilities of sepsis onset presently and within next three hours. The clinician-related information includes the viewed examination data and sequence, viewed time, preliminary and final diagnosis decisions with or without AI assistance, and recommended treatment.

Despite the abundance of medical data across various facilities - including hospitals, outpatient clinics, and physician practices - the intersectoral exchange of information remains largely hindered due to differences in data structure, content, and adherence to data protection regulations. In response to this challenge, the Medical Informatics Initiative (MII) was launched in Germany, focusing initially on university hospitals to foster the exchange and utilization of real-world data through the development of standardized methods and tools, including the creation of a common core dataset. Our aim, as part of the Medical Informatics Research Hub in Saxony (MiHUBx), is to extend the MII concepts to non-university healthcare providers in a more seamless manner to enable the exchange of real-world data among intersectoral medical sites. Methods: We investigated what services are needed to facilitate the provision of harmonized real-world data for cross-site research. On this basis, we designed a Service Platform Prototype that hosts services for data harmonization, adhering to the globally recognized Health Level 7 (HL7) Fast Healthcare Interoperability Resources (FHIR) international standard communication format and the Observational Medical Outcomes Partnership (OMOP) common data model (CDM). Leveraging these standards, we implemented additional services facilitating data utilization, exchange and analysis. Throughout the development phase, we collaborated with an interdisciplinary team of experts from the fields of system administration, software engineering and technology acceptance to ensure that the solution is sustainable and reusable in the long term. Results: We have developed the pre-built packages "ResearchData-to-FHIR," "FHIR-to-OMOP," and "Addons," which provide the services for data harmonization and provision of project-related real-world data in both the FHIR MII Core dataset format (CDS) and the OMOP CDM format as well as utilization and a Service Platform Prototype to streamline data management and use. Conclusion: Our development shows a possible approach to extend the MII concepts to non-university healthcare providers to enable cross-site research on real-world data. Our Service Platform Prototype can thus pave the way for intersectoral data sharing, federated analysis, and provision of SMART-on-FHIR applications to support clinical decision making.

Abstract:Collaborative intelligence reflects the promise and limits of leveraging artificial intelligence (AI) technologies in clinical care. It involves the use of advanced analytics and computing power with an understanding that humans bear responsibility for the accuracy, completeness and any inherent bias found in the training data. Clinicians benefit from using this technology to address increased complexity and information overload, support continuous care and optimized resource allocation, and to enact efforts to eradicate disparities in health care access and quality. This requires active clinician engagement with the technology, a general understanding of how the machine produced its insight, the limitations of the algorithms, and the need to screen datasets for bias. Importantly, by interacting, the clinician and the analytics will create trust based on the clinician's critical thinking skills leveraged to discern value of machine outputs within clinical context. Utilization of collaborative intelligence should be staged with the level of understanding and evidence. It is particularly well suited to low-complexity non-urgent care and to identifying individuals at rising risk within a population. Clinician involvement in algorithm development and the amassing of evidence to support safety and efficacy will propel adoption. Utilization of collaborative intelligence represents the natural progression of health care innovation, and if thoughtfully constructed and equitably deployed, holds the promise to decrease clinician burden and improve access to care.

Abstract:Objectives: Artificial intelligence (AI), including predictive analytics, has great potential to improve the care of common chronic conditions with high morbidity and mortality. However, there are still many challenges to achieving this vision. The goal of this project was to develop and apply methods for enhancing chronic disease care using AI. Methods: Using a dataset of 27,904 patients with diabetes, an analytical method was developed and validated for generating a treatment pathway graph which consists of models that predict the likelihood of alternate treatment strategies achieving care goals. An AI-driven clinical decision support system (CDSS) integrated with the electronic health record (EHR) was developed by encapsulating the prediction models in an OpenCDS Web service module and delivering the model outputs through a SMART on FHIR (Substitutable Medical Applications and Reusable Technologies on Fast Healthcare Interoperability Resources) web-based dashboard. This CDSS enables clinicians and patients to review relevant patient parameters, select treatment goals, and review alternate treatment strategies based on prediction results. Results: The proposed analytical method outperformed previous machine-learning algorithms on prediction accuracy. The CDSS was successfully integrated with the Epic EHR at the University of Utah. Conclusion: A predictive analytics-based CDSS was developed and successfully integrated with the EHR through standards-based interoperability frameworks. The approach used could potentially be applied to many other chronic conditions to bring AI-driven CDSS to the point of care.
