Abstract:Healthcare faces significant challenges in exchanging and utilizing health information across diverse providers, necessitating innovative solutions for improved interoperability. This study presents a comprehensive exploration of scalable technical and semantic solutions for patient care integration, emphasizing the implementation of these solutions within the framework of the Fast Healthcare Interoperability Resources (FHIR) standard. Our approach revolves around the development and deployment of Technical Interoperability Suite (TIS) and Semantic Interoperability Suite (SIS) technology solutions to disparate health information systems, predominantly Electronic Health Records (EHRs) into a unified Patient Care Platform, fostering comprehensive data exchange and utilization. The integration process involves importing data from various EHR systems and transforming imported patient data into FHIR-standardized formats. The provided solution supports various functionalities, including automatic and manual importation of patient data, through standard computer-readable templates. The integration of TIS and SIS solutions is underpinned by a robust technological framework, incorporating technologies such as Typescript, Deno, and document-oriented databases such as MongoDB. The effectiveness of our interoperability solutions was validated through deployment in multinational EU projects: ADLIFE and CAREPATH. The scalability and generalizability of our approach underscore its potential for diverse healthcare settings.

Abstract:HL7 Fast Healthcare Information Resources (FHIR) is rapidly becoming the de-facto standard for the exchange of clinical and healthcare related information. Major EHR vendors and healthcare providers are actively developing transformations between existing EHR databases and their corresponding FHIR representation. Many of these organizations are concurrently creating a second set of transformations from the same sources into integrated data repositories (IDRs). Considerable cost savings could be realized and overall quality could be improved were it possible to transformation primary FHIR EHR data directly into an IDR. We developed a FHIR to i2b2 transformation toolkit and evaluated the viability of such an approach.

Abstract:Introduction: Obtaining real-world data from routine clinical care is of growing interest for scientific research and personalized medicine. Despite the abundance of medical data across various facilities - including hospitals, outpatient clinics, and physician practices - the intersectoral exchange of information remains largely hindered due to differences in data structure, content, and adherence to data protection regulations. In response to this challenge, the Medical Informatics Initiative (MII) was launched in Germany, focusing initially on university hospitals to foster the exchange and utilization of real-world data through the development of standardized methods and tools, including the creation of a common core dataset. Our aim, as part of the Medical Informatics Research Hub in Saxony (MiHUBx), is to extend the MII concepts to non-university healthcare providers in a more seamless manner to enable the exchange of real-world data among intersectoral medical sites. Methods: We investigated what services are needed to facilitate the provision of harmonized real-world data for cross-site research. On this basis, we designed a Service Platform Prototype that hosts services for data harmonization, adhering to the globally recognized Health Level 7 (HL7) Fast Healthcare Interoperability Resources (FHIR) international standard communication format and the Observational Medical Outcomes Partnership (OMOP) common data model (CDM). Leveraging these standards, we implemented additional services facilitating data utilization, exchange and analysis. Throughout the development phase, we collaborated with an interdisciplinary team of experts from the fields of system administration, software engineering and technology acceptance to ensure that the solution is sustainable and reusable in the long term. Results: We have developed the pre-built packages "ResearchData-to-FHIR," "FHIR-to-OMOP," and "Addons," which provide the services for data harmonization and provision of project-related real-world data in both the FHIR MII Core dataset format (CDS) and the OMOP CDM format as well as utilization and a Service Platform Prototype to streamline data management and use. Conclusion: Our development shows a possible approach to extend the MII concepts to non-university healthcare providers to enable cross-site research on real-world data. Our Service Platform Prototype can thus pave the way for intersectoral data sharing, federated analysis, and provision of SMART-on-FHIR applications to support clinical decision making.

Abstract:In the age of intelligent gadgets and interconnected communities, the widespread surveillance and provision of healthcare to patients are made feasible through the Internet of Medical Things (IoMT). The number of implanted electronic devices that can be monitored remotely is increasing, leading to a rise in the amount and intricacy of biological data. The collected data can offer valuable diagnostic information that can be used to intervene and maintain implanted devices promptly, therefore enhancing the quality of treatment provided. Current remote monitoring processes are not fully using device diagnostics because of the lack of compatibility and data integration between exclusive applications and Digital Health Record (DHR) platforms. Establishing a technology framework that establishes information and enhances interoperability has the potential to strengthen remote monitoring. This article introduces the Medical Data Integration and Interoperability via Remote Monitoring of Healthcare Devices (MDII-RMHD) framework, which facilitates the collaboration of healthcare devices. MDII-RMHD enhances a cloud-based IoMT system using conversion resources at the network's boundary. This is achieved through the use of inquiring and conversion agents. The investigating devices keep track of a list of MDII-RMHD devices on the regional network and allow one device to seek information conversion from another gadget when the requesting device cannot do the task independently. The converting agent transforms the data into the necessary format and returns it to the original entity. These agents enable IoMT devices to utilize their surplus computational capabilities for data translations, reducing the need for cloud access. Conventional devices are compatible with fog resource managers that have MDII-RMHD support. We assess the MDII-RMHD framework by rigorous experimentation under different scenarios. The obtained results demonstrate that the proposed framework decreases the amount of uplink traffic and enhances the response time. This improvement in response time is significant for real-time healthcare applications.

Abstract:Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on the German Medical Informatics Initiative. Future medicine will be predictive, preventive, personalized, participatory and digital. Data and knowledge at comprehensive depth and breadth need to be available for research and at the point of care as a basis for targeted diagnosis and therapy. Data integration and data sharing will be essential to achieve these goals. For this purpose, the consortium Data Integration for Future Medicine (DIFUTURE) will establish Data Integration Centers (DICs) at university medical centers. Objectives: The infrastructure envisioned by DIFUTURE will provide researchers with cross-site access to data and support physicians by innovative views on integrated data as well as by decision support components for personalized treatments. The aim of our use cases is to show that this accelerates innovation, improves health care processes and results in tangible benefits for our patients. To realize our vision, numerous challenges have to be addressed. The objective of this article is to describe our concepts and solutions on the technical and the organizational level with a specific focus on data integration and sharing. Governance and policies: Data sharing implies significant security and privacy challenges. Therefore, state-of-the-art data protection, modern IT security concepts and patient trust play a central role in our approach. We have established governance structures and policies safeguarding data use and sharing by technical and organizational measures providing highest levels of data protection. One of our central policies is that adequate methods of data sharing for each use case and project will be selected based on rigorous risk and threat analyses. Interdisciplinary groups have been installed in order to manage change. Architectural framework and methodology: The DIFUTURE Data Integration Centers will implement a three-step approach to integrating, harmonizing and sharing structured, unstructured and omics data as well as images from clinical and research environments. First, data is imported and technically harmonized using common data and interface standards (including various IHE profiles, DICOM and HL7 FHIR). Second, data is preprocessed, transformed, harmonized and enriched within a staging and working environment. Third, data is imported into common analytics platforms and data models (including i2b2 and tranSMART) and made accessible in a form compliant with the interoperability requirements defined on the national level. Secure data access and sharing will be implemented with innovative combinations of privacy-enhancing technologies (safe data, safe settings, safe outputs) and methods of distributed computing. Use cases: From the perspective of health care and medical research, our approach is disease-oriented and use-case driven, i.e. following the needs of physicians and researchers and aiming at measurable benefits for our patients. We will work on early diagnosis, tailored therapies and therapy decision tools with focuses on neurology, oncology and further disease entities. Our early uses cases will serve as blueprints for the following ones, verifying that the infrastructure developed by DIFUTURE is able to support a variety of application scenarios. Discussion: Own previous work, the use of internationally successful open source systems and a state-of-the-art software architecture are cornerstones of our approach. In the conceptual phase of the initiative, we have already prototypically implemented and tested the most important components of our architecture.

Abstract:The electronic health record (EHR) targets the systematized collection of patient-specific electronically-stored health data. Currently the EHR is an evolving concept driven by ongoing technical developments and open or unclear legal issues concerning used medical technologies, data integration from other domains and unclear access roles. This paper addresses cross-domain data integration, data fusion and access control using the specific example of a Unique Device Identification (UDI) expanded hip implant. In fact, the integration of technical focus data into the hospital information system (HIS) is discussed and presented based on surgically relevant information. Moreover, the acquisition of social focus databased on mHealth is approached, which also covers data integration and networking with therapeutic intervention or acute diagnostics data. Data integration from heterogeneous domains is covered while using a data ecosystem with hierarchical access based on a shell embedded role model, which includes staggered access scenarios.
Data integration between clinical research and patient care: A framework for context-depending data sharing and in silico predictions

Abstract:The transfer of new insights from basic or clinical research into clinical routine is usually a lengthy and time-consuming process. Conversely, there are still many barriers to directly provide and use routine data in the context of basic and clinical research. In particular, no coherent software solution is available that allows a convenient and immediate bidirectional transfer of data between concrete treatment contexts and research settings. Here, we present a generic framework that integrates health data (e.g., clinical, molecular) and computational analytics (e.g., model predictions, statistical evaluations, visualizations) into a clinical software solution which simultaneously supports both patient-specific healthcare decisions and research efforts, while also adhering to the requirements for data protection and data quality. Specifically, our work is based on a recently established generic data management concept, for which we designed and implemented a web-based software framework that integrates data analysis, visualization as well as computer simulation and model prediction with audit trail functionality and a regulation-compliant pseudonymization service. Within the front-end application, we established two tailored views: a clinical (i.e., treatment context) perspective focusing on patient-specific data visualization, analysis and outcome prediction and a research perspective focusing on the exploration of pseudonymized data. We illustrate the application of our generic framework by two use-cases from the field of haematology/oncology. Our implementation demonstrates the feasibility of an integrated generation and backward propagation of data analysis results and model predictions at an individual patient level into clinical decision-making processes while enabling seamless integration into a clinical information system or an electronic health record.

Abstract:Background: Increasing digitalization in the medical domain gives rise to large amounts of health care data, which has the potential to expand clinical knowledge and transform patient care if leveraged through artificial intelligence (AI). Yet, big data and AI oftentimes cannot unlock their full potential at scale, owing to nonstandardized data formats, lack of technical and semantic data interoperability, and limited cooperation between stakeholders in the health care system. Despite the existence of standardized data formats for the medical domain, such as Fast Healthcare Interoperability Resources (FHIR), their prevalence and usability for AI remain limited. Objective: In this paper, we developed a data harmonization pipeline (DHP) for clinical data sets relying on the common FHIR data standard. Methods: We validated the performance and usability of our FHIR-DHP with data from the Medical Information Mart for Intensive Care IV database. Results: We present the FHIR-DHP workflow in respect of the transformation of "raw" hospital records into a harmonized, AI-friendly data representation. The pipeline consists of the following 5 key preprocessing steps: querying of data from hospital database, FHIR mapping, syntactic validation, transfer of harmonized data into the patient-model database, and export of data in an AI-friendly format for further medical applications. A detailed example of FHIR-DHP execution was presented for clinical diagnoses records. Conclusions: Our approach enables the scalable and needs-driven data modeling of large and heterogenous clinical data sets. The FHIR-DHP is a pivotal step toward increasing cooperation, interoperability, and quality of patient care in the clinical routine and for medical research.

Abstract:In this paper we present the methodology and decisions behind an implementation of a telehealth data management framework, aiming to support integrated care services for chronic and multimorbid patients. The framework leverages an OWL ontology, built upon HL7 FHIR resources, to provide storage and representation of semantically enriched EHR data following Linked Data principles. This is presented along with the realization of the persistent storage solution and communication web services that allow the management of EHR data, ensuring the validity and integrity of the exchanged patient data as self-describing ontology instances. The framework concentrates on flexibility and reusability, which is addressed by regarding the aforementioned ontology as a single point of change. This solution has been implemented in the scope of the EU project WELCOME for managing data in a telemonitoring system for patients with COPD and co-morbidities and was also successfully deployed for the INLIFE EU project with minimal effort. The results of the two applications suggest it can be adopted and properly adapted in a series of integrated care scenarios with minimal effort.

Abstract:Purpose of Review:This article addresses the occurrence of acute symptomatic seizures in the setting of many medical illnesses. Recent Findings:Many medical illnesses can cause seizures. Metabolic derangements, such as disorders of serum glucose metabolism, cause seizures, as well as other neurologic manifestations. Many medicines (such as antibiotics, antivirals, antidepressants, and antipsychotics), whether used at typical doses or in overdosage, can lead to acute symptomatic seizures. Summary:Acute symptomatic seizures occur most often in illnesses that directly injure the brain. Trauma, stroke, CNS tumors, and CNS infection very commonly cause seizures. However, many medical illnesses do not directly injure the brain but lead to neurologic signs and symptoms, such as seizures. Recognizing these effects, especially in critically ill patients, is important for optimizing medical care.

Abstract:There is a great opportunity to use high-quality patient journals and health registers to develop machine learning-based Clinical Decision Support Systems (CDSS). To implement a CDSS tool in a clinical workflow, there is a need to integrate, validate and test this tool on the Electronic Health Record (EHR) systems used to store and manage patient data. However, it is often not possible to get the necessary access to an EHR system due to legal compliance. We propose an architecture for generating and using synthetic EHR data for CDSS tool development. The architecture is implemented in a system called SyntHIR. The SyntHIR system uses the Fast Healthcare Interoperability Resources (FHIR) standards for data interoperability, the Gretel framework for generating synthetic data, the Microsoft Azure FHIR server as the FHIR-based EHR system and SMART on FHIR framework for tool transportability. We demonstrate the usefulness of SyntHIR by developing a machine learning-based CDSS tool using data from the Norwegian Patient Register (NPR) and Norwegian Patient Prescriptions (NorPD). We demonstrate the development of the tool on the SyntHIR system and then lift it to the Open DIPS environment. In conclusion, SyntHIR provides a generic architecture for CDSS tool development using synthetic FHIR data and a testing environment before implementing it in a clinical setting. However, there is scope for improvement in terms of the quality of the synthetic data generated. Abstract:The international healthcare systems interoperability is an unresolved technological area at the moment. This paper demonstrates the results of the software engineering research for simplex syntactic and semantic technical interoperability of hospital information systems, eHealth smart device technology and clinical telemedicine instruments through the recently developed Telemedicine Interoperability Hub. Several similar experiments exist. This research is unique in building a prototype interconnecting not only healthcare information systems with each other, but aiming to establish a general healthcare interoperability scheme including also the eHealth smart devices and telemedicine instruments. The aim of this research is to establish cloud-based data interchange capability with the newly developed information technology system interconnected with the emerging eHealth Internet of Things solutions and the classical hospital information system architecture. Notwithstanding the international information technology medical data exchange standards, like Health Level Seven, the adoption of an industry-wide open telemedicine syntactic and semantic interoperability standard is necessary. The research studied varying simplex, duplex, full-duplex, data package- and file-based information technology modalities establishing stable system interconnection among clinical instruments, healthcare systems and eHealth smart devices. This research is the manifestation of the trilateral cooperation of the University of Debrecen Department of Information Technology, Semmelweis University Second Paediatric Clinic and T-Systems Healthcare Competence Center Central and Eastern Europe. The developed experimental software engineering solution was embedded in hybrid cloud architecture after testing private cloud Infrastructure-as-a-Service and Public Cloud Software-as-a-Service technical solutions.

Abstract:Advances in Internet of Things (IoT) devices and in Machine Learning (ML) applications can provide valuable insights and predictions on personal health by optimizing data generation and processing.Nevertheless, the flow of data about the health status of a patient brings a variety of technical, legal and economic challenges that need to be addressed through an interdisciplinary approach.In this context, based on the action research methodology, the paper introduces an exemplary health-related activity recognition platform based on IoT, developed as a part of European-funded project Horizon 2020 in collaboration with academia and industry.The platform proposes innovative solutions on how personal healthcare data can be processed and analysed, protecting users' privacy. The main strength of the platform is the interdisciplinary approach used within a triple-helix model, involving a variety of institutions, companies and researchers from different academic fields. In this perspective, the paper shows the potential that the integration of IoT and ML models have to offer and the main challenges that still need to be addressed.
