Evaluation of Denture Adhesive versus Single Implant on Complete Denture Retention and Patient Satisfaction

Abstract:Purpose: Evaluate the effect of denture adhesive versus single implant on complete denture retention and patient satisfaction. Materials and Methods: Fourteen completely edentulous male patients participated in a crossover study. Acrylic complete denture was constructed for each patient and after 30 days of the adaptation period, retention of lower denture was measured without using denture adhesive. Patients were instructed to use Protefix paste denture adhesive for 6 months. After 3 months of denture insertion with the use of denture adhesive, for each patient a single implant (3.5*13mm Anyone Two– piece dental implant, Mega Gen Implant System, Korea) was placed at the anterior midline region to retain mandibular denture and patients were permitted to continue using their complete dentures using denture adhesive for an additional three months to ensure implant osseointegration. After 3months of implant osseointegration, patients were recalled and abutments were attached to implant fixtures and attachments were then placed. Patients were then informed to avoid the use of denture adhesive. Retention of the mandibular denture and patient satisfaction were evaluated before using denture adhesives and at 1, 3, and 6 months after using denture adhesive and at 1, 3, and 6 months after implant placement without using denture adhesive.Data were statistically analysed by t student test. Results: Retention of mandibular denture and patient satisfaction were increased in both groups with no significance difference between them. Conclusions: Within the limitations of study, denture adhesive and a single implant can increase mandibular denture retention and patient satisfaction in completely edentulous patients.
Effect of a denture adhesive on the satisfaction and kinesiographic parameters of complete denture wearers: a cross-over randomized clinical trial

Abstract:The aim of the present study was to assess the effect of a denture adhesive (DA) on patient satisfaction and kinesiographic parameters of complete denture wearers by a cross-over study. Fifty edentulous patients received a set of new complete dentures. After an adaptation period, the participants were enrolled in the trial and randomized to receive a sequence of treatment protocols: Protocol 1- DA use during the first 15 days, followed by no DA for the next 15 days; Protocol 2- no DA during the first 15 days, followed by use of DA for the next 15 days. Outcomes were assessed after 15 days of each sequence of treatment. A questionnaire was used to assess the patients' satisfaction. A kinesiograph was used to record mandible movements and patterns of maxillary complete denture movement during chewing. The Wilcoxon test (α=0.05) and a paired sample t-test (α=0.05) were used to compare satisfaction levels and kinesiographic data, respectively. Use of DA improved the overall level of patient satisfaction (p<0.001). The kinesiographic recordings revealed a significant increase (1.7 mm) in vertical mandible movements (p<0.001) during chewing and a lower (0.3 mm) vertical intrusion of the maxillary complete dentures (p=0.002) during chewing after using the DA. Use of DA in complete denture wearers improved the patients' satisfaction and altered mandible movements, with increases in vertical movements during chewing and less intrusion of maxillary complete dentures.
A randomized crossover trial assessing denture adhesive for prevention of food infiltration among partial denture wearers

Abstract:Purpose: This two-treatment, four-period, double-blind, randomized controlled crossover trial assessed the ability of two denture adhesives, both applied with a thin nozzle in a continuous application pattern, to prevent food infiltration beneath partial dentures. Methods: Participants with mandibular partial dentures and a history of food particle infiltration were enrolled. All participants used both an optimized calcium/zinc partial salt of polyvinyl methyl ether/maleic acid (PVM/MA) denture adhesive and a calcium/sodium partial salt of PVM/MA test denture adhesive, twice each, throughout four study periods, according to a randomly assigned sequence. At each visit, participants underwent two assessments: once with no denture adhesive (baseline) and once with denture adhesive, 1 hour after adhesive application. For each assessment, participants ate one-half of the top of a poppy seed muffin, and a dental professional counted the seeds retained on the denture and mucosa, which was the primary variable. The change-from-baseline comparison was made for each treatment separately using a paired t-test or Wilcoxon Signed Rank test depending on the normality of the data. A between-treatment comparison for the change from baseline was performed using a crossover ANCOVA with treatment and period as fixed effects and participant as a random effect. The baseline poppy seed count was used as a covariate. Results: 30 participants were enrolled; 29 completed the trial. Both denture adhesives achieved statistically significantly fewer retained seeds versus baseline (P< 0.001). The calcium/zinc adhesive reduced the seed count from baseline by 85.9% (6.18 vs 0.86), and the calcium/ sodium adhesive reduced seed count by 76.6% (6.04 vs 1.43). Comparing the two denture adhesives, the reduction in seed count from baseline was statistically significantly greater for the calcium/zinc adhesive versus the calcium/sodium formulation (P= 0.008). Clinical significance: These results support the recommendation of denture adhesive use for the prevention of food infiltration beneath partial dentures, with optimized calcium/zinc denture adhesive showing the greatest prevention benefit.

Abstract:Aim: The aim of this within-subject prospective clinical study was to investigate the scope of single implant mandibular overdenture by assessing its masticatory efficiency, clinical performance, and patient satisfaction compared to conventional complete dentures. Settings and Design: Prospective In Vivo Study. Material and Methods: This prospective in vivo study was conducted in the Department of Prosthodontics and Crown and Bridge, Christian Dental College, Ludhiana. A total of 12 completely edentulous patients received a single implant in the mandibular anterior midline region. After the healing period, the conventional maxillary and mandibular dentures were fabricated. 15 days post insertion of the conventional dentures, patients were evaluated for masticatory efficiency, clinical performance including retention and stability, and patient satisfaction. To evaluate the masticatory efficiency blue raspberry and original pink “Hubba Bubba tape gum” were used as a test food. Colorimetric analysis was done to assess variance of hue. To assess clinical performance, retention, and stability of the mandibular denture was recorded using a digital force gauge and was tabulated as per CU-modified Kapur's criteria. OHIP-14 index was used to assess patient satisfaction. After evaluation of the parameters of conventional dentures, the denture was converted into an implant-retained mandibular denture by chairside conversion with locator attachments. 15 days post-implant loading, parameters of the implant retained mandibular overdenture were assessed again followed by statistical analysis. Statistical Analysis Used: The masticatory efficiency was assessed using a paired t -test. The patient satisfaction was sequentially assessed with Wilcoxon signed rank test and thereafter paired t -test was used to compare between conventional complete denture and overdenture. Clinical performance was assessed using the Wilcoxon signed rank test. Results: The masticatory efficiency of single implant mandibular overdenture was higher than that of conventional complete dentures. The clinical performance of the overdenture was higher than that of the conventional denture. Stability being a time-dependent parameter might need longer follow-ups for further conclusions. Patient satisfaction with single implant retained overdenture was significantly higher than conventional complete dentures. Conclusion: Single implant mandibular overdenture has improved masticatory efficiency, clinical performance, and patient satisfaction compared to conventional complete dentures.

Abstract:ABSTRACT Background: Dentures are common prosthetic devices used to replace missing teeth; however, wearers often encounter challenges related to stability, comfort, and esthetics. Understanding these challenges and preferences for different denture types is crucial for improving oral health outcomes and quality of life. Materials and Methods: We conducted a comprehensive study involving 500 denture wearers. Participants were assessed for denture stability, comfort, and esthetics using standardized measures. In addition, their preferences for various denture types, including conventional complete dentures, implant-supported overdentures, and partial dentures, were explored through structured interviews. Results: In this study, 25% of participants found denture stability to be “very challenging,” 40% considered it “somewhat challenging,” and 35% did not find it challenging. Twenty percent of participants found denture comfort to be “very challenging,” 36% considered it “somewhat challenging,” and 44% did not find it challenging. In the study, 15% of participants found denture esthetics to be “very challenging,” 30% considered it “somewhat challenging,” and 55% did not find it challenging. Thirty percent of participants prefer implant-supported dentures, while 70% do not prefer them. Conclusions: Denture wearers commonly experience challenges associated with stability, comfort, and esthetics. Preferences for denture types differ, highlighting the need for personalized treatment options based on individual needs and circumstances.
Comparative Evaluation of Antimicrobial Effects of Herbal Extracts Incorporated in Dentifrices among Adults: An In-vitro Study and a Double Blinded Randomized Control Trial

Abstract:Introduction: A positive correlation has been observed between Streptococcus mutans, Candida albicans and periodontal diseases. Several substances including synthetic chemicals and natural agents have shown promising antimicrobial activity against the S. mutans- C. albicans complex. Extracts of Aloe Vera, Triphala, Tulsi, Pomegranate, and Acacia catechu have established therapeutic effects in medicine. The aim of the study was to evaluate the antimicrobial activity and the Minimum Inhibitory Concentration (MIC) of five herbal extracts in-vitro and to compare the antibacterial effects of 5 dentifrices containing the herbal extracts and a commercially available dentifrice on the plaque sample counts of S. mutans and C. albicans in-vivo Methodology: Extracts were obtained using a standardized method. Minimum inhibitory concentration was calculated for the test microorganisms using modified agar-well diffusion and macro broth dilution technique and the inhibition zone diameters were calculated. Dentifrices containing the extracts were formulated and the in vivo antimicrobial activity was evaluated by studying the CFU count from plaque samples of 420 participants. Results: Highest antimicrobial activity against S Mutans and C albicans was shown by Acacia catechu and Tulsi respectively and least was by pomegranate. Mean reduction in microbial counts (CFU/ml) by inoculating nutrient broth from baseline to 60 days was highest in Acacia catechu and Triphala for S Mutans and C albicans respectively. All herbal extracts performed better than the commercially available toothpaste. Conclusion: Dentifrices with herbal extracts of Aloe Vera, Triphala, Tulsi, Pomegranate, and Acacia catechu could serve as an alternative to commercially available dentifrices for treating oral diseases whose causative organisms are S. mutans and C. albicans.

Influence of Removable Denture Cleaning Agents on Adhesion of Oral Pathogenic Microflora In Vitro: A Randomized Controlled Trial

Abstract:Introduction: Removable dentures are used by 20% of the population. These may be accompanied by denture stomatitis in 15-70% of patients. The choice of the optimal cleansing agent for removable dental prostheses is of high significance. Aim: The aim of our research was to study the influence of removable denture cleansing products on the adhesion of microorganisms and yeast. Materials and Methods: We manufactured 144 specimens of standardized round shape with a diameter of 10 mm from 4 types of modern polymeric materials used by prosthetic dentistry to produce removable dentures, 12 specimens of each material were placed into suspensions of bacterial cultures of Staphylococcus aureus , Escherichia coli , Candida albicans , then into “ClearaSept” (Test group 1), “Рrotefix active cleanser” (Test group 2), saline solution (Control group), followed by nutrient media. The adhesion index was calculated and analyzed. Results: There was no reliable lowering of adhesion index of Staphylococcus Aureus to all materials detected in Test group 1 (U=6, p>0.05 for Bio XS; U=8, p>0.05 for Dental D, Denotokeep Peek, Vertex Rapid Simplified). In Test group 2, the adhesion index of Staphylococcus Aureus reliably decreased to all materials compared to the Control group (U=0, p≤0.01). The adhesion index of Candida albicans and Escherichia coli to all materials in Test group 1 had a minor to moderate reliable reduction compared to the Control group (U=0, p≤0.01). Test group 2 showed a significant reliable decrease in Candida albicans and Escherichia coli adhesion index to all materials in comparison with the Control group (U=0, p≤0.01). Conclusion: The research showed an unreliable or minor and moderate reliable decrease in microorganisms adhesion index depending on the microorganism species after treatment of denture material specimens by antibacterial soap “ClearaSept” and a reliable significant decrease in microbial and yeast adhesion after application of Protefix active cleaner solution, which demonstrates a more significant antimicrobial effect in comparison to “ClearaSept” against Staphylococcus aureus , Escherichia coli, and Candida albicans.

Abstract:Relevance . The isolation of prosthetic periodontitis among inflammatory periodontal pathology has significantly supplemented the generally accepted notions of dentists concerning the etiology and progression of dental prosthetics complications. Improving the methods of preventing inflammatory periodontal lesions caused by the presence of denture structures in the oral cavity is very relevant nowadays, especially in persons using removable dental prostheses. This applies to older age groups, which for financial reasons are more accessible removable acrylic dentures, which they have the opportunity to get at the expense of the budget of the region. Aim . This applies to those in the older age groups who, for financial reasons, have more access to removable acrylic dentures because of the possibility of obtaining dentures at the expense of the regional budget. Therefore, the aim of the study was to assess the possibility of using a new domestic denture fixation cream for the prevention of prosthetic periodontitis in patients using removable dentures. Materials and methods . 63 (18 male and 45 female) elderly patients (61-74 years old) with partial tooth loss on both jaws were examined. Partial removable acrylic dentures were made for patients 1st group (n = 30) and 2nd group (n = 33). The patients of the second group constantly used the domestic ASEPTA РARODONTAL denture fixation cream with herbal extract for 9 months of the study. Assessment of the impact of dentures on the oral tissues and periodontium was carried out on the basis of clinical examination and by studying the antimicrobial potential of saliva. Results . The use of partial removable acrylic dentures has been found to contribute to the occurrence of prosthetic periodontitis, which is associated with the technology used in their manufacture. It is possible to reduce the incidence of prosthetic periodontitis not only by timely correction of prosthesis but also by using adhesive means for fixation of prosthesis, the application of which permitted to reduce the occurrence of prosthetic periodontitis by 47,58% and to reduce the occurrence of inflammatory changes of gingiva in the restored teeth area by 16,06%. The research of the antimicrobial potential of saliva in persons who constantly used ASEPTA РARODONTAL denture fixation cream with herbal extract showed that the immune balance achieved before dental prosthetics can be maintained in the oral cavity. Conclusion . The results of the clinical and laboratory study allow us to recommend more widely the use of adhesive means for fixation of removable dentures, including the ASEPTA PARODONTAL cream with herbal extracts.

Abstract:OBJECTIVE:To assess the masticatory efficiency and patient' satisfaction in long-centric occlusal pattern complete denture wearers.METHODS:The anatomic occlusal pattern and long-centric occlusal pattern complete dentures were made for each of 10 edentulous patients with severe alveolar rigde absorption simultaneously using the technique of interchangeable artificial teeth. The order of delivery of different occlusal pattern complete denture was determined according to randomized principle. For each kind of occlusal pattern complete denture, the masticatory efficiency was measured with spectrophotometer after the dentures were worn 2, 4, 6, 8, 10 and 12 weeks. A psychometric questionnaire was completed by the patients after wearing the dentures for more than three months and analyzing the patients' rating of the denture satisfaction.RESULTS:No significant differences in masticatory efficiency was found between the long-centric occlusal pattern complete denture wearers and the anatomic occlusal complete denture wearers. The patients' grade of denture satisfaction in long-centric occlusal pattern complete dentures wearers was significantly better than that in anatomic occlusal denture wearers (P<0.05).CONCLUSION:Complete denture with long-centric occlusion can be used to improve the clinical effect of edentulous patients with severe absorption of residual alveolar rigde.
Efficacy of ultrasonic home-care denture cleaning versus conventional denture cleaning: A randomised crossover clinical trial

Abstract:Objective: To compare the efficacy of test (ultrasonic cleaner combined with immersion in denture cleanser solution) and control (immersion in denture cleanser solution followed by conventional brushing) denture cleaning interventions in enhancing denture cleanliness, reducing denture stomatitis, and improving patient satisfaction. Methods: A prospective, single-blind, block-randomised, two-period crossover, superiority-controlled clinical trial was conducted of a 3-month intervention. The study design included a pre-intervention period (2 weeks), intervention period one (3 months), washout period (2 weeks), and intervention period two (3 months). A total of 56 community-dwelling elders were block-randomized into either sequence Test/Control or sequence Control/Test. The intervention, period, and carryover effects for the changes in the cleanliness of extensive partial and complete acrylic dentures, denture stomatitis, and changes in patient satisfaction were estimated using Generalized Estimating Equations models. Results: Percentage plaque area coverage, patient satisfaction, and denture stomatitis were significantly improved for both intervention and control arms after 3 months (P < 0.05). The intervention arm was found to significantly improve denture cleanliness (P < 0.001) and patient satisfaction (P = 0.002) more than the control arm. Denture-wearing habits and denture age were also significantly associated with the changes in denture plaque coverage (P < 0.05). However, the effect of the test intervention on denture stomatitis was not significantly different compared to the control arm (P = 0.284). Conclusion: This study revealed that the test intervention group was significantly more effective than the control group in improving denture cleanliness and patient satisfaction among community-dwelling elders. This test intervention is recommended for maintaining optimum denture hygiene among older adults. Clinical significance: Removable dentures can harbor opportunistic pathogens, emphasizing the need for effective denture hygiene intervention using ultrasonic cleaner combined with immersion in denture cleanser solution to eliminate denture biofilm in community-dwelling elders.
Complete denture palatal rugae design impact on satisfaction and quality of life

Abstract:Objectives To compare the impact of three complete denture palatal rugae designs on participants' satisfaction and oral‐health‐related quality of life. Background Complete dentures palatal rugea are usually polished to a smooth finish, which can affect the patient's adaptation. Roughening or keeping an opening in the rugae area to replicate the natural sensation of the palatal rugae has been suggested but lacks clinical evidence. Methods This randomised crossover trial included participants with complete dentures randomly allocated to six sequences. Each sequence alternated between polished, roughened, and open rugae designs. Participants evaluated general and domain‐specific satisfaction (eating, taste, speaking, phonetics, and ease of cleaning) on a 100‐mm visual analogue scale. They also completed the Oral Health Impact Profile for Edentulous Patients (OHIP‐EDENT) and indicated their preferred design at the end. Repeated‐measure ANOVA compared satisfaction, and one‐way ANOVA with Tukey's test compared OHIP‐EDENT scores. Results Forty‐two participants were randomised. Seven dropped out. General satisfaction ratings were comparable for the polished (Mean = 80.2, SD = 19.7) and roughened designs (Mean = 79.5, SD = 20.2). However, the open design had a significantly lower satisfaction rating (Mean = 41.1, SD = 32.2). OHIP‐EDENT scores for the open design (Mean = 44.9, SD = 17.5) were significantly worse than those for the polished (Mean = 36.1, SD = 12.7) and roughened designs (Mean = 36.2, SD = 13.3). Two‐thirds of the participants chose the polished design. One‐third chose the roughened design; of those, 83% were first‐time denture wearers. Conclusions Complete dentures with polished and roughened rugae designs were perceived similarly. Both designs could be recommended based on the patient's preference. However, the open design should be avoided due to negative perception.

Abstract:Conventional complete denture treatment course requires six appointments, but modified protocol only takes four appointments. This study compared the conventional and modified protocol for complete denture fabrication regarding patient satisfaction and clinical outcomes.A total of 24 patients accepted complete denture treatment. According to complete denture treatment protocol, these patients were divided into the conventional group (group C, n = 12) and the modified group (group M, n = 12). Group C used the conventional protocol and required six appointments. Group M used the one-appointment master impression and jaw relation record technique, and it took four appointments. Data of oral health impact profile-14 (OHIP-14), satisfaction scale and the number of recalls in the first year were collected for the statistical analysis.The mean OHIP-14 scores in group C and group M were 13.79 ± 3.81 and 15.33 ± 5.25, respectively. In terms of satisfaction, the mean scores in group C and group M were 8.33 ± 0.61 points and 8.66 ± 1.13 points, respectively. There were no statistically significant differences between the group C and M in terms of participant ratings for satisfaction and OHIP-14. At the same time, the results indicated that group M significantly reduced the number of postinsertion visits (<em>P</em> < 0.05).In terms of OHIP-14 and patients' satisfaction, the modified treatment protocol is comparable to the conventional protocol. Based on the number of recalls in the first year, the modified treatment protocol has a better clinical outcome.

Abstract:Problem: Good scientific evidence is lacking on the impact of improvement in masticatory function after prosthodontic restoration of missing teeth. Purpose: This study compares 1-week dietary intakes of 218 healthy, male patients, 111 patients restored with mandibular unilateral or bilateral distal base extension removable partial dentures and 107 patients with fixed partial dentures. Material and methods: Dietary logs recorded before the initiation of treatment and 6-months after the baseline period after partial denture treatment were analyzed for 30 nutritional variables of food intakes including total calories, fats, carbohydrates, proteins, fiber, and a number of vitamins and minerals. Results: Both before and after treatment, intakes of various nutrients were more than 25% above or below the RDAs for approximately half of the patients in both groups. No significant differences were observed between the mean scores of the two groups for any of the 30 variables either before or after treatment. However, beneficial treatment effects were seen in subsets of patients with low and high caloric intakes at entry. Both treatments increased the intakes of calories and 27 nutrients in the low caloric group and decreased the intakes of calories and 27 nutrients in the high caloric group. The decreases in the caloric intake and eight nutrients, including total protein, fat, carbohydrates, and cholesterol, were significantly greater (p < 0.05) in the fixed partial denture group than those in the removable partial denture group. Conclusions: These beneficial effects of partial dentures, if verified by other studies, may have profound clinical implications for the undernourished and obese patients.

Clinical comparative study of lingualized and anatomical occlusal pattern of complete dentures in mandible with severely ab-sorbed alveolar ridge

Abstract:Objective To compare clinical treatment effect of the lingualized and anatomical occlusal pattern of complete dentures on mandible with severely absorbed alveolar ridge. Methods Twelve edentulous patients with absorbed levelⅢ orⅣ mandible were se-lected, and were divided randomly into two groups, to receive the complete dentures with lingualized and anatomical occlusal pattern for three months in different orders using single blind method. Then the patients were asked to fill in the questionnaire of denture quality. The chewing efficiency of peanuts was measured by absorbance spectrophotometry, and the modification number resulting from pain was recorded. The data were statistically analyzed by SPSS 22.0. Results The satisfaction rate of two occlusal patterns had no significant difference in esthetics, pronunciation, and comfort ( P>0.05) , while lingualized occlusion obtained higher satisfaction rate in retention and stability of the prostheses, chewing ability than anatomical occlusion ( P<0.05) . Two occlusal patterns had no significant difference in the chewing efficiency of peanuts ( P>0.05) , while lingualized occlusion had smaller number of modification than that of anatomical occlusion (P<0.05). Conclusion Compared with the anatomical occlusion, the lingualized occlusion of complete denture is more suitable for the mandible severely absorbed alveolar ridge.

Abstract:Patients wearing conventional removable dentures often complain particularly about the instability of the mandibular denture. Due to instability of mandibular dentures, there is an increase in the severity of mandibular atrophy. Furthermore, denture instability leads to a feeling of insecurity, inefficient mastication, and overall dissatisfaction with the prosthesis. Placement of endosseous dental implants to retain and support mandibular overdentures or fixed complete dentures have been developed and studied as a method for solving the problem of instability associated with conventional dentures. Implant-retained dentures have many advantages compared to conventional removable dentures. Occasionally, anatomical or financial limitations may prohibit the patient to use of a fixed implant-retained prosthesis. In these cases, a removable implant-retained denture with a limited number of implants may be the possible choice. The aim of this article is to review the literature to compare implant-retained or supported dentures versus conventional dentures in terms of masticatory efficiency, stability, and patient satisfaction.
