Abstract:DNA methylation data plays a crucial role in estimating chronological age in mammals, offering real-time insights into an individual’s aging process. The Epigenetic Pacemaker (EPM) model allows inference of the biological age as deviations from the population trend. Given the sensitivity of this data, it is essential to safeguard both inputs and outputs of the EPM model. In a recent study by Goldenberg et al., a privacy-preserving approach for EPM computation was introduced, utilizing Fully Homomorphic Encryption (FHE). However, their method had limitations, including having high communication complexity and being impractical for large datasets Our work presents a new privacy preserving protocol for EPM computation, analytically improving both privacy and complexity. Notably, we employ a single server for the secure computation phase while ensuring privacy even in the event of server corruption (compared to requiring two non-colluding servers in Goldenberg et al.). Using techniques from symbolic algebra and number theory, the new protocol eliminates the need for communication during secure computation, significantly improves asymptotic runtime and and offers better compatibility to parallel computing for further time complexity reduction. We have implemented our protocol, demonstrating its ability to produce results similar to the standard (insecure) EPM model with substantial performance improvement compared to Goldenberg et al. These findings hold promise for enhancing data security in medical applications where personal privacy is paramount. The generality of both the new approach and the EPM, suggests that this protocol may be useful to other uses employing similar expectation maximization techniques.

Privacy Preserving Federated Unsupervised Domain Adaptation with Application to Age Prediction from DNA Methylation Data

Abstract:In computational biology, predictive models are widely used to address complex tasks, but their performance can suffer greatly when applied to data from different distributions. The current state-of-the-art domain adaptation method for high-dimensional data aims to mitigate these issues by aligning the input dependencies between training and test data. However, this approach requires centralized access to both source and target domain data, raising concerns about data privacy, especially when the data comes from multiple sources. In this paper, we introduce a privacy-preserving federated framework for unsupervised domain adaptation in high-dimensional settings. Our method employs federated training of Gaussian processes and weighted elastic nets to effectively address the problem of distribution shift between domains, while utilizing secure aggregation and randomized encoding to protect the local data of participating data owners. We evaluate our framework on the task of age prediction using DNA methylation data from multiple tissues, demonstrating that our approach performs comparably to existing centralized methods while maintaining data privacy, even in distributed environments where data is spread across multiple institutions. Our framework is the first privacy-preserving solution for high-dimensional domain adaptation in federated environments, offering a promising tool for fields like computational biology and medicine, where protecting sensitive data is essential.

Homomorphic Encryption: An Application to Polygenic Risk Scores

Abstract:Background: Polygenic risk scores (PRS) have emerged as a powerful tool in precision medicine, enabling personalized risk assessments for complex diseases. However, using sensitive genomic data in PRS calculations raises concerns about privacy and security. Fully Homomorphic Encryption (FHE) offers a promising solution by allowing computations on encrypted data, preserving the privacy of both genomic information and PRS models. Results: In this study, we present a novel application of FHE for secure and private PRS calculations using the CKKS protocol within the Lattigo library. Our approach involves a three-party system: clients (doctors with sensitive genetic data), modelers developing a PRS (academics or a company), and evaluators (a "local hospital" running the models while maintaining data confidentiality). We demonstrate the feasibility and accuracy of our protocol by applying it to synthetic datasets of various sizes and a robust 110k-SNP model for schizophrenia. The normal PRS calculation results are essentially identical to the encrypted calculation: between the two results is .999 & MSE is 2.27 times 10 . Moreover, while the encrypted calculation is roughly 1000 times slower than conventional non-encrypted ones (when only considering the core PRS calculation), it is quite feasible on a single-CPU node - e.g. running on ≈ 1100 individuals with ≈ 110k SNPS took six minutes and ≈ 65G memory on a laptop computer. In addition, we investigate the impact of encryption parameters (modulus) on this computational time and accuracy in detail. Conclusion: By enabling secure PRS calculations on encrypted genomic data, our approach addresses the pressing need for privacy-preserving solutions in the era of precision medicine. The ability to perform accurate risk assessments while maintaining patient confidentiality paves the way for broader adoption of PRS and personalized medicine in healthcare, particularly with the advent of large-scale computing power.

Abstract:Real-world healthcare data sharing is instrumental in constructing broader-based and larger clinical datasets that may improve clinical decision-making research and outcomes. Stakeholders are frequently reluctant to share their data without guaranteed patient privacy, proper protection of their datasets, and control over the usage of their data. Fully homomorphic encryption (FHE) is a cryptographic capability that can address these issues by enabling computation on encrypted data without intermediate decryptions, so the analytics results are obtained without revealing the raw data. This work presents a toolset for collaborative privacy-preserving analysis of oncological data using multiparty FHE. Our toolset supports survival analysis, logistic regression training, and several common descriptive statistics. We demonstrate using oncological datasets that the toolset achieves high accuracy and practical performance, which scales well to larger datasets. As part of this work, we propose a cryptographic protocol for interactive bootstrapping in multiparty FHE, which is of independent interest. The toolset we develop is general-purpose and can be applied to other collaborative medical and healthcare application domains.
Age Prediction Using DNA Methylation Heterogeneity Metrics

Abstract:Dynamic changes in genomic DNA methylation patterns govern the epigenetic developmental programs and accompany the organism's aging. Epigenetic clock (eAge) algorithms utilize DNA methylation to estimate the age and risk factors for diseases as well as analyze the impact of various interventions. High-throughput bisulfite sequencing methods, such as reduced-representation bisulfite sequencing (RRBS) or whole genome bisulfite sequencing (WGBS), provide an opportunity to identify the genomic regions of disordered or heterogeneous DNA methylation, which might be associated with cell-type heterogeneity, DNA methylation erosion, and allele-specific methylation. We systematically evaluated the applicability of five scores assessing the variability of methylation patterns by evaluating within-sample heterogeneity (WSH) to construct human blood epigenetic clock models using RRBS data. The best performance was demonstrated by the model based on a metric designed to assess DNA methylation erosion with an MAE of 3.686 years. We also trained a prediction model that uses the average methylation level over genomic regions. Although this region-based model was relatively more efficient than the WSH-based model, the latter required the analysis of just a few short genomic regions and, therefore, could be a useful tool to design a reduced epigenetic clock that is analyzed by targeted next-generation sequencing.

Abstract:BackgroundThe deployment of Genome-wide association studies (GWASs) requires genomic information of a large population to produce reliable results. This raises significant privacy concerns, making people hesitate to contribute their genetic information to such studies.ResultsWe propose two provably secure solutions to address this challenge: (1) a somewhat homomorphic encryption (HE) approach, and (2) a secure multiparty computation (MPC) approach. Unlike previous work, our approach does not rely on adding noise to the input data, nor does it reveal any information about the patients. Our protocols aim to prevent data breaches by calculating the χ2 statistic in a privacy-preserving manner, without revealing any information other than whether the statistic is significant or not. Specifically, our protocols compute the χ2 statistic, but only return a yes/no answer, indicating significance. By not revealing the statistic value itself but only the significance, our approach thwarts attacks exploiting statistic values. We significantly increased the efficiency of our HE protocols by introducing a new masking technique to perform the secure comparison that is necessary for determining significance.ConclusionsWe show that full-scale privacy-preserving GWAS is practical, as long as the statistics can be computed by low degree polynomials. Our implementations demonstrated that both approaches are efficient. The secure multiparty computation technique completes its execution in approximately 2 ms for data contributed by one million subjects.

Abstract:Machine Learning (ML) alleviates the challenges of high-dimensional data analysis and improves decision making in critical applications like healthcare. Effective cancer type from high-dimensional genetic mutation data can be useful for cancer diagnosis and treatment, if the distinguishable patterns between cancer types are identified. At the same time, analysis of high-dimensional data is computationally expensive and is often outsourced to cloud services. Privacy concerns in outsourced ML, especially in the field of genetics, motivate the use of encrypted computation, like Homomorphic Encryption (HE). But restrictive overheads of encrypted computation deter its usage. In this work, we explore the challenges of privacy preserving cancer detection using a real-world dataset consisting of more than 2 million genetic information for several cancer types. Since the data is inherently high-dimensional, we explore smaller ML models for cancer prediction to enable fast inference in the privacy preserving domain. We develop a solution for privacy preserving cancer inference which first leverages the domain knowledge on somatic mutations to efficiently encode genetic mutations and then uses statistical tests for feature selection. Our logistic regression model, built using our novel encoding scheme, achieves 0.98 micro-average area under curve with 13% higher test accuracy than similar studies. We exhaustively test our model's predictive capabilities by analyzing the genes used by the model. Furthermore, we propose a fast matrix multiplication algorithm that can efficiently handle high-dimensional data. Experimental results show that, even with 40,000 features, our proposed matrix multiplication algorithm can speed up concurrent inference of multiple individuals by approximately 10x and inference of a single individual by approximately 550x, in comparison to standard matrix multiplication.

Abstract:Abstract Background Privacy-preserving computations on genomic data, and more generally on medical data, is a critical path technology for innovative, life-saving research to positively and equally impact the global population. It enables medical research algorithms to be securely deployed in the cloud because operations on encrypted genomic databases are conducted without revealing any individual genomes. Methods for secure computation have shown significant performance improvements over the last several years. However, it is still challenging to apply them on large biomedical datasets. Methods The HE Track of iDash 2018 competition focused on solving an important problem in practical machine learning scenarios, where a data analyst that has trained a regression model (both linear and logistic) with a certain set of features, attempts to find all features in an encrypted database that will improve the quality of the model. Our solution is based on the hybrid framework Chimera that allows for switching between different families of fully homomorphic schemes, namely TFHE and HEAAN . Results Our solution is one of the finalist of Track 2 of iDash 2018 competition. Among the submitted solutions, ours is the only bootstrapped approach that can be applied for different sets of parameters without re-encrypting the genomic database, making it practical for real-world applications. Conclusions This is the first step towards the more general feature selection problem across large encrypted databases.

Privacy-Preserving Artificial Intelligence: Application to Precision Medicine

Abstract:Motivated by state-of-the-art performances across a wide variety of areas, over the last few years Machine Learning has drawn a significant amount of attention from the healthcare domain. Despite their potential in enabling person-alized medicine applications, the adoption of Deep Learning based solutions in clinical workflows has been hindered in many cases by the strict regulations concerning the privacy of patient health data. We propose a solution that relies on Fully Homomorphic Encryption, particularly on the MORE scheme, as a mechanism for enabling computations on sensitive health data, without revealing the underlying data. The chosen variant of the encryption scheme allows for the computations in the Neural Network model to be directly performed on floating point numbers, while incurring a reasonably small computational overhead. For feasibility evaluation, we demonstrate on the MNIST digit recognition task that Deep Learning can be performed on encrypted data without compromising the accuracy. We then address a more complex task by training a model on encrypted data to estimate the outputs of a whole-body circulation (WBC) model. These results underline the potential of the proposed approach to outperform current solutions by delivering comparable results to the unencrypted Deep Learning based solutions, in a reasonable amount of time. Lastly, the security aspects of the encryption scheme are analyzed, and we show that, even though the chosen encryption scheme favors performance and utility at the cost of weaker security, it can still be used in certain practical applications.

Abstract:Forensic analysis heavily relies on DNA analysis techniques, notably autosomal Single Nucleotide Polymorphisms (SNPs), to expedite the identification of unknown suspects through genomic database searches. However, the uniqueness of an individual's genome sequence designates it as Personal Identifiable Information (PII), subjecting it to stringent privacy regulations that can impede data access and analysis, as well as restrict the parties allowed to handle the data. Homomorphic Encryption (HE) emerges as a promising solution, enabling the execution of complex functions on encrypted data without the need for decryption. HE not only permits the processing of PII as soon as it is collected and encrypted, such as at a crime scene, but also expands the potential for data processing by multiple entities and artificial intelligence services.

A Pervasive, Efficient and Private Future: Realizing Privacy-Preserving Machine Learning Through Hybrid Homomorphic Encryption

Abstract:Machine Learning (ML) has become one of the most impactful fields of data science in recent years. However, a significant concern with ML is its privacy risks due to rising attacks against ML models. Privacy-Preserving Machine Learning (PPML) methods have been proposed to mitigate the privacy and security risks of ML models. A popular approach to achieving PPML uses Homomorphic Encryption (HE). However, the highly publicized inefficiencies of HE make it unsuitable for highly scalable scenarios with resource-constrained devices. Hence, Hybrid Homomorphic Encryption (HHE) -- a modern encryption scheme that combines symmetric cryptography with HE -- has recently been introduced to overcome these challenges. HHE potentially provides a foundation to build new efficient and privacy-preserving services that transfer expensive HE operations to the cloud. This work introduces HHE to the ML field by proposing resource-friendly PPML protocols for edge devices. More precisely, we utilize HHE as the primary building block of our PPML protocols. We assess the performance of our protocols by first extensively evaluating each party's communication and computational cost on a dummy dataset and show the efficiency of our protocols by comparing them with similar protocols implemented using plain BFV. Subsequently, we demonstrate the real-world applicability of our construction by building an actual PPML application that uses HHE as its foundation to classify heart disease based on sensitive ECG data.

Towards a Privacy Preserving Cohort Discovery Framework for Clinical Research Networks.

Abstract:Background: The last few years have witnessed an increasing number of clinical research networks (CRNs) focused on building large collections of data from electronic health records (EHRs), claims, and patient-reported outcomes (PROs). Many of these CRNs provide a service for the discovery of research cohorts with various health conditions, which is especially useful for rare diseases.Background: Supporting patient privacy can enhance the scalability and efficiency of such processes; however, current practice mainly relies on policy, such as guidelines defined in the Health Insurance Portability and Accountability Act (HIPAA), which are insufficient for CRNs (e.g., HIPAA does not require encryption of data which can mitigate insider threats). By combining policy with privacy enhancing technologies we can enhance the trustworthiness of CRNs. The goal of this research is to determine if searchable encryption can instill privacy in CRNs without sacrificing their usability.Methods: We developed a technique, implemented in working software to enable privacy-preserving cohort discovery (PPCD) services in large distributed CRNs based on elliptic curve cryptography (ECC). This technique also incorporates a block indexing strategy to improve the performance (in terms of computational running time) of PPCD. We evaluated the PPCD service with three real cohort definitions: (1) elderly cervical cancer patients who underwent radical hysterectomy, (2) oropharyngeal and tongue cancer patients who underwent robotic transoral surgery, and (3) female breast cancer patients who underwent mastectomy) with varied query complexity. These definitions were tested in an encrypted database of 7.1 million records derived from the publically available Healthcare Cost and Utilization Project (HCUP) Nationwide Inpatient Sample (NIS). We assessed the performance of the PPCD service in terms of (1) accuracy in cohort discovery, (2) computational running time, and (3) privacy. afforded to the underlying records during PPCD.Results: The empirical results indicate that the proposed PPCD can execute cohort discovery queries in a reasonable amount of time, with query runtime in the range of 165-262 s for the 3 use cases, with zero compromise in accuracy. We further show that the search performance is practical because it supports a highly parallelized design for secure evaluation over encrypted records. Additionally, our security analysis shows that the proposed construction is resilient to standard adversaries.Conclusions: PPCD services can be designed for clinical research networks. The security construction presented in this work specifically achieves high privacy guarantees by preventing both threats originating from within and beyond the network. (C) 2016 Elsevier Inc. Abstract:Age-dependent changes in DNA methylation allow chronological and biological age inference, but the underlying mechanisms remain unclear. Using ultra-deep sequencing of >300 blood samples from healthy individuals, we show that age-dependent DNA methylation changes are regional and occur at multiple adjacent CpG sites, either stochastically or in a coordinated block-like manner. Deep learning analysis of single-molecule patterns in two genomic loci achieved accurate age prediction with a median error of 1.46-1.7 years on held-out human blood samples, dramatically improving current epigenetic clocks. Factors such as gender, BMI, smoking and other measures of biological aging do not affect chronological age inference. Longitudinal 10-year samples revealed that early deviations from epigenetic age are maintained throughout life and subsequent changes faithfully record time. Lastly, the model inferred chronological age from as few as 50 DNA molecules, suggesting that age is encoded by individual cells. Overall, DNA methylation changes in clustered CpG sites illuminate the principles of time measurement by cells and tissues, and facilitate medical and forensic applications.

Privacy-preserving model evaluation for logistic and linear regression using homomorphically encrypted genotype data

Abstract:Objective: Linear and logistic regression are widely used statistical techniques in population genetics for analyzing genetic data and uncovering patterns and associations in large genetic datasets, such as identifying genetic variations linked to specific diseases or traits. However, obtaining statistically significant results from these studies requires large amounts of sensitive genotype and phenotype information from thousands of patients, which raises privacy concerns. Although cryptographic techniques such as homomorphic encryption offers a potential solution to the privacy concerns as it allows computations on encrypted data, previous methods leveraging homomorphic encryption have not addressed the confidentiality of shared models, which can leak information about the training data. Methods: In this work, we present a secure model evaluation method for linear and logistic regression using homomorphic encryption for six prediction tasks, where input genotypes, output phenotypes, and model parameters are all encrypted. Results: Our method ensures no private information leakage during inference and achieves high accuracy (≥93% for all outcomes) with each inference taking less than ten seconds for ∼200 genomes. Conclusion: Our study demonstrates that it is possible to perform linear and logistic regression model evaluation while protecting patient confidentiality with theoretical security guarantees. SAFETY: Secure gwAs in Federated Environment Through a hYbrid solution with Intel SGX and Homomorphic Encryption

Abstract:Recent studies demonstrate that effective healthcare can benefit from using the human genomic information. For instance, analysis of tumor genomes has revealed 140 genes whose mutations contribute to cancer. As a result, many institutions are using statistical analysis of genomic data, which are mostly based on genome-wide association studies (GWAS). GWAS analyze genome sequence variations in order to identify genetic risk factors for diseases. These studies often require pooling data from different sources together in order to unravel statistical patterns or relationships between genetic variants and diseases. In this case, the primary challenge is to fulfill one major objective: accessing multiple genomic data repositories for collaborative research in a privacy-preserving manner. Due to the sensitivity and privacy concerns regarding the genomic data, multi-jurisdictional laws and policies of cross-border genomic data sharing are enforced among different regions of the world. In this article, we present SAFETY, a hybrid framework, which can securely perform GWAS on federated genomic datasets using homomorphic encryption and recently introduced secure hardware component of Intel Software Guard Extensions (Intel SGX) to ensure high efficiency and privacy at the same time. Different experimental settings show the efficacy and applicability of such hybrid framework in secure conduction of GWAS. To the best of our knowledge, this hybrid use of homomorphic encryption along with Intel SGX is not proposed or experimented to this date. Our proposed framework, SAFETY is up to 4.82 times faster than the best existing secure computation technique.

Privacy-Preserving Microbiome Analysis Using Secure Computation

Abstract:Motivation: Developing targeted therapeutics and identifying biomarkers relies on large amounts of patient data. Beyond human DNA, researchers now investigate the DNA of micro-organisms inhabiting the human body. An individual's collection of microbial DNA consistently identifies that person and could be used to link a real-world identity to a sensitive attribute in a research dataset. Unfortunately, the current suite of DNA-specific privacy-preserving analysis tools does not meet the requirements for microbiome sequencing studies. Results: We augment an existing categorization of genomic-privacy attacks to incorporate microbiome sequencing and provide an implementation of metagenomic analyses using secure computation. Our implementation allows researchers to perform analysis over combined data without revealing individual patient attributes. We implement three metagenomic analyses and perform an evaluation on real datasets for comparative analysis. We use our implementation to simulate sharing data between four policy-domains and measure the increase in significant discoveries. Additionally, we describe an application of our implementation to form patient pools of data to allow drug companies to query against and compensate patients for the analysis. Abstract:DNA methylation indicates the individual's aging, so-called Epigenetic clocks, which will improve the research and diagnosis of aging diseases by investigating the correlation between methylation loci and human aging. Although this discovery has inspired many researchers to develop traditional computational methods to quantify the correlation and predict the chronological age, the performance bottleneck delayed access to the practical application. Since artificial intelligence technology brought great opportunities in research, we proposed a perceptron model integrating a channel attention mechanism named PerSEClock. The model was trained on 24,516 CpG loci that can utilize the samples from all types of methylation identification platforms and tested on 15 independent datasets against seven methylation-based age prediction methods. PerSEClock demonstrated the ability to assign varying weights to different CpG loci. This feature allows the model to enhance the weight of age-related loci while reducing the weight of irrelevant loci. The method is free to use for academics at www.dnamclock.com/#/original.

Abstract:Background Epigenetic clocks are promising tools for the study of aging in humans. The clocks quantify biological aging above and beyond chronological age, demonstrate systematic associations with risk factors that accelerate aging, and predict age-related morbidity and mortality. There is interest in using them as surrogate endpoints in intervention studies. However, the large number of clocks, decentralized publication and explosive popularity in the last decade has made for poor accessibility and standardization. This has hampered the abilities of new researchers to conduct truly hypothesis driven research—whether by not knowing about the best available clocks for a given question, or by systematically testing many or all as they become available. Results We report a centralized R package which can be installed and run locally on the user’s machine, and provides a standardized syntax for epigenetic clock calculation. The package includes a set of helper functions to assist with navigating clock literature and selecting clocks for analysis, as well as affording the user with the details of clock calculation. We describe each clock’s resilience to missing CpG information, combined with functionality to assess the need for imputation in the user’s own data. Furthermore, we demonstrate that while CpGs may not be shared among clocks with similar outputs, many clocks have highly correlated outputs. Conclusions Due to the previous decentralization of epigenetic clocks, gathering code and performing systematic analysis, particularly in protected datasets, has required significant information gathering effort. Here, we offer an R package with standardized implementation and potential for future growth and clock incorporation to assist with hypothesis driven investigation of aging as measured by epigenetic clocks. We show the potential of this package to drive the user to think globally about signals captured by epigenetic clocks, as well as to properly identify the potential and limitations of each clock in their current research.
Rare case of Helicobacter pylori-positive multiorgan IgG4-related disease and gastric cancer.

Abstract:A 61-year-old male from Northeast China presented with a 2-mo history of abdominal distension, pruritus and jaundice. Laboratory testing revealed an elevated serum IgG4 level. A computed tomography scan showed a typical feature of autoimmune pancreatitis (AIP) and cholecystocholangitis. Early gastric cancer was incidentally discovered when endoscopic untrasound-guided fine needle aspiration (EUS-FNA) of the pancreas was carried out. The patient underwent radical subtotal gastrectomy for gastric cancer combined with cholecystectomy. Helicobacter pylori (H. pylori) and IgG4-positive plasmacytes were detected in gastric cancer tissue, pancreatic EUS-FNA sample and resected gallbladder specimen by immunohistochemistry. The patient was diagnosed with H. pylori-positive IgG4-related AIP and sclerosing cholecystocholangitis as well as H. pylori-positive gastric cancer. He responded well to steroid therapy and remains healthy with no signs of recurrence at one year follow-up. We speculate that H. pylori might act as a trigger via direct or indirect action in the initiation of onset of gastric cancer and multiorgan IgG4-related disease.
