Abstract:Real-world healthcare data sharing is instrumental in constructing broader-based and larger clinical datasets that may improve clinical decision-making research and outcomes. Stakeholders are frequently reluctant to share their data without guaranteed patient privacy, proper protection of their datasets, and control over the usage of their data. Fully homomorphic encryption (FHE) is a cryptographic capability that can address these issues by enabling computation on encrypted data without intermediate decryptions, so the analytics results are obtained without revealing the raw data. This work presents a toolset for collaborative privacy-preserving analysis of oncological data using multiparty FHE. Our toolset supports survival analysis, logistic regression training, and several common descriptive statistics. We demonstrate using oncological datasets that the toolset achieves high accuracy and practical performance, which scales well to larger datasets. As part of this work, we propose a cryptographic protocol for interactive bootstrapping in multiparty FHE, which is of independent interest. The toolset we develop is general-purpose and can be applied to other collaborative medical and healthcare application domains.

Abstract:Privacy concerns on sharing sensitive data across institutions are particularly paramount for the medical domain, which hinders the research and development of many applications, such as cohort construction for cross-institution observational studies and disease surveillance. Not only that, the large volume and heterogeneity of the patient data pose great challenges for retrieval and analysis. To address these challenges, in this paper, we propose a Federated Patient Hashing (FPH) framework, which collaboratively trains a retrieval model stored in a shared memory while keeping all the patient-level information in local institutions. Specifically, the objective function is constructed by minimization of a similarity preserving loss and a heterogeneity digging loss, which preserves both inter-data and intra-data relationships. Then, by leveraging the concept of Bregman divergence, we implement optimization in a federated manner in both centralized and decentralized learning settings, without accessing the raw training data across institutions. In addition to this, we also analyze the convergence rate of the FPH framework. Extensive experiments on real-world clinical data set from critical care are provided to demonstrate the effectiveness of the proposed method on similar patient matching across institutions.

Abstract:Multisite medical data sharing is critical in modern clinical practice and medical research. The challenge is to conduct data sharing that preserves individual privacy and data usability. The shortcomings of traditional privacy-enhancing technologies mean that institutions rely on bespoke data sharing contracts. These contracts increase the inefficiency of data sharing and may disincentivize important clinical treatment and medical research. This paper provides a synthesis between two novel advanced privacy enhancing technologies (PETs): Homomorphic Encryption and Secure Multiparty Computation (defined together as Multiparty Homomorphic Encryption or MHE). These PETs provide a mathematical guarantee of privacy, with MHE providing a performance advantage over separately using HE or SMC. We argue MHE fulfills legal requirements for medical data sharing under the General Data Protection Regulation (GDPR) which has set a global benchmark for data protection. Specifically, the data processed and shared using MHE can be considered anonymized data. We explain how MHE can reduce the reliance on customized contractual measures between institutions. The proposed approach can accelerate the pace of medical research whilst offering additional incentives for healthcare and research institutes to employ common data interoperability standards.

Abstract:Background Data sharing in multicenter medical research can improve the generalizability of research, accelerate progress, enhance collaborations among institutions, and lead to new discoveries from data pooled from multiple sources. Despite these benefits, many medical institutions are unwilling to share their data, as sharing may cause sensitive information to be leaked to researchers, other institutions, and unauthorized users. Great progress has been made in the development of secure machine learning frameworks based on homomorphic encryption in recent years; however, nearly all such frameworks use a single secret key and lack a description of how to securely evaluate the trained model, which makes them impractical for multicenter medical applications. Objective The aim of this study is to provide a privacy-preserving machine learning protocol for multiple data providers and researchers (eg, logistic regression). This protocol allows researchers to train models and then evaluate them on medical data from multiple sources while providing privacy protection for both the sensitive data and the learned model. Methods We adapted a novel threshold homomorphic encryption scheme to guarantee privacy requirements. We devised new relinearization key generation techniques for greater scalability and multiplicative depth and new model training strategies for simultaneously training multiple models through x-fold cross-validation. Results Using a client-server architecture, we evaluated the performance of our protocol. The experimental results demonstrated that, with 10-fold cross-validation, our privacy-preserving logistic regression model training and evaluation over 10 attributes in a data set of 49,152 samples took approximately 7 minutes and 20 minutes, respectively. Conclusions We present the first privacy-preserving multiparty logistic regression model training and evaluation protocol based on threshold homomorphic encryption. Our protocol is practical for real-world use and may promote multicenter medical research to some extent.

Abstract:BackgroundThe deployment of Genome-wide association studies (GWASs) requires genomic information of a large population to produce reliable results. This raises significant privacy concerns, making people hesitate to contribute their genetic information to such studies.ResultsWe propose two provably secure solutions to address this challenge: (1) a somewhat homomorphic encryption (HE) approach, and (2) a secure multiparty computation (MPC) approach. Unlike previous work, our approach does not rely on adding noise to the input data, nor does it reveal any information about the patients. Our protocols aim to prevent data breaches by calculating the χ2 statistic in a privacy-preserving manner, without revealing any information other than whether the statistic is significant or not. Specifically, our protocols compute the χ2 statistic, but only return a yes/no answer, indicating significance. By not revealing the statistic value itself but only the significance, our approach thwarts attacks exploiting statistic values. We significantly increased the efficiency of our HE protocols by introducing a new masking technique to perform the secure comparison that is necessary for determining significance.ConclusionsWe show that full-scale privacy-preserving GWAS is practical, as long as the statistics can be computed by low degree polynomials. Our implementations demonstrated that both approaches are efficient. The secure multiparty computation technique completes its execution in approximately 2 ms for data contributed by one million subjects.

Abstract:The digitization of healthcare data has presented a pressing need to address privacy concerns within the realm of machine learning for healthcare institutions. One promising solution is federated learning, which enables collaborative training of deep machine learning models among medical institutions by sharing model parameters instead of raw data. This study focuses on enhancing an existing privacy-preserving federated learning algorithm for medical data through the utilization of homomorphic encryption, building upon prior research. In contrast to the previous paper, this work is based upon Wibawa, using a single key for HE, our proposed solution is a practical implementation of a preprint with a proposed encryption scheme (xMK-CKKS) for implementing multi-key homomorphic encryption. For this, our work first involves modifying a simple “ring learning with error” RLWE scheme. We then fork a popular federated learning framework for Python where we integrate our own communication process with protocol buffers before we locate and modify the library’s existing training loop in order to further enhance the security of model updates with the multi-key homomorphic encryption scheme. Our experimental evaluations validate that, despite these modifications, our proposed framework maintains a robust model performance, as demonstrated by consistent metrics including validation accuracy, precision, f1-score, and recall.

Abstract:The Cox proportional hazards model is one of the most widely used methods for analyzing survival data. Data from multiple data providers are required to improve the generalizability and confidence of the results of Cox analysis; however, such data sharing may result in leakage of sensitive information, leading to financial fraud, social discrimination or unauthorized data abuse. Some privacy-preserving Cox regression protocols have been proposed in past years, but they lack either security or functionality. In this paper, we propose a privacy-preserving Cox regression protocol for multiple data providers and researchers. The proposed protocol allows researchers to train models on horizontally or vertically partitioned datasets while providing privacy protection for both the sensitive data and the trained models. Our protocol utilizes threshold homomorphic encryption to guarantee security. Experimental results demonstrate that with the proposed protocol, Cox regression model training over 9 variables in a dataset of 113,035 samples takes approximately 44 min, and the trained model is almost the same as that obtained with the original non-secure Cox regression protocol; therefore, our protocol is a potential candidate for practical real-world applications in multicenter medical research.

Abstract:In multicentric studies, data sharing between institutions might negatively impact patient privacy or data security. An alternative is federated analysis by secure multiparty computation. This pilot study demonstrates an architecture and implementation addressing both technical challenges and legal difficulties in the particularly demanding setting of clinical research on cancer patients within the strict European regulation on patient privacy and data protection: 24 patients from LMU University Hospital in Munich, Germany, and 24 patients from Policlinico Universitario Fondazione Agostino Gemelli, Rome, Italy, were treated for adrenal gland metastasis with typically 40 Gy in 3 or 5 fractions of online-adaptive radiotherapy guided by real-time MR. High local control (21% complete remission, 27% partial remission, 40% stable disease) and low toxicity (73% reporting no toxicity) were observed. Median overall survival was 19 months. Federated analysis was found to improve clinical science through privacy-friendly evaluation of patient data in the European health data space.

Abstract:Federated learning-based medical data privacy sharing can promote the development of medical industry intelligence, but limited by its own security and privacy deficiencies, federated learning still suffers from a single point of failure and privacy leakage of intermediate parameters. To address these problems, this paper proposes a privacy protection framework for medical data based on blockchain and cross-silo federated learning, using cross-silo federated learning to establish a collaborative training platform for multiple medical institutions to enhance the privacy of medical data, introducing blockchain and smart contracts to realize decentralized federated learning to enhance trust between distrustful medical institutions and solve the problem of a single point of failure. In addition, a secure aggregation scheme is designed using threshold homomorphic encryption to prevent the privacy leakage problem during parameter transmission. The experimental and analytical results show that the accuracy of this paper's scheme is consistent with the original federated learning scheme, effectively deals with the problems of single-point failure and inference attacks of federated learning, improves system robustness, and is suitable for medical scenarios with more stringent requirements on security and accuracy.

Homomorphic Encryption: An Application to Polygenic Risk Scores

Abstract:Background: Polygenic risk scores (PRS) have emerged as a powerful tool in precision medicine, enabling personalized risk assessments for complex diseases. However, using sensitive genomic data in PRS calculations raises concerns about privacy and security. Fully Homomorphic Encryption (FHE) offers a promising solution by allowing computations on encrypted data, preserving the privacy of both genomic information and PRS models. Results: In this study, we present a novel application of FHE for secure and private PRS calculations using the CKKS protocol within the Lattigo library. Our approach involves a three-party system: clients (doctors with sensitive genetic data), modelers developing a PRS (academics or a company), and evaluators (a "local hospital" running the models while maintaining data confidentiality). We demonstrate the feasibility and accuracy of our protocol by applying it to synthetic datasets of various sizes and a robust 110k-SNP model for schizophrenia. The normal PRS calculation results are essentially identical to the encrypted calculation: between the two results is .999 & MSE is 2.27 times 10 . Moreover, while the encrypted calculation is roughly 1000 times slower than conventional non-encrypted ones (when only considering the core PRS calculation), it is quite feasible on a single-CPU node - e.g. running on ≈ 1100 individuals with ≈ 110k SNPS took six minutes and ≈ 65G memory on a laptop computer. In addition, we investigate the impact of encryption parameters (modulus) on this computational time and accuracy in detail. Conclusion: By enabling secure PRS calculations on encrypted genomic data, our approach addresses the pressing need for privacy-preserving solutions in the era of precision medicine. The ability to perform accurate risk assessments while maintaining patient confidentiality paves the way for broader adoption of PRS and personalized medicine in healthcare, particularly with the advent of large-scale computing power.

Abstract:The digitalization of the medical field has resulted in a vast amount of medical data stored in Electronic Health Record (EHR) systems. Among the various valuable applications of EHRs, medication efficacy analysis stands out as a powerful tool for drug development and enhancing patient treatment. This analysis relies on multi-dimensional medical data, encompassing various factors. Currently, medical research is increasingly inclined towards inter-institutional sharing of medical data to enhance the accuracy and reliability of analysis results. However, due to the sensitivity of personal medical data, privacy and security considerations must be taken into account when conducting medical analyses across institutions. In this paper, we present a novel privacy-preserving scheme for distributed medication efficacy analysis. Our work allows for the analysis of multi-dimensional medical data by leveraging the private set intersection protocol and the Paillier cryptosystem. To validate the feasibility of our scheme, we implement it using some real medical datasets. Experimental results demonstrate that our approach offers enhanced security and lower computational and communication overhead compared to the original approach, which solely processes single-dimensional data at a time.

Abstract:Federated Learning (FL) is a machine learning methodology that allows remote devices to collectively train a learning system without sharing their data. FL-based methods provide enhanced secure privacy by transmitting only localized model variables, learned with local information, from dispersed devices to a centralized controller. There is a potential for a centralized server or malicious individuals to deduce or get sensitive private data by analyzing the structure and variables of regional learning networks. This study incorporates the FL process into the deep learning process of medical prototypes in an Internet of Things (IoT)-based medical facility. A Secured Medical Homomorphic Encryption Algorithm (SMHEA) is proposed in this research to ensure medical data privacy. Cryptographic primitives, such as masking and homomorphic cryptography, are used to enhance the security of local modeling. This prevents adversaries from deducing confidential health information via assaults like model restoration or modeling inversion. The primary determinant for assessing the regional modeling's contributions to the universal model during each training stage is the quality of the databases possessed by various individuals rather than the typically used metric of database dimension in deep learning. A dropout-tolerant approach is suggested, where the FL procedure would continue as long as the total amount of online customers remains over a certain level. By doing a security evaluation, it is evident that the suggested approach effectively ensures data privacy. Theoretical analysis is conducted on accuracy, computation time, and communication error. An example of clinical applications is the categorization of skin lesions using training photos from the HAM10000 medical database. The experimental findings demonstrate that the suggested system had favorable performance and privacy preservation outcomes compared to current methods.

Abstract:Federated learning avoids centralizing data in a central server by distributing the model training process across devices, thus protecting privacy to some extent. However, existing research shows that model updates (e.g., gradients or weights) exchanged during federated learning may still indirectly leak sensitive information about the original data. Currently, single-key homomorphic encryption methods applied in federated learning cannot solve the problem of privacy leakage that may be caused by the collusion between the participant and the federated learning server, whereas existing privacy-preserving federated learning schemes based on multi-key homomorphic encryption in semi-honest environments have deficiencies and limitations in terms of security and application conditions. To this end, this paper proposes a privacy-preserving federated learning scheme based on multi-key fully homomorphic encryption to cope with the potential risk of privacy leakage in traditional federated learning. We designed a multi-key fully homomorphic encryption scheme, mMFHE, that encrypts by aggregating public keys and requires all participants to jointly participate in decryption sharing, thus ensuring data security and privacy. The proposed privacy-preserving federated learning scheme encrypts the model updates through multi-key fully homomorphic encryption, ensuring confidentiality under the CRS model and in a semi-honest environment. As a fully homomorphic encryption scheme, mMFHE supports homomorphic addition and homomorphic multiplication for more flexible applications. Our security analysis proves that the scheme can withstand collusive attacks by up to N−1 users and servers, where N is the total number of users. Performance analysis and experimental results show that our scheme reduces the complexity of the NAND gate, which reduces the computational load and improves the efficiency while ensuring the accuracy of the model.

Abstract:Decentralized deep learning plays a key role in collaborative model training due to its attractive properties, including tolerating high network latency and less prone to single-point failures. Unfortunately, such a training mode is more vulnerable to data privacy leaks compared to other distributed training frameworks. Existing efforts exclusively use differential privacy as the cornerstone to alleviate the data privacy threat. However, it is still not clear whether differential privacy can provide a satisfactory utility-privacy trade-off for model training, due to its inherent contradictions. To address this problem, we propose D-MHE, the first secure and efficient decentralized training framework with lossless precision. Inspired by the latest developments in the homomorphic encryption technology, we design a multiparty version of Brakerski-Fan-Vercauteren (BFV), one of the most advanced cryptosystems, and use it to implement private gradient updates of users'local models. D-MHE can reduce the communication complexity of general Secure Multiparty Computation (MPC) tasks from quadratic to linear in the number of users, making it very suitable and scalable for large-scale decentralized learning systems. Moreover, D-MHE provides strict semantic security protection even if the majority of users are dishonest with collusion. We conduct extensive experiments on MNIST and CIFAR-10 datasets to demonstrate the superiority of D-MHE in terms of model accuracy, computation and communication cost compared with existing schemes.

Abstract:The need for data privacy and security -- enforced through increasingly strict data protection regulations -- renders the use of healthcare data for machine learning difficult. In particular, the transfer of data between different hospitals is often not permissible and thus cross-site pooling of data not an option. The Personal Health Train (PHT) paradigm proposed within the GO-FAIR initiative implements an 'algorithm to the data' paradigm that ensures that distributed data can be accessed for analysis without transferring any sensitive data. We present PHT-meDIC, a productively deployed open-source implementation of the PHT concept. Containerization allows us to easily deploy even complex data analysis pipelines (e.g, genomics, image analysis) across multiple sites in a secure and scalable manner. We discuss the underlying technological concepts, security models, and governance processes. The implementation has been successfully applied to distributed analyses of large-scale data, including applications of deep neural networks to medical image data.

Abstract:The privacy-preserving problem for distributed fusion estimation scheme is concerned in this paper. Due to the existence of potential eavesdropper, the data exchanged among sensors, FC and user imperatively require privacy preservation. Hence, we propose a distributed confidentiality fusion structure against eavesdropper by using Paillier homomorphic encryption approach. In this case, FC cannot acquire real values of local state estimates, while it only helps calculate encrypted DFEs. Then, the legitimate user can successfully obtain the true values of DFEs according to the encrypted information and secret keys, which is based on the homomorphism of encryption. Finally, an illustrative example is provided to verify the effectiveness of the proposed methods.

Abstract:Patient datasets contain confidential information which is protected by laws and regulations such as HIPAA and GDPR. Ensuring comprehensive patient information necessitates privacy-preserving entity resolution (PPER), which identifies identical patient entities across multiple databases from different healthcare organizations while maintaining data privacy. Existing methods often lack cryptographic security or are computationally impractical for real-world datasets. We introduce a PPER pipeline based on AMPPERE, a secure abstract computation model utilizing cryptographic tools like homomorphic encryption. Our tailored approach incorporates extensive parallelization techniques and optimal parameters specifically for patient datasets. Experimental results demonstrate the proposed method's effectiveness in terms of accuracy and efficiency compared to various baselines.

Abstract:Machine Learning (ML) has become one of the most impactful fields of data science in recent years. However, a significant concern with ML is its privacy risks due to rising attacks against ML models. Privacy-Preserving Machine Learning (PPML) methods have been proposed to mitigate the privacy and security risks of ML models. A popular approach to achieving PPML uses Homomorphic Encryption (HE). However, the highly publicized inefficiencies of HE make it unsuitable for highly scalable scenarios with resource-constrained devices. Hence, Hybrid Homomorphic Encryption (HHE) -- a modern encryption scheme that combines symmetric cryptography with HE -- has recently been introduced to overcome these challenges. HHE potentially provides a foundation to build new efficient and privacy-preserving services that transfer expensive HE operations to the cloud. This work introduces HHE to the ML field by proposing resource-friendly PPML protocols for edge devices. More precisely, we utilize HHE as the primary building block of our PPML protocols. We assess the performance of our protocols by first extensively evaluating each party's communication and computational cost on a dummy dataset and show the efficiency of our protocols by comparing them with similar protocols implemented using plain BFV. Subsequently, we demonstrate the real-world applicability of our construction by building an actual PPML application that uses HHE as its foundation to classify heart disease based on sensitive ECG data.
