Abstract:Background and purpose: Clinical targeted volume (CTV) delineation accounting for the patient-specific microscopic tumor spread can be a difficult step in defining the treatment volume. We developed an intelligent and automated CTV delineation system for locally advanced non-small cell lung carcinoma (NSCLC) to cover the microscopic tumor spread while avoiding organs-at-risk (OAR). Materials and methods: A 3D UNet with a customized loss function was used, which takes both the patients' respiration-correlated ("4D") CT scan and the physician contoured internal gross target volume (iGTV) as inputs, and outputs the CTV delineation. Among the 84 identified patients, 60 were randomly selected to train the network, and the remaining as testing. The model performance was evaluated and compared with cropped expansions using the shape similarities to the physicians' contours (the ground-truth) and the avoidance of critical OARs. Results: On the testing datasets, all model-predicted CTV contours followed closely to the ground truth, and were acceptable by physicians. The average dice score was 0.86. Our model-generated contours demonstrated better agreement with the ground-truth than the cropped 5 mm/8 mm expansion method (median of median surface distance of 1.0 mm vs 1.9 mm/2.0 mm), with a small overlap volume with OARs (0.4 cm(3) for the esophagus and 1.2 cm(3) for the heart). Conclusions: The CTVs generated by our CTV delineation system agree with the physician's contours. This approach demonstrates the capability of intelligent volumetric expansions with the potential to be used in clinical practice.

Abstract:Purpose: To investigate whether a deep learning-assisted contour (DLAC) could provide greater accuracy, inter-observer consistency, and efficiency compared with a manual contour (MC) of the clinical target volume (CTV) for non-small cell lung cancer (NSCLC) receiving postoperative radiotherapy (PORT).Materials and Methods: A deep dilated residual network was used to achieve the effective automatic contour of the CTV. Eleven junior physicians contoured CTVs on 19 patients by using both MC and DLAC methods independently. Compared with the ground truth, the accuracy of the contour was evaluated by using the Dice coefficient and mean distance to agreement (MDTA). The coefficient of variation (CV) and standard distance deviation (SDD) were rendered to measure the inter-observer variability or consistency. The time consumed for each of the two contouring methods was also compared.Results: A total of 418 CTV sets were generated. DLAC improved contour accuracy when compared with MC and was associated with a larger Dice coefficient (mean ± SD: 0.75 ± 0.06 vs. 0.72 ± 0.07, p < 0.001) and smaller MDTA (mean ± SD: 2.97 ± 0.91 mm vs. 3.07 ± 0.98 mm, p < 0.001). The DLAC was also associated with decreased inter-observer variability, with a smaller CV (mean ± SD: 0.129 ± 0.040 vs. 0.183 ± 0.043, p < 0.001) and SDD (mean ± SD: 0.47 ± 0.22 mm vs. 0.72 ± 0.41 mm, p < 0.001). In addition, a value of 35% of time saving was provided by the DLAC (median: 14.81 min vs. 9.59 min, p < 0.001).Conclusions: Compared with MC, the DLAC is a promising strategy to obtain superior accuracy, consistency, and efficiency for the PORT-CTV in NSCLC.

Abstract:Patient-specific clinical target volume (CTV) delineation is the most difficult step in defining the planning target volume from the gross tumor volume. Delineating patient specific CTVs is a labor-intensive process, prone to human error, with poor inter-observer consistency. Automated volumetric expansions have been used to reduce these concerns, but these ignore patient specific anatomic and clinical variables, as well as the clinical experience of the radiation oncologist. We developed an automated CTV delineation system for lung cancer, which was trained to learn the physician’s CTV contour pattern using a deep 3D neural network. Our network took both patients’ CT images and their corresponding internal target volume (ITV) as input, and output CTV delineation. ITVs were contoured by physicians after studying the internal motion of the target from the 4D CT scans. In our study, the network was trained from twenty-five NSCLC patients in right upper lung lobe (RUL). Then, the trained network was tested on four additional datasets from RUL NSCLC patients. Each CTV delineation output by the trained network was compared with the corresponding CTV contoured by physicians. One challenge to train a 3D network for CTV delineation is the high imbalance between the CTV volume and the background volume. Instead of using hyperparameters to balance as existing methods did, we directly optimize a loss function robust to high imbalance. This improves the stability of the model, especially when the balance dynamically changes in images. Three out of four CTV contours that were automatically delineated by our trained model agree very well with the physician’s CTV contour. Only one has visible discrepancies near the apex of the upper lobe, but still looks reasonable. As shown in Table 1, the performance of the trained neural network was also quantitatively evaluated using precision and recall between the predicted CTVs and the ground truth CTVs. The average precision is 0.902, and recall is 0.871. The recall and precision are the fraction of true volume among the total ground-truth and the fraction of true volume among the predicted volume, respectively. The automated CTV delineation was utilized for RUL NSCLC patients using the deep learning method solely based on CT images with manually contoured ITV. On the limited testing samples, the model contoured CTVs reasonably well. Abstract:Accurate delineation of clinical target volume (CTV) is one of the most crucial aspects of treatment planning in radiation therapy. However, the quality of this process depends on the expertise level of the individual observer. This study aimed to investigate whether deep-learning based automatic delineation could provide greater accuracy, inter-observer consistency and efficiency of CTV contour compared with manual delineation for pN2 NSCLC receiving post-operation radiation therapy (PORT). A very deep dilated residual network was used to achieve the effective automatic delineation of CTV. The dilated module with different atrous rates is able to extract multi-scale features from CT, leading to a greater robustness of the model. Eleven junior radiation oncologists (work experience ≤ 5 years) delineated CTV on 19 patients using two contour methods: (1) a manual contour from scratch (MC) and (2) a user adjustment of the deep-learning based auto-delineation contour (UADC). Three senior radiation oncologists (work experience > 10 years) also contoured the CTV on these patients and the majority voting was used to generate the ground truth (GT). The accuracy of junior’s delineation was evaluated in terms of mean distance to agreement (MDA) and Dice similarity coefficient (DSC) with the GT as reference. MDA is a distance parameter while DSC is a volume overlap index. Therefore, smaller MDA and higher DSC indicate a greater contour accuracy. The coefficient of variation (COV) was rendered to measure the inter-observer variability. COV is defined as the standard deviation (SD) divided by the mean CTV volume of all observers and a larger COV implies a greater variability. For each of the CTV contouring tasks, the time consumption was recorded. A total of 418 unique CTV sets were generated. The UADC presented a significantly smaller MDA (mm) per individual CTV set compared with MC (mean ± SD: 2.79 ± 0.91 vs. 3.07 ± 0.98, P < 0.001). Similarly, the DSC of UADC was also significantly greater than that of MC (mean ± SD: 0.75 ± 0.06 vs. 0.72 ± 0.07, P < 0.001). For inter-observer variability, UADC introduced a remarkably decreased COV in comparison with MC (mean ± SD: 0.129 ± 0.040 vs. 0.183 ± 0.043, P < 0.001). The median contouring time for MC and UADC was 14.99 min (P25, P75: 9.80, 20.87) and 10.42 min (P25, P75: 7.20, 17.03) respectively, resulting in a 31% reduction (absolute: 4.57 min) of time consumption (P < 0.001). Compared with the manual contour, user adjustment of deep-learning based auto-delineation of PORT-CTV for pN2 NSCLC is a promising strategy to offer superior accuracy and consistency within a shortened time span, leading to higher quality and efficiency of CTV contouring.
Deep learning based clinical target volumes contouring for prostate cancer: Easy and efficient application

Abstract:Background: Radiotherapy has been crucial in prostate cancer treatment. However, manual segmentation is labor intensive and highly variable among radiation oncologists. In this study, a deep learning based automated contouring model is constructed for clinical target volumes (CTVs) of intact and postoperative prostate cancer. Methods: Computed tomography (CT) data sets of 197 prostate cancer patients were collected. Two auto-delineation models were built for radical radiotherapy and postoperative radiotherapy of prostate cancer respectively, and each model included CTVn for pelvic lymph nodes and CTVp for prostate tumors or prostate tumor beds. Results: In the radical radiotherapy model, the volumetric dice (VD) coefficient of CTVn calculated by AI, was higher than that of the one delineated by the junior physicians (0.85 vs. 0.82, p = 0.018); In the postoperative radiotherapy model, the quantitative parameter of CTVn and CTVp, counted by AI, was better than that of the junior physicians. The median delineation time for AI was 0.23 min in the postoperative model and 0.26 min in the radical model, which were significantly shorter than those of the physicians (50.40 and 45.43 min, respectively, p < 0.001). The correction time of the senior physician for AI was much shorter compared with that for the junior physicians in both models (p < 0.001). Conclusion: Using deep learning and attention mechanism, a highly consistent and time-saving contouring model was built for CTVs of pelvic lymph nodes and prostate tumors or prostate tumor beds for prostate cancer, which also might be a good approach to train junior radiation oncologists.

Abstract:Background:Postoperative radiotherapy (PORT) is an important treatment for lung cancer patients with poor prognostic features, but accurate delineation of the clinical target volume (CTV) and organs at risk (OARs) is challenging and time-consuming. Recently, deep learning-based artificial intelligent (AI) algorithms have shown promise in automating this process.Objective:To evaluate the clinical utility of a deep learning-based auto-segmentation model for AI-assisted delineating CTV and OARs in patients undergoing PORT, and to compare its accuracy and efficiency with manual delineation by radiation oncology residents from different levels of medical institutions.Methods:We previously developed an AI auto-segmentation model in 664 patients and validated its contouring performance in 149 patients. In this multi-center, validation trial, we prospectively involved 55 patients and compared the accuracy and efficiency of 3 contouring methods: (i) unmodified AI auto-segmentation, (ii) fully manual delineation by junior radiation oncology residents from different medical centers, and (iii) manual modifications based on AI segmentation model (AI-assisted delineation). The ground truth of CTV and OARs was delineated by 3 senior radiation oncologists. Contouring accuracy was evaluated by Dice similarity coefficient (DSC), Hausdorff distance (HD), and mean distance of agreement (MDA). Inter-observer consistency was assessed by volume and coefficient of variation (CV).Results:AI-assisted delineation achieved significantly higher accuracy compared to unmodified AI auto-contouring and fully manual delineation by radiation oncologists, with median HD, MDA, and DCS values of 20.03 vs. 21.55 mm, 2.57 vs. 3.06 mm, 0.745 vs. 0.703 (all P<0.05) for CTV, respectively. The results of OARs contours were similar. CV for OARs was reduced by approximately 50%. In addition to better contouring accuracy, the AI-assisted delineation significantly decreased the consuming time and improved the efficiency.Conclusion:AI-assisted CTV and OARs delineation for PORT significantly improves the accuracy and efficiency in the real-world setting, compared with pure AI auto-segmentation or fully manual delineation by junior oncologists. AI-assisted approach has promising clinical potential to enhance the quality of radiotherapy planning and further improve treatment outcomes of patients with lung cancer.
Autocontouring of primary lung lesions and nodal disease for radiotherapy based only on computed tomography images

Abstract:Background and purpose: In many clinics, positron-emission tomography is unavailable and clinician time extremely limited. Here we describe a deep-learning model for autocontouring gross disease for patients undergoing palliative radiotherapy for primary lung lesions and/or hilar/mediastinal nodal disease, based only on computed tomography (CT) images. Materials and methods: An autocontouring model (nnU-Net) was trained to contour gross disease in 379 cases (352 training, 27 test); 11 further test cases from an external centre were also included. Anchor-point-based post-processing was applied to remove extraneous autocontoured regions. The autocontours were evaluated quantitatively in terms of volume similarity (Dice similarity coefficient [DSC], surface Dice coefficient, 95th percentile Hausdorff distance [HD95], and mean surface distance), and scored for usability by two consultant oncologists. The magnitude of treatment margin needed to account for geometric discrepancies was also assessed. Results: The anchor point process successfully removed all erroneous regions from the autocontoured disease, and identified two cases to be excluded from further analysis due to 'missed' disease. The average DSC and HD95 were 0.8 ± 0.1 and 10.5 ± 7.3 mm, respectively. A 10-mm uniform margin-distance applied to the autocontoured region was found to yield "full coverage" (sensitivity > 0.99) of the clinical contour for 64 % of cases. Ninety-seven percent of evaluated autocontours were scored by both clinicians as requiring no or minor edits. Conclusions: Our autocontouring model was shown to produce clinically usable disease outlines, based on CT alone, for approximately two-thirds of patients undergoing lung radiotherapy. Further work is necessary to improve this before clinical implementation.

Abstract:BACKGROUND AND PURPOSE: The delineation of the gross tumor volume (GTV) is a critical step for radiation therapy treatment planning. The delineation procedure is typically performed manually which exposes two major issues: cost and reproducibility. Delineation is a time-consuming process that is subject to inter- and intra-observer variability. While methods have been proposed to predict GTV contours, typical approaches ignore variability and therefore fail to utilize the valuable confidence information offered by multiple contours.MATERIALS AND METHODS: In this work we propose an automatic GTV contouring method for soft-tissue sarcomas from X-ray computed tomography (CT) images, using deep learning by integrating inter- and intra-observer variability in the learned model. Sixty-eight patients with soft tissue and bone sarcomas were considered in this evaluation, all underwent pre-operative CT imaging used to perform GTV delineation. Four radiation oncologists and radiologists performed three contouring trials each for all patients. We quantify variability by defining confidence levels based on the frequency of inclusion of a given voxel into the GTV and use a deep convolutional neural network to learn GTV confidence maps.RESULTS: Results were compared to confidence maps from the four readers as well as ground-truth consensus contours established jointly by all readers. The resulting continuous Dice score between predicted and true confidence maps was 87% and the Hausdorff distance was 14 mm.CONCLUSION: Results demonstrate the ability of the proposed method to predict accurate contours while utilizing variability and as such it can be used to improve clinical workflow.

Automated Lung Cancer Segmentation Using a Dual-Modality Deep Learning Network with PET and CT Images

Abstract:Purpose/Objective(s) To develop an automated lung cancer segmentation method using dual-modality imaging and deep learning, and to perform clinical evaluation of the method. Materials/Methods A 3D neural network with dual inputs from diagnostic PET and simulation CT was constructed based on U-Net. The architecture consisted of two parallel convolution paths for independent feature extractions from PET and CT at multiple resolution levels and a single deconvolution path. At each resolution level, the extracted features from the convolution arms were concatenated and fed into the deconvolution path through skip connections. The network was trained/validated/tested by a 3:1:1 split on a dataset of 290 pairs of PET and CT from lung cancer patients treated at our institution, with manual physician contours as the ground truth. The performance of the 3D dual-modality network was compared against that of a CT-only network. evaluated both the manual and the network-produced tumor contours of a randomly selected subset of 20 cases (10 large and 10 small) in a blinded fashion. Results The mean Dice similarity coefficient (DSC), Hausdorff Distance (HD), and bi-directional local distance (BLD) comparing the automatic contours versus the ground truth were 0.77 ± 0.12, 7.6 ± 4.7 mm, and 2.9 ± 1.4 mm, and 0.79 ± 0.10, 5.8 ± 3.2 mm, and 2.8 ± 1.5 mm for dual modality inputs, respectively. The stratification method delivered the best results when the model for the large GTV subset (> 25 ml) was trained with GTVs of all sizes (DSC, HD, BLD of 0.85 ± 0.05, 9.5 ± 3.8 mm, and 3.8 ± 1.8 mm), and that for the small GTV subset (< 25 ml) was trained with small GTVs only (DSC, HD, BLD of 0.82 ± 0.08, 3.7 ± 1.7 mm, and 2.2 ± 1.1 mm). From the stratified results, the best combined overall DSC, HD, and BLD were 0.83 ± 0.07, 5.9 ± 2.5 mm, and 2.8 ± 1.4 mm, respectively. In the multi-observer review, on average 91.25% of manual vs. 88.75% of automatic contours were Accepted or Accepted with Modifications. (50% Accepted, 41.25% Accepted w/ Mods and 6.25% Rejected for manual vs. 18.75%, 70%, and 8.75% for automatic), the modifications on the automatic contours were relatively minor with a mean DSC of 0.92 ± 0.04 between the original and modified, comparable to the mean DSC of 0.90 ± 0.04 between all the modified contours and their manual ground truths. Conclusion By utilizing an expansive clinical PET and CT image database and a dual-modality architecture, the proposed 3D network with a novel GTV volume-based stratification strategy was able to generate clinically useful lung cancer contours that were quantitatively similar to the ground truth and highly acceptable in physician review.

Comparative Clinical Evaluation of Deep-Learning-Based Algorithms in Auto-Segmentation of Organs-At-Risk for Head and Neck Cancers

Abstract:This IRB-approved study evaluated the quality of contours auto-generated by two deep learning (DL) contouring algorithms for organs-at-risk (OAR) volumes in head and neck cancers. Eleven consecutive head and neck (HandN) patients treated by Tomotherapy were selected for evaluation. Dose prescriptions ranged from 60-70Gy in 30-35 fractions. Each patient had three sets of OAR volumes generated, one clinically used and drawn by humans (physician and dosimetrist) and two auto-generated with DL contouring solutions, trained using convolutional neural network algorithms in large external datasets. The two DL models used for comparison were a HandN model (DLCExpert, Mirada Medical, Oxford, UK) and a Ua-Net model (DeepVoxel Inc, Irvine, CA). Using human-generated volumes as the ground truth, we evaluated the performance of these two models using 3 spatial overlap based metrics (Dice coefficient, Jaccard index(JAC) and True positive rate sensitivity(TPR)), 2 surface distance metrics (95% Hausdorff distance(HD) and average distance(AD)), and 1 volume matrix (volume similarity index(VS)). Seventeen common OAR structures were evaluated including brachial plexus, brainstem, esophagus, eyes, larynx, lenses, mandible, optical nerves and chiasm, oral cavity, parotids, pharyngeal constrictors (PC), submandibular glands (SMGs), spinal cord and trachea. Both DL models offered a feasible solution to delineate structures from CT images. The Mirada model had only 10 common organs for comparison. As shown in Table 1, both models produced comparable results while the DeepVoxel matched human contour better in most OARs. Different image segmentation metrics showed consistent results. DL contours were most similar to human generated contours for brainstem, esophagus, eyes, larynx, lens, mandible, parotids, SMGs, spinal cord and trachea where Dice, JAC, TPR, HD, AD, VS in DeepVoxel model were 0.80(range 0.68-0.91), 0.68(0.53-0.84), 0.78(0.67-0.94), 5.1(2.1-11.4), 1.9mm(1.0-3.9) and 0.88(0.76-0.97) respectively. Brachial plexus, optical nerves and chiasm, oral cavity and PC still needed improvement, partly due to the differences in organ definition. For example, teeth were included in DeepVoxel's oral cavity but not in Mirada and human-generated contours. Those discrepancies will be corrected in our next DL model. DL auto-generated contours from two different models showed high similarity to human generated ones for a variety of OARs in the head and neck, with potential to be adopted in routine clinical practice. In contrast with atlas-based or active shape model approaches, DL models are capable of producing contours with a high level of clinical acceptance and show promise to be indistinguishable from human generated ones. Abstract:To quantitatively and qualitatively evaluate a deep learning auto contouring model for prostate radiotherapy patients with pretreatment insertion of a hydrogel spacer (about water equivalent with no contrast) between prostate and rectum. The model employs convolutional neural networks (CNN) to learn features from input images that can be used to generate semantic segmentation. The study used 163 patients from three specialized GU radiation oncologists (referred to as A/B/C). The first 135 patients (A/B/C = 82/39/14) were used for training (125) and validation (10). The validation patients were randomly selected. The validated model was tested on 28 patients (A/B/C = 18/6/4) accrued during model development. There was no change of practice during the whole period. A simulation CT and MR were taken on the same day for each patient. In manual contouring, with MR fused to CT, spacer was contoured on T2 MR, prostate on CT with MR guidance, and other structures on CT only. The model was trained to auto contour prostate, proximal seminal vesicles (SV), bladder, rectum, penile bulb, femurs and spacer on CT without MR. Quantitatively, auto contours were evaluated against manual contours using the following metrics: sensitivity (% of voxels correctly drawn), false positive rate (FPR, % of voxels overdrawn), dice similarity coefficient (DSC), 95-percentile of Hausdorff distance (HD) and mean distances (dmean) between the two contours over all slices. The structures with high DSC were qualitatively evaluated by the original attending using a 1 (acceptable with minor editing), 2 (editable with efficiency gain over manual contouring) and 3 (rejected for no efficiency gain or gross error) scoring system. A gross error on rectum occurred for two patients (A/B = 1/1). These two points were excluded from quantitative analysis but counted as rejected in qualitative evaluation. On average, DSC was high for femurs (>0.95) and bladder (0.91), moderate for prostate (0.85) and rectum (0.81), but low for bulb (0.67), proximal SV (0.62) and spacer (0.52). For right femur/left femur/bladder/prostate/rectum, sensitivity = 0.93/0.92/0.88/0.86/0.81, FPR = 1.8%/1.5%/4.5%/15%/17%, 95% 95%-HD = 2.8/2.6/12.1/7.4/9.5 mm, and dmean = 0.9/1.0/2.6/2.5/2.4 mm. Qualitatively, femurs scored 1 in all cases. The average scores for bladder/prostate/rectum = 1.28/1.44/1.50, 1.83/2.17/1.67, 1.25/1.50/1.25 for physicians A, B, C, respectively, and 1.39/1.61/1.50 overall. Prostate and rectum both scored well below 2, despite their lower quantitative performance, as some errors caused by the inaccurate prediction of spacer without MR were deemed easily correctable by the physicians. The model produced clinically satisfactory results, both quantitatively and qualitatively, for femurs, bladder, prostate and rectum. The results for proximal SV and bulb were less ideal. The model drew the spacer in the correct location, but could not draw it accurately due to lack of contrast on CT.
Deep learning‐based auto‐segmentation of clinical target volumes for radiotherapy treatment of cervical cancer

Abstract:OBJECTIVES: Because radiotherapy is indispensible for treating cervical cancer, it is critical to accurately and efficiently delineate the radiation targets. We evaluated a deep learning (DL)-based auto-segmentation algorithm for automatic contouring of clinical target volumes (CTVs) in cervical cancers.METHODS: Computed tomography (CT) datasets from 535 cervical cancers treated with definitive or postoperative radiotherapy were collected. A DL tool based on VB-Net was developed to delineate CTVs of the pelvic lymph drainage area (dCTV1) and parametrial area (dCTV2) in the definitive radiotherapy group. The training/validation/test number is 157/20/23. CTV of the pelvic lymph drainage area (pCTV1) was delineated in the postoperative radiotherapy group. The training/validation/test number is 272/30/33. Dice similarity coefficient (DSC), mean surface distance (MSD), and Hausdorff distance (HD) were used to evaluate the contouring accuracy. Contouring times were recorded for efficiency comparison.RESULTS: The mean DSC, MSD, and HD values for our DL-based tool were 0.88/1.32 mm/21.60 mm for dCTV1, 0.70/2.42 mm/22.44 mm for dCTV2, and 0.86/1.15 mm/20.78 mm for pCTV1. Only minor modifications were needed for 63.5% of auto-segmentations to meet the clinical requirements. The contouring accuracy of the DL-based tool was comparable to that of senior radiation oncologists and was superior to that of junior/intermediate radiation oncologists. Additionally, DL assistance improved the performance of junior radiation oncologists for dCTV2 and pCTV1 contouring (mean DSC increases: 0.20 for dCTV2, 0.03 for pCTV1; mean contouring time decrease: 9.8 min for dCTV2, 28.9 min for pCTV1).CONCLUSIONS: DL-based auto-segmentation improves CTV contouring accuracy, reduces contouring time, and improves clinical efficiency for treating cervical cancer.

Abstract:Background: The current clinical workflow for esophageal gross tumor volume (GTV) contouring relies on manual delineation of high labor-costs and interuser variability. Purpose: To validate the clinical applicability of a deep learning (DL) multi-modality esophageal GTV contouring model, developed at 1 institution whereas tested at multiple ones. Methods and Materials: We collected 606 esophageal cancer patients from four institutions. 252 institution-1 patients had a treatment planning-CT (pCT) and a pair of diagnostic FDG-PETCT; 354 patients from other 3 institutions had only pCT. A two-streamed DL model for GTV segmentation was developed using pCT and PETCT scans of a 148 patient institution-1 subset. This built model had the flexibility of segmenting GTVs via only pCT or pCT+PETCT combined. For independent evaluation, the rest 104 institution-1 patients behaved as unseen internal testing, and 354 institutions 2-4 patients were used for external testing. We evaluated manual revision degrees by human experts to assess the contour-editing effort. The performance of the deep model was compared against 4 radiation oncologists in a multiuser study with 20 random external patients. Contouring accuracy and time were recorded for the pre-and post-DL assisted delineation process. Results: Our model achieved high segmentation accuracy in internal testing (mean Dice score: 0.81 using pCT and 0.83 using pCT+PET) and generalized well to external evaluation (mean DSC: 0.80). Expert assessment showed that the predicted contours of 88% patients need only minor or no revision. In multi-user evaluation, with the assistance of a deep model, inter-observer variation and required contouring time were reduced by 37.6% and 48.0%, respectively. Conclusions: Deep learning predicted GTV contours were in close agreement with the ground truth and could be adopted clinically with mostly minor or no changes.

Abstract:Purpose To develop and externally validate an automatic Artificial Intelligence (AI) tool for delineating gross tumor volume (GTV) in esophageal squamous cell carcinoma (ESCC) patients, which can assist in the neo-adjuvant or radical radiation therapy treatment planning. Methods and Materials In this multi-institutional study, contrast-enhanced CT images from 580 eligible ESCC patients were retrospectively collected. The GTV contours delineated by two experts via consensus were used as ground truth. A three-dimensional deep learning model was developed for GTV contouring in the training cohort and internally and externally validated in three validation cohorts. The AI tool was compared against twelve board-certified experts in 25 patients randomly selected from the external validation cohort to evaluate its assistance in improving contouring performance and reducing variation. Contouring performance was measured using dice similarity coefficient (DSC) and average surface distance (ASD). Additionally, our previously established radiomics model for predicting pathological complete response (pCR) was utilized to compare AI-generated and ground truth contours, in order to assess the potential of the AI contouring tool in radiomics analysis. Results The AI tool demonstrated good GTV contouring performance in multi-center validation cohorts, with median DSC values of 0.865, 0.876, and 0.866, and median ASD values of 0.939 mm, 0.789 mm, and 0.875 mm, respectively. Furthermore, the AI tool significantly improved contouring performance for half of twelve board-certified experts (DSC values, 0.794-0.835 vs 0.856-0.881, P = 0.003-0.048), reduced the intra- and inter-observer variations by 37.4% and 55.2%, respectively, and saved contouring time by 77.6%. In the radiomics analysis, 88.7% of radiomic features from ground truth and AI-generated contours demonstrated stable reproducibility, and similar pCR prediction performance for these contours ( P = 0.430) were observed. Conclusions Our AI contouring tool can improve GTV contouring performance and facilitate radiomics analysis in ESCC patients, which indicated its potential on GTV contouring during radiation therapy treatment planning and radiomics studies.

Novel dosimetric validation of a commercial CT scanner based deep learning automated contour solution for prostate radiotherapy

Abstract:Purpose: OAR delineation accuracy influences: (i) a patient's optimised dose distribution (PD), (ii) the reported doses (RD) presented at approval, which represent plan quality. This study utilised a novel dosimetric validation methodology, comprehensively evaluating a new CT-scanner-based AI contouring solution in terms of PD and RD within an automated planning workflow. Methods: 20 prostate patients were selected to evaluate AI contouring for rectum, bladder, and proximal femurs. Five planning 'pipelines' were considered; three using AI contours with differing levels of manual editing (nominally none (AIStd), minor editing in specific regions (AIMinEd), and fully corrected (AIFullEd)). Remaining pipelines were manual delineations from two observers (MDOb1, MDOb2). Automated radiotherapy plans were generated for each pipeline. Geometric and dosimetric agreement of contour sets AIStd, AIMinEd, AIFullEd and MDOb2 were evaluated against the reference set MDOb1. Non-inferiority of AI pipelines was assessed, hypothesising that compared to MDOb1, absolute deviations in metrics for AI contouring were no greater than that from MDOb2. Results: Compared to MDOb1, organ delineation time was reduced by 24.9 min (96 %), 21.4 min (79 %) and 12.2 min (45 %) for AIStd, AIMinEd and AIFullEd respectively. All pipelines exhibited generally good dosimetric agreement with MDOb1. For RD, median deviations were within ± 1.8 cm3, ± 1.7 % and ± 0.6 Gy for absolute volume, relative volume and mean dose metrics respectively. For PD, respective values were within ± 0.4 cm3, ± 0.5 % and ± 0.2 Gy. Statistically (p < 0.05), AIMinEd and AIFullEd were dosimetrically non-inferior to MDOb2. Conclusions: This novel dosimetric validation demonstrated that following targeted minor editing (AIMinEd), AI contours were dosimetrically non-inferior to manual delineations, reducing delineation time by 79 %.

Abstract:Segmentation of lung tumors is an important step in the planning of clinical treatment and operative procedures. Automatic segmentation of advanced lung tumors is challenging due to local invasion and heterogeneous metastasis. Fully automatic deep learning models often fail to accurately delineate advanced disease boundaries and do not allow the user to provide hints to improve segmentation performance. In contrast, interactive segmentation accepts user inputs and is an efficient way to segment lesions while reducing radiologist effort. This paper describes an interactive segmentation approach for lung lesion segmentation in CT scans. Our pipeline is built atop the SwinUNETR neural network architecture and can be expanded to support alternate models. The backbone model accepts radiologists' hints in the form of mouse clicks that indicate whether a location on the scan corresponds to a lesion. Our approach has the following novel contributions: (1) Context-aware encoding of guidance clicks by combining geodesic and Gaussian smoothing, resulting in improved segmentation of lesion boundaries. (2) An iterative prompting strategy to achieve higher accuracy with fewer clicks at inference time. (3) Guidance-aware Conditional Random Fields to refine the produced segmentation masks. (4) Volume cropping around guidance clicks at inference to improve segmentation precision and inference speed. (5) Linearly scaling our approach on multiple GPUs, to improve inference speed. On a public non-small cell lung cancer dataset predominantly containing advanced stage cancers (68\% stage III, and 27\% stage IV), our interactive approach delivers a Dice score of 0.74 with a single click, a 21\% improvement from the fully automatic approach (Dice Score = 0.61), and a Dice score of 0.81 with 5 clicks, which is a 33\% improvement. Inference speed is an important metric for an interactive pipeline. On a typical 350 350 150 voxel chest CT scan volume, our interactive approach infers 5 times faster than the original fully automated approach on a single GPU. After scaling to 4 GPUs, we infer in only 0.5 seconds; 20 times faster than the original approach. Qualitative evaluation from three independent radiologists indicates that our interactive pipeline significantly improved their standard clinical segmentation experience. Abstract:Purpose This study aimed to investigate the dosimetric impact of deep‐learning‐based auto‐contouring for clinical target volume (CTV) and organs at risk (OARs) delineation in prostate cancer radiotherapy planning. Additionally, we compared the geometric accuracy of auto‐contouring system to the variability observed between human experts. Methods We evaluated 28 planning CT volumes, each with three contour sets: reference original contours (OC), auto‐segmented contours (AC), and expert‐defined manual contours (EC). We generated 3D‐CRT and intensity‐modulated radiation therapy (IMRT) plans for each contour set and compared their dosimetric characteristics using dose‐volume histograms (DVHs), homogeneity index (HI), conformity index (CI), and gamma pass rate (3%/3 mm). Results The geometric differences between automated contours and both their original manual reference contours and a second set of manually generated contours are smaller than the differences between two manually contoured sets for bladder, right femoral head (RFH), and left femoral head (LFH) structures. Furthermore, dose distribution accuracy using planning target volumes (PTVs) derived from automatically contoured CTVs and auto‐contoured OARs demonstrated consistency with plans based on reference contours across all evaluated cases for both 3D‐CRT and IMRT plans. For example, in IMRT plans, the average D95 for PTVs was 77.71 ± 0.53 Gy for EC plans, 77.58 ± 0.69 Gy for OC plans, and 77.62 ± 0.38 Gy for AC plans. Automated contouring significantly reduced contouring time, averaging 0.53 ± 0.08 min compared to 24.9 ± 4.5 min for manual delineation. Conclusion Our automated contouring system can reduce inter‐expert variability and achieve dosimetric accuracy comparable to gold standard reference contours, highlighting its potential for streamlining clinical workflows. The quantitative analysis revealed no consistent trend of increasing or decreasing PTVs derived from automatically contoured CTVs and OAR doses due to automated contours, indicating minimal impact on treatment outcomes. These findings support the clinical feasibility of utilizing our deep‐learning‐based auto‐contouring model for prostate cancer radiotherapy planning.

Abstract:Purpose/objective(s): Auto-segmentation with artificial intelligence (AI) offers an opportunity to reduce inter- and intra-observer variability in contouring, to improve the quality of contours, as well as to reduce the time taken to conduct this manual task. In this work we benchmark the AI auto-segmentation contours produced by five commercial vendors against a common dataset. Methods and materials: The organ at risk (OAR) contours generated by five commercial AI auto-segmentation solutions (Mirada (Mir), MVision (MV), Radformation (Rad), RayStation (Ray) and TheraPanacea (Ther)) were compared to manually-drawn expert contours from 20 breast, 20 head and neck, 20 lung and 20 prostate patients. Comparisons were made using geometric similarity metrics including volumetric and surface Dice similarity coefficient (vDSC and sDSC), Hausdorff distance (HD) and Added Path Length (APL). To assess the time saved, the time taken to manually draw the expert contours, as well as the time to correct the AI contours, were recorded. Results: There are differences in the number of CT contours offered by each AI auto-segmentation solution at the time of the study (Mir 99; MV 143; Rad 83; Ray 67; Ther 86), with all offering contours of some lymph node levels as well as OARs. Averaged across all structures, the median vDSCs were good for all systems and compared favorably with existing literature: Mir 0.82; MV 0.88; Rad 0.86; Ray 0.87; Ther 0.88. All systems offer substantial time savings, ranging between: breast 14-20 mins; head and neck 74-93 mins; lung 20-26 mins; prostate 35-42 mins. The time saved, averaged across all structures, was similar for all systems: Mir 39.8 mins; MV 43.6 mins; Rad 36.6 min; Ray 43.2 mins; Ther 45.2 mins. Conclusions: All five commercial AI auto-segmentation solutions evaluated in this work offer high quality contours in significantly reduced time compared to manual contouring, and could be used to render the radiotherapy workflow more efficient and standardized.

Abstract:Background and purpose: Deep Learning (DL) technique has shown great potential but still has limited success in online contouring for MR-guided adaptive radiotherapy (MRgART). This study proposed a patient-specific DL auto-segmentation (DLAS) strategy using the patient's previous images and contours to update the model and improve segmentation accuracy and efficiency for MRgART. Methods and materials: A prototype model was trained for each patient using the first set of MRI and corresponding contours as inputs. The patient-specific model was updated after each fraction with all the available fractional MRIs/contours, and then used to predict the segmentation for the next fraction. During model training, a variant was fitted under consistency constraints, limiting the differences in the volume, length and centroid between the predictions for the latest MRI within a reasonable range. The model performance was evaluated for both organ-at-risks and tumors auto-segmentation for a total of 6 abdominal/pelvic cases (each with at least 8 sets of MRIs/contours) underwent MRgART through Dice Similarity Coefficient (DSC) and 95% Hausdorff Distance (HD95), and was compared with deformable image registration (DIR) and frozen DL model (no updating after pre-training). The contouring time was also recorded and analyzed. Results: The proposed model achieved superior performance with higher mean DSC (0.90, 95 % CI: 0.88-0.95), as compared to DIR (0.63, 95 %CI: 0.59-0.68) and frozen DL models (0.74, 95 % CI: 0.71-0.79). As for tumors, the proposed method yielded a median DSC of 0.95, 95 % CI: 0.94-0.97, and a median HD95 of 1.63 mm, 95 % CI: 1.22 mm-2.06 mm. The contouring time was reduced significantly (p < 0.05) using the proposed method (73.4 ± 6.5 secs) compared to the manual process (12 ∼ 22 mins). The online ART time was reduced to 1650 ± 274 seconds with the proposed method, as compared to 3251.8 ± 447 seconds using the original workflow. Conclusion: The proposed patient-specific DLAS method can significantly improve the segmentation accuracy and efficiency for longitudinal MRIs, thereby facilitating the routine practice of MRgART.
