Abstract:Recently, there has been an increase in the incidence of malignant tumors among the older population. Moreover, there is an association between aging and cancer. During the process of senescence, the human body suffers from a series of imbalances, which have been shown to further accelerate aging, trigger tumorigenesis, and facilitate cancer progression. Therefore, exploring the junctions of aging and cancer and searching for novel methods to restore the junctions is of great importance to intervene against aging-related cancers. In this review, we have identified the underlying pathogenetic mechanisms of aging-related cancers by comparing alterations in the human body caused by aging and the factors that trigger cancers. We found that the common mechanisms of aging and cancer include cellular senescence, alterations in proteostasis, microbiota disorders (decreased probiotics and increased pernicious bacteria), persistent chronic inflammation, extensive immunosenescence, inordinate energy metabolism, altered material metabolism, endocrine disorders, altered genetic expression, and epigenetic modification. Furthermore, we have proposed that aging and cancer have common means of intervention, including novel uses of common medicine (metformin, resveratrol, and rapamycin), dietary restriction, and artificial microbiota intervention or selectively replenishing scarce metabolites. In addition, we have summarized the research progress of each intervention and revealed their bidirectional effects on cancer progression to compare their reliability and feasibility. Therefore, the study findings provide vital information for advanced research studies on age-related cancers. However, there is a need for further optimization of the described methods and more suitable methods for complicated clinical practices. In conclusion, targeting aging may have potential therapeutic effects on aging-related cancers.

Abstract:Background and Objective: Cancer can develop at any age, but the incidence of cancer rises dramatically at a later stage in life. Cancer management in older people can be complex. Comorbidity and Physiologic age-related changes can affect how and when cancer is diagnosed, whether or not treatment is offered and the individual tolerance to anticancer therapy. This retrospective study aimed to assess the clinical profiles of elderly cancer patient to optimize cancer care. Materials and Methods: This retrospective study comprised cancer patients attended our oncology department between 2018 to 2022 who were above the age ≥65 years. All the information regarding the type of cancer, performance status, intent of treatment, treatment completion and follow up status at 3 years were documented and analyzed. Results: A total of 230 patients were included in the study. The common malignancies were Carcinoma Head and Neck and Lung Cancer followed by Esophagus and Colorectal malignancies. Most of the patients presented with Performance Status of 1 (57.8%). The intent of therapy was curative in 49.1% and Palliative in 30.1% patients. At median follow up 36 months 15% of treated patients were on regular follow-up and they were alive without disease. Conclusion: Head and Neck Cancer was the most common cancer seen in our study. Elderly patients with cancer presented with good performance status, hence Elderly Cancer patients deserve same opportunity as younger patients for treatment and survival options.
Epidemiology of Cancer in Older Adults: A Systematic Review

Abstract:Purpose of review: What are the prevalence, incidence and mortality rates of cancer among individuals aged 60 or older on a national, regional, and global scale? What factors affect differences in cancer survival between older and younger adults? Recent findings: The epidemiological literature on cancer in older adults, particularly in low- and middle-income countries (LMICs) and that focusing on the oldest adults, is expanding. These studies consistently show increasing global cancer incidence rates in older populations. Recent research also highlights a widening survival gap between middle-aged and older adults, with the stage at diagnosis being the primary driver. More research is needed to describe the cancer burden in older adults, especially focusing on the oldest population and LMICs, to better understand global healthcare challenges. Additionally, further exploring patient-related, clinical, and tumour-related factors which drive age-related survival differences could improve cancer outcomes in older adults.

Abstract:The global demographic and epidemiological transition have led to a rapidly increasing burden of cancer, particularly among older adults. There are scant data on the prevalence and demographic pattern of cancer in older Indian persons. This was a multicentric observational study conducted between January 2019 and December 2020. Data were retrieved from existing electronic databases to gather information on two key variables: the total number of patients registered with oncologists and the number of patients aged 60 years and above. The primary objective was to determine the percentage of older adults among patients with cancer served by these hospitals. Secondary objectives included understanding the prevalence of different types of cancer in the older population, and the sex- and geographic distribution of cancer in older Indian patients. We included 272,488 patients with cancer from 17 institutes across India. Among them, 97,962 individuals (36 %) were aged 60 years and above. The proportion of older adults varied between 20.6 % and 53.6 % across the participating institutes. The median age of the older patients with cancer was 67 (interquartile range, 63-72) years. Of the 54,281 patients for whom the details regarding sex were available, 32,243 (59.4 %) were male. Of the 56,903 older patients, head and neck malignancies were the most prevalent, accounting for 11,158 cases (19.6 %), followed by breast cancer (6260 cases, 11 %), genitourinary cancers (6242 cases, 10.9 %), lung cancers (6082 cases, 10.7 %), hepatopancreaticobiliary (6074, 10.7 %), and hematological malignancies (5226 cases, 9.2 %). Over one-third of Indian patients with cancer are aged 60 years and above, with a male predominance. Head and neck, breast, and genitourinary cancers are the most prevalent in this age group. Characterizing the burden of cancer in older adults is crucial to enable tailored interventions and additional research to improve the care and support for this vulnerable population.
[Regional eco-security assessment of southwest Beijing].

Abstract:Based on the zoning regulation which integrated climate, vegetation and soil as a complex, a regional eco-security index system consisting of regional physical background, ecosystem stability, landscape structure, and external interference was established, and, with the support of geographical information system technology and fuzzy mathematics, a set of regional eco-security assessment methods was constructed. By means of these methods, the eco-security index of southwest Beijing in 2004 was calculated based on the recent 30 years background data of climate and soil. The results showed that in southwest Beijing, the eco-security degree was higher in its central area and western hilly-shallow hilly areas, but lower in its southern and southeastern plain areas. The mining activity in southwest Beijing had threatened the environment, and should be stopped immediately to maintain the regional eco-security. It was proved in the case study that the assessment index system and related methods established in this paper could solve the existing problems in assessing regional eco-security, and the assessment results could reflect the regional ecological issues and the status of eco-security at different locations within the region.
Trends in cancer mortality among the elderly in China, 2005-2035

Abstract:Background: With the appearance and aggravation of the aging society, cancer has become one of the major problems that threaten the life and health of Chinese residents seriously. Objective: To explore the cancer epidemiological characteristics among the elderly in China from 2005 to 2016, and to provide strategies for cancer prevention and treatment. Methods: Stratified analysis was conducted on the cancer mortality data of the elderly aged ≥60 years in China, which were selected from the Chinese Cancer Registry Annual Report. Joinpoint regression model was used to calculted average annual percentage change (AAPC) to estimate the time trends. Age-period-cohort (APC) model was used to explore the age, period and birth cohort effect on the risk of cancer death. Bayesian age-period-cohort (BAPC) model was used to predict trends in cancer mortality among elderly by gender and region to 2035. Results: 2005-2016, cancer mortality in the elderly in China showed a decreasing trend (AAPC = -1.2%, P<0.001). Cancer mortality in rural areas was higher than that in urban areas, but the urban-rural difference gradually narrowed (t = 6.1, P<0.01). The APC model showed that cancer mortality increased with age. The relative risk (RR) for the period effect decreased. RR was lower for the later- born cohort than that for the earlier-born cohort in rural areas. Lung cancer mortality ranked first in both male and female, and showed an increasing trend among female in the 60-64, 80-84 and ≥85 age groups (AAPC60-64 = 1.0%, AAPC80-84 = 0.8%, AAPC≥85 = 2.0%, all P<0.05). By 2035, cancer mortality for the elderly was predicted to decline nationally, by sex and in rural areas, while rising in urban areas. Conclusion: Cancer mortality in the elderly in China showed a decreasing trend from 2005 to 2016, but it was still higher than the world average. Early cancer screening is important, especially in the elderly male and in rural areas.

Abstract:OBJECTIVE: We examine international incidence trends of lung, colorectal, prostate, and breast cancers, as well as all cancers combined excluding non-melanoma skin cancer (NMSC) in adults aged 50 and older, over a fifteen-year period using data from 113 high quality population-based cancer registries included in the Cancer in Five Continents (CI5) series and NORDCAN.MATERIALS AND METHODS: We calculated annual incidence rates between 1998 and 2012 for ages 50-64, 65-74, and 75+, by sex and both sexes combined. We estimated average annual percentage change (AAPC) in rates using quasi-Poisson regression models.RESULTS: From 1998 to 2012, incidence trends for all cancers (excluding NMSC) have increased in most countries across all age groups, with the greatest increase observed in adults aged 75+ in Ecuador (AAPC = +3%). Colorectal cancer incidence rates increased in the majority of countries, across all age groups. Lung cancer rates among females have increased but decreased for males. Prostate cancer rates have sharply increased in men aged 50-64 with AAPC between 5% and 15% in 24 countries, while decreasing in the 75+ age group in 21 countries, by up to -7% in Bahrain. Female breast cancer rates have increased across all age groups in most countries, especially in the 65-74 age group and in Asia with AAPC increasing to 7% in the Republic of Korea.CONCLUSIONS: These findings assist with anticipating changing patterns and needs internationally. Due to the specific needs of older patients, it is urgent that cancer systems adapt to address their growing number.

Abstract:Global growth of the elderly population is requiring health care providers to cater to an expanding elderly cancer subpopulation. Surgeons are called upon to provide optimal cancer management for this heterogeneous group in par with care provided for younger patients, an ethical dilemma that is compounded by an ill-understood tumor biology. Delivery of suboptimal surgical cancer treatment due to ageist attitudes and personal beliefs is no longer acceptable. Cancer surgeons have to offer optimal surgery in consideration of individualized operative risk. The need for a tool to assess operative risk and predict postoperative outcome is obvious. This article wishes to raise awareness among surgeons about epidemiology, physiologic changes, performance status and comorbidities, and patterns of care and ways to improve them. We visit existing tools to assess operative risk, reveal interim results from the Preoperative Assessment of Cancer in Elderly (PACE) study, and discuss the future of oncogeriatrics.

Abstract:Abstract Background In ageing societies such as the United States, evaluating the incidence and survival rates of cancer in older adults is essential. This study aimed to analyse the incidence and survival rates of cancer in individuals aged 55 years or older in the United States. Methods This retrospective study (1975–2019) was conducted using combined registry data from the Surveillance, Epidemiology, and End Results database. Data from the 9, 12, and 17 Registries (Nov 2021 Sub) datasets were used. Results In 2019, the incidence of cancer in individuals older than 55 years and the overall population was 1322.8 and 382.1 per 100,000 population, respectively. From 2000 to 2019, the incidence of cancer in individuals older than 55 years showed a decreasing trend, whereas their five-year survival rates showed an increasing trend. The incidence of cancer in the 75–79 and 80–84 year age groups was the highest among all age groups. Conclusions The incidence of colon cancer declined significantly, whereas that of intrahepatic bile duct cancer increased considerably. These trends may be due to increased screening for cancers with high incidence rates and improved control of the risk factors for cancer. Rapid development of targeted therapy and immunotherapy combined with early tumour detection may be an important reason for the improved survival rates.

Abstract:Purpose: First, to present the epidemiological data of aging and of cancers and to describe the respectives expected evolutions. Second, to present biological and genetic data on aging and on the relationships between aging and oncogenesis. Method: Bibliographic search from the Medline bibliographic database (NLM Pubmed tool) and Embase, as well as from the web sites of geriatric scientific societies, the United Nations, the World Bank, the World Health Organization, the Institut National du Cancer and the Ligue Contre le Cancer from the following keywords: aging, elderly, cancer, epidemiology, biology, genetics. Results: The entire world population is aging very significantly and very rapidly. In France, new cases of cancer are diagnosed in 62.4% of cases in patients over 65 and in 11.5% of cases in patients over 80 years. Cancer mortality occurs in 75.3% of cases in patients over 65 years of age and in 24.8% of cases in patients over 80 years of age. Cancer-specific mortality is consistently higher in patients older than 75 years compared to younger patients; this reflects, among other things, an age discrimination which is called agism. It has been established that cellular aging is marked by 9 major families of biological and genomic abnormalities. Biological aging and oncogenesis are intertwined with increasingly well established relationships. They are both the product of natural selection and they are found in all species with both renewal tissues and a distinction between germinal tissue and somatic tissue. Conclusion: Epidemiological data predict that oncology, including urological oncology, is becoming very predominantly geriatric oncology; it is critical and urgent that society be prepared for it and that every care-giver be prepared, that is, be specifically trained. Biological and genetic data argue for a great entanglement between aging and oncogenesis; research in each of these areas should be reconciled for mutual benefit.

Abstract:Abstract Purpose: To quantitatively evaluate contributions of trends in incidence, relative survival, and stage at diagnosis to the dynamics in the prevalence of major cancers (lung, prostate, colon, breast, urinary bladder, ovaries, stomach, pancreas, esophagus, kidney, liver, and skin melanoma) among older U.S. adults age 65+. Methods: Trend partitioning was applied to the Surveillance, Epidemiology, and End Results Program data for 1973-2016. Results: Growth of cancer prevalence in older adults decelerated or even decreased over time for all studied cancers due to decreasing incidence and improving survival for most of cancers, with a smaller contribution of the stage at cancer diagnosis. Changes in prevalence of cancers of lung, colon, stomach, and breast were predominantly due to decreasing incidence, increasing survival and more frequent diagnoses at earlier stages. Changes in prevalence of some other cancers demonstrated adverse trends such as decreasing survival in localized and regional stages (urinary bladder and ovarian) and growing impact of late-stage diagnoses (esophageal cancer). Conclusion: While decelerating or decreasing prevalence of many cancers were due to a beneficial combination of decreasing incidence and increasing survival, there are cancers for which decelerating prevalence is due to the lack of improvement in their stage-specific survival and/or increasing frequency of diagnosis at advanced stages. Overall, if the observed trends persist, it is likely that cancer burden due to its prevalence in older U.S. population will be lower than it was projected in earlier studies.

Abstract:Endometrial cancer incidence and related mortality are on the rise due to aging demographics. This population often presents with unfavorable features, such as myometrial invasion, non-endometrioid histology, high-grade tumors, worse prognosis, etc. The role of age as an independent prognostic factor is still debated, and screening tools addressing frailty emerge as pivotal in guiding treatment decisions; however, they are still underutilized. Treatment disparities are evident in the case of older patients with endometrial cancer, who frequently receive suboptimal care, hindering their survival. Radiotherapy and minimally invasive surgical approaches could be performed in older patients. Data on chemotherapy and immunotherapy are scarce, but their potential remains promising and data are being gathered by recent trials, contingent on optimal patient selection through geriatric assessments. Overall, we recommend personalized, screening tool-guided approaches, adherence to guideline-recommended treatments, and inclusion of older people in clinical trials to help identify the best course of treatment.
Epidemiology of HPB malignancy in the elderly

Abstract:The incidence of hepatopancreatobiliary (HPB) cancers is increasing worldwide. Despite improvements in cancer surveillance and earlier access to therapy, these cancers still have poor survival rates, especially in elderly patients. There are several challenges in elucidating the epidemiology of these malignancies especially in view of the heterogeneous exposure to recognised risk factors and the coding systems used. Early diagnosis, crucial to improved survival, remains challenging as many patients present sporadically. Furthermore, the indolent nature of these tumours means they tend to present late, when curative treatments are no longer an option. Importantly, even when patients are diagnosed early and treated with curative intent, recurrence rates remain high. HPB cancers display a heterogenous molecular profile resulting in poorly effective systemic therapies in patients for whom curative treatments cannot be considered. Globally, people now live longer, and it is predicted that the population of individuals older than 85 will double by 2033. With increasing age comes increased cancer risk. There is a plethora of recognised challenges in elderly patients presenting with cancer and this is no different with HPB cancers. Complex co-morbidities including chronic disease, polypharmacy, cognitive decline and increasing psychosocial needs confer an extra layer of complexity in the management of this elderly sub population. The frailty that usually accompanies advancing age often means that patients take longer to recover and develop more complications after cancer therapies. In this article, we review the epidemiology of malignant HPB tumours with a focus on the elderly.

Abstract:Context: Carcinoma of the breast is the most common malignancy in females. At present carcinoma breast is the leading cancer in urban India and has overtaken cancer cervix in incidence. Epidemiological studies at regional and global levels suggest the occurrence of carcinoma breast at a younger, premenopausal age in Indian and Asian women as compared with western women. Knowledge of this factor emphasizes the need to modify the timing of modalities of detection of early carcinoma and its management. According to literature, majority of carcinoma breast cases in the western countries present in Stages I and II of the disease whereas in India majority cases present in Stage III of the disease. The objective of this study is to observe age of occurrence of breast cancer and stage of cancer in this health facility. Other epidemiological factors such as diabetes and obesity were also recorded. Materials and Methods: A record based descriptive study was conducted in 100 patients of histopathologically confirmed breast cancer. Results: Mean age of subjects was 50.1 ± 10.5 years. Age distribution showed two peaks at 41-50 years and 51-60 years with 42 and 24 patients, respectively. Subjects who also had co-existing diabetes, hypertension, and obesity were 14, 14 and 6%, respectively. This study shows that 45% of the total patients were having advanced carcinoma breast (Stage III) at the time of reporting and 77% of these patients were below 50 years of age. Conclusion: Breast cancer is now occurring increasingly in younger age groups in India when compared with western countries and a more aggressive nature of the disease strikes Indian women in their reproductive period. This stresses the need for change in modalities of early cancer detection, modifying, and adjusting control efforts and multidisciplinary therapeutic efforts.

Abstract:12040 Background: Clinical trials dedicated to the elderly cancer patients are essential to help to define optimal cancer therapy for this rapidly growing population. Our study aimed to analyze the characteristics and the evolution of elderly-specific oncology trials registered in ClinicalTrials.gov. Methods: A dataset of 61120 oncology trials registered in ClinicalTrials.gov between Jan 1th, 2000 and Dec 17th, 2019 was downloaded. Interventional trials were identified and systematically reviewed to validate classification into elderly-specific (at least using a chronological landmark to define the elderly) and age-unspecified trials. Cancer type and other registration information were extracted. Characteristics of elderly-specific trials were compared with characteristics of age-unspecified trials. Chronological shifts in elderly-specific trials between 2000 to 2009, and 2010 to 2019 were analyzed. Results: Of the 49273 trials eligible for analysis, only 791 (1.6%) were elderly-specific. The most frequently used threshold to define the old was 60 years (36%), followed by 70 years (28%) and 65 years (26%). More than half of the elderly-specific trials were phase 2 (56.8%) trials and enrolled 100 or fewer participants (59.4%). Compared with age-unspecified trials, elderly-specific trials were less likely to be funded by industry (28.3% vs 37.1%; p < 0.001), and more likely to be conducted in Europe (42.1% vs 24.5%, p < 0.001). During the two time periods between 2000 to 2009, and 2010 to 2019, the proportion of elderly-specific trials of all the oncology trials remained stable (1.57% vs 1.62%). The proportion of treatment-oriented trials decreased from 91.9% to 71.4% (p < 0.001) while supportive care-oriented trials increased from 1.9% to 13.2% (p < 0.001). Industry-funded elderly-specific trials decreased from 34.0% to 25.6% (p = 0.014). Concerningly, the use of clinically meaningful end points in elderly such as disease-specific survival, patient-reported outcomes (PROs) and functional status as a primary end point were uncommon (0.5%, 8.0%, and 6.9%, respectively). However, the use of PROs as a primary end point tended to increase in the second time period, from 2.4% to 10.5% (p < 0.001). There was no correlation between the number of trials for a given cancer type and relative incidence and mortality. 336/791 (42.5%) of the trials were conducted for patients with blood cancer, although the incidence and mortality of blood cancer were relatively low. The most common solid cancer types were breast cancer (13.9%), lung cancer (10.0%), and colorectal cancer (7.8%). Conclusions: Although the majority of all new cases of cancer occur in the elderly population, elderly-specific trials account for only a minority of all oncology trials. Our study helps us to better understand the current state of elderly-specific trials and provides insights for future development that can improve the care of elder patients with cancer.

Worldwide trends in breast cancer incidence from 1993 to 2012: Age-period-cohort analysis and joinpoint regression

Abstract:BACKGROUND: Breast cancer is the most common cancer accounting for about one-fourth of total cancer cases and 15% of all cancer deaths among women worldwide. It is important to determine its trend across the regions in the world to find the high-focus regions. Hence, the current study was done to assess the global trends and deviations in the incidence of breast cancer.MATERIALS AND METHODS: A descriptive trend analysis was done using the data on breast cancer incidence from the WHO Cancer Incidence Data of Five Continents plus database. Joinpoint regression was performed to determine the average annual percent change (AAPC), and age-period-cohort analysis was done to obtain age-, period-, and cohort-specific deviations and rate ratio.RESULTS: All the regions showed an increasing trend in breast cancer incidence, with an exception of America. Maximum increase was observed in Asia (AAPC = 2.6%; 95% confidence interval [CI]: 2.4%-2.9%) followed by Europe (AAPC = 0.7%; 95% CI: 0.5%-1%). There was consistent rise in the breast cancer incidence across the age groups in all the four continents with maximum burden in elderly (P < 0.001). Except in America, all other regions showed consistent rise in the incidence of breast cancer through the periods 1998-2002 to 2007-2012 (P < 0.001). There was consistent increase across the cohorts from 1923-1927 to 1978-1982 in continents such as Asia and Oceania (P < 0.001).CONCLUSION: To summarize, the incidence of breast cancer shows an increasing trend globally with a maximum increase in the Asian region. This makes a strong need for newer strategies irrespective of current prevention and control interventions.

Descriptive epidemiology of the head and neck cancers in old patients

Abstract:Background: In Europe, as in other high-income (HI) countries, quite half of the newly diagnosed patients with head and neck (H and N) cancers are older than 65 years of age and their proportion within the prevalent cases is even higher. Moreover, the incidence rate (IR) for all H and N cancers sites increased with age and the survival rate is lower in older patients (≥65), compared with younger patients (<65). The number of older patients affected by H and N cancers will increase because of the increase in life expectancy. The aim of the article is to provide an epidemiological description of H and N cancers in the elderly population. Material and methods: Incidence and prevalence data by time periods and continents were extracted from the Global Cancer Observatory. The survival information for Europe is obtained from the EUROCARE and RARECAREnet projects. In 2020, according to the results from these data, slightly more than 900,000 cases have been diagnosed with H and N cancers in the world, and approximately 40% were older than 65 years of age. This percentage was higher, reaching approximately 50% in the HI countries. The highest number of cases was in the Asiatic populations, while the highest crude IR was in Europe and Oceania. Among H and N cancers occurring in the elderly, laryngeal and oral cavity cancers were the most common, while nasal cavities and nasopharyngeal cancers were the rarest. This was true for all the countries, excluding some Asiatic populations, in which tumour of the nasopharynx was more common. The five-year survival rate in the European population was low in the elderly, compared with the younger for all H and N cancers, and it ranged from approximately 60% for both salivary-gland type and laryngeal to 22% for hypopharyngeal tumors. For the elderly, the conditional 5-year survival after surviving one year became more than 60% for many H and N epithelial tumors. Conclusion: The high variability in the H and N cancer incidence around the world is due to the distribution of the major risk factors which for the elderly are mainly alcohol and smoking. The reasons for low survival in the elderly are most likely due to the complexity of treatment, the late arrival of patients at diagnosis, and the difficult access to specialized centers.
