Abstract:To assess the biochemical, mechanical and structural characteristics of retained dentin after applying three novel bromelain-contained chemomechanical caries removal (CMCR) formulations in comparison to the conventional excavation methods (hand and rotary) and a commercial papain-contained gel (Brix 3000). Seventy-two extracted permanent molars with natural occlusal carious lesions (score > 4 following the International Caries Detection and Assessment System (ICDAS-II)) were randomly allocated into six groups (n = 12) according to the excavation methods: hand excavation, rotary excavation, Brix 3000, bromelain-contained gel (F1), bromelain-chloramine-T (F2), and bromelain-chlorhexidine gel (F3). The superficial and deeper layers of residual dentin were examined by Raman microspectroscopy and Vickers microhardness, while the surface morphology was assessed by the scanning electron microscope (SEM). A multivariate analysis of variance followed by Tukey's test (p > 0.05) was performed for data analysis. The novel formulations showed an ability to preserve the partially demineralized dentin that showed a reduced phosphate content with a higher organic matrix. This was associated with lower Vickers microhardness values in comparison to sound dentin and rotary excavation. The collagen integration ratio in all methods was close to sound dentin (0.9–1.0) at the deeper dentin layer. The bromelain-chloramine-T gel (F2) produced the smoothest smear-free dentin surface with a higher number of opened dentinal tubules. In contrast, dense smearing covering the remaining dentin was observed in the manual and rotary methods with obstructed dentin tubule orifices. The bromelain-contained formulations can be considered a new minimally invasive approach for selectively removing caries in deep cavitated dentin lesions.

Abstract:Objective:To compare the caries removal efficiency of Carisolv with that of drilling in deciduous teeth.Methods:Thirty-two children,aged from 6 to 9 years,with symmetric coronal medium or deep caries in deciduous teeth,were involved in the clinical study.According to a split-mouth design,in each patient,caries tissue was removed by using a round bur on one side,and Carisolv on the other side.After caries removal,the teeth were restored with composite resin.Patients were asked to fill in a postoperative questionnaire,including patient acceptance of technique preference.6 months after operation the restorations were checked.Results:The time(min) for Carisolv caries removal and for drilling method were 4.33±1.25 and 2.58±0.82 respectively(P<0.0001).The patients felt more painful(78%) in the operation by drilling than by Carisolv(9%).97% patients accepted the taste of Carisolv gel.The majority of patients preferred Carisolv to bur.In 6-month follow-up,there was no significant difference in the incidence of marginal microleakage, secondary caries and restoration lost between Carisolv and drilling treatment(P>0.05).Conclusion:Carisolv is a time-consuming technique,but the majority of patients accepte Carisolv because of less pain.Carisolv may have no effect on the incidence of marginal microleakage and secondary caries.

Abstract:To evaluate the efficiency and effectiveness of three minimally invasive (MI) techniques in removing deep dentin carious lesions. Forty extracted carious molars were treated by conventional rotary excavation (control), chemomechanical caries removal agent (Brix 3000), ultrasonic abrasion (WOODPECKER, GUILIN, China); and Er, Cr: YSGG laser ablation (BIOLASE San Clemente, CA, USA). The assessments include; the excavation time, DIAGNOdent pen, Raman spectroscopy, Vickers microhardness, and scanning electron microscope combined with energy dispersive X-ray spectroscopy (SEM–EDX). The rotary method recorded the shortest excavation time (p < 0.001), Brix 3000 gel was the slowest. DIAGNOdent pen values ranged between 14 and 18 in the remaining dentin and laser-ablated surfaces recorded the lowest reading (p < 0.001). The Ca:P ratios of the remaining dentin were close to sound dentin after all excavation methods; however, it was higher in the ultrasonic technique (p < 0.05). The bur-excavated dentin showed higher phosphate and lower matrix contents with higher tissue hardness that was comparable to sound dentin indicating the non-selectiveness of this technique in removing the potentially repairable dentin tissue. In contrast, the MI techniques exhibited lower phosphate and higher organic contents associated with lower microhardness in the deeper dentin layers. This was associated with smooth residual dentin without smearing and patent dentinal tubules. This study supports the efficiency of using MI methods in caries removal as conservative alternatives to rotary excavation, providing a promising strategy for the clinical dental practice.

Abstract:To the Editor.— The human T-cell leukemia-lymphoma virus (HTLV) has been noted to be extremely prevalent in several populations, including southeastern blacks. 1 Interestingly, the four cases of Blayney et al include three patients who worked in health-related professions: case 1 "worked as a hospital nursing and dietetic aide," case 2 "worked in the supply department of a hospital," and case 3 "worked as a medical assistant." As a physician who trained and worked in the southeast for more than 15 years, I would like to make the following observation: southern blacks work in hospitals in numbers far out of proportion to their population in any given town. Could there be a connection in this observation relating to cases 1, 2, and 3? Should populations of all races working in hospitals be screened for HTLV?

Abstract:AIM The aim of the present study was to compare and evaluate the efficacy of two minimally invasive methods in the removal of infected carious dentin in primary molars. MATERIALS AND METHODS Eighty primary molars with class I occlusal caries involving dentin were selected and allocated to either of the two groups using convenient sampling. Group A (n = 40) caries were excavated using BRIX 3000 and in Group B (n = 40) atraumatic restorative treatment (ART) caries were excavated using hand instruments only. In both groups, caries removal efficacy was evaluated clinically using caries detection dye (Sable and Sleek), and the patient's discomfort level was noted using the Wong-Baker faces pain rating scale (WBFPS). RESULTS The results indicated that BRIX 3000 was effective in the removal of caries from primary molars with a significant difference. While there was no statistically significant difference seen in pain perception on caries excavation using both the methods. Although, a mild discomfort was noted in Group B. CONCLUSION Comparing the two minimally invasive techniques, the new chemomechanical solution (BRIX 3000) was an effective method for the removal of caries from primary molars. So, it can be considered an alternative to the conventional caries removal method in treating pediatric dental patients. CLINICAL SIGNIFICANCE BRIX 3000 gel is effective in the removal of caries in children who are anxious and apprehensive as it is a noninvasive method of treatment in the pediatric age group.

Abstract:BACKGROUND: The use of chemomechanical agents for caries removal has been indicated as a non-invasive treatment option; however, their possible deleterious effects on the dental-pulp complex have been insufficiently studied. This study assessed the direct cytotoxicity of two chemomechanical caries removal agents (Brix 3000™ - BX and Papacarie Duo™ - PD) on pulp cells from deciduous teeth, as well as to assess the morphology and chemical compositions of the dentin surface after the application of these materials.MATERIAL AND METHODS: The cells were seeded (50,000 cells/cm²) in a culture medium (DMEM with 10% fetal bovine serum - FBS). After 24 hours, the BX and PD materials were added to 1:20, 1:100, and 1:1000 dilutions. Non-exposed cells were considered as the control group. The viability test (MTT), Trypan Blue assay (TB), and cell morphology (Scanning Electron Microscopy - SEM) were performed after 24 hours of agent application. For the SEM and chemical (energy-dispersive X-ray spectrometry - EDS) dentin evaluation, 0.3-mm-thick dentin discs were obtained and divided into control group (no treatment) and surfaces covered with 37% phosphoric acid, BX, or PD. Data were compared by one-way ANOVA and Tukey's test (p<0.05).RESULTS: Decreases in cell viability and numbers of viable cells were observed for both materials, at all dilutions, when compared with the control group (p<0.05). The BX and PD materials did not cause visually perceptible changes, according to SEM, on the surfaces of dentin discs. The EDS analysis did not indicate a statistically significant difference in the levels of calcium (Ca) and phosphorus (P) between the materials and the control group (p>0.05).CONCLUSIONS: Both materials showed cytotoxicity when in direct contact with the pulp cells from deciduous teeth, and the BX material presented lower cytotoxicity than the PD material. Abstract:Background Pulpotomy treatment is one of the vital pulp therapies that can play a major role in the preservation of primary teeth until their natural exfoliation. The objective of this current clinical trial was to assess the clinical and radiographical success of diode and Er,Cr:YSGG lasers as a viable alternative to formocresol (FC) and sodium hypochlorite in the primary molar pulpotomies. Materials and methods Sixty primary molars were selected and randomly allocated to four groups. All treatment groups followed the same clinical protocol, except for the techniques used for hemostasis of the pulpotomy sites. In group A, hemostasis was achieved by applying a 1:5 dilution of FC solution, whereas in group B, 3% sodium hypochlorite was applied to achieve hemostasis. In group C, exposure to a diode laser of 940 nm was performed, whereas for group D, erbium laser irradiation with Er,Cr:YSGG laser of 2,780 nm was employed to achieve hemostasis. Radicular pulp stamps were covered with a 2 mm layer of mineral trioxide aggregate (MTA) paste. Stainless steel crowns were utilized for the final restorations of the primary teeth. The clinical and radiographic outcomes were evaluated at the six- and 12-month follow-up intervals. The investigation was registered with the ClinicalTrials.gov Protocol and Registration System (ID: NCT06002646). Results The overall clinical and radiographic success rates of pulpotomy were 92.3% for FC, 89% for sodium hypochlorite, 98.3% for a diode laser, and 98.7% for Er,Cr:YSGG lasers. There were no statistically significant differences among the four groups (p > 0.05). Conclusions Both the diode and Er,Cr:YSGG lasers showed outcomes comparable to those of FC and sodium hypochlorite. Therefore, they could be promising alternatives to primary tooth pulpotomies.

Abstract:Thirty-one patients less than 40 years of age with carcinoma of the colon were treated at this hospital during the ten years between 1968 and 1978. This accounted for 3.6 per cent of the total number of patients with carcinoma of the colon and rectum seen during this period. Pain, rectal bleeding, change in intestinal habits and weight loss were the most common presenting symptoms. The average delay between the onset of symptoms and treatment was 6.4 months. Mucin-producing and poorly differentiated adenocarcinomas were present in 49 per cent of the patients in this study. Metastases to the ovaries occurred in 23 per cent of the female patients. The five year survival rate was only 22 per cent. The importance of early diagnosis and treatment and, in the female patient, of oophorectomy is emphasized.
Effectiveness of Three Agents in Pulpotomy Treatment of Permanent Molars with Incomplete Root Development: A Randomized Controlled Trial

Abstract:The aim of this study was to investigate and compare, radiographically and clinically, the impacts of calcium-silicate based-cement (CSBC), nano-hydroxyapatite and platelet-rich fibrin (PRF) as pulpotomy agents in permanent immature molars with incomplete root development. Sixty-three participants (63 permanent immature molars) were included in this study. The patients were randomly divided into three equal groups. Fast setting MTA (MM-MTA), nano-hydroxyapatite and platelet-rich fibrin were used as pulpotomy agents. The teeth were evaluated clinically and radiographically after 6 and 12 months by two blinded examiners. Apical closure and pulp canal obliteration percentages were recorded. The in vitro reaction of the tested materials after a 7-day immersion period of the different materials in phosphate-buffered solution was analyzed using scanning electron microscopy to associate the in vitro mineralization with in vivo pulp canal obliteration percentages. Data were analyzed using Chi-square and ANOVA tests (α = 0.05). No significant difference was found between the three tested groups in terms of clinical and radiographic success (p > 0.05). All cases demonstrated evidence of root growth, including complete apical closure or continued apical closure. At 12 months, complete apical closure was found among the MM-MTA group (50%), nano-hydroxyapatite group (55%) and platelet-rich fibrin group (60%) (p > 0.05). After 12 months, pulp canal obliteration was more observed in the MM-MTA and nano-hydroxyapatite groups than in the PRF group (p < 0.05). MM-MTA (auto-mixed), NHA (hand-mixed) and PRF (autologous) could be used as pulpotomy agents since they exhibit comparable high clinical and radiographic success rates. However, the fact that the groups managed with MM-MTA and NHA have a higher tendency to canal obliteration might indicate that PRF should be considered the first choice material as pulpotomy agent, as it would make retreatment considerably easier.

Abstract:Objective:To evaluate Er,Cr:YSGG laser system objectively through randomized control trial in clinic.Method:30 patient were selected randomly and the method of self-comparison was adopted.Each patient had at least two teeth of which caries degree were similar.Caries was removed and cavities were prepared using Er,Cr:YSGG laser(Biolase,USA) technique and traditional handpiece method respectively.Each patient were required to select which method he prefer through a questionnaire.The time of caries removal and cavity preparation was recorded.Patients were called back for checking restorations 6 months and 12 months after treatment.The pain degree of each patient was recorded with WongBaker Facial Pain Scale.Result:Average time of Er,Cr:YSGG laser and turbine method for were 504 s and 390s respectively.Er,Cr:YSGG laser can alleviate pain obviously compared with traditional method,the difference is significant(x2=21.99,P< 0.01).80% patients prefer Er,Cr:YSGG laser to traditional method.There is no significant difference between two group restorations 6 months and 12 months after treatment.Conclusion:Er,Cr:YSGG laser can alleviate treatment pain and discomfort obviously,so it is a new and effective technology for dental hard tissue surgery.

Abstract:Objectives: Evaluation of efficacy of bioactive glass (BAG), tricalcium phosphate, and ozone remineralizing agents on artificial carious lesion. Materials and Methods: Freshly extracted 48 human premolar teeth were included for this in vitro study. Remineralization of demineralized teeth was done with respective remineralizing agents (Group A – calcium sodium phosphosilicate [BAG], Group B – tricalcium phosphate [Clinpro Tooth Crème], Group C – ozone remineralizing agents, and control group: Group D – deionized water) three times a day for 12 days for 4 min. The degree of demineralization and remineralization was evaluated with Vickers hardness number. Results: There was a decrease in microhardness from baseline to demineralization in all the groups. The difference in microhardness values from demineralization to remineralization in all the three test groups was found to be statistically highly significant. Conclusion: BAG and Tricalcium phosphate had higher remineralizing capacity, which can be used clinically to treat early carious lesions.

Abstract:Objectives: in primary teeth, the clinical success of pulpectomy depends on the eradication of pathogenic microorganisms such as Enterococcus faecalis (E. faecalis) from the infected canals. Various chemical and herbal compounds are used to achieve a reasonable level of root canal disinfection. Thus, this study directed to evaluate the antimicrobial effect of citric acid and chitosan in comparison with sodium hypochlorite (NaOCl) on E. faecalis in primary teeth. Materials and Methods: A total of 30 children aged between 4-7 years were included in the present study. The enrolled children have primary carious teeth that are indicated for pulpectomy treatment. The involved teeth were categorized into three main groups according to the type of irrigant solution; 6% citric acid (A), 6% chitosan (B), and 1% NaOCl (C). Then each main group was further subdivided into non-activated (A1, B1, and C1) or ultrasonic activated (A2, B2, and C2). E. faecalis count was carried out using a colony-forming unit(CFU) Conclusion: Activation of irrigant solution significantly improves their antibacterial effect. chitosan has a significant effect on the reduction of E. faecalis count. Citric acid cannot be used alone as an irrigant solution.

Abstract:Negative dental experiences in childhood can lead to dental phobias in adult patients. The dentist’s role is to lessen dental anxiety by providing a pain-free procedure. This could be achieved by using painless procedures in caries removal such as air abrasion, laser, chemo-mechanical, enzymatic techniques, as well as the use of polymer or ceramic burs. Ceramic burs are rotary cutting instruments made of alumina-yttria stabilized zirconia round burs. They work by selective removal of carious infected dentin consequently avoiding pain which could be more appropriate for very young children.The aim of this study was to assess the extent of pain perception using ceramic burs during caries removal without local anesthesia in pediatric patients. This was performed on 51 patients between 4-8 years of age with 2 primary molars showing dentinal caries requiring class I restoration. The first visit (group I) caries was removed by ceramic bur on low speed hand-piece without administration of local anesthesia then the second visit (group II) caries was removed by carbide round bur on low speed hand-piece without administration of local anesthesia. During each procedure, objective assessment for pain was performed using FLACC scale. Upon completion, the pain threshold for caries removal was measured subjectively by Wong Baker faces pain rating scale. The various responses were gathered and subjected to statistical analysis. It was concluded that ceramic burs reduce the pain sensation during removal of infected dentin which ensures excellent patient comfort. There could be some advantages in using the ceramic bur: high acceptance by the patient, reduced pain response and sound pulp protection in deep caries lesions.

Abstract:ObjectivesThis study investigated the ability of modified phosphoric acids containing chlorhexidine (CHX) or grape seed extract (GSE) for promoting simultaneous biomodification during acid etching on bonding properties in caries-affected dentin (CAD).Materials and methodsThirty-two human molars (8 with sound dentin [SD] and 24 naturally CAD) were selected for the study. The SD and CAD were initially exposed, then randomized and etched according to the following groups: (1) SD (SD-CT) and CAD (CAD-CT) both with 37% phosphoric acid, (2) CAD with 2% CHX containing 37% phosphoric acid (CAD-CHX), and (3) CAD with 2% GSE containing 10% phosphoric acid (CAD-GSE). The bonding procedure and composite build-ups were performed after acid etching. Subsequently, they were sectioned in resin-dentin specimens. The specimens were submitted for chemical profiling by micro-Raman, microtensile bond strength (μTBS), failure mode with chemical characterization by FEG/SEM-EDX, and in situ zymography by CLSM. The data from μTBS and CLSM were statistically analyzed (1-way ANOVA and Tukey’s test; α = 0.05).ResultsThe highest μTBS results were shown for SD-CT in comparison with all CAD groups (p < 0.001), and the lowest for CAD-CT and CAD-CHX (p < 0.001). The etching with CHX did not increase the μTBS for CAD when compared with CT (p = 0.52). However, the etching with GSE improved significantly the μTBS for CAD when compared with CT and CHX (p < 0.001). The chemical profile detected chemical and structural changes in collagen peaks for CAD-CT, which were not detected when the CAD was etched by modified acids. Also, the poorest hybridization ability was detected in CAD for CT, which was significantly improved with modified acids, especially the GSE, as evaluated by chemical profile and failure mode. A significant reduction of MMP activity on CAD was promoted by modified acids in comparison with CT (both p < 0.001).ConclusionsThe GSE-containing acid was able to promote biomodification during the acid etching, increasing the bonding properties and reducing the activity of the MMPs within the hybrid layer.Clinical relevanceThe use of GSE-containing phosphoric acid can be a promising alternative to improve the bonding performance on caries-affected dentin, since it is capable of biomodifying the dentin during the acid etching, without adding any extra step in bonding procedures.

Evaluation of 17% EDTA and 10% citric acid in smear layer removal and tubular dentin sealer penetration

Abstract:The purpose of this study was to compare the efficacy of different chelating solutions (17% EDTA and 10% citric acid) on the smear layer removal, and their effect on tubular dentin sealer penetration. Sixty root canals were prepared and distributed into four groups (n = 15) according to the final irrigation protocol: G1, final irrigation with 2.5 mL of distilled water; G2, final irrigation with 2.5 mL of 2.5% sodium hypochlorite solution; G3, final irrigation with 2.5 mL of 17% EDTA; and G4, final irrigation with 2.5 mL of 10% citric acid. Five specimens from each group were not filled to assess smear layer removal by scanning electron microscopy. Ten specimens from each group were filled for analysis of sealer penetration into dentinal tubules by confocal laser scanning microscopy. Smear layer removal (Kruskal-Wallis and Dunn's tests) and sealer penetration (F and Tukey's tests) were statistically analyzed with 95% of significance level. G3 and G4 had greater smear layer removal rates in the cervical and middle thirds, in comparison with G1 and G2 (p < .05). G3 and G4 had the highest percentages of sealer penetration in all thirds, in comparison with G1 and G2 (p < .05). Smear layer removal was effective only at the cervical and middle thirds when the chelating solutions were used. Sealer penetration into the dentinal tubules significantly increased in all root thirds when the specimens were treated with both chelating solutions.
